Full textRecently, many studies have shown that viral agents can be involved in the pathogenesis of atherosclerosis. However, their role in destabilizing atherosclerotic plaques has not yet been elucidated, and neither have their underlying molecular mechanisms. On the basis of the data currently available in the literature, we hypothesized that viral agents can promote the destabilization of already present atherosclerotic plaques, through direct infection or by mediating an immune response with systemic and especially local changes in the expression of inflammatory cytokines and chemokines, and with consequent activation of molecular pathways that would lead to the upregulation of the expression of scavenger receptors with greater affinity for oxidized LDL (CD36 and Lectine-Like-oxLDL-receptor 1 - LOX-1) on the macrophages. The aim of the study was therefore to investigate the presence of genetic material of some viral agents in the carotid plaques of patients undergoing carotid endarterectomy (CEA) at IRCCS Policlinico San Donato (Vascular Surgery Unit), the expression of local and systemic inflammatory cytokines and PPAR-γ, correlating the results obtained with the vulnerability of the removed plaque. Data from 50 patients (mean age 73.7+8.8 years) were analyzed, 31 of whom had a vulnerable plaque (62%). The comorbidities of patients with vulnerable plaque were comparable to those of patients with stable plaque. The genome of Cytomegalovirus (CMV), Herpes-Simplex (HSV), Varicella Zoster (VZV) and Influenza Virus (IV) was not found in any of the vascular sample, while the Epstein-Barr (EBV) genome was found in two vulnerable plaques, in particular in both cases it was identified in the "core" but not in their respective control area. The serological determination of anti-EBV, CMV, HSV, VZV IgM and anti-IV IgA was negative in all cases, while for IgG it revealed that all patients had been exposed to EBV, 98% to CMV, 68% to VZV and 12% to HSV. No patients were operated on in a possible active phase of influenza and nearly half of them (22/50, 44%) had been vaccinated against IV in the previous 6 months, and resulted protected by the IgG. The viral exposure of patients with vulnerable plaque was not significantly different from that of patients with stable plaque. However, 82% of patients not receiving influenza vaccination (23/28) had a vulnerable carotid plaque, compared with 36% of vaccinated patients (8/22, P=0.001). Plasma levels of inflammatory cytokines (particularly TNF-α and IL-6) were higher in patients with vulnerable plaque than in patients with non-vulnerable plaque, however independently of comorbidities, viral exposure or flu vaccination. Gene expression of the same cytokines in the plaque was greater in the "core" than in the control, especially in vulnerable plaques. Again, there were no significant differences in the expression of these cytokines based on comorbidities or viral exposure, however in patients with vulnerable plaque the expression of TNF-α in the "core" was greater in those who had not received the influenza vaccination compared to those who did. For IL-10 the opposite was observed. In addition, in vulnerable plaques, PPAR-γ tended to be more expressed in the “core” than in the control, especially in patients who had not undergone influenza vaccination. Moreover, in the group of patients with vulnerable plaque, the gene expression of PPAR-γ in the “core” of the plaques was found to be directly related to the gene expression of TNF-α, IL-1β and IL-10. Finally, the immunohistochemical examination revealed a greater presence of CD36 and LOX-1 in the “core” of the vulnerable plaques compared to stable ones. In light of our results, it is therefore not currently possible to define a possible relation between plaque vulnerability and the viral agents analyzed. However, there was a significant difference in the incidence of plaque vulnerability among patients undergoing influenza vaccination compared to those not vaccinated.

Full textAtherosclerosis is a multifactorial and complex pathology. Some studies show a possible involvement of microorganisms, but the results are contradictory. The aim of this study is to evaluate the presence of anti- Chlamydia pneumoniae and anti-Simkania negevensis antibodies in sera of patients waiting for carotid endarterectomy (CEA) surgery. In addition, after the surgery it was possible to search DNA of microorganisms into the removed pathological tissue. For the serological evaluation it was used a home-made immunoenzymatic technique, for the molecular evaluation a nested PCR. For this study, 50 patients were enrolled. The obtained results pointed out a positivity of the 28% for IgG anti-Simkania negevensis and of the 10% for IgA; for C. pneumoniae the percentages were of 20% of IgG and 8% for IgA. Molecular data of the PCR obtained were: 34% of positivity for S. negevensis and 26% for C. pneumoniae. The results of this study support the hypothesis that these microorganisms can develop a quiescent or persistent status of the infection, in which it is possible to observe, through technique of PCR, DNA of them into the pathological tissue.

