Abstract:
A perspective in critical social science is concerned with knowledge, power and critique. This thesis explores the question: What would physiotherapy practice look like if it were informed by critical social science? This question originated from four observations: (1) physiotherapists work with constantly changing health care demands, (2) traditional practice approaches underpinned by rational objectivity widen the gap between theory and practice, (3) professional judgments are based on more than objective, rational thinking, and (4) concluding from the first three observations clinical physiotherapists rely more and more on thinking for themselves. If physiotherapists were to adopt a critical social science perspective they would question their practice, identify taken-for-granted, unreflected assumptions and unnecessary system constraints and liberate themselves, their practice and patients, thereby enhancing both the quality of patient care and the practitioner’s professional work experience. Following the hermeneutic tradition I constructed texts from pertinent literature as well as transcripts from participants’ interviews, action plans and field notes. I developed an integrative design to interpret these texts drawing from philosophical and critical hermeneutics as well as action research. The question and answer dialogue methodology consisted of four cycles including deep, critical and transformative dimensions. These I labelled critical transformative dialogues. The first dialogue was with the critical social science literature and with the Gadamer-Habermas and Foucault- Habermas debates in particular. These debates addressed issues of rationality, knowledge and power. Further, I reviewed relevant education, nursing and health promotion literature that addressed these critical social science themes. This first dialogue crystallised my identification of key CSS dimensions relevant to physiotherapy practice. The second dialogue comprised physiotherapy literature that related to these identified critical social science dimensions, as well as transcripts from physiotherapists’ interviews. This dialogue critically interpreted current practice models in their historical, educational and practice contexts. It highlighted the finding that physiotherapy practice is currently dominated by instrumental thinking rather than critical thinking, and that there is a lack of engagement of physiotherapy practice with CSS. The third dialogue was with physiotherapists trialling CSS in practice. Physiotherapists of this trialling group designed action learning “contracts” where they set out to change their practice in the sense of adopting CSS principles and activities in their practice. I explored with these participants how CSS could work or fit in their practice and practice contexts and how this would be experienced. Through this action learning project of endeavouring to transform their practice towards a CSS model I explored participants’ capacity to learn about posing problems concerning their practice, recognise practice contradictions, experience practice challenges and recognise their motivations and interests. This exploration illuminated the viability of CSS in their practice. The fourth dialogue was with physiotherapists who operationalised CSS values or who could visualise a CSS framework for their practice whether they used this terminology or not. This dialogue brought critical understanding of the advantages and potential limitations of realising a CSS-centred physiotherapy practice. I conclude the thesis with twelve propositions arising from these four critical transformative dialogues. Based on the trialling, transforming and visioning of CSS as a model for physiotherapy practice, the relevance of these propositions for critical physiotherapy practice is asserted and implications for education and further research are discussed. The contribution that CSS can make to physiotherapy practice is to add critical transformative dialogues as a strategy to advance practice that is patient-centred and multidisciplinary in approach, inclusive of sociopolitical environments, mindful of professional power and open about professional values.

Abstract:
Despite the increasing number of individuals taking computer-based tests, little is known about how examinees perceive computer-based testing environments and the extent to which these testing environments are perceived to affect test performance. The purpose of the present study was to assess the testing environment as perceived by individuals taking the National Physical Therapy Examination (NPTE), a high-stakes licensure examination. Perceptions of the testing environments were assessed using an examinee self-report questionnaire. The questionnaire included items that measured individuals’ preference and perception of specific characteristics of the environment, along with demographic information and one open-ended item. Questionnaires were distributed by email to the 210 accredited physical therapy programs at the time, encouraging programs to forward the instrument by email to the most recent class of physical therapy graduates. Two hundred and sixteen respondents completed the study, representing 101 testing centers in 31 states. Data from these 216 examinees were used to answer four research questions. The first research question focused on the examinees’ environmental preferences for the NPTE testing environment and the relation between these preferences and examinees’ background characteristics (e.g., sex, program GPA, age, online experience, online testing experience, comfort level with online testing, and preferred testing time). A clear preference toward one end of the scale was observed for preferring a quiet room and a desktop area that had a great deal of adjustability. Examinees’ preferences and their demographic characteristics were not strongly related with the seven demographic variables accounting for < 7% of the variability in examinees’ environmental preferences. The second research question used the data from multiple examinees nested within the same testing center to examine the within- and between-center variability in examinees’ perceptions of the testing environment and their satisfaction with the environment. Results indicated that the majority of the variance in these variables was within testing centers with average between-center variability equal to .032 for the perception ratings and .078 for the satisfaction ratings. Research questions (RQ) three and four explored whether examinees’ background characteristics (RQ 3) and center characteristics (RQ 4) were significantly related to the 12 environmental perception ratings, 12 satisfaction ratings, and two items representing examinees’ perceptions of the effect of the testing environment on their performance and the likelihood they would choose the same center again. In terms of examinee characteristics, age, online testing experience, and comfort with online testing were the most consistent predictors of the various examinee ratings. The most consistent predictors for the satisfaction ratings were examinees’ online test comfort, online test experience, and age. For center characteristics, the newness of the center and the room density of the center were the most consistent predictors of examinee ratings. For satisfaction ratings, the most consistent predictor was the newness of the center. Center newness was significantly related to the outcome variables related to the size, lighting and sound of the center which may reflect changes in building standards and materials. The results of the study suggest the need for further exploration of the environmental and human factors that may impact individuals taking high stakes examinations in testing centers. Although there may not be an effect on all examinees, there may be subsets of individuals who are more sensitive to the effects of the testing environment on performance. Further exploration of the uniformity of testing environments is also needed to minimize error and maximize potential threats to test security.

