Abstract:
This study aimed to determine whether associations between the perceived environment and physical activity are moderated by urban-rural status among midolder aged adults. Environmental (safety, aesthetics, physical activity environment) and physical activity (total, leisure, transport) data from 3,888 adults (55 to 65 years) from urban and rural areas of Victoria, Australia, were analyzed. Multinomial logistic regression examined interactions between urban-rural status and environments in associations with physical activity. Significant (P < .05) interactions were evident and indicated positive associations only among older rural adults for both safety and aesthetics with total and transport physical activity (e.g., rural adults reporting higher safety were 91% to 118% more likely to have higher activity than rural adults reporting low safety). In contrast, the physical activity environment was positively associated with leisure activity among only urban adults. Findings suggest that some tailoring of physical activity promotion strategies targeting the environment may be required for urban and rural midolder aged adults.

Ao longo do trabalho apresentamos algumas reflexões em torno da pergunta da personagem Victoria Soller. Teoricamente nos baseamos em Carvalhal (2004) com a obra Literatura Comparada, e em Bakhtin Dialogismo e Polifonia (2016). Notamos através das relações intertextuais que o conto “Encontros na Península” dialoga com alguns textos machadianos. Palavras-chave: Machado de Assis; Milton Hatoum; Literatura comparada. Abstract: In this article we aim to present a comparative reading of the short story “Encontros na Península”, by Milton Hatoum with “A causa secreta”, by Machado de Assis. Resumo: O presente estudo objetivou analisar as respostas fisiológicas, físicas, técnicas e táticas de jovens futebolistas em função do resultado em jogos reduzidos. A amostra foi constituída por 39 jogadores (idade 15,9 ± 1,0 anos) agrupados pelo desfecho dos jogos, vitória (17), empate (6) e derrota (16). As respostas físicas e fisiológicas foram quantificadas por meio da utilização de um GPS portátil e um monitor cardíaco. A percepção subjetiva de esforço foi medida utilizando a escala de BORG 6-20. As respostas técnico-táticas foram avaliadas conforme a frequência e qualidade de execução de habilidades técnicas e princípios táticos fundamentais. Na análise foram empregados os testes de Shapiro-Wilk, Kruskal-Wallis, U de Mann Whitney e a Regressão Logística Multinomial. A significância foi estabelecida em 5%. Jogadores que conquistaram a vitória ou o empate percorreram maiores distâncias em corrida de baixa-intensidade e menores distâncias em caminhada. O grupo empate realizou mais passes do que o grupo derrota (14,0 vs 10,0). O grupo vencedor apresentou maior eficiência de execução em três princípios ofensivos e três princípios defensivos em comparação com o grupo perdedor. Verificou-se que o aumento na eficiência de execução de princípios ofensivos e defensivos aumenta as chances de os jogadores estarem nos grupos empate (11 a 24%) ou vitória (10% a 21%) com relação a derrota. Conclui-se que as características fisiológicas, físicas, técnicas e táticas são fatores que exercem influência sobre o êxito competitivo, sendo que o aumento na eficiência tática resulta em mais chances para conquista da vitória ou empate. Abstract: The present study aimed to analyze the physiological, physical, technical, and tactical responses of young soccer players according to the result in small-sided games. The sample consisted of 39 players (15.9 ± 1.0 years of age) grouped by the outcome of the games, winning (17), drawing (6), and losing (16). Physical and physiological responses were quantified using a portable GPS and a cardiac monitor. The rating of perceived exertion was measured using the BORG 6-20 scale. Technical-tactical responses were assessed according to the frequency and quality of execution of technical skills and core tactical principles. Shapiro-Wilk, Kruskal-Wallis, Mann Whitney U and Multinomial Logistic Regression tests were used in the analysis. Significance was set at 5%. Players in the groups “winning” and “drawing” covered greater distances in low-intensity running and shorter distances in walking. The “drawing” group executed more passes than the “losing” group (14.0 vs 10.0). The “winning” group showed greater efficiency of execution in three offensive and three defensive principles compared to the “losing” group. It was found that the increase in the efficiency of execution of offensive and defensive principles increases the chances of the players being in “drawing” (11 to 24%) or “winning” (10% to 21%) groups in relation to “losing”. It is concluded that the physiological, physical, technical, and tactical responses influence competitive success, and the increase in tactical efficiency results in more chances to achieve winning or drawing. Resumen: El presente estudio tuvo como objetivo analizar las respuestas fisiológicas, físicas, técnicas y tácticas de los futbolistas jóvenes dependiendo del resultado en juegos reducidos. La muestra consistió en 39 jugadores (edad 15.9 ± 1.0 años) agrupados por el resultado de los juegos, victoria (17), empate (6) y derrota (16). Las respuestas físicas y fisiológicas se cuantificaron utilizando un GPS portátil y un monitor cardíaco. La percepción subjetiva del esfuerzo se midió utilizando la escala BORG 6-20. Las respuestas técnico-tácticas se evaluaron de acuerdo con la frecuencia y calidad de ejecución de las habilidades técnicas y los principios tácticos fundamentales. Shapiro-Wilk, Kruskal-Wallis, Mann Whitney U y pruebas de regresión logística multinomial se utilizaron en el análisis. La significancia se estableció en 5%. Los jugadores que han ganado la victoria o el empate han cubierto distancias más grandes en carreras de baja intensidad y distancias más cortas al caminar. El grupo de empate hizo más pases que el grupo de derrota (14.0 vs 10.0). El grupo victoria mostró una mayor eficiencia de ejecución en tres principios ofensivos y tres defensivos en comparación con el grupo derrota. Se encontró que el aumento en la eficiencia de la ejecución de los principios ofensivos y defensivos aumenta las posibilidades de que los jugadores estén en el grupo empate (11 a 24%) o victoria (10% a 21%) en relación con la derrota. Abstract:
El objetivo de este estudio fue determinar cuáles de las acciones finales de juego discriminan la victoria y derrota en categorías de formación de voleibol femenino en los sets cerrados resueltos por reglamento con una ventaja mínima de dos puntos. Se analizaron un total de 57 sets en categoría infantil y 69 sets en categoría cadete de los Campeonatos de España de Voleibol de Valladolid 2008 y Huelva 2009 en edad escolar (12-16 años). Los resultados mostraron diferencias entre la condición de ganador y perdedor del set en el ataque positivo en categoría infantil y en el bloqueo positivo y error en categoría cadete. Por su parte, los resultados del análisis discriminante, prueba estadística que determina aquellas acciones de juego más significativas a la hora de establecer diferencias entre los equipos ganadores y perdedores, mostraron como el ataque positivo, servicio negativo y error en categoría infantil y el bloqueo positivo y error en cadete predice la victoria y derrota de set. Estos sugieren que en momentos decisivos de set y en ambas categorías, tanto el control de los gestos técnicos como el error resultante de una infracción reglamentaria, predicen su rendimiento final. Palabra clave: Voleibol femenino, análisis notacional, acción de juego, edad escolar.Abstract: The aim of this study was to establish which final game actions discriminate final victories and defeats in women volleyball formative stages during ended sets, which are solved by regulation with a minimal advantage of two points. A total of 57 sets were analyzed in infantile category and 69 sets in cadet category during the national volleyball championship at school age (12-16), Valladolid 2008 and Huelva 2009. Statistics analysis shows differences between the condition of both the winner and the loser teams in sets in the positive attack in infantile category and in positive block and error in cadet category. Discriminative analysis, statistical test that determines those most significant game actions when establishing the differences existing between the winner and loser teams, shows how to predict victory and defeat in the ended sets regarding the positive attack, negative service and error in infantile category and positive block and error in cadet category. These ones suggest that during decisive set moments in both categories, either technical gestures control or errors which arise from a regulation infraction may predict their final performance.Key words: Female volleyball, notational analysis, play action, scholar age.

