Objective: Preventative approaches to mental illness are becoming a focus of clinical and research settings. To date, however, few clinical programs have been established with this primary aim. This descriptive paper summarises patterns of referral to one service providing clinical care for young people known to be at high risk of developing a psychotic illness. Methods and results: A 20-month survey of referrals to the service revealed that most patients had a prolonged and circuitous route to assessment. Additionally, a lengthy time period elapsed between the onset of symptoms and initiation of help seeking. Conclusions: Information arising from this survey may influence the development of strategies to improve access to this service and others aimed at the prevention of psychosis. Further, this information may affect the development of generalised pre-ventative mental health services for young people.

Ian Kennedy (1981) pointed out that for us to have a rational health care policy, we must concentrate much more on primary preventative medicine. He went on to suggest that if this meant that some aspects of modern curative health care must be neglected or abandoned, then that would be a necessary sacrifice. Sue Dowling (1983) believed that the Primary Health Care Team, and GPs in particular, hold the crucial key to future developments in preventative health care. Yet research has shown that not all GPs feel that prevention is necessarily better than cure. Preventative health care can take many forms; ranging from overt policies of screening for cervical cancer or hypertension, to more covert policies of hygiene education. But physical health is not the only criteria of good preventative medicine. Mental and emotional health and well-being are of equal importance. The use of counselling, in particular, as a preventative measure against potential neurotic or psychotic illness has been shown to be very effective, despite being infrequently used at the primary health care level. In 1985, I undertook a research project looking at the provision of preventative health care facilities offered in general practice in Bristol. The project looked at all preventative health care resources offered by each general practice within the sample. However, this paper will only concern itself with the findings relating to the mental well-being of the patients within the sample.

Elderly persons with developmental disabilities are living longer, and a lifelong preventative aging program is essential to ensure a high quality of life during later years. Normal aging changes coupled with age-prevalent illness may lead to decreased physical functioning and increased dependency. Although normal aging is not preventable, these changes must not be accelerated; diseases must be noted early and treated promptly. This article proposes a lifelong preventative aging program for persons with developmental disabilities.

Background: Health literacy and use of preventative care are important aspects of health care. Health literacy, or the ability to understand basic medical knowledge, affects a person’s understanding and use of preventative health care. Aim: This study was designed to investigate the perception of health literacy and use of preventative care resources by female undergraduate college students. Method: This is a quantitative, non-experimental descriptive research study with a cross-sectional design. A 30-item demographic yes or no and a Likert-type scale was used to survey 62 female college students living in a dormitory. The goal was to discover if additional education is needed to obtain optimal utilization of health care resources for this population. Result: Surveying the undergraduate nursing and non-nursing students helped raise awareness of health literacy of the female students. Conclusion: Understanding the use of preventative care resources by this population may influence the way nursing interventions are formulated.

Abstract That many caregivers experience some degree of burden is well-established, but little research has investigated the association of burden on caregiver preventative health behaviors. A lack of adherence to these behaviors, including to recommended colonoscopy and mammography screenings, routine vaccinations, annual clinic visits, and HIV testing, may portend worsening health outcomes for family caregivers if diseases are left undetected or untreated; such outcomes could also be caused by and contribute to the crisis of lack of care to meet demand. Survey questionnaires (N=326) were completed by 214 current caregivers and 112 former caregivers from the national MIT AgeLab Caregiver Panel. Respondents predominately identified as white (88%) and female (83%), with a median age of 40-49 years old. Questions assessed caregiving intensity, health status, reported caregiving burden, and adherence to preventative health screenings and vaccinations (per USPTF age- and gender-based recommendations). Among all caregivers, just 17% reported that they were up-to-date with appropriate preventative health screenings. Current caregivers were less likely than previous caregivers to be up-to-date. Controlling for age, income level, and health status, caregiving intensity (as measured by hours per week) was an independent predictor of adherence to preventative health screenings. These findings suggest that current caregivers who are providing high-intensity care are less likely to be engaging in preventative health behaviors for themselves. This finding is concerning for the future health of these caregivers as well as their care recipients and highlights the need for primary care preventative health initiatives specifically targeting family caregivers.