Abstract:
Physical activity plays a crucial role in contributing to psychological stability regarding physical concept. However, the evidence for showing the association between levels of physical activity and the psychological state has not been closed to focus on college students. This study was to examine the importance of physical activity by investigating the effects of different levels of physical activity on physical self-concept and social physique anxiety among college students. This study used a secondary data set approved by Institutional Review Board, and obtained a written permission and approval for data use. The data set used for this study was not previously analyzed and published. A total of 238 participants (99 males and 139 females) were 4-year comprehensive university students in Southern California. Godin Leisure-Time Exercise Scale (GLTES), Physical Self-Description Questionnaire (PSDQ), and Social Physique Anxiety Scale- 7(SPA-7) were used to analyze levels of physical activity, physical self-concept, and social physique anxiety, respectively. The SPSS version 19 was used to analyze the data for the purpose of this study. Results showed that there were statistically significant differences in physical self-concept and social physique anxiety among the levels of physical activity. There were statistically negative correlations between social physique anxiety and physical self-concept.

Questions about knowledge flows between different fields of science are important from a policy perspective. This thesis focuses on knowledge transfer between physical sciences and life sciences. Science and technology are increasingly intertwined in a complex continuum. This complexity of the science and technology today asks for a concerted, articulated and comprehensive understanding of the process of science and technology. The approach that this research has taken is to analyse the process of science and technology. The thesis asks: What is the trade of science and technology? In order to answer this question we developed an anatomy of knowledge and we analysed the internal developments in science via the analysis of the role of the researchers as carriers and producers of knowledge. Secondly the thesis asks: What are the mechanisms and directions on which scientfic knowledge migrates? This research postulates that the analysis of the process of science and technology translates to the analysis of the production and transfer of scientific and technological knowledge. What is obvious and essential for science and technology is the difference between the specific mechanisms of knowledge production. This thesis suggests that the modem mode of knowledge production is characterized by an increasing density of communication on three levels: between science and technology - on one hand - and society on the other-; between scientific practitioners; and with the entities of the physical and social world. Central to our research is the concept of 'mode of knowledge production ' with mode 1 and mode 2 being defined by Gibbons. The four case studies employed emphasise on how collaboration across disciplines is highly important for the production of new knowledge. The main characteristic of newly emerging fields in an increasing synergy between disciplines, which leads to several types of communication between them. With the increasing of the interdisciplinary intensity the border between the production of knowledge and the transfer of knowledge begins to be blurred. The transfer of knowledge occurs today at a more conceptual level. It follows that the production of knowledge has a large component of knowledge transfer. To study it, this thesis proposes a quasiquantitative model. In this unified framework for the knowledge transfer mechanisms, transfer is seen as a process with a number of stages and forms. We tested our framework on four case studies. The third part of the thesis proposes a taxonomy of interdisciplinarity. and deals with the social engineering of knowledge transfer that is the design of adequate guidelines for policies aiming at maximization of knowledge transfer. In this way the thesis aims to contribute to the understanding of processes of development of new emerging scientific fields.