Abstract:
El objetivo de este estudio fue analizar la secuencia de los dos últimos golpeos del punto en pádel, y clasificarlos a través de árbol decisional multivariante. Se analizaron un total de 2110 acciones de juego, siendo las variables analizadas: golpeo, zona de la pista, eficacia, dirección, resultado y lado de juego. Los partidos fueron analizados mediante observación sistemática. Los resultados mostraron que mantener posiciones cercanas a la red aumenta las probabilidades de victoria, observándose que las secuencias de finalización más frecuentes son las de fondo-volea y globo-remate. Las parejas ganadoras realizan mayor número de puntos ganadores en la red y menos errores en el fondo de la pista. Además, utilizar trayectorias cruzadas en el penúltimo golpe aumentará las posibilidades de un error posterior de los rivales. Estos datos tienen aplicación práctica en el entrenamiento perceptivo y decisional del jugador de pádel, y la aplicación de feedbacks por parte del entrenador.

Esta investigación analiza en el entorno de una competición deportiva el impulso en la motivación que para los equipos tiene el obtener una segunda oportunidad de alcanzar el objetivo deseado. Para ello, utilizamos la liga profesional de baloncesto estadounidense (NBA), donde dos equipos compiten por la victoria en cada partido. A través del análisis de 390 contiendas que acabaron en empate en su tiempo reglamentario, y por ende dirimieron el resultado final en la prórroga, mostramos que el equipo que consigue empatar el partido en la última jugada del mismo tiene mayor probabilidad de ganar en la prórroga si actúa como visitante pero no si actúa como local. Anotar primero en la prórroga, asimismo, es un factor que incrementa la probabilidad de victoria tanto para el equipo local como para el visitante.Abstract. This research analyzes the boost in motivation that teams experience when getting a second chance to achieve the desired objective within competition settings. To achieve this aim we analyzed the NBA framework, where two teams compete for victory in each game. Through the analysis of 390 matches that ended in a draw, and therefore the final result was reached in the overtime, we show how teams tying a game on the final play of the match are more likely to win in the overtime when playing away, but not at home. Being the first team to score in the overtime increases the probability of winning the game for both home and away situations.

