Abstract:
The low overall post-operative mortality rate conceals the existence of a sub-group of high-risk patients, which accounts for over 80% of post-operative deaths. Age, co-morbid disease, limited functional capacity, and an emergency presentation for major surgery are hallmarks risk. The magnitude, duration, and consequences of post-operative morbidity are determined by a complex interplay between the indication for surgery, the resulting tissue injury, and patient factors. A number of methods including risk scoring and cardiopulmonary exercise testing can be used to identify the high-risk group. Efforts should be made throughout the peri-operative period to prevent the occurrence of any post-operative complications, as they all carry significant long-term implications.

The decades 1960–80 witnessed a seismic shift in modern drama. The rage that came to define, and fuel, much of the drama in the 1960s and 1970s is directed at the audience. ‘Absurdism, protest, and commitment’ shows it is a post-war rage stemming from many sources: the Vietnam War, the Cold War, a feeling of betrayal by government and politicians, the Civil Rights Movement, Black Power, gay rights, feminism, the growing gap between rich and poor, and ethnic oppression. It is all about denying the audience what it expects of a play, provoking it out of real or perceived complacency, startling, and offending it. The plays of Pinter, Shepard, Beckett, Stoppard, Friel, and Fugard are discussed.

Abstract:
In the 1980s, sudden infant death syndrome (also known as SIDS or cot death) was one of the most common post-neonatal causes of death in the UK. The dramatic 80% fall in these deaths over the last three decades is a testament to evidence-based research using the observational case–control study and even more impressive given the difficulties in funding studies without a randomized controlled trial design. This chapter will describe the initial breakthrough in the early 1990s, the characteristic profile of SIDS, the associated factors identified, the potential causal mechanisms, and the current risk reduction messages. Most of the risk reduction messages are non-controversial and have been readily accepted by health professionals and parents alike. The divergence of opinion regarding how we give preventative advice surrounding infant bed sharing and the strategies employed is addressed in more detail.

Abstract:
For decades, researchers have investigated the impact of market economies on indigenous peoples' lifeways and natural resources. This chapter reveals how incorporation of Navajo pastoralists into the American wool and livestock markets via the trading-post system initiated a turning point in Diné history. The passage of the Dawes Act in 1887, triggered the loss of over 80 million acres of tribal lands and ultimately impoverished thousands of Native Americans. That same year, revisions proposed to the wool tariff initiated a change in federal policy that ultimately held profound consequences for Navajo woolgrowers and weavers. Although their reservation was periodically enlarged to accommodate the need for increased grazing lands, Navajos' livelihood was significantly compromised. As livestock owners and weavers, Navajo women were doubly disadvantaged by changes in the domestic wool tariff coupled with patriarchal assumptions that obliterated their contributions to subsidizing the reservation economy from 1880 to World War II.

Overlooked yet reassuringly familiar, the ageing woman has been central to the narrative landscape of British cinema. Historicising and contextualising enduring archetypes, this book establishes a taxonomy of female ageing in British cinema from the 1930s to the present day, encompassing bluestockings, battleaxes, witches, widow-women, duchesses, matriarchs and spinsters. These archetypes draw on well-established mythologies regarding ageing femininity. The prevalence of various iterations of female ageing is essential in understanding the nature of British cinema and how it developed to define itself under the shadow of Hollywood. The book examines the work of ageing British film stars including Ciceley Courtneidge, Margaret Rutherford, Maggie Smith and Sybil Thorndike, in performing female ageing. Centred on close textual analysis of a broad range of films from over 80 years of British cinema, the book casts a fresh light on both the familiar and lesser known, including Blithe Spirit (1945), The Prime of Miss Jean Brodie (1968), and The Best Exotic Marigold Hotel (2012). The films are considered within the context of the broader historical factors which impacted on ageing femininities, including the Second World War, the post-war settlement and the Welfare State, and looks at the implications for the women’s movement as a whole.

Black Americans are by far the most unified racial group in American electoral politics, with 80 to 90 percent identifying as Democrats—a surprising figure given that nearly a third now also identify as ideologically conservative, up from less than 10 percent in the 1970s. Why has ideological change failed to push more black Americans into the Republican Party? This book answers this question with a pathbreaking new theory that foregrounds the specificity of the black American experience and illuminates social pressure as the key element of black Americans' unwavering support for the Democratic Party. The book argues that the roots of black political unity were established through the adversities of slavery and segregation, when black Americans forged uniquely strong social bonds for survival and resistance. It explains how these tight communities have continued to produce and enforce political norms—including Democratic Party identification in the post-Civil Rights era. The social experience of race for black Americans is thus fundamental to their political choices. Black voters are uniquely influenced by the social expectations of other black Americans to prioritize the group's ongoing struggle for freedom and equality. When navigating the choice of supporting a political party, this social expectation translates into affiliation with the Democratic Party. The book explores where and how black political norms are enforced, what this means for the future of black politics, and how this framework can be used to understand the electoral behavior of other communities.

Anaesthetists have a central role in cardiopulmonary resuscitation (CPR). The incidence of treated out-of-hospital cardiopulmonary arrest is 40 per 100 000 population and is associated with a survival rate to hospital discharge of 8–10%. The incidence of in-hospital cardiac arrest (IHCA) is 1–5 per 1000 admissions and is associated with a survival rate to hospital discharge of 13–17%. The most effective strategy for reducing mortality from IHCA is to prevent it occurring by detecting and treating those at risk or to identify in advance those with no chance of survival and to make a decision not to attempt resuscitation. The European Resuscitation Council and the Resuscitation Council (UK) publish guidelines for CPR every 5 years and the evidence supporting these is described in the international consensus on CPR science. The advanced life support algorithm forms the core of the guidelines but the precise interventions depend on the circumstances of the cardiac arrest and the skills of the healthcare providers. High-quality CPR with minimal interruptions will optimize survival rates. Shockable rhythms are treated with defibrillation while minimizing the pause in chest compressions. Although adrenaline (epinephrine) is used in most cardiac arrests, no studies have shown that it improves long-term outcome. The post-cardiac arrest syndrome is common and requires multiple organ support in an intensive care unit. Therapy in this phase is aimed at improving neurological (e.g. targeted temperature management) and myocardial (e.g. percutaneous coronary intervention) outcomes. Based on standard outcome measurements (e.g. cerebral performance category), 75–80% of survivors will have a ‘good’ neurological outcome, but many of these will have subtle neurocognitive deficits.
