L'articolo passa in rassegna la lunga e complessa evoluzione, politica e normativa, delle procedure di bonifiche dei siti contaminati, individuando criticità, contraddizioni, opacità, lentezze e manifestazioni di illegalità. A livello generale, almeno due sono emerse come le principali criticità di sistema, che hanno fatto da freno alle attività concrete di bonifica: 1) criteri non idonei di scelta per l'identificazione del sito da bonificare; 2) errata ripartizione delle responsabilità tra gli enti territoriali, che spesso non hanno adeguati mezzi e risorse, soprattutto in termini di personale qualificato per rispondere a procedure così complesse. Anche da questa prospettiva, l'inquinamento diffuso provato anche dalla presenza dei siti da bonificare rappresentano il punto finale di un processo di sviluppo industriale e consumistico privo di regole e di limiti. Deregulation che è valsa per almeno un quarantennio, un ampio arco temporale in cui il territorio nazionale ha visto l'esplosione dei siti inquinati (di interesse nazionale, regionale o comunale), dove per almeno 200 casi l'Unione Europea ha addirittura aperto due distinte procedure d'infrazione. Procedure che sono costate all'Italia oltre 40 milioni, più una penalità semestrale iniziale di 42.800.000 euro. Per far fronte a questo salasso il 24 marzo 2017 il Governo dell'epoca ha costituto il Commissario Straordinario di Governo per svolgere la missione di bonifica e messa in sicurezza degli 81 siti di discarica abusivi sotto procedura di infrazione a seguito della Sentenza di condanna sanzionatoria della Corte di giustizia dell'Unione Europea del 2 dicembre 2014.

Negli ultimi venti anni la chirurgia endoscopica del basicranio ha osservato continui sviluppi tecnici e tecnologici. Lendoscopia 3D e l alta definizione (HD) 4K hanno fornito grandi vantaggi in termini di visualizzazione e di risoluzione spaziale. L ultra HD 4K, recentemente introdotta nella pratica clinica, determinerà i prossimi passi soprattutto nella chirurgica endoscopica del basicranio. I pazienti sono stati operati attraverso un approccio transnasale transfenoidale endoscopico, utilizzando un endoscopio Olympus NBI 4K UHD con ottica 4 mm 0 ° Ultra Telescope, lampada allo xeno 300 W (CLV-S400) predisposto per la tecnologia narrow band imaging (NBI) collegato con una videocamera ad un alta qualità unità di controllo (OTV-S400 - VISERA 4K UHD) (Olympus, Tokyo, Giappone). Due schermi, un 31 Monitor - (LMD-X310S) e quello principale ultra-HD 55 a pollici ottimizzati per la riproduzione immagini UHD (LMD-X550S). In casi selezionati abbiamo usato un sistema di navigazione (Stealthstation S7, Medtronic, Minneapolis, MN, Stati Uniti). Abbiamo valutato 22 adenomi ipofisari (86,3% macroadenomi; 13,7% microadenomi). Il 50% non erano secernenti (NS), 22,8% GH, 18,2% ACTH, 9% PRLsecernenti. 3/22 erano recidive. Nel 91% dei casi abbiamo raggiunto la rimozione totale, mentre nel 9% la resezione subtotale. Un followup medio di 187 giorni, durata media del ricovero era 3,09 ± 0,61 giorni. Tempo chirurgico 128,18 ± 30,74 minuti. Abbiamo avuto solo 1 caso di fistola intraoperatoria a basso flusso senza ulteriori complicazioni nel follow up. Il 100% dei casi non ha richiesto emotrasfusione. La visualizzazione e lalta risoluzione del campo operatorio hanno fornito una vista dettagliata di tutte le strutture anatomiche e patologie e permesso il miglioramento della sicurezza e lefficacia della procedura chirurgica. Il tempo operatorio è stato simile a quello dellendoscopio HD standard 2D e 3D, come la fatica fisica era paragonabile ad altri in termini di ergonomicità e peso.

Il Covid-19 è comparso a Wuhan, in Cina, nel dicembre del 2019. La malattia causa difficoltà respiratorie e numerose altre complicazioni quali stanchezza, febbre, dolori muscolari e, in alcuni casi, diarrea e vomito, e può essere fatale.La correlazione tra malattie infettive e disagio psicologico è già emersa durante altre epidemie, come quella di SARS (Severe Acute Respiratory Syndrome) e di Ebola. Questa revisione sistematica della letteratura è volta ad indagare le ripercussioni psicologiche a breve termine che il Covid-19 può avere sulla popolazione generale. Mediante una procedura di analisi tematica sono stati individuati dei temi ricorrenti negli articoli selezionati. I risultati più significativi riguardano l'impatto dei social media e le ripercussioni a livello di ansia e depressione.In particolare, le notizie false trasmesse dai media alimentano ulteriormente l'ansia provata dalla maggior parte della popolazione e fattori quali l'essere giovani o anziani, l'essere studenti, poco istruiti, divorziati o vedovi contribuiscono ulteriormente a peggiorare la sintomatologia. Seguire, invece, le linee-guida proposte dal Governo e avere informazioni chiare e comprensibili si sono rivelati due fattori di protezione contro l'insorgenza di sintomi ansiosi. Dalla ricerca è emerso un bisogno di monitoraggio per le popolazioni più a rischio e la necessità di studiare questi fenomeni sul lungo periodo, replicando gli studi in Paesi occidentali.

