Abstract:
Background and Purpose: Patient-reported experience measures (PREMs) assess patients’ perception of health care. We aimed to identify all reported PREMs for stroke care and critically appraise psychometric properties of PREMs validated for patients with stroke. Methods: Studies on the development, validation, or utilization of PREMs for adult patients with stroke were systematically identified. The Consensus-Based Standards for the Selection of Health Measurement Instruments criteria were used to appraise psychometric performance. Results: We included 18 studies, examining 13 PREMs. Two PREMs had been developed for stroke care: Consumer Quality Index: Cerebrovascular Accident and Riksstroke. Consumer Quality Index: Cerebrovascular Accident was given a positive psychometric assessment, but its length and limited language applicability impede clinical implementation. Riksstroke was appraised as doubtful. Eleven PREMs were generic. The psychometric performance of 5 generic PREMS, validated for patients with stroke, received conflicting assessments. Six generic PREMs had not been validated in patients with stroke and were therefore not assessed for instrument performance. Conclusions: Thirteen PREMs have been published for use in stroke care. The stroke-specific Consumer Quality Index: Cerebrovascular Accident has favorable psychometric performance but lacks practical feasibility. Other PREMs have inadequate or unknown psychometric properties. This indicates the need for developing stroke-specific PREMs to support quality improvement and enhance patient-centered care.

Abstract:
Background Patient centeredness is an integral part of the quality of care. Patient-reported experience measures (PREMs) are assumed to be an appropriate tool to assess patient-centredness. An evaluation of German-speaking PREMs is lacking. Objective To perform a systematic review and qualitative analysis of psychometric measurement qualities of German-language PREMs using for the first time a comprehensive framework of patient centredness. Methods A systematic literature search was performed in Medline, PsycInfo, CINHAL, Embase, Cochrane database (last search 9th November 2021) for studies describing generic, surgery- or cancer care-specific PREMs. All questionnaires that were developed in or translated into German were included. The content of the included PREMs was evaluated using a comprehensive framework of patient centredness covering 16 domains. Baseline data of all PREM studies were extracted by two independent reviewers. Psychometric measurement qualities of the PREMs were assessed using current COSMIN guidelines. Results After removal of duplicates 3,457 abstracts were screened, of which 3,345 were excluded. The remaining 112 articles contained 51 PREMs, of which 12 were either developed in (4 PREMs) or translated into German (8 PREMs). Eight PREMs were generic (NORPEQ, PPE-15, PEACS, HCAHPS, QPPS, DUQUE, PEQ-G, Schoenfelder et al.), 4 cancer care-specific (EORTC IN-PATSAT32, PSCC-G, Danish National Cancer Questionnaire, SCCC) and none was surgery-specific. None of the PREMs covered all domains of patient-centeredness. Overall rating of structural validity was adequate only for PEACS and HCAHPS. High ratings for internal consistency were given for NORPEQ, Schoenfelder et al., PSCC-G and the SCCC. Cross-cultural validity for translated questionnaires was adequate only for the PSCC-G, while reliability was adequately assessed only for the EORTC IN-PATSAT32. Due to a lack of measurement gold standard and minimal important change, criterion validity and measurement invariance could not be assessed for any of the PREMs. Conclusion This is the first systematic review using a comprehensive framework of patient centredness and shows that none of the included PREMs, even those translated from other languages into German, cover all aspects of patient centredness. Furthermore, all included PREMS show deficits in the results or evaluation of psychometric measurement properties. Nonetheless, based on the results, the EORTC IN-PATSAT32 and PSCC-G can be recommended for use in cancer patients in the German-language region, while the German versions of the HCAHPS, NORPEQ, PPE-15 and PEACS can be recommended as generic PREMs. Trial Registration Registration. ObjectiveTo develop and test patient-reported experience measures (PREMs) for children and young people in a specialist paediatric hospital setting.DesignSix PREMs were developed and tested by children and young people for children and young people aged 8–11, 12–13 and 14–16 years in inpatient and outpatient settings. A week-long pilot was implemented across inpatient wards and outpatient clinics to identify facilitators and barriers to the routine use of PREMs in a real-time setting across our organisation.SettingTertiary paediatric hospital.Main outcome measuresFinal PREMs; identified facilitators and barriers to implementation.Participants543 children and young people aged 8–16 years attending outpatient clinics or inpatient wards across a range of specialties.ResultsThree key themes about hospital experience were identified during focus groups: facilities, treatment and tests and people working at the hospital, and these provided the structure for the questionnaires. During cognitive testing the questionnaires were generally understood but some revisions to language and length of the questionnaires were required. Two designs were selected for the final PREMs. During acceptability and feasibility testing it was evident that children and young people liked the PREMs and wanted to give feedback on their hospital experience. Particular challenges for routine use of the PREMs focused on sustainability and resources.ConclusionsThe new PREMs will provide children and young people receiving care in specialist paediatric hospitals with the opportunity to provide feedback on their experience. Sustainability and ensuring that feedback results in improvements need to be addressed in future work.