Abstract:
Study Objectives: Nocturnal awakenings may constitute a unique risk for falls among older adults. We describe differences in gait and balance between presleep and midsleep testing, and whether changes in the lighting environment during the midsleep testing further affect gait and balance. Methods: Twenty-one healthy, late middle-aged and older (64.7 ± 8.0 y) adults participated in this repeated-measures design consisting of four overnight laboratory stays. Each night, participants completed baseline visual acuity, gait, and balance testing. After a 2-h sleep opportunity, they were awakened for 13 min into one of four lighting conditions: very dim white light (< 0.5 lux); dim white light (∼28.0 lux); dim orange light (∼28.0 lux); and white room-level light (∼200 lux). During this awakening, participants completed the same sequence of testing as at baseline. Results: Low-contrast visual acuity significantly decreased with decreasing illuminance conditions (F(3,45) = 98.26, p < 0.001). Our a priori hypothesis was confirmed in that variation in stride velocity and center of pressure path length were significantly worse during the mid-sleep awakening compared to presleep baseline. Lighting conditions during the awakening, however, did not influence these parameters. In exploratory analyses, we found that over one-third of the tested gait and balance parameters were significantly worse at the midsleep awakening as compared to baseline (p < 0.05), and nearly one-quarter had medium to large effect sizes (Cohen d ≥ 0.5; r ≥ 0.3). Conclusions: Balance and gait are impaired during midsleep awakenings among healthy, late middle-aged and older adults. This impairment is not ameliorated by exposure to room lighting, when compared to dim lights.

Abstract:
The United States economy depends on small businesses, which represent 99% of all businesses in the United States, yet approximately 50% of small businesses cease operations in the first 5 years. The purpose of this multiple case study was to explore the marketing strategies used by owners of small businesses to develop and maintain their organization through the initial 5 years of business operation. The population included 5 physical therapy business owners in northeastern Florida who had sustained their business for a minimum of 5 years. The conceptual framework for this study was the brand equity model, and data were collected through semistructured interviews, online company marketing materials, and archival company documents. Yin's 5-step analysis guided the data analysis process: (a) collecting data, (b) grouping data into codes (c) grouping data into themes (d) assessing the themes, and (e) developing conclusions. Member checking, transcript review, and triangulation were used to validate the study data. The 5 study themes were brand awareness, relationship marketing, perceived quality, social media and online marketing, and word-of-mouth marketing. The implications of this study for positive social change include the potential for economic growth in the physical therapy practice market, as well as improved patient access to physical therapy services through a greater number of physical therapy practices.

Abstract:

Stuttering and social anxiety disorder (SAD) share characteristics such as fear and anxiety toward social situations. Despite similarities, the underlying cognitive biases that perpetuate these characteristics in SAD have only begun to be studied in stuttering. We investigated judgment bias, the overestimation of probability and cost of negative outcomes for social situations, in a sample of persons who stutter (PWS) and a sample of age- and gender-matched typically fluent controls (NPWS). There were no between-group differences for trait anxiety. Although results indicated no between-group differences in judgment bias for social situations, further investigation revealed significant differences in judgment bias for social situations between PWS with high FNE and PWS with low FNE. Group differences were observed between PWS with high FNE, PWS with low FNE, and NPWS for distribution of judgment bias for different types of situations. PWS with high FNE perceived positive and mildly negative social scenarios as more threatening that the other two groups did. Future directions to evaluate potential treatment options for PWS such as measurement of faulty attention or interpretation bias in PWS are discussed.

Abstract:
The purpose of this study was to describe participant experiences of how a rehabilitation centre can facilitate social inclusion for people with physical disabilities in Mexico. This research was a qualitative interview study with a semi-structured interview approach. Ten participants were recruited using purposive sampling. The participants were individuals with physical disabilities, including both mobility and visual impairments, who had a connection with the rehabilitation centre. Content analysis was used while examining the collected data. The result consisted of one main category ‘individual experiences of achieving social inclusion’, four categories; ‘being supported by a community’, ‘acceptance of disability’, ‘the centre's work and its effect in the society’ and ‘the importance of work’. Each category contained several sub-categories. In conclusion, the rehabilitation centre facilitates inclusion through work opportunities and the sense of belonging to a community which has a positive impact on persons with a disability. Changing attitudes in the society by raising awareness about disability and generating an acceptance of one’s disabilities were important facilitating factors.

In light of the existing literature and research conducted in the area of domestic violence, several recurrent symptoms continue to surface as a result of both physical and non-physical abuse. Two of the most prevalent psychological symptoms requiring intervention tend to be depression and low self-esteem. Therefore, it is the intention of this study to empirically examine the type of abuse female victims of domestic violence experience and its link to their psychological distress. Specifically, it is hypothesized that victims of non-physical abuse will be more likely to experience low self-esteem than depression. Victims of physical abuse will have a greater prevalence of depression than low self-esteem.

Abstract:
One hundred and fifty undergraduate students at a Southwestern university rated the attractiveness of the face, body and full, face and body of 5 male and 5 female stimuli. The subjects also rated the full, face and body stimuli on 6 social and relationship variables. These ratings were used to determine the relative influence of facial attractiveness versus body attractiveness on overall assessments of attractiveness and social and relationship variables. Both facial and body attractiveness were predictive of all the overall assessments, but face was a more powerful predictor. A second analysis related perceived similarity of attractiveness and liking. Perceived similarity of attractiveness was not a significant factor in how much the subjects indicated they liked the stimuli.