RiassuntoScopo- Presentare una esaustivareviewdegli studi riguardanti l'epidemiologia del DPTS condotti nella popolazione generale, tra i soggetti a rischio, e, infine, tra gruppi clinicamente selezionati.Disegno- AttraversoExcepta Medica PsychiatryCD-ROM 1980-1993 (ottobre), utilizzando come parola chiave «Post-Traumatic Stress Disorder», sono stati identificati 1.057 articoli pubblicati nel periodo considerate Sono stati anche consultati altridata basedella letteratura medica (MEDLINE CD-ROM 1988-1993); è stata quindi operata una ricerca manuale su tutti i numeri delJournal of Traumatic Stress. Risultati- In totale, 135 lavori che hanno soddisfatto i criteri di inclusione prescelti sono stati inclusi nella review. I due terzi (n = 86, 64%) di queste ricerche sono state condotte negli USA. Solo 8 (6%) sono le indagini effettivamente realizzate nei paesi del Terzo Mondo. L'ampiezza del campione varia da un minimo di 11 soggetti, numero riscontrato in due studi, sino ad un massimo di 22.436, per un campione medio di 500 e mediano di 108. Per quanto attiene ai metodi di valutazione, in un terzo degli studi (n = 45, 33%), i ricercatori hanno impiegato un questionario (auto- o etero- somministrato). In un altro terzo delle ricerche elencate (n = 44, 33%) e stata somministrata un'intervista strutturata (la DIS, la SCID, o la SADS), mentre nei rimanenti studi la valutazione diagnostica si è basata o su una procedura clinica non strutturata, o sulla somministrazione di altri strumenti specifici dai quali è possibile inferire una diagnosi di DPTS (M-PTSD, IES, SCL-90-R,o pochi altri). In 77 studi (57%) i ricercatori hanno basato la loro valutazione sui criteri diagnostici propri del DSM-III, mentre in altri 55 (41%) su quelli del DSM-III-R. La prevalenza del DPTS e analizzata quindi separatamente per le diverse popolazioni studiate.Conclusioni- Nell'arco di soli 13 anni, a partire cioe dalla definizione di criteri diagnostici operazionali ben definiti per il DPTS, sono stati condotti numerosi studi volti ad indagare la prevalenza, i fattori di rischio, la storia naturale, il decorso e l' esito di questo disturbo tra campioni diversi di popolazioni a rischio; inoltre, anche il livello qualitativo di queste ricerche, per quanto attiene alia sofisticazione metodologica, si è accresciuto sensibilmente in un tempo tutto sommato breve. Molte aree, pero', restano tuttora inesplorate, ed inoltre appare imperativo avviare ricerche estensive tra le popolazioni dei paesi in via di sviluppo, maggiormente esposte a disastri naturali o provocati dall'uomo.

AbstractIamblichus took a clear stand in the late-antique debate on the origin of matter: in his view, matter is generated, in the sense that it is brought about by a higher principle (primary causes) outside physical time. Two passages testify to this conception: In Tim. fr. 38 Dillon (= Procl. In Tim. I, p. 386.8-13 D.) and Ad Porph. (De myst.) p. 196.14-19 Saffrey-Segonds (= VIII3, p. 265.6-10 Parthey). The relation between the two, however, is rather problematic and scholars stand divided: some maintain that Proclus drew the conception expounded in In Tim. fr. 38 from Iamblichus’ commentary by the same title, others that he drew it from Ad Porph. The question is a far from marginal one. It is connected to the problem of the attribution of Ad Porph. and strongly bears upon the issues of Iamblichus’ work method and of his doctrine. Moreover, the very reconstruction of the text of Ad Porph. closely depends on how the analogy connecting it to In Tim. fr. 38 is understood. A reading of the two passages, first each in its own context and then in the light of one another, reveals that In Tim. fr. 38 is based on the lost commentary on the Timeaeus. Consequently, the two passages are seen to adopt a reverse perspective on the subject, so to speak: In Tim. fr. 38 elucidates Plato through Hermes, while Ad Porph. elucidates Hermes by introducing Platonic notions. Finally, with regard to Ad Porph. p. 196.14-19, a suggestion is made to retain the transmitted text.

Dopo aver esaminato le relazioni tra questi testi e altri commenti antichi della Bibbia da cui essi dipendono, l'Autore dimostra che Galileo non ha probabilmente esaminato tutti e sette i testi in prima persona, ma che la sua fonte era in realtà il commento di Magalhaens al libro di Giosuè, pubblicato nel 1612 (il più recente dei sette in cui gli altri sei sono citati); e che, oltre alla confusione già esistente nei testi citati, Galileo ha ulteriormente confuso i riferimenti testuali.

O-FEES (O-FEES) è una procedura endoscopica, concepita per visualizzare direttamente la fase orale della deglutizione. Nella prospettiva di un utilizzo clinico, la fattibilità, la sicurezza e laccettabilità dellO-FEES è stata inizialmente valutata. Successivamente, la procedura è stata confrontata con il gold standard radiologico. Laccettabilità dellO-FEES è stata confrontata con quello della FEES per mezzo di un questionario a dieci punti, sottoposto ad un campione di 52 pazienti ambulatoriali che lamentavano di disturbi della deglutizione. Il modello Repeated measure analysis of variance (rm-ANOVA) è stato utilizzato per testare la differenza media di accettabilità delle due procedure nello stesso soggetto. Successivamente un altro campione di 8 pazienti ambulatoriali di sesso maschile, è stato sottoposto alla registrazione simultanea di O-FEES e videfluoroscopia (VFSS). Laffidabilità inter-individuale, utilizzando 10 parametri radiologici di riferimento, fra O-FEES e VFSS, è stata determinata alla cieca tra due giudici. La concordanza inter-individuale tra i due giudici, per i punteggi di O-FEES e della VFSS è stata determinata con il Single score intra-class correlation coefficient (ICC). Le differenze FEES e O-FEES tra le risposte per ogni domanda e tra tutte le domande considerate, sono risultate statisticamente significative (rm-ANOVA; F-statistica p < 0,001). La concordanza inter-individuale fra la valutazione endoscopica e radiologiche tra i due valutatori, ha mostrato una forte correlazione intra-classe (ICC) (intervallo di confidenza al 95%): 0,875 (0,373-0,979) e 0,921 (0,542-0,986) rispettivamente. Il test di Bland-Altman suggerisce che le due metodiche producono risultati analoghi. Nella pratica clinica e confrontata con la FEES, lO-FEES è una procedura tollerata e sicura. Rispetto al gold standard radiologico, lO-FEES offre informazioni affidabili sulla preparazione e propulsione orale del bolo.

