Abstract:

Abstract:

Abstract:
Aims and methodThe patient's experience of the clinician is an increasingly important area in time of ‘consumer choice’ and appraisal of the individual practitioner. Validated, easy-to-use scales are scarce. The aim was to validate a user-friendly, brief scale measuring patient satisfaction (PatSat scale). Over three phases, patients were involved in developing and validating the scale against the Verona satisfaction subscale.ResultsA highly significant correlation was found between the two scales (Spearman's correlation coefficient 0.97, two-tailed P <0.001).Clinical implicationsThe PatSat is a new patient satisfaction scale validated in a psychiatric out-patient population. It appeared popular with patients and took less than 1 minute to fill in. The use of validated scales measuring patient satisfaction is a pivotal part of mental health delivery and advancing overall quality of care.

Abstract:
The pervasiveness of “stacking” between traditional and clean fuels/stoves is moving the household clean cooking research and policy agenda from programs that are centered on one-fuel or stove, to multi-clean fuel/device interventions that could result in a more effective displacement of traditional biomass stoves. However, there is little recognition and knowledge of the benefits realistic clean-stacking cooking solutions can have on indoor air quality and health. In this paper, particulate matter (PM2.5) and carbon monoxide (CO) concentration levels that are associated with stove-stacking options (Patsari-U-shaped open fire (U-type), Patsari-liquefied petroleum gas (LPG) stove, U-type-LPG, and Patsari-U-type-LPG) common within rural Mexico were evaluated while using a controlled cooking cycle (CCC) from the Purepecha Highlands, which mimics the most common dishes prepared daily within the region as well as their sequential arrangement. The results confirms that the WHO Interim Target-1 (IT1) of 35 μg/m3 for PM2.5 is exceeded whenever woodburning open fires are part of the stacking combination (Patsari-U-type 107 ± 9 μg/m3, U-type-LPG 131 ± 55 μg/m3 and Patsari-U-type-LPG 107 ± 22 μg/m3). However, well-designed and properly operated woodburning chimney stoves, such as the Patsari can meet the IT1, either used exclusively (21 ± 8 μg/m3) or as a “clean stacking” option with LPG (24 ± 5 μg/m3). Given stove stacking patterns, evaluating the health and environmental consequences of stove transitions while assuming the total replacement of traditional fires by clean cooking options will lead to misplaced expectations, and programs should evaluate more realistic “clean-stacking” options.

Abstract:
Objective. To establish thresholds for rheumatoid arthritis (RA) activity categories according to the RA Disease Activity Index-5 (RADAI-5).Methods.Three hundred ninety-two patients with RA were categorized according to Disease Activity Score 28-joint count (DAS28), Clinical Disease Activity Index (CDAI), and their satisfaction (PATSAT) with disease status. These measures built the basis for the calculation of disease activity limits for the RADAI-5. Patient assessments simultaneously meeting the identical DAS28, CDAI, and PATSAT categories were taken as the references to establish the thresholds for the respective RADAI-5 categories by calculating the third quartile of the corresponding RADAI-5 values. Subsequently, these new thresholds were applied to all assessments.Results.Seven hundred fifty-eight assessments in 392 patients (2 assessments median/patient) could be obtained, most patients having mild to moderate disease according to DAS28 and CDAI. Calculating the third quartile, the RADAI-5 thresholds were as follows: 0.0–1.4 for a remission-like state, 1.6–3.0 for mild disease activity, 3.2–5.4 for moderate, and 5.6–10.0 for high disease activity. Categorization according to the RADAI-5 showed a normal distribution, while DAS28 and CDAI were somewhat shifted to the left. DAS28 and CDAI levels, as well as tender and swollen joint counts and physician’s global assessment and erythrocyte sedimentation rate, proved to be highly significantly different within the different RADAI-5 categories (Kruskal-Wallis test p < 0.001).Conclusion.RADAI-5 thresholds for RA activity could be elaborated. Patient self-report questionnaires may substitute composite disease activity scores and may contribute significantly to improving documentation in routine patient care.

