Full textFrom the onset of the COVID-19 pandemic, molecular biology tools for detection and analysis of the SARS-CoV-2 genome were developed and deployed on an unprecedented scale. PCR techniques have been used to diagnose millions of people on a daily basis, while the democratization of next-generation sequencing (NGS) has enabled the rapid generation of hundreds of thousands of viral genomes. These tools have been a major source of information in the study and understanding of the pathophysiology of SARS-CoV-2, but also in the real-time monitoring of the dynamics of the pandemic and the evolution of this virus. We undertook this thesis to evaluate how these molecular biology tools can contribute to the study and surveillance of SARS-CoV-2, in three studies with different objectives. With the PhyloCoV study, we first provided a precise description of the evolution of viral diversity observed during the year 2020 in the Ile-de-France area, thanks to the sequencing of 736 SARS-CoV-2 genomes from patients and healthcare workers of the Pitié-Salpêtrière and Bichat Claude-Bernard University Hospital. This study also allowed us to develop a phylogenetic clustering method allowing the detection of intra-hospital transmission of COVID-19. Then, in the framework of an observational study within the GH Sorbonne Université, we estimated the frequency and mortality associated with nosocomial transmissions of SARS-CoV-2. Between September and November 2020, 209 cases of nosocomial COVID-19 were identified, representing 13.9% of hospitalized patients infected with SARS-CoV-2 and associated with a mortality of 31%. Sequencing of the viral genomes disproved nearly one third of nosocomial transmissions and revealed valuable information about transmission routes within hospital wards. Finally, with the COCOPREV study, we studied the evolution of the SARS-CoV-2 genome during infection in patients treated with monoclonal antibodies, mostly immunocompromised. Several resistance mutations emerge in the SPIKE protein under the three treatments studied. Patients treated with Tixagevimab/Cilgavimab had a 5-fold increased risk of developing mutations at residues R346 and K444. Unlike other mutations found, R346 and K444 substitutions have repeatedly emerged in different variants due to their impact on SARS-CoV-2 fitness. They were finally selected by the currently dominant BQ.1 and XBB variants. In conclusion, we have demonstrated the utility of molecular biology tools in monitoring SARS-CoV-2 transmission in hospitals and the general population, as well as in identifying emerging resistance mutations in patients treated with monoclonal antibodies. These tools will most likely continue to be used in the context of the COVID-19 pandemic but also in monitoring the emergence and spread of other viruses

Full textGovernments are increasingly turning to social media platforms such as Twitter to disseminate public health information to the public, as evidenced during the COVID-19 pandemic. The purpose of this paper is to gain a better understanding of Canadian government and public health officials' use of Twitter as a dissemination platform during the pandemic, and to explore the public's engagement with and sentiment towards these messages. We examined the account data of 93 Canadian public health and government officials during the first wave of the pandemic (December 31, 2019 – August 31, 2020). Our objectives were to: 1) determine the engagement rates of the public with Canadian federal and provincial/territorial governments and public health officials' Twitter posts, 2) illustrate the evolution of the Canadian public discourse during the pandemic's first wave by a hashtag trends and 3) provide insights on the public's reaction to the Canadian authorities' tweets through sentiment analysis. To address these objectives, we extracted Twitter posts, replies and associated metadata available during the study period in both English and French. Our results suggest members of the public demonstrated increased engagement with federal officials' Twitter accounts as compared to provincial/territorial Twitter accounts. Hashtag trends analyses illustrated the topic shift in the Canadian public discourse, which initially focused on COVID-19 mitigation strategies and evolved to address emerging issues such as COVID-19 mental health effects. Additionally, we identified 11 sentiments in response to officials' COVID-19 related posts. This study illustrates the potential to leverage social media to understand public discourse during a pandemic. We suggest that routine analyses of such data can provide real-time recommendations to government and public health officials on public sentiments during a public health emergency and can provide useful insights on the accounts/actors with which members of the public are most engaged, which can be leveraged to disseminate key messages.

Full textAbstract This study is part of a field of research focusing on organizational health at work. Particularly, the objective of this master's research is to identify; in the context of the pandemic linked to Covid-19; the demands and resources present in the daily life of police officers in Quebec having an impact on their health and well-being at work. Also, this research makes it possible to identify the adaptation strategies put in place by the actors to work on their overwhelming situations and balance their well being. To understand this problem, we were inspired by two theoretical models, namely JD-R (Bakker & Demerouti, 2007) which postulates that the characteristics of work are classified as demands and resources, and Job Crafting model (Tims& Bakker, 2010), who argues that individuals rely on coping strategies to improve their well-being at work and balance their workload. As a result, we adopted an exploratory qualitative methodology and carried out semi-structured interviews in videoconference mode with 20 police officers patrolling in one of the largest cities in Quebec, including 5 female officers and 15 male officers. Data collection took place during the months of july, August, and september 2020. The results of this study show that the constraints and demands raised in the daily life of police officers such as the transformation of work, the adjustment of schedules, the reorganization of teams have impacts on their health and well-being, namely professional stress and problems of work-family balance. Also, it was concluded that the police have a wide range of resources categorized into four IGLO types (Nielsen et al., 2017), namely individual resources (experiences, personal background, training and skills and also family support), group resources (team cohesion and social support), leadership resources (recognition of superiors) and organizational resources (call screening). Likewise, our study has shown that police officers rely on adaptation strategies that can be individual (personalized work techniques, defensive strategies) or collective (mutual aid).