Filippo Zamboni (Trieste, 1826-Vienna, 1910) si contraddistingue per i suoi studi su Dante, riportati, per esteso, in gran parte delle sue opere. Tra queste, ci siamo soffermati su due in particolare Gli Ezzelini, Dante, e gli schiavi. Pensieri storici e letterari (1864) e Il bacio nella luna: Pandemonio. Ricordi e bizzarrie (1911). Oltre all'analisi delle sue interpretazioni di alcuni passi sue interpretazioni di alcuni passi della Divina Commedia, è stata approfondita la singolare riscrittura del contrappasso dantesco destinato ai peccatori del quarto cerchio dell’Inferno. Nel Pandemonio, l’autore immagina puniti, tra gli avari, i proprietari delle miniere che non si potevano limitare a spingere pesanti massi, come nell’immaginario dantesco, ma si vedono condannati a simulare per l’eternità il lavoro massacrante dei minatori. Sono state sviluppate inoltre le considerazioni sullo scrittore daparte della critica, riportando le recensioni e i giudizi espressi nella stampa periodica con particolare riferimento agli scritti della poetessa istriana Giuseppina Martinuzzi, che più di altri si è occupata di Filippo Zamboni. A tale proposito, ci si è concentrati su un manoscritto della Martinuzzi, intitolato Alcuni stampati e manoscritti di Filippo Zamboni ed altri che di lui dicono, conservato presso in Museo popolare di Albona, che ci ha permesso di approfondire l’importanza di questo dantista triestino, oggi poco conosciuto.

Prendendo spunto dal dibattito sul rapporto fra clinica e ricerca, l'Autore ripercorre i primi passi dell'attuale "ricerca concettuale" a partire dal pionieristico lavoro di Joseph Sandler e dei suoi colleghi alla Hampstead Clinic di Londra. Lo scopo è quello di mostrare come lo spirito della tradizione di ricerca a cui ha dato avvio Sandler possa rivelarsi uno strumento prezioso affinché si rafforzi l'interscambio fra la "cultura della ricerca" e la "cultura della cura", impedendo cosě che si cristallizzino due comunità isolate e divise in quanto a filosofie di lavoro, metodi conoscitivi e obiettivi.

Il presente lavoro intende realizzare una ricostruzione critica dell’evoluzione storico-normativa concernente la disciplina dell’introduzione e trattazione della controversia negli ordinamenti italiano e spagnolo fra XIX e XX secolo. In questa prospettiva si dedica particolare attenzione alla genesi dei rispettivi modelli di codificazione ‘moderna’ e al successivo dibattito sulle riforme e tentativi di riforma precedenti l’emanazione dei codici attualmente in vigore. Il tutto nell’ottica di evidenziare significative affinità fra esperienze evolutive tradizionalmente considerate a sé stanti.Abstract This work intends to provide a critical reconstruction of historical development concerning the legal framework of civil pre-trial procedure in Italian and Spanish legal systems between the 19th and 20th century. In this perspective special attention is focused on respective models of ‘modern’ codification as well as on subsequent reforms until the enactment of current civil procedure codes. All with a view to highlighting relevant similarities between historical evolutions traditionally considered in their own right.

