Abstract:
Background: The aim of this study was to evaluate efficacy and safety of poly-de-prescribing (PDP) based on the Garfinkel method in older people with polypharmacy. Methods: A longitudinal, prospective, nonrandomized study in Israel was carried out between 2009 and 2016. Comprehensive geriatric assessments were performed at home in people age ⩾66 years consuming ⩾6 prescription drugs. Exclusion criteria were life expectancy <6 months and a seeming unwillingness to cooperate (poor compliance). PDP of ⩾3 prescription drugs was recommended. Follow up was at ⩾3 years. Between April 2015 and April 2016 Likert scale questionnaires were filled by all participants/families to evaluate overall satisfaction and clinical outcomes. The outcome measures were change in functional, mental and cognitive status, sleep quality, appetite, continence; major complication, hospitalizations, mortality, and family doctor’s cooperation. Results: Poly-de-prescribing of ⩾3 drugs was eventually achieved by 122 participants (PDP group); ⩽2 drugs stopped by 55 ‘nonresponders’ (NR group). The average age was 83.4 ± 5.3 in the PDP group, and 80.8 ± 6.3 in the NR group ( p = 0.0045). Follow up was 43.6 ± 14 months (PDP) and 39.5 ± 16.6 months (NR) ( p = 0.09). The prevalence of most diseases/symptoms was comparable except for a higher prevalence for dementia, incontinence and functional decline in the PDP group. The main barrier to de-prescribing was the family doctor’s unwillingness to adopt PDP recommendations ( p < 0.0001). The baseline median number of medications taken by both groups was 10 (IQR 8 to 12) ( p = 0.55). On the last follow up, the drug count was 11 (IQR 8 to 12) in the NR group and 4 (IQR 2 to 5) in the PDP group ( p =0.0001). The PDP group showed significantly less deterioration (sometimes improvement) in general satisfaction, functional, mental and cognitive status, sleep quality, appetite, sphincter control, and the number of major complications was significantly reduced ( p < 0.002 in all). The rate of hospitalizations and mortality was comparable. Health improvement occurred within 3 months after de-prescribing in 83%, and persisted for ⩾2 years in 68%. Conclusions: This self-selected sample longitudinal research strongly suggests that the negative, usually invisible effects of polypharmacy are reversible. PDP is well tolerated and associated with improved clinical outcomes, in comparison with outcomes of older people who adhere to all clinical guidelines and take all medications conventionally. Future double-blind studies will probably prove beneficial economic outcomes as well.

Diabetes mellitus, following cancer and cardiovascular disease, become one of the most serious diseases that threat to human health. The treatment of diabetes is imminent for the health of people. Thus, the developments of efficient insulin administration exhibit great practical significance. The glucose-responsive insulin delivery system (GRIDS) is developing rapidly which is expected to be a promising therapy approach to replace the frequent insulin injection administration. A novel glucose-responsive nanogel was conveniently prepared through one-pot copolymerization of pentaerythritol tetra (3-mercaptopropionate), poly (ethylene glycol) diacrylate, methoxyl poly (ethylene glycol) acrylate and N-acryloyl-3-aminophenylboronic acid. The successful incorporation of phenylboronic acid (PBA) in the nanogel endowed the nanogel high glucose sensitivity in phosphate buffer saline (PBS). Therefore, the PBA incorporated nanogel with high glucose-responsiveness and good biocompatibility may have great potential for self-regulated drug release

The purpose of the study was to determine the need, demand, and utility of public health services in the work area of the Marsabut Lake Health Center, South Tapanuli Regency. Types of quantitative research with a cross-sectional approach. The population is a community living in the working area of Lake Marsabut, South Tapanuli Regency. With a sample number of 60 people. The study was conducted in June 2022. The results of research related to need showed that the main needs of respondents related to health service facilities were quality and distance ≤ 3 km. Community demand is dental poly and general poly services provided by Marsabut Lake, South Tapanuli Regency.

ABSTRACT Objective Nutritional diseases such as metabolic syndrome, cardiovascular disorder, chronic inflammation or even cancer are observed in people who sustain their lifestyle by Western diet due to high calorie intake. The origin of these diseases are the degraded deoxyribonucleic acid structure. In this study, we investigated whether Western diet produced endogenous oxidative deoxyribonucleic acid damage, apoptosis or inflammation. Methods Twenty-eight male Wistar rats, aged 10-12 weeks, were divided into four groups. The rats in control group received the standard diet and the remaining rats were given one of the following three diets for four weeks: a high-fat diet containing 35% fat, a high-sucrose diet containing 69% sucrose and Western diet comprising both two types of diets. After treatment the serum 8-hydroxy-2-deoxyguanosine, poly (adenosine diphosphate ribose) polymerase-1, chitinase-3-like protein 1, soluble urokinase-type plasminogen activator receptor, Fas ligand and cytochrome c levels were measured. Results It was observed no changes in the serum soluble urokinase-type plasminogen activator receptor, Fas ligand and cytochrome c levels whereas a statistically significant increase in the serum 8-hydroxy-2-deoxyguanosine, poly (adenosine diphosphate ribose) polymerase-1 and chitinase-3-like protein 1 levels were found only in rats that were given Western diet. Conclusion The findings show that Western diet produced endogenous oxidative deoxyribonucleic acid damage, which then increased serum poly (adenosine diphosphate ribose) polymerase-1 levels, eventually leading to inflammation.

Abstract:
Osteoarthritis is a bone-joint disease prevalent in older people characterized by joint inflammation. In traditional Chinese medicine, polydatin plays an important anti-inflammatory role. This study analyzed the potential effects and possible internal mechanisms of polydatin on osteoarthritis. First, lipopolysaccharide-induced osteoarthritis injury was established in chondrogenic ATDC5 cells. Lipopolysaccharides significantly stimulated inflammatory injuries in ATDC5 cells as exemplified by a decrease in cell viability and an increase in inflammatory cytokine secretions including interleukin-6, tumor necrosis factor-a, and interleukin-1. Moreover, lipopolysacchrides also increased Cleaved caspase-3 and Cleaved Poly (ADP-ribose) polymerase to promote cell apoptosis. Second, polydatin showed significant protective effects against lipopolysaccharide-induced inflammatory injury, again exemplified by increased cell viability, decreased inflammatory cytokines, Cleaved caspase-3, and Cleaved poly (ADP-ribose) polymerase. Lastly, miR-125b and its binding target Rho-Associated Coiled-Coil Containing Protein Kinase 1 were closely associated with regulatory effects of polydatin against lipopolysaccharide-stimulated ATDC5 cell inflammatory injuries. Polydatin alleviated lipopolysaccharide-stimulated inflammatory injuries via the down-regulation of miR-125b. The present study concludes that polydatin plays an anti-inflammatory role in lipopolysaccharide-stimulated ATDC5 cell inflammatory injuries via the down-regulation of miR-125b.

The purpose of this study was to study the relationship of treatment programs to self-concept in women with mammary carcinoma in the Polyclinic Surgery Clinic of RSUD Dr. Achmad Mochtar Bukittinggi. The design of this study uses descriptive analytic methods. The results of this study were obtained by more than half of respondents who undertook treatment programs at the poly surgical clinic of RSUD Dr. Achmad Mochtar Bukittinggi is 19 people (63.3%) and more than half obtained a low self-concept in women with carsinoma mammaes as many as 16 respondents (53, 3%) in the poly surgical clinic of Dr. Achmad Mochtar Bukittinggi Hospital. The statistical test results obtained p value = 0.046, it can be concluded that there is a relationship between the treatment program for women with mammary mobilinoma in the surgical clinic of RSUD Dr. Achmad Mochtar Bukittinggi with an OR value = 0.130. Abstract:
The article provides characteristics, statistics and factual material on countries connected with the current global economic crisis of the cost of living. The notion of the current poly-crisis was defined as a sum total of old and new risks in their close interaction and their adverse impact on global and national economy and finance markets. The focus was made on tightening of monetary and credit policy of national central banks in 2022–2023 in view of combating inflation, which resulted in more expensive credits, higher profitability of state treasury bonds, bankruptcy of corporations and declining standard of living of people. On the background of the global poly-crisis the author assessed macro-prudential policy of the Bank of Russia and standing of its banking system, including market of preferential mortgage with state support. Within the frames of crisis-management risks of Russian insurance and re-insurance industries were shown, as well as non-credit life insurance as a today’s driver of insurance market growth. In view of import-substitution policy an important innovation of the Moscow exchange – the informational-analytical, financial and trade terminal ‘Trade Radar’ was put forward. The author showed economic essence, trends and tools of de-dollarization policy of Russia and friendly countries, including BRICS project aiming at development of common supranational payment unit.

