Abstract:
Afin de mieux appréhender les facteurs de risque des Syndromes Myélodysplasiques (SMD) et des Leucémies Myéloïdes chroniques (LMC), nous avons réalisé deux études cas témoins incluant les cas incidents et des témoins de population générale, appariés sur le sexe et l’âge. Les données personnelles, professionnelles et extra professionnelles ont été recueillies par entretien direct. Les expositions ont été validées par un comité d’experts. L’étude portant sur 204 SMD et 204 témoins montre un lien significatif entre la maladie et le tabagisme, sans relation dose-effet. Le jardinage, la vie à proximité d’un site industriel, les métiers de santé, vendeurs et représentants de commerce, ouvriers, ouvriers du textile, agriculteurs et les exposition aux solvants, huiles, pesticides et autres nuisances agricoles sont significativement plus fréquents chez les cas. Il n’y a pas de relation entre les expositions étudiées et une anomalie spécifique du caryotype. L’étude portant sur 110 LMc et 440 témoins, ne montre pas de lien avec le tabagisme ou la vie à proximité d’une ligne à haute tension ou d’une centrale nucléaire. Les cas vivent plus souvent près d’un site industriel et exercent plus souvent des métiers exposant à des substances chimiques : ouvriers, ouvriers de filature, manœuvres, imprimeurs ou photographes, femmes de ménage, dessinateurs et sont plus souvent exposés aux : formaldéhyde, peintures, acides, produits phytosanitaires, solvants
In order to assess the relationship between occupational and environmental risk factors of Myelodysplastic syndromes (MDS) and Chronic Myeloid Leukaemias (CML), we performed 2 case-control studies. Incident cases and sex and age-matched controls of the general population gave data on their occupational exposures and hobby activities by interview. Exposure assessment was reviewed by a group of experts. The study on 204 SMD and 204 controls shows relationship between MDS and smoking habits, life near an industrial plant, gardening, occupations such as health professionals, technical and sales representatives, machine operators, agricultural workers, textile workers, exposure to oil, ammonia, solvents, pesticides and other agricultural hazards. No relation between the exposures studied and a specific cytogenetic abnormality was observed. The study on 110 LMC cases and 440 controls does not show any relation with smoking, life near a nuclear power plant or under a High voltage power line. Cases used to live more often near an industrial plant and had more often occupations exposing to chemicals : labourer, spinning worker, printer or photographer, housekeeper, draughtsperson and were more often exposed to formaldehyde, paints, acids, pesticides, solvents

Abstract:
The chief goal of the present study was to test whether exposing young adult males to female centerfold images causes them to believe more strongly in a set of beliefs clinical psychologist Gary Brooks terms "the centerfold syndrome." In addition to testing the straightforward effect of exposure to centerfold images on males' centerfold syndrome beliefs, the present study explored the moderating potential of three variables: sexual explicitness of the centerfold images, males' past exposure to objectifying media, and recency of exposure to the centerfold images.Participants were randomly assigned to either a control condition that did not feature centerfold stimuli, a "nonexplicit" condition that featured female centerfolds who did not expose their nipples or genitalia, or an "explicit" condition that featured female centerfolds exposing either their nipples, genitalia, or both. Past exposure to objectifying media was assessed by asking participants how frequently they viewed pornography in the prior year. Items indexing the five centerfold syndrome beliefs - voyeurism, sexual reductionism, masculinity validation, trophyism, nonrelational sex - were administered immediately after exposure and approximately 48 hours after exposure.Exposure to centerfold images had an immediate strengthening effect on the sexual reductionism and nonrelational sex beliefs of males who view objectifying media about once a month or less and this effect persisted approximately 48 hours after exposure. Likewise, exposure to centerfold images had an immediate strengthening effect on the masculinity validation beliefs of males who view objectifying media about once a month or less, and this effect persisted at a marginally significant level approximately 48 hours after exposure. No difference were found between males exposed to nonexplicit vs. explicit images.These findings are consistent with a growing body of literature indicating that mainstream media sex can affect the sexuality of young people. Furthermore, the findings of the present study affirm the suspicions of some that objectifying depictions of females affect the sexual beliefs of some males in ways that are likely unrelated to sexual aggression but are still antisocial.