Purpose: The purpose of this study was twofold, to determine (1) if obesity is more prevalent among youths with functional limitations than among youths without special needs; and (2) to determine if variables related to the domains of youth lifestyle, parenting experience, perceived social capital, and youth social participation mediate the relationship between youths with functional limitations and obesity. Design: This study is a secondary analysis of cross-sectional data from the nationally representative dataset, the National Survey of Children’s Health. Exposure: Youths between the ages of 10 and 17 with functional limitations or no special healthcare needs. Outcome: Sex-specific body mass index ≥ 95th for age percentile values using 2000 Center of Disease Control growth charts. Variables of interest: Potential mediators were examined in four domains: (1) youth lifestyle, defined as participation in after-school sports, physical activities, hours spent watching television, having a television set in the youth’s bedroom, eating family meals together, and getting enough sleep; (2) parenting experience, defined as parental aggravation, coping, and emotional support; (3) perceived neighborhood social capital; and (4) youth social participation, defined as participation in after-school club activities. Results: Gender was an effect modifier, with 27.7% of females with functional limitations being obese, as compared to 12.9% of females with no special needs. There was no statistically significant difference between the prevalence of obesity in male youths with functional limitations and in those with no special needs. The association between female youths with functional limitations and obesity was partly mediated by participation in after-school sports and participation in physical activities. Conclusion/Implications: The findings from this study suggest that lack of female participation in after-school sports and physical activities is partly responsible for the increased prevalence of obesity in those with functional limitations compared to those with no special needs. Future interventions that reduce barriers to and increase facilitators of after-school sports and physical activities could reduce the difference in obesity prevalences between females with functional limitations and those with no special needs.

Abstract:
This thesis contains the work from two inter-related studies focusing on the self-perceptions of pupils with Down Syndrome. After a brief account of my value position in relation to the research in Chapter 1, Chapter 2 discusses the literature on self-perceptions. This discussion provides the rationale for study 1. In Chapter 3 the developmental profile of children with Down Syndrome is discussed. This provides the rationale for the choice of research instruments. In study 1 the self-perceptions of 96 pupils with Down Syndrome between the ages of 8 and 16 years were assessed in three school related domains: academic competence, physical competence and social acceptance Results from the Pictorial Scale of Perceived Competence and Acceptance (Harter & Pike, 1981/1984) and a Situations Grid suggested that pupils with Down Syndrome hold very positive self-perceptions in each school related domain, that self-perceptions become more positive with age, are more positive for female than for male pupils and remain positive regardless of school placement type (mainstream or special school). The findings from study 1 left unanswered a number of questions concerning the self- perceptions of pupils with Down Syndrome. Furthermore, examination of individual scores revealed considerable differences in the self-perceptions of individual pupils. Therefore, the aim of study 2 was to investigate factors associated with high and low self-perceptions. Study 2 also allowed pupils, teachers and parents more freedom to participate in the research. Fourteen qualitative case studies were completed. The interview and observation data arising from study 2 revealed the heterogeneity in factors associated with the self-perceptions of pupils with Down Syndrome. In the closing chapters: possible explanations for the study findings and suggestions for further research are raised, the utility of quantitative and qualitative approaches for studying pupils with Down Syndrome is discussed, and the implications of the findings for existing theories are covered.

Postoperative spinal patients were randomly assigned to one of three treatment conditions and were taught five adaptive movements by occupational therapists. The Control group received routine hospital occupational therapy; the Placebo group participated in an imagery relaxation task unrelated to the mental practice task of the Imagery group, which was shown line drawings of the adaptive movements under study, provided movement instructions, and asked to mentally practice each movement in a familiar, daily living situation. Thirty-five patients returned for follow-up, and a measure of outcome was obtained through the use of a quantified movement assessment instrument. Subjective ratings for anxiety, rumination, and imagery were made by the occupational therapists. An occupational motoric-symbolic rating scale was developed to assess the symbolic portion of the patient's job experience. Statistical procedures including chi square, analysis of variance, and Pearson correlation were performed. Results were in the predicted direction although statistical significance was not achieved. Possible explanations for the obtained results were discussed.

