Abstract:
Abstract Objectives Mindfulness-based approaches to treatment of sexual dysfunction associated with vulvar pain appear promising, but little is known about the specific processes of mindfulness in sexual interactions or if mindfulness may be of particular benefit to women who experience pain associated with sexual activity. This study aimed to examine the associations between sexual mindfulness and sexual function and distress among women who currently experience pain with sexual activity as compared to women who do not. Methods Women over the age of 18 were invited to complete an online survey, including measures of mindfulness in sexual activity, pain associated with sexual activity, sexual function, and sexual distress. Of the participants, 134 (42.1%) were experiencing pain and 184 (57.9%) were pain free. Results Higher levels of mindfulness during sexual activity were associated with greater sexual functioning and less sexual distress both among women with and without pain. However, the association between Observing skills and sexual functioning among women with pain was weaker if the capacity for nonreactivity was low. Sexual mindfulness contributed significantly to the explanation of sexual function and distress in both groups. Of the mindfulness facets, Acting with awareness consistently contributed, beyond the other facets, to the explanation of sexual outcomes. Conclusions The capacity to stay mindful in sexual interactions appears to be important to sexual functioning both among women who experience pain with sexual activity and among women who do not. These findings encourage further examination of the role of mindfulness and its facets in sexual outcomes.

Abstract In this study priapism is defined as a prolonged erection (> 45 minutes in the absence of sexual stimulation) or any painful erection. Boys and young men with SCD were eligible to participate in completing daily diary pages and overnight hormonal monitoring; the data presented are from 1486 diary days collected from 6 young men with mean age of 20.7 years (range 17 to 23). Eighty-four percent of these episodes were painful but not prolonged, ten percent were prolonged but not painful, and 4% were both. Of the painful erections, 42% lasted <15 minutes, 45% were >15 to 45 min, and 12% were >45 min to 3 hrs. The mean pain score was 4.3 out of 10. Painful events occurred most frequently between 6 am – 12 noon (37%); midnight (MN) to 6 am was the next most frequent time (29%). Erections occurred on 36% of days following a day with priapism; 75% of these erections were also painful or prolonged. Priapism and Normal Erectile Function: Erections occurred on a mean of 38% of all diary days. The proportion of erection days which were priapism days were .02, .06, .23, .24, .48 and .51 for the 6 subjects. Normal erections occurred most frequently between 6am – 12 noon (45%); 6pm – 12 MN was the next most frequent time (24%). Sexual activity (sex), defined as viewing stimulating media, masturbation, sexual intercourse or contact was reported on 22% of the total diary days. On 72% of days where sex was reported, erections were reported, vs 28% of days with no sex. Of erection days, painful erections were reported on 12% where sex occurred and on 20% of days where no sex occurred. Erection days were 50% of days following days with sex, vs 33% of days following days without sex. On erection days following sex days participants reported painful erections on 14% of the days versus 17% of the days following days with no sex. Conclusions: The data suggest some differences with the usual conception of priapism. Most of the episodes which the individuals reported as painful were short, the majority between 15 and 45 minutes, and a sizeable minority were < 15 minutes. The proportion of all erections that were priapic varied greatly within the group, but for 4 of the 6, 25–50% of days on which there was any erection there was priapism. The relationship between sexual activity and priapism in these data suggests fewer painful erections on days where there are both sex and erections, than on days when there are erections but no sexual activity. This is also true on the day following a day with sex—if any erection occurs on these days, fewer are painful than on erection days that occur after a day with no sex. Confirmation of the trends awaits the analysis of more subjects.

Objective.To investigate the association between plasma adipokine levels and the burden of painful joints among individuals with hip and knee osteoarthritis (OA).Methods.Adipokines (leptin, adiponectin, adipsin, resistin) were determined by ELISA (n = 78). Individuals reported painful joints on a homunculus. Associations were examined by sex-stratified Poisson analyses.Results.Adjusted for age, body mass index, and hip/knee OA, higher leptin and adiponectin and lower adipsin levels were associated with greater painful joint burden (i.e., counts) among women (p < 0.01). Among men, higher resistin levels were associated with lower counts (p = 0.03).Conclusion.Findings support the likelihood of a systemic-dependent sex-specific pain burden among individuals with OA.