Abstract:
Patient experience is defined as a major quality indicator that should be routinely measured during and after a colonoscopy, according to current ESGE guidelines. There is no standard approach measuring patient experience after the procedure and the comparative performance of the different colonoscopy-specific patient-reported experience measures (PREMs) is unclear. Therefore, the aim was to develop a conceptual model describing how patients experience a colonoscopy, and to compare the model against colonoscopy-specific PREMs. A systematic search for qualitative research published up to December 2021 in PubMed, Cochrane, CINAHL, and PsycINFO was conducted. After screening and quality assessment, data from 13 studies were synthesised using meta-ethnography. Similarities and differences between the model and colonoscopy-specific PREMs were identified. A model consisting of five concepts describes how patients experience undergoing a colonoscopy: health motivation, discomfort, information, a caring relationship, and understanding. These concepts were compared with existing PREMs and the result shows that there is agreement between the model and existing PREMs for colonoscopy in some parts, while partial agreement or no agreement is present in others. These findings suggest that new PREMs for colonoscopy should be developed, since none of the existing colonoscopy-specific PREMs fully cover patients’ experiences.

Abstract:
Across mammals, prenatal maternal stress (PREMS) affects many aspects of offspring development, including offspring growth. However, how PREMS translates to offspring growth is inconsistent, even within species. To explain the full range of reported effects of prenatal adversity on offspring growth, we propose an integrative hypothesis: developmental constraints and a counteracting adaptive growth plasticity work in opposition to drive PREMS effects on growth. Mothers experiencing adversity reduce maternal investment leading to stunted growth (developmental constraints). Concomitantly, the pace of offspring life history is recalibrated to partly compensate for these developmental constraints (adaptive growth plasticity). Moreover, the relative importance of each process changes across ontogeny with increasing offspring independence. Thus, offspring exposed to PREMS may grow at the same rate as controls during gestation and lactation, but faster after weaning when direct maternal investment has ceased. We tested these predictions with a comparative analysis on the outcomes of 719 studies across 21 mammal species. First, the observed growth changes in response to PREMS varied across offspring developmental periods as predicted. We argue that the observed growth acceleration after weaning is not “catch-up growth,” because offspring that were small for age grew slower. Second, only PREMS exposure early during gestation produced adaptive growth plasticity. Our results suggest that PREMS effects benefit the mother’s future reproduction and at the same time accelerate offspring growth and possibly maturation and reproductive rate. In this sense, PREMS effects on offspring growth allow mother and offspring to make the best of a bad start.

Abstract:
This study assessed the correlation of 9 questions addressing communication effectiveness (the Communication Effectiveness Questionnaire [CEQ]) with other patient-reported experience measures (PREMs; satisfaction, perceived empathy) as well as patient-reported outcome measures (PROMs; pain intensity, activity tolerance) in patients with musculoskeletal illness or injury. In a cross-sectional study, 210 patients visiting an orthopedic surgeon completed the CEQ and measures of satisfaction with the visit, perceived empathy, pain intensity, and activity tolerance. We evaluated correlations between CEQ and other PREMs and CEQ and PROMs. We measured ceiling effects of the PREMs. Communication effectiveness correlated moderately with other PREMs such as satisfaction (ρ = 0.54; P < .001) and perceived empathy (ρ = 0.54; P < .001). Communication effectiveness did not correlate with PROMs: pain intensity (ρ = −0.01; P = .93) and activity tolerance (ρ = −0.05; P = .44). All of the experience measures have high ceiling effects: perceived empathy 37%, satisfaction 80%, and CEQ 46%. The observation of notable correlations of various PREMs, combined with their high ceiling effects, direct us to identify a likely common statistical construct (which we hypothesize as “relationship”) accounting for variation in PREMs, and then develop a PREM which measures that construct in a manner that results in a Gaussian distribution of scores. At least within the limitations of current experience measures, there seems to be no association between illness (PROMs) and experience (PREMs).