Abstract:
The purpose of the present study was to increase our understanding of the relations among embarrassment, Theory of Mind (ToM), and emotion dysregulation in adolescents with Asperger's Syndrome and High Functioning Autism (AS/HFA), topics that have not previously been the foci of research in this population. The research sample consisted of 42 participants, split equally between adolescents with AS/HFA and typically developing (TD) adolescents. Participants with AS/HFA were matched with TD participants for chronological age and gender. Parents of all participants, typically mothers, were also required to complete measures. Participants were presented with vignettes of embarrassing or anger inducing scenarios, following which they were asked to provide ratings indicating the degree to which they would be embarrassed or angry in the protagonists' positions. Next they were asked to justify those ratings. Results indicated that the AS/HFA group experienced greater difficulty than the TD group with measures requiring ToM abilities. This was particularly true of embarrassment/social faux pas situations. In contrast, both groups performed similarly on measures involving anger-inducing situations that require less ToM. The significant difficulty of the AS/HFA group in understanding ToM in embarrassment measures was corroborated by their poor performance on an independent ToM measure. In addition to having significant difficulty in understanding embarrassment, the AS/HFA group was significantly less able than the TD group to recount personally embarrassing experiences. Regarding emotion regulation, participants with AS/HFA were significantly less able than their TD peers to regulate their emotions through reappraisal. Similarly, parents of the AS/HFA participants reported a significantly higher level of emotion dysregulation in their children than did the parents of the TD participants. Further, participants with AS/HFA had a significantly higher utilization frequency of negative strategies than their TD peers when embarrassed, which aligned with parent report. Negative strategies included internal, verbal, and physical self-injurious behaviors, as well as destructive interpersonal behaviors, e.g., falsely accusing, yelling at, or hitting others. These findings emphasize the critical and potentially harmful impact of embarrassing experiences in the daily lives of adolescents with AS/HFA.

Abstract:
This research report documents the therapeutic intervention undertaken with a 23-year-old Swazi rape victim. The format of this research report takes the form of a case study that follows the principles proposed by Fishman (2005). Its aim is to document the treatment process of an individual of African decent in order to establish whether the treatment model can be effective in clinical settings and in contexts and cultural settings different from that in which it was developed. The Ehlers and Clark (2000) cognitive therapy model for post-traumatic stress disorder (PTSD) was utilised to assess, conceptualise, and treat the case. The client entered therapy three years after being raped for a third time. The case formulation identified factors maintaining the disorder as well as how other traumatic and abusive events earlier in her life influenced her response to the rapes. Data consisted off audio-tape recordings and detailed written synopses of each assessment and therapy session, psychometric measurement instruments and self-report scales completed throughout the intervention, material written by the client, and a research interview conducted by an independent party. She was treated for PTSD and comorbid depression over a period of five months in accordance with the principles described by Ehlers and Clark and a narrative of the treatment process was written. The case narrative in conjunction with quantitative data suggested that this model assisted the client in initiating a healing process. As such the model was found to be both effective and transportable to an African context. Various points of discussion are highlighted, including the challenges of working with PTSD and comorbid major depression, the client-therapist relationship, and that a client and therapist from different cultures, backgrounds, and with different home languages can work together effectively using the Ehlers and Clark model.