El propósito del estudio fue analizar las etapas de cambio ante la práctica de actividad física en el tiempo libre en estudiantes de educación secundaria de la Ciudad de Victoria de Durango, México. El marco teórico empleado fue el Modelo Transteórico. Se diseñó un estudio cuantitativo, observacional-descriptivo de corte transversal. La muestra fue 1112 estudiantes de secundaria, edades entre los 12 y 15 años (M= 13.52; ±.92); el 52.6% fueron chicas y 47.4% chicos; 37.2% de primer curso, 33.9% segundo, 28.9% de tercero; 69% de turno matutino y 31% vespertino; el 25.5% inscritos en de secundarias Generales Federales, 36.6% Técnicas Federales y 37.9% Estatales; 15.5% en secundarias con índice de marginación urbana alto, 29% medio, 12.2% bajo y 43.4% muy bajo. Se administró el cuestionario de etapas de cambio hacia la práctica del ejercicio físico. El análisis de datos fue realizado por medio de estadística descriptiva y prueba de Chi2. Los principales resultados advierten que la mayoría de los estudiantes(60.4%) están en etapas de cambio poco activas o inactivas (no ofrecen beneficios a la salud); los chicos son más activos (51.7%)que las chicas (29.1%); los estudiantes más activos son los de secundarias Federales Generales así como los de primer curso; cuando el índice de Marginación Urbana bajael porcentaje de estudiantes en etapas más activas o con beneficios para la salud aumenta. Concluyendo que es importante considerar las etapas de cambio de comportamiento para planificar estrategias de modelos educativos. The purpose of the study was to analyze the change stages to practice of physical activity within free time in high school students of the City of Victoria of Durango, México. The theoretical framework used was the trans-theoretical model. It was designed a quantitative, observational-descriptive cross-sectional study. The sample was 1112 high school students, with ages between 12 and 15 years (M = 13.52; ± .92), (52.6% girls and 47.4% boys, 37.2% first, 33.9% second and 28.9% of third deggre 69% morning shift and 31% evening shift, 25.5% Federal General High Schools, 36.6% Federal Techniques and 37.9% Local, 15.5% high urban marginalization index, 29% medium level, 12.2% low and 43.4% very low. The questionnaire of stages of change towards the practice of physical exercise was administered. The data analysis was performed by means of descriptive statistics and proof of Chi2. The main results the majority of students (60.4%) are established in non-active or inactive stages of change that do not offer any health benefits; boys are more active (51.7%) than girls (29.1%); the students of the Federal General High Schools are the most active, as well as those in the first year; In relation to the IMU, as it improves, the percentages of students increases with more active stages or with health benefits. Concluding It is important to consider the stages of behavior change to plan strategies for educational models.

This study aimed to demonstrate that routinely collected hospital data can be used to evaluate population-level programme effectiveness.MethodA controlled ecological design was used to evaluate the effect of FootyFirst, an exercise-training programme, on the number of hospital-treated lower limb injuries sustained by males aged 16–50 years while participating in community-level Australian Football. FootyFirst was implemented with ‘support’ (FootyFirst+S) or ‘without support’ (FootyFirst+NS) in different geographic regions of Victoria, Australia: 22 clubs in region 1: FootyFirst+S in 2012/2013; 25 clubs in region 2: FootyFirst+NS in 2012/2013; 31 clubs region 3: control in 2012, FootyFirst+S in 2013. Interrupted time-series analysis compared injury counts across regions and against trends in the rest of Victoria.ResultsAfter 1 year of FootyFirst+S, there was a non-statistically significant decline in the number of lower limb injuries in region 1 (2012) and region 3 (2013); this was not maintained after 2 years in region 1. Compared with before FootyFirst in 2006–2011, injury count changes at the end of 2013 were: region 1: 20.0% reduction (after 2 years support); region 2: 21.5% increase (after 2 years without support); region 3: 21.8% increase (after first year no programme, second year programme with support); rest of Victoria: 12.6% increase.ConclusionEcological analyses using routinely collected hospital data show promise as the basis of population-level programme evaluation. The implementation and sustainability of sports injury prevention programmes at the population-level remains challenging.

In Armenia children with disabilities are often stigmatized and separated from the community. Some are sent to institutions or orphanages, some are hidden in the home, and others are hastily placed into schools and ignored in classrooms. However, Armenia is moving towards a more inclusive society. Victoria Armineh Babikian volunteered with Children of Armenia Charitable Fund (COAF) in 2017; she was their first ever Occupational Therapist. She introduced a different perspective- providing holistic care by addressing self-care needs and enhancing physical, cognitive, and sensory skills for children with intellectual and developmental disabilities. Upon completion of her volunteer experience, Babikian needed a sustainable approach to continue increasing knowledge on the care of children with disabilities. Therefore, she compiled the Occupational Therapy Guidebook to Inclusion, an 80-page manual that provides clear information about common diagnoses, simple exercises, and functional activities using homemade tools to enhance strength, coordination, cognitive processing, sensory regulation, and behavior among children with disabilities. This guidebook will be published and distributed throughout Armenia. The purpose of the Occupational Therapy Guidebook to Inclusion is to enhance disability resources for every region of Armenia, especially within rural communities.

This paper examines whether involvement in an observational study may prompt participants to change their exercise behaviors. Data were collected from 394 older community dwellers in Victoria, Australia using a baseline survey, and 245 of these participated in a follow-up survey one year later. Survey domains were drawn from constructs of relevant health behavior models. Results showed that the proportion of respondents who were currently participating in exercises to prevent falls at follow-up was 12% higher than at baseline (Wilcoxon p value < .001). Twenty-nine percent reported they had changed their perceptions about falls and their risk of falls, with comments focused on threat appraisal. Forty-four percent reported having taken strategies to reduce their risk of falling, with comments based on implementation of different preventive strategies. Respondents who held favorable views toward exercises for the prevention of falls appear to change their behaviors that might address falls when participating in observational studies.