Abstract:
Post-extraction of M3 tooth extraction is an effort to speed up the healing process and prevent and reduce pain and swelling. Pasien will be given instructions and education on how to take care of the possible tooth extractions. In-depth knowledge of the treatment of M3 tooth extraction wounds can be known in the instruction after removal. This study aims to determine the relationship between the level of patient knowledge about wound care after the removal of molars 3 with the success of wound care at the Dental and Oral Hospital, Padjadjaran University. This type of research is analytics. The research sample was taken by accidental sampling with a total population of 31 respondents who performed M3 tooth extraction at the Minor Surgical Poly of the Dental and Oral Hospital, Padjadjaran University. The results showed that respondents who had a level of knowledge of post-revocation wound care in the good category were 12 people (38.7%). The results of wound treatment of respondents after tooth extraction M3 in the normal wound healing category were 15 people (48.8%). There is a relationship between the level of knowledge of m3 tooth extraction wound care and the success of wound care p(0.03<0.05).

Abstract:
Cigarette smokers and people exposed to second-hand smoke are at an increased risk for pulmonary viral infections, and yet the mechanism responsible for this heightened susceptibility is not understood. To understand the effect of cigarette smoke on susceptibility to viral infection, we used an air-liquid interface culture system and exposed primary human small airway epithelial cells (SAEC) to whole cigarette smoke, followed by treatment with the viral mimetic polyinosinic polycytidylic acid (poly I:C) or influenza A virus (IAV). We found that prior smoke exposure strongly inhibited production of proinflammatory (interleukin-6 and interleukin-8) and antiviral [interferon-γ-induced protein 10 (IP-10) and interferons] mediators in SAECs in response to poly I:C and IAV infection. Impaired antiviral responses corresponded to increased infection with IAV. This was associated with a decrease in phosphorylation of the key antiviral transcription factor interferon response factor 3 (IRF3). Here, we found that cigarette smoke exposure inhibited activation of Toll-like receptor 3 (TLR3) by impairing TLR3 cleavage, which was required for downstream phosphorylation of IRF3 and production of IP-10. These results identify a novel mechanism by which cigarette smoke exposure impairs antiviral responses in lung epithelial cells, which may contribute to increased susceptibility to respiratory infections.

Abstract:
Background Among illicit drugs, the prevalence of amphetamine-type stimulant (ATS) use is second only to cannabis. Currently, there are no approved pharmacotherapies for ATS problems, but some face-to-face psychotherapies are effective. Web-based interventions have proven to be effective for some substance use problems, but none has specifically targeted ATS users. Objective The objective of the study was to evaluate the effectiveness of a Web-based intervention for ATS problems on a free-to-access site compared with a waitlist control group. Methods We used a randomized controlled trial design. The primary outcome measure was self-reported ATS use in the past three months assessed using the Alcohol, Smoking, Substance Involvement Screening Test (ASSIST). Other measures included quality of life (EUROHIS score), psychological distress (K-10 score), days out of role, poly-drug use, general help-seeking intentions, actual help-seeking, and “readiness to change”. The intervention consisted of three fully automated, self-guided modules based on cognitive behavioral therapy and motivation enhancement. The analysis was an intention-to-treat analysis using generalized estimating equation models, with a group by time interaction as the critical assessment. Results We randomized 160 people (intervention n=81, control n=79). At three months, 35/81 (43%) intervention and 45/79 (57%) control participants provided follow-up data. In the intervention group, 51/81 (63%) completed at least one module. The only significant group by time interaction was for days out of role. The pre/post change effect sizes showed small changes (range d=0.14 to 0.40) favoring the intervention group for poly-drug use, distress, actual help-seeking, and days out of role. In contrast, the control group was favored by reductions in ATS use, improvements in quality of life, and increases in help-seeking intentions (range d=0.09 to 0.16). Conclusions This Web-based intervention for ATS use produced few significant changes in outcome measures. There were moderate, but nonsignificant reductions in poly-drug use, distress, days partially out of role, and increases in help-seeking. However, high levels of participant attrition, plus low levels of engagement with the modules, preclude firm conclusions being drawn on the efficacy of the intervention and emphasize the problems of engaging this group of clients in a fully automated program. Abstract:
A large number of non-degradable materials have severely damaged the ecological environment. Now, people are increasingly pursuing the use of environmentally friendly materials to replace traditional chemical materials. Polyhydroxyalkonates (PHAs) are receiving increasing attention because of the unique biodegradability and biocompatibility they offer. However, the applications of PHAs are still limited due to high production costs and insufficient study. This project examines the optimal electrospinning parameters for the production of PHA-based fibrous membranes for air filtration. A common biodegradable polyester, Poly(3-hydroxybutyrate-co-3-hydroxyvalerate) (PHBV), was electrospun into a nanofibrous membrane with a well-controlled surface microstructure. In order to produce smooth, bead-free fibers with micron-scale diameters, the effect of the process parameters (applied electric field, solution flow rate, inner diameter of hollow needle, and polymer concentration) on the electrospun fiber microstructure was optimized. The well-defined fibrous structure was optimized at an applied electric field of 20 kV, flow rate of 0.5 mL/h, solution concentration of 12 wt.%, and needle inner diameter of 0.21 mm. The morphology of the electrospun PHBV fibrous membrane was observed by scanning electron microscopy (SEM). Fourier transform infrared (FTIR) and Raman spectroscopy were used to explore the chemical signatures and phases of the electrospun PHBV nanofiber. The ball burst strength (BBS) was measured to assess the mechanical strength of the membrane. The small pore size of the nanofiber membranes ensured they had good application prospects in the field of air filtration. The particle filtration efficiency (PFE) of the optimized electrospun PHBV fibrous membrane was above 98% at standard atmospheric pressure.

In recent years, the rising number of bone diseases which affect millions of people worldwide has led to an increased demand for materials with restoring and augmentation properties that can be used in therapies for bone pathologies. In this work, PMMA- MBG composite scaffolds containing ceria (0, 1, 3 mol%) were obtained by the phase separation method. The obtained composite scaffolds were characterized by X-ray diffraction, infrared spectroscopy, and scanning electron microscopy. UV–Vis measurement and EDX analysis confirmed the presence of cerium ions in the composite scaffolds. Evaluation of the in-vitro biocompatibility using MTT assay showed that composite scaffold containing 1 mol% of ceria presented higher viability than control cells (100%) for concentrations ranging between 5 and 50% after 96 h of incubation.