mondo universitario ha subito un’ondata di cambiamenti che si possono ricondurre alla ricerca dell’eccellenza, declinata secondo le due dimensioni della valutazione e della rendicontazione. Tre sono quelli più evidenti: il primo, il passaggio da una ricerca curiosity driven a una ricerca funzionale al raggiungimento di risultati valutabili in tempi brevi; dalla ricerca pura a quella applicata, da un approccio problem-making a uno problem-solving, da una conoscenza come processo a una conoscenza come prodotto, da un modello disinteressato a uno utilitaristico (Barnett, 1994); il secondo, riguardante l’offerta formativa: dal momento che si è modificato il modo di concepire l’apprendimento; i curricula tendono a essere definiti in termini di risultati di apprendimento predefiniti (Blackmore, 2016); il terzo, peculiare della struttura amministrativa: dal momento in cui sono divenute essenziali una serie di nuove sovrastrutture (programmazione, valutazione, controlli, comunicazione) rispetto al mandato originario della struttura universitaria si registra un aumento consistente del personale delle strutture amministrative. Questi cambiamenti devono fare i conti con la perdita di prestigio della vita accademica, il cambiamento del ruolo dello studente, che è diventato sempre più importante e l’aumento delle procedure burocratiche che rischiano di ingessare un sistema un tempo caratterizzato da un’elevata autonomia.
Per consentire alle strutture universitarie di affrontare le sfide culturali a partire dagli anni Settanta nelle università nord-americane si sono strutturate iniziative finalizzate allo sviluppo e alla promozione di una migliore offerta formativa. Tali iniziative vengono definite con l’espressione Faculty Development (FD), una policy accademica finalizzata a creare le condizioni per un miglioramento delle competenze di tutti coloro che sono coinvolti nelle attività svolte in un ateneo. Nella realtà italiana emerge la mancanza di una vera politica di formazione al teaching per i ricercatori e i docenti universitari, per non parlare dell’esigenza di superare il pregiudizio, di gentiliana memoria, secondo il quale non è necessario apprendere a insegnare, ma sia sufficiente avere successo nella ricerca, cui si aggiunge nell’ultimo decennio una continua e affannata richiesta al personale accademico di azioni organizzative, valutative e documentali, che assorbono tempo e energie senza il supporto di adeguati apparati gestionali e senza predisporre indagini valutative capaci di misurare l’effettivo esito di tutte queste azioni. L’effetto finale è un evidente declino (Capano et al., 2017) dell’istituzione universitaria. Si può ipotizzare che la cultura del organizzazione propria del Faculty Development possa contribuire nel contesto italiano a fornire azioni a supporto del cambiamento: è quanto mai essenziale dotare gli atenei di risorse funzionali a riqualificare la vita accademica, fornendo al personale accademico gli strumenti necessari per performare una buona scholarship, realizzare un’efficace offerta formativa e attuare adeguate forme di terza missione, capaci di incrementare la vita culturale della comunità. Il presente studio si propone come un’analisi sistematica della letteratura sul tema del Faculty Development, che persegue l’obiettivo di sviluppare una disamina estesa dell’oggetto, in modo che l’esplicitazione della datità raccolta fornisca un’analisi del fenomeno che possa essere di supporto a un’avveduta educational policy nel campo della formazione universitaria.
Nel contesto italiano ad oggi non esiste una cultura di attenzione ai contesti di apprendimento universitario. L’offerta formativa è concepita come offerta di pacchetti curriculari e la predisposizione delle condizioni di apprendimento per il conseguimento del titolo universitario si risolve nella organizzazione di una serie di lezioni, frontali o laboratoriali, senza che tutto questo sia innervato da una specifica intenzionalità didattica. Questa immagine poco confortante non intende affatto trascurare tutti i casi di buone prassi sviluppati nei vari corsi di studio, ma il buono che emerge è demandato all’impegno del singolo, senza che l’istituzione universitaria si interroghi sul come predisporre le condizioni per il potenziamento della qualità dei processi di apprendimento. A fronte di questa situazione la necessità di migliorare la qualità dell’insegnamento non è mai stata così stringente e sfidante come lo è oggi, in un clima di continuo cambiamento della formazione superiore. Nuove tendenze definiscono la formazione superiore, attraversando confini istituzionali e nazionali. Essi influiscono sul modo in cui un insegnamento efficace viene concettualizzato, condotto e supportato, valutato, valorizzato e riconosciuto. È necessario affrontare temi quali l’inadeguata preparazione per il lavoro accademico nei corsi di studio magistrali, l’incapacità dei docenti a trasferire competenze, la crescente complessità degli ambienti accademici, le attese e le responsabilità istituzionali, la necessità di preparare meglio gli studenti con bisogni diversi, e la necessità di stare al passo con i balzi della conoscenza e i cambiamenti nelle professioni. Migliorare la qualità della didattica è inoltre essenziale perché consente di ridurre il numero degli abbandoni. È venuto il momento di transitare da un’offerta formativa di tipo episodico a una prospettiva di esperienze di apprendimento in continuità nel tempo, per accompagnare la formazione dei docenti in un modo strutturalmente organizzato (Webster-Wright, 2009).
Sulla base della rilevazione fenomenica, sono emerse le seguenti domande di ricerca: che cosa è il FD? Cosa consente di fare? Come si mette in pratica? Quali sono le potenzialità? Quali sono i limiti? Il FD ha il compito di incentivare i docenti ad interessarsi ai processi di insegnamento e apprendimento e a procurare un ambiente sicuro e positivo nel quale fare ricerca, sperimentare, valutare e adottare nuovi metodi (Lancaster et al. 2014). È finalizzato a promuovere cambiamento sia a livello individuale sia a livello organizzativo. Occupa un posto centrale il miglioramento delle competenze di teaching (Steinert, 2014). Due importanti obiettivi sono rappresentati dalla promozione delle capacità di leadership e di gestione dei contesti (Steiner et al., 2012). Una volta definite le metodologie del teaching, che possono essere oggetto di apprendimento da parte del personale accademico, è risultato necessario identificare le principali modalità formative che un centro di Faculty Development (FDc) dovrebbe mettere in atto per favorire l’apprendimento delle competenze didattiche. Per comprenderne la funzione reale è stato utile prendere in esame le attività proposte dai più importanti centri del panorama accademico nordamericano, analizzandone la struttura organizzativa, le risorse disponibili ed identificandone le due figure principali: il responsabile dell’organizzazione dei processi formativi e il responsabile della struttura. L’analisi dei casi ha consentito di evidenziare i molteplici servizi che possono essere forniti da un FDc. Questa analisi di realtà è risultata molto utile poiché ha offerto indicazioni pragmatiche ai fini di una politica accademica innovativa anche in ambito italiano. Alla luce degli argomenti sviluppati è stato possibile ipotizzare anche per gli atenei italiani l’istituzione di “Centri per la professionalità accademica”, indicando possibili iniziative da essi realizzabili, che potrebbero trovare spazio nella realtà del nostro paese.

