Full textProviding care for elderly people with dementia represents a considerable challenge for our societies, to which current models of dementia partially struggle to respond. For many years, person-centered care has advocated for a prioritized refocusing of caregiving for persons with dementia on their psycho-social needs and their personhood, regardless of cognitive deficits and disabilities.Implementing its principles into concrete clinical practices has been associated with beneficial effects for persons with dementia and their caregivers. However, person-centered care remains underdeveloped as comprehensive caregiving models, partially due to a lack of suitable tools. The Montessori method adapted for the elderly has been gaining popularity worldwide, particularly in France, as a comprehensive person-centered model of care. The objective of this thesis is to provide theoretical contributions, as well as valid and applicable tools to facilitate the implementation of such approach.In our first study, we conducted a literature review of the Montessori method to outline its development as a person-centered approach and to clearly define its core principles. In our second study, we conducted a literature review focusing on its impacts on caring for persons with dementia. This review highlighted positive effects on engagement, emotions, psychological and behavioral manifestations, independence, and caregiver experiences. It also highlighted the need for additional studies based on more rigorous methodologies and evaluating its effects within the French cultural context.In our third study, we assessed the psychometric properties of an assessment tool of preserved capabilities in individuals with severe dementia: the Montessori Assessment System (MAS). Our results demonstrated entirely satisfactory psychometric qualities of MAS. It holds significant potential to contribute to the construction of person-centered care plans. In our fourth study, we evaluated the impact of behavioral manifestations on the administration and outcomes of MAS. Our findings revealed a high number of behavioral manifestations within the studied population. The presence of hallucinations and euphoria was associated with a slightly extended administration time. Conversely, the presence of hallucinations and aberrant motor behaviors was linked to significantly lower MAS total scores. However, behavioral manifestations did not hinder the complete administration of MAS.Lastly, in our fifth study, we translated a tool assessing the implementation of person-centered care in institutions: the Person-Centered Assessment Tool (P-CAT) and conducted a psychometric validation study about it. Our results indicated satisfactory psychometric qualities of the French version of P-CAT, along with a factorial structure organized into three factors: Organizational and Environmental Support, Care Planning, and Adaptation to the Resident. The French version of P-CAT can be employed within research projects or for monitoring the implementation of person-centered care approaches in institutions for elderly people.Collectively, the findings of this thesis suggest that implementation of a person-centered approach, such as the Montessori method, is feasible in caring for persons with dementia. Such approach is yielding potentially significant benefits. Furthermore, our results offer tools aimed at contributing to the implementation of person-centered care within the French context

Full textLong-stay homes, retirement homes and care homes have been the subject of an increasing number of novels and filiation narratives in France, particularly since the 1980s. As a result, this thesis establishes the history of this place in literature since the 19th century, justifying the relative institutional silence of the period 1890-1970, in terms of relationship between literature, social and medical humanities, and society. A selected corpus of filiation narratives and realistic fiction is compared with a corpus of eleven semi-directive interviews with nursing home residents in order to highlight the many ways in which people experience growing old and dying in institutions. This research in literary studies combined with a field survey reveals a disturbance in the representations of old age and dependency, reinforced by the prism of filiation, to which an ethic of literature can attempt to respond. Enunciative means - work on the voice of the elderly characters, polyphony of humour, polysemy of literary address - appear to be the most fruitful in restoring the link with vulnerability. Finally, thanks to narrative medicine, the aim is to consider how literary texts can be used to learn about and understand ageing in teaching and training contexts

Full textCe mémoire en recherche-création est articulé en deux parties distinctes, soit un roman et un essai. Nord Cap est un roman qui emprunte le genre du monologue intérieur, naviguant dans les pensées d’un vieil homme blessé qui repose dans une chambre d’hôpital. Hanté par son passé qui refait surface parfois sans prévenir, le narrateur propose un va-et-vient constant entre ses souvenirs et la réalité présente, qui se dégrade, pas à pas, jusqu’à sa mort qui devient inéluctable. L’essai sur La vie littéraire de Mathieu Arsenault, questionne cette œuvre à la narration et au genre incertains. L’objectif de cette étude est de montrer comment la narration éclatée de La vie littéraire convoque les formes du récit de la pensée et plus largement du récit intime, mais y échappe également et à quelles fins. Il est donc question de comprendre les caractéristiques de sa narration afin d’en saisir les enjeux selon qu’on l’associe au mode général du monologue intérieur ou qu’on y voit le résultat du croisement de plusieurs genres narratifs et de modes de discours issus des nouvelles formes de communication.