Abstract:
Abstract BACKGROUND Isocitrate dehydrogenase wild-type (IDHwt) gliomas are uncurable primary tumors of the brain and impose a high burden on affected patients and caregivers. In addition to neurological symptoms, changes in cognition and personality are common and should be diagnosed and treated early. About 75% of patients with brain tumors suffer from psychosocial distress at their first consultation. Health-related quality of life (HR-QoL) is significantly reduced by anxiety and negative mood, following glioma diagnosis. Brain tumor diagnosis also affects caregivers negatively. Recent publications show that caregiver and patient satisfaction influence each other, with caregivers generally being less satisfied than patients. In addition to specific tumor and symptomatic therapy, psycho-oncological care is an important element in a comprehensive treatment approach and may lead to increased HR-QoL and patient and caregiver satisfaction. However, there are few prospective datasets focusing on these aspects. MATERIAL AND METHODS We investigated the impact of psycho-oncological care on HR-QoL and patient and caregiver satisfaction in a large brain tumor center. Data from the regional brain tumor registry and electronic medical charts were used to compile the prospective patient cohort. Standardized questionnaires were used to investigate HR-QoL (EORTC QLQ C-30 and BN-20) and satisfaction (EORTC PATSAT-C33). Univariable and multiple linear regression analysis was applied to test the influence of psycho-oncological care on both aspects. RESULTS A homogenous cohort of 61 patients was investigated, of whom 29 (47.5%) received psycho-oncological care. We analyzed 141 EORTC QLQ-C30, 139 EORTC QLQ-BN20, 129 EORTC PATSAT-C33 questionnaires from patients, and 115 EORTC PATSAT-C33 questionnaires from caregivers. We show that patients who received at least one psycho-oncological consultation had significantly worse role functioning (p = 0.001) and a negatively influenced global health status (p = 0.038). Psycho-oncological care had no significant correlation with the satisfaction of patients. Caregivers of patients with psycho-oncological care documented inferior satisfaction with information exchange of doctors (p = 0.036). CONCLUSION We conclude that receiving psycho-oncological care is associated with changes in HR-QoL and patient and caregiver satisfaction. Utmost efforts should be made to integrate intensively and frequently psycho-oncological care on a daily-practice basis. It will be subject of future prospective studies to investigate kind and frequency of interventions that are suited to improve HR-QoL and treatment satisfaction in patients with IDHwt gliomas and their caregivers, and how to complement supportive therapies better to achieve optimal psychosocial outcomes for these patients and their caregivers.

Abstract:
AbstractPurposeThis work presents a study on the female involvement in patent applications in all 23 Ibero-American countries that are WIPO members, in order to measure gender inequalities in institutional collaborations and technological fields, across time.Design/methodology/approachThe data used in this paper come from EPO Worldwide Patent Statistical Database (PATSTAT). PATSTAT contains bibliographical data relating to more than 100 million patent documents from leading industrialized and developing countries, as well as legal event data from more than 40 patent authorities contained in the EPO worldwide legal event data (INPADOC). The extracted subset is composed of 150,863 patent applications with priority years between 2007 and 2016.FindingsOur observations indicate that even in more dynamic economies such as Portugal and Spain, the participation of women per patent applications does not exceed 30%. Additionally, the distribution of female participation among institutional sectors and technological fields is consistent with previous studies in other regions and indicate a socio-cultural divide.Research limitationsUnisex names were not considered and were counted as gender unknown, and patent applications for which no inventor information was available were discarded, but further effort of data analysis may provide more information about gender inequalities.Practical implicationsWhile patents are imperfect variables of inventive step and therefore should be considered as a variable proxy of innovation, our findings may help to guide the implementation of policies for balancing gender participation in innovative activities, as well as instigating research into the issues causing divisive participation along gender lines.Originality/valueWhile there is a widespread effort into evaluating and improving the participation of groups recognized as minorities within state-of-the-art activities, research about women participation in the innovation sector is fragmented due to differing regional characteristics: industrial and academic segmentation, socio-economic disparities, and cultural factors. Thus, localized studies present an opportunity of filling the gaps of knowledge on societal participation in innovation activities.

