Abstract:
Purpose Chemotherapy preparation units face peaks in activity leading to high workloads and increased stress. The present study evaluated the impact of work overloads on the safety and accuracy of manual preparations. Method Simulating overwork, operators were asked to produce increasing numbers of syringes (8, 16, and 24), with markers (phenylephrine or lidocaine), within 1 h, in an isolator, under aseptic conditions. Results were analyzed using qualitative and quantitative criteria. Concentration deviations of < 5%, 5%–10%, 10%–30%, and >30% from the expected concentration were considered as accurate, weakly accurate, inaccurate, and wrong concentrations, respectively. Results Twenty-one pharmacy technicians and pharmacists carried out 63 preparation sessions (n = 1007 syringes). A statistically significant decrease in the manufacturing time for one syringe was observed when workload increased (p < 0.0001). Thirty-nine preparation errors were recorded: 30 wrong concentrations (deviation > 30%), 6 mislabeling, 2 wrong diluents, and 1 wrong drug. There was no statistically significant difference in the mean concentration accuracy of final preparations across the three workloads. The overall error rate increased with the number of preparations made in 1 h: 1.8% for 8 preparations, 2.7% for 16 preparations, and 5.4% for 24 preparations (p < 0.05). Conclusion Although pharmacy technicians and pharmacists were able to increase production speeds with no effect on mean concentration accuracy under stressful conditions, there were greater probability errors being made. These results should encourage actions to spread workloads out over the day to avoid peaks in activity.

Abstract:
AbstractBackgroundAdequate bowel preparation is essential for a high-quality colonoscopy. Many randomized controlled trials (RCTs) have investigated bowel preparation protocols, including split-dose and low-volume regimens. However, RCTs are conducted in an ideal, controlled setting, and translation of trial results to clinical practice is challenging. In this study, we compared the quality of bowel preparations of real-world patients from clinical practice with those enrolled in several prospective trials.MethodsBowel preparation quality, defined by the Ottawa Bowel Preparation Scale (OBPS), from four RCTs and one prospective trial were compared with two observational diary studies. Bowel preparations were polyethylene glycol preparation (PEG) or sodium picosulfate plus magnesium citrate (P/MC) taken via traditional or split-dose timing regimen. Age, sex, average number of bowel movements per day, comorbidities, colonoscopy indication and colonoscopy completion rates were also collected.ResultsPatients enrolled in prospective trials had a better OBPS by one point when compared with those in observational clinical practice studies (P<0.049), after controlling for age, sex, colonoscopy indication and type of bowel preparation used. We also found that each 10-year increase in age was associated with a 0.2 point increase in OBPS (P=0.008), and men were associated with a 0.5 increase in OBPS when compared with women P=0.014).ConclusionPatients from clinical practice have higher OBPS than prospective trial patients. Increased age and male sex were also associated with increased OBPS. We believe increased patient motivation and education around bowel preparation regimen plays an important role in the success of bowel preparations.

Bell, Katrina. "TEACHER, TUTOR, SCHOLAR, ADMINISTRATOR: PREPARATION FOR AND PERCEPTIONS OF GRADUATE WRITING CENTER WORK." OpenSIUC, 2018. https://opensiuc.lib.siu.edu/dissertations/1620.

Full text
Abstract:
This research uses a mixed methods approach to explore the both the preparation for and perceptions of graduate consultant writing center work. A review of literature shows a gap in both the knowledge surrounding graduate writing consultant education and the long-term outcomes or transfer of writing center training and work to post-graduate careers. The survey instruments in this study draw from two established studies, the Peer Writing Tutor Alumni Research Project and the National Census of Writing, while a request for curricular artifacts draws on case study research conducted by Jackson et al. Findings indicate that graduate consultants are being prepared for their work in writing centers, but that directors are not intentionally including discussions of how that work may transfer into academic careers, particularly those in writing center leadership. Despite this, current and alumni graduate consults report both immediate and long-term transfer of writing center experiences, skills, and knowledge into their occupations. The transfer of learning is perceived as being most profound for those who have remained in the academy as either professors or administrators. This research has implications for graduate students, directors, and institutions, and I conclude with an analysis of how directors can be more intentional in their work with graduate consultants in order to better prepare a new generation of writing center administrators who are aware of the academic, political, and scholarly opportunities that are possible through writing center careers.