Le by-pass gastrique Roux-en-Y (BPG) est une chirurgie de l'obésité qui induit des améliorations spectaculaires de l'homéostasie glucidique indépendamment de la perte de poids. Un mécanisme proposé pour expliquer ces améliorations est une augmentation de la production intestinale de glucose (PIG) qui induit des effets bénéfiques sur l'organisme (satiété, amélioration de la sensibilité hépatique à l'insuline). Cette augmentation de la PIG, retrouvée chez la souris ayant subi un BPG simplifié, est également responsable des effets bénéfiques des régimes enrichis en protéines via l'inhibition des récepteurs mu-opioïdes (RMO) par les peptides. Nous avons donc testé l'hypothèse selon laquelle les effets bénéfiques du BPG dépendraient d'une inhibition des RMO par les protéines alimentaires et nous avons également testé le rôle causal de la PIG dans ces améliorations métaboliques. Pour cela, nous avons réalisé un by-pass duodéno-jéjunal (BDJ), ie un BPG sans restriction gastrique, chez des souris sauvages (WT), des souris invalidées pour le gène du RMO (MOR-/-) et des souris dépourvues de PIG (I-G6pc-/-). Chez les souris obèses, Le BDJ induit une forte perte de poids (–30%), en partie expliquée par une malabsorption lipidique, ainsi qu'une amélioration des paramètres glucidiques dépendante de cette perte de poids. Au contraire, chez la souris de poids normal, le BDJ n'induit ni perte de poids ni malabsorption mais améliore la tolérance au glucose. Les effets sont les mêmes chez les souris WT, MOR-/- et I-G6pc-/- ce qui montre que les récepteurs mu-opioïdes et la PIG ne semblent pas avoir de rôle causal dans les améliorations du métabolisme énergétique et glucidique après BDJ
Roux-en-Y gastric bypass procedure (GBP) is an obesity surgery that induces dramatic glucose homeostasis improvements independently of weight loss. A proposed mechanism to explain these glucose homeostasis improvements is an increase in intestinal glucose production (IGP) that induces beneficial effects on metabolism (satiety, improved liver insulin sensitivity). This increase in IGP is found in mice that have undergone a simplified GBP and is also responsible for the beneficial effects of protein-enriched diets through the inhibition of mu-opioid receptors (MOR) by alimentary peptides. We therefore hypothesized that the beneficial effects of GBP could depend on MOR inhibition by dietary proteins and we also tested the causal role of IGP in these metabolic improvements. For this purpose, we performed a duodenal-jejunal bypass surgery (DJB), ie GBP without gastric restriction, in wild-type mice (WT), in mice lacking MOR gene (MOR-/-) and in mice lacking IGP (IG6pc-/-). In obese mice, DJB induced a rapid and substantial weight loss (-30%), partly explained by fat malabsorption, and weight loss-dependent improvements of glucose homeostasis. In contrast, in the non-obese mice, DJB did not induce weight loss nor malabsorption but improved glucose tolerance. Effects were similar in WT, MOR-/- and I-G6pc-/- mice showing that mu-opioid receptors and IGP did not appear to have a causal role in glucose and energy metabolism improvements after DJB

Le remodelage ventriculaire gauche est une complication fréquente des patients ayantprésenté un syndrome coronarien aigu, pouvant conduire à terme à une situation d’insuffisancecardiaque. Il est donc important de connaître les facteurs associés à la survenue d’un remodelageventriculaire afin de dépister plus précocement les patients à plus haut risque d’insuffisance cardiaqueet ainsi optimiser leur prise en charge. Ce travail comprend deux axes. Le premier porte sur larecherche de nouveaux paramètres d’imagerie associés à la survenue du remodelage. Nous avonsdans un premier temps réalisé une revue de la littérature concernant la définition du remodelageventriculaire gauche en imagerie par résonance magnétique. Puis, nous avons conduit deux étudesayant pour but de rechercher une association entre (i) le strain atrial gauche et, (ii) le gradient depression intraventriculaire gauche diastolique, évalués en échocardiographie 24-48 heures après lesyndrome coronarien aigu et le remodelage ventriculaire gauche au cours du suivi. Le second axe portesur les biomarqueurs associés au remodelage ventriculaire post-infarctus. Nous avons réalisé une revuede la littérature au sujet des biomarqueurs qui, dosés lors de l’hospitalisation initiale, sont associés àl’existence d’un remodelage lors du suivi. Left ventricular remodeling is a common complication in patients following acutemyocardial infarction and may lead to heart failure. Some baseline parameters are associated withremodeling at follow-up, allowing to better discriminate patients with an increased risk of heart failureto optimize therapeutics. This work has two axes, focused