Abstract:
Introduction. The unique demands of firefighting results in acute, recurrent, or chronic pain complications. We aimed to describe the percentage distribution of number and location of painful sites among FFs and determine whether work limitations differed based on the number or location of painful sites, age, and/or sex. Methods. About 325 firefighters completed a work limitation questionnaire (WLQ-26) and a checklist to indicate painful regions of the body using either a paper format or an online survey. A one-way ANOVA was employed to analyze the transformed work limitation scores; this was a two-sided test with a significance level of <0.05, to determine if work limitations differed among firefighters based on the number or location of painful sites, age, and/or sex. Results. The data analyzed consisted of 325 ( men = 216 , women = 109 ) FFs in total. The percentage distribution of the number of painful sites in our study cohort was 43% no pain, 17% one painful site, 19% two painful sites, and 21% three or more painful sites. The percentage distribution of the locations of painful sites was 43% no pain, 41% spine, 9% lower extremity, and 7% upper extremity. An estimated 31% of FFs ( n = 102 ) reported non-MSK comorbidities with 23% ( n = 76 ) reporting at least one non-MSK comorbidity and 8% ( n = 26 ) reported having two or more comorbidities. FFs > 45 years of age experienced more physical work limitations than FFs ≤ 45 years (mean difference: 0.74/10; 95% CI .19-1.29; p = 0.008 ). Conclusions. The majority of firefighters reported having at least one painful site and indicated the spine as the most common painful location. Age, the number of painful sites, and location of pain were identified as a potential contributor to physical/mental and work output limitations.

Category: Hindfoot; Other Introduction/Purpose: Plantar heel spur has been considered one of the reasons for heel pain in plantar fasciitis although its pathophysiology is not clear. The purpose of this study is to investigate the demographic and radiographic differences between the plantar fasciitis cohort and age/sex-matched control cohort, and identify risk factors that make plantar heel spurs painful. Methods: Patients who visited the office under the diagnosis of plantar fasciitis from September 2020 to December 2021 and age/sex-matched control cohort were included. Demographics and radiographic findings between the two groups were compared. A subgroup analysis was performed on only patients who had plantar heel spur. Patients who had plantar feel spur from the study group and the control group were extracted. All demographics and radiographic data were compared between the two subgroups and a multivariable logistic regression analysis was performed to identify independent risk factors that make plantar heel spur painful. Results: 76 patients were enrolled in the study group (Painful heel) and age/sex-matched control group (Non-painful heel). BMI was significantly higher in the study group compared to the control group: 33.2 vs 28.7 (P < .001). In the study group, plantar heel spur was present in 67.1% (51 out of 76), while in 38.2% (29 out of 76) in the control group (P < .001). In the subgroup analysis, BMI in the study group was significantly higher compared to the control group: 34.6 vs 30.3 (P < .001). All variables which can have an effect on painful plantar heel spur in patients with plantar fasciitis were included in a multivariable logistic regression analysis. The analysis through this subgroup comparison identified high BMI as an independent risk factor of painful plantar heel spur. Conclusion: High BMI was the one single independent predictor of painful heel spur resulting in plantar fasciitis heel pain. Concerning the treatment for plantar fasciitis, we need to counsel patients on emphasizing protection and cushioning for plantar heel to relieve axial loading and weight loss to decrease BMI.

Resumo: A Leucemia aguda se caracteriza pela proliferação clonal da célula tronco hematopoética substituindo o tecido normal da medula óssea. A recente classificação da OMS incorpora características cito genéticas e imunofenotípicas que apresentam impacto prognóstico. Nós examinamos a distribuição e impacto na sobrevida de vários antígenos de linhagem e maturação usados na rotina de imunofenotipagem dos pacientes com leucemia aguda ao diagnóstico.Os casos foram diagnosticados pelo hemograma, citologia da medula óssea, citoquímica, citogenética e imunofenotipagem através de um painel de triagem de 3 cores (CD45, CD3, CD7, CDI9, CD13, CD33 e HLA-DR) e painéis secundários para LLA de linhagem B (CDI0, CD20, k e À de citoplasma e superfície) e LMA (CD2, CDI4, CDI5, CD34, CD56 e MPO). A expressão dos antígenos foi medida pela intensidade média de fluorescência (CMF) por citometria de fluxo usando o programa Paint-a-gate. Nós analisamos 5 pacientes com LLA de linhagem T. Mediana de idade: 29 anos (14-51).80% dos casos foram diagnosticadosapenas com o painel de triagem.O CD13 foi o marcador de linhagem cruzada mais expresso (80%). Analisamos 31 pacientes com LLA de linhagem B. Mediana de idade: 27 anos (7-67). O painel de triagem definiu a linhagem de 61% dos casos e com o secundário diagnosticamos