243 Background: Androgen deprivation therapy (ADT) is commonly prescribed for advanced prostate cancer (PCa) patients, but ADT has many side effects that can impair patients’ quality of life. In various Canadian cities, we are running an educational program to help PCa patients and their partners deal with the side effects of ADT, and maintain a strong relationship with each other while on ADT. Methods: Patients, who are about to start or have been on ADT, and their partners are invited to attend an educational program. Each patient is given a copy of the book Androgen Deprivation Therapy: An essential guide for men with prostate cancer and their partners (Wassersug et al., 2014) and attends a 1.5 hour educational class, where they learn strategies for managing ADT side effects and how to effectively make healthier lifestyle changes using goal-setting exercises. At the end of the class, participants are given the option to attend a monthly follow-up session. To evaluate the effectiveness of the program, participants are asked to complete questionnaire package before attending the class and again 2-3 months later. Results: As of August 2015, 179 patients and 113 partners have attended the ADT Educational Program at Victoria, Vancouver, and Calgary. About 40% of patients returned for the follow-up session. 62 attendees participated in the research evaluation portion of the program. Uniquely designed for this study, the questionnaire package assesses ADT side effect frequency, bother associated with side effects, use of management strategies, and self-efficacy regarding side effect management. An assessment of physical activity and relationship adjustment, and feedback about the class are also included. Conclusions: Patients and partners appreciate being informed about ADT side effects managements and how to make healthier lifestyle changes while on ADT. It remains to be seen how effective the program is in limiting the bother from ADT side effects and helping couples maintain a strong dyadic relationship in the fact of the challenges brought on by ADT. Good enrollment in the in-person program in the 3 cities has propelled the development of the program in Halifax and Toronto starting in fall 2015, and an online version to be available in early 2016.

PurposeTo identify patterns of health care use in truck drivers with work-related injury or illness and to identify demographic, occupation, injury/condition, claim and geographic factors associated with patterns of care.Method13 371 accepted workers compensation claims from truck drivers lodged between 2004 and 2013 in the state of Victoria were included. Episodes of health care were categorised according to practitioner type as General Practitioner (GP), Specialist Physician, Mental Health, Surgery, Return to Work, or Physical Therapy. Latent class analysis was used to identify and characterise the distinct profiles of users with different patterns of health service use. Multinomial logistic regression was used to examine the associations between latent class and predictors including demographic, claim and injury-related factors.ResultsFour profiles of heath service use were identified: (1) Low Service Users (55% of the sample) were more likely to be younger, have an injury that did not result in time off work and have conditions other than a musculoskeletal injury; (2) High Service Users (10%) tended to be those who were aged between 45 and 64 years, lived in major cities and had musculoskeletal conditions that resulted in time off work; (3) Physical Therapy Users (25%) were more likely to be aged between 45 and 64 years, live in major cities and have non-traumatic injuries that resulted in time off work; and (4) GP/Mental Health Users (10%) were more likely to be over 24 years of age, from the lowest socio-economic band, be employed by smaller organisations and be claiming benefits for a mental health condition.ConclusionsIt is possible to identify distinct patterns of health care use following work-related injury and disease using workers’ compensation claims data. Nature of injury/disease, sociodemographic characteristics and geographic proximity to health services affect patterns of care.

Teaching basic swimming, water safety and rescue skills is recommended by the World Health Organization for all school-aged children. However, there is a lack of evidence on effective pedagogies to develop swimming competency and the success of swimming lessons as a drowning prevention intervention. This study used a self-report questionnaire and practical testing procedures to examine the effectiveness of a 10-week student-centred aquatic programme designed for children aged 10–12 years. The study also determined whether the non-traditional swimming programme was accepted by swim teachers, school teachers and principals, and parents from a range of schools from different geographical regions in Victoria, Australia. A total of 204 students were enrolled in the programme. The pre-programme results indicated a good level of swimming, water safety and aquatic knowledge, but low swimming ability. Swimming ability significantly improved from pre-programme to post-programme, with no significant post-programme ability differences between male and female children or for participants from different programmes. Qualitative feedback collected through questionnaires, interviews and/or focus groups from students ( n = 73) and parents ( n = 69), school teachers and principals ( n = 14), swim teachers and swim school managers ( n = 21) indicated strong support from principals and swim teachers for the student-centred pedagogy, and all stakeholders valued the focus on survival swimming competencies. This research highlights the importance of including stakeholders when designing and implementing aquatics programmes. The study has resulted in a well-founded, effective programme with tailored resources and instructional materials that are available for swim centres and schools that would enable schools globally to adopt and implement this programme.

