Abstract:
Dalam menjaga kestabilan dari kondisi bayi premature dibutuhkan perawatan yang intensif dikarenakan belum matangnya organ dari bayi. Untuk menjaga kestabilan dari frekuensi nadi diperlukan adanya sebuah intervensi keperawatan yakni Gentle Human Touch. Penelitian ini bertujuan untuk melihat bagaimana pengaruh gentle human touch terhadap tekanan darah pada bayi premature dengan menggunakan metode penelitian kuantitatif dengan desain Quasi Experimental mengujicobakan suatu intervensi pada sekelompok subjek dengan atau tanpa kelompok pembanding, namun tidak dilakukan randomisasi untuk memasukkan subjek ke dalam kelompok perlakuan atau kontrol dengan jumlah sampel sebanyak 9 orang di ruang perinatologi RS Ibnu Sina Makassar. Hasil penelitian memperlihatkan bahwa p value : 0.001 yang menandakan bahwa ada pengaruh gentle human touch terhadap frekuensi nadi bayi premature di ruang perinatology RS Ibnu Sina Makassar. Diharapkan kepada tenaga perawat yang bertugas pada ruang perinatology untuk dapat menerapkan intervensi ini.

The development of science and technology and in parallel with this, advancements in new applications in health area, confront nurses with ethical problems. It is considerably important that nurses decide based on the ethical problems and ethical principles they face and the effective professional care. Perinatology nursing are the area, in which the ethical dilemmas are most experienced. Due to the fact that nurses do not mostly have freedom of choice and the environmental factors, they are obliged to make a choice between professional obligation and ethical responsibility Approach in obstetric care has to be conducted by considering the health of both mother and baby. Although the legal regulations, policies, and cooperation with women organizations concerning women enhance quality of care, they influence nursing applications. While the nurses working in the area of perinatology realize the consultancy, educator, and care roles in their every sorts of intervention and decisions in presenting health service, it is highly important for them to behave in the framework of ethical principles. Due to the fact that nurses are the closest health profession members to the patient, have the changing roles and responsibilities in their working areas, and sometimes make decisions and applications on the name of patients, they face to the ethical problems and experience confliction. Therefore, nurses need guidance and support about solution of ethical problems. In this study; ethical dilemmas the nurses working on the perinatology area face and how to be acted in the face of these dilemmas were discussed. ​Extended English summary is in the end of Full Text PDF (TURKISH) file. Özet Bilim ve teknolojinin gelişmesi, buna paralel olarak sağlık alanında yeni uygulamalarda ilerlemeler hemşireleri etik sorunlarla karşı karşıya bırakmaktadır. Hemşirelerin, karşılaştıkları etik sorunlara etik ilkelere dayanarak karar vermesi ve etkili profesyonel bakım oldukça önemlidir. Perinatoloji hemşireliği etik ikilemlerin en çok yaşandığı alandır. Hemşirelerin çoğu kez seçme hakkının olmaması ve çevresel faktörler nedeniyle kendilerini mesleki yükümlülük ve ahlaki sorumluluk arasında ikilemde bulmalarına neden olmaktadır. Obstetrik bakımda yaklaşım hem annenin hem de bebeğin yararı düşünülerek yapılmalıdır. Kadınları ilgilendiren yasal düzenlemeler, politikalar, kadın kuruluşlarıyla işbirliği yapılması bakım kalitesini artırmakla birlikte hemşirelik uygulamalarını etkilemektedir. Perinatoloji alanında çalışan hemşireler sağlık hizmeti sunmadaki her türlü müdahale ve kararlarında, aynı zamanda danışmanlık, eğitici ve bakım rollerini gerçekleştirirken etik ilkeler çerçevesinde davranış göstermeleri oldukça önemlidir. Hemşireler hastaya en yakın sağlık meslek üyeleri olmaları, çalışma alanlarında değişen rol ve sorumlulukları, kimi zaman hastalar adına karar verme ve uygulamalarda bulunmalarından dolayı etik sorunlarla karşı karşıya kalmakta ve çatışma yaşamaktadır. Bu nedenle etik sorunların çözümü konusunda hemşirelerin rehberliğe ve desteğe gereksinimleri vardır. Bu derlemede perinatoloji alanında çalışan hemşirelerin karşılaştıkları etik ikilemeler ve bu ikilemler karşısında nasıl hareket edilmesi gerektiği amacıyla gerçekleştirilmiştir.