on imaging and biological parametersassociated with left ventricular remodeling, respectively. First, we reviewed past studies that definedremodeling using cardiac magnetic resonance imaging. Then, we studied the association betweensome echocardiographic parameters (left atrial strain and diastolic intraventricular pressure gradient)and left ventricular remodeling after ST-elevation myocardial infarction. In the other axis, wereviewed biomarkers that have been associated with left ventricular remodeling in prior studies. World of Aspergers Annotation In my diploma thesis, which I called the Asperger's World, I elaborate on subject of specific perception of people with autistic spectrum disorder. The aim of the thesis is to evaluate the following main aspects: how people with Asperger syndrome perceive their disability and whether they are recognized as part of society or stigmatized. In the theoretical part I define the term autism and Asperger syndrome. I describe certain aspects that exclude this syndrome from other autistic manifestations and point out differences in behavior and experience of everyday life of these individuals. Emphasis is placed above all on other sensory perceptions and ethical issues concerning children and adolescents. Their integration into common school classes, the perception of teachers, classmates and parents. The practical part contains three case studies on which I would like to show how difficult it is to deal with this disability from the perspective of the parents of the affected child. Furthermore, the expressions of children and adults themselves with Asperger syndrome, pedagogues, classmates, friends and co-workers. The thesis deals with the issue of spontaneous abortion as well as induced termination of pregnancy and its potential negative impacts on woman's mental and physical health. The theoretical part summarizes the findings about particular kinds of pregnancy loss and defines the possibilities of psychological adaptation to this loss. The author paid the close attention to Post-abortion syndrome (PAS), which is regarded as a special form of Post- Traumatic Stress Disorder. The thesis is further focused on the emotional, social, psychological and physical effects following abortion, the most threatened group of women with a higher probability of having PAS and finally the way of medical treatment. In the empirical part, the main symptoms of PAS were detected by means of the Helpline database analysis. Detection of these syndromes enabled to compile the questionnaire related to pregnancy loss. The main task was to discover the perceived changes in a woman's life after the pregnancy loss. In the final part of the work, the questionnaire regarding pregnancy loss had been tested and finally the main benefits together with limits had been critically evaluated.

This research aims to find out the results taking Xenical for 12 weeks and 24 weeks. The research findings will be discussed in this paper is a path analysis in four exogenous variables (two different hospitals, teams, genders, and the first week data) effect the metabolism symptom groups which include insulin, triglyceride, low density cholesterol, high density cholesterol, total cholesterol, uric acid, systolic pressure and diastolic pressure. After taking Xenical for 12 weeks, five results will show in this paper. First, taking Xenical is helpful to decrease the BMI, low density cholesterol, total cholesterol and systolic pressure. Second, the finding says the effects in females are better than males. Based upon the data, females can decrease insulin, triglyceride and uric acid. Third, hospital B is better than hospital A in the decrease of BMI. Fourth, hospital A in controlling high density cholesterol is better than hospital B. The data in 12 week will be decreased if you compare with the data in first week. That is, the more you take Xenical, the lower data in four exogenous variables will be demonstrated. Fourth, the less in uric acid you have, and the less high density cholesterol you get. Fifth, if you decrease the BMI, your systolic pressure will also become lower. Taking Xenical for 24 weeks, the research finding shows it gets lower in BMI, low density cholesterol and total cholesterol. Second, the data indicate triglyceride and uric acid in females are lower than males. Third, the insulin in hospital B lower than hospital A. Fourth, hospital A is superior to hospital B in decreasing the high density cholesterol. The data in 24 week are decreasing if you compare with the data in first week. However, two important findings challenge our thinking. One is the more you decrease in BMI, the lower data you get in insulin. The other is the lower you decrease in insulin, the lower data you diminish your uric acid.