Abstract:
OBJECTIVE. To assess the risk of relapse in patients with pulmonary tuberculosis (PTB). MATERIALS AND METHODS. 569 people with relapsed PTB (RPTB) were selected from the TB register (e-TB manager programme): 300 with a first case treatment outcome of “completed treatment” and 269 “cured”. The groups were compared according to the course and outcome of treatment of RPTB, clinical characteristics of the first case, medical and social risk factors, and all patients with RPTB were compared with the group of newly diagnosed PTB (NDPTB). Mathematical and conventional statistical methods were used to analyse. RESULTS AND DISCUSSIONS. It was found that patients with RPTB are a difficult group because more than 1⁄2 of them have lung destruction, 3⁄4 isolate mycobacterium tuberculosis (MBT), and more than 1⁄2 have resistance to antimycobacterial drugs (AMBD). In 73.6 % of them, medical and social risk factors were noted that may have caused or contributed to TB relapse and negatively affected the results of treatment. These factors include: severe comorbidities in 53.7 %, alcohol and drug abuse in 34.9 %, HIV infection in 30.5 %, and others (homelessness, release from prison, contact with a TB patient, unemployment, smoking, etc.); 62.1 % of patients had ≥2 risk factors at the same time. We consider the effectiveness of treatment of this category of patients to be insufficient, as the outcome of the main course of treatment was unsatisfactory in more than half of them (48.5 % of unsuccessfully treated, 4.7 % interrupted treatment, 8.3 % died), and taking into account repeated courses, the rate of effective treatment in some of them was only 65.6 %. There was no significant difference (neither in clinical characteristics nor in medical and social risk factors) between the groups of patients “cured” or “completed treatment” as a result of treatment of the first case of TB, but it was found that only 23.2 % of patients with RPTB were found to be resistant to AMBD at the first case of the disease, and 50.0 % of them had it, with multidrug resistance (MDR) in half of them. It has been statistically proven that patients with RPTB are more likely to have the following symptoms than patients with NDPTB: lung destruction (43.2±3.2 vs. 20.4±4.0 %, p<0.05), sputum isolation of MBT (38.1±3.3 vs. 24.9±3.9 %, p<0.05), multi- and poly-resistance of MBT to AMBD (30.2±8.1 vs. 9.5±4.3 %, p<0.05), various medical and social risks (73.6±2.2 vs. 44.5±3.4 %, p<0.05), including alcohol abuse (21.3±3.7 vs. 8.7±4.3 %, p<0.05 %), unemployment (51.1±2.9 vs. 21.9±4.0 %, p<0.05), concomitant diseases (39.5±3.3 vs. 8.5±4.3 %, p<0.05), HIV infection (22.5±4.0 vs. 4.9±4.4 %, p<0.05) and others – lack of a fixed place of residence, release from prison, contact with a TB patient, lack of permanent employment, smoking, etc. (64.3±2.5 vs. 8.3±4.3 %, p<0.05), as well as ≥2 risk factors at the same time (45.7±3.1 vs. 20.0±4.0 %, p<0.05), which confirms the role of the above factors as probable risks of relapse in TB patients. CONCLUSIONS. Since the role of unfavourable NDPTB course, medical and social risk factors such as unemployment, severe comorbidities, alcohol abuse, HIV infection and some others (belonging to persons of no fixed abode, release from prison, contact with a TB patient) as possible causes of the development (or contributing to) RPTB, which subsequently negatively affect treatment outcomes, patients with these factors should be considered a particularly dangerous population for the development of PTB and, accordingly, should be screened and monitored for life in risk groups accordingly. Given that the detection of MDR-TB in the first case of PTB is the most dangerous risk factor for RPTB, it is necessary to introduce the determination of MDR-TB resistance in all patients with NDPTB and, despite the results of their treatment, to follow up such persons in risk groups. Late detection of RPTB (in 3⁄4) indicates organisational shortcomings of primary care in TB control, primarily in the formation of risk groups and work with them, and requires the implementation of preventive measures taking into account the new realities and challenges of wartime.

The Covid-19 pandemic, which began in early 2020, has affected more than 3 million people in Indonesia. All health services are mobilized for p revention and treatment. The Public Health Center of Perampuan in West Lombok is no exception, which carries out a screening program for suspected visitors as visitors. In mid-2021, when a new variant of covid-19 emerged, namely the delta variant, visitors to the Poli ISPA were known to experience an increase in their covid-19 status through the Rapid Diagnostic Test (RDT) antigen. The purpose of this study was to describe the characteristics of poly ISPA visitors who received a COVID-19 swab examination and to find out whether there was a relationship between these characteristics and the incidence of Covid-19 at the Perampuan Health Center. The research design is descriptive retrospective because it uses secondary data from the registration book of Poli ISPA in Public Health Center of Perampuan. A total of 183 samples were taken from the total population, namely those who received an RDT antigen swab examination. The statistical analysis test used to find the relationship was the chi square test (?=5%). The result of this research is that most of the characteristics of the visitors to the ARI poly at the Perampuan Health Center are mostly in the age group category of 18 years and over, working as employees both public and private, and male. The number of cases of covid-19 there is 52%. The conclusion of this study is that there is no relationship between all characteristics of visitors to the ARI polyclinic as measured by the incidence of Covid-19 at the Perampuan Public Health Center (p>0.05). It is better if the Perampuan Health Center continues to carry out the program for handling and preventing this COVID-19 disease because this disease attacks all levels of society equally regardless of age, gender, and type of work. The innate immune response to bacterial and viral molecules involves the coordinated production of cytokines, chemokines, and type I interferons (IFNs), which is orchestrated by toll-like receptors (TLRs). TLRs, and their intracellular signalling intermediates, are closely associated with multiple sclerosis (MS) pathogenesis. Recent data from our laboratory reported that the plant-derived cannabinoids, Δ9-tetrahydrocannabinol (THC) and cannabidiol (CBD), regulate viral and bacterial inflammatory signalling pathways controlled by TLR3 and TLR4 in macrophages. The aim of this study was to assess the impact of THC and CBD, when delivered in isolation and in combination (1:1), on TLR3- and TLR4-dependent signalling in peripheral blood mononuclear cells (PBMCs) from people with MS (pwMS; n = 21) and healthy controls (HCs; n = 26). We employed the use of poly(I:C) and lipopolysaccharide (LPS) to induce viral TLR3 and bacterial TLR4 signalling, and PBMCs were pre-exposed to plant-derived highly purified THC (10 μM), CBD (10 μM), or a combination of both phytocannabinoids (1:1 ratio, 10:10 μM), prior to LPS/poly(I:C) exposure. TLR3 stimulation promoted the protein expression of the chemokine CXCL10 and the type I IFN-β in PBMCs from both cohorts. THC and CBD (delivered in 1:1 combination at 10 μM) attenuated TLR3-induced CXCL10 and IFN-β protein expression in PBMCs from pwMS and HCs, and this effect was not seen consistently when THC and CBD were delivered alone. In terms of LPS, TLR4 activation promoted TNF-α expression in PBMCs from both cohorts, and, interestingly, CBD when delivered alone at 10 μM, and in combination with THC (in 1:1 combination at 10 μM), exacerbated TLR4-induced TNF-α protein expression in PBMCs from pwMS and HCs. THC and CBD displayed no evidence of toxicity in primary PBMCs. No significant alteration in the relative expression of TLR3 and TLR4 mRNA, or components of the endocannabinoid system, including the cannabinoid receptor CB1 (encoded by CNR1 gene) and CB2 (encoded by CNR2 gene), and endocannabinoid metabolising enzymes, fatty acid amide hydrolase (FAAH) and monoacylglycerol lipase (MGLL), was determined in PBMCs from pwMS versus HCs. Given their role in inflammation, TLRs are clinical targets, and data herein identify CBD and THC as TLR3 and TLR4 modulating drugs in primary immune cells in vitro. This offers insight on the cellular target(s) of phytocannabinoids in targeting inflammation in the context of MS.

Capgras Syndrome is a subcategory of delusional disorder. People affected by this syndrome believe that a close associate such as a friend or family member has been replaced by an identical imposter. This case report describes a 23-year-old woman with no prior psychiatric history, whom developed Capgras syndrome, via folie a deux, in the setting of poly-substance use. In this patient, a combination of Aripiprazole 10 mg daily and Escitalopram 10 mg daily were effective in resolving symptoms. Clonazepam was utilized for anxiety and Omega-3 fatty acids 1 g for anti-oxidative effects. Further studies are needed to investigate the effects of a variety of causes and treatments for Capgras Syndrome.

Objectives: There is limited research on combined use of cigarettes, snus and e-cigarettes in the Nordic countries. This study will investigate whether this pattern of use occurs among adolescents in Norway. In addition, we will investigate the association between combined use of several tobacco products, e-cigarettes and socio-demographic and psychosocial factors (SPF). Method: A school-based cross-sectional survey of adolescents in the 8th to the 12th grade ( N = 875) in 2014. Information on the use of cigarettes, snus, e-cigarettes and SPF was collected using an electronic questionnaire. Results: Of the adolescents, 31% reported having tried or used one or more tobacco product and/or e-cigarette. Of these, over half reported having tried or used one product, 1/3 two products and 1/10 three products. There were more boys among poly-users (63%) than among single-users (44%). Compared to single-users, poly-users also reported weaker school performance (30% and 11%, respectively), more often reported drinking alcohol to intoxication (91% and 75%, respectively), and having tried cannabis (28% and 9%, respectively). Poly-users also scored higher on impulsivity, sensation-seeking and depression. Conclusion: The study shows that combined use of tobacco products and e-cigarettes occurs among young people in Norway. Such use was associated with psychosocial and socio-demographic risk factors. There is a need to investigate this phenomenon in more details in representative samples.