Zazzarino, Anthony. "Experiences of Counselors Who Work With Sexual Minorities With a Serious Mental." ScholarWorks, 2018. https://scholarworks.waldenu.edu/dissertations/5596.

Full text
Abstract:
Sexual minorities (SM) are at a greater risk for experiencing a serious mental illness (SMI) compared to their heterosexual counterparts. Furthermore, SM with a SMI continue to experience stigma and discrimination that leads to more negative outcomes and a greater need for counseling services. Current researchers have not adequately addressed the specific needs of SM with a SMI and how to prepare counselors to work with this population. Furthermore, most SM with a SMI find that counseling services are inadequate and do not meet their unique needs. The purpose of this transcendental phenomenological study, grounded in a Husserlian philosophical and Minority Stress Model conceptual framework, was to explore the experiences and perceptions of counselors who provide counseling services to SM with a SMI. Data was collected from six participants using a semistructured interview and followed a thematic data analysis process, ensuring thematic saturation. The results of this study highlighted many themes regarding the unique needs of SM with a SMI, such as their multiple minority stressors, negative counseling experiences, and the impact of family, as well as counselor's perception regarding the lack of preparation in graduate school to work with SM with a SMI. Study findings may improve counselors' understanding of the needs of SM with a SMI so they may provide more effective counseling services. Also, this study highlights the importance of training counselors to work with this population and may bolster the efforts of counselor educators.

Abstract:
Group-work plays a significant part in all undergraduate courses while working effectively as a member of a group is one of the most important generic skills that students need to develop while at university in preparation for their future working lives. Many stakeholders, however, have reservations regarding group-based assignments, in particular about whether or not individual group members are rewarded appropriately for their contribution to the overall group achievement. The success of group-work is dependent on both extrinsic factors, such as assignment design, management, assessment, etc. as well as intrinsic factors, such as the knowledge, skills, etc. of individual group members. In this paper the challenges presented by the extrinsic factors that impact on the success of group-based assignments are considered against the background of the of formal summative assessment of group-based assignments on BSc Computing undergraduate courses in the Department of Computing at Canterbury Christ Church University in the United Kingdom. In particular the effectiveness of group-work in terms of improved learning, improved development of non-cognitive skills and assessment validity/reliability are considered.

Abstract:
The key question of this study is to ask: what does it take to reproduce a household in urban India? Using a series of time-use surveys, we measure the time taken for 33 different tasks within activity clusters such as domestic services (cleaning, food preparation, procurement, upkeep) and caregiving services (child care and elderly care). Within this, we assess both unpaid work done by members of the household and paid work done by an externally engaged domestic worker. We do so across 9,636 households in two large metropolitan Indian cities– Bengaluru and Chennai – with variations across socio-economic status, caste, religion, neighbourhood type and across households with and without women working for wages.

Analysis of modern works on the organization of control of educational achievements of students; the log books of progress and attendance of students in classes, conversations with teachers and our own observations have proved that in educational practice there are different models of the organization of control of educational achievements of students in pedagogical disciplines and professional subject training, validation should be carried out using various schemes and scales of evaluation present different approaches to the calculation of rating of students (in some cases even within the same University) and others. The analysis proved that the existing complex control tasks and tasks for independent work is only seventy percent of jobs differentiated by professional orientation, the rest of the job for the overall development of pedagogical competence of students. In our opinion, well developed task, that is, those that consist mainly of problems of professional and pedagogical orientation that enhance future teachers ‘ motivation to learn pedagogical disciplines. The quality of education becomes the main reference point that determines the credibility and competitiveness of educational institutions on regional, national level and international arena.