Abstract:
For over 20 years, the phenomenon of parental alienation and its related characteristics has been described in literature. Various clinicians independently researched and described the pathological alignment of a child within the context of divorce. In the 1980’s Richard Gardner coined this “Parental alienation syndrome” (PAS). According to Gardner PAS is characterised by a child’s profound preoccupation with criticism against a parent. This criticism is overstated and ungrounded, and it is caused by both conscious and unconscious behaviour by the alienating parent, which influences the child negatively against the alienated parent. In essence, PAS is the subjective contamination of the child’s understanding and beliefs about his/her environment. The alienating parent gives the impression that the targeted parent is unworthy, dangerous, unloving and deserving of the child’s rejection. This is done by a series of alienation strategies like bad-mouthing, blaming, limiting contact and belittling. The alienated child, as a result responds with unjustified expressions of hate and discontent towards the targeted parent. The experience of divorce can be very challenging to children. Research about the relationship between divorce and child adjustment holds that a child’s exposure to inter parental conflict and the quality of the parent-child relationship are the two major predictors of children’s adjustment during divorce. Research suggests that the negative effects of PAS may include guilt, self-hatred, distortion of reality testing, and general emotional and psychological problems. The aim of this study was to provide an in-depth exploration and description of how PAS is experienced, and the possible effect it has on children from the perspective of young adults who was possible exposed to PAS as children. This was done by exploring their memories and their recalled experiences of their parents’ divorce and the possible effect on their current lives. In this study a collective exploratory/descriptive case study design was used. Nine voluntary participants, between 18 and 28 years of age, were chosen for this study by means of purposeful sampling strategies. Data were collected through in-depth semi-structured interviews that were audio taped and transcribed verbatim. Transcribed data were analysed by means of thematic analysis from which themes and sub-themes were derived. Two main themes with sub-themes were identified. It was found that some of the parental behaviour evident in PAS cases may fall under specific subtypes of psychological maltreatment and leave children feeling angry, worthless, flawed, unloved, unwanted, incompetent and sceptical about trusting other individuals. It was found that parents who engage in alienation strategies are likely to discourage autonomous behaviour and lack nurturance and emotional responsiveness towards their children’s needs. As result children exposed to PAS learn parental love and acceptance is conditional and is based needs fulfilment of the alienating parent. These expectations are brought forward from the early relationship into adulthood and influences individual’s expectations, behaviour, and beliefs about relationships across the lifespan. The results indicate that the lack parental support, encouragement, and responsiveness may negatively influence the self-esteem, autonomy, competence, and relatedness of individuals exposed to PAS. They reported difficulties with trust, intimacy and social skills and depression as adults.

Abstract:
This theoretical thesis covers the issues of Autism Spectrum Disorders (ASD) in relation to the brain connectome research. ASD belong to the group of neurodevelopmental syndromes and are characterized by deficits in communication skills, social interaction and stereotypic behaviors. The prevalence of ASD increases, its etiopathogenesis is very likely multifactorial. Within the ASD syndrome, precise differential diagnostic algorithms are difficult to implement in the absence of objective biomarkers. Extensive neuroscientific research, including the connectome projects, might improve the diagnostic and therapeutic procedures in the field of Psychiatry, including ASD. The individual's connectome profile might well serve as a new biomarker in psychiatric diagnostic and therapeutic approaches. The brain connectome represents the net of all neuronal connections in the brain. Mapping of the connectome across all ages, in health and in disease, is the main goal of the Human Connectome Project (HCP). The first HCP data show great interindividual variability with the environmental factors playing a crucial role. Abstract:
Objectives: After mild traumatic brain injury (mTBI), psychological factors can contribute to persisting post-concussion symptoms (PCS). Consistent with constructive theories of memory, negative expectations for increased symptoms after mTBI may contribute to misattributing symptoms to the mTBI and underestimating pre-injury symptoms, called the “good old days’ bias” (Gunstad & Suhr, 2001). The good old days’ bias is not thought to be a general retrospective recall bias but studies to date have largely not controlled for normative memory processes including those that lead to a biased, more positive recall of the past. Therefore, the current study examines whether there is a good old days’ bias after mTBI above and beyond normal memory biases. This study also examines how soon after mTBI the good old days’ bias affects recall of pre-injury symptoms in the first month after mTBI in adolescents as well as whether the good old days’ bias causes pre-injury symptom severity to be underestimated or if symptoms are entirely forgotten. Finally, the clinical significance of symptom recall biases is investigated. Method: The sample is 42 adolescents who sustained an mTBI (ages 13-18 years; 24 males) and 42 uninjured adolescents (ages 13-18 years; 24 males, ). The mTBI group rated current and retrospective post-concussion symptom ratings within one week and again, at one month, post-injury. The control group rated current and retrospective post-concussion symptoms at baseline and one month later. Cross-sectional and longitudinal comparisons using non-parametric statistical tests were used. Results: Wilcoxon signed-rank tests showed that, by one month post-mTBI, adolescents report fewer total, physical, and emotional pre-injury symptoms than they had reported within one week of their concussion. The control group did not demonstrate this good old days’ bias. There were no between-group differences in retrospective PCS ratings at either time point. Chi-square analyses found that the mTBI group was as likely as the control group to recall “no” pre-injury/past symptoms one month post-injury after having initially reported some pre-injury symptoms. Only four more adolescents were classified as “recovered” if their one-month PCS ratings were compared with pre-injury PCS ratings made within 1-week post-concussion rather than pre-injury ratings from 1-month post-injury. Discussion: There was mixed evidence for a good old days’ bias by one month post-concussion. This bias was not demonstrated in healthy adolescents, suggesting that the good old days’ bias is found specifically after concussion. During the acute post-injury period, the good old days’ bias may only be apparent by studying changes in concussed individuals’ own PCS ratings. The good old days’ bias leads to underestimating the severity of pre-injury symptoms rather than forgetting them entirely. The good old days’ bias does not greatly affect symptom recovery tracking by one month post-concussion. Future studies should directly examine expectations about concussion and their effect on current and retrospective symptom reporting.