Abstract:
The use of a structured goal setting program in injury rehabilitation has not been empirically tested, but many others have noted that its use could increase compliance. The primary purpose of this study was to determine the effect of a structured goal setting program on individuals' compliance to injury rehabilitation; secondly, to examine the hardiness levels of those individuals, and determine if there was a relationship between their levels of hardiness and compliance to the treatment. Individuals who sought services of Ball Memorial Hospital Health Strategies for a back injury (N = 15) participated by first completing the Personal Views Survey. Then, the control group ( = 6) completed their normal rehabilitation program, while the experimental group ( = 9) completed their rehabilitation program using a structured goal setting program. Finally, both groups completed a post-hardiness survey. The researcher found no significant difference in compliance rates between the two groups. All of the injured participants recorded moderate hardiness levels and a low correlation was observed between hardiness and compliance to treatment.

This study examined the relationships between social physique anxiety (SPA) and physical activity and sedentary behaviours among older adolescents. The research was grounded in self-determination theory (SDT; Deci & Ryan, 1985). Three hundred and eighty one males and females completed scientifically-supported questionnaires. Multivariate and univariate analyses of variance revealed that male adolescents reported lower SPA and higher competence, relatedness, self-determined motivation, and physical activity levels compared to females. Regression analyses supported the integration of SPA in SDT, and the main tenets of SDT were maintained. Specifically, the basic psychological needs were important correlates of motivation, and motivation was a positive correlate of PA behaviour and a negative, albeit weak, correlate of sedentary behaviour. Collectively, these findings suggest that SPA experiences can be understood within a motivational framework that explains the functional role played by SPA on health behaviour.

The purpose of this study was to determine the effects of evaluation apprehension and achievement on subjects' internal causal attributions in a physical skills task. One hundred and twenty private school male students, aged 12 to 19, were assigned to one of two conditions of evaluation. Feedback on level of achievement was given upon completion of the task and the students asked to complete a seven item attribution questionnaire. A 2 x 2 (evaluation x achievement) multivariate analysis of variance was used to examine differences across conditions among the four internal attributions. The results indicated that the subjects' attributions were affected by evaluation, as well as different levels of achievement. The predicted interactive effect of evaluation and achievement on attribution was not found. It was concluded that there was a flaw in the logic which suggested that internal attributions would be affected by an interaction of evaluation apprehension and achievement.

ENGLISH ABSTRACT: The aim of this study was to improve the measurement of postural dynamism in the sitting position using a three-dimensional (3D) motion analysis system. The primary objective was to describe pilot data for postural dynamism of the cervical and thoracic spines while working at a desktop computer. The secondary objective was to refine the process of posture measurement and analysis by decreasing data processing time. Certain factors in 3D motion analysis can lead to an increase in gaps in data collected during trial capture, which in turn will lead to a longer time of data processing. In the first phase of this study, a number of such factors were identified and altered. A series of pilot studies was performed to test the improvement of data processing time when altering these factors. In the first two pilot studies, camera and tripod positionings were explored and refined, workstation layout and anatomical landmark marker placement were investigated, and optimal capture frequency was established. In both these pilot studies, outcomes were established by means of trial and error by experimenting with a variety of different options for the different outcomes. In the third pilot study, computer software which provides computer tasks for the participant during primary trial capture was tested. Two independent computer users performed all the activities as per software, after which they were required to give oral feedback and suggestions on improvement in terms of user friendliness. The objective of the fourth and final pilot study was to include all of the outcomes from the preceeding pilot studies and attempt a trial run of the actual data collection process. A study participant with no affiliation to the research project was used and a complete trial run was performed after which the measurement process was deemed feasible. In the primary study, 18 student volunteers completed a sequence of computer tasks, including keyboard, mouse and reading activities. Prior to data capture, full range of motion of the thoracic and cervical spines were measured in three dimensions for every participant. Data capture took place for the full duration of performance of all computer activities. Outcome parameters for postural dynamism included true range of motion (degrees), proportional range of motion (percentage) and motion frequency (movement per minute) in all three planes of motion of the cervical and thoracic spines. Typing tasks were associated with biggest movement ranges and motion frequencies. Mouse activity was associated with the most stationary posture, exhibiting the least frequent movement as well as the smallest ranges of motion. The results from this study allow us to better understand the dynamic nature of posture, as well as postural dynamism associated with different computer tasks. This study provides a baseline for future research of 3D motion analysis of the sitting posture. It also marks the need for further research regarding ergonomics, use and potential alternatives in the computer workstation and input devices.