Abstract Chronic Fatigue Syndrome (CFS) is a debilitating disease of unknown etiology that affects ~17 million people worldwide. Patients suffer persistent viral infections and may develop hematopoietic malignancies. The patients have reduced Natural Killer (NK) cell activities which could contribute to the diseases, and, if restored, potentially reduce symptoms. We characterized the in vitro effects on NK cells of the interferon inducer poly (I): poly (C12U) (Ampligen) which in some CFS patients abates disease symptoms. The 30 CFS patients were infected with the gammaretrovirus xenotropic murine leukemia virus-related virus (XMRV). We cultured their peripheral blood mononuclear cells with the drug for 24 hrs and monitor NK activity to K562 cells by flow cytometry, concurrently measuring degranulation by externalization of CD107a, and expression of Grz B and perforin. Treatment markedly increased CD107a externalization in the NK cell population as indicated by 5-fold increases in CD107a-positive cell frequencies and 3-fold increases in their CD107a MFI, with slight positive shifts in intracellular Grz B and perforin. In contrast, T cells showed little change in CD107a externalization. Our results suggest that degranulation rates may be more affected than the levels of cytotoxic proteins, indicating a novel mechanism by which NK activity was affected by the drug. The increase in degranulation per NK cell indicates a mechanism by which Ampligen treatment can improve NK cell function.

The basis of the study is an expert survey. In order to identify the primary information and communication needs of this category of media audience, thesurvey was conducted among social workers who deal with people with disabilities in Dnipropetrovsk, Donetsk, Zaporizhia, Lviv and Khmelnitsky regions. Via online questionnaireservice «Google Forms»there were collected 66responses from January 20, 2020 tillFebruary 3, 2020. The comments which were received earlier or later the survey timelines were disregarded.Results. The results of the survey among social workers whose activities are related to people with disabilities in Dnepropetrovsk, Donetsk, Zaporizhzhya, Lviv and Khmelnitsky regions give grounds to state that for people with disabilities there are promising prospects of constructing an «elastic web» of inclusive social and communication mediaspace in a complex of contact and digital projects, especially initiated and implemented in the media sector. In particular, the experts highlighted the absolute need ofpeople with disabilities to enhance socialization by expanding their daily circle of communication.Conclusions.Within the framework of scientific discourse, the comprehension of this problem is promising in terms of the development of a model of communication relations establishment in moderninclusive poly-platform media space.

General poly patient visits in the last 3 months showed a decrease of 27%.Objective This research aims to analyze the willingness and ability to pay for General Polyclinic patients at the Pratama Medica Zahra Clinic. Research methods quantitatively and qualitatively. Data collection is by questionnaire with Google Form, interviews, observation and document review.Research result namely the respondent's ATP (Ability To Pay) of Rp. 114,000,- while most respondents chose the rate set by the clinic as the lowest category rate, namely Rp. 45,000 – Rp. 50,000,- it has been confirmed that almost all people can afford to pay up to the high category, namely IDR. 60,000 – Rp. 65,000 but chose the low category. Likewise, the WTP (Willingness To Pay) of respondents who are willing to pay is greater than those who do not. This proves that public awareness regarding the willingness and ability to spend on a health budget is quite good, where the level of public education is also in the good category, so that the public can know the importance of assumptions and obligations in having a health budget.Conclusion, ATP with determining general poly service rates does not have a relationship with a p value of 0.05, but the results of the WTP research with determining general poly service rates have a relationship with a p value of 0.03.Suggestion, it is hoped that ATP and WTP can be used as input for clinic owners in making proposals for adjustments to tariff determination based on the Bandung City PERDA, as well as the need to improve service quality elements such as doctor services, medicines and health equipment which must be maximized by adding facilities and infrastructure to support the quality of health services.

Phases in the data analysis phenomenology study read interviews, summarize coding, analyze transcripts, return to participant texts, compare and distinguish, theme integration. This study produced two themes (1) Less attention (2) Self Acceptance. BackgroundOsteoarthritis is the most common degenerative bone and joint disease. Hypothesis has been widely accepted that osteoarthritis (OA) starts from cartilage injury and loss, while emerging evidences suggest that synovial inflammation precedes cartilage loss during the progression of OA [1]. Activation of macrophages plays a crucial part in synovial inflammation in OA [2]. In our previous researches, we have demonstrated NOD2 as an inhibitor of macrophage activationin vitro. Besides, bioinformatic analysis suggested a potential link between NLRP12 and NOD2.ObjectivesTo investigate the role of NOD2 in OAin vivoand explore the potential interaction between NOD2 and NLRP12.MethodsWe established CIOA (Collagenase-induced OA) model with 8-week-old C57BL/6J mice, and injected NOD2 over-expression (oe-NOD2) lentiviral vectors into the knee joint cavity as experimental group (CIOA + oe-NOD2), with the empty vectors as control (CIOA + Mock). 8 weeks later, the knee joints were harvested and stained with Safranin O/fast green. Besides, three-dimensional reconstruction of Micro-CT images were employed to evaluate the pathological changes of OA. In addition, we applied lentiviral transfection, co-immunoprecipitation (Co-IP), and ubiquitination assays to investigate the interaction between NOD2 and NLRP12, as well as its mechanism of action in the regulation of macrophage activation.ResultsIn vivoover-expression of NOD2 showed significant inhibition of pathological changes (Figure 1A, 1B). Though NLRP12 had no impact on mRNA level of NOD2 in RAW264.7 macrophages (Figure 1C), NOD2 expression at protein level was negatively correlated with NLRP12 (Figure 1D), suggesting that NLRP12 may influence the degradation of NOD2. Co-IP experiments also confirmed the existence of interaction between NLRP12 and NOD2 at protein level, which was influence by MG132, inhibitor of ubiquitin-proteasome pathway (Figure 1E). Besides, NLRP12 over-expression impaired inhibition of macrophage inflammation by NOD2 (Figure 1F). Since HSP90 binds NOD2 to prevent its proteasomal degradation via poly-ubiquitination (Poly-Ub) [3], we therefore performed Co-IP and Poly-Ub assays. NLRP12 was associated with reduced binding of HSP90 with NOD2, greater scale of K48 Poly-Ub assembling on NOD2, and thus lower level of NOD2 protein. Furthermore, proteasomal degradation of NOD2 was blocked by MG132, though interrupted binding between NOD2 and HSP90, and accumulation of Poly-Ub chains (Figure 1G). These findings suggested sequestration of HSP90 from binding NOD2 by NOD2-NLRP12 interaction, which leaded to assembly of K48 Poly-Ub chains onto NOD2, and its proteasomal degradation (Figure 1H).ConclusionNOD2 was demonstrated as an inhibitor of OAin vivo, in consistence with our previousin vitrodata. Besides, NLRP12 interacted with NOD2, and promoted its Poly-Ub and subsequent proteasomal degradation. ObjectivesMemory impairment is an extremely common complaint in people with epilepsy (PWE). The purpose of our cross sectional study was to identify factors associated with self-reported memory impairment in PWE.Methods514 PWE and 52 controls completed the Liverpool Adverse Event Profile, a 19 item questionnaire which includes items on memory and depression and provided clinical data on age, sex, medication, seizure onset, frequency and severity. For memory impairment, sleep disturbance, depression and seizure type scores were dichotomised into minor deficits (Likert scores 1 and 2), and major deficits (Likert scores 3 and 4).ResultsIn univariate analysis we found significant associations for mono vs polytherapy (χ2=14.85, p<0.001); minor/major depression (χ2=29.41, p<0.001); minor/major sleep disturbance (χ2=65.34, p<0.001); seizure freedom/persistence (χ2=14.40, p<0.001), the number of seizures in the last four weeks (p<0.001, U=17530.50, z=−5.11, but not sex, mean age of onset, mean duration of epilepsy or seizure type. Multinomial logistic regression revealed that sleep disturbance (p<0.001), AED poly therapy (p=0.004), depression (p<0.001) all retained significance whereas seizure persistence did not (p=0.377). Applying ANOVA to examine mean age with memory deficit outcomes, older age was significantly linked to poorer memory (F=3.34, p=0.002).ConclusionsSleep disturbance, poly therapy and depression are potentially treatable factors associated with self-reported memory impairment in PWE.

