Full textPosttraumatic stress disorder (PTSD) is a highly heterogeneous condition, ranging from individual traumatic incidents such as car accidents to national tragedies such as natural disasters. Every individual has a different depending on their personality and past experiences, especially regarding their tendency to depression. Hence the condition is better termed psychotrauma spectrum disorder (PSD). Its heterogeneity hinders reliable diagnosis, as detection is entirely dependent upon a clinician’s subjective impression and sensitivity to comorbidities and there is always the possibility of concealment. Yet early diagnosis is essential, as the earlier PSD is detected the more likely treatment will be successful. Furthermore, reliable biomarkers of PSD would allow for much more accurate detection and monitoring of progression. Here we propose a new computational approach building on our prior work on the early detection of Parkinson’s, Alzheimer’s and depression. We will use a new analysis tool, called the Brain Code (BC). This concept was developed to integrate many different kinds of data, for e.g. the often fragmented and incomplete outputs from body sensors that record balance, dexterity, postural, facial and vocal movements combined together with cognitive or clinical outputs such as the intentional or emotive content of speech. The Brain Code allows us to fit all these different data streams together in such a way as to compensate for the deficiencies of each individually. It can put disparate physiological and cognitive data into the same ‘coordinate system’, so that we will be able to develop a reliable quantitative ‘signature’ of PSD. These quantitative biomarkers will be designed so that they are useful for both physicians in a clinical setting and for communities affected by a large-scale traumatic event

Full textMelancholia is a profound sadness attached to the invasive emptiness of the lost object to which the subject identifies. In reaction to the content of melancholia, symptoms of mania as hyperactive agitation, metonymic chain of speech, playfulness and flight of ideas in acceleration, reveal the excess of an unrestrained attempt of not being the lost object. Drive movement expresses the unbinding of desire, the ideal substituting for the nostalgia of the loss. The oscillation in cycles of reactivity interrogates the experience of "jouissance-enjoyment of the signifier" that is the temporality of the subject in association with psychotrauma. The elimination of the lost object maintained in "suspended animation" establishes an absence of renunciation of the ideal, the absence of gaps leading to crystallization of psychic time of differentiation. My work relies on analyzing the experience of temporality among eight adults with mania-melancholia, through language, the association of ideas in the narrative of the self and the process of historicisation. It addresses resonances-dissonances, the familiar-strange and synchrony-diachrony. It integrates the prefigurative function of mirroring synchronicity and repetition of the trial of chronological construction. The alternation between time in excess and anesthesia is considered in relation to the unsustainable of the real in individuation. Passage to the act marks the temporal rupture of an attempt of separation from the frozen. Temporal gaps are thought in relation to desire and interdiction. Time holes indicate the emptiness of meaning. The metaphor of time refers to talker-time in the incapacity of language

Full textIn a sensorially complex world, human beings need to efficiently and effectively filter and respond to relevant stimuli. Stimuli are prioritized according to their saliency. Especially, the salience network is readily identified as an intrinsically connected large-scale network including prominent nodes as the anterior insula, the dorsal anterior cingulate cortex, the amygdala, the ventral striatum, and the substantia nigra/ventral tegmental area. The salience network not only plays an important role in saliency detection and reactivity but also facilitates access to attention and working memory resources once a salient event has been detected. Stress reactions have been previously linked to activation of the salience network. Moreover, network models are now being widely used to characterize deficits in a wide range of psychiatric and neurological disorders. These studies have provided evidence for prominent salience network dysfunctions in frontotemporal dementia, mood and anxiety disorders, schizophrenia, drug addiction, or pain. Especially, the role of the salience network in intrusive experiences has been suggested, during hallucinations in patients with schizophrenia, and more recently, during re-experiencing in patients suffering from post-traumatic stress disorders. The goal of the present thesis is to improve knowledge about the role of the salience network in intrusive thoughts in these two disorders. In a first part, we studied the role of the salience network in hallucinations in patients with schizophrenia. In a second part, we studied its role in re-experiencing in post-traumatic stress disorder. In a coordinate-based meta-analysis, we explored the neural bases of salience in schizophrenia, focusing on reward processing. We showed that the hypoactivation of the ventral striatum found in patients with schizophrenia during such tasks was correlated with positive symptoms of schizophrenia. Furthermore, several ‘trait’ and ‘state’ studies found that the salience network has a modulatory function in the occurrence of hallucinatory experiences. In a second study, we thus validated a method for hallucinations’ capture, making possible the comparison between the time-course of brains areas overactivated during these experiences and conventional resting-state networks, which is a mandatory step for the study of the dynamic role of the salience network during hallucinations. Finally, in a third study, we explored the role of insular cortex in re-experiencing symptoms in post-traumatic stress disorders, and especially its role in response to pre-reactivation propranolol therapy. The neural bases of treatment-response are indeed still poorly understood, notably via effective connectivity analysis. We showed that the anterior insula exerted causal influences over the brain which correlate with reexperiencing in post-traumatic stress disorder. These studies pave the way for future developments. Especially, improvement in the knowledge about the physiopathology of intrusive experiences, both in schizophrenia and post-traumatic stress disorder, are important to develop re-experiencing capture methods, and for the development of personalized medicine in these two disorders

