Abstract:
The purpose of the study was to outline possible relationships between public awareness of psychotropic drugs, stigmatization of people suffering from mental disorders and discontinuation of drugs without consultation with a doctor in Ukraine. Materials and methods. This study is based on the analysis of data obtained through an electronic questionnaire filled out by the Ukrainian public. Until April 2021 the questionnaire was freely available to everyone who wanted to take part in the survey. The questionnaire was distributed via an Internet link. Based on the obtained data, the respondents were divided into different categories depending on their education, profession (a medical worker was analyzed separately). Also, the respondents who had personal experience of using psychotropic drugs were interviewed especially deeply and in more details, depending on which group of drugs they used (antidepressants / antipsychotics / anxiolytics). The separate place in the study was given to identifying the general awareness of the Ukrainian public about correct and erroneous judgments about psychotropic drugs. Results and discussion. Based on the obtained data, main factors which are influencing people awareness of psychotropic drugs were identified, as well as the approximate percentage of citizens with erroneous judgments about psychotropic drugs. Thanks to the results of the questionnaire, the main possible reasons for the withdrawal of treatment with psychotropic drugs by patients without consultation with the attending physician were suggested (identified). Conclusion. It can be concluded from the obtained data that a person's awareness in the field of psychotropic drugs is most influenced by their own experience of their use. For this reason, the author would be interested to know how psychiatric patients are informed by their physicians. The author is aware that the amount of data obtained is not completely sufficient to create a significant picture of the general situation, but she hopes that the obtained data will still allow to get some idea of the situation as a whole. In conclusion, not only the obtained results in this study were outlined, but also possible further actions to obtain a more accurate picture of the situation were mentioned, and the topic of the next study was indicated in order to solve the identified problem

Abstract:
Efavirenz is in the non-nucleoside reverse transcriptase inhibitor category of HIV antiretroviral medicines. It is an in vivo inducer of the CYP3A4 isoenzyme within the cytochrome P450 (CYP450) system, and an in vitro inhibitor of the system’s CYP2C9/2C19, 3A4 and 2B6 isoenzymes; as a result, concentrations of psychotropic drugs can be increased or decreased depending on the specific enzyme pathway involved in their metabolism. CYP3A4 is responsible for metabolizing many benzodiazepines and other psychotropics, as well as selective serotonin reuptake inhibitors and tricyclic antidepressants. As an inducer of CYP3A4, efavirenz can increase the rate at which these agents are metabolized, resulting in administered psychotropic drug levels that are below their therapeutic thresholds. Conversely, efavirenz is an inhibitor of CYP2B6, which metabolizes agents such as bupropion; consequently, bupropion levels in the blood can increase. Given the existing conflicting data, the clinician may find it impractical to use an evidence-based approach when concomitantly prescribing efavirenz and psychotropic drugs to their HIV patients. Instead, it may be preferable to use a more pragmatic approach that applies knowledge of the most current pharmacological and pharmacokinetic data for psychotropics and non-nucleoside reverse transcriptase inhibitors, which may help better predict their potential interactions.

Abstract:
Objective: The purpose of the present study was to investigate the prevalence of psychotropic drug use and the predictors of use during hospitalization. Method: We investigated 728 patients admitted to medical, gynecological, and surgical wards of two non-university general hospitals in Austria for psychotropic drug use, psychiatric morbidity, and sociodemographic characteristics. Results: The use of psychotropics was highest in medical wards (67.6%), followed by surgical (59.3%), and gynecological wards (37.8%). Older age, psychiatric caseness, consultation by psychiatrists and use of psychotropics before admission were predictors for psychotropic drug use. Psychotropics were given for a longer duration to psychiatric cases than to non-cases. Anxiolytics were prescribed most often (39.6%), followed by hypnotics (16.9%), neuroleptics (10.7%), and antidepressants (7.0%). Conclusions: The fact that psychiatric illness is a significant predictor of psychotropic drug use suggests that these drugs were prescribed aptly. The longer duration of use among psychiatric cases supports the idea of appropriate prescriptions.

Dazu gehören Neuroleptika, Antidepressiva, Tranquilizer, Hypnotika, Psychostimulantien, Nootropika und pflanzliche Substanzen. Am häufigsten werden die zu der Gruppe der Tranquilizer zählenden Benzodiazepine verordnet. Die Verordnung von Psychopharmaka erfolgt syndrom- und nicht nosologiespezifisch. Die Aufklärung ihrer Wirkmechanismen im Gehirn hat wesentlich zum besseren Verständnis der Ätiologie und Pathogenese psychischer Störungen beigetragen.