Full textOver the past decade, more and more elderly people are choosing to live alone. Therefore, in order to provide them with continuous home assistance, the notion of "intelligent home" has emerged. Our aim is to combine three technologies (Bluetooth Low Energy, Audio and LiFi) to provide an efficient and accurate hybrid indoor localization system that locates an elderly person inside a smart home. The principle of hybridization of these three subsystems is based on the combination of their respective results by proposing three new DOP-Like metrics to evaluate "precision" and "accuracy" of the result of each subsystem. This evaluation serves to constitute a weighting of the intermediate results in order to calculate the final position of the target to be localized. Thanks to the introduction of these indicators, the localization error of our system decreased from an average of 0.5m to 0.2m

Full textImproving the quality of care for nursing home residents is a real concern. A better understanding of the factors determining a good quality of care in nursing homes (NH) is necessary. This thesis deals with three aims: 1) Determining whether facility characteristics - and in particular the number of attending general practitioners (GPs) in NH - are associated with inappropriate neuroleptic prescribing ; 2) Studying the effect of a quality assurance approach in a NH, with or without the intervention of a geriatrician, on potentially inappropriate neuroleptics prescription at 18 months (T18); 3) Offering practical actions improving medical care in NH in order to reduce potentially avoidable transfers to emergency rooms and to prevent disability and functional decline for NH residents. To meet the two first objectives, we used the data from the Impact of a QUAlity exercise study on the change in practices and the functional decline of Residents in EHPAD (IQUARE), a comparative multicenter trial aiming at assessing the impact of a global education intervention for NH staff based on quality indicators. As for the third objective, on the one hand, we implemented a new day hospital characterized by its being exclusively dedicated to NH residents and its ability to provide patient care within a short period of time. On the other hand, we took part in a Task Force discussion of experts in NH care and research, to implement strategies to prevent or to slow disability and functional decline for NH residents.Among the 6275 residents included in IQUARE study, 1532 (24.4%) had at least one prescription for a neuroleptic drug. Compared with nursing homes with <10 GPs/100 beds, nursing homes with more than 30 GPs/100 beds were exposed to a greater risk of potentially inappropriate neuroleptic prescribing. We have not shown the added benefit of geriatrician intervention in a global effort to decrease potentially inappropriate neuroleptic prescribing. This result can be explained by the strong impact of the quality assurance audit restoration at each NH with a 20% decrease of the potentially inappropriate neuroleptic prescribing for the two study groups. Over this thesis, we have opened a responsive day hospital dedicated to NH residents. This day hospital could be a practical response to the problem of inappropriate and avoidable transfers of NH residents to emergency rooms. Improving the quality of care for nursing home residents also means preventing and/or slowing the functional decline of residents. A Task Force of experts emphasized the need for cultural change to incorporate physical activity for nursing home residents and implement multidomain interventions to delay disability. To conclude, this work has identified factors having an influence on potentially inappropriate care and suggests simple areas to improve the quality of care

Full textOlder people living in nursing homes (NHs) suffer from numerous comorbidities and functional decline. Polymedication is frequent in this population. This increases the risk of potentially inappropriate drug prescribing (PIDP), which can lead to adverse drug events such as falls and hospitalization. Most French studies did not examine PPI with a global perspective, but focused on specific drug classes. Moreover, few studies have investigated, irrespective of the individual characteristics of residents, the structural and organizational characteristics of nursing homes on the quality of drug prescribing. Thesis work aimed to develop a new indicator of PIDP, which best reflects the global medication use of residents. Thesis works have been performed using the data of the IQUARE study (Impact d'une démarche Qualité sur l'évolution des pratiques et le déclin fonctionnel des REsidents), a quasi-experimental study (trial registration number: NCT01703689) investigating the impact of an intervention based on geriatric education with NH staff on quality indicators of care. First, we developed an indicator of PIDP, combining explicit and implicit criteria, identifying 71% of PIDP in NH residents at baseline. NH organizational (access to psychiatric advice and/or to hospitalization in a psychiatric unit) and structural (presence of a special care unit for dements) variables explained part of PIDP. In a second part, in order to validate the usefulness of this PIDP detection tool, we verified the long-term clinical impact of PIDP on adverse outcomes (death, number of hospitalizations...). We did not find any significant association between PIDP and death, nor between PIDP and number of hospitalizations. Finally, the general intervention implemented in the IQUARE study significantly reduced PIDP among NH residents at 18-month follow-up. Our research has provided important aspects that should be consider when constructing further new studies seeking to change prescribing patterns and to reduce the total number of drugs taken, but also to determine the final impact of these changes on clinical outcomes

