Shearer, Andrew Jackson. "SELF-REPORTED ADHERENCE TO PHYSICAL ACTIVITY FOR CANCER SURVIVORS: AN UPDATE FROM THE 2015 NHIS DATABASE." UKnowledge, 2017. http://uknowledge.uky.edu/khp_etds/44.

Full text
Abstract:
Cancer is the second leading cause of death in America. It’s been suggested that regular physical activity (PA) can improve health outcomes in cancer survivors. An estimate from BRFSS data (2009) suggested that 47% of all cancer survivors met recommended guidelines and that this estimate was not different from the population at large (48%). Several factors were examined from these BRFSS data to determine whether subgroups of survivors existed who might benefit from interventions aimed at improving their PA status. The purpose of this investigation was to obtain more recent estimates of adherence to established PA guidelines for cancer survivors. Data from 2015 NHIS were obtained from the CDC website. Of the survivors, 40% met PA guidelines. Additionally, 79% were 54 years or older, more likely to be female (60%), predominantly white (80%), with more than 2 comorbidities (41%), and with some form of functional limitation (66%). Compared to a study based on 2009 BRFSS data, an even smaller proportion of survivors met PA guidelines in this study. This might be due to differences in age distributions and no limitation of the analysis according to time since diagnosis. Targeted interventions to increase activity in cancer survivors continue to be warranted.

Abstract:
The aim of this study was to examine the experience, of four different actors’ with diverse occupational roles, of the existing rehabilitation process in the organization, with its limitations and possibilities. A further aim was to examine the preventive work and hopefully contribute with some practical solutions to counteract sick leave within the organization. Four mini focus group interviews with two to four people were carried out with two fictitious cases as a base for discussion. Data was analyzed by an inductive thematic analysis and resulted in four thematic areas; Tools to improve the health work, Efforts to avoid sick leave, Health work limitations and The manager’s role in health work. The study showed that the participants considered that the rehabilitation process was a good tool that simplifies the rehabilitation work, but it needs further development. The results also showed that the participants had many suggestions of how to improve the physical and psychosocial work environment.

Hall, Courtney D., Samuel C. Karpen, Brian Odle, Peter C. Panus, and Zachary F. Walls. "Development and Evaluation of the Medication-Based Index of Physical Function (MedIP)." Digital Commons @ East Tennessee State University, 2017. https://dc.etsu.edu/etsu-works/2706.

Full text
Abstract:
Background: The development of an objective and comprehensive drug-based index of physical function for older adults has the potential to more accurately predict fall risk. Design: the index was developed using 862 adults (ages 57–85) from the National Social Life, Health, and Aging Project (NSHAP) Wave 1 study. The index was evaluated in 70 adults (ages 51–88) from a rehabilitation study of dizziness and balance. Methods: The prevalence among 601 drugs for 1,694 side effects was used with fall history to determine the magnitude of each side effect's contribution towards physical function. This information was used to calculate a Medication-based Index of Physical function (MedIP) score for each individual based on his or her medication profile. The MedIP was compared to the timed up and go (TUG) test as well as drug counts using receiver operating characteristic (ROC) analysis. The associations between various indices of physical function and MedIP were calculated. Results: Within the NSHAP data set, the MedIP was better than drug counts or TUG at predicting falls based on ROC analysis. Using scores above and below the cutpoint, the MedIP was a significant predictor of falls (OR = 2.61 [95% CI 1.83, 3.64]; P < 0.001). Using an external data set, it was shown that the MedIP was significantly correlated with fall number (P = 0.044), composite physical function (P = 0.026) and preferred gait speed (P = 0.043).

Abstract:

Abstract:
TeamGym, coach, children, injury prevention, rehabilitation, psychosocial,  physical therapy
Purpose: To investigate the experiences of a group of TeamGym coaches regarding physical injuries, injury prevention and how to enhance a stimulating and motivating environment for  training and competition for children in the ages of 8-12, in the best way possible. Design and method: Descriptive and explorative qualitative design. Semi-structured interviews with five coaches in TeamGym. Data analysis was conducted by using a qualitative content analysis. Results: The coaches have experienced physical injuries and work with injury prevention such as using preventive exercises and maintaining safety at all times. They found it challenging to assess the severity of an injury, how to prevent it and how to pursue rehabilitation. They need increased knowledge through education, commitment from their clubs and cooperation with physical therapists. A positive environment for training was considered vital concerning well-being in sports. Coaches should be role models and provide positive feedback and maintain a good team spirit. The most important aspect is to have fun rather than the gymnastic performance. Conclusion: Coaches in Teamgym have perceived lacks of knowledge regarding injuries and a limited experience from injury prevention and rehabilitation. Possible solutions are increased education and consultation from physical therapists. The coaches are encouraging a rewarding psychosocial training environment in a way which is coherent with guidelines from Swedish sports federations and is supported by existent scientific evidence. Hence they ought to maintain it.