Psychotropics can lead to dependence if misused or used without control and supervision. Psychotropic drugs can only be given using a prescription in healt facilities such as pharmacies, clinics or hospitals. One way to control and supervise psychotropic drugs is to plan. This study aims to determine the pattern of prescribing psychotropic drugs at the Karangrejo’s Pharmacy, Semarang City. This research is a type of descriptive observational research. The design of this research is a Cross-Sectional Study. The data collected is retrospective, in the period October - December 2022. The data source is a psychotropic drug prescription sheet at the Karangrejo Pharmacy, Semarang City. The data collected included date of prescription, prescription number, age, gender, origin of prescription, psychotropic drug used and dosage. The data were analyzed qualitatively, grouping data based on patient and prescription characteristics. Patient characteristics include the patient's age and gender while prescription characteristics include the origin of the prescription, and the psychotropic drugs used include the name of the drug, drug class, amount of drug therapy and drug combinations. Quantitative analysis is by calculating the percentage of data that has been processed. From this study, the data obtained were 106 patients who received psychotropic drugs at the Karangrejo Pharmacy, Semarang City. From the results of this study, it was obtained that the most age group was 56-65 years (34.91%) with the most gender, namely women (65.09%), the origin of the prescription that prescribed the most psychotropics was prescription from external doctors (50.94%) with the highest speciality of prescribing doctors, namely specialist doctors (82.07%), the most widely used type of psychotropic drug, namely Chlordiazepoxide (50.73%), the highest number of drug therapies, namely single drug therapy (76.42%), the highest drug combinations namely Chlordiazepoxide 5mg and diazepam 2mg (56%), and the most common class of therapy was antispasmodics (50.72%).

Abstract:
Despite improvements in cancer therapy, overall survival for most cancer types has changed a little in the past decades. Drug repositioning represents a promising approach for discovering new therapeutic strategies for cancer therapy. Since several epidemiological studies reported lower cancer incidence in individuals receiving long term psychotropic drugs treatment, in this project we investigated 27 psychotropic drugs for their cytotoxic activity in several cancer cell lines. Consistent with the cationic amphiphilic structure of the most cytotoxic compounds, we investigated their effect on mitochondrial and lysosomal compartments. Penfluridol, ebastine, pimozide, fluoxetine, fluspirilene and nefazodone showed significant cytotoxicity, in the low micromolar range, in all cell lines tested. In MCF7 cells these drugs triggered mitochondrial membrane depolarization, increased the acidic vesicular compartments and induced phospholipidosis. Neither caspase nor autophagy inhibitors rescued cells from death induced by fluoxetine, fluspirilene and nefazodone. Treatment with 3-methyladenine rescued cell death induced by pimozide and spiperone. Conversely, inhibition of lysosomal cathepsins significantly reduced cell death induced by ebastin, penfluridol, pimozide, spiperone and mildly by fluoxetine. Lastly, spiperone caused apoptosis in colorectal and breast. Our unpublished data on the characterization of spiperone activity on adherent and stem-like colorectal cancer cells demonstrated that its cytotoxicity is linked to perturbations of intracellular calcium (Ca2+) homeostasis, which likely result in mitochondrial Ca2+ overload and membrane depolarization, cell cycle block in G1 phase, and apoptosis. Spiperone induced a PLC dependent Ca2+ release from the endoplasmic reticulum (ER) along with ER stress and unfolded protein response activation, resulting in CHOP upregulation. Altogether these data support the clinical development of psychotropic drugs for cancer therapy.

Antipsychotic drugs are the mainstay of treatment for psychotic disorders according to the Standard Treatment Guidelines (2012). However, these drugs are associated with multiple severe adverse drug reactions. In order to limit the effect of adverse drug reactions on patient care, documentation is necessary. Documentation of adverse drug reactions entails recording the reaction experienced, as well as supplementary information. Proper documentation can prevent future occurrences of the same or similar adverse drug reactions. The aim of this study was to determine the effects of an educational intervention targeting increasing documentation of the adverse effects of antipsychotic drugs. The objectives of the study were: to determine the pre-intervention extent and frequency of documentation of antipsychotic-related adverse drug reactions in the patient medical record; to implement an intervention aimed at educating the relevant healthcare professionals, focusing on the adverse drug reactions of antipsychotic drugs and how to record or document these reactions; to assess the post-intervention extent and frequency of documentation of antipsychotic-related adverse drug reactions in the patient medical record; and to assess the attitude of healthcare providers towards the documentation of antipsychotic related adverse drug reactions before and after the intervention.