Abstract:
Introduction:Co-production relates to patients and health professionals working in equal partnership with shared decision-making. Patient-reported outcome measures (PROMs) and patient-reported experience measures (PREMs) are increasingly being used to involve patients and measure healthcare quality. We set out to develop a set of universal experience questions for use across Wales. These will be used in various settings, including the national electronic PROMs and PREMs platform, which is already collecting outcome data across Wales and has received over 7,000 responses to date.Methods:Patient experience leads and clinical leads were invited to a workshop to discuss standardized PREMs collection in Wales, with all health boards and trusts represented. It was agreed that quantitative patient experience data collection, while limited, would be a pragmatic way to collect responses from a large cohort. It was agreed that a previously developed set of PREMs questions could be adapted for use in all healthcare settings. Patient focus groups reduced the number of questions to a shortlist of those considered most important by patients. Wording was improved and an additional question was added.Results:In partnership with stakeholders we developed and agreed on a set of universal PREMs questions. These have been added to the national electronic platform, with collection commencing imminently. This will allow patients accessing secondary care in Wales to provide PREMs and PROMs responses.Conclusions:Development of a standardized set of PREMs has allowed us to initiate collection on a national basis. Addition of PREMs to the national electronic platform provides a unique means of collecting large volumes of data consistently, allowing us to benchmark across and within organizations. It will also allow experience teams to target improvement initiatives and identify good practice. Together with outcomes responses, the data will be used to measure experience of care in Wales.

ZusammenfassungAuskünfte von Patient*innen über die eigene Lebensqualität, Erfahrungen in der Versorgung sowie individuelle Einschätzungen zu Therapieverläufen und -ergebnissen gewinnen zunehmend an Bedeutung. Patient*innenberichtete Merkmale können sich auf subjektive Informationen zur eigenen Gesundheit (Patient-reported Outcome Measures [PROMs]) oder auf objektive Informationen zur Erfahrung während des Behandlungsprozesses (Patient-reported Experience Measures [PREMs]) beziehen. Dieser Artikel gibt einen Überblick über die Gemeinsamkeiten und Unterschiede von PROMs und PREMs. Anschließend werden Möglichkeiten zur Erfassung von PROMs und PREMs vorgestellt und hierbei ein Einblick in die probabilistische Testtheorie (Item-Response-Theory) sowie in das computeradaptive Testen gegeben. Am Beispiel von nationalen und internationalen Initiativen wird die Umsetzung von PROMs und PREMs in Gesundheitssystemen dargestellt und zukünftige Implementierungsstrategien innerhalb eines Ausblicks diskutiert.

Abstract:
Мета: проаналізувати дані літературних джерел щодо використання анкет PREMs (показники досвіду, про які повідомляють пацієнти) та PROMs (показники результатів, про які повідомляють пацієнти) і визначити їх роль у покращенні поточних клінічних послуг та розробки подальших послуг як на користь пацієнтів, так і надавачів послуг. Матеріали і методи. Проведено аналіз літературних джерел щодо використання анкет PREMs (показники досвіду, про які повідомляють пацієнти) і PROMs (показники результатів, про які повідомляють пацієнти). Використано бібліосемантичний (вивчення фахової сучасної міжнародної літератури з досліджуваної проблеми) та аналітичний методи дослідження. Результати. Програми PROMs і PREMs доповнюють традиційні показники результатів і дають змогу краще зрозуміти результати та ефективність медичної допомоги. PROMs (показники результатів, які повідомляють пацієнти) і PREMs (показники досвіду, які повідомляють пацієнти) – це заходи, які забезпечують орієнтований на пацієнта погляд на медичну допомогу. Програми PROMs дозволяють клініцистам отримати уявлення про вплив хвороби та лікування на їх спосіб життя та якість життя. PREMs є життєздатним і надійним рішенням для оцінки заходів процесу та оцінки якості допомоги. Висновок. Клініцисти визнали, що високоякісна клінічна допомога вимагає від пацієнтів надавати інформацію, що стосується їхніх почуттів, їх симптомів та наслідків методів лікування, які вони застосували.

