This chapter provides information about a genetic counselor’s role in prenatal screening, including discussing and offering options to a patient, interpreting and providing results, or managing referrals based on abnormal results. It discusses how a screen is evaluated using sensitivity, specificity, positive predictive value, negative predictive value, and personal utility. It provides a detailed description of both maternal serum screening and cell-free DNA testing. The maternal serum screening discussion includes information on multiples of median, calculating risk, timing, pattern association, limitations, and follow-up. The review of cell-free DNA testing includes fetal fraction, methodology, test performance, limitations and considerations for testing, and follow-up. Abstract:

According to the narrative crisis model, as people with trait vulnerability to suicidal behavior experience stressful life events, they develop a perception of their life story as moving toward “the dead end,” which gives rise to the acute suicide crisis syndrome. This chapter details the suicidal narrative component of the narrative crisis theory, which organizes the common themes of suicidal narrative into seven phases that follow a coherent life story of progressive failure and alienation until the future becomes impossible: Setting up unrealistic life goals, entitlement to happiness, humiliating personal or social defeat, failure to redirect to more realistic goals, perceived burdensomeness, thwarted belongingness, and perception of no future. A person’s perception of his or her life in terms of coherent suicidal narrative is associated with imminent suicide risk. This chapter contains an interview algorithm to probe the suicidal narrative, three representative case examples, and a test case.

Abstract:

Diagnostic disclosure of Alzheimer’s disease is generally accepted to be good practice; however, there are important factors for clinicians to consider when preparing to impart such a serious diagnosis. In particular, this chapter explores the issues around timing of the disclosure and determining the correct amount of information to give. It highlights the need for good knowledge of the individual—knowledge of the person’s current mental capacity, the person’s own views, as well as those of the family—in order to provide really good care. It also addresses issues of uncertainty such as making a very early diagnosis and giving an accurate prognosis. The chapter suggests that it is the person (rather than the professional) who should control the situation, and that professionals must be attuned to what the person and family require from them. It also highlights the need for sound post-diagnostic support and follow-up.

Abstract:

The Montreal study was the first to conduct a long-term follow-up for children with ADHD into adolescence and adulthood. Contrary to the conventional belief at the time, the study found that the children did not generally outgrow ADHD as they entered adolescence. Many continued to have symptoms of the disorder and to show functional impairments, such as academic underachievement, employment instability, antisocial behavior, and increased rates of psychiatric comorbidity. Outcomes in adulthood fell into three general categories: (1) ~30% had fairly normal functioning; (2) ~60% had continuing problems with ADHD symptoms, as well as work, family, interpersonal, social, and emotional difficulties; (3) ~10% had serious psychiatric and antisocial pathology, problematic substance use, and criminality. Outcome predictors were identified, including personal characteristics (e.g., symptom severity, IQ), family characteristics (e.g., parenting styles), and social context (e.g., peer relationships).

Abstract:

Beyond the Doctrine of Man responds to the question of how individuals and communities can live and have lived beyond the way the human person is defined in colonial modernity. This volume brings together essays that interrogate the problem of modern/colonial definitions of the human person and that take up the struggle to decolonize these descriptive statements. As the problem of coloniality transcends disciplinary constructions, so do the contributions in this book. They engage work from various fields, including ethnic studies, religious studies, theology, queer theory, philosophy, and literary studies. The essays in Beyond the Doctrine of Man were catalyzed by Sylvia Wynter’s questioning of modern/colonial descriptions of the human person. Wynter asks this question within a larger project of unsettling and countering these definitions. Contributors to this collection follow in this move—sometimes in direct reference to Wynter’s work and sometimes primarily focusing on the work of others—of asking how Western modernity has naturalized itself through a discourse on the human. This analytical work taken up by contributors is at the service of unsettling and countering this naturalization.

Abstract:

Community Health Workers (CHW) are an important cadre of health care personnel, particularly in impoverished countries. CHW program began with China’s “Barefoot Doctor” campaign. The Barefoot Doctors were lay people educated to provide basic medical education and care in rural China in the 1950s–1970s. CHWs have evolved over the years and now perform a variety of functions in many countries, from education to prevention, case finding to treatment, and referral to follow-up. This cadre is often used to fill the gap in human resources. There are a number of challenges in scaling up and sustaining CHW programs, including the adequate training, compensation, and supervision of CHWs within the health system. This chapter will begin with a historical look at the roots of CHW programs, explore their current status, and address some of the challenges inherent in fully utilizing the potential of this group of health workers.

