Abstract:
The clinical description of psychotrauma has undergone a huge change since the advent of the new versions of international classifications, in particular the DSM 5, which through basic clinical criteria (reliving/hypervigilance/avoidance), helps the clinician to make the diagnosis of post-traumatic stress disorder (PTSD). However, psychotrauma presents transonographic and transcultural characteristics, as it can take on different clinical pictures, essentially a psychotic picture, which can mislead the diagnosis and in this case the therapeutic management. Through a clinical vignette rich in clinical polymorphism, we will expose the different difficultes encountered, highlighting the need for an approach based on a multidimensional approach criteriological perhaps, but psychopathological and phenomenological certainly. Therefore, it is important that the clinician takes into consideration the cultural, social and biological dimension in order to guarantee a good management both with medication and psychotherapy.

Abstract:
Introduction. War is a super-powerful extreme event that causes adaptive capacity to decline. The need for a clearer understanding of the specifics of war psychotrauma and its clinical consequences for all components of human health determines the relevance of the topic of this study. The aim of the work is to study the peculiarities of psychosomatic relationships in persons with the consequences of war psychotrauma. Materials and methods. The experimental study group consisted of 32 patients from the number of temporarily displaced persons, the control group – 34 patients who did not change their place of residence during the entire period of the full-scale war. The diagnosis was aimed at identifying neurotic, stress-related and somatoform disorders, according to the rubric (F40-F48) of the ICD-10. Assessment of the somatic condition was carried out on the basis of analysis of anamnestic data, clinical examination of the patient and indicators of laboratory tests. Psychodiagnostic methods were used to assess the mental states of patients (Clinical Questionnaire for the Detection and Assessment of Neurotic States; Methodology for the express diagnosis of neurosis by K. Heck and H. Hess; Questionnaire SAN (Mood, Activity, Mood); Methodology for the diagnosis of Taylor's anxiety level; Self-assessment of mental states according to Eysenck; Ch. Spibleger's Reactive and Personal Anxiety Assessment Scale in the modification of Y. Hanin. Statistical processing of the research results was carried out in the Microsoft Excel program and with the help of the Social Science Statistics online calculator. Results. As a result of the analysis of anamnestic data, clinical examination and analysis of laboratory tests, psychosomatic disorders were found in patients of both research groups: in 29 (90.63%) patients of the experimental group and in 26 (74.47%) patients of the control group. Conclusions. It has been confirmed that a prolonged stressful extreme situation of a full-scale war causes the formation of negative mental states accompanied by psychosomatic manifestations, the formation of a closed vicious circle of a pathological psychosomatic process. A high close direct correlation was found between the level of reactive anxiety, autonomic disorders, well-being, activity, mood of patients and psychosomatic symptoms. The absence of a statistically significant difference in the risk of developing psychosomatic disorders as a result of war psychotrauma in temporarily displaced persons and those who did not change their place of residence was revealed.

Abstract:

Abstract:
IntroductionRecent studies in the neurobiology of schizophrenia highlighted the role of neuropsychoendocrine activations as a consequence of psychostress followed by the activation of the HPA axis with an excess of endogenous cortisol. The relation endogenous cortisol–glutamatergic hyperactivation enhances the excito-toxic mechanisms and the cortical-subcortical alterations in schizophrenia.MethodWe conducted a retrospective study on 40 patients, with ages between 25 and 55 years, admitted in the university clinic of Craiova between January 1, 2015 and December 31, 2015 for paranoid schizophrenia according to ICD-10 criteria and with positive history of psychotrauma and physical abuse in childhood and adolescence.ResultsThe frequency of psychotraumas, social stress and physical abuse in our group was significantly higher in women (63.33%), in patients with urban residence (80.00%) and age group 36–45 years (46.67). There was a pattern of residual defectuality reflected by positive symptoms (83.33%), alcohol abuse (80.00%), aggressive behavior (66.67%) and suicide attempts (30.00%). The poor course with minimal social functioning (GAFS < 40; 36.67%) was correlated with a high number of relapses and hospitalizations (> 9 hospitalizations; 43.33%), cognitive deficit (MMSE < 23; 76.67%). The psychosocial factors involved in the pathogenesis and course of schizophrenia were social stress (60.00%), physical abuse in childhood and adolescence (20.00%) and psychotraumas (20.00%).Conclusions.Psychotrauma and physical abuse in childhood and adolescence and during the course of paranoid schizophrenia constitute a risk factor for a poor outcome with cognitive deterioration, aggressive and suicidal behavior that call for prophylactic measures and qualified psycho-social interventions associated to the pharmacological treatments.Disclosure of interestThe authors have not supplied their declaration of competing interest.

Abstract:
The article addresses the issue of experiences of cultural mysticism in war psychotrauma in an African cultural context. It starts from the clinical observations of a fear of the mysticism among certain soldiers engaged in the war against the separatist groups “Amba Boys” in North-West and South-West Cameroon. The objective of the study was to understand the experiential experience of Cameroonian soldiers psychotraumatized by war in their cultural signifiers. The study was conducted using a phenomenological approach. The data was collected at the RAPHA-Psy Psychology Foundation, from four psychotraumatized soldiers, through semi-structured interviews. The results reveal that these soldiers experienced the psychotrauma as resulting from a complex of mysterious conflicts. Also, the psychotraumatic symptomatology was for them a set of manifestations of supernatural attacks. According to them, there are two worlds in the battlefield, one visible and the other invisible.

Abstract:
Clinical observations at the RAPHA-Psy Psychology Center have revealed resilience with the appearance of sociocultural adaptation in some psychotraumatized soldiers of war. This has led to an attempt to understand the role of culture in this resilience, through cultural signifiers. The study was guided by the clinical method with five resilient psycho-trauma war soldiers. The results obtained after semi-structured interviews show that among these soldiers, the work of resilience was structured around the strengthening of the link between the Soldier and the bio-lineage Family, the strengthening of the link between the Soldier and the Ancestor, the consolidation of the link between the Bio-lineage Family and the Ancestor and the emergence of the Soldier’s mastery over the Enemy. The cultural signifiers in the work of resilience therefore have a function of reconstruction and solidification of the Soldier’s links in his different existential axes. The study thus made it possible to work out a model of resilience of the psychotraumatized ones of war based on the African cultural signifiers.

Abstract:
Introduction. The article is devoted to the analysis of theoretical approaches to understanding the intrapsychic mechanisms that generate combat stress and psychotrauma. This article draws the attention of specialists to the psychophysiological and individual-typical factors affecting the occurrence and course of combat PTSD. Theoretical analysis. The stages of research on the consequences of human participation in hostilities conducted over the past century are presented. Scientific views on the role of such innate properties of the nervous system as the strength of nervous processes, their balance and dynamism in the development of combat PTSD are considered. The significance of the type of human temperament in his reactions to specific combat stress is revealed. The role of plasticity of higher nervous activity as a factor of overcoming pathological environmental factors, including its application in the conditions of military events, is also revealed. Conclusion. Abstract:
Fibromyalgia (FM), a chronic disabling disorder causing diffuse musculoskeletal pain, presents strong comorbidity with psychopathological disorders, including post-traumatic stress disorder (PTSD). We report the case of a 35-year old Syrian Refugee, diagnosed with both Fibromyalgia and PTSD related to traumatic events (killed husband, expulsion from home, rape with death threats, difficult socio-economic situation). An antidepressant treatment combined with psychotherapeutic and psycho-educational intervention led to satisfactory improvements. Considering the clinical case and other related studies, it seems that early management of the traumatic dimension is essential, in order to prevent the chronicity of the suffering and the emergence of psychiatric comorbidities.