Tydens die derde loodsstudie is rekenaarsagteware getoets wat die rekenaaraktiwiteit vir die studiedeelnemers verskaf het tydens primêre data-insameling. Twee onafhanklike persone het al die aktiwiteite volgens die sagteware voltooi en het verbale terugvoer en aanbevelings gegee oor hoe om die program te verbeter. Die vierdie en finale loodsstudie het gepoog om al die uitkomste van die eerste drie loodsstudies in te sluit en „n toetsmeting te doen van die ware dataversamelingsproses. „n Onafhanklike studiedeelnemer met geen affiliasie tot die navorsingsprojek nie het „n toetslopie van die hele versamelingsproses gedoen en die metingsproses is haalbaar verklaar. Uitkomsparameters vir posturale dinamisme het die volgende ingesluit: Omvang van beweging (grade), proporsionele omvang van beweging (persentasie) en bewegingsfrekwensie (bewegings per minuut) in al drie bewegingsvlakke van die servikale en torakale werwelkolomme. Sleutelbord-aktiwiteite is geässosieer met die grootste bewegingsomvange en die meeste bewegingsfrekwensie. Muisaktiwiteit is geässosieer met die mees stasionêre postuur en het die heel minste gereelde beweging getoon in die algemeen. Die resultate van hierdie studie help om die dinamise natuur van postuur beter te verstaan, sowel as posturale dinamisme wat met verskillende rekenaartake verbind word. Die studie bied „n basislyn vir die toekomstige navorsings wat posturale dinamisme met verskillende rekenaartake meet. Dit merk ook die behoefte aan verdere navorsing aangaande ergonomika, gebruik en alternatiewe tot rekenaarwerkstasie en –toerusting.

Musculoskeletal disorders are common among musicians at all levels of performance. Since music teachers train our future musicians it is important to understand their work environment. By creating good examples of a healthy work environment, they can teach their students how to stay healthy and to prevent pain. The aim of this thesis was to study the work environment of music teachers at municipal music schools, with regard to physical and psychosocial factors and musculoskeletal disorders with the focus on neck and shoulder disorders. An additional aim was to investigate the variability of the playing technique in string players and to investigate if they could play with greater variation in the trapezius muscle activity pattern after a training intervention program. In a cross-sectional study at 23 municipal music schools, 171 out of the 208 (82%) music teachers reported that they had experienced work related musculoskeletal disorders (WMSDs) during the previous year. Women reported significantly more symptoms in the neck, the shoulders and the upper back compared to men. Both physical and psychosocial work factors were associated with neck and shoulder disorders. For women “high mental work demands” and “teaching at many schools” could be seen as risk factors and for men “lifting”, “playing the guitar” and “low social support at work” were risk factors. The occurrence of WMSDs was also investigated, over an eight-year period, in music teachers at one music school. The result showed that neck, shoulder and lower back disorders were common and tended to be of long duration and to increase over the years. In an interview study, nine music teachers focused on what they perceived to be important for their health and well-being. Replenishing and using up energy was found to be the core category. Creativity in the music and working with other musicians were perceived as sources of energy, while the goals of the organisation were experienced as stressful and used up energy. Whether the work was regarded as pedagogical or musical could affect the perception of health and the strategies for dealing with the strains of work. In two studies using electromyography, the variation in the trapezius muscle activity pattern in string musicians was investigated. The results suggested that each musician could repeat their muscular activity pattern in a similar way between two playing sessions. No difference was found in the trapezius muscle activity between five violinists who trained basic Body Awareness Therapy (BAT), a technique having its roots in Tai Chi Chuan tradition, compared to a reference group of nine violinists who did not take part in any training. However, the training group perceived positive changes in breathing, muscular tension, postural control and concentration during practice sessions. Neck and shoulder disorders were associated with physical and psychosocial factors at work. A process of replenishing and using up energy was important for music teachers’ health. The playing technique in string musicians seemed to be repeatable but difficult to affect over a short-term period. For future musicians it is crucial to learn good working technique at an early age. In the learning process the music teacher is a vital role model.

Our society has become less physically active (U.S. Department of Health and Human Services, 2010) and less connected to nature than ever before (Berger & Mcleod, 2006). Spending leisure time indoors, technological advancements, urban living, and car dependent communities have led to these changes (Dustin, Bricker, & Schwab, 2010; Hansen-Ketchum, Marck, & Reutter, 2009; Norman & Mills, 2004). As a result, physical health and mental health is deteriorating (Dustin et al., 2010; Maller, Townsend, Pryor, Brown & Leger, 2005). Physical activity and nature can each produce mental and physical health benefits; some approaches such as adventure-based counseling and wilderness therapy already incorporate these elements. A promising alternative approach using physical activity and nature has received attention in recent years. Walk and talk therapy has been described as an intervention that combines counseling, walking, and the outdoors (Doucette, 2004). Despite, a small number of therapists using the approach (Gontang, 2009), anecdotal research (Hays, 1994), and a description of the approach (Doucette, 2004), little is known about walk and talk therapy. In this qualitative study 11 therapists were interviewed about their experiences with walk and talk therapy. Main themes of the study suggested characteristics, a procedure, reasons walk and talk therapy evolved, limitations, outcomes, and a framework for practice for walk and talk therapy. Therapists believe walk and talk therapy is beneficial for clients as well as therapists. Implications for therapists, researchers, and counselor educators are provided.