todos os outros. Os antígenos de linhagem cruzada mais expressos foram os CD13 (35%) e CD33 (38%). Na análise multivariada de Cox a idade e o CMF do CD45 foram fatores preditivos de pior sobrevida.Analisamos 35 pacientes com LMA.Mediana de idade: 51 anos (11-79). O painel de triagem definiu todos os casos. Os subtipos FAB predominantes foram M2 e M4. Nós detectamos 2 sub populações de blastos com fenótipos diferentes em 5 casos e 3 em 1 caso. O marcador de imaturidade CD7 foi expresso em 20% dos casos, os marcadores de linhagem cruzada, CD2 e CD19 em 11% e observamos assincronismode maturação em 20% com CD34/CD15 e 22% com CD34/CD56positivos.Na análise multivariadade Cox o CMF do CD45 e MPO, a idade, cariótipo e número de leucócitos foram fatores preditivos de pior sobrevida. Pela medida da intensidade média de fluorescência dos antígenos nós pudemos detectar parâmetros prognósticos bem como sub populações de blastos fenotipicamente diferentes, que não tiveram influência na sobrevida, mas podem dificultar o estudo de doença residual mínima por citometria de fluxo, visto que a quimioterapiapode selecionar uma das populações
Abstract: Acute leukemia is characterized by a c1onal proliferation of haematopoietic stem cell substituting the normal bone marrow. The recent WHO c1assification for acute leukemia separates entities by recurrent cytogenetic abnormalities and immunophenotypic features presenting prognostic impact. We have examined the distribution and impact on survival of the expression of severallineage and maturation linked antigens used in routine immunophenotyping of patients with de novo acute leukemía. Cases were diagnosed by peripheral blood counts,bone marrow cytology, cytochemistry, cytogenetics and immunophenotyping using a first screening panel with 3 colors (CD45, CD3, CD7, CDI9, CD13, CD33 e HLA-DR) and secondary panel for precursor B ALL (CDI0, CD20, k e À) and AML (CD2, CDI4, CD15, CD34, CD56 e MPO). Antigen expression was measured by mean fluorescence intensity (MFI) by flow cytometry using the Paint-a-gate software. We analysed 5 cases of T-ALL. Median age: 29 years (14-51). In 80% of the cases were defined by primary panel. CD13 was the most expressed cross-lineage marker (80%). We analysed 31 cases ofB-ALL. Median age: 27 years (7-67). In 61% of the cases were defined by screening panel and the remaining, by secondary panel defined others. CD13 (35%) and CD33 (38%) were the cross-lineage markers most expressed. In Cox multivariate analysis, MFI of CD45 and age were significant for worse prognosis. We analysed 35 cases of AML. Median age: 51 years (11-79). The screening panel defined all cases. Predomínant FAB types were M2 and M4. In 5 cases we could detect 2 phenotypically different blast subpopulations and in 1 case we detected 3 subpopulations. The immaturity marker CD7 was expressed in 20% ofthe cases, the cross-lineage CD2 and CD19 were expressed in 11% of the cases and we could observe asynchronous in 20% with co-expression of CD34/CD15 and 22% with co-expression of CD34/CD56. In Cox multivariate analysis, MFI of CD45 and MPO, age, WBC and karyotype were significant for worse survival. By measuring the fluorescence intensity of the antigens, we could detect prognostic parameters as well as phenotypically different blast subsets. These did not influence survival, but may represent a pitfall in studies of mínimal residual diseaseby flow cytometry, as chemotherapy may select one of these subsets

O presente trabalho avaliou numérica e experimentalmente a perda de transmissão sonora de painéis sanduíches com núcleo tipo colmeia utilizados internamente em aeronaves. Para tal, modelos numéricos foram desenvolvidos utilizando a metodologia de Análise Estatística de Energia (SEA) através do software SEAM3D e os resultados numéricos foram correlacionados com os resultados obtidos experimentalmente através do auxílio de duas câmaras reverberantes adjacentes. Para correlacionar o modelo, foi obtido experimentalmente às constantes elásticas de cada uma das camadas do painel sanduíche. Realizou-se um ensaio de análise modal experimental em uma placa plana e em uma viga, sendo esta última de acordo com a norma ASTM E 1876 - 01. Neste ensaio apurou-se que a norma ASTM E 1876 - 01 é ineficaz para a caracterização de painéis sanduíches com núcleo tipo colmeia. Foi construído um modelo de elementos finitos da amostra tipo placa utilizando os softwares PATRAN-NASTRAN e as propriedades mecânicas foram ajustadas através das formas e frequências modais obtidas numérica e experimentalmente. Também, fora alimentado o modelo de elementos finitos com as constantes elásticas fornecidas pelo fabricante do painel sanduíche, as quais proporcionaram erros de até 25% entre as frequências modais. Os fundamentos teóricos de materiais compostos, perda de transmissão sonora e análise estatística de energia são também apresentados