Bien prendre en charge le bon patient au bon moment au bon endroit selon les dernières recommandations des sociétés savantes est aujourd’hui la préoccupation de chaque professionnel de santé dans le but d’améliorer l’état de santé des populations. A travers la mise en place du parcours de soins innovant accéléré «Diagnostic en un jour» pour les pathologies hépato-biliopancréatiques aux Hôpitaux Universitaires de Strasbourg et la réalisation d’une étude comparative au parcours classique avec respectivement 330 vs 152 patients, l’efficacité́ et la soutenabilité́ financière de celui-ci sont prouvées. Nous montrons la possible diffusion de ce modèle à plus grande échelle pour d’autres pathologies et régions. Par ailleurs, la démarche VBHC appliquée à travers les questionnaires standardisés établis pour les cancers du foie et du pancréas permettra le recueil des résultats qui impotent aux patients PROMs et cliniques CROMs ayant pour objectif de tendre vers la pertinence et l’excellence des soins
Nowadays, taking good care of the right patient at the right time in the right place according to the latest guidelines established by professional societies is the main concern of each health care provider, aiming to improve populations’ state of health. From the example of the innovative and accelerated care pathway “Oneday diagnosis” for hepatobiliary and pancreatic diseases taking place in Strasbourg university hospital and the comparative study to ordinary pathway with respectively 330 vs 152 patients, we demonstrated its efficiency and sustainability. We confirmed the possible replication elsewhere and for other pathologies. Furthermore, the application of the VBHC concept through standardized questionnaires for liver and pancreatic cancers would allow to collect outcomes that matter to patients PROMs and clinical outcomes CROMs with the objective of reaching relevant and excellent care

Introduzione: i patient reported experience measure (PREMs) sono indicatori di qualità dell’assistenza e misurano il processo di cura riferito dal paziente. Non sono disponibili questionari PREMs disegnati e validati per descrivere l’esperienza assistenziale durante il ricovero in neonatologia. Obiettivo: costruire un questionario PREMs attraverso un processo di definizione con il metodo Delphi di una lista di domande che investighino l’esperienza dell’assistenza neonatale dal momento del ricovero a quello della dimissione e di validare lo strumento. Materiali e metodi: è stata condotta una revisione rapida della letteratura per identificare questionari sulla percezione dell’assistenza riportati negli studi di validazione pubblicati in 2008-2018. Gli item sull’esperienza, selezionati da ogni questionario incluso, sono stati sottoposti agli stakeholder in 3 round Delphi per il giudizio di rilevanza. I criteri predefiniti di raggiungimento del consenso sono stati applicati alla fine del round 2 e agli item trattenuti è stata attribuita una priorità di inclusione nel questionario, somministrato successivamente, alla dimissione, a genitori di neonati ricoverati in neonatologia. L’analisi fattoriale esplorativa è stata utilizzata per esaminare la struttura dello strumento. Risultati: sono stati identificati 380 studi, 4 sono stati inclusi e 69 item sottoposti al giudizio degli stakeholder (54 genitori e 52 professionisti). Alla fine del processo Delphi, il questionario finale di 18 item è stato validato su un campione di 97 genitori. L’analisi fattoriale con scoring dicotomico degli item ha identificato 2 fattori con buon adattamento al modello: esperienza dell’assistenza durante la degenza ed esperienza di presa in carico e dimissione. Conclusione: il questionario è il primo strumento validato disponibile per misurare l’esperienza dell’assistenza in neonatologia attraverso due indicatori specifici, che consentono di comparare, fra strutture omogenee o all’interno della stessa struttura nel tempo, l’esperienza del processo di cura riferita dai genitori, per identificare aree critiche verso cui orientare interventi di miglioramento della qualità assistenziale.
Background: patient reported experience measures (PREMs) are quality indicators of care which measure the patient-reported care process. No PREMs questionnaires have been developed and validated to investigate the experience of care in neonatal units. Objective: to develop a PREMs questionnaire through a process of selection via the Delphi method of a list of questions that investigate the experience of neonatal care from admission to discharge, and to validate this instrument. Materials and methods: a rapid review of the literature was conducted to identify questionnaires on the perception of care reported in validation studies published between 2008-2018. The items referring to experience, selected from each included questionnaire, were submitted to the stakeholders in 3 Delphi rounds for the judgment of relevance. Previously defined consensus criteria were used to reach an agreement at the end of round 2 and the retained items were given a priority for inclusion in the questionnaire. This was administered after discharge to parents of newborns admitted to the neonatal unit. An exploratory factor analysis was used to examine the structure of the instrument. Results: 380 studies were identified, 4 were included and 69 items submitted for the stakeholders’ decision (54 parents and 52 professionals). At the end of the Delphi process, the final 18-item questionnaire was validated using 97 parents. The factor analysis with dichotomous scoring of the items identified 2 factors with that fit the model well: the experience of in-hospital assistance and the experience during admission and discharge. Conclusion: the questionnaire is the first validated instrument to measure the experience of care in neonatal units through two specific indicators, which allow for a comparison of the experience of care reported by the parents between homogeneous structures or within the same structure over time, and to identify critical areas for improvement in the quality of care.