Abstract:
Preventative care services allow patients to be fully equipped with the knowledge, tools, and other resources to help them discover and treat many diseases and illnesses so that the burden of costs will not fall on patients and their families. Since the passage of the Affordable Care of Act (ACA) by President Barack Obama, the requirement for health insurance coverage has not translated to utilization of preventative care services. The purpose of this study is to determine the motivation for African Americans who have insurance coverage and access to care who are not taking advantage of opportunities for screenings and health education. The health belief model was used to determine how belief and modifying factors influence health decisions. The quantitative study required use of a secondary dataset to determine utilization of preventative care services, insurance affordability, future access to care and understanding of the health care law. The study included testing the statistical significance of these factors among African Americans, White Americans, and Hispanic Americans who participated in the Healthy Americas Survey. Using the program SPSS to process data analyzation and organize output, results reveal that African American participants are concerned about the future ability to access and afford care. With a history of distrust amongst African Americans and the health industry, social implications are for administrators and providers to bridge the gap by offering health education to those in their immediate communities and requesting and implementing feedback from those same individuals.

Purpose/hypothesis: The purpose of this study was to use the National Survey of Children's Health 2011/2012 (NSCH 2011/12) data set to determine if children with diagnosed mental health disorders (MHD) (depression and/or anxiety) have delayed or missed preventative healthcare services (medical, dental, vision care), and if their parents reported difficulty getting them the care they need at a different rate than children without MHD. Population of interest: Children (0-18 years of age) and their guardians who completed the survey were included in the analyses. Methods: For both hypotheses, we defined subject’s exposure (having MHD) and outcome (missed or delayed care/problem getting care) based on parental response to interview questions (N = 73,632,881). Covariates adjusted for in our models included child/parent demographics, insurance status, household employment, and DHHS poverty level. Both research questions were evaluated using multi-variable logistic regression (adjusted as necessary using the correct sampling methods as suggested by the NSCH). Results: The odds of having missing or delayed care for a child with MHD were 2.24 times higher (95% CI: 1.82-2.77) than a child without MHD. The odds of the parent reporting problems getting care for a child with MHD were 2.11 times higher (95% CI: 1.74-2.60) than a child without MHD. Clinical Relevance: These findings highlight a need to focus on finding ways to promote and provide all of the healthcare that a child needs. Children with MHD have many healthcare needs that other children may not, but they still need adequate preventative care. This may bring the specific needs of a population needing extra help to the attention of healthcare providers and outreach programs.

The purpose of the research was to describe the relationships between social support and role strain and preventative health behaviors. The sample consisted of 62 critical care nurses employed in three southwest acute care facilities. Subjects completed instruments measuring social support, role strain, and four preventative health care behaviors. Pearson correlations revealed significant positive relationships between social support and personal/household roles women perform and ways women handle stress. Additional significant negative relationships existed between marital/relationship roles women perform and leisure physical activities, a subset of preventative health behaviors. The parental roles, obligations, and responsibilities women perform were also significantly related with leisure physical activities. Conclusions drawn indicate that the critical care nurses did not perceive themselves susceptible to cardiovascular disease and therefore did not participate in preventative health care activities, regardless of perceived helpful social support and an absence of role strain.

Well-Child Examinations are an integral part of monitoring growth and development for children. These visits allow for establishment of a therapeutic relationship between patient and caregiver, and provide opportunities to screen for underlying conditions while simultaneously following growth and development milestones. Well-child examinations provide opportunities for parents to voice concerns and help to identify those children at risk for delays or underlying medical conditions. When these conditions are identified early, they tend to have an improvement of outcomes. Since the core items to be included in wellness examinations vary by age, insurance provider, and risk factors, our aim is to measure and improve the knowledge and comprehension of examination components among a group of Family Medicine resident physicians that provide primary care to a pediatric population. Provider knowledge and understanding was measured by means of a set of multiple-choice questions prior to an educational session. A post-educational examination was then administered to assess recruitment and retention of information. There appeared to be an overall positive trend toward increased knowledge base following the education session, indicating and improvement of understanding and medical knowledge.

