Abstract:
This chapter deals with an increasingly important topic: the recognition that individuals with severe mental illness die nearly 20 years before they should. The situational factors contributing to this excess mortality are outlined—failure to register with a GP, homelessness, and dysfunctional help-seeking behaviour. Individual risks, including self-neglect, co-morbid conditions, and the impact of treatments (e.g. metabolic syndrome caused by novel antipsychotics), are also outlined. The role of the outreach worker can involve building liaison with the GP and, on occasions, taking direct responsibility for the physical care of some of the more severely ill patients. There are risks of blurred confidentiality, marginalization, and withdrawal by GP services in this approach, but sometimes it is inevitable.

Abstract:
While physical activity is considered essential for the positive physical and psychosocial development of children, our understanding of the types and amounts of physical activity which are necessary to positively impact current and future (i.e. adult) health status remains unclear. Emerging evidence and advances in scientific assessment and analysis have shed new light on the activity thresholds that might be required to promote positive health in children, the extent to which children are achieving these thresholds, and the various factors, processes, and mechanisms that explain individual differences in activity and sedentary behaviour. It is equally evident, however, that much future work is required if we are to gain a more comprehensive understanding of physical activity and health in children, and to encourage children to adopt and maintain a more active, less sedentary lifestyle.

Abstract:
The incidence of morbidity and mortality related to CVD is rather low in a paediatric population. Studies investigating the relationship between physical activity, physical fitness, and cardiovascular health in children and adolescents are therefore mostly limited to CVD risk factors as outcome measures. For this reason, this chapter will focus on the association of physical activity and physical fitness with CVD risk factors in children and adolescents. These risk factors can be divided into the so-called traditional CVD risk factors; that is, lipoproteins [total cholesterol, low-density lipoprotein (LDL) cholesterol, high-density lipoprotein (HDL) cholesterol, triglycerides (TG)], blood pressure, body fatness, and diabetes, and ‘new’ CVD risk factors; that is, other lipoproteins [lipoprotein(a) (Lp(a)), apolipoprotein (apo)B, and apoA-1], coagulation and inflammation markers [fibrinogen, C-reactive protein (CRP)], homocysteine, and heart rate variability.

Abstract:
This chapter introduces the research on hope and physical health. The various conceptualizations and measures of hope in the medical literature are addressed, although the research covered focuses on Snyder’s hope theory. The research on hope and health behaviors is presented, followed by a critical discussion of research on hope and specific areas of health outcomes, including pain, cancer, spinal cord injury, rehabilitation/injury, and chronic illness. Some of the mechanisms through which hope influences health are discussed, and the concepts of related health behaviors and health outcomes are explored. These health effects include coping and buffering against stress and depression. This chapter concludes with questions for future research.