Abstract:
Mainstream press tends to give visibility to immigrants only when problems arise. Through this visibility based on problems a virtual social identity is constructed and given to the immigration population. This identity is built on stereotypes and is not necessarily correct. However, the neighbourhood press of Barcelona, inscribed within the Catalan regional press’ own idea of journalism, offers a different approach to this issue due to its history, proximity and commitment to the region, and hence, the different characteristics of these journalists’ field where different capitals are sought and taken into consideration and different habitus are found. Therefore this press constructs and gives a different virtual social identity by using a differentiated set of frames. Through studying this mechanism the process through which this particular sort of press, and specifically its journalists, approach the idea of journalism was unearthed and its journalistic field explained.

No obstant això, la premsa de barri de Barcelona, dintre de la tradició de la premsa regional Catalana, ofereix, a causa de la seva història, proximitat i compromís amb la regió, un enfocament diferent a aquest problema i, per tant, les característiques diferents del camp social d'aquests periodistes fan que es busquin i es tinguin en compte diferents habitus i diferents capitals. D’aquesta manera aquesta premsa construeix i dóna una identitat social virtual diferent mitjançant l'ús d'un conjunt diferenciat de marcs. A través de l'estudi d'aquest mecanisme el procés a través del qual aquest tipus particular de premsa, i en concret els seus periodistes, s'acosten a la idea del periodisme ha estat estudiat i el seu camp periodístic explicat.

A l'efecte adduït s'han recercat, identificat, localitzat –sempre que ha estat possible, en la majoria dels casos– i registrat les 469 publicacions periòdiques editades al conjunt de la demarcació, sense distincions de cap tipus. Tal vegada, la particularitat i principal novetat i aportació de la present recerca descansa, en definitiva, en l'extensió del seu marc socioterritorial, en l'ambició del seu marc cronològic i, sobretot també, en la concreció i la diversificació del seu corpus d'estudi.

The main objective is to analyse and settle the multiple and different rhythms, degrees and dynamics of the implementation of the press in the demarcation of Lleida, according to each territory and society, and by extension of society and mass culture in the characteristic period of its gestation, configuration and consolidation in the country as a whole. To this end, the 469 periodicals published throughout the demarcation have been searched, identified, located –whenever possible, in most cases– and recorded, without distinction of any kin. Perhaps the particularity and main novelty and contribution of this research lies, in short, in the extension of its socio-territorial framework, in the ambition of its chronological framework and, above all, also in the concretion an diversification of its corpus of study.

