Abstract:
This book presents a collection of chapters written by women psychiatrists working in, or who have recently worked in, the National Health Service (NHS). In addition, there are chapters by a medical journalist and a senior NHS mental health nurse manager, as well as short profiles of some women whose work has made a difference to psychiatry. The subjects covered are diverse and include descriptions of women’s roles in perinatal psychiatry, women-only units, forensic, old age, and intellectual difficulties services. There are chapters analysing roles of women as psychiatry trainees and trainers, as managers, and as doctors with mental illnesses. Chapters on the history of women in medicine and psychiatry are included. The modern legal framework for decision-making for vulnerable people is covered and the psychoanalytical perspective is particularly emphasized in chapters on historic abuse and the functioning of mental health units. Some chapters are autobiographical and some are biographical. This book is of interest to specialists and non-specialists alike and explores some of the main changes to mental health services in the twentieth century, from the perspective of women doctors. These chapters will help reduce stigma about mental illnesses and demystify the world of psychiatry. Young readers may be inspired to consider careers in this fascinating field that not only seeks to understand the human mind but also uses science and evidence to improve psychiatric treatments.

Abstract:
Much research has focused on understanding why women are at increased risk of serious mental health symptoms during pregnancy and the postpartum. Although psychosocial stressors play a major role in perinatal psychiatric disorders, not every woman who experiences adverse psychosocial circumstances develops a major psychiatric illness during this time. As such, attention has focused on exploring how biological factors might impact the development of perinatal psychopathology. This chapter reviews biological changes during pregnancy and the postpartum that may contribute to the onset and/or exacerbation of psychiatric symptoms and disorders in the perinatal period. It discusses heritability and genetics research suggesting that some women may have a biological predisposition to developing psychopathology in the perinatal period. Then, the chapter focuses on pregnancy- and childbirth-related biological changes in sex hormones; the neurotransmitter, endocrine, and immune systems; and sleep that may be contributing biological factors in perinatal psychopathology for women at risk.

Abstract:
Women and men with neurological disease more often suffer from depression in relation to pregnancy and delivery than other parents. Perinatal depression may harm the parent-child relationship as well as the health of the child. Postnatal psychosis, suicide, and infanticide are rare but severe consequences of the disorder. Symptoms of perinatal depression may overlap with symptoms of neurological disease. Both disorders may aggravate each other. Side effects from neurological treatment could mimic symptoms of depression, and antidepressive drugs could worsen neurological symptoms and interact with other treatment. Neurological patients should be evaluated for risk factors for perinatal depression before delivery. These include previous psychiatric disease, sexual or psychical abuse, sleep problems, high neurological disease activity, and low social support. Pregnant women with previous psychotic episodes or bipolar disease should be referred for psychiatric evaluation before delivery. All patients should be screened for depressive symptoms during follow-up using a 3-step method.

Cultural context is important to understanding cross-cultural difference in adjustment to pregnancy and the postpartum period. Culture is complex, with interrelated variables posing challenges for research. Highlighted with examples of research with women from Western, Eastern, Native, and Other cultures, the chapter discusses variables such as acculturation and acculturative stress, social support, religious and spiritual beliefs and practices, and help-seeking and utilization of services in perinatal mental health and adjustment. Although rates of psychiatric symptoms and disorders vary across cultures, postpartum depression is universal and most often reflected in the perinatal mental health literature. Research on interventions and services mainly examine Western approaches as standard models of health care; however, understanding cultural context can help to inform directions for intervention adaptations or tailoring through a “cultural lens.” There are growing segments of cross-cultural perinatal mental health research, but many gaps still remain.

Approximately 26% of postpartum women meet diagnostic criteria for a psychiatric disorder. Untreated psychopathology in the postpartum period is associated with a range of adverse outcomes for both infant and mother. Fortunately, the perinatal period provides an opportune time to intervene with and prevent postpartum psychopathology. Women have increased contact with health care providers during this time, providing an avenue through which access to prevention can be improved. Furthermore, with increased knowledge to identify high-risk women, preventive interventions can be delivered to assist both the woman and infant. Preventive efforts for postpartum psychopathology are aimed at modifying risk factors or protective factors to prevent the psychiatric disorder and primarily use three distinct approaches: universal, selective, and indicated. This chapter provides a review of the empirical research in the prevention of postpartum psychopathologies including postpartum depression, anxiety, bipolar disorder, psychosis, and addictions (tobacco, alcohol, and illicit drugs).
