Abstract:
An understanding of the normal anatomy of the brain is essential to the diagnosis of a number of conditions that may be encountered in patients in the intensive care unit (ICU). Common structural cerebral conditions causing patients to be admitted to the ICU include cerebral trauma (traumatic brain injury), cerebrovascular accidents (both ischaemic and haemorrhagic), and infections. Cerebral conditions with a structural basis occurring after admission to the ICU are not as common as functional abnormalities, such as delirium, and peripheral complications, such as critical illness neuropathy and myopathy. An understanding of brain physiology, in particular factors that control or influence intracranial pressure (ICP) and cerebral blood flow (CBF) underpin much of the theory behind the management of acute brain injuries and syndromes.

Abstract:
The wide use of benzodiazepines is associated with some inconveniences and are most frequently implicated in acute self-poisoning and accidental poisoning in children. Some of them are recognized as submission drugs, used to commit date rape or robbery. Prolonged use of a benzodiazepine leads to dependence, with a risk of developing a life-threatening withdrawal syndrome. Overdose has usually a good prognosis—most patients recover well with careful observation and prevention of complications, although care should be taken with elderly people, and patients with chronic obstructive pulmonary disease or liver dysfunction. Fast-acting agents and co-ingestion of other central nervous system depressants may be present greater risk. Early administration of activated charcoal in patients able to protect their airway is only needed if there are co-ingestants. Flumazenil may help confirm the diagnosis, improve alertness, and prevent the need for respiratory support in some patients, especially after accidental poisoning in children. Contraindications include patients on long-term treatment and/or dependent on benzodiazepines, or those who have simultaneously ingested proconvulsant or prodysrhythmic substances or at risk of increased intracranial pressure.

Abstract:
Postdural puncture headache (PDPH) may follow either deliberate or unintentional (accidental) penetration of the interdigitating meninges, the dura and arachnoid mater. It is one of the most common and clinically important complications of regional anaesthesia and analgesia in the obstetric population. The headache develops as a consequence of cerebrospinal fluid loss, low intracranial pressure and cerebrovascular changes in the upright position and can prove debilitating. The diagnosis is clinical, making thorough assessment and regular review all the more important, to revise treatment plans, exclude rare serious pathology such as subdural haematoma, and avoid misdiagnosis. This chapter reviews the pathophysiology, incidence, risk factors (needle, technical and patient related), features, natural history, diagnosis, and management of PDPH. High level evidence supports prevention by using small gauge, non-cutting spinal needles, but other preventative strategies against either unintentional dural puncture or PDPH are poorly supported. The absent or poor efficacy of measures such as bed rest, hydration, cerebral vasoconstrictor therapy, epidural or intrathecal saline injection, intrathecal catheter placement or prophylactic epidural blood patch, is noted. Validation of better evidence supporting epidural morphine or intravenous cosyntropin is required. Symptomatic treatment of PDPH is also unreliable. Very limited evidence that requires substantiation supports a modest benefit from caffeine, gabapentinoids or intravenous hydrocortisone. The intervention of epidural blood patch is highly likely to relieve post-spinal PDPH, but only completely resolves epidural needle-induced PDPH in 30–50% of cases. Much detail about EBP remains undetermined, but delayed intervention and injection of approximately 20 mL of autologous blood appear appropriate.

La hipertension es el principal factor de riesgo modificable de algunas enfermedades graves como las enfermedades cardiovasculares (accidentes cerebrovasculares y cardiopatías isquémicas), la preeclampsia y la eclampsia (una causa muy importante de muerte en las embarazadas, así como de retraso del crecimiento fetal y mortinatos) y la enfermedad renal crónica. A nivel mundial, más de mil millones de personas tienen hipertensión, y la prevalencia es mayor en los países de ingresos bajos y medianos. La medición exacta de la presión arterial es esencial para detectar y tratar adecuadamente a las personas con hipertensión, un trastorno que constituye un asesino silencioso que causa pocos síntomas. La falta de acceso a dispositivos de determinación de la presión arterial exactos y asequibles constituye un obstáculo importante para una atención médica adecuada, en particular en los entornos de recursos escasos. La medición manual está siendo reemplazada gradualmente por la medición automatizada debido a los problemas ambientales derivados del mercurio, la falta de calibración y las mediciones incorrectas de los dispositivos aneroides en la práctica clínica, así como por la exactitud uniforme superior que ofrecen los dispositivos automáticos validados. Sin embargo, con frecuencia existe cierta preocupación respecto a la exactitud de los dispositivos automatizados que no se han validado. Este documento actualiza la orientación de la OMS sobre dispositivos de medición de la presión arterial del 2005.