Abstract:

If morality could plausibly have emerged in the manner described, then various lessons follow for ethics. In moral metaphysics, that ethics presupposes only a naturalistic basis and that the desire to be moral can be associated with the desire to live up to the persona we each project in speaking for ourselves in avowals and pledges. In moral semantics, that ethical judgments may be true or false, and that ethical terms may ascribe bona fide properties, despite having a wholly naturalistic base. In moral epistemology, that our ability to make judgments of desirability and responsibility, as well as other moral judgments, depends on our being immersed in practices like those of avowal and pledging. In moral psychology, that moral judgments are closely tied up with desire and that they are effective in motivating us, not in their own right, but in virtue of the robustly attractive desiderata that they rely on for support. And in moral theory or normative ethics, that it is perfectly understandable why in the ordinary world, moral thinkers should divide on issues like that between consequentialist and non-consequentialist approaches.

Persistent pain is an unpleasant sensory and emotional experience that continues for a prolonged period of time and that may or may not be associated with a recognizable disease process. Older people may suffer from conditions such as knee osteoarthritis, low back pain, neck pain and headache, neuropathic pain, fibromyalgia, and cancer-related pain. Pain may impact physical function, psychological function, and other aspects of quality of life. A thorough history and physical examination may optimally assess a person with a persistent pain complaint in the context of a multifactorial pathway from accumulated impairments in multiple systems. Older people may perceive that integrative medicine treatments based on lifestyle and lower-risk therapies may help them get relief from pain and improve quality of life. Follow-up evaluation of positive and negative effects of therapeutic modalities and medications may guide the treatment plan.

Abstract:

In the United Kingdom, >4,000 people die of bladder cancer every year. This reflects around one-third of affected patients and occurs in those with primary metastatic disease, with invasion at presentation, and in persons whose tumour progresses to invasion from non-invasive disease. The outcome from invasive cancers has not dramatically altered over the last 30 years, due to a lack of screening programmes, a lack of advances in treatment, and the fact that many patients present with tumours at an advanced stage. Around 50% of patients with invasive disease die from bladder cancer despite radical treatment, suggesting the disease is metastatic at presentation. Cure is rarely possible in patients with locally advanced tumours and lymph node metastases. Therapeutic options include systemic chemotherapy and salvage radical treatment for responders or palliation. Following radical cystectomy for cancer, patients require lifelong follow-up for both oncologic and functional reasons.

Abstract:

Psychodrama is a method of research and intervention in interpersonal relationships. Objective: The aim of the present study is to investigate and train group relationships in a multidisciplinary healthcare team. Material & Methods: Participants: The study population included employees who work in a Basic Family Health (UBSF), which was randomly selected. This facility was located in a medium-sized city within the state of São Paulo. We used the following tools: Form of Profile Survey and Interview Guide associated to Socio-demographic Data, Relational Functioning, and the team’s Sociometric choices. The researchers designed other Instruments of Protocol. Procedure: The participants responded to the study instruments and subsequently they underwent the Role Playing Program. They were re-evaluated at the end of the Development and Training Roles Program and reassessed at the end of the program. Patients show good clinical evaluation free of complications in a 60-day follow-up. Conclusions: The results show changes and improvements in the personal lives of those involved in their performance at work, and in the creation of coping strategies due to the professional role. Our results also indicate the importance of continuous and permanent training to maintain the properly functioning of the team. The participants also need a greater time to achieve internal and subjective changes identified with the intervention. We achieved the proposed objectives, which were as follows: effectiveness of the sociopsychodramatic methodology in groups regarding training and role play and changes in interdisciplinary relationships. However, more research on a case by case basis is recommended in order to generalize the results.

Abstract:

Screening and assessment are a core component of psychiatric care in any setting. In jails and prisons, the process, structure, content and timing of screenings and assessments are vital parts of the healthcare system. While the number of incarcerated persons is clear, the actual number of incarcerated prisoners who suffer from a mental disorder or independent psychiatric symptoms is difficult to determine because of methodological issues (e.g., different definitions of mental illness, different thresholds of severity, etc.) as well as wide variation in the nature (e.g. prison, jail, police lockup), size, and mental health service delivery systems of various settings. However, despite differences in methodology, geographic area, and other issues (e.g., types of facility, when studies were conducted, etc.), virtually every relevant study has concluded that a significant number of prisoners have serious mental illnesses and that the numbers of mentally ill prisoners are increasing. Because people with mental illnesses are at risk of suicide and exacerbations of their mental illnesses, correctional institutions need to identify such persons in a timely manner and provide appropriate clinical interventions. This chapter addresses the initial mental health screening of persons entering prisons and jails, with a special emphasis on suicide risk screening and follow-up clinical assessments of prisoners whose receiving or intake screening results suggest the likelihood that treatment or suicide prevention efforts will be necessary.