In addition, it prevents cardiovascular diseases, diabetes and obesity. The objective of this work is to analyze the effect of a program of physical exercise and habits of healthy life on fatigue, functionality and body composition in breast cancer patients of the Medical Oncology Unit of the Hospital Virgen de la Victoria, Malaga. Methods: Prospective study on 97 patients with breast cancer after completing locoregional treatment and systemic chemotherapy. Treatment with hormone therapy and trastuzumab was allowed. All patients were without disease, without cardiovascular history that contraindicated the program and have signed the relevant informed consent. The duration of the program was 3 months and the parameter measurement was performed before and after. The main objective was analyze cancer-related fatigue and secondary functional parameters, anthropometric and adherence to Mediterranean diet. Fatigue, parameters functional and adherence to Mediterranean diet were evaluated by tests specific and anthropometric by different measurements including use of Tanita TBF-300A. Results: 97 patients were recruited, with a median age of 52 years (32 and 69), 60% had received neo/adjuvant chemotherapy, 84% hormonal therapy and 12% trastuzumab. Most patients underwent conservative surgery. (76%). After 3 months of intervention, women improved significantly cancer-related fatigue (p = 0.000), levels of diet adherence Mediterranean (p = 0.000), as well as functional parameters. One was observed statistically significant improvement in limb functionality/mobility upper, lower limb and general functionality (p = 0.005, p = 0.013 and 0.000, respectively). No differences were observed in terms of mass index body, fat mass or lean mass. Conclusions: This study shows the benefit in functionality and fatigue of an exercise program therapeutic and healthy lifestyle habits in breast cancer operated patients. Despite a significant increase in adherence to the Mediterranean diet, it is not they observed differences in anthropometric parameters.

Background: Haemophilia A and B are the most common severe bleeding disorders and are inherited as X linked recessive pattern. The main clinical manifestations include bleeding into musculoskeletal sites or soft tissues mainly causing joint impairment and thus resulting in various morbidities. Prophylaxis therapy and inhibitor management have contributed a lot to the management of haemophilia cases. However in resource poor setting countries, like India, availability of factors, prophylactic therapy is farfetched leading to joint abnormalities, decreased physical activity and thus leading to different nutritional states. Studies from developed countries reveal obesity and overweight instances in children with Hemophilia. However not many studies have been undertaken to evaluate the nutritional status of such children in India.Methods: This study was conducted in the Comprehensive hemophilia Care Centre, Victoria Hospital, attached to Bangalore Medical College. A total of 50 children were included in the study. Children aged between 4 and 18 years attending the hemophilia Clinic were included in the study. Observations and review of relevant documents were done.Results: Among 50 children of haemophilia, 18(36%) children were aged less than 10 years and 32(64%) children were aged more than 10 years. The mean age of onset of disease in haemophilia A was 27.5 months (SD of 24.84; range 6-120) and in haemophilia B was 8 months (SD of 2.72; range 3-12). The mean BMI among children aged more than 10 years was more (21.35; SD= 4.02) compared to the children less than 10 years (16.87; SD= 3.41).Conclusions: The prevalence of overweight and obesity among children with Haemophilia is more in adolescent age group compared to children in the first decade.

El objetivo de la investigación fue conocer la asociación entre la toma de decisiones y el rendimiento, de jugadores de voleibol, en las acciones finalistas e intermedias, con el resultado del set. Se analizaron 9285 acciones: 4364 acciones intermedias (1454 acciones de recepción, 1377 acciones de colocación y 1548 acciones de defensa), 4921 acciones finalistas (1533 acciones de saque, 1555 acciones de ataque y 1818 acciones de bloqueo), correspondientes a los 21 equipos participantes en el Campeonato de España Juvenil Masculino, celebrado en 2012. Se observaron dos partidos de cada uno de los equipos participantes en el Campeonato. Las variables de estudio fueron el resultado del set, la toma de decisiones y el rendimiento, en las acciones de juego intermedias y finalistas en voleibol. Los resultados mostraron asociación significativa entre el resultado del set y las variables toma de decisiones y el rendimiento en las acciones de juego, tanto en las acciones intermedias como en las acciones finalistas. Una mejor toma de decisiones por parte de los jugadores, y un mejor rendimiento en las acciones intermedias y finalistas, se asocian con la victoria del set. En el proceso de entrenamiento de jugadores de categoría juvenil sería apropiado considerar la posibilidad de empleo de actividades que contribuyan a la mejora de la capacidad decisional de los jugadores.Abstract. The aim of the study was to determine the association between decision-making and performance during intermediate and terminal actions in volleyball, and set results. A total of 9,285 game actions were collected: 4,364 intermediate actions (1,454 receptions, 1,377 setting actions, and 1,548 defence actions) and 4,921 terminal actions (1,533 serves, 1,555 attacks, and 1,818 blocks), gathered from 21 teams participating in the U-19 Spanish Championship held in 2012. Two matches of each team were observed. Study variables were: set result, decision-making, and performance during intermediate and terminal game actions in volleyball. Results showed a significant association between set result and the variables decision-making and performance in both intermediate and terminal game actions. In fact, better players’ decision-making and higher players’ performance in both intermediate and terminal actions were significantly associated with winning sets. When coaching U-19 players, carrying out activities that improve players’ decision-making abilities should be highly recommended.