AbstractIn graphic and industrial printing applications inkjet technologies are gaining attention and market coverage. Beside some key success factors as scalability there is an inherent difficulty of temporally or permanent defective/non operating nozzles. These non working nozzle have a severe impact on printed quality. Several approaches are known to reduce these artefacts. One is to use multi-pass printing, sometimes in combination with the use of defect nozzle information. Another is to detect these nozzles direct or indirect and use for example so called reserve nozzles. A predecessor thereof is the detection of pen lines (Beauchamp et al (1992) Hewlett-Packard J 12:35–41). In multi-nozzle systems individual or groups of nozzles are addressed to fire using a matrix scheme. This method reduces the number of used address lines.Here an automated working scheme is sketched to firstly identify defect nozzles using a sample print by means of a dedicated pattern. A digital picture taken by scan or camera is the basis of an image analysis procedure afterwards. Secondly it is shown that, once the matrix configuration is known, possible defects of the address lines may be identified too. Finally a pattern dedicated to a specific print head is introduced to check the later fact visually.

Coronary artery by-pass graft (CABG) is the main mode of revascularization procedure for significant left-main coronary artery (LMCA) disease unless contraindicated or unsuitable for surgery, and in patients with complex coronary anatomy. Percutaneous coronary intervention (PCI) of left-main (LM) is emerging as an alternative to CABG especially in patients with low syntax score with suitable coronary anatomy for PCI, and life-saving emergency situations like acute coronary syndrome (ACS) with hemodynamically unstable, and high risk group patients who are unsuitable coronary anatomy for grafting or due to associated co-morbidities.

Abstract Chronic urinary retention is chronic or repeated inability to empty the bladder, despite the ability to pass some urine. High-pressure chronic retention is a risk factor for renal failure. If renal failure is present, catheterization is needed to recover renal function. Catheterization may be followed by post-obstructive diuresis, due to the impaired concentrating ability of the renal tubules, and the accumulated salt, water, and toxins. Consequently, fluid replacement might be needed while renal function improves. Once stabilized, a period of intermittent catheterization may help improve function. Urodynamic testing enables determination of likelihood of response to surgical intervention. Where bladder outlet obstruction is the cause of chronic urinary retention, there is a good chance of responding to procedures such as transurethral resection of the prostate and laser prostatectomy. If detrusor underactivity is the cause, there may be minimal benefit from interventional procedures.

Constipation is common among older adults, in general. However, it is very common among people with Lewy body disorders, and the reason is dysautonomia. Lewy body disorders tend to impair control of gut motility by the autonomic nervous system. At the stomach level, bloating may develop when the stomach fails to empty into the upper small intestine. At the other end, constipation is the consequence of Lewy processes affecting motility in the colon. Colon motility (peristalsis) is what moves the remnants of digested food (stool) to the rectum for expulsion. These regions are shown in Figure 15.1. Drugs that block the neurotransmitter acetylcholine are notorious for worsening constipation; these include medications used to treat urinary urgency (overactive bladder). All of the anticholinergic drugs for bladder overactivity that were listed in Table 12.1 cause constipation, as does another bladder drug, trospium (Sanctura). The tricyclic drugs for depression shown in Table 12.1 have variable anticholinergic properties and also tend to be constipating. One needs to balance benefits against the side effect of constipation if considering these medications. In the setting of DLB or PDD, constipation is typically due to autonomic nervous system dysfunction, often exacerbated by medication side effects. However, there are exceptions and the primary care clinician or internist should consider whether colonoscopy is appropriate. This procedure involves inserting a scope into the anus and then advancing the instrument to visualize the entire colon. In this way hidden colon cancers are detected before they become deadly. It is common knowledge that several natural remedies help prevent constipation: fruits, vegetables, fluids, and fiber. Individuals with constipation should make sure that their diet includes adequate fruits, which make a good snack. Meals should include vegetables, such as green beans, peas, and squash; catsup and potatoes do not count as vegetables. Intake of six to eight tall glasses of water, juice, or other fluids may help maintain moisture in the stool, making it easier to pass. Finally, fiber needs to be included in the diet in order to give the stool bulk. These strategies are often insufficient for persons with Lewy disorders, and additional measures are often necessary.

Chromatography has been employed for the separation of proteins and other biological macromolecules on the basis of molecular size since the mid 1950s when Lathe and Rutheven employed modified starch as a media for separation. Porath and Flodin developed the technique further using cross-linked dextran and coined the term gel filtration. Some confusion over nomenclature has been created by the term gel permeation, used to describe separation by the same principle in organic mobile phases using synthetic matrices. It is now generally agreed that the terms gel filtration and gel permeation do not accurately reflect the nature of the separation. Size exclusion Chromatography (SEC) has been widely accepted as a universal description of the technique and in line with the IUPAC nomenclature this term will be adopted. The historical development of SEC for protein separation has been reviewed. SEC is a commonly used technique due to the diversity of the molecular sizes of proteins in biological tissues and extracts. In addition to isolating proteins from crude mixtures, SEC has been employed for many roles including buffer exchange (desalting), removal of non-protein contaminants (DNA, viruses), protein aggregate removal, the study of biological interactions, and protein folding. The principle of size exclusion is based on a solid phase matrix consisting of beads of defined porosity which are packed into a column through which a mobile liquid phase flows. The mobile phase has access to both the volume inside the pores and the volume external to the beads. Unlike many other chromatographic procedures size exclusion is not an adsorption technique. Separation can be visualized as reversible partitioning into the two liquid volumes. The elution time is dependent upon an individual protein’s ability to access the pores of the matrix. Large molecules remain in the volume external to the beads as they are unable to enter the pores. The resulting shorter flow path means that they pass through the column relatively rapidly, emerging early. Proteins that are excluded from the pores completely, elute in the void volume, V0. This is often determined experimentally by the use of a high molecular weight component such as blue dextran or calf thymus DNA.