Voice problems are common among teachers due to their job characteristics. Thus, the quality of teaching, daily communication, emotion would be adversely affected. However, the past studies mainly focused on primary school and preschool teachers. There is lack of investigation on voice problems of secondary school teachers. Moreover, the need for voice use training for secondary school teachers was not addressed in the past. This study investigated the prevalence of voice problems in secondary school teachers in Hong Kong and the need for preventive education. A questionnaire was decided with domains in demographics, job natures, impact on voice disorders, risk factors, history of seeking help because of voice problems, view on different treatment options and opinion towards the need of voice protection training. Subjects were recruited from four local secondary schools in Hong Kong. 85 questionnaires were collected and analyzed. The results showed a high prevalence of voice disorders (75%) among secondary school teachers. Also it revealed different levels of impact on work performance, emotion and daily communication. Laryngitis was the only risk factor found to be significantly correlated with the voice disorder. On the other hand, the rate of seeking professional help and their knowledge about voice rehabilitation were low. Most of the respondents realized the need for voice protection training. It suggested that the voice problems of secondary school teachers should be highlighted and more voice protection information and voice training should be provided.

AbstractImmediately after the Great Hanshin-Awaji Earthquake in 1995 and the Great East Japan Earthquake in 2011, pneumonia outbreak among the elderly increased the rate of fatalities. What caused this? To use lessons learned during the large-scale disaster, specialists concerned with the field of dentistry began to take action using new perspectives. Consequently, they noticed the importance of giving care to the entire oral cavity as well as the teeth. Based on reports from a dentist and a dental hygienist who tackled the oral care of the Kumamoto Earthquake victims in 2016, their methods of effective health care following a disaster is presented in this paper. We also propose a preventative medical activity that can be conducted sustainably for anyone, not just medical personnel.

AbstractIn healthcare companies, shiftwork organization is fundamental to ensure continuous 24-h patient care. This chapter gives an overview of health-related problems associated with shift work and the preventative actions that can be taken to protect workers’ health and well-being. Shift work, in particular night work, results in a disruption of biological circadian rhythms with serious social and psychophysical ramifications for the worker. The adverse health effects of shift work can be both in the short-term (sleep, digestive, mental, and menstrual disorders) and in the medium- to long-term (increased gastrointestinal, neuropsychic, metabolic, and cardiovascular diseases). In 2007, the IARC classified shift work as “probable carcinogen” for humans due to the destructive effects on the circadian rhythm. The modification of the sleep/wake cycle also negatively influences worker’s vigilance and performance (“jet-lag syndrome”) leading to a consequently greater risk of accidents and errors. Shift work can be harmful to the safety of both the worker and the patient. Appropriate shift scheduling that respects ergonomic criteria is important to protect worker and patient health and well-being. Medical residents should be conscious of the legislation and rights regarding shift work to ensure they provide appropriate assistance to patients and to preserve their own social and psychophysical well-being.

Healthcare costs are escalating due to public demand for, and the increasing availability of, treatment. National frameworks that use economic evaluation as a basis for health technology assessment have been successful at constraining expenditure on low-value treatments primarily by explicitly considering the opportunity cost of new technology adoption or service redesign. At the end of life and in palliative contexts, such methods have not been widely applied and are underdeveloped despite evidence that healthcare costs typically increase with proximity to death. There may be a requirement for the adaptation of standard methods for healthcare resource allocation in this setting, where the goals of care may differ from a curative or preventative context. Health service and financing models may be complex and specific to this setting, with a greater consideration for third-sector provision and informal care. This chapter outlines the core concepts in health economics that are relevant in the planning of palliative services, with specific considerations in the developed and developing world contexts.