This study evaluated numerically and experimentally the sound transmission loss of honeycomb panels used to cover aircrafts internally. For this, numerical models were developed using Statistical Energy Analysis Methodology (SEA) through of SEAM3D software and the numerical results were correlated with those obtained experimentally using as facility, two couple reverberation rooms. For match the model, the elastic constants for each layer of the honeycomb panel were obtained experimentally. Tests of experimental modal analysis were conducted on a flat plate and beam, for the beam, the tests were made in accordance with ASTM 1876 - 01 Standard. In this way it was found that this Standard is ineffective for characterization of honeycomb panels. A finite element model (FEM) of a plate was built using PATRAN-NASTRAN softwares and the mechanical properties were matched through of the modal shapes and frequencies obtained numerically and experimentally. The elastic constants provides by manufacturer of a honeycomb panel were assessed in this thesis using the same FEM, which have shown errors around of 25% between the modal frequencies. The theoretical foundations of composite materials, sound transmission loss and statistical energy analysis are also presented

Nos ?ltimos anos, a eleva??o cont?nua no volume de emiss?es de di?xido de carbono (CO2) e sua concentra??o na atmosfera colocou o tema da mudan?a clim?tica no centro do debate pol?tico-econ?mico mundial. Atualmente, os maiores emissores de CO2 do mundo s?o os pa?ses emergentes, denominado BRICS, que v?m apresentando um crescimento econ?mico expressivo e respondendo por mais de 40% das emiss?es totais deste poluente no planeta (EIA, 2016). Em termos anal?ticos, o estudo da rela??o entre crescimento econ?mico e emiss?es de poluentes t?m sido feito com base na abordagem da Curva de Kuznets Ambiental (CKA), a qual implica a exist?ncia de uma rela??o no formato de um ?U? invertido entre polui??o e renda. Desta forma, ? medida que a renda alcan?a um determinado patamar as emiss?es de poluentes come?am a declinar, fazendo com que haja uma melhora na qualidade ambiental. Neste sentido, o objetivo geral desta pesquisa foi o de verificar se existe uma CKA para o grupo de pa?ses do BRICS utilizando a metodologia econom?trica de cointegra??o em dados em painel. Al?m da vari?vel PIB per capita, foram adicionadas mais tr?s vari?veis ao modelo: consumo per capita de energia, fluxo de com?rcio e uma dummy para captar os efeitos do Protocolo de Quioto sobre o n?vel de emiss?es destes pa?ses. Os resultados do modelo econom?trico indicaram a exist?ncia de uma rela??o no formato de um ?U? invertido entre renda per capita e emiss?es de CO2, com pontos de inflex?es de US$ 2.033,89 e US$ 2.057,61 respectivamente, nos estimadores FMOLS e DOLS. O consumo per capita de energia apresentou uma rela??o positiva com as emiss?es de CO2, enquanto a rela??o da vari?vel de com?rcio internacional foi negativa. Al?m disso, o coeficiente da vari?vel dummy n?o apresentou signific?ncia estat?stica. Assim, julga-se necess?rio a implementa??o de pol?ticas ambientais nos pa?ses do BRICS que estimulem o crescimento econ?mico com aumento da participa??o de fontes renov?veis, melhor efici?ncia energ?tica e transa??es comerciais que envolvam a transfer?ncia de tecnologia menos poluente para estes pa?ses. In recent years, a steady rise in carbon dioxide (CO2) emissions and its own concentration on the atmosphere put the climate change issue at the center of the global political-economic debate. Today, the world's largest emitters of CO2 are the emerging BRICS countries, which show significant economic growth and account for more than 40% of total emissions of this pollutant on the planet (EIA, 2016). In analytical terms, the study of the relationship between economic growth and pollutant emissions has been made based on the Environmental Kuznets Curve (CKA) approach, which implies an shaped relationship of an inverted "U" between pollution and Income In this way, as income reaches a certain level the emissions of pollutants begin to decline, resulting an improvement in environmental quality. In this sense, the general objective of the research is to verify if there is an evidence of CKA hypothesis for the BRICS group of countries using an economic methodology of panel data cointegration. In addition to the GDP per capita variable, three more variables were added to the model: per capita energy consumption, trade flow and a dummy to capture the effects of the Kyoto Protocol on countries' emissions levels. The results of the econometric model indicated an existence of a relationship of an inverted "U" between per capita income and CO2 emissions, with inflection points of US $ 2,033.89 and US $ 2,057.61, respectively, in the FMOLS and DOLS. The per capita consumption of energy showed a positive relation with CO2 emissions, while a relation of the international trade variable was negative. In addition, the coefficient of the dummy variable, which measures the effects of the Kyoto Protocol, was not statistically significant. Thus, it is considered necessary to implement environmental policies in BRICS countries that stimulate economic growth by increasing the share of renewable sources, better energy and transactions that imply a transfer of less polluting technology to these countries. In addition, it is believed that an international agreement to reduce CO2 emissions that also includes developing countries is extremely important to reduce the level of emissions and keep the planet's temperature at a stable level.