Los pacientes con diabetes mellitus tipo 2 presentan mayor prevalencia de enfermedad ateromatosa subclínica que los sujetos sin diabetes mellitus. El rol de los profesionales de la enfermería es esencial en la prevención y promoción de la salud, mediante la realización de procedimientos ecográficos, la identificación de los factores de riesgo cardiovasculares y la educación para la salud. Se ha desarrollado y estandarizado un método ecográfico de cuantificación de la señal de vasa vasorum de la adventicia de la carótida común que es válido, fiable y reproducible. La evaluación de la señal de vasa vasorum en la adventicia carotidea puede ser un nuevo marcador para la exploración y monitorización del proceso aterosclerótico. Los pacientes con diabetes mellitus tipo 1 presentan mayor señal de vasa vasorum que aquellos que no tienen diabetes, sugiriendo que la pared de las grandes arterias se puede considerar también una diana de la microangiopatía diabética. No se ha podido demostrar una asociación entre la señal de vasa vasorum y la presencia de retinopatía diabética
This thesis is composed of 4 articles published in journals of international impact and the main conclusions are discussed below. Patients with diabetes mellitus type 2 have a higher prevalence of subclinical atheromatous disease that subjects without diabetes mellitus. The role of nursing professionals is essential in prevention and health promotion, through the realization of ultrasound procedures, identification of cardiovascular risk factors and the education for health. It has been developed a standardized ultrasound method for quantifying the signal of vasa vasorum in the adventitious common carotid which is valid, reliable, and reproducible. The signal evaluation of vasa vasorum in the adventitia carotid may be a new marker for exploration and monitoring of the atherosclerotic process. Patients with type 1 diabetes have greater vasa vasorum signal that those without diabetes, suggesting that the wall of the large arteries can also be considered a target of diabetic microangiopathy. It was unable to demonstrate an association between VV signal and the presence of diabetic retinopathy

Full text• Contributions and new knowledge: This work demonstrates that the peripheral artery tonometry is a useful technique for the cardiovascular risk evaluation and is associated to endothelial dysfunction markers, arterial stiffness and morphologic lesions of the arterial wall. Therapeutic life-style changes may cause improvement on digital reactive hyperaemia index. These findings may contribute to a more accurate cardiovascular risk stratification of patients. • Methods and main relevant conclusions: The studies described are based in 816 patients without cardiovascular disease and at moderate cardiovascular risk recruited from the Vascular Medicine and metabolism clinic of the Hospital Universitari Sant Joan in Reus. Clinical visits, blood analyses, therapeutic life-style changes, carotid ultrasonography methods and endothelial function and arterial stiffness evaluation are described in each study. Main results showed that small artery reactivity is directly associated with HDL cholesterol levels and indirectly with tobacco habit, abdominal circumference and triglyceride levels. Central arterial stiffness calculated by the augmentation index was the main determinant of the carotid intimae-media thickness together with the LDL cholesterol levels and the systolic arterial pressure. A one-year prospective study on the therapeutic life-style changes impact on arterial reactivity in 150 volunteers showed that those patients with better adherence presented endothelial function improvement and lower carotid intimae-media thickness progression. Furthermore, the augmentation index measured in 125 patients with familial hypercholesterolemia was positively associated with carotid intimae-media thickness and Apo B100 levels. Endothelial function biomarkers were analysed in order to investigate the relation of this techniques with biochemical markers of cardiovascular risk. The E-selectin, sVCAM levels and the oxLDL/LDL ratio were associated to a lower reactive hyperaemia in a subgroup of 407 patients. The FABP4 was analysed in 257 patients with type 2 diabetes mellitus and was associated with a worse endothelial function.
