BEFORE the introduction of decimal currency into the United Kingdom in 1971, the pound sterling was divided into twenty shillings, with twelve pence (pennies) to the shilling. The lowest value coin was the farthing (a quarter of a penny). The crown was five shillings—hence the popular 2s 6d coin known as the half-crown—while the guinea, much used in commercial transactions, was twenty-one shillings. Abstract:
Most patients who seek penile enhancement have a normally sized and functioning penis. Any man seeking penile augmentation benefits from a careful clinical assessment, together with counselling regarding the treatments options available and the results and complications of surgery. Indeed, for those with a normal penile size, careful psychological assessment is essential before any physical treatment should be considered. If a man perceives his penis to be too small, when actually it is of normal size, the condition is termed penile dysmorphophobia. However, demand for penile enhancement in men with a normally sized penis is increasing. Surgical outcomes are often disappointing, and there may be severe complications that can involve permanent disfigurement and impaired urinary and sexual function. Penile enhancement should therefore be offered only to patients with a true micropenis. We describe current surgical augmentation techniques with an emphasis on their cosmetic and functional outcomes.

Sexual stimuli (tactile, visual, olfactory, and imaginative) are processed and integrated in the central nervous system which then activates certain autonomic and somatic pathways within the peripheral nervous system. This coordinated activation of the central and peripheral nervous systems leads to penile erection which is actually a result of relaxation of vascular and cavernosal smooth muscle in the penis. In the flaccid (detumescent) penis, the smooth muscle tone is heightened. Penile erection (tumescence) requires a decrease in the smooth muscle tone. The tone of the penile smooth muscle therefore is the main determinant of erectile function. In this chapter, the current information on the control of erectile function by this central-peripheral-smooth muscle axis will be reviewed.

Abstract:
Penile cancers are rare primary malignancies located on the glans, foreskin, or shaft of the penis, excluding the urethra. The vast majority of penile cancers are epithelial tumors representing histological subtypes of squamous cell carcinoma (SCC). Most penile SCCs are believed to develop through pre-invasive lesions known as penile intraepithelial neoplasia and penile carcinoma in situ. They account for 0.1%–0.3% of all incident cancers (excluding non-melanoma skin cancers) in the United States and other developed countries and up to 1% of all cancers in some countries in sub-Saharan Africa. Penile cancers are rare in men younger than 40 years, and are typically diagnosed among men above age 60. The two most important risk factors are pathological phimosis and infection with high-risk types of human papillomaviruses (HPV), both of which are preventable conditions.

Male genitourinary trauma with penile loss or deformity can have devastating physical and psychosocial consequences for patients and their loved ones. The return of many servicemen from the recent conflicts in Iraq and Afghanistan with these types of injuries has brought this problem into focus. Traditional penile reconstructive techniques are often inadequate in addressing these injuries; penile transplantation may provide better outcomes. To date, however, only two penis transplants have been performed, with mixed results. To maximize the likelihood of success, a number of considerations must be addressed; these include optimizing the technical surgical approach and minimizing the risks associated with immunosuppression and rejection. The psychological implications of the procedure also must be taken into account, and a multidisciplinary team approach is of utmost importance.

Circumcision is a commonly performed operation. Regional analgesia is provided either by penile block or caudal block. Each approach to anesthetizing the penis has attendant risk and benefits. The provider has several decision points to observe when choosing which, if either, technique to apply. The basic anatomic factors (including the identification and significance of sacral dimples), technical steps, risks, and considerations of each are discussed. Overall, each block offers benefit to the patient, and choosing between them can be based on need for ambulation postoperatively, operator experience, and availability of appropriate equipment. When available, ultrasound can enhance the efficacy of penile nerve blocks.

Although the last decades have been characterized by the continuous evolution of reconstructive surgery and in particular of free tissue transfer techniques, the repair and reconstruction of the penis remains anatomically, functionally, and aesthetically a great challenge. This is due to the unique architecture of the penis and to the absence in the whole human body of an alternative tissue that could adequately replace the corpora cavernosa in terms of colour, texture, structure, and ultimately, function. The aim of this chapter is to describe the state of the art in penile reconstructive surgery with an emphasis on the cosmetic and functional outcome.