A falta de água potável em algumas localidades do mundo, mais precisamente no semi-árido, é um problema de caráter secular, agravado pela poluição, distribuição irregular e existência de águas subterrâneas de utilização bastante limitada por causa dos altos índices de salinidade. Por outro lado, essas regiões possuem um grande potencial de irradiação solar que favorece a utilização de painéis fotovoltaicos, principalmente em locais distante da rede elétrica. Visando melhorar a qualidade de vida e minimizar o problema da falta de água potável nessas regiões, esse trabalho tem como objetivo estudar o desempenho de um protótipo de equipamento para fins de dessalinizar águas salobras via osmose inversa, fazendo uso de um gerador fotovoltaico com a utilização de um circuito eletrônico, no qual substitui acumuladores de carga (baterias). Com o uso do circuito eletrônico foi possível produzir água potável sem a necessidade da utilização de acumuladores de carga (baterias) e sem danificar o motor-bomba. A ausência do banco de baterias proporciona uma redução dos custos e da necessidade de manutenção; mas o sistema torna-se subordinado às condições instantâneas da radiação solar e da temperatura ambiente. Esse sistema foi instalado no Laboratório de Referência em Dessalinização (LABDES), na Universidade Federal de Campina Grande. O desempenho do sistema foi avaliado com soluções de cloreto de sódio com concentração de 1.000 mg/L e 2.500 mg/L e água de poço com concentração de 3.800 mg/L. As variáveis de medidas, como: tensão, corrente, vazão e pressão foram obtidas em tempo real no Software Data Aquisition, através de sensores elétricos. A partir desses dados foram avaliados os seguintes parâmetros: potência elétrica, vazão do permeado e do concentrado, recuperação e consumo de energia por m3 de água produzida. As condições meteorológicas e as concentrações da água de alimentação influenciam diretamente na pressão, e consequentemente na produção de água. Mesmo com a grande variação de tensão em função da irradiação solar, o sistema mostrou condições técnicas de produzir 0,25 m3/h de água dessalinizada. A pressão máxima obtida foi de 8,2 kgf/cm2. O balanço econômico, que representa o custo-benefício de implantação, operacional e manutenção para atender comunidades isoladas, mostrou que o menor custo de água dessalinizada é obtido quando o sistema de dessalinização é instalado em localidade na qual já possui o poço tubular e um local para servir de abrigo para o sistema de dessalinização. Para essas localidades é viável o uso de painéis fotovoltaicos.
The lack of drinking water in some places of the world, more precisely in the semi-arid region of Brazil, is an old problem, worsened by the pollution, irregular distribution and high salinity of underground waters. On the other hand, those areas possess a great solar irradiation rate that favors the use of photovoltaic panels, mainly where the electric net is not available. Seeking to improve the life quality and to minimize the problem of the lack of drinking water in those areas, the objective of this work is to study and evaluate an osmosis reverse desalination system, using photovoltaic solar energy coupled to an electronic circuit. This electronic circuit replaces the use of batteries without damaging the motor-bomb. The absence of batteries provides a reduction of the cost and maintenance need; but the system is subordinated to the instantaneous conditions of the solar radiation. This system was installed at the Reference Laboratory of Desalination (LABDES) in the Federal University of Campina Grande. The system was evaluated with sodium chloride solutions of 1,000 and 2,500 mg/L NaCl concentrations, and a well water of 3,800 mg/L TDS concentration. The meteorological conditions and the concentrations of the feeding water influence directly the applied pressure and consequently the permeate production. Even with the intermittent energy delivered by the solar photovoltaic panels, the desalination system showed good technical conditions to produce 0.25 m3/h of potable water. The economical statements , that the implantation cost-benefit represents, operational and maintenance to take care of isolated communities, showed that the lesser desalinizaded water cost is gotten when the desalination system is installed in locality in which already possesss the well and a place to serve of shelter for the desalination system. For these localities the use of photovoltaics panels is viable.