Kegiatan penyuluhan ini dilakukan dengan memberikan materi tentang Perawatan Bayi BBLR dan Pelaksanaan Metode Kangguru, Melakukan sesi tanya jawab dan evalausi. kegiatan ini diikuti oleh 14 peserta orang tua yang memiliki bayi BBLR dirawat Diruang Perinatologi. Hasil Kegiatan. Terdapat peningkatan 88,7% pengetahuan terkait Perawatan Bayi BBLR dan Pelaksanaan Metode Kangguru.Abstract: The effectiveness of BBLR infant care and the implementation of the kangaroo method depends on the knowledge of the parents. Knowledge is given until people are able to do it independently and then BBLR babies are allowed to go home. Activity Objectives: Increase Knowledge about BBLR Infant Care and the Implementation of the Kangaroo Method. Activity Method: Through counseling conducted in the Perinatology Room with lecture and role play methods. This outreach activity was carried out by providing material on BBLR Baby Care and the Implementation of the Kangaroo Method, Conducting question and answer sessions and evaluations. This activity was attended by 14 participants, parents who have LBW babies who are treated in the Perinatology Room. Activity Results: There was an 88.7% increase in knowledge related to LBW Baby Care and the Implementation of the Kangaroo Method.

Perinatal death is the birth of life is the perfect outcome of conception from the mother who has been separated has signs of his life regardless of age of pregnancy. This study aims to study the relationship of low birth weight babies (BBLR) with perinatal mortality in the Perinatology Room RSUD. dr. M. Yunus Bengkulu. This research use cross sectional design. Population in this research all newborn in perinatology room of RSUD dr. M. Yunus Bengkulu in January-December 2016 as many as 653 people consisting of 293 people with LBW and 360 people not BBLR. Proportional sampling random sampling technique so that the number of samples will be taken as many as 87 people who consist of 39 babies babies with LBW and 48 babies who are not LBW. The data collection in this study used secondary data by looking at documentary documentation in the Perinatology Room of RSUD. dr. M. Yunus Bengkulu. Data analysis technique done by univariate and bivariat analysis with Chi-Square statistical test (c²) through SPSS program. The results of the study were : there were 52 (59.8%) infants who did not experience perinatal death, 48 (55.2%) of babies born not BBLR, there was a significant correlation between low birth weight infant with perinatal mortality in room perinatologi RSUD. dr. M. Yunus Bengkulu with moderate category, and low birth weight babies were at risk 5 times perinatal mortality when compared with infants who are not low birth weight (LBW). It was expected that midwives could provide counseling to pregnant women to routinely check pregnancy, so that later could reduce complications arising from pregnancy in mothers who were at high risk.Keywords : baby, low birth weight, perinatal death

Abstract:
Although modem neonatal surgery is a very young subject, it started only after the second world war, the progress is remarkable. The well intergrated interdisciplinary team among pediatric surgeons, pediatricians, neonatologits, anaesthetists, and other relevant specialists made this progress possible. Neonatal surgrey has been influenced by the advances in prenatal diagnosis. Neonatal surgical emergencies are related to perinatal conditions such as better antenatal care, including ultrasonogram examination, encourage the high risk pregnant women to deliver at pediatric/perinatal centers where obstetricians, pediatricians or neonatologist, pediatric surgeon, anesthetist are working as a team. Prenatal diagnosis has been one of the most challenging aspects in perinatology, and may directly related to the task of the pediatric surgeons to salvage certain type of malformations. Some experience of perinatal surgery is also discussed.