Francesc Mestre i Noè (Tortosa, 1866-†1940) -periodista, dinamitzador cultural, escriptor, llibreter i Cronista Oficial de Tortosa- és una figura polifacètica, complexa i fecunda, fruit de dues generacions: la dels fills de la Renaixença i dels Jocs Florals i la dels republicans de la Septentrina i del Pacte de Tortosa. Ell ha nascut en un territori convuls per les guerres, les inundacions i les epidèmies, on la vida humana ha deixat de tenir cap valor. La comarca dertusana, política i socialment, es una olla de grills on la dinastia borbònica, les estructures d’Estat i el bipartidisme governamental han aconseguit fincar i arrelar la corrupció, el caciquisme i el cunerisme. La presa de decisions respon a uns interessos ben allunyats d’aquest territori, de manera que els seus habitants pateixen els seus efectes i les seves conseqüències amb acarnissament. Els conceptes català, catalanitat i catalanisme no existeixen ni en el vocabulari de la gent ni en el seu imaginari. Els que parlen la llengua de la terra ho fan sota la denominació acomplexada de les variants locals; viuen a la comarca de Tortosa com a tortosins, roquetencs, ampostins, rapitencs... o com espanyols. L’escolarització, portada a terme única i exclusivament en llengua castellana, és una assignatura pendent. La major part de l‘alumnat, només te estudis elementals o primaris i, tot i saber llegir i escriure, no estan en disposició de comprendre els textos que tenen al seu davant. Francesc Mestre plantejarà la seva vida centrant-se en un objectiu: Tortosa, el seu territori i la recuperació identitària d’aquestes terres. Ell entén el catalanisme com una forma de veure, de tractar i de treballar el territori; com una manera de viure i d’enfocar la vida. Aquest és un concepte apte per a tothom i compatible amb tota mena d’ideologies no excloents. La premsa en català és la seva eina, el seu mitja i la seva arma de combat; les “Veus” de Tortosa i de Catalunya els seus principals altaveus. Mestre es una figura de consens que posa en valor la llengua catalana, el catalanisme i la catalanitat de tots els territoris de parla catalana; els reivindica, connecta amb ells i els recupera dins el seu ideari. Ell representa el catalanisme inclusiu, plural i divers; uns principis que, amb la seva manera de ser i de fer, son reconeguts com a valors d’ordre i d’honestedat. Francesc Mestre i Noè (Tortosa, 1866 -† 1940) - journalist, cultural impeller, writer, bookkeeper and Official Chronicler of Tortosa - is a multifaceted, complex and fruitful figure, the result of two generations: the heirs of the Renaixença and the Jocs Florals, and the republicans of the Septentrina and the Tortosa Pact. He was born in a convulse territory by wars, floods and epidemies, where human life ceased to have any value. The Dertusana region, politically and socially, is a conflicted zone where the Bourbon dynasty, state structures and the governmental bipartidism have succeeded in establishing and rooting corruption, caciquisme and cunerisme. Decision-making responds to interests that reside far away from the territory, so that its inhabitants suffer their effects and their consequences with extreme cruelty. Catalan, Catalanity and Catalanism are concepts that do not exist neither in the vocabulary of the people nor in their imagination. Those who speak the land’s tongue do it under the complexed dominance of the local variants; They live in the region of Tortosa as tortosins, roquetencs, ampostins, rapitencs ... or as Spaniards. The schooling, carried out exclusively in Spanish, is a pending subject. Most students only have elementary or primary studies, and, even though they can read and write, are not able to understand the texts that are presented to them. Francesc Mestre will focus his life on a single objective: Tortosa, his territory and the recovery of this land’s identity. He understands Catalan as a way of seeing, treating and working the territory; as a way of living and understanding life. This is a concept that is appropriate for everyone and compatible with all kinds of non-exclusive ideologies. The press in Catalan is his tool, his medium and his weapon of combat; the "Veus" de Tortosa and of Catalunya their main speakers. Mestre is a figure of consensus that highlights the Catalan language, Catalanism and Catalanity in all the Catalan-speaking territories; He vindicates them, connects with them and retrieves them within his ideals. He represents an inclusive, plural and diverse Catalanism; principles that, with his way of being and acting, are recognized as values ​​of order and honesty. His causes are closely linked to its country, the counties of its diocese and its city. The issues of the country will be shared with the rest of the Catalanist movement. La tesi analitza les representacions de la pobresa a la premsa hegemònica argentina a través de l'estudi dels editorials publicats pels diaris La Nació i Clarín entre els anys 2000 i 2018. L'enfocament teòric parteix d'una perspectiva crítica enfocada en els processos de producció