Parents will face a backlash in any care and treatment such as guilt, anxiety, fear, sadness, economic problems and worry about not getting the proper care and treatment.This research is a quantitative descriptive research that aims to describe the level of anxiety of parents of children with thalassemia in Poly Children's Hospital. H. Abdul Manap Jambi City in 2015. The population in this study were all parents whose children suffer from thalassemia as many as 56 people with a total sample of 33 people. This research was conducted on the 26th of June to July 26, 2015 with interview techniques and questionnaires. Results showed that of the 33 respondents, it is as much as 26 respondents (78.8%) had moderate anxiety level, as many as six respondents (18.2%) had mild anxiety level and 1 respondent (3%) had severe anxiety level. It is expected that nurses need to pay attention to and involve parents in performing nursing care in children with thalassemia, and improve the provision of information about the treatment of children to parents, so as to reduce anxiety.Keywords: Anxiety, Parents, Thalassemia

The study design was observational using secondary data in the form of medical record records. The study population was HIV sufferers who registered to run an HIV / AIDS antiretrovial treatment program at the Cendana Poly Ngudi Waluyo Regional Hospital in Blitar in June 2017. Samples in the study were 30 people according to inclusion and exclusion criteria with a sampling technique using a purposive sampling method. Research instruments using questionnaires. Data analysis uses descriptive method. The results showed that this CD4 cell count had a CD4 cell count of 200 cells / mm 3 by 10%, 200-499 by 90% in patients who were on ARV programs. Objectives People with bipolar disorder (BD) spend more time depressed than manic/hypomanic, and depression is associated with greater impairments in psychosocial functioning and quality of life than mania/hypomania. Emerging evidence suggests psilocybin, the psychoactive compound in “magic mushrooms,” is a promising treatment for unipolar depression. Clinical trials of psilocybin therapy have excluded people with BD as a precaution against possible adverse effects (e.g., mania). Our study centered the experiences of adults living with BD who consumed psilocybin-containing mushrooms, and aimed to (1) understand its subjective impacts on BD symptoms, (2) deepen understanding of Phase I survey results, and (3) elucidate specific contextual factors associated with adverse reactions in naturalistic settings. Methods Following an international survey (Phase I), follow-up interviews were conducted with 15 respondents (Phase II) to further understand psilocybin use among adults with BD. As part of a larger mixed-methods explanatory sequential design study, reflexive thematic analysis was used to elaborate findings. Results Three major themes containing sub-themes were developed. (1) Mental Health Improvements: (1.1) decreased impact and severity of depression, (1.2) increased emotion processing, (1.3) development of new perspectives, and (1.4) greater relaxation and sleep. (2) Undesired Mental Health Impacts: (2.1) changes in sleep, (2.2) increased mania severity, (2.3) hospitalization, and (2.4) distressing sensory experiences. (3) Salient Contextual Factors for psilocybin use included: (3.1) poly-substance use and psilocybin dose, (3.2) solo versus social experiences, and (3.3) pre-psilocybin sleep deprivation. Conclusion Our findings demonstrate both benefits and risks of psilocybin use in this population. Carefully designed clinical trials focused on safety and preliminary efficacy are warranted.

With the increasing aging of the world population, the share in the population with mild Alzheimer's (m-ADs) - also referred to as type-3 diabetes - is increasing. Rough estimates indicate that by 2050 around 140 million people will suffer from this disease, especially in developed countries but with an increasing morbidity in Low- and Middle-Income countries. m-ADs can be characterized by senile plaques that consist of extracellular amyloid beta (Aβ) deposition while neurofibrillary tangles (NFT) -consisting of hyperphosphorylated tau which are located intracellular in Alzheimer Diseased brain- are a second feature. It must be emphasized that these are the consequences of the disease and not the cause. These amyloid plaques have also been observed in other organs of patients with poorly treated type 2 diabetes such as pancreas, liver, kidney and heart, so it must be emphasized that they are not characteristic of the brain alone. But what is then consequently the cause of m-ADs? The idea was first that the brain produced Insulin to a lesser extent, but a landmark study using LC-MS techniques by van Ginneken, et al. indicated that in post mortem Type Diabetes brain donors more saturated bonds occur, making the brain more rigid and the Insulin molecule less likely to reach its destination, leading to neural death. A second hypothesized cause is the formation of plaques in the fluid mosaic membrane of poly unsaturated fatty acids (PUFAs), as a result of which the Insulin molecule is less able to invade the human brain, which also leads to neural death. Finally, the role of Sphingomyelins as lipids in foods such as soy and milk should be mentioned, which can be converted via the Land's cycle in the brain to toxic Ceramide, which also could lead to neural death. In addition, in a small cohort of patients from the Dutch brain bank -where post mortem histopathological the brain disease has been assessed in addition to body characteristics- it appears that m-ADs or also Type-3 Diabetes is independent of Body Mass Index (BMI).

Age-related changes in the oral cavity are accompanied by the development of age-related pathology, such as chronic periodontitis (CP). Although apoptosis plays a certain role in its pathogenesis, this fact, however, has not been evaluated clinically, and the diagnostic information content of biomarkers of apoptosis and aging has not been determined. The aim of the study was to evaluate the content of cleaved poly-(ADP-ribose)-polymerase (cPARP) and caspase-3 (Casp3) in mixed saliva of elderly patients with age-related dental diseases and in mature patients with mild to moderate CP. The study included 69 people. The control group included 22 healthy young volunteers aged 18 to 44 years. The main group included 22 elderly patients aged 60 to 74 years. They were divided into subgroups according to clinical manifestations: occlusion (comparison group), periodontal, and dystrophic syndromes. Additionally, a group of 25 patients of mature age from 45 to 59 years old with mild to moderate CP was analyzed. The content of salivary Casp3 in patients with occlusion syndrome was lower than in healthy young people (p=0.014). In patients with the periodontal syndrome, the content of cPARP was higher than in the comparison group (p=0.031). The group with dystrophic syndrome had the highest level of Casp3 in comparison with the control group and the comparison group (p=0.012, p=0.004, respectively). There were no statistically significant differences between patients of different age groups with mild to moderate CP. Evaluation of the correlation between cPARP and Casp3 levels revealed a direct relationship in the group of elderly patients and in patients with mild CP (r=0.69, r=0.81, respectively). We assessed the effect of Casp3 levels on changes in the cPARP levels by means of a simple linear regression analysis. The cPARP level correlated with the content of Casp3 (r2=0.555). According to the results of the ROC analysis, it was found that using the cPARP indicator it would be possible to distinguish between groups of elderly patients with periodontal and occlusion syndromes (AUC=0.71), while using Casp3 it would be possible to distinguish patients with the occlusion syndrome and the control group (AUC=0.78). Since the level of Casp3 in young people is significantly higher than in the elderly patients, its decrease can be considered as a potential salivary biomarker of aging. The level of studied cPARP in the elderly has clinical value in periodontal syndrome and low age dependence.

Background:Sjogren’s syndrome is one of the most common autoimmune diseases, with a prevalence of 0.33% to 0.77% in Chinese people, characterized by focal infiltration of lymphocytes in glands and the production of multiple autoantibodies. Studies have shown that virus infection may play a crucial role in the occurrence and development of this disease.Objectives:It has been shown that airway stimulation with poly(I:C) can mimic respiratory tract viral infection to some extent. Thus, this study was aimed to investigate the dynamic immune responses in salivary gland after respiratory tract poly(I:C) stimulation in NOD mice.Methods:The 5-week-old NOD mice were given respiratory tract poly(I:C) stimulation to mimic the respiratory virus infection once every other day for a total of 5 times (the total dose is 100μg), and the control group were given the same dose of sterile PBS. After 8 weeks, the mice were sacrificed to obtain and analyze the salivary gland tissues.Results:We found that the salivary gland flow rate was decreased and the blood glucose was influenced by the Viroid stimulation during the early stage in poly(I:C) stimulated group compared with that in PBS group. Accordingly, the pathological injury of salivary gland tissues in poly(I:C) stimulated group was more serious, including decreased volumes of the salivary glands, increased number of pathological focus score and the increased area of lymphocyte infiltration. Furthermore, we found that the expression of IL-33 in salivary glands of poly(I:C) stimulated NOD mice was increased, especially the expression of IL-33 in the acini and ducts. Electoral ergonomics pertains to the interface between electoral psychology and electoral design. It moves beyond traditional models of electoral organisation that often focus on mechanical effects or changes to who actually votes to investigate the ways in which different forms of electoral organisation will switch on and off various electoral psychology buttons (in terms of personality, memory, emotions and identity) so that the very same person’s electoral experience, thinking process, and ultimately electoral behaviour will change based on the design of electoral processes. This article illustrated this phenomenon based on two case studies, one which showed that young people seemed more likely to vote for radical right parties if they voted postally than in person at the polling station based on panel study evidence from the UK, and another which showed that the time citizens deliberate about their vote varied from 1 to 3 depending on whether they were asked to vote using materialised or dematerialised mono-papers or poly-paper ballots. The article suggested that electoral ergonomics, as the interface between electoral psychology and election design, exceeded the sum of its parts.