Children, especially infants, are more susceptible to diseases and unhealthy living conditions. Therefore, various efforts have been made by various parties to improve the health of infants, especially infants during the perinatal period (Ministry of Health Republic of Indonesia, 2018). Based on data from Dr. RSUD's medical records H. Abdul Moeloek from 2017 - 2019 saw an increase in the incidence of Neonatal sepsis in the Regional General Hospital (RSUD) Dr. H. Abdul Moeloek, where in 2017 there were 193 incidents, in 2018 there were 242 incidents and in 2019 there were 317 sepsis neonatorum incidents.Purpose : to find out the factors related to the occurrence of Sepsis Neonatorum in the Perinatology Room of RSUD Dr. H. Abdul Moeloek Lampung Province in 2020.Methods : This type of quantitative analytic research with a case control approach, the subjects in this study were all babies born in 2019 in RSUD Dr. H. Abdul Moeloek Lampung Province in 2020. The object of the study was the incidence of Sepsis Neonatorum, premature rupture of membranes, infection during pregnancy, LBW, and the sex of the baby. The research will be carried out after the proposal is approved.Research Result : It is known that out of 178 respondents, 144 (80.9%) of respondents did not have sepsis neonatorum, most of the respondents were 113 (63.5%) without PROM, most of the respondents had no infection as much as 158 (88.8%). Most of the respondents who were LBW were 99 (55.6%). There is a relationship between premature rupture of membranes and Sepsis Neonatorum in the Perinatology room of Dr. H. Abdul Moeloek, Lampung Province in 2020, (p value 0,000 OR 17,400) There is a relationship between infection during pregnancy and Sepsis Neonatorum in the Perinatology room of Dr. H. Oleh sebab itu, dilakukan berbagai upaya oleh berbagai pihak untuk meningkatkan kesehatan bayi, terutama bayi pada masa perinatal (Kemenkes RI, 2018). Berdasarkan Data Rekam medis RSUD Dr. H. Abdul Moeloek dari tahun 2017 — 2019 terlihat peningkatan kejadian sepsis Neonatorum di Rumah Sakit Umum Daerah (RSUD) Dr. H. Abdul Moeloek, dimana tahun 2017 terdapat 193 kejadian, di tahun 2018 terdapat 242 kejadian dan di tahun 2019 terdapat 317 kejadian sepsis neonatorum.Tujuan Penelitian : untuk mengetahui faktor-faktor yang berhubungan dengan kejadian Sepsis Neonatorum di ruang Perinatologi RSUD Dr. H. Abdul Moeloek Provinsi Lampung tahun 2020.Metode Penelitian : Jenis penelitian analitik kuantitatif dengan pendekatan cros sectional, subjek dalam penelitian ini adalah seluruh bayi yang lahir di tahun 2019 di RSUD Dr. H. Abdul Moeloek Provinsi Lampung tahun 2020. Obyek dalam penelitian adalah kejadian Sepsis Neonatorum, ketuban pecah dini, infeksi saat hamil, BBLR, dan Jenis kelamin bayi. Penelitian akan dilaksanakan setelah proposal disetujui.Hasil Penelitian : Diketahui bahwa dari 178 responden, sebanyak 144 (80,9%) responden tidak Sepsis neonatorum, sebagian besar responden Tidak KPD sebanyak 113 (63,5%), sebagian besar responden tidak ada infeksi sebanyak 158 (88,8%). Sebagian besar responden BBLR sebanyak 99 (55,6%). Ada hubungan ketuban pecah dini (p value 0,000 OR 17,400) Ada hubungan infeksi (p value 0,001 OR 5,583). Abdul Moeloek, where in 2017 there were 193 