i reproducció social de la pobresa i centrada específicament en l'anàlisi de la forma en què la desigualtat social i la pobresa es reprodueixen discursivament. A nivell metodològic s'utilitza un enfocament hermenèutic i interpretatiu que, a partir d'un procés de triangulació, posa en relació els textos dels editorials, l'anàlisi de les dades i esdeveniments del context històric i les concepcions teòriques i ideològiques que fonamenten les argumentacions. A partir d'aquest enfocament teòric i metodològic, s'analitzen les representacions mediàtiques sobre la pobresa i els pobres atenent especialment a tres qüestions: l'anàlisi comparativa entre les representacions de la pobresa com un fenomen general i abstracte i les representacions de situacions de pobresa específiques que concerneixen a grups concrets; la modalitat que assumeix la vinculació entre discurs mediàtic i discurs científic observant quins són els paradigmes teòrics que estan més presents en el discurs mediàtic; i el posicionament ideològic des del qual els editorials interpreten la pobresa i la manera en què es situen en relació als diferents valors i models socials. Els resultats de la investigació confirmen, en termes generals, les troballes d'estudis precedents referits a una representació passiva i negativa de la pobresa com un fenomen individual en el qual les persones pobres apareixen com necessitades, dependents, desviades de les normes socials i ubicades fora o al marge de la societat. Al seu torn, en relació a les preguntes específiques de la investigació, es constata que quan els editorials parlen de pobresa abstracta predomina un discurs centrat en la commiseració, mentre que quan al·ludeixen a la pobresa concreta emfatitzen un discurs criminalitzador. Entre tots dos tipus de discursos s'observa un de transversal basat en el caràcter moralitzador i pedagogizant del treball i l'educació com a fórmules idònies per a resoldre les situacions de pobresa. Pel que fa a la vinculació entre discurs mediàtic i discurs científic s'observa una confluència parcial amb el paradigma funcionalista, amb el qual els editorials comparteixen les nocions d'ordre, equilibri i unitat basades en la institucionalització i el control social i la pressuposició de la legitimitat de l'ordre social existent on les causes estructurals de la pobresa no són posades en qüestió. Finalment, els editorials busquen legitimar la ideologia dominant i justifiquen els postulats de la doctrina neoliberal com els més idonis per interpretar i resoldre el fenomen de la pobresa. La temàtica de la pobresa és instrumentalitzada com un ítem central en l'estratègia de polarització entre el model liberal-conservador i el model nacional-popular, al què es responsabilitza per la creació de pobresa. Aquesta representació ideològica de la pobresa es legitima fàcticament en programes específics de disciplinament destinats als pobres construïts com classes laborioses i classes perilloses.
La tesis analiza las representaciones de la pobreza en la prensa hegemónica argentina a través del estudio de los editoriales publicados por los diarios La Nación y Clarín entre los años 2000 y 2018. El enfoque teórico parte de una perspectiva crítica enfocada en los procesos de producción y reproducción social de la pobreza y centrada específicamente en el análisis de la forma en que la desigualdad social y la pobreza se reproducen discursivamente. A nivel metodológico se utiliza un enfoque hermenéutico e interpretativo que, a partir de un proceso de triangulación, pone en relación los textos de los editoriales, el análisis de los datos y acontecimientos del contexto histórico y las concepciones teóricas e ideológicas que fundamentan las argumentaciones. A partir de este enfoque teórico y metodológico, se analizan las representaciones mediáticas sobre la pobreza y los pobres atendiendo especialmente a tres cuestiones: el análisis comparativo entre las representaciones de la pobreza como un fenómeno general y abstracto y las representaciones de situaciones de pobreza específicas que atañen a grupos concretos; la modalidad que asume la vinculación entre discurso mediático y discurso científico observando cuáles son los paradigmas teóricos que están más presentes en el discurso mediático; y el posicionamiento ideológico desde el cual los editoriales interpretan la pobreza y el modo en que se sitúan en relación a los diferentes valores y modelos sociales. Los resultados de la investigación confirman, en términos generales, los hallazgos de estudios precedentes referidos a una representación pasiva y negativa de la pobreza como un fenómeno individual en el cual las personas pobres aparecen como carenciadas, dependientes, desviadas de las normas sociales y ubicadas afuera o al margen de la sociedad. A su vez, en relación a las preguntas específicas de la investigación, se constata que cuando los editoriales hablan de pobreza abstracta predomina un discurso centrado en la conmiseración, mientras que cuando aluden a la pobreza concreta enfatizan un discurso criminalizador. Entre