Full textMusic therapy in psychotrauma has been scantily explored in Sweden. The few results available suggest that music therapy in psychotrauma can be an effective treatment. The question of resilience and its importance in treating psychotrauma with music therapy has also been sparsely explored. The purpose of the present thesis is, therefore, to explore how three music therapists with experience in psychotrauma treatment look at resilient approaches in patient's treatments. The research questions are: How do music therapists understand the concept of resilience? In what ways do music therapists think they can contribute to resilient approaches? Which music therapeutic methods can contribute to the development of resilience? The study is based on three semi-structured interviews that were analyzed using qualitative content analysis. The result is presented in the form of an overall theme: Resource-oriented music therapy contributes to resilience in the treatment of psychotrauma. The theme consists of three main categories, which in turn comprise a number of subcategories. Finally, the results are discussed with theoretical points of departure on resilience and music therapy in treatment in psychotrauma. An insight into the importance of the work to music therapy is provided, and ideas on future research work that can be further developed are presented.

Full textThe confrontation of the Moroccan soldiers with violence is a phenomenon that inscribes itself in the time. It took many forms as Moroccan soldiers fought under the French flag, and issues related to war trauma became a real social issue quite recently. From this point of view, Moroccan soldiers under French rule (protectorate) offer a field of investigation hitherto little treated in Morocco and France. However, from 1914 to 1954, these Moroccan soldiers, particularly present in Indochina, suffered from the consequences of the experience of war violence on their psyche. We sought to interpret their behaviour.To understand the tensions experienced in Indochina by the Moroccan fighters, it was necessary to first, locate the fighter in his socio-cultural situation and in the series of military experiences lived before the Indochina war (Great War and World War II) based on archival sources (military, diplomatic, medical) and the evaluation of psychiatric and medical approaches of the time. For post-1945, work is enriched by a triple collection (testimonials, life stories, and post-traumatic semiology) with some veterans. Thus, the psychological impact linked to wartime violence over forty years of experience in the African army (1914-1954) could be identified in a new way.All that pertains to psychological or psychiatric pathologies, especially during the Indochina war, was analysed and allowed to re-evaluate the tracks followed by these soldiers, who go as far as desertion and the passage to the Vietnamese enemy or murderous or suicidal feats of madness, and to enlighten the relation to politics of these men caught between causes which were not theirs. In the case of Indochina, some of them experienced this war as the experience of a place of ideological affirmations. The exile of King Mohamed V sometimes led them to join the Indochinese resistance. Other soldiers, taken prisoner by the Viet-Minh, were upset by the experience of captivity.This thesis emphasises the relation to the commitment in the colonial army, the relation to the violence suffered and given, and offers an interpretation of the observed behaviours, which shows the discontinuous functioning of the relation with the religious, the impact of the traumas on the ability to remembrance and weaving of social relationships after wars. This sheds new light on the available archive sources of these Moroccan soldiers of the Army of Africa and the way the violence has weighed on them. This research suggests that sidelining this dimension in the decades following the protectorate era has produced an erosion that has partially distorted the perception of the story of these fighters