This study aimed to analyze and describe the effect of experiential marketing, patient satisfaction, and patient loyalty at As-Syifa Husada Clinic. This research uses quantitative research. The population of this study is people who are currently or have used the services of the As-Syifa Husada Clinic, and the sample is 110 respondents who have used the services of the KIA Poly (Mother & Child Clinic) at least two times. Data collection techniques in this study used a questionnaire distributed to respondents. The analytical method used is path analysis and hypothesis testing using T-test and mediation test using the SPSS analysis tool. The results of this study indicate that: (1) Experiential marketing has a positive and significant effect on patient loyalty, (2) Experiential marketing has a positive and significant effect on patient satisfaction, (3) Patient satisfaction has a positive and significant effect on patient loyalty, (4) experiential marketing has an influence on patient loyalty through patient satisfaction. Good experiential marketing can shape patient satisfaction which will also have an impact on creating patient loyalty at As-Syifa Husada Clinic.

Since the industrial revolution era, the Earth was suffering from serious air pollution. Millions of people are now suffering from indoor air pollution related diseases, especially in the industrialized countries such as China. One method to improve the indoor air quality is by oxygen enhancement. Membrane technology has been a key research over the past decades due to its low energy usage, minimum chemical consumption as well as small setting up layout. In this study, polyetherimide (PEI) membranes coated with polydimethylsiloxane (PDMS) or poly(ether block amide) (PEBAX) at different concentration (1, 3 or 5 wt%) were used to evaluate the oxygen/nitrogen gas separation. Prior to the gas permeation study, the membranes were characterized using scanning electron microscope (SEM) for morphology observation and surface elemental analysis by energy dispersive X-ray spectroscope (EDX). The morphology of the self-fabricated PEI membranes is composed of a thin and dense structure supported by the finger-like structure. The results obtained from oxygen/nitrogen separation studies shows membrane coated with 3 wt% PDMS yield a good separation results, exhibiting an improvement of oxygen and nitrogen permeance by 28.2% and 24.9%, selectivity by 10.4% (up to 5.08) relative to the base PEI membrane. Meanwhile, the 3 wt% PEBAX-coated PEI membrane only achieved selectivity of 3.56. The PDMS-coated PEI membrane yield a better separation performance attributed to the fact that PDMS coating on the hollow fiber membrane improve the surface morphology by reducing the defects.

The results of the pre-survey in the working area of Puskemas Rebang Tangkas, Way Kanan Regency towards 10 respondents, 7 of whom prefer to seek treatment at a dentist or a dental clinic. Meanwhile, 3 of them continued to carry out dental examinations at the health center, with consideration of cheaper payments. Purpose: Therefore, this study aims to determine the enabling factors associated with the use of dental clinic at the Rebang Tangkas Community Health Center, Way Kanan Regency in 2020.Method: This type of research used in this research is quantitative. The research design used in this study was cross sectional. The population in this study were all 118 people in the work area of Puskesmas Rabang Tangkas, Way Kanan Regency. Sampling using purposive sampling.Results: Based on the results of data collection, it is known that the use of dental polyclinic services to respondents in the work area of the Rebang Tangkas Community Health Center, Way Kanan Regency in 2020 with categories that utilize dental polyclinic services as many as 44 respondents (37.3%) and those who do not utilize dental poly services are 74 respondents (62.7%). Based on the results of statistical tests, it was obtained p value 0.014, and 0.038 and or p value <0.05, which means that there is a relationship between facilities and infrastructure factors and the low utilization of dental health services at Puskesmas Rebang Tangkas Way Kanan Regency in 2020.Conclusion: There is a relationship between health facilities and infrastructure and the low utilization of dental poly services at Puskesmas Rebang Tangkas, Way Kanan district. Suggestions from researchers that good service will be obtained if facilities and infrastructure that meet the standards are available in health facilities. Here, we explored the effects of BI 409306 treatment in the polyriboinosinic-polyribocytidilic acid (poly[I:C])-based mouse model of maternal immune activation (MIA), which is relevant to schizophrenia and APS. In Study 1, adult offspring received BI 409306 0.2, 0.5, or 1 mg/kg or vehicle to establish an active dose. In Study 2, adult offspring received BI 409306 1 mg/kg and/or risperidone 0.025 mg/kg, risperidone 0.05 mg/kg, or vehicle, to evaluate BI 409306 as add-on to standard therapy for schizophrenia. In Study 3, offspring received BI 409306 1 mg/kg during adolescence only, or continually into adulthood to evaluate preventive effects of BI 409306. We found that BI 409306 significantly mitigated MIA-induced social interaction deficits and amphetamine-induced hyperlocomotion, but not prepulse inhibition impairments, in a dose-dependent manner (Study 1). Furthermore, BI 409306 1 mg/kg alone or in combination with risperidone 0.025 mg/kg significantly reversed social interaction deficits and attenuated amphetamine-induced hyperlocomotion in MIA offspring (Study 2). Finally, we revealed that BI 409306 1 mg/kg treatment restricted to adolescence prevented adult deficits in social interaction, whereas continued treatment into adulthood also significantly reduced amphetamine-induced hyperlocomotion (Study 3). Taken together, our findings suggest that symptomatic treatment with BI 409306 can restore social interaction deficits and dopaminergic dysfunctions in a MIA model of neurodevelopmental disruption, lending preclinical support to current clinical trials of BI 409306 in patients with schizophrenia. Moreover, BI 409306 given during adolescence has preventive effects on adult social interaction deficits in this model, supporting its use in people with APS.

Little is known about the psychosocial impact on people who live in polluted areas, and its consequences for the parental role have been neglected. This study addresses this gap, proposing qualitative research referring to the case of per- and poly-fluoroalkyl substances (PFAS) water pollution in the Veneto Region of Italy. The purpose of this study was to investigate the chronic exposure contamination (CEC) experience of parents. Semi-structured interviews were conducted with 32 parents living in the so-called ‘Red Area’ considered to have had maximum exposure. Grounded theory was used to analyse the data. The three themes to emerge were three phases of a dynamic process: shock around the discovery (phase 1), lifestyle change (phase 2), and living with PFAS (phase 3). The two transitions (loss of innocence and environmental adaptation) linked the phases. Our findings show that PFAS CEC experience is a process whereby parents need to move from the shock of discovery to adapting to the new situation in order to incorporate a change into their daily life, especially in regard to children. Two emerging aspects that characterised the process as a constant were uncertainty and health concerns, while role strains could be a stress source in the context of CEC. We suggest introducing the notion of chronic role strain (CRS).