Full textRecently, the world witnesses an increasing number of the dementia cases in the older population, which causes an important impact in the society. This phenomenon attracts the attention of many researchers in multi-disciplines. Various socio-medical non-drug solutions have been proposed in which cognitive stimulation takes a place among the possible choices. In parallel, video games have been involved in many healthcare projects as privileged support for both extending and solving the limits of non-drug treatments. However, using video game for older adults with dementia in long period of time is considered as a difficult activity. They do not have the same gaming experiences as the young generation and cannot use actively and independently this tool because of memory and cognitive troubles caused by the disease. In this thesis, an approach with the purpose of maintaining the engagement of older adults with dementia while playing video game is proposed. This approach aims to provide dynamically the appropriate interactions to the users based on the current situation encompassing their physical activities and game activities. A support system is developed for validating the approach throughout 3 experiments with participants recruited from the memory center. The results show that : 1) older adults with dementia can interact successfully with the support system, 2) their game performance (playing time, accuracy rate, learning effect) is maintained, 3) the effectiveness of the support system is similar to the one of a therapist. Future work aims at testing the support system at home for long periods of time with older adults with dementia

Full textIf dementia appears as the disease of the link, the psychotherapeutic approach, at stake here, concerns the problems of the demented subject. Approach through groups is a way of restoring to the patient a place and a time to appropriate again what he can not think and that in a loud voice. The methodology consist of clinical observation owing to video and observers aids. The quality of the relationship within the psychotherapeutic framework, determines the creation of link as creation of oneself and enriches the capacity of being on ones' own in the presence of someone. This last one has the function of motherly listening. Relying on the idea that memory is also established in the presence of someone, I also supposed that memory is brought back in the presence of someone. The patient invites to the present at the meeting. A work of co-reading is engaged. It means on one hand, a reading of the review of life and the history of life; how this link has been tied up and built up? And on the other hand a work of linking up what makes self-destructive return because confrontation between the self and its image is "mortifying". The orientation of my work led to the connections with the mirror's matter which contribute to the individual construction, and the matter of the "ante-mirror"

Full textThe ageing of the population and the growing number of very old people in loss of autonomy, represents, a real social problem. In France, retirement homes (EHPAD) are the main actors in the public policies implemented to face these demographic trends. This thesis aims at providing a framework of comprehension and guidelines for the development of professional competencies of nursing staff in retirement homes (EHPAD). In this context it is tackling the practices of work and organization which can contribute to the learning and the development of competencies, through an exploratory research which is based on case studies, the analysis of social representations of the actors and the analysis of the concrete situations of care of the elderly. Our results and analysis showed professional practices in shift with the systems of thoughts and beliefs of nurses and in addition, the complexity of the care to the elderly and ethical problems involved in this care. On the conceptual level, our research enabled us to build a model to analyze and evaluate the practices of development of competencies in the process of care of the very old people. If this model could be applied only to retirement homes in France, an investigation carried out in USA enabled us to collect information on some foreign practices of care of old people and provides a perspective to our research