Abstract:
A field experiment on intercropping of okra and leafy vegetables was conducted at Bangladesh Agricultural Research Institute (BARI), Joydebpur, Gazipur and Agricultural Research Station, Burirhat, Rangpur during Kharif-I season of 2010 and 2011 to find out suitable crop combination for higher productivity and economic return. Seven treatments viz., sole okra (50 cm × 40cm), okra 100% (in row) + red-amaranth 100% (broadcast), okra 100% (in row) + red-amaranth 75% (broadcast), okra 100% (in row) + leaf amaranth 100% (broadcast), okra 100% (in row) + leaf amaranth 75% (broadcast), okra 100% (in row) + jute as patshak 100% (broadcast), okra 100% (in row) + jute as patshak 75% (broadcast) were used. Intercropping reduced okra yield but total productivity increased due to additional yield of vegetables. In both the locations, sole okra produced the highest yields (15.82 t/ha at Joydebpur and 13.79 t/ha at Burirhat). Among the intercropping treatments, the highest okra yields (15.42 t/ha at Joydebpur and 12.64 t/ha at Burirhat) were obtained from okra 100%+ red amaranth 75% combination. The lowest okra yield (13.16 t/ha at Joydebpur and 11.75 t/ha at Burirhat) was recorded in okra 100% + jute as patshak 100% combination. The highest okra equivalent yield (23.00 t/ha) was recorded in okra 100% + red amaranth 100% at Joydebpur and in okra 100% + leaf amaranth 100% (21.79 t/ha) at Burirhat. These treatment combinations also gave the highest gross margin (Tk. 227180/ha at Joydebpur and Tk. 214600/ha at Burirhat) and benefit cost ratio (5.66 at Joydebpur and 5.58 at Burirhat). The results revealed that cent percent red amaranth (broadcast) or leaf amaranth intercropped with cent percent okra might be suitable combination for higher productivity and economic return at Joydebpur and Burirhat, respectively. Abstract:

Abstract:
A traditional use of bioenergy is the main source of residential energy in developing countries, essentially using firewood to cook, boil water or heating affecting people in developing countries. Improved kitchens are more efficient and less polluting, and there is a need to evaluate different design options to facilitate their adoption. There are different types of very economical improved cook stoves with high combustion efficiency. In Mexico, multiple projects about cook stoves have been carried out, the root of this study being one of them, the Patsari cook stoves. The goal of this project was to modify previous designs to align with the habits and traditions. This study shows the importance of including the participation of the users in the design of the portable model of the Patsari cook stoves, applying Design Thinking methodology. Many designs or design changes have been done on cook stoves by different researchers, but users do not adapt to them. This article, through a field study, shows the importance of theoretical analyses along with user experiences to facilitate the adoption of these improves cook stoves and ensure their success.

