Abstract:
Adverse events are an unfortunate reality of caring for patients in our current healthcare system. Preventing and mitigating these events are an important part of quality improvement. First, an understanding of what events occur and how often they are occurring is critical to planning improvements. Incident reporting systems are one way of gathering this information. Then, events should be categorized and analyzed for improvement. The failure mode and effects analysis (FMEA) and bow-tie diagram are two tools for this purpose. Once an event has occurred, consideration should be given to the caregivers as well as the patient when managing and resolving adverse events. Prevention requires strong analysis of events and recognition of both latent (system) and human causes. Interventions have different degrees of effectiveness, ranging from highly effective forcing functions, to marginally effective encouraging statements. There are four steps to event management: mitigation, immediate management, refractory management, and follow-up.

Abstract:
Paediatric pain management has made great strides in the past few decades in the understanding of developmental neurobiology, developmental pharmacology, the use of analgesics in children, the use of regional techniques in children, and of the psychological needs of children in pain. The consequences of a painful experience on the young nervous system are so significant that long-term effects can occur, resulting in behavioural changes and a lowered pain threshold for months after a painful event. Accurate assessment of pain in different age groups and the effective treatment of postoperative pain are constantly being refined, with newer drugs being used alone and in combination with other drugs, and continue to be explored. Systemic opioids, paracetamol, non-steroidal anti-inflammatories, and regional anaesthesia alone or combined with additives are currently used to provide effective postoperative analgesia. These modalities are often best utilized when combined as a multimodal approach to treat acute pain in the perioperative setting. The safe and effective management of pain in children includes the prevention, recognition, and assessment of pain; early and individualized treatment; and evaluation of the efficacy of treatment. This chapter discusses selected topics in paediatric acute pain management, with more specific emphasis placed on pharmacology and regional anaesthesia in the treatment of acute postoperative pain management.

Abstract:
Pain is a frequent but poorly controlled aspect of cancer and other medical conditions which may, in part, stem from the clinician’s lack of understanding of its severity or impact. However, even with the best of care, current approaches are associated with poorly tolerated side effects and frequently fall far short of complete control. Rehabilitation medicine has developed strategies that may reduce pain in general, but are particularly targeted to movement-related pain. These approaches can be grouped into four general categories: (1) modulating nociception, (2) stabilizing and unloading painful musculoskeletal structures, (3) influencing pain perception, and (4) alleviating musculotendinous pain. This chapter reviews each of these categories in detail and offers examples to illustrate their clinical application. It will be noted that the majority are focused on minimizing pain during periods of mobility and the performance of activities of daily living.

Abstract:
Diagnosis and treatment of acute stroke has advanced considerably in the past 2 decades. Most notably, in cases of ischemic stroke, intravenous alteplase has become the standard of medical treatment despite its multiple contraindications and limited time window. More recently, trials have proven that endovascular thrombectomy is superior to medical therapy alone, advancing the standard of care for patients who present with acute ischemic stroke from a large vessel occlusion and salvageable brain tissue. The treatment of hemorrhagic stroke now involves the use of novel pharmacological agents and advanced minimally invasive technology. Important changes have also occurred at the levels of hospital organization and treatment decision-making. Such changes in organization and designation of hospitals with distinct levels of stroke care and the variety of stroke protocols now requires team work of emergency medical services (EMS), Emergency Department, stroke neurologists, neurosurgeons, and neurointerventionalists. This book provides an overview of the modern medical and surgical options for the treatment of patients with acute ischemic and hemorrhagic strokes. The pivotal role of EMS in prehospital evaluation and triage of a stroke patient and the levels of stroke systems of care are discussed. In addition, the current guidelines on the management of acute stroke, with the focus on early care of acute stroke patients at the Emergency Department and the first 24 hours of hospital admission, are reviewed. Each chapter contains a discussion of common clinical scenarios including initial management steps, practical points, and common pitfalls.

This chapter initially provides an overview of the current distribution and status of the common raccoon in North America. Its overall numbers and distribution, and even its utilization of certain habitats have changed dramatically; all largely within the past century. This contribution then examines how the numbers of raccoons that are killed or ‘harvested’ have changed during the last century. The term ‘harvest’ is commonly used by wildlife managers in North America to refer to the number of animals taken by hunters and trappers, for sport, or to utilize and sell their fur. The policies that determine raccoon exploitation by such practices are herein reviewed. There is an assessment of whether and how population variables are considered in establishing these hunting and trapping policies. Ultimately, this analysis reveals the state of raccoon management and offers thoughts about its potential paths.