Today a significant amount of research has focused on trying to apply the advances in information technology to governmental services. One endeavor has been the attempt to apply it to “electronic voting.” Unfortunately, while questionable secure e-voting technology has been widely deployed, the same cannot be said for cryptographic based ones. There is one type of “voting” which has received only limited attention concerning applying these technology advances, the type of voting that takes place within a legislative body. At first glance, it may not appear difficult to institute electronic voting in a legislature, for it may seem that one only needs to apply the traditional security mechanisms that are used to safeguard networked systems, but as we soon outline there will be significant security risks associated with an electronic legislature. One of our concerns is that entities may attempt to implement an electronic version of a legislature without realizing all the risks and implementing all the needed security mechanisms. In fact, there have been occasional instances of some entities attempting to create some electronic/digital form of legislature, for example (Weidenbener, 2004). In any legislative vote, the legislature’s ability to pass or to not pass legislation should be interpreted as the legislature deciding whether to “sign the proposal” into “law.” Thus, “law” is a signature; anyone can verify that a “proposal” is a “law” by applying the signature verification procedure. As we move towards electronic applications of governmental services, it is only natural when this is applied towards legislatures we will replace the “written law” by a “digital signature” (here the use of the term law can be replaced by any internal regulation and a legislature by any regulatory body). The underlying aspect of the article is the security considerations that need to be applied when this is implemented. The question why consider an electronic legislature is important. The fundamental reasons for applying today’s information technology to government and its services have always focused on that it would bring improved services and allow greater accessibility of government to its constituents. An electronic legislature would most certainly improve the legislative service. It will allow for the legislators to be mobile, they will no longer need to be tied to the legislative house to provide representation. Many industrial employers allow their workers to telecommute to work, it is a realization by the employers that these workers are valuable, as well as a recognition that the workforce and the time constraints on the workforce has changed. In many cases, without this option, these workers may leave the workplace. This same reasoning of a valued worker should be applied to our legislators. Further, it does not make sense that today we would allow a subset of the legislature to make and pass laws due to absenteeism, especially in light that many of the required mechanisms to bring about a mobile “electronic legislature” are available. One can argue that by allowing legislators to occasionally telecommute will provide an improved workforce (this argument is motivated by the same reason that private industry utilizes “telecommuting”). We also observe that an electronic legislature should provide the constituents greater access to their legislators. A final argument for an electronic legislature is that it will provide continuation of government in the case of some drastic action like a terrorist attack. In the fall of 2001, the legislative branch of the U.S. federal government came under two attacks. The first attack was performed by Al Qaeda operatives (who it is speculated intended to fly one of the planes into the U.S. capital), and a second attack by an unknown entity who contaminated parts of the U.S. senate (and it offices) with anthrax spores. This second attack was successful in that it denied the Senate the ability to convene for several days. Although such terrorist’s attacks on the legislative branch may appear novel, at least in the U.S., such attacks have been precipitated in other countries for some years (PBS, 2001). The U.S. government has recognized the need to develop a means for the continuity of government in the wake of such disasters (Continuity of Government Commission, 2002), one such solution is to utilize an e-legislature.

The aim of the chapter is to analyze the significance and role of Roman law as ius commune in East Central Europe (Ostmitteleuropa) from the Middle Ages up until today. The notion of East Central Europe will be pragmatically exemplified for the purposes of this contribution within the context of the Lands of the Crown of Saint Stephen. This territory was and is ‘the very heart’ of East Central Europe, as it comprises, in their entirety or partly, the following present-day states: Hungary, Croatia, Slovakia, Austria, Poland, Romania, Serbia, Slovenia, and Ukraine. The centrality and importance of the Lands of the Crown of Saint Stephen within East Central Europe also guarantee that the experience of Roman law as ius commune in these territories is not unimportant and has a certain level of paradigmaticity. The most important source of traditional pre-1848 law in the Lands of the Crown of Saint Stephen was undoubtedly the Tripartitum (1514), which represents one of the milestones of East Central Europe’s legal tradition and culture. Despite the explicit declaration that Roman law and canon law are the very basis of the law of Archiregnum Hungaricum (omnia fere iura regni huius originaliter ex pontificiis caesareique iuris fontibus progressum habeant), this legal collection was, in reality, a compilation of customary law and a powerful legal practice forming work that hindered any major legal transfer. Regardless of the fact that European ius commune was not a direct source of law in the pre-1848 period, there were definitely some ‘channels’ through which Roman legal tradition exerted a considerable influence and impact in the Lands of the Crown of Saint Stephen (e.g., procedural law manuals like Kitonich’s Directio Methodica and the inclusion of Digesta 50, 17 in Corpus Iuris Hungarici), creating the phenomenon called tacita receptio. Only since the second half of the 19 th century onward has Hungarian judicial practice and doctrine – due to the withering away of feudal relations and consecutive failed attempts to pass a modern national civil code – gradually elevated Roman private law in the form of ius commune to the level of a subsidiary source of law. The last part of the contribution deals with the role and significance of Roman law as ius commune in the former Lands of the Crown of Saint Stephen in the last hundred years, emphasizing that a possible wider scope of the application of the ius commune rules in the national judicial practice, especially in the form of regulae iuris, would not just represent a nostalgic quest for the hidden treasure of the European legal tradition but rather a part of a long-term creative effort toward the non-legislative Europeanization of the contemporary legal orders on the firm foundations of the common legal culture.