Driven by forensic intelligence, illicit drug chemical profiling via Gas Chromatography-Mass Spectrometry (GC-MS) remains the leading approach that forensic scientists and law enforcement agencies pursue in a bid to counteract the ever-increasing drug epidemic. An alternative approach for forensic drug investigations that delivers intelligence at a more specific level than that of chemical profiling is isotopic profiling via Isotope Ratio Mass Spectrometry (IRMS). The analysis of selected stable isotopes (18O:16O,15N:14N, 13C:12C and 2H:1H, for example) that compose drug molecules has been suggested as a credible technique that can aid in forensic drug investigations. This research, consisting of four individual projects, was designed to investigate the potential of using isotopic profiling for source, synthetic procedure and batch linkage or discrimination of selected designer drugs/Novel Psychoactive Drugs (NPDs).

Objectives: A prescription claims database from COPE Behavioral Services in Tucson, Arizona was used to retrospectively assess the differences between patients receiving <4 and those receiving > 4 psychotropic medications over a 12-month period in adult patients with schizophrenia. Methods: Medication groups (i.e., < 4 versus > 4 concomitant psychotropic agents) were compared for differences in gender, age, duplication of antipsychotic therapy, adherence rates, court order treatment status, psychiatric hospitalization rates and length of stay, cost of services provided, and concomitant psychotropic medications. Results: A total of 506 adult patients with schizophrenia (F=214 and M=292) met the inclusion criteria for receiving psychotropic medications during the 12-month study. Of those, 388 patients (76.7%) were found to have an average of < 4 medications, while 118 patients (23.3%) were found to have > 4 medications. Duplication of antipsychotic therapy was more common in the > 4 group (29.7%) compared to the < 4 group (3.1%), p < 0.001. Psychotropic adherence rates were significantly higher in the > 4 group based on month’s supply of prescriptions. Demographic differences between groups included: increased age, more women, fewer court order status, and higher cost of care in the > 4 compared to the < 4 medication group. No differences in hospitalizations, length of stay, and hospital costs were found between groups. Conclusions: Our findings suggest that patients with schizophrenia with increased rates of polypharmacy have higher adherence rates, more duplication of antipsychotics, and a higher cost of care (i.e., case management, laboratory, other services, total prescription costs) compared to patients receiving < 4 psychotropic medications.

Abstract:
In March 2016, the Assembly of the Republic of North Macedonia amended the Law on Control of Narcotic Drugs and Psychotropic Substances, giving the possibility for North Macedonia to join a growing number of countries that allow for the cultivation and exportation of medical cannabis. As a result, for the past five years it has been legal to grow cannabis for medical purposes, and to refine, extract and produce hemp seed and cannabis oil. Given the current economic unprofitability of cannabis production, primarily due to high production costs and uncertain marketing, many companies are idle, i.e., waiting for legal changes that will make it possible for them to export dried cannabis flowers. The purpose of this paper is to answer the following hypotheses: (1) Does the inability to achieve the required quality of cannabis imply a new draft Law on Control of Narcotic Drugs and Psychotropic Substances? (2) Which are the positive versus the negative implications regarding the legal changes? (3) Determining the security implications, given that Macedonia is a transit point on the marijuana smuggling route from Albania and Kosovo to Turkey, where it is replaced by heroin, which is later sold to the Balkans and Western Europe. (4) Can the Government establish an effective system of control? For the purposes of this paper, a desk analysis of primary and secondary literature will be conducted as well as interviews with the relevant stakeholders. Abstract:
The significant variety of properties and wide distribution of opiates and opioids in the modern pharmaceutical industry entail not only positive, but also negative consequences in people's lives. The constant search for new drugs entails the emergence of new substances with psychotropic effects. The widespread use of vinegar essence in the food industry, nitrogen oxides in agriculture, and the frequent appearance of carbon disulfide in everyday life create an increased risk to public health. Positional compression syndrome very often accompanies poisoning with psychotropic substances, which leads to a high risk of disability for victims or even death. This electronic educational resourse consists of seven theoretical educational modules: opioid poisoning, acute psychostimulant poisoning, vinegar essence poisoning, acute carbon monoxide poisoning, acute nitrogen oxide poisoning, acute hydrogen sulfide and carbon disulfide poisoning, positional compression syndrome. The theoretical block of modules is presented by presentations, the text of lectures with illustrations. Control classes in the form of test control accompany each theoretical module. After studying all modules, the student passes the final test control. Mastering the electronic educational resourse will ensure a high level of readiness to provide specialized toxicological care by doctors of various specialties.
