The development of effective pharmacological treatments for severe mental disorders between the 1950s and 1970s was one of the great triumphs of medical science. However, progress since then has been slow. Most drugs in current use are variants of those previously on the market, albeit with less worrisome side effects. Yet prescription rates have skyrocketed. Moreover, atypical antipsychotics, despite their side effects and warnings from watchdog organizations, are currently being widely used for sedation. Psychiatrists are prescribing these drugs for many, if not most, of their patients. All have a proper use—one cannot treat psychosis without antipsychotics. However, the situation in common mental disorders is different. Although antidepressants are essential in melancholic depressions, they are much less effective in mild to moderate cases. Moreover, protocols that advise drug combinations for “treatment-resistant” cases are not well based in evidence.

Abstract:
When properly prescribed, benzodiazepines and related “Z” drugs, are usually safe and effective. However, some patients experience lack of efficacy, severe adverse effects, and/or protracted withdrawal symptoms. Unfortunately, there is no reliable way to predict outcome prior to treatment. Use has dramatically expanded, to the point where some experts suggest a disconnect with actual medical need. With increased and longer prescribing there has been a corresponding increase in the “down-side” of these drugs. Benzodiazepines, as all drugs, produce some degree of normal physiologic tolerance and physical dependence. But for some patients withdrawal can result in a bewildering array of symptoms, that can persist for protracted time periods, difficult to understand and live with. Although there is currently no clear mechanistic explanation, some potentials include alterations of receptor number, promoters of receptor protein synthesis or degradation, absorption, distribution, metabolism, and elimination, GABAA-receptor function or subtype-distribution, or involvement of peripheral benzodiazepine binding/receptor sites. This book attempts to bring benzodiazepine use under a more rational paradigm and reduce the incidence of side-effects and drug–drug interactions (DDI). It is the first devoted to take on this responsibility. Use, overuse/misuse, side-effects, DDI, physiology, and withdrawal are reviewed by expert clinicians and basic scientists in-depth. The book challenges the medical community to take seriously the use of this class of drug and to ameliorate prescribing behavior. The case is made for limiting initiation and duration (2–4 weeks) of use, and careful, supported discontinuation. We laud and suggest increased research into this class of drug and it’s “down-side.”

Abstract:
Gastrointestinal motility disturbances in critically-ill patients often require treatment with prokinetic drugs. The aetiology of motility disturbances is complex, and involves electrolyte imbalances, hypervolaemia, reduced intestinal secretion, adverse effects of drugs (catecholamines, opioids, or sedatives) and disease- or treatment-related changes of microflora. However, the choice of prokinetics is narrow, and the multiplicity of pathophysiological mechanisms often limits their efficacy. Gastroparesis can be managed with gastrokinetics such as domperidone, metoclopramide and erythromycin. Their choice depends not only on efficacy, but also on adverse effect profile. The arrhythmogenic potential of domperidone limits maximum daily dose and treatment duration. Metoclopramide and erythromycin induce tachyphylaxis, which restricts treatment duration. The combination of metoclopramide and erythromycin serves as rescue therapy in severe gastroparesis. Neostigmine and laxatives are used to manage colonic paralysis, and these treatment options may eventually be extended by drug candidates, such as prucalopride, lubiprostone, and linaclotide, whose utility in the ICU awaits to be evaluated. Neostigmine’s prokinetic efficacy in colonic paralysis is limited, but well documented in patients with acute colonic pseudo-obstruction (Ogilvie syndrome). Care is advocated in dosing because higher doses of neostigmine inhibit motility. Alternative options include osmotic and stimulant laxatives, especially for prophylactic use. The opioid receptor antagonist alvimopan is used for the short-term management of post-operative ileus, while methylnaltrexone is indicated in palliative care and chronic pain management. Since its efficacy in critically-ill patients remains to be proven, the use of methylnaltrexone in the ICU is off-label and requires proper documentation.

The rise in the incidence of infections is caused by multi drug resistant (MDR) bacteria, it is essential to elucidate the basic mechanism of antibiotic resistance to discover effective methods for diagnosis and treatment of infections. The use of pathogen-specific probes offers a faster alternative for pathogen detection and could improve the diagnosis of infection. High resolution melting analysis techniques are useful for the detection of multi drug resistant pathogens. Rational Structural Based Drug Design is a common method to identify a lead compound and take it forward for further developments. This book provides information about recent strategies involved in the diagnosis and treatment of infections caused by MDR bacteria. The volume covers the use of molecular probes for the quantification of pathogenic bacteria, along with other techniques mentioned above. Chapters also cover the use of identification of novel drug targets from the Lipid A biosynthesis and also from quorum sensing mediated biofilm formation in MDR bacteria. Chapters also cover herbal alternatives for the treatment of MDR bacteria like the use of Cassia aungustifolia in treatment of various diseases. The reference is suitable for biomedical students, cellular and molecular biologists.

This chapter focuses on diagnostic and procedural imaging techniques that are essential for the pain medicine practitioner. Attention is given to most modern imaging modalities, including ultrasonography, fluoroscopy, computed tomography, and magnetic resonance imaging. A discussion regarding the use of imaging to elucidate a problem with an implanted intrathecal drug delivery system is included as well. The acquisition of antimicrobials without a prescription is a global concern. This practice is thriving in countries that lack adequate legislation or where regulations are not properly enforced. The Pan American Health Organization (PAHO) and its member states in the Region of the Americas approved the Global Action Plan on Antimicrobial Resistance, which recognizes antimicrobial resistance as a threat to global public health that requires a multisectoral response. To tackle antimicrobial resistance, a worldwide change in behavior is needed in terms of how these drugs are used and acquired. National approaches are required to address the indiscriminate use and over-prescription of antimicrobials, and to enforce regulations on prescription and acquisition practices. The objective of this communication handbook is to help communication professionals and health program officials develop strategies to raise awareness and promote the importance of the appropriate use of antimicrobials among different stakeholders; raise public awareness about the importance of obtaining antimicrobials with a prescription in order to achieve multisectoral collaboration to ensure compliance with laws and regulations on this issue; and promote a change in behavior regarding the appropriate use and acquisition of antimicrobials by everyone involved. The target audiences for this handbook are the general population (including adolescents, children, and child caregivers/parents of children), healthcare professionals (including pharmacists and pharmacy staff), and various stakeholders (government officials, professional societies, medical organizations, the private sector, local leaders, and health-influencers, among others).