In addition to being infected by Micobacterium Leprae itself, hygiene factors and initial therapy will determine the success of treatment and the prognosis of the disease which will then affect the health and quality of life of the community. In the research that has been done, it is said that leprosy in Indonesian society, especially Nusa Tenggara, is still very large, even many people do not know about the disease and come with a certain level of severity. Situation Analysis: Health workers play an important role in disseminating knowledge of leprosy (Morbus Hansen) which has begun to appear at a certain level of severity, which can affect the quality of life of sufferers. Through this educational activity, it is hoped that there will be an increase in public knowledge about early detection of leprosy which may appear with an already poor prognosis and complications that affect the success of further therapy. Approach Method: The approach steps taken in this program consist of counseling the general public regarding the early detection of leprosy (Morbus Hansen) in the Sumbawa community at the Manambae Hospital. Results and Conclusions: Of the 20 questionnaires distributed, only 3 got a score of 75 and 17 others got a score below 50. After education was done in the form of watching videos simultaneously and reassessment 15 people got a score above 50 and 5 people got a score below 50 Visitors who are skin and sex polyclinic and general poly at Manambae Hospital have very little knowledge about Morbus Hansen, this can be seen from the results of filling out questionnaires before being given education. There is a very significant comparison between scores before being given education and after being given education. In medical principles prevention is better than cure, so it is important to conduct education with the aim of preventing disease.

Abstract Background Rapid frailty screening remains problematic in primary care. The diagnostic test accuracy (DTA) of several screening instruments has not been sufficiently established. We evaluated the DTA of several screening instruments against two reference standards: Fried’s Frailty Phenotype [FP] and the Adelaide Frailty Index [AFI]), a self-reported questionnaire. Methods DTA study within three general practices in South Australia. We randomly recruited 243 general practice patients aged 75+ years. Eligible participants were 75+ years, proficient in English and community-dwelling. We excluded those who were receiving palliative care, hospitalized or living in a residential care facility. We calculated sensitivity, specificity, predictive values, likelihood ratios, Youden Index and area under the curve (AUC) for: Edmonton Frail Scale [EFS], FRAIL Scale Questionnaire [FQ], Gait Speed Test [GST], Groningen Frailty Indicator [GFI], Kihon Checklist [KC], Polypharmacy [POLY], PRISMA-7 [P7], Reported Edmonton Frail Scale [REFS], Self-Rated Health [SRH] and Timed Up and Go [TUG]) against FP [3+ criteria] and AFI [&gt;0.21]. Results We obtained valid data for 228 participants, with missing scores for index tests multiply imputed. Frailty prevalence was 17.5% frail, 56.6% prefrail [FP], and 48.7% frail, 29.0% prefrail [AFI]. Of the index tests KC (Se: 85.0% [70.2–94.3]; Sp: 73.4% [66.5–79.6]) and REFS (Se: 87.5% [73.2–95.8]; Sp: 75.5% [68.8–81.5]), both against FP, showed sufficient diagnostic accuracy according to our prespecified criteria. Conclusions Two screening instruments—the KC and REFS, show the most promise for wider implementation within general practice, enabling a personalized approach to care for older people with frailty.

Abstract Selective serotonin reuptake inhibitors (SSRIs) are the treatment of choice for clinical depression and a range of anxiety-related disorders. They are well tolerated over extended periods with more than 50 million people worldwide benefiting from their use. Here we show that 3 structurally distinct SSRIs—fluoxetine, paroxetine, and citalopram—act directly on Burkitt lymphoma (BL) cells to trigger rapid and extensive programmed cell death. SSRIs unexpectedly stimulated calcium flux, tyrosine phosphorylation, and down-regulation of the c-myc and nm23 genes in Burkitt lymphoma cells remaining faithful to the biopsy phenotype. Resultant SSRI-induced apoptosis was preceded by caspase activation, poly(ADP-ribose) polymerase-1 (PARP-1) cleavage, DNA fragmentation, a loss of mitochondrial membrane potential, and the externalization of phosphatidylserine, and reversed by the overexpression of bcl-2. Normal peripheral blood mononuclear cells and tonsil B cells, whether resting or stimulated into cycle, were largely resistant to SSRI-induced death as were 5 non-BL lymphoid cell lines tested. We discuss these findings within the context of whether the SSRI class of antidepressants could find future application as potential therapeutics for the highly aggressive and—because of its association with AIDS—increasingly more common Burkitt lymphoma.

Oral squamous cell carcinoma (OSCC) affects tens of thousands of people worldwide. Despite advances in cancer treatment, the 5-year survival rate of patients with late-stage OSCC is low at 50–60%. Therefore, the development of anti-OSCC therapy is necessary. We evaluated the effects of marine-derived triterpene stellettin B in human OC2 and SCC4 cells. Stellettin B dose-dependently decreased the viability of both cell lines, with a significant reduction in OC2 cells at ≥0.1 µM at 24 and 48 h, and in SCC4 cells at ≥1 µM at 24 and 48 h. Terminal deoxynucleotidyl transferase dUTP nick-end labeling (TUNEL)-positive cells were significantly observed at 20 µM of stellettin B at 48 h, with the overexpression of cleaved caspase3 and cleaved poly(ADP-ribose) polymerase (PARP). Moreover, mitochondrial respiratory functions were ablated by stellettin B. Autophagy-related LC3-II/LC3-I ratio and Beclin-1 proteins were increased, whereas p62 was decreased. At 20 µM at 48 h, the expression levels of the endoplasmic reticulum (ER) stress biomarkers calnexin and BiP/GRP78 were significantly increased and mitogen-activated protein kinase (MAPK) signaling pathways were activated. Further investigation using the autophagy inhibitor 3-methyladenine (3-MA) demonstrated that it alleviated stellettin B-induced cell death and autophagy. Overall, our findings show that stellettin B induces the ER stress, mitochondrial stress, apoptosis, and autophagy, causing cell death of OSCC cells.

The present work aimed to design and develop a gellan gum (GG)-based composite system for the local delivery of the neuroprotective sigma-1 receptor agonist, 1-[3-(1,1′-biphen)-4-yl] butylpiperidine (RC-33), as a potential tool for the treatment of tissue nervous injuries. The system, consisting of cross-linked electrospun nanofibers embedded in a RC-33-loaded freeze-dried matrix, was designed to bridge the lesion gap, control drug delivery and enhance axonal regrowth. The gradual matrix degradation should ensure the progressive interaction between the inner fibrous mat and the surrounding cellular environment. Nanofibers, prepared by electrospinning polymeric solutions containing GG, two different grades of poly (ethylene oxide) and poloxamer, were cross-linked with calcium ions. GG-based matrices, loaded with different amounts of RC-33, were prepared by freeze-drying. Dialysis studies and solid-state characterization pointed out the formation of an interaction product between GG and RC-33. RC-33-loaded freeze-dried matrices were characterized by the capability to absorb a high buffer content, forming a gel with marked viscoelastic properties, and by RC-33 controlled release properties. The presence of cross-linked nanofibers increased matrix mechanical resistance.

Articular cartilage (AC) degradation is a recurrent pathology that affects millions of people worldwide. Polyvinyl alcohol (PVA) hydrogels have been widely explored for AC replacement. However, their mechanical performance is generally inadequate, and these materials need to be reinforced. Moreover, to be used in a clinical setting, such materials must undergo effective sterilisation. In this work, a PVA hydrogel reinforced with poly(p-phenylene-2,6-benzobisoxazole) (PBO) nanofibres was submitted to three non-conventional sterilisation methods: microwave (MW), high hydrostatic pressure (HHP), and plasma (PM), in order to evaluate their impact on the properties of the material. Sterilisation was achieved in all cases. Properties such as water content and hydrophilicity were not affected. FTIR analysis indicated some changes in crystallinity and/or crosslinking in all cases. MW was revealed to be the most suitable method, since, unlike to PM and HHP, it led to a general improvement of the materials’ properties: increasing the hardness, stiffness (both in tensile and compression), and shear modulus, and also leading to a decrease in the coefficient of friction against porcine cartilage. Furthermore, the samples remained non-irritant and non-cytotoxic. Moreover, this method allows terminal sterilisation in a short time (3 min) and using accessible equipment.

