Abstract:
Background and objectivesDuring the last decades, the survival rates in patients with congenital heart disease have increased dramatically, particularly in patients with complex heart malformations. However, the survival in patients with simple defects is still unknown. We aimed to determine the characteristics and the risk of mortality in patients with isolated pulmonary valve stenosis (PS).MethodsSwedish inpatient, outpatient and cause of death registries were used to identify patients born between 1970 and 2017 with a diagnosis of PS, without any other concomitant congenital heart lesion. For each patient with PS, 10 control individuals without congenital heart disease were matched by birth year and sex from the total population registry. We used median-unbiased method and Kaplan-Meier survival analysis to examine the risk of mortality.ResultsWe included 3910 patients with PS and 38 770 matched controls. The median age of diagnosis of PS was 0.7 years (IQR 0.3–7.0). During a median follow-up of 13.5 years (IQR 6.5–23.5), 88 patients with PS and 192 controls died; 500 patients with PS (12%) underwent at least one transcatheter or surgical valve intervention. The overall mortality rate was significantly higher in patients with PS compared with matched controls (HR 4.67, 95% CI 3.61 to 5.99, p=0.001). Patients with an early diagnosis of PS (0–1 year) had the highest risk of mortality (HR 10.99, 95% CI 7.84 to 15.45).ConclusionsIn this nationwide, register-based cohort study, we found that the risk of mortality in patients with PS is almost five times higher compared with matched controls. Patients with an early diagnosis of PS appears to be the most vulnerable group and the regular follow-up in tertiary congenital heart units may be the key to prevention.

Abstract:
The paper tries to analyze the tax reforms in new EU member states, and to understand how these developments came into existence. Despite several commonalities we argue that across country variation is very much present, especially in the areas of degree of dependence on indirect taxes as well as the share of each component in total tax revenue. Our preliminary evidence indicates that there are three identifiable groupings; Hungary, Slovenia and Czech Republic as the first, Poland and Slovakia as the second, and the rest of the countries as the third. From the clusters we claim that these differences reflect the underlying the structural characteristics of the economies, particularly the scope of the informal economy and the intensity of competition to attract foreign capital. The paper contributes to the literature in a number of ways. First, it gives a comprehensive overview of reforms and possible explanatory factors. Second, it attempts to categorize the transition countries according to the features of the tax systems adopted. Finally, it establishes a linkage between the informal sector, capital from external sources and why particular modifications were implemented. (author's abstract)

Formulary apportionment is an intensively debated mechanism for allocating tax base within multinational groups. Systems under which the formula is identical in all jurisdictions and systems under which jurisdictions can determine the weights on the formula factors individually can be observed. The latter systems produce uncertainty about the overall tax-liable share of the future group tax base. Counter-intuitively, I identify scenarios under which increased uncertainty leads to higher expected future group income. My results provide helpful insights for firms and policy makers debating the specific design of a formulary apportionment system. (author's abstract)

Abstract:
This paper analyzes the effects of globalization on implicit tax rates (ITRs) on labour income, capital income, and consumption in the EU15 and Central and Eastern European New Member States (CEE NMS). We find a positive effect of globalization on the ITR on labour income in the EU15, but no effect on the ITR on capital income, and a negative effect on ITR on consumption. There is a significant negative effect on the ITR on capital income in the social-democratic and southern welfare regimes, a marginally significant negative effect in the liberal regime; a negative effect on the ITR on consumption in the social-democratic, conservative, and liberal regimes; and a positive effect on the ITR on labour income in all welfare regimes. In the CEE NMS there is no effect of globalization on any ITRs. (author's abstract)

Abstract:
In this chapter, an interactive tool using graphic user interface (GUI) environment-based MATLAB is proposed to solve practical optimal power system planning and control. The main particularity of the proposed tool is to assist student and researchers understanding the mechanism search of new metaheuristic methods. The proposed tool allows users to interact dynamically with the program. The users (students or experts) can set parameters related to a specified metaheuristic method to clearly observe the effect of choosing parameters on the solution quality. In this chapter, a new global optimization method named grey wolf optimizer (GWO) and pattern search algorithm (PS) have been successfully applied within the interactive tool to solve the optimal power flow problem. The robustness of the two proposed metaheuristic methods is validated on many standard power system tests. The proposed interactive optimal power flow tool is expected to be a useful support for students and experts specialized in power system planning and control.

Abstract:
Fluorescence lifetimes of coronary arteries with normal intima, white atherosclerotic plaque, and calcified atherosclerotic plaque were measured in the wavelength range beyond 360 nm and in the bandwidths centered on 390 nm, 450 nm, and 500 nm. The fluorescence profiles were fitted to a double exponential with a fast and a slow component. At wavelengths above 360 nm, arteries with white atherosclerotic plaque appear to have a fast component (113 31 ps) that is shorter than that of normal arteries (126 17 ps) or arteries with calcified plaque (133 35 ps). In the band centered at 390 nm, the fast components of normal arteries (148 41 ps) and arteries with white plaque (112 141 ps) are slower than that of calcified plaque arteries (81 56 ps). In the 450 nm band, both arteries with white plaque and calcified plaque arteries have fast components that are faster (x ps) than that of normal arteries. The slow component of arteries with white plaque in the 390 nm band is greater (3 0.4 ps) than that of normal arteries or arteries with calcified plaque (2.2 0.2 ps, 2.6 0.2 ps). These preliminary results indicate that arteries with white plaque, arteries with calcified plaque, and normal arteries may be distinguished from one another.

Abstract:
Coupled cavity modelocking with a resonant nonlinearity in the coupled cavity is a new technique to passively modelock laser media with small gain saturation cross section and was introduced with a Ti:Sapphire laser [1,2]. As an extention of this work, we passively modelocked a cw pumped Nd:YLF laser with an InGaAs/GaAs strained layer MQW [3] resonant nonlinearity in the coupled cavity and generated 4 ps pulses at a wavelength of 1.047 µm with 390 mW average output power at a 250 MHz repetition rate, corresponding to a 1.6 nJ pulse energy. The RPM Nd:YLF laser consists of a linear folded, astigmatically compensated cw Nd:YLF cavity with a 3% coupler to an external low-Q cavity with a nonlinear InGaAs/GaAs quantum well end reflector. The overall tunable output coupler is in the coupled cavity and is typically ≈ 50%. The Nd:YLF rod is cw end- pumped with only 1.5 W at 798 nm from a Ti:Sapphire laser. RPM does not require active cavity length control for stable modelocked pulse trains [4], Diode pumping and higher repetition rates should deliver a practical high power high repetition source for photonic switching applications.