Preexisting literature informed the current study on art making’s ability to manage psychological and physical stressors that can accompany the development of a physical disability due to a trauma experience. The current study utilized a focus group to gain a better understanding of art making’s benefits by obtaining firsthand accounts from individuals with experience incorporating art making in their own recovery process. Data collected during the focus group suggests that the art making process is beneficial as part of the recovery efforts. Data showed that art making possesses the ability to aid with the processing of psychological and physical stressors encountered during recovery, and functions as an integrative process that helps individuals to accommodate their new identities. Such findings suggest that the inclusion of art therapy, of which art making is the core foundation, can be a beneficial component to incorporate into recovery efforts.

Abstract:
Achieving adequate and appropriate recovery from exercise is essential in ensuring optimal performance during repeated bouts of exercise. The use of various recovery interventions has become popular in an attempt to enhance subsequent performance and accelerate post-exercise recovery. The application of various post-exercise hydrotherapy interventions has become increasingly popular, however, the majority of current recovery practices appear to be based largely on anecdotal evidence as opposed to rigorous scientific research or evidence based findings. Physiologically, various hydrotherapy protocols have been shown to affect the body via fluid shifts (interstitial to intravascular space), changes in blood flow and cardiovascular function, and reductions in oedema. The possible psychological effects of water immersion must also be considered, with athletes commonly reporting reduced sensations of fatigue and soreness following immersion. Current literature suggests both hydrostatic pressure and water temperature to be important factors influencing the success of hydrotherapy. The overall aim of the present thesis was to enhance current knowledge and understanding with regards to the physiological and performance effects of various forms of hydrotherapy, used as a post-exercise recovery intervention. Initially, four cold water immersion interventions were compared to active recovery, performed between two bouts of high intensity cycling in hot environmental conditions. Effectiveness of recovery was determined via performance in a subsequent exercise bout; in addition, core body temperature, lactate, and heart rate were recorded. The remaining studies were designed to investigate the effects of cold water immersion, hot water immersion, contrast water therapy, and passive recovery 4 (control) following exercise-induced fatigue and exercise-induced muscle damage. Rate of recovery was assessed through changes in performance, core body temperature, thigh girths, blood markers, and perceived exertion/soreness. The results of the combined studies indicate cold water immersion to be more effective than active recovery when performed immediately post-exercise between two bouts of high intensity cycling in hot environmental conditions. Additionally, both cold water immersion and contrast water therapy were effective in aiding recovery from exercise-induced fatigue and exercise-induced muscle damage. Performance variables indicated an improved maintenance or return of performance following these recovery protocols. The present studies have provided additional information to the limited knowledge base regarding the effect of post-exercise hydrotherapy interventions, specifically, the effect of such interventions on subsequent athletic performance. In conclusion, cold water immersion and contrast water therapy appear to be superior to hot water immersion, active recovery, and passive recovery following fatiguing and muscle damaging exercise. Functional and physiological recovery was enhanced following the use of these two recovery protocols.

This thesis aims to understand the production of social place through the practice of physical therapists at their work place. The traditional work of physical therapy as exercised by professional tends to be characterized by actions both fragmented and decontextualized. In an effort to explain this situation, geography offers theoretical perspectives useful for understanding how workplaces are constructed. This empirical research used includes 89 interviews with physical therapists, data from the “First São Paulo State Physical Therapists Census” and Brazil’s Ministry of Education studies on the trajectory of undergraduate physical therapy programs. Most of these programs are located in private colleges, that are regionally concentrated in southeast of Brazil, especially the state of São Paulo. The analysis of this information demonstrated that the workplace of the physical therapist is built around rehabilitation services that are tightly linked to the individualist, own private and liberal ideal. 40% of residents living in care homes in the United Kingdom have significant depressive symptoms, compared with 9% living in the community. Care homes can appear to be depressing places, but whether the physical environment of homes directly affects depression in care home residents is unknown. The overall aim of the thesis was to establish whether the physical environment or specific features of the physical environment predict depressive symptoms in older people living in care homes. A mixed methods approach was used: 1. A systematic review was conducted to identify and evaluate instruments designed to measure the physical environment of healthcare settings and informed the choice of instrument used to measure the physical environment of care homes in an observational study. 2. Semi-structured interviews with 15 participants living in four care homes identified features of care home design that were considered important to investigate in the observational study and assisted with interpretation of the results. 3. An observational study explored the longitudinal relationship between the physical environment and depressive symptoms using data from an interventional trial among older people living in care homes. The physical environments of 50 care homes were assessed using The Sheffield Care Environment Assessment Matrix (SCEAM). Depressive symptoms were measured using the Geriatric Depression Scale (GDS-15) for 510 residents living in the care homes at baseline, six and twelve months. Multi-level modelling was used to analyse the data. Interviews revealed residents valued environments that increased opportunity for social interaction, maximised physical function, and allowed access to outdoor space. In the observational study the overall physical environment of care homes (overall SCEAM score) did not predict depressive symptoms. Care homes that scored highly on the SCEAM domain community weakly predicted lower depressive symptoms. Controlling for dependency (Barthel Index), social engagement (Index of Social Engagement) and home type (nursing/residential) ‘access to outdoor space’ predicted greater depressive symptoms. This finding initially appeared counterintuitive; however evidence from the interview study suggested access to outdoor space was restricted for residents in many different ways: locked doors, lack of paths, inadequate seating, steep steps, and needing permission or assistance of staff to go outside. Residents may have ‘free access to outdoor space’ but are prevented from using the outdoors for reasons that negatively affect their decisional autonomy and consequently their mood. The research provides evidence to support care home design that encourages social interaction and allows unrestricted access to outdoor spaces in order to positively influence mood in care home residents.