Background: Chronic Achilles and patellar tendinopathy are a significant burden in physically active populations. High-volume image-guided injection (HVIGI) proposes to strip away associated neovascularity, disrupt painful nerve ingrowth, and facilitate rehabilitation. Purpose: To investigate the efficacy of HVIGI with and without steroid relative to placebo. Study Design: Randomized controlled trial; Level of evidence, 1. Methods: A total of 62 participants were recruited between May 25, 2016, and March 5, 2020. Participants were men aged 18 to 55 years with Achilles or patellar tendinopathy of at least 6-month chronicity that had not improved with nonoperative management (including physical therapy and shockwave therapy), with ultrasound evidence of neovascularization, tendon thickening, and echogenic changes. They were assigned to the following groups: control (3 mL of subcutaneous 0.5% bupivacaine), HVIGI (10 mL of 0.5% bupivacaine and 30 mL of normal saline, ultrasound-guided between tendon and underlying fat pad), or HVIGI with steroid (HVIGIwSteroid; 0.25 mL of 100 mg/mL hydrocortisone). Clinicians and assessors were blinded. All participants were supervised through a pain-guided progressive loading program for 6 months postinjection. The main outcome measures were the Victoria Institute of Sport Assessments (VISA) for Achilles and patellar tendinopathy and the visual analog scale (VAS) for pain at 6 months postinjection. Results: The VISA score improved by a mean of 22.8 points (95% CI, 10.4-35.3 points; effect size [ES], 1.51) in the control group (n = 21), 18.6 points (95% CI, 9.1-28.0 points; ES, 1.31) in the HVIGI group (n = 21), and 18.5 points (95% CI, 3.4-33.6 points; ES, 0.88) in the HVIGIwSteroid group (n = 20). VAS pain improved by a mean of 15 points (interquartile range [IQR], –38.75, 8 points; ES, 0.39) in controls, 13 points (IQR,–34.0, 3.75 points; ES, 0.47) in the HVIGI group, and 27 points (IQR,–38.0, –1.0 points; ES, 0.54) in the HVIGIwSteroid group. The main effects were significant for time ( P < .001) but not group ( P ≥ .48), with no group × time interaction ( P = .71). One participant was lost to follow-up from each group, multiple imputation was used for missing data points. No adverse events occurred. Conclusion: Study findings did not demonstrate superiority of HVIGI over control injection. Registration: EU Clinical Trials Register (EudraCT: 2015-003587-36).

ObjectivesTo assess the efficacy of exercise, orthoses and splinting on function, pain and quality of life (QoL) for the management of mid-portion and insertional Achilles tendinopathy, and to compare different types, applications and modes of delivery within each intervention category.DesignSystematic review and meta-analysis.Data sourcesMedline, CINAHL, Embase, AMED, WHO ICTRP, Web of Science, PEDro and Cochrane Library from inception to October 2017. Citation tracking of published studies and conference proceedings and contacting experts in the field.Study eligibility criteriaControlled clinical trials evaluating either exercise, orthoses or splinting for the management of Achilles tendinopathy.MethodsIndependent reviewers undertook searches, screening and risk of bias appraisal. Primary outcomes of interest were function, pain and QoL.ResultsTwenty-two studies were included (1137 participants). Moderate level evidence favoured eccentric exercise over control for improving pain and function in mid-portion tendinopathy. Moderate level evidence favoured eccentric exercise over concentric exercise for reducing pain. There was moderate level evidence of no significant difference in pain or function between eccentric exercise and heavy slow resistance exercise. There was low level evidence that eccentric exercise was not superior to stretching for pain or QoL. There was moderate level evidence that a combined exercise protocol was not superior to a lower dosage protocol for improving functional performance. There was moderate to low level evidence of a significant difference in pain (mean difference (MD) 6.3 mm, 95% CI −4.45 to 17.04, moderate) or function (MD 1.83 Victoria Institute of Sport Assessment points, 95% CI −7.47 to 11.12, low) between high-dose and low-dose eccentric training. There was high to moderate level evidence of no difference in pain (moderate) or function (high) between orthoses and control. There was low level evidence of no significant benefit in adding a night splint to an eccentric exercise programme for function, and moderate level evidence for no reduction in pain (MD −3.50, 95% CI −10.49 to 3.48). Eccentric exercise was not superior to splinting for pain (moderate evidence) or function (low level evidence).SummaryWe conditionally recommend exercise for improving pain and function in mid-portion Achilles tendinopathy. The balance of evidence did not support recommendation of one type of exercise programme over another. We conditionally recommend against the addition of a splint to an eccentric exercise protocol and we do not recommend the use of orthoses to improve pain and function in Achilles tendinopathy.