Desde a década de 1970 que a investigação em saúde ocupacional tem vindo a destacar a mútua interferência Trabalho-Família como uma importante fonte de stresse. A dificuldade de conciliação da vida profissional com os papéis familiares tem demonstrado significativos efeitos negativos sobre a saúde emocional, mental e física de um indivíduo. Avaliar os mecanismos que podem explicar o aparecimento e desenvolvimento do stresse ocupacional torna-se essencial. Um desses mecanismos refere-se aos processos de avaliação cognitiva que indicam até que ponto uma dada situação de stresse é avaliada pelo próprio individuo e como esta avaliação influencia os níveis de conflito entre a vida familiar e a vida profissional. Neste trabalho aplicam-se Modelos com Equações Estruturais a um conjunto de dados reais da área da Psicologia, tendo como principais objetivos avaliar se um modelo teórico estabelecido a priori se ajusta aos dados e testar se a autoavaliação cognitiva que educadores/professores portugueses fazem da sua atividade profissional desempenha um papel mediador na relação entre a mútua interferência Trabalho-Família e a síndrome de Burnout. Os resultados obtidos confirmam que o modelo teórico proposto se ajusta aos dados e que a avaliação cognitiva apenas desempenha um papel mediador na relação entre o Conflito Trabalho-Família e a síndrome de Burnout.
Since the 1970’s occupational health research has been highlighting work-family conflicts as an important source of stress. The difficulty of reconciling professional and family roles has demonstrated significant negative effects on an individual’s emotional, mental, and physical health. Assessing the mechanisms that may explain the onset and development of occupational stress becomes essential. One of these mechanisms refers to cognitive appraisal that indicates the extent to which a given stress situation is assessed by the individual and how this assessment influences the levels of conflict between work and family life. In this work we apply Structural Equation Models to a set of real data from the domain of Psychology. The main objectives are to evaluate if a theoretical model established a priori fits the data and test if the cognitive self-evaluation that Portuguese teachers make of their professional activity plays a mediating role in the relationship between work-family conflicts and the Burnout syndrome. The results confirm that the proposed theoretical model fits the data and that cognitive self-evaluation only plays a mediating role in the relationship between Work-Family Conflict and Burnout.

Mr. Eduard Němeček, MD Divorce issue and its consequences Diploma Thesis 5. Summary: This work proposes a closer look at the divorce-related topics with consequential impacts both on financial situation of divorced spouses and destinies of minor children as resulting from divorced marriages in Czech Republic. Till date, this issue has been solved in different ways at both district and regional courts. In our republic, a more unifying element is represented by decisions of the Supreme and Constitutional Courts. This is a very serious social phenomenon which, according to statistics affects a large number of parents, children, but also childless partners. Yet from ancient times this phenomenon is known to be socially undesirable being almost entirely rejected by the Catholic Church in its canon law. The increase in divorces has resulted namely from the advancement procedure of society. It is a societal phenomenon known to occur irrelatively of the states and political systems, from the totalitarian regimes through those autocratic, up to highly democratic systems of developed countries worldwide. During the creation of this work, up to 40% of marriages have been critically monitored as expected to be ended in divorce.
