Abstract:
Chapter 2 provides a historical overview of information and communications technology (ICT) development through the lens of political communication orders (PCOs) and political communication revolutions (PCRs). This chapter begins a focus on the first stage of the political communication cycle (PCC): the technological imperative. This historically rich chapter details the social and technological history of the four PCOs that have existed through American political history and the revolutions that disrupted them. First is the Elite PCO from the colonial era through the 1830s, when newspapers were printed for small, elite audiences. The Mass PCO emerged as printing technology and political access expanded in the early nineteenth century, creating the first mass media in the United States. Next, the Broadcast PCO grew out of the expansion of radio and later television use across the country. Finally, the Information PCO is linked to the internet and digital communication since the 1990s.

This chapter shifts the focus to the third and final stabilization phase of the political communication cycle (PCC). During the stabilization phase, a new political communication order (PCO) takes shape through the building of norms, institutions, and regulations that serve to fix the newly established status quo in place. This status quo occurs when formerly innovative political communication activities become mundane, yet remain powerful. Much of the chapter details the pattern of communication regulation and institution construction over time. In particular, this chapter explores the instructive similarities and key differences between the regulation of radio and the internet, which offers important perspectives on the significance of our current place in the PCC and the consequences of choices that will be made over the next few years.

Abstract:
This introduction serves several important goals. It lays out both the research objective and theoretical framework placing this study on an interdisciplinary foundation that combines work from political science, American political development, mass communication, history, and diffusion studies. It introduces the core concepts of the book, concentrated around a recurring multistage process called the political communication cycle (PCC). The three stages of the PCC, detailed in the following chapters, include the information and communications technology (ICT)–focused technological imperative phase; the political choice phase, which emphasizes the behavioral process central to innovation; and stabilization through the establishment of new norms, regulations, and institutions. This process has repeated throughout history, where long periods of relative stability, known as political communication orders (PCOs), are disrupted by shorter periods of permanent change, identified as political communication revolutions (PCRs). The introduction concludes by introducing the three claims that are used throughout the book and outlining the chapters that follow.

Abstract:
This chapter uses a case study approach to explore the political choice phase of the political communication cycle (PCC) over time. Focusing on interest groups and detailing the long history of these organizations in America, the chapter primarily examines innovations made by four of the largest interest groups in American history: AARP, the Sierra Club, the National Rifle Association (NRA), and MoveOn.org. These four interest groups have spanned multiple political communication orders (PCOs) and their overall lack of innovativeness until recent years is tied to the distinct nature of their shared political communication goals. These goals are far narrower than campaigns or social movements and therefore are much less likely to motivate innovative efforts. These trends have started to change in the internet-based era as new organizational and communication strategies have opened up interest groups to greater innovation along the lines of MoveOn.org.

Abstract:
This chapter explores how epilepsy can affect reproductive function. The National Institute of Health consensus definition of PCOS includes the presence of menstrual dysfunction, clinical evidence of hyperandrogenism, and exclusion of other endocrinopathies, such as Cushing’s syndrome and hypothyroidism. The etiology of PCOS is felt to be heterogenous, related to a complex interaction between both genetic and environmental factors. PCOS develops when the ovaries are stimulated to produce excessive testosterone. The diagnosis and pathogenesis of polycystic ovarian syndrome is explored; in particular, how valproate contributes to the condition in women with epilepsy. Valproate is the AED most associated with PCOS, as it can directly increase ovarian testosterone production. It can also cause weight gain leading to insulin resistance, another mechanism contributing to PCOS.