Abstract:
Background:Patient-reported outcomes (PROs) reflect the patient’s perspective and are used in rheumatoid arthritis (RA) routine clinical practice. However, PROs may be associated with other aspects of health, such as psychological distress or comorbidities, which leads to situations of discordance with objective RA assessments. Objectives:The aim of this study was to determine whether PROs were associated with objective assessment of disease activity. Methods:We conducted a cross-sectional study including patients with RA (ACR/EULAR 2010). Demographic data were collected. The following PROs were assessed: number of nocturnal awakenings, morning stiffness duration, estimation of spontaneous pain and fatigue by Visual Analog Scale (VAS), and global patient assessment (GPA). In addition, patients rated their current satisfaction with their disease state according to the Austrian school mark system (PATSAT: 1=excellent, 2=good, 3=average, 4=moderate (fair), 5=unsatisfactory). Disease activity was assessed using the 28-joint disease activity score with erythrocyte sedimentation rate (DAS28 ESR) and C reactive protein (DAS28 CRP). We used Cohen’s kappa (κ) to determine the agreement between PATSAT and DAS28 ESR. The κ result was interpreted as follows: values ≤ 0 as indicating no agreement and 0.01–0.20 as none to slight, 0.21–0.40 as fair, 0.41– 0.60 as moderate, 0.61–0.80 as substantial, and 0.81–1.00 as almost perfect agreement. A p-value inferior to 0.05 was considered significant. Results:We included 54 patients (45 women and nine men) with a mean age of 55±11 years old [23-69]. The mean disease duration was 9.9±5.9 years [0-20]. The mean number of nocturnal awakenings was 1.1 [0-4] and the mean morning stiffness duration was 25.1 minutes [0-120]. The mean GPA was 5.3±2.2 cm [0-10]. The mean pain VAS was 5.4±2.2 cm [0-10] and the mean fatigue VAS was 4±2.5 cm [0-8]. None of the patients described his disease state as ‘excellent’. It was considered ‘good’ in 23.1% of cases, ‘average’ in 36.5% of cases, and ‘moderate’ to ‘unsatisfactory’ in 40.4% of cases. The mean ESR and CRP were 46.3±29.3 mm [5-120] and 15.8 mg/l [0.6-100] respectively. The mean DAS28 ESR was 4.68±1.35 [1.50-7.16] and the mean DAS28 CRP was 3.9±1.1 [1.02-6.05].A significant positive correlation was noted between both DAS28 ESR and DAS28 CRP and, number of nocturnal awakenings (r=0.385, p=0.013 and r=0.448, p=0.002), morning stiffness duration (r=0.495, p=0.001 and r=0.617, p<0.001), GPA (r=0.485, p<0.001 and r=0.530, p<0.001), and pain VAS (r=0.594, p<0.001 and r=0.598, p<0.001). No correlation was found between the two scores and fatigue VAS.No significant agreement was noted between PATSAT and DAS28 ESR (κ=0.077, p=0.478).Conclusion:PROs showed moderate to strong correlation with disease activity scores. The timely and effective use of PROs could encourage physicians to focus more on the impact of RA on patients and how patients are feeling. This in turn would facilitate shared decision making between patients and physicians.Disclosure of Interests:None declared

Abstract:
The article considers one of the methods of identity creation in a song used by lyricists in the early 21st c.: it involves a combination of profane language typical of an urban outcast (patsan, gopnik) and metaphysical, i. e. sacred, themes. Enjoying numerous representations in contemporary Russian visual arts, the discourse of the ‘metaphysical macho guy culture (patsanstvo)’ is also inseparable from the country’s literary tradition, which, however, is trying to marginalise it yet again and confine it to a song format. Analysing the poetic output of several Russian-speaking musicians (E. Limonov, P. Korolenko, Branimir, M. Elizarov, etc.), the author identifies the key features of the discourse: its faux-playfulness, emblematic quality, and adherence to low culture. The article also proposes a genre typology of the discourse in question, noting its critical, humoristic and paradoxographical varieties. In the end, the author discovers that the image of a macho guy / urban outcast (patsan) confronted with a supernatural experience is heavily influenced by the sacral archetype, which absorbs the most extreme — the lowest and the loftiest — aspects of human experience.

Abstract:
This paper examines Tencent’s economic performances over time in terms of its technological catch-up and business strategies, with Facebook as a benchmark. Patent and citation data from 2010 to 2019, extracted from the Worldwide Patent Statistical Database (PATSTAT), was used to analyze the technological catching up made by Tencent and Facebook. Results indicate that Tencent has overtaken Facebook in terms of market performances, and that the overtaking in the number of patents preceded that of the market performances, but Tencent has not yet overtaken Facebook in terms of the quality of patents. Tencent has surpassed Facebook in terms of technological independence from external sources and further widened the gap in this aspect over the years. Patterns in the cycle time of the patented technology and citations of non-patent scientific literatures illustrate Tencent’s strategies to seek a niche using recent technologies and avoid issues related to intellectual property rights. Overall, there is some evidence that technological prowess contributes to Tencent’s overtake of Facebook in market performances.