Abstract:
Este estudio tiene como objetivos: i) conocer cómo influye el efecto de marcar primero en el resultado final del partido; ii) comparar el efecto de marcar primero, los goles totales, el periodo de marcar en el partido y el minuto de marcar el primer gol (ganador y perdedor), en las finales de ambas competiciones; y iii) analizar la evolución que tienen estas variables a lo largo de la historia. La muestra estuvo formada por todas las finales (N = 78) disputadas a lo largo de la historia a partido único y en campo neutral de las competiciones europeas UEFA Champions League (UCL) y UEFA Europa League (UEL). La probabilidad de ganar el equipo que marca primero fue del 75.6%. Cuando se analizó el efecto de marcar primero los resultados no mostraron diferencias estadísticamente significativas entre ambas competiciones, aunque sí se aprecian al analizar la evolución de este efecto a lo largo de la historia en la UCL a favor de las décadas de 1950 y 1960 (X2 = 14.15; p < .05). Se concluyó que el efecto de marcar primero en una final continental resulta determinante pues el equipo que se adelanta en el marcador aumenta en gran medida sus probabilidades de conseguir la victoria. En este sentido, los cuerpos técnicos de los clubes que luchan por el título en estas competiciones deben utilizar esta información para trabajar en los entrenamientos y plantear estrategias que les permitan salir al terreno de juego con la intención clara de abrir el marcador.Abstract. The objectives of this research were: i) to study the effect of scoring first on the final result of the match; ii) to compare the effect of scoring first, the total goals, the goal scoring period, and scoring minute of the first goal (winner and loser) in the finals of both competitions; and iii) to analyse the evolution of these variables throughout history. The sample consisted of all the single-match, neutral-field finals (N = 78) played over the history of the UEFA Champions League (UCL) and UEFA Europa League (UEL) European competitions. The team scoring first had 75.6% probabilities of winning. When analysing the effect of scoring first, the results showed no statistically significant differences between the two competitions, although they become significant in regard to the evolution of the scoring first effect throughout the UCL history, in favour of the finals played within 1950s and 1960s (X2 = 14.15, p <.05). As a conclusion, the effect of scoring first in a continental final is decisive because any team leading the scoreboard has major chances of winning. In this sense, technical staff at clubs should use this information to work on the training and propose strategies that allow them to go out onto the pitch with the clear intention of scoring first.

Abstract:
La presión por la búsqueda del rendimiento óptimo en el deporte está presente desde edades tempranas. Algunas veces dicha presión procede de la familia, particularmente, de los progenitores, y condiciona la experiencia del joven deportista, pudiendo convertirse en una vivencia negativa. El objetivo de esta investigación fue mostrar cómo influyó la presión proveniente de su padre en la experiencia deportiva del ex-tenista profesional Andre Agassi durante su infancia. Para ello, seguimos el método biográfico analizando la autobiografía publicada de Agassi, “Open. Memorias”. Después de una primera fase exploratoria del libro completo nos centramos en la presión parental, de cuyo análisis obtuvimos las siguientes categorías: pasión de su padre por el tenis, estrategias de presión parental, pensamiento de abandonar el tenis, ausencia de alternativas al tenis, victoria como forma para complacer las expectativas de su padre, ansiedad ante la derrota, miedo al rechazo paterno, identidad y discrepancia. Los resultados son discutidos a la luz de diferentes investigaciones y reflexiones teóricas sobre la presión parental en el deporte. Concluimos que la presión parental puede ser un arma de doble filo pues, por un lado, puede favorecer la motivación del individuo o, por el contrario, crear situaciones de angustia y ansiedad que pueden culminar con el abandono deportivo, o en importantes crisis de identidad. Finalmente, resaltamos la importancia de analizar en el seno de la educación física escolar el fenómeno deportivo de una manera crítica, señalando las autobiografías publicadas de deportistas profesionales como un óptimo recurso tanto pedagógico como de investigación. Palabras clave: presión deportiva; infancia; alto rendimiento; celebridad; autobiografía. Abstract: The pressure to seek optimal performance in sport is present from an early age. Sometimes this pressure comes from the family, particularly from the parents, and conditions the experience of the young athlete, which can become a negative experience. The aim of this research was to show how pressure from his father influenced the sporting experience of professional tennis player Andre Agassi during his childhood. To do this, we follow the biographical method by analyzing Agassi's published autobiography, “Open, an autobiography ". After a first exploratory phase of the complete book we focused on parental pressure, from whose analysis we obtained the following categories: his father's passion for tennis, parental pressure strategies, thoughts of giving up tennis, lack of alternatives to tennis, victory as a way to meet his father's expectations, anxiety in the face of defeat, fear to parental rejection, identity and discrepancy. The results are discussed in the light of different investigations and theoretical reflections on parental pressure in sport. We conclude that parental pressure can be a double-edged sword since, on the one hand, it can favor the motivation of the individual or, on the contrary, create situations of anguish and anxiety that can culminate in sports abandonment, or in important identity crises. Finally, we highlight the importance of analyzing the sporting phenomenon in a critical way within school physical education, pointing to the published autobiographies of professional athletes as an excellent pedagogical and research resource. Abstract:
Objectives: To determine the causative factors and management of Gynaecomastia. Study Design: Observational case series study. Place and Duration of Study: Surgical Unit-1 Bahawal Victoria Hospital Bahawalpur, from 1st January 200 till 31st December 2007. Patient and Methods: Thirty male patients having breast swelling were included in this study. Relevant history was obtained. Appropriate physical examination was performed. Necessary investigations were done and after making a diagnosis appropriate treatment was done. A total of 30 patients were studied in the study period of 18 months. Those male patients who were having breast lesion other than benign enlargement (e.g. Ca. Breast, Breast/Abscess) were not included in the study. Necessary investigations were done. Subcutaneous Mastectomy was performed. Results: The most common age group developing gynaecomastia was of 20-30 years (60%). Bilateral gynaecomastia was observed in 76.66% and unilateral gynaecomastia in 23.33%. Idiopathic gynaecomastia was observed in 73.33% cases. Medical treatment was given with tamoxifen 10mg twice a day for the period of three months in 6 case (20%) and this remained effective in 5 case (83.33%). Subcutaneous mastectomy was performed in 17 cases (56.66%). Post-operative complications were seen in 4 cases (23.52%), the most common complication being wound hematoma in 2 cases (11.76%). Most of the patients (88.9%) were fully satisfied with the results of subcutaneous mastectomy. Conclusion: Gynaecomastia is the most common benign lesion of the male breast. As far as physiological gynaecomastia is concerned, patients should be observed for at least 2 years from the onset of their condition. In most of the cases spontaneous resolution occurs. Surgical treatment should be planned in whom spontaneous resolution does not occur. Surgery remains the mainstay of therapy and is frequently indicated for psychological and cosmetic reasons.