This chapter covers the operation of the National Health Service (NHS). While the constitution of the NHS defines the service as belonging to the people, the NHS has fronted continual criticisms with regards to its expertise in the medical profession, negligence, and malpractice. Preventative healthcare had not been one of the NHS's major focuses, and medical care had been heavily individualised. Thus, the concept of public health is not based on arguments by conventional medical care and individualisation. Occasionally, the media coverage of the NHS tends to imply that it had been slow to reform and modernise. However, the criticisms abated amidst the coronavirus crisis.

Abstract:
This chapter links the global and national perspectives discussed in the previous chapters to individual empirical data in three case studies. The first case study is in the sphere of public or preventative health, which is examined through a discussion of ‘active ageing’ policy, which encourage older people to participate in physical activity. This case study exemplifies the ‘active ageing’ paradigm, discussing data collected before the onset of austerity policy in the UK. The second case study is located within the sphere of pre-emergency care and draws on data to elucidate the vulnerability of older people in this care setting. This illustrates both paramedics’ experiences of working as frontline health and social care workers in a challenging environment, and the experiences of older people, who often end up being left waiting on the floor for several hours after a fall. End of life care, is the final case study discussed, examining the problems caused by a lack of trained senior clinicians who can make end of life care decisions. The lack of dedicated time and an appropriate environment in which to have these sensitive discussions is also highlighted.

There is an increase in demand for health promotion and preventative medicine playing a vital role in managing chronic illnesses. Many of these conditions stem from a poor diet, sedentary lifestyle and smoking, all of which are risk factors for age-related macular degeneration (AMD). To combat chronic diseases, the root of the conditions may be addressed through the concept of health promotion. Health promotion thoroughly assesses how a population’s environmental, political, socioeconomic, behavioral, and cultural practices influence its health. This concept can be applied in a primary care setting which takes on a broader approach in treating and managing patients. Primary care providers need to be aware of the connections between common chronic illnesses and AMD. All primary care providers and eyecare specialists must be patients’ advocate and help improve their systemic and ocular prognosis.

Cold has impact to every person as on allied forces as well on enemy forces. The topicality of the study is based on the analysis of the experience of military campaigns in cold environments in the history of war and the emphasis on the importance of preventive measures that reduce the influence of low temperature as a natural environmental factor on the tactical readiness of troops. The aim of the study was analyses of cold environment impact on outcome (personnel losses) of military campaign in historical aspect, characterized the main health problems of personnel. We provided theoretical analysis of military campaigns` from ancient time till nowadays. Results of our study showed that realisation of preventative measures as well as planning and providing troops` activities in low temperature environment is one of the greatest challenges for leaders and commanders of military operations; in case of failure environmental factors played crucial role to wars` outcomes, battles` results and could lead to great disaster for military units and armies. The most important conclusion is the temperature of surrounding condition is one of essential factor for combatant’s survival, for activity of the personnel in low temperature surrounding, for combat capacity of military personnel and military unit. Timely identification of low-air temperature environment hazard slop down personnel losses in warfare. The most important conclusion stressed that assessment of cold weather, prevent and manage cold injuries allow keep combat capacity and fulfil the mission. Results of our study shown significance the effective program of military force health protection that included adequate logistic as well with nutritional supply. Commander authority in management personnel readiness is essential, physical preparedness, practical skills, and training of personnel in cold environment were important in pre-mission phase of campaign. Research results shown that cold (low temperature) impact as environmental risk factor must be identified and carefully assessed before military action.