IntroduÃÃo: MutaÃÃes nos genes GNAS e AIP estÃo presentes em 35% e 3%, respectivamente, dos somatotrofinomas esporÃdicos. Recentemente, observa-se importÃncia biolÃgica crescente dos microRNAs (miRNAs) na tumorigÃnese hipofisÃria. Entretanto, mecanismos moleculares envolvidos na patogÃnese de 60% desses tumores permanecem nÃo elucidados. Objetivos: Identificar a prevalÃncia de mutaÃÃes nos genes GNAS e AIP em um grupo de somatotrofinomas esporÃdicos. Comparar parÃmetros clÃnicos e bioquÃmicos ao diagnÃstico como idade, tamanho tumoral e agressividade (escore Hardy), hormÃnio do crescimento (GH), prolactina (PRL) e Fator de Crescimento Insulin-Like I (IGF-1) e resposta as terapÃuticas entre os grupos com (gsp+) e sem (gsp-) mutaÃÃo no GNAS. Analisar a expressÃo dos genes GNAS e BTG2 e miRNAs entre somatotrofinomas e hipÃfises normais (HN) e a associaÃÃo entre a expressÃo com agressividade, a resposta à cirurgia e a todas as terapÃuticas adjuvantes disponÃveis. Material e MÃtodos: 26 pacientes com diagnÃstico de acromegalia. Tamanho tumoral foi avaliado por RNM/CT e o grau de invasibilidade pelo escore de Hardy (I a IV). GH basal ≤2.5μg/L ou nadir de GH apÃs o GTT≤1μg/L e IGF-1 normal para idade e sexo foram utilizados como critÃrio de cura apÃs cirurgia transesfenoidal (CTE). Como controle com o anÃlogo da somatostatina (AS), adotamos a normalizaÃÃo dos nÃveis de IGF-1 para idade e sexo. As amostras tumorais (n=26) foram obtidas durante a CTE, realizado histopatolÃgico e armazenadas a -70 ÂC, para estudos moleculares. HN (07) foram obtidas durante autÃpsias. RNA e DNA total foram extraÃdos pelo TRIzolÂ. Os cÃdons 201 e 227 do gene GNAS e o AIP completo foram sequenciados. ExpressÃo relativa dos genes GNAS e BTG2 e dos miRNAs let-7a, miR-16a, miR-21, miR-141, miR-143, miR-15a, miR-145, miR-23a, miR-23b e miR-24-2 foi avaliada por qPCR (sondas TaqMan), pelo mÃtodo 2-ΔΔCt. Resultados: A frequÃncia de mutaÃÃes no GNAS foi de 35% e no AIP 3,8%. NÃo houve diferenÃa entre as mÃdias de idade (39,0Â11,5 vs 43,6Â9,0 anos; p=0,32), nas concentraÃÃes plasmÃticas basais de GH (62,4Â128,1 vs 39,9Â48,3Âg/L; p=0,39), IGF-1 (435,5Â230,8 vs 556,9 238,3 %ULNR; p=0,32), PRL (25,7Â29,8 vs 30,9Â32,8 ng/L; p=0,69) e agressividade tumoral entre os gsp+ e gsp-(p=1,00). Ao analisar o uso do AS como terapÃutica adjuvante à CTE, observamos que 04/05 (80%) dos indivÃduos com somatotrofinoma gsp+ obtiveram controle da doenÃa, enquanto que no grupo gsp- 02/06 (33%) obtiveram controle (p=0,08). Quando associamos ao AS, os agonistas dopaminÃrgicos e/ou radioterapia externa, observamos que 05/05 (100%) dos pacientes gsp+ tiveram critÃrio de controle da doenÃa, contra (04/09) 44% no grupo gsp- (p=0,09). NÃo houve diferenÃa na expressÃo de GNAS entre os somatotrofinomas e as HN (1,07Â0,55 vs 0,98Â0,28; p=0,97), e entre os gsp+ e gsp- (1,04Â0,59 vs 1,10Â0,55; p=0,97, respectivamente). Os tumores Hardy I / II apresentaram maior expressÃo do GNAS do que os tumores classificados como III / IV (p=0,02). NÃo houve associaÃÃo entre a expressÃo do GNAS e o controle da doenÃa com cirurgia isolada ou com o uso de todas as terapÃuticas adjuvantes. Observamos hipoexpressÃo do BTG2 e dos miR-16a e miR-141 em somatotrofinomas quando foram comparados com as HN (p=0,002, fold=-6,63; p=0,01, fold=-10,00; p=0,0003, fold=-50,00, respectivamente) sem diferenÃas entre os gsp+ e gsp-. Houve hiperexpressÃo do miR-21 (p=0,02;fold=10,18) em somatotrofinomas (20,16Â18,48) quando comparado com as HN (2,52 Â3,56), sem diferenÃa entre os gsp + e gsp-. NÃo houve diferenÃa na expressÃo entre os grupos gsp+ e gsp- para os miRNAs let-7a, miR-21, miR-143, miR-15a, miR-23a e miR-24-2. Entretanto, miR-145 e miR-23b foram mais hipoexpressos no grupo gsp+ quando comparados ao gsp- (p=0,03, fold=-4,83 e p=0,02, fold=-2,77, respectivamente). NÃo houve associaÃÃo entre a expressÃo do BTG2 e o painel de miRNAs com agressividade e com o controle da doenÃa. ConclusÃo: Na presente sÃrie de somatotrofinomas, assumidos como esporÃdicos, a frequÃncia de mutaÃÃes nos genes GNAS (35%) e AIP (3,8%) foram semelhantes aos relatados na literatura. NÃo houve diferenÃas nas caracterÃsticas clÃnicas e bioquÃmicas, agressividade, resposta Ãs terapÃuticas, e na expressÃo diferencial do GNAS entre os pacientes com tumores gsp+ e gsp-. HipoexpressÃo de BTG2 (gene supressor tumoral relacionado Ãs vias de sinalizaÃÃo do p53 e do Rb), baixa expressÃo de miRNAs (supressores tumorais) e alta expressÃo de oncomirs em somatotrofinomas sugerem um papel desses na tumorigÃnese somatotrÃfica.