Inclusion of HIV protease inhibitors (PIs) in the treatment of people living with HIV+ has markedly decreased mortality but also increased the incidence of metabolic abnormalities, causes of which are not well understood. Here, we report that insulinopenia is exacerbated when Zucker fa/fa rats are exposed to a PI for 7 wk, suggesting that chronic PI exposure adversely affects pancreatic islet β-cell function. In support of this possibility, we find increased apoptosis, as reflected by TUNEL fluorescence analyses, and reduced insulin-secretory capacity in insulinoma cells and human pancreatic islet cells after in vitro exposures (48–96 h) to clinically relevant PIs (ritonavir, lopinavir, atazanavir, or tipranavir). Furthermore, pancreatic islets isolated from rats administered an HIV-PI for 3 wk exhibit greater cell death than islets isolated from vehicle-administered rats. The higher incidence of HIV-PI-induced cell death was associated with cleavage and, hence, activation of caspase-3 and poly(ADP)-ribose polymerase but not with activation of phospho-pancreatic endoplasmic reticulum (ER) kinase or induction of ER stress apoptotic factor C/EBP homologous protein. Exposure to the HIV-PIs, however, led to activation of mitochondria-associated caspase-9, caused a loss in mitochondrial membrane potential, and promoted the release of cytochrome c, suggesting that HIV-PIs currently in clinically use can induce β-cell apoptosis by activating the mitochondrial apoptotic pathway. These findings therefore highlight the importance of considering β-cell viability and function when assessing loss of glycemic control and the course of development of diabetes in HIV+ subjects receiving a protease inhibitor.

Diabetes mellitus (D.M.) is a metabolic disease that has affected over 500 million people globally. Bioactive compounds such as β-carotene and Quercetin have gained research interest for their potential antidiabetic properties, and bioactives have reported superior combinatorial effects in several ailments, including D.M. However, poor oral bioavailability has limited their potential application. Thus, the present study was focused on developing ultrasonically fabricated β-Carotene nanoemulsion (βC-NE) by employing capmul as the oil phase, Gelucire 44/14 as surfactant and Acconon MCM C8 as co-surfactant. The 3 factor- 3 level Box-Behnken design (BBD) was applied to optimise the βC-NE and study the impact of selected independent variables such as % Smix (5 to 9%), amplitude (20–30%) and sonication time (2.5–7.5 min) on responses including globule size (G.S.), poly dispersibility Index (PDI) and entrapment efficiency (E.E.). Further, the combinatorial effect of βC-NE with Quercetin Nanoemulsion (QU-NE) in the streptozotocin-induced diabetic rat model was evaluated. The results exhibited that 7% Smix at 25% amplitude for 5 min produced βC-NE with a droplet size of 153.1 ± 12.25 nm, 0.200 ± 0.04 PDI, and 73.25 ± 3.25% E.E. The βC-NE showed superior in-vivo bioavailability by 5.38 folds. The βC-NE, combined with QU-NE, exhibited potential therapeutic benefits in controlling body weight, blood sugar level, lipid levels, and tissue damage markers. Additionally, the pancreatic cells and hepatic cells were well protected. These results demonstrate the potential benefits of βC-NE and QU-NE in combination and recommend them as a substitute strategy for diabetes.

The ineffectiveness of the referral system in Indonesia has an impact on the accumulation of patients in advanced health facilities, resulting in a decline in the quality of health services provided. The main indicator of knowing hospital standards is patient satisfaction with services from the hospital. Various facts show that there are serious problems in the quality of health services in Indonesia. This is due to the absence of the best quality control system that can be applied. To determine the level of satisfaction of National Health Insurance (JKN) patients with the quality of health services provided by dental clinic officers at Panembahan Senopati Hospital. Descriptive analytic observational with a cross sectional study design. The population in this study were patients who received dental and oral health services. Samples were taken using 86 accidental sampling techniques. Data was collected using a questionnaire consisting of 14 structured question items based on the performance and attitude of dental clinicians. Data analysis using the Chi-Square test followed by logistic regression test. From the results of multivariate analysis, the P value of 0.035 performance is significant and has a greater chance of 10,588 times compared to performance that is not appropriate. P value of 0.023 attitude is significant and has a 4.315 times the opportunity compared to attitudes that do not include 3 components. Factors related to satisfaction include intelligence, skills. emotional stability, a person's characteristics include attitudes, personal traits, physical traits, desires and motivation. There is a relationship between the performance and attitude of dental clinicians that performance is a variable X which is more influential with a value of P = 0.035 compared with attitude with a value of P = 0.023 to the variable Y about patient satisfaction of dental dentists.

The intestinal protozoan parasite, Giardia duodenalis, infects a large number of people in the world annually. Giardia infection has been considered a negative effect on intestinal epithelial cell growth, while the underlying mechanisms remain to be explored. Here we evaluated reactive oxygen species (ROS) production and apoptotic events in Giardia trophozoites-stimulated Caco-2 cells via fluorescence microscopy, transmission electron microscopy, flow cytometry, western blot, and cell counting kit-8 analyses. The results showed that Giardia trophozoite treatment could induce lactate dehydrogenase release and Caco-2 cell apoptosis. The ROS levels were increased post treatment. The observed typical characteristics of mitochondria damage include significant swelling and degeneration of matrix and cristae. After trophozoite treatment, the level of Bax protein expression was increased, while Bcl-2 protein decreased. Trophozoite stimulation also led to reduction of mitochondrial membrane potential and release of cytochrome c from the mitochondria to the cytoplasm, and this process was accompanied by activation of caspase-9 and caspase-3 and poly (ADP-ribose) polymerase 1 cleavage. Pretreatment with N-acetyl-L-cysteine, a ROS inhibitor, reversed G. duodenalis-induced Caco-2 cell apoptosis. Taken together, we indicated that G. duodenalis could induce Caco-2 cell apoptosis through a ROS- and mitochondria-mediated caspase-dependent pathway. This study furthers our understanding of the cellular mechanism of the interaction between Giardia trophozoites and host cells.

The aim of the study was to study the distribution of the allele frequency of the rs10830963 MTNR1B polymorphic marker gene in healthy donors, as well as to search for a relationship between variants of this gene and heart rate parameters in healthy donors with different types of exercise. Materials and methods. Study design – cross-sectional, non-randomized. The study included 80 healthy donors with various types of physical activity, as well as a control group made up of men leading a sedentary lifestyle. Venous blood samples from donors were used for DNA isolation. The polymorphism of the polymorphic marker rs10830963 of the MTNR1B type melatonin receptor gene was studied by PCR. Heart rate parameters were assessed using the Poly-Spectrum-8 / E software and hardware complex (Neurosoft, Russia) according to the generally accepted method. Results. Significantly low values of muscle mass in the control group are shown in comparison with donors of strength specialization. No associations were found between the polymorphic marker rs10830963 of the MTNR 1B gene and deviations from normal body weight and BMI, as well as parameters of heart rate variability in the studied donors. Conclusion. The rs10830963 polymorphic marker of the MTNR 1B gene is not associated with BMI and heart rate parameters of healthy donors. This sample of examined patients (less than 100 people) does not allow to reliably estimate the parameters of the heart rate in donors with a certain type of physical activity and carriage of a certain allele rs10830963 of the MTNR1B gene.

Abstract:
The present study was carried out to explore rooting performance of Punica granatum (Dalim) in Bangladesh through clonal propagation by stem cutting under 3 different doses of rooting hormone IBA (Indole Buetairic Acid) and planted in the perforated plastic tray filled with coarse sand and gravel placed in the non-mist propagator. The experiment was laid out following a Randomized Complete Block Design (RCBD) with 4 treatments and 4 replications (blocks). The treatments were T0= control, T1 = 0.2% IBA, T2 = 0.4% IBA, T3 = 0.8% IBA. The rooting ability of cuttings was significantly influenced by the application of IBA. The results showed that the highest rooting percentage (70%) and longest root length (11.75 cm) of P. granatum stem cuttings were obtained from the cuttings treated with 0.2% IBA followed by 0.4% IBA (47.5% and 7.95 cm respectively) whereas the highest root number (32) and maximum root diameter (2.7 mm) were recorded with 0.4% IBA followed by 0.8% IBA (28 and 2.47 mm respectively). Survival percentage of the cutlings (the rooted cuttings) after 3 months of transferring them into poly bags was also significantly enhanced by exogenous rooting hormone (IBA) application. The highest survival percentage (73.5%) was observed with 0.4% IBA treatment followed by 0.8% IBA (68.5%). Findings of the present study reveal that P. granatum is highly amenable for clonal propagation by stem cuttings using low-cost non-mist propagator and 0.4% IBA treatment may be recommended for mass production of quality planting stocks for the cultivation of the species in homestead agroforestry or in fruit orchards. These outcomes can assist to provide edible fruit to poor rural people of natural disaster vulnerable Bangladesh.