Abstract:
Abstract During drilling of permeable reservoirs, drilling fluid may penetrate the formation and induce damage to the reservoir rock. Specifically, solids present in the drilling fluid may enter the formation and cause subsequent reduction in reservoir permeability in the area near the wellbore. When drilling with a water-based drilling fluid in a reservoir, various polymer-based additives are normally applied to reduce the filtration loss. These additives, such as Xanthan Gum, Poly Anionic Cellulose (PAC) and Starch may help in reducing losses to the formation in presence of small pore-throats and low differential pressures. If the pore throats exceed e.g. 20μm and differential pressures reach 500psi, these additives have little effect on reducing loss of drilling fluid to the formation and thereby little effect in preventing solids from entering the formation. Lost circulation is particularly challenging when losses occur in the reservoir section. This is because LCM treatment may create formation damages. Green et al. (SPE-185889) showed the nature of drilling fluid invasion, clean-up, and retention during reservoir formation drilling. They also showed the lack of direct relation between fluid loss and formation damage. In light of such ideas, a development of new Non-Invasive Fluid (NIF) additives was conducted. These additives were able to handle downhole pressure differences and create a preventative sealing of a permeable formation when applied into a solids-free drilling fluid. Ceramic discs of various permeability and mean pore-throat size were installed into a HTHP pressure cell. Drilling fluid was pumped through the cell and a filter cake was formed across the ceramic disc. A pressure of 500psi was applied and filtration loss was measured over a 30-minute period. Examples are herein presented showing how filter cake materials were applied into the drilling fluid and effectively sealing the permeable surface of the ceramic disc. Also, it will be shown how the filter cake was effectively removed from the discs using a breaker solution. Furthermore, a selection of experiments is presented, showing the possibility to heal lost circulation in permeable reservoirs without the presence of weighing materials, clays or drill-solids in the drilling fluid. A test was also conducted in such a way that the disc was fractured inside the test cell to investigate the impact on fluid loss.

Abstract Accurate measurement of microscale mass changes is crucial for various applications including toxic gas detection and medical diagnosis. Recent exploration in microelectromechanical systems (MEMS) has brought significant advancements in microscale sensing. Despite unprecedented sensing resolution, linear resonance-based MEMS sensors often require carefully controlled environment for operation due to unavoidable damping and noise. Nonlinear MEMS sensors exploiting large structural responses have shown enhanced robustness against noise and damping; yet, sophisticated preventative control is often required to extend their structural reliability. Recent investigations on nonlinear circuits and feedback controls have focused on detecting mass adsorption by indirectly introducing the essential nonlinearity. To overcome these challenges, this research presents a novel amplitude-based strategy for microscale mass sensing using an array of adaptive bistable circuits. A hybrid linear and bistable system is devised which utilizes linear cantilever-based resonators integrated with an array of adaptive bistable circuits for quantifying micro-mass adsorption at the linear cantilever beam tip by monitoring sudden and drastic response changes (bifurcation) in the bistable circuit array. The structural response is reflected into the piezoelectric voltage which is provided as an input to the bistable circuit array through the electromechanical coupling of piezoelectric transducer. The input amplitude is adjusted to be near the bifurcation thresholds of the bistable circuits, which are adaptively tuned by the gain of each circuits. The thresholds are distributed by four standard deviations of the bifurcation distribution, which determines the theoretical minimum sensing resolution with 95% probability given ambient noise level. The output levels of each circuit are evaluated whether they exhibit unmistakably different intra- or interwell responses that depend on the minute input voltage variations induced by mass adsorption. By comparing the array of results between the initial and mass-adsorbed states, we could accurately quantify the input amplitude change that linearly corresponds to the adsorbed mass. The proposed sensing approach utilizing bifurcation phenomena was verified by numerically simulating the integrated electromechanical system. A numerical case study under the presence of Gaussian white noise with 28 dB SNR is performed demonstrating that the proposed bifurcation-based approach shows 300% enhanced resolution compared to the conventional peak detection method. Experimental investigations conducted on a mesoscale proof-of-concept platform also verify the enhanced performance of the proposed approach. The proposed approach yields an added mass ratio of 9.8e−4, which shows more than 700% enhancement compared to that of the prior bifurcation-based detection results. Overall, numerical and experimental results verify that the proposed approach provides remarkably enhanced resolution compared to prior advancements, and suggest that the proposed strategy holds promise for various applications such as low-cost diagnostics and structural health monitoring, which require precise and accurate measurement. Future work will focus on applying the enhanced sensing capability of proposed strategy to linear MEMS resonators which are expected to yield higher resonance frequency and baseline mass sensitivity, for enhancing various applications in medical diagnosis, public health, and safety.