Previous research indicates that women are judged on the amount of food eaten and that both men and women are judged on the type of food eaten. This study is an attempt to determine whether meal size or type predominantly accounts for these findings on the variables of masculinity, femininity, attractiveness, thinness, fitness, and likability. Physical activity was also included to determine its effect on these variable. Subjects used were 313 undergraduate students. Results indicate that meal type is more influential than meal size and that physical activity significantly influences judgements of others. The results are discussed in terms of future research and relatedness to socio-cultural theories of eating disorders.

ENGLISH ABSTRACT: STUDY AIM The aim of the study is to explore the psychosocial characteristics of HIV positive clients who are yet to achieve viral load suppression after six months of commencing ARV at Sundumbili CHC in order to plan positive intervention strategies. RESEARCH DESIGN Non-experimental quantitative design was used in carrying out the study. The data was collected through retrieval of information from clinic records and completion of questionnaires to clients on ARV who met the inclusion criteria and consented to participating in the study. A total of 51 adults aged more than 18 years that were initiated in 2010/2011 and still access their treatment at Sundumbili CHC were enrolled into the study. They were selected through convenience sampling. FINDINGS Psychosocial challenges still exist among research participants whose viral load results were not suppressed after six months on ARV. This affected the adherence of some of them to their antiretroviral treatment. CONCLUSION Given the rural nature of Sundumbili and surroundings where the bulk of the patients reside, there are several psychosocial challenges affecting the patients. No known previous study has been undertaken to ascertain the psychosocial characteristics of this group of patients and the impact they may have on viral load suppression after six months of treatment. The study is therefore significant as the findings have provided more insight into the plight of the patients. It is envisaged that the recommendations from the study will assist the relevant management staff in the department in planning and subsequently implementing more positive intervention strategies. The strategies should be targeted at improving the quality of care of the HIV positive clients and attending to their psychosocial needs.
AFRIKAANSE OPSOMMING: STUDIEDOELWIT Die doel van die studie was om ondersoek in te stel na die psigososiale kenmerke van MIV-positiewe kliënte wie se virustellings ná ses maande van antiretrovirale (ARV) behandeling by die gemeenskapsgesondheidsentrum op Sundumbili steeds nie onder beheer was nie, ten einde positiewe intervensiestrategieë te beplan. NAVORSINGSONTWERP ’n Nie-eksperimentele kwantitatiewe ontwerp is gebruik om die studie te onderneem. Die data is ingesamel deur die herwinning van inligting uit klinieklêers sowel as die afneem van vraelyste onder kliënte op ARV’s wat aan die insluitingsmaatstawwe voldoen en tot deelname aan die studie toegestem het. Abstract:
The purpose of the present study was to determine whether perceptions of cohesion served as a mediator between motivation and future participation (operationalized as intention to return). The participants were 162 intramural athletes participating in various team sport activities. Each participant completed a questionnaire that assessed cohesion (individual attractions to the group-task and-social; group integration-task and-social), motivation (amotivation, external regulation, introjected regulation, identified regulation, intrinsic motivation), and intention to return (using a one-item statement). The results found two mediational relationships: (a) individual attractions to the group-task served to mediate the relationship between intrinsic motivation and intention to return, (b) group integration-task served to mediate the relationship between intrinsic motivation and intention to return. A number of aspects related to the specific results are discussed.