Abstract:
INTRODUCTION: The purpose of this study was to investigate the effect of a 12-month Balance Training Program on balance, mobility and falling frequency in women with osteoporosis. METHODS: Sixty-six consecutive elderly women were selected from the Osteometabolic Disease Outpatient Clinic and randomized into 2 groups: the Intervention Group, submitted for balance training; and the Control, without intervention. Balance, mobility and falling frequency were evaluated before and at the end of the trial using: the Berg Balance Scale (BBS), the Clinical Test Sensory Interaction Balance (CTSIB) and the Timed \"Up & Go\" Test (TUGT). Intervention used techniques to improve balance consisting of one hour sessions/week and a home-based exercise program. RESULTS: Sixty women completed the study and were analyzed. The BBS difference was significant higher in the intervention compared to control (5.5 ± 5.67 vs. -0.5 ± 4.88 score, P<0.001). Similarly, the number of patients in intervention group presented improvement in 2 conditions of CTSIB compared to controls (eyes closed and compliance surface condition: 13 vs. 1 patient, P<0.001; eyes open, visual conflict and compliance condition: 12 vs. 1 patient, P=0.001). Additionally, the difference between the TUGT were reduced in the intervention group compared to control (-3.65 ± 3.61 vs. 2.27 ± 7.18 seconds, P<0.001). Notably, this improvement was paralleled by a reduction in the number of falls in the intervention group compared to control (-0.77 ± 1.76 vs. 0.33±0.96, P<0.018). CONCLUSION: This longitudinal prospective study demonstrated that a intervention using balance training is effective in improving functional and static balance, mobility and falling frequency in elderly women with osteoporosis.

Background and objectives Strokes are the third leading cause of death and the leading cause of disabilities among adults. While many advances have been made in stroke prevention and rehabilitation, some issues remain to be resolved. The aims of the thesis were to investigate the associations of occupational physical activity (OPA) and leisure-time physical activity (LTPA) with the risk of stroke and mortality; to investigate the association between acute glucose level and functional outcome after ischemic stroke 3 months later; to evaluate the relationship between acute glucose levels and mortality among patients with acute ischemic stroke; to investigate the relationship between the intensity of inpatient rehabilitation therapy and mortality among patients with acute ischemic stroke. Materials and Methods Studies I, were based on data from the Chin-Shan Community Cardiovascular Cohort study with 1,706 participants during a median follow-up period of 16.8 years. Studies II, and III were based on data from a hospital-based database with 1277 consecutive ischemic stroke patients admitted to Mackay Memorial Hospital in Taiwan between January 1, 2008 and June 30, 2009. Results: In the first study: During a median follow-up period of 16.8 years, 118 subjects had incident stroke events and 438 deaths were identified. With the lowest tertile as the reference, the relative risks (RRs) for stroke and mortality in the highest tertiles of LTPA were 0.71 (95% confidence interval [CI], 0.38-0.97) and 0.73 (95%CI, 0.58-0.92). In contrast, the RRs for stroke and mortality in the highest tertiles of OPA were 1.74 (95%CI, 0.90-2.78) and 1.53 (95%CI, 1.06-2.22). We found that LTPA decreased the risk of stroke and mortality, while OPA increased the risk of stroke and mortality. In study II: We collected data from 1277 patients with acute ischemic stroke. Patients within the highest quartile of admission glucose level had a 2.68-fold relative risks (RR) (95% confidence interval [CI], 1.87-3.85) for disability and a 2.18-fold RR (95% CI, 1.36-3.48) for mortality compared with those within the lowest quartile level. Higher acute glucose level was associated with disability and mortality. In addition, the association was relatively modest among non-diabetic patients. In study III, the intensity of inpatient rehabilitation therapy and disability and mortality were significantly inversely related in the ischemic stroke patients. Compared with the lowest tertile, the highest tertile of rehabilitation intensity was associated a 43% lower risk of disability (95% CI, 0.47–0.70), with a 55% lower risk of all-cause mortality (95% CI, 0.30–0.65). Conclusion: In the thesis, we found higher levels of OPA are associated with increased risk of stroke and mortality, while LTPA decreased the risk of stroke and mortality. Future studies need to confirm these associations. A significant association between acute glucose level and disability and mortality in non-diabetic ischemic stroke patients were established. Further study regarding the relationship of acute glucose levels and outcomes among ischemic stroke patients with diabetes is warranted. Besides, we found intensive rehabilitation therapy may benefit ischemic stroke patients.

Abstract:
Basketball is one of the most injury-risk team sports. Taking into consideration high frequency of knee joint injuries in both young and professional basketball-players and especially issues of overloads of locomotor apparatus in adult basketball, it is crucially important to develop and propose an effective prevention programs to avoid acute injuries and traumas of knee joints. The proposed study was focused on the development of special training prescriptions and recommendations for coaches and physical trainers aimed to implement prevention methods in the training process. The study was conducted in the Ural Federal University Sports Technologies Research Lab. Twenty young athletes born in 2002 from basketball youth sport schools (Yekaterinburg, Russia) were recruited for the study. The significant interrelations between functional state, oxygen transport and utilization systems and power of working muscles were established for the first time. A novel prevention program of knee joint injuries was introduced, effectiveness of which was proved during the study.