Abstract:
This study evaluates the impact of a U.S. government-sponsored research program on advanced natural gas combined cycle (NGCC) innovations in the 1990s. From 1992–2000, the U.S. Department of Energy (U.S. DOE) partnered with turbine manufacturers General Electric (GE) and Siemens Westinghouse Power Corporation (SWPC) in a cost-sharing partnership called the Advanced Turbine System program to promote efficiency innovations for NGCC technology. Using data from the European Patent Office’s worldwide patent database (PATSTAT), this study evaluates advanced turbine technology innovations by the program participants and their competitors. Using a negative binomial model, this approach shows GE increased the relative quantity of their patents towards the end of the program and afterwards, indicating the program led to more advanced NGCC innovations for GE. SWPC, on the other hand, had higher patent citations for patents filed during the DOE program relative to competitors, indicating SWPC had higher-quality advanced NGCC innovations due to new partnerships from the U.S. DOE program. However, this analysis reveals there was not a lack of this activity taking place before the program started, and that the overall impact of the program appears small based on the patent analysis.

Abstract:
Since the statistical classification of economic activities is not able to adequately display companies’ R&D expenditures, the aim of this paper is to create a concordance list between industry sectors and technologies, enabling us to report the business R&D expenditures not only by industries but also by technology fields. To construct the concordance, we match data on R&D expenditures with patent data at the micro-level, i.e. at the level of companies and patent applicants, respectively. In a further step the business R&D expenditures are aggregated at the level of technology fields. This concordance table also allows us to provide patent statistics at the level of industries. The patent data for the matching were extracted from the “EPO Worldwide Patent Statistical Database” (PATSTAT). The data on German business R&D expenditures are provided by the SV Wissenschaftsstatistik. The two data sources are matched by applying a string matching algorithm based on the distance between two text strings. The matching covers 44% of all German patent applicants and 83% of all patent filings at the EPO and the German Patent and Trademark Office in 2009.

Abstract:
Introduction: Identification and attainment of the goals of cancer patients is an important aspect of personalized treatment. Aim of the study: The study aimed to assess the following aspects in patients treated surgically for endometrial cancer: 1) level of satisfaction with hospitalization using the EORTC IN-PATSAT32 nomothetic questionnaire; 2) degree of goals attainment using the Goal Attainment Scaling (GAS) idiographic questionnaire; 3) correlation between these evaluation methods. Material and method: The study included 123 patients with endometrial cancer (FIGO I–II) treated surgically at the Department of Obstetrics and Gynecology in Rzeszów in 2012–2014. EORTC IN-PATSAT32 and GAS questionnaires were used. The collected material was analyzed using the Statistica 10.0 software. Results: The overall level of satisfaction measured with the IN-PATSAT32 scale was 72.2 ± 20.5. The technical skills were rated the highest in nurses (74.5 ± 17.6) and doctors (69.3 ± 17.8), while the lowest score was awarded for hospital assess (54.7 ± 23.3). The overall satisfaction with care was 72.2 ± 20.5. In the personalized GAS scale, the patients listed individual expectations before the surgery, assigning ranks to their importance. For most of them, it was a very high (A) or high (B) rank. The patients assigned the highest ranks to quick mobilization, success of the operation, and willingness to be healthy. The average value of the level of goal attainment on the discharge date was 63.7 ± 9.4 points. Statistically significant correlations between the questionnaires were found for the level of goal attainment and the assessment of various aspects of hospital care. Conclusions: The study proved that the EORTC IN-PATSAT32 questionnaire was correlated with GAS questionnaire, and additionally provided knowledge about individual goals of care and the degree of their attainment. The use of nomothetic and idiographic tools gives wider possibilities in the planning and implementation of personalized care.