Abstract Over 350,000 percutaneous translumenal coronary angioplasty (balloon angioplasty) procedures are performed each year. This procedure offers a less invasive alternative to coronary by-pass surgery for patients whose coronary vessels have become occluded due to the process of atherosclerosis. Its potential has not been fully realized due to the high rate of restenosis — the rapid reocclusion of the vessel due to the pathological growth of the vascular smooth muscle (VSM) in response to the trauma of the balloon inflation. Despite the recognition of smooth muscle injury as an initiating event in the process of restenosis, there has been no systematic study to determine the mechanical loading conditions required to produce VSM injury and elicit the restenosis response. In this study, a cell culture model was developed to define the loading conditions required to produce VSM injury. The model system allows precise control of the applied strain and strain rate and quantification of the injury severity in terms of membrane damage. The determination of the threshold criteria for cell injury will allow the angioplasty procedure to be modified, and possibly automated, to minimize VSM injury and avoid the restenosis response.

The potential for weld hydrogen cracking, that can also manifest itself as delayed cracking due to formation well after weld deposition, is controlled by three factors: the presence of hydrogen, the susceptibility of the weldment microstructure and tensile stresses. The tensile stresses promoting hydrogen cracking may result from either welding residual stresses or construction or operations based stresses, while the susceptibility of a microstructure is a function of its carbon equivalent and cooling rate. Since all arc welding processes introduce hydrogen into welds to some extent, and in general, base material selection and weld stress levels are not controllable in welding procedure development, the prevention of hydrogen cracking must be accomplished through hydrogen management. This paper describes a means of considering the roles of welding procedure parameters (heat input, preheat, post-heat, inter-pass temperature and time, etc.) in the management of hydrogen in multi-pass welds to preclude delayed cracking. Some results obtained using a multi-pass weld hydrogen and thermal diffusion model are presented to demonstrate the models utility in understanding the effects of welding procedure parameter effects on the risk of delayed cracking.

One of the major concerns in terms of Structural Integrity for Steel Catenary Risers (SCR) or Fatigue Sensitive Flowlines (FSFL) consists on their strength to withstand dynamic loading along their service life. SCR and FSFL systems always experience more considerable fatigue loading due to floating structures adopted for deep and ultra-deep water oil and gas recovery, as well as for free spanning due to seabed geography and marine currents. In this context, a Double Joint (DJ) Welding Procedure Specification (WPS) has been developed to comply with stringent fatigue requirements, as well as to assess their actual fatigue behavior. The benefits of having DJ are: improving the installation time (S-lay, J-lay or reel-lay) having half of the welding joint performed out of the firing line and reducing the need of Field Joint Coating by two, which results on decreasing project’s cost. This DJ development is focused on a more productive WPS applicable for sour service environments and fatigue endurance requirements considering a Narrow Groove (NG) with J-Bevel design, STT® root pass, SAW for fill and cap passes in 1G welding position. The obtained WPS results are presented on an X65 Steel pipe grade, 273.1 mm OD and 25.4 mm WT.

Abstract Wire Electro-Discharge Machining (WEDM) is a material removal process that incorporates heat generation through electrical energy and mechanical movement to produce finished designs in electrically conductive workpieces. The WEDM workcenter used in this research is a JAPAX 4G. The unit uses nozzles to guide the wire and apply the dielectric fluid to the workpiece. The wire is the conductive electrode and the dielectric fluid produces a localized ionized field between the workpiece and the wire. The dielectric fluid also, clears the debris and cools the wire. The most common problem in WEDM is the optimization of cycle times. The amount of time needed to produce a finished part determines the profitability of the workcenter. Industry is continually searching for procedures to decrease cycle times and increase profit. This research uses a design of experiments (DOE) method to optimize the WEDM process. The key parameters of the WEDM process are modified during the DOE trials. The results are applied to produce an optimal machining condition. The three conditions: Manufacturers Settings, E-Chip Settings, and Optimized Settings are used to produce the rough-cut of three identical designs. The rough-cut is the first pass to produce a finished workpiece. It is during this pass that material removal rate and the risk of wire rupture are the greatest. The overall cycle time is mainly impacted by the time required to perform the rough-cut. The three designs produced are examined for surface finish. The key parameter settings that are used during the rough-cut must not produce a surface finish that requires the addition of cutting passes to produce the final design. Also, the risk of wire rupture during the rough-cut must be acceptable. If wire rupture were to occur during the rough-cut then all benefits of increased speed are lost.

Abstract Precise isolation and resolution of scan chain defects are more critical than ever due to increased reliance on scan-based design to achieve desired test content. At the same time, its diagnosis is becoming more difficult as product design increases in complexity alongside shrinking fabrication processes. In this paper, we present a new scan chain diagnosis procedure that is centered on Load Pass Unload Fail/Load Fail Unload Pass (LPUF/LFUP) and Scan Shift Logic State Mapping (SSLSM) techniques to isolate both stuck-at and timing scan chain faults without the design overhead and defect assumptions of previously proposed methods. More importantly, this procedure is extended to analyze scan chain with multiple defects, which is becoming a more frequent occurrence as process scales down in size.

Economic and environmental incentives encourage operators to maintain pipeline operation during repair and maintenance procedures including hot-tap branch fitting installation onto pipelines. Welding onto a liquid-filled pipeline induces accelerated cooling of the weld and heat affected zone (HAZ), increasing the propensity for cracking. In-service welding codes and due diligence requires that several key factors be considered during the design of an in-service welding procedure specification for its intended purpose. The level of restraint (LoR) imposed by the geometry, material, or dimensional differences of the branch compared to the run pipe has also been shown to be a significant contributor to cracking. Finite element analysis (FEA) was utilized to investigate the geometric effects of an in-service weld procedure to approximate the LoR of hot-tap branch installation. The LoR was quantified and compared by simulating multi-pass weld sequences on two configurations: a branch-on-pipe (BoP) configuration of various dimensions and a configuration using perpendicular plates (PP) that has been used as an alternative to the branch-on-pipe configuration. The highest LoR, as measured by transverse tensile stress at the fillet weld toe, was the branch-on-pipe configuration with the largest branch wall thickness, the smallest branch diameter, the largest run pipe diameter, and the largest run pipe wall thickness. FEA modeling revealed that the PP configuration has lower LoR, thus it is not recommended to use for simulating in-service branch weld procedures.