Abstract:
IntroductionWhile tobacco smoking prevalence is falling in many western societies, it remains elevated among high-priority cohorts. Rates up to 95% have been reported in women whose pregnancy is complicated by other substance use. In this group, the potential for poor pregnancy outcomes and adverse physical and neurobiological fetal development are elevated by tobacco smoking. Unfortunately, few targeted and effective tobacco dependence treatments exist to assist cessation in this population. The study will trial an evidence-based, multicomponent tobacco smoking treatment tailored to pregnant women who use other substances. The intervention comprises financial incentives for biochemically verified abstinence, psychotherapy delivered by drug and alcohol counsellors, and nicotine replacement therapy. It will be piloted at three government-based, primary healthcare facilities in New South Wales (NSW) and Victoria, Australia. The study will assess the feasibility and acceptability of the treatment when integrated into routine antenatal care offered by substance use in pregnancy antenatal services.Methods and analysisThe study will use a single-arm design with pre–post comparisons. One hundred clients will be recruited from antenatal clinics with a substance use in pregnancy service. Women must be <33 weeks’ gestation, ≥16 years old and a current tobacco smoker. The primary outcomes are feasibility, assessed by recruitment and retention and the acceptability of addressing smoking among this population. Secondary outcomes include changes in smoking behaviours, the comparison of adverse maternal outcomes and neonatal characteristics to those of a historical control group, and a cost-consequence analysis of the intervention implementation.Ethics and disseminationProtocol approval was granted by Hunter New England Human Research Ethics Committee (Reference 17/04/12/4.05), with additional ethical approval sought from the Aboriginal Health and Medical Research Council of NSW (Reference 1249/17). Abstract:
The purpose of this article was to trace the growth and development of the Guild of St. Cecilia. This late nineteenth century organisation was founded by Frederick Kill Harford in London to provide music therapy to hospitalised patients. All information was derived from letters written by Harford and editorials that appeared in British medical and music periodicals. Initially, the Guild enjoyed great success and was endorsed by important people such as Florence Nightingale and Sir Richard Quain, physician to Queen Victoria. The Rev. Harford was astute in his observations that the effects of music must be tested to find the most beneficial ways for it to be used as therapy. He envisaged an association that would provide live and transmitted music via telephone to London's hospitals. Ultimately, due to the lack of support from the press, limited financial resources and Harford's ill health the organisation failed to prosper. Despite this, the Guild of St. Cecilia remains important because it kept alive the idea that music could be used therapeutically to benefit physically and mentally ill people.

BackgroundImmune checkpoint inhibitors (ICIs) have revolutionized cancer therapy but can result in toxicities, known as immune-related adverse events (irAEs), due to a hyperactivated immune system. ICI-related inflammatory arthritis has been described in literature, but herewith we introduce and characterize post-ICI activated osteoarthritis (ICI-aOA).MethodsWe conducted a multi-center, retrospective, observational study of patients with cancer treated with ICIs and diagnosed with ICI-aOA by a rheumatologist. ICI-aOA was defined by (1) an increase in non-inflammatory joint pain after ICI initiation, (2) in joints characteristically affected by osteoarthritis and (3) lack of inflammation on exam. Cases were graded using the CTCAE (Common Terminology Criteria for Adverse Events) V6.0 rubric for arthralgia. RECIST (Response evaluation criteria in solid tumors) V.1.1 (v.4.03) guidelines determined tumor response. Results were analyzed using Chi-squared tests of association and multivariate logistic regression.ResultsThirty-six patients had ICI-aOA with mean age at time of rheumatology presentation of 66 years (51–81yrs). Most patients had metastatic melanoma (10/36, 28%) and had received a PD1/PDL1 inhibitor monotherapy (31/36, 86%) with 5/36 (14%) combination therapy. Large joint involvement (hip/knee) was noted in 53% (19/36), small joints of hand 25% (9/36), and spine 14% (5/36). Two-thirds (24/36) suffered multiple joint involvement. Three of 36 (8%) had CTCAE grade 3, 14 (39%) grade 2 and 19 (53%) grade 1 manifestations. Symptom onset ranged from six days to 33.8 months with median of 5.2 months after ICI initiation; 5 patients suffered ICI-aOA after ICI cessation (0.6, 3.5, 4.4, 7.3 and 15.4 months after ICI cessation) (figure 1). Most common form of therapy was intra-articular corticosteroid injections only (15/36, 42%) followed by NSAIDs only (7/36, 20%) (figure 2). Twenty patients (56%) experienced other irAEs, with rheumatic and dermatologic being the most common. All three patients with high-grade ICI-aOA also had another irAE diagnosis at some point after ICI initiation.ConclusionsICI-aOA should be recognized as an adverse event of ICI immunotherapy.