ambos tipos de discursos se observa uno transversal basado en el carácter moralizador y pedagogizante del trabajo y la educación como fórmulas idóneas para resolver las situaciones de pobreza. En cuanto a la vinculación entre discurso mediático y discurso científico se observa una confluencia parcial con el paradigma funcionalista, con el que los editoriales comparten las nociones de orden, equilibrio y unidad basadas en la institucionalización y el control social y la presuposición de la legitimidad del orden social existente donde las causas estructurales de la pobreza no son puestas en cuestión. Finalmente, los editoriales buscan legitimar la ideología dominante y justifican los postulados de la doctrina neoliberal como los más idóneos para interpretar y resolver el fenómeno de la pobreza. La temática de la pobreza es instrumentalizada como un ítem central en la estrategia de polarización entre el modelo liberal-conservador y el modelo nacional-popular, al que se responsabiliza por la creación de pobreza. Esta representación ideológica de la pobreza se legitima fácticamente en programas específicos de disciplinamiento destinados a los pobres construidos como clases laboriosas y clases peligrosas.
This thesis analyzes the representations of poverty in the Argentinian hegemonic press through the study of the editorials published by the newspapers La Nación and Clarín between the years 2000 and 2018. The theoretical approach takes a critical perspective focused on the processes of social production and reproduction of poverty, and it is specifically centered on the analysis of how social inequality and poverty are reproduced at the discursive level. A hermeneutic and interpretative approach is used as methodological tool. Based on a triangulation process, this approach relates the texts of the editorials, the analysis of data and the events within the historical context and the theoretical and ideological conceptions upon which the argumentations are built. From this theoretical and methodological approach, the media representations of poverty and the poor are analyzed, with emphasis on the following issues: the comparative analysis between the representations of poverty as a general and abstract phenomenon, and the representations of specific situations of poverty that affect specific groups; the modality that assumes the relationship between mediatic and scientific discourse observing which are the theoretical paradigms that are more present in the mediatic discourse; and the ideological positioning from which the editorials interpret poverty and where they stand in relation to the different social values and models. The results of this research confirm the findings of preceding studies, which refer to a passive and negative representation of poverty seen as an individual phenomenon in which poor people appear as deprived, dependent, deviated from social norm and located outside or on the margin of society. Related to the specific questions of the research, it is confirmed that a discourse centered on commiseration is predominant when editorials speak about abstract poverty, whereas a criminalizing one is emphasized when they allude to specific poverty. Between both type of discourses a transversal one is observed, in which work and education are morally and pedagogically presented as the most suitable formula to solve situations of poverty. Between the media discourse and the scientific discourse a partial confluency with the functionalist paradigm is observed, with which the editorials share the notions of order, equilibrium and unity based on social institutionalization and control, and where they assume the legitimacy of existing social order without questioning the structural causes of poverty. Finally, the editorials attempt to legitimate the dominant ideology and to justify the postulates of the neoliberal doctrine as the most suitable ones to interpret and solve the phenomenon of poverty. The topic of poverty is instrumentalized as a central item in the strategy of polarization between the liberal-conservative model and the national-popular model, the latter being responsible for the creation of poverty. This ideological representation of poverty is factually legitimated in discipline programs directed to the poor, seen as a laborious and dangerous class.

Abstract:
This Evidence Check identifies patient‐reported experience measures (PREMs) used to evaluate integrated services. The review was conducted to support the development of a survey to use with families accessing the NSW Vulnerable Families program in order to better understand their experiences. Only a small number of PREMs were identified for use with people with both chronic or complex health conditions and social vulnerabilities. While all the PREMs identified are for use in integrated or coordinated care, only a few have direct applicability to the complex, vulnerable populations participating in Vulnerable Families or to the co-located, cross-service system Vulnerable Families model.