However, the molecular mechanisms involved in the pathogenesis of 60% of these tumors remain to be elucidated. Objectives: To identify the prevalence of mutations in GNAS and AIP genes in a series of sporadic somatotropinomas. Compare clinical, bioquimical parametrer at diagnosis as age, tumor size and theirs aggressiveness, pre-operative growth hormone (GH), prolactin (PRL) and insulin-like growth factor-I (IGF-1) levels and treatment responsiveness between somatotropinomas with (gsp+) and without (gsp-) GNAS mutation.To analyze the expression of GNAS and BTG2 genes and a panel of miRNAs between somatotrofinomas and normal pituitaries (NP) and the association between the expression of these genes and miRNAs with aggressiveness, as well as disease control with surgery or control with all adjuvant therapeutic approaches. Material and Methods: 26 patients with acromegaly. GH basal ≤2.5μg/L or nadir after OGTT ≤1μg/L and normal IGF-I matched for age and sex were used as diagnosis and for cure criteria after transsphenoidal surgery (TS). As control after somatostatin analogues (SA), we adopted the normalization of IGF-I matched for age and sex. Tumor size was evaluated by MRI/CT and the degree of invasiveness by Hardy score (I to IV).Tumor samples (26) were obtained during TS, processed for histopathology and stored at -70ÂC for molecular studies. NP (07) were obtained during autopsy. Total DNA and RNA were extracted by TRIzolÂ. Codons 201 and 227 of the GNAS gene and the whole AIP gene were sequenced. Relative expression of BTG2 and GNAS genes and miRNAs let-7a, miR-16a, miR-21, miR-141, miR-143, miR-15a, miR-145, miR-23a, miR-23b, and miR-24-2 was measured by qPCR (TaqMan probes) using 2-ΔΔCt method. Results: Frequencies of GNAS and AIP mutations were 35% and 3.8%, respectively. Material e Métodos: 26 pacientes com diagnóstico de acromegalia. Tamanho tumoral foi avaliado por RNM/CT e o grau de invasibilidade pelo escore de Hardy (I a IV). GH basal ≤2.5μg/L ou nadir de GH após o GTT≤1μg/L e IGF-1 normal para idade e sexo foram utilizados como critério de cura após cirurgia transesfenoidal (CTE). Como controle com o análogo da somatostatina (AS), adotamos a normalização dos níveis de IGF-1 para idade e sexo. As amostras tumorais (n=26) foram obtidas durante a CTE, realizado histopatológico e armazenadas a -70 °C, para estudos moleculares. HN (07) foram obtidas durante autópsias. RNA e DNA total foram extraídos pelo TRIzol®. Os códons 201 e 227 do gene GNAS e o AIP completo foram sequenciados. Expressão relativa dos genes GNAS e BTG2 e dos miRNAs let-7a, miR-16a, miR-21, miR-141, miR-143, miR-15a, miR-145, miR-23a, miR-23b e miR-24-2 foi avaliada por qPCR (sondas TaqMan), pelo método 2-ΔΔCt. Resultados: A frequência de mutações no GNAS foi de 35% e no AIP 3,8%. Não houve diferença entre as médias de idade (39,0±11,5 vs 43,6±9,0 anos; p=0,32), nas concentrações plasmáticas basais de GH (62,4±128,1 vs 39,9±48,3µg/L; p=0,39), IGF-1 (435,5±230,8 vs 556,9± 238,3 %ULNR; p=0,32), PRL (25,7±29,8 vs 30,9±32,8 ng/L; p=0,69) e agressividade tumoral entre os gsp+ e gsp-(p=1,00). Ao analisar o uso do AS como terapêutica adjuvante à CTE, observamos que 04/05 (80%) dos indivíduos com somatotrofinoma gsp+ obtiveram controle da doença, enquanto que no grupo gsp- 02/06 (33%) obtiveram controle (p=0,08). Quando associamos ao AS, os agonistas dopaminérgicos e/ou radioterapia externa, observamos que 05/05 (100%) dos pacientes gsp+ tiveram critério de controle da doença, contra (04/09) 44% no grupo gsp- (p=0,09). Não houve diferença na