incidents, in 2018 there were 242 incidents and in 2019 there were 317 sepsis neonatorum incidents.Purpose : to find out the factors related to the occurrence of Sepsis Neonatorum in the Perinatology Room of RSUD Dr. H. Abdul Moeloek Lampung Province in 2020.Methods : This type of quantitative analytic research with a case control approach, the subjects in this study were all babies born in 2019 in RSUD Dr. H. Abdul Moeloek Lampung Province in 2020. The object of the study was the incidence of Sepsis Neonatorum, premature rupture of membranes, infection during pregnancy, LBW, and the sex of the baby. The research will be carried out after the proposal is approved.Research Result : It is known that out of 178 respondents, 144 (80.9%) of respondents did not have sepsis neonatorum, most of the respondents were 113 (63.5%) without PROM, most of the respondents had no infection as much as 158 (88.8%). Most of the respondents who were LBW were 99 (55.6%). There is a relationship between premature rupture of membranes and Sepsis Neonatorum in the Perinatology room of Dr. H. Abdul Moeloek, Lampung Province in 2020, (p value 0,000 OR 17,400) There is a relationship between infection during pregnancy and Sepsis Neonatorum in the Perinatology room of Dr. H. Anak terutama bayi, lebih rentan terhadap penyakit dan kondisi hidup yang tidak sehat. Oleh sebab itu, dilakukan berbagai upaya oleh berbagai pihak untuk meningkatkan kesehatan bayi, terutama bayi pada masa perinatal (Kemenkes RI, 2018). Berdasarkan Data Rekam medis RSUD Dr. H. Abdul Moeloek dari tahun 2017 — 2019 terlihat peningkatan kejadian sepsis Neonatorum di Rumah Sakit Umum Daerah (RSUD) Dr. H. Abdul Moeloek, dimana tahun 2017 terdapat 193 kejadian, di tahun 2018 terdapat 242 kejadian dan di tahun 2019 terdapat 317 kejadian sepsis neonatorum.Tujuan Penelitian : untuk mengetahui faktor-faktor yang berhubungan dengan kejadian Sepsis Neonatorum di ruang Perinatologi RSUD Dr. H. Abdul Moeloek Provinsi Lampung tahun 2020.Metode Penelitian : Jenis penelitian analitik kuantitatif dengan pendekatan cros sectional, subjek dalam penelitian ini adalah seluruh bayi yang lahir di tahun 2019 di RSUD Dr. H. Abdul Moeloek Provinsi Lampung tahun 2020. Obyek dalam penelitian adalah kejadian Sepsis Neonatorum, ketuban pecah dini, infeksi saat hamil, BBLR, dan Jenis kelamin bayi. Penelitian akan dilaksanakan setelah proposal disetujui.Hasil Penelitian : Diketahui bahwa dari 178 responden, sebanyak 144 (80,9%) responden tidak Sepsis neonatorum, sebagian besar responden Tidak KPD sebanyak 113 (63,5%), sebagian besar responden tidak ada infeksi sebanyak 158 (88,8%). Sebagian besar responden BBLR sebanyak 99 (55,6%). Ada hubungan ketuban pecah dini (p value 0,000 OR 17,400) Ada hubungan infeksi (p value 0,001 OR 5,583). Abstract:
Latar Belakang : Prematuritas merupakan penyebab kematian kedua pada balita setelah pneumonia dan merupakan penyebab utama kematian neonatal. Tiga puluh lima persen kematian neonatal di dunia disebabkan oleh komplikasi kelahiran prematur. Salah satu bentuk intervensi keperawatan selain metode kanguru yang dapat dilakukan pada BBLR adalah nesting. Nesting digunakan untuk memberikan posisi yang tepat pada neonatus. Intervensi keperawatan tersebut menggunakan alat berbentuk seperti kondisi rahim ibu yang terbuat dari bahan yang halus yang berisi potongan kain seperti dacron. Panjang alat ini sekitar 121-132 cm dan dapat disesuaikan dengan panjang tubuh bayi. Alat ini diletakkan sebagai pelindung posisi bayi, sehingga berada dalam posisi ekstensi dan menjaga perubahan posisi bayi yang diakibatkan karena gravitasi. Masalah penelitian : masih banyaknya bayi premature yang keadaan fisiologisnya tidak stabil di Ruang Perinatologi RSUD dr. M. Yunus Bengkulu. Tujuan: Mengetahui pengaruh nesting terhadap perubahan fisiologis bayi premature di Ruang perinatology RSUD DR. M. Yunus Bengkulu. Metode Penelitian : Rancangan penelitian yang digunakan adalah quasi eksperiment dengan rancangan non equidment control group design dengan satu kelompok intervensi dan satu kelompok kontrol. Hasil Penelitian : Hasil penelitian di dapatkan Rata-rata saturasi oksigen pada kelompok kontrol adalah 96,22 sedangkan pada kelompok intervensi didapatkan rata-rata saturasi oksigen adalah 98,17 di Ruang Perinatologi RSUD dr. M. Yunus Bengkulu. Rata-rata frekuensi nafas pada kelompok kontrol adalah 85,44 edangkan pada kelompok intervensi didapatkan rata-rata frekuensi nafas adalah 97,61 di Ruang Perinatologi RSUD dr. M. Yunus Bengkulu. Rata-rata frekuensi nadi pada kelompok kontrol adalah 97,44 sedangkan pada kelompok intervensi didapatkan rata-rata frekuensi nadi adalah 110,50 di Ruang Perinatologi RSUD dr. M. Yunus Bengkulu. Ada pengaruh nesting terhadap saturasi oksigen pada bayi prematur di Ruang Perinatologi RSUD DR. M. Yunus Bengkulu dengan p value 0,007. Ada pengaruh nesting terhadap frekuensi nafas pada bayi prematur di Ruang Perinatologi RSUD DR. M. Yunus Bengkulu dengan p value 0,003 dan ada pengaruh nesting terhadap frekuensi nadi pada bayi prematur di Ruang Perinatologi RSUD DR. M. Yunus Bengkulu dengan p value 0,047. Antibiotic stewardship program (ASP) adalah strategi untuk mengurangi pemakaian antibiotik pada kasus sepsis neonatus awitan dini (SNAD) di unit perinatologi.Tujuan. Mengurangi pemakaian antibiotik di unit perinatologi dengan ASP. Metode. Penelusuran literatur elektronik PubMed, Cochrane dan Google Scholar dengan kata kunci ”antibiotic stewardship” atau “antibiotic duration” dan “perinatology” dan “neonatal sepsis” dalam 6 tahun terakhir (2013-2019). Hasil. Uji klinis acak terkontrol oleh Rohatgi dkk mencakup 132 bayi dengan SNAD, diberikan antibiotik empiris selama 7 dan 10 hari. Tidak ditemukan perbedaan bermakna pada durasi pemakaian oksigen, inotropik, transfusi darah dan waktu minum enteral penuh (p>0,05). Cantey dkk melakukan studi time series prospektif dengan menghentikan antibiotik setelah 48 jam terhadap 2502 bayi berat lahir <2100 gram dengan SNAD, hari pemakaian antibiotik menurun 27% (p<0,0001). Studi time series restrospektif pada 537 bayi dengan diagnosis SNAD oleh Grant dkk melakukan pemeriksaan c-reactive protein (CRP) pada usia 36 jam dan pemberhentian antibiotik empiris setelah 48 jam. Angka kepatuhan tenaga medis pada akhir penelitian mencapai 97,5%, dan pemakaian antibiotik lebih dari 48 jam menurun dari 50,4% menjadi 0,8% (p<0,0001). Kesimpulan. Antibiotic Stewardship Program dapat mengurangi pemakaian antibiotik untuk kasus SNAD pada unit perinatologi.