Full textThis research work is grounded in the development and evaluation of congregate housing concept carried by MEDeTIC non profit organization. Its purpose is to answer the need of elderly people who wish to successfully age in place by providing evolutionary ambient assisted living services at home. In order to support MEDeTIC developments, the ambition of our work was to engage a psychosocial diagnosis of elderly people needs concerning home, technologies and services. Through the focus of psychosocial and ergonomics tools, these three sources of innovations are conjointly analyzed in the present dissertation in order to identify what is hindering and driving congregate housing implementation (studies A1, A2, A3). Providing acceptability issues, this thesis firstly analyzes on the one hand service functionalities compatibility with activity limitations encountered by elderly people (study B1) and on the other hand familiar experience with technologies (study B2). Secondly, experiments C1 and C2 focus on model prediction and on individual (i.e. age, experience), socials (i.e. care givers opinions) and organizational (i.e. voluntary service usage settings) determinants in order to apprehend cognitive and social processes that drive usage of technical services by the elderly. Based on our findings, practical recommendations are address to MEDeTIC non profit organization in order to foster its evolution

Hence, the objective of our study would be to draw up a comparative, prospective and long-term inventory and statement of state of the ageing of the regions under consideration (Languedoc-Roussillon, PACA, Auvergne, Rhône-Alpes) and to measure their geographic inequalities. At the same time as the list is established, it is necessary to localize and to define the state of health of old and even very old people. Let us underline that the geography of the ageing process is fit to supply precious indicators for the organization of medical cares, to show whether such functioning is adapted or not, to reveal the inequalities or the insufficiencies, and to interprete the disparities in prescriptions, in medical practises. Its compared nature appears to be helpful for the evaluation of the different types of answers to these questions, in accordance with the politics of elderness

Full textAt the end of the 19th century, the collective management of old age was, fundamentally, one of the forms taken by the policies directed at the poor. The extension of the pension system enabled the emergence of the "retired", as a category of perception associated to the idea of rebirth after an active life. But a number of old people experience the progressive deterioration of their physical and mental faculties, and therefore of their autonomy in the everyday life, paving the way for dependency. Such biographical crises lead to a renewed questioning on how to manage a population of "improdutive" old people, wich is framed within the new category of thought linked to the equation of "old age" and "retirement", involving a human vision of care ("respect", "life project", "individual tailoring of care", "care giving"). Within the management structures for dependent elderly people, this evolution is especially felt through the tension between an enchanted involvement that leads to perceive care for dependent people as an opportunity to establish affective relationships and/or to satisfy humanistic leanings, and on the other hand, and the "unglamorous" realities of care giving to the elderly, wich make it difficult to maintain enchantment

Full textAlthough there is abundant literature on the link between physical activity and cognitive aging, several of these studies aroused questions about the choice of tools to assessed physical activity, the collection of physical activity, and methods of statistical analyses. In this context, we studied the link between physical activity, dementia, and cognitive functions in the elderly, by analyzing the different types of physical activity through questionnaires specific to the elderly, while keeping the greatest statistical power thanks to models adapted to longitudinal data. Our results have highlighted the difficulty of quantifying physical activity in the elderly and analyzed all the physical activities specific to the elderly and not only sports. The results obtained on the link between physical activity and cognitive aging allow us to question the choice of the most relevant indicators that should be used, but especially the place of lifestyle activities (domestic and transport). Our work contributes to the debate on the implementation of prevention policies on dementia and wider on aging and the autonomy of the elderly which must be preserved

Full textFor many years, research in the field of aging has focused on understanding this complex phenomenon and identifying solutions to medical, economic, psychological or sociological issues linked to it. In this context, the aim of this thesis was to identify factors that facilitate the use of video games as a medium for cognitive stimulation in older adults. To answer this question, two studies were carried out: the first one focused on exploring the gameplay of the Nintendo Wii, and the second one aimed at determining the factors of acceptability for video game in older adults. The work on the Wii was focused on the criterion of learnability, based on the theoretical framework of usability in Human Machine Interfaces. The objective was to analyze the data collected throughout a behavioral experimentation in order to assess the process by which the older adults were able to master the game (either bowling or tennis). Participants had to play four sessions and each session encompassed two sets of each game. Three groups of participants were implemented: younger patients, older adults without cognitive impairment and suffering from Mild Cognitive Impairment respectively. Three patients suffering from Alzheimer's disease also participated in this study. The results confirm an effect of age, which is moderated by the learning effect. The effect of the cognitive profile was more subtle. The analysis of the data for each volunteer with dementia showed that positive results in the games were probably due to the preservation of implicit motor learning, but also required the mandatory presence of a human mediator. The investigation on older adults' representations of entertainments "dedicated to young people" was carried out in two focus groups. The aim was to let older adults play with different modern games, and explore the world of video games from different angles, including gameplay, ethical issues or financial investment. Different themes were brought up in order to encourage free discussion between participants. The collected material was subsequently subjected to thematic content analysis: the different items of material collected during the two focus groups were labeled with key words, and then the tags that we obtained were used to build a theme tree. The results of this study showed that older adults had various interests in video games, but a strong desire to preserve their cognitive functions. For older adults, fun is possible when playing videogames but seems to be linked to very heterogeneous criteria. However, our results showed a consensus among older adults on criteria for appropriate games such as stimulating thinking, creativity and new knowledge. From a more pragmatic point of view, there is a clear refusal to invest in specific equipment for games. Finally, games like First Person Shooter, put forward because of their potential benefit on visual and attentional functions, were rejected because of the violence associated with the gameplay. In the last part of this work, we compared our results with those of the literature in order to identify our contributions as well as the experimental limits of our studies. Then our reflection focused on a summary of guidelines for the implementation of games aiming at supporting cognitive training or rehabilitation. We also analyzed the research questions and issues identified at the end of this intellectual and human process