expressão de GNAS entre os somatotrofinomas e as HN (1,07±0,55 vs 0,98±0,28; p=0,97), e entre os gsp+ e gsp- (1,04±0,59 vs 1,10±0,55; p=0,97, respectivamente). Os tumores Hardy I / II apresentaram maior expressão do GNAS do que os tumores classificados como III / IV (p=0,02). Não houve associação entre a expressão do GNAS e o controle da doença com cirurgia isolada ou com o uso de todas as terapêuticas adjuvantes. Observamos hipoexpressão do BTG2 e dos miR-16a e miR-141 em somatotrofinomas quando foram comparados com as HN (p=0,002, fold=-6,63; p=0,01, fold=-10,00; p=0,0003, fold=-50,00, respectivamente) sem diferenças entre os gsp+ e gsp-. Houve hiperexpressão do miR-21 (p=0,02;fold=10,18) em somatotrofinomas (20,16±18,48) quando comparado com as HN (2,52 ±3,56), sem diferença entre os gsp + e gsp-. Não houve diferença na expressão entre os grupos gsp+ e gsp- para os miRNAs let-7a, miR-21, miR-143, miR-15a, miR-23a e miR-24-2. Entretanto, miR-145 e miR-23b foram mais hipoexpressos no grupo gsp+ quando comparados ao gsp- (p=0,03, fold=-4,83 e p=0,02, fold=-2,77, respectivamente). Não houve associação entre a expressão do BTG2 e o painel de miRNAs com agressividade e com o controle da doença. Conclusão: Na presente série de somatotrofinomas, assumidos como esporádicos, a frequência de mutações nos genes GNAS (35%) e AIP (3,8%) foram semelhantes aos relatados na literatura. Não houve diferenças nas características clínicas e bioquímicas, agressividade, resposta às terapêuticas, e na expressão diferencial do GNAS entre os pacientes com tumores gsp+ e gsp-. Hipoexpressão de BTG2 (gene supressor tumoral relacionado às vias de sinalização do p53 e do Rb), baixa expressão de miRNAs (supressores tumorais) e alta expressão de oncomirs em somatotrofinomas sugerem um papel desses na tumorigênese somatotrófica.

Neste estudo pretendo acompanhar alguns aspectos do imaginário moderno no alto sertão da Bahia, tomando como pontos de partida os efeitos da Revolução Científico-Tecnológica e a questão da modernidade. Ferrovias, automóveis, aparelhos sonoros e luminosos, transformações urbanas, controle dos recursos naturais, velocidade de comunicação, eram elementos orquestradores da vida moderna no imaginário de muitos moradores do alto sertão entre aos anos de 1910 e 1930. Esse período pode ser caracterizado pelo alargamento de expectativas com a promessa da chegada da Estrada de Ferro Central da Bahia, além dos planejamentos de sistemas viários e a possibilidade da utilização de modernos meios de transporte no comércio e na agricultura. Percorrendo diversas fontes como jornais, relatórios, atas, fotografias e correspondências – em sua maioria do acervo do Arquivo Público Municipal de Caetité – procuro demonstrar os alcances e contradições presentes na criação de um imaginário moderno, focalizando especialmente a cidade de Caetité e seus arredores
In this study, I intend to analyse some aspects of modern imaginary in the Alto Sertão da Bahia, having as its axis of research the effects of Scientific-Technological Revolution and the question of modernity. Railways, automobiles, sound and light equipment, urban transformations, control of natural resources, communication speed, were elements that orchestrated the imaginary of many residents modern life, in the high arid lands of Bahia between 1910 and 1930. This period can be characterized by the rise of expectations with the promise of the arrival of Bahia Central Railway (Estrada de Ferro Central da Bahia), in addition to the planning of road systems and the possibility of the use