Abstract:
In today’s highly competitive markets, firms can no longer ensure competitive advantage through the development of highly complex technologies or differentiation based solely on technological and functional factors. Simultaneously, there is growing recognition that design and aesthetic factors can contribute to firms’ competitive advantage and profitability. Hence, this study examines the effects of design innovation, captured by formal design registration, on firms’ financial performance. We investigated the relationship between technology complexity, design innovation, and firm performance to unveil design innovation’s role in exploiting complex technologies. Our main hypothesis is that design innovation positively moderates the negative effect that technology complexity yields on firm performance. We used a unique dataset that aggregates all patent data published by US-headquartered publicly-traded firms to the parent company level. We combined this with firms’ financial information and design patent data from the Computat and PATSTAT databases based on their matching firm identifier. Focusing on 1,765 US firms from 1980 to 2015, we calculated firm-level technology complexity based on the structural diversity of technologies. The results confirmed our hypotheses. This study contributes to firms’ resource-based view by recognizing design as a strategic resource that firms can utilize to gain a competitive advantage and by discovering design innovation’s role in generating economic profits.

Abstract:
Successful new product development requires the integration of design and engineering, bridging the gap between technological feasibility and user-centric considerations. However, direct collaboration between designers and engineers with heterogeneous knowledge presents challenges. In this context, the role of design engineers—professionals skilled in both design and engineering—becomes pivotal. This study categorizes inventors into three primary groups: engineers, designers, and design engineers based on the type of patent applications they hold and investigates their differences in knowledge portfolios and collaboration patterns. The study relies on patent data for 4,665 US publicly-traded firms from 1980 to 2015 from the PATSTAT database, and constructs two networks for each firm period: a social network of inventors and a knowledge network of knowledge elements. Findings show that design engineers are highly connected within the social network but have disconnected knowledge in the knowledge network in comparison to engineers. While design engineers may not be the primary drivers of firms’ technological innovations, they facilitate interdisciplinary communication and decision-making, fostering a design-technology integrated new product development environment. This research has practical implications for firms seeking to optimize their innovation processes by creating interdisciplinary teams that harness the complementary strengths of engineers and design engineers.

Abstract:
AbstractThe heterogeneity of the Higher Education (HE) Institutions is one of the main critical issues in the assessment of their performance. This paper adopts a multi-level and multi-dimensional perspective, combining national (macro) and institution (micro) level data, and measuring both research and teaching activity, using performance indicators derived from the European Tertiary Education Register, CWTS Leiden Ranking, and PATSTAT patent database. Clustering and efficiency analysis are combined to characterize the heterogeneity of national HE systems in European countries, and reveal the potential of using micro level data to characterize national level performance. Large differences are observed between the European countries, partially due to the fact that they are in different phases of their scientific (and economic) development and of the re-structuring of their HE systems. Evidence is found that universities specializing either in teaching or in research tend to have a higher efficiency than those institutions balancing research and teaching. Tradeoffs are observed between undergraduate and post-graduate activities, and a “Matthew cumulative effect” seems in place on the European institutions analysed: high quality research is able to attract external funds that stimulate innovative and patenting activities that in turn are self-reinforcing to the scientific activities. The results reveal once more the limits and dangers of one-dimensional approaches to the performance of HEIs.

Abstract:
Currently, in the world, there is a use of obsolete technologies to satisfy cooking and water heating needs in rural areas. These technologies have emissions with environmental impacts, which contribute to climate change. This study performs a Life Cycle Assessment in the Global Warming Potential (CO2e) impact category in six firewood devices representative of Mexico and Central America. As part of this analysis, measurements of emissions and energy consumption in a typical day during water heating and cooking tasks were integrated. Devices such as open fire have zero CO2e contributions in the construction but represent the highest emissions and energy consumption in a typical cooking day. On the other hand, cookstoves such as the Patsari and the Onil contribute to mitigating CO2e emissions in the long term. The main CO2e contributions are produced in the final use. This analysis is useful in making decisions for the implementation of massive cookstove programs.