Traffic noise is a major noise source in the study of environmental noise. Various noise generation mechanisms depict different spectral features. Some are wide-band noise, such as engine knocks; some have signature frequencies, such as gear transmissions; and some are in a certain frequency region, such as tire/road noise. These spectral features affect the façade design of a building in order to achieve sufficient exterior noise insulation and satisfactory interior noise due to the traffic noise. ISO standard 11819-1 specifies the measurement procedure of statistical pass-by tests. There are three ranges of vehicle speed: slow, medium, and fast. However, it requires that the vehicle must maintain constant speed when passing by the test point. Unfortunately, a vehicle tends to generate higher noise when accelerating, especially at low frequencies. Therefore, it is necessary to distinguish the noise levels at an intersection versus middle-points of the road between two intersections. Presumably, the traffic noise levels at an intersection would be higher. This research measured the traffic noise at various locations of different speed limits. Statistical analyses were conducted to compare the spectra at these locations. This is also an effort to refine the noise map.

Gas turbine packages provide a major portion of mechanical drive and power supply for the offshore operating oil and gas platforms. These packages are typically installed in acoustic enclosures, which need to be ventilated for both removing the heat rejected from the engine and package components, and properly diluting explosive gasses in case of a leak. Considering importance of safety and reliability of gas turbine equipment operating in the offshore environment, and also near industrial and populated areas, authors of the paper emphasize the need for experimental validation of a CFD prediction practice for effective ventilation of the turbine package enclosures. In a properly designed acoustic enclosure, ventilation system has to prevent overheating of the electrical and engine control components, as well as, dilution of potential fuel leakages to eliminate stagnant zones that could cause an ignition within enclosure. Conversely, an excessive flow of the vent air may result in masking local fuel leakages, which might pass undetected through explosion protection devices. Therefore, the optimum enclosure ventilation design has to be based on proper vent flow distribution to ensure acceptable temperature distribution within the enclosure, proper flow distribution to ensure no stagnation area, combined with appropriate gas detection setting. In order to achieve an optimum enclosure design, rather complex flow and heat transfer phenomena have to be studied to select the optimal configuration of the vent system. Numerical analysis of the enclosures with commercially available CFD codes is usually based on a number of simplifying assumptions and approximations. Therefore, to satisfy critical safety requirements in the offshore environment, authors of the paper emphasize role of experimental validation of the CFD predictions. The presented paper provides details of the enclosure design validation using a CFD study based on an earlier experimental validation of the numerical predictions. A midsize gas turbine package model was selected to demonstrate this procedure. The main features from the actual engine package were included in the CFD model. Effectiveness of ventilation was studied for both cold and heated engine surfaces. CFD analyses were also carried out for local CO2 injection emulating natural gas leakage. Both scenarios with CO2 and natural gas (methane) leakages were considered reducing uncertainty of predictions due to the differences in the density and buoyancy between these gasses. Based on presented study certain improvements in design of the enclosure were recommended and described in the paper.

Recent developments of the advanced welding processes such as laser beam welding (LBW), solid state friction stir welding (FSW) and hybrid welding, numbers of advanced structures are being designed and constructed in industries such as aerospace, power generation, oil and gas transmission and transportation. Development of new structural aluminum and magnesium alloys as well as high strength steels provide further possibilities for the welded structures in similar and dissimilar (material-mix) configurations. Consequently, there is an increasing demand for “Fitness-for-Service” (FFS) assessment of those advanced welded structures by considering the specific features of these weld joints (such as narrow weld width, high strength mis-match, etc.). In year 1999, Structural Integrity Assessment Procedure SINTAP has been developed for analysis of flaws to avoid fracture within the European Commission funded project SINTAP. Recently, the European Community funded project FITNET in the form of a Thematic Network (TN) organisation has started to review the existing FFS procedures and develop an updated, unified and verified European FITNET FFS Procedure to cover structural integrity analysis to avoid failures due to fracture, fatigue, creep and corrosion. This new FFS Procedure has adopted the SINTAP approach for assessing of the welded structures. This paper describes the FITNET FFS weld assessment route and also aims to demonstrate suitability of weld joint assessment route of the FITNET FFS Procedure in prediction of the critical conditions of various advanced welded joints containing flaw. The welded specimens used in this work cover conventional multi-pass welded Inconel-718 turbine blade (T-joint), center cracked wide plates of electron beam welded 13% Cr supermartensitic stainless steel, laser beam welded shipbuilding C-Mn steel and aluminum alloy. The results are showing that the weld strength mismatch analysis option of the FITNET FFS is conservative and degree of conservatism is similar to the analysis options for the homogeneous materials. This provides confidence in the use of the FITNET FFS procedure for assessing of the structural significance of flaws in welded structures.

Abstract This paper investigates a new strategy for early detection of defects in a power transmission pair of spur gears. Sensitivity to local defects is enhanced by processing the signal as follows. The orthogonal discrete wavelet transform (ODWT) of the band-pass filtered averaged signal is first obtained. This is followed by thresholding in the wavelet domain, thereby removing the low amplitude noise contribution. The inverse wavelet transform then essentially reconstructs the component of the signal that is due to the defect. Experimental results demonstrate the efficiency of this procedure.
