Abstract:
Preeclampsia (PE) is a multi-organ system disorder defined as hypertension with blood pressures greater than 140/90 on two occasions and proteinuria of more than 300 mg/24 hours. Eclampsia is defined as when seizures occur in a woman with preeclampsia. The pathophysiology of preeclampsia/eclampsia is felt to be related to incomplete penetration of the cytotrophoblasts of the placenta into the myometrium, leading to local ischemia, propagation of ischemic factors causing hypertension, resulting in endothelial dysfunction. The clinical features are related to which end organ is involved: in the kidney, proteinuria; in the liver, coagulopathy; and in the brain, posterior white matter dysfunction. The involvement of the parietal and occipital lobes explains the associated neurological features of confusion and visual changes. MRI reflects the white matter changes associated with eclampsia in posterior reversible encephalopathy syndrome (PRES). Eclampsia is treated with blood pressure control and magnesium to treat the seizures.

Abstract:
An illustrative case of reversible cerebral vasoconstriction syndrome (RCVS) is presented. The clinical and radiographic features are reviewed. There is an overlap between RCVS and posterior reversible encephalopathy syndrome due to their shared pathophysiology related to endothelial injury. Furthermore, preeclampsia/eclampsia (PEE) is often associated with RCVS. Thunderclap headache is the most characteristic feature of the disorder. RCVS may also cause subarachnoid hemorrhage, typically located over the cerebral convexities; intracranial hemorrhage; and ischemic stroke. The diagnosis is made by seeing vasoconstriction on vessel imaging, either magnetic resonance angiography or computerized tomography angiography. There are several medications that can precipitate the condition by increasing sympathetic activity. Treatment is similar to that of PEE, with antihypertensives and magnesium. Calcium channel blockers are helpful symptomatic therapy for the headache.

Offering an understanding of postpartum psychosis, this riveting book explains what happens and why during this temporary and dangerous disorder that develops for some women rapidly after childbirth. Most of us are familiar with the baby blues, a passing sadness that strikes 50 to 75 percent of new mothers after delivery. And most of us understand postpartum depression, a sadness post-delivery that lingers for weeks or months for an estimated one in every 10 new mothers. But a more serious form of disorder that strikes up to one in every 500 is postpartum psychosis - triggering severe agitation, confusion, insomnia, hallucinations, delusions, mania, and possible thoughts of suicide or murder. Every year, women in the United States and around the world kill their babies, children, and themselves as a result of this mental illness. Here, author Twomey, an official with Postpartum Support International, gives us insight into the psychological, personal, medical, legal, and historical perspectives on this little-understood mental illness, which is both preventable and treatable. While most women who suffer postpartum psychosis eventually recover without harming anyone, they most often do so in silence. Paranoia is a common symptom, explains Twomey, and that moves women to hide their symptoms from everyone around them. The woman can hence appear normal, but be putting both herself and her baby at risk. We can prevent and treat this, but we need to recognize it by better screening of women postpartum, says Twomey.