Full textThis PhD thesis aims to provide greater insight into the characteristics of depression in demented residents in nursing home wards. It was initially made an evaluation of clinical characteristics of depression in patients with dementia living in nursing homes of the Limousin region (study DNPI-D) and a second time, to construct and validate an instrument for rapid use and convenient for healthcare staff in these institutions to identify elderly patients with depression (study EDDI). The study "Depression and behavioral disorders in dementia (DNPI-D)" was a cross-sectional study conducted between 2005 and 2006 in 17 nursing homes in the Limousin region. All subjects meeting the inclusion criteria were given a standardized evaluation that included an assessment of neuropsychiatric symptoms (Neuropsychiatric Inventory), depression (Cornell Scale for Depression in Dementia) and cognition (Mini Mental State Examination). In total, 319 patients with dementia were included and 137 (42. 9%) had depression. The disorders most correlated with depression among institutionalized demented subjects were disinhibition, irritability, agitation and anxiety. The study "Nursing homes short depression inventory (NH-SDI)" was performed in 2 stages. This tool has been created from an evaluation of 328 patients randomly selected in 17 nursing homes in the Limousin. A large variety of instruments designed to measure depression was examined by an expert group of psychogeriatricians in order to select item content. Experts have identified four tools studied depression and a behavioral disorder. These instruments should reflect the fatigability of elderly, have an easy to use and exist in validated French version. In total, the construction of EDDI has developed a scale containing 16 dichotomous items. Validation of EDDI has been performed in 99 patients resident in retirement homes of the Limousin or Marseille. This validation has 3 stages (a psychiatrist made the first diagnosis of depression according to DSM-IV (gold standard for comparison) and the EDDI has been performed by the nurses during their staff and finally, a trained interviewer spent the Cornell scale for Depression in Dementia (CSDD) and the Mini Mental State Examination (MMSE)). The EDDI has a satisfactory internal validity with Cronbach's α coefficient > 0. 80, a sensitivity of 86. 5% and a specificity of 85. 1% for score threshold of 5 / 6. Recognition of depression in dementia is a complex task for healthcare staff and, as a consequence, under‐recognition is high. This is partly caused by an overlap of some of the symptoms of dementia with prominent symptoms of depression. Improved detection of depression in these patients by the development of tools for using quick and practical for healthcare staff is essential

Full textThe objective was to analyse the indoor air quality in dwellings occupied by old people and to correlate pollutants with life habits and health statement. A sociological survey on 96 elderly people living in social housing was firstly undertaken in order to determine risk factors responsible of poor health. Then measurements of several pollutants were carried out in six typical dwellings. Results were analyzed at the light of activities and reported symptoms. Besides discomfort due to CO2 accumulation, several pollutants with levels near or over guidelines were identified in particular particulate matter under 1 um, airborne microbiological counts and permanent TVOC levels. Dust was at the origin of cough, eyes and throat irritations and flows of the nose, fungi and bacteria seems to be responsible of skin irritations, digestive disorders, sneezes and rhinitis. However, most of symptoms appeared after 10 hours of exposure time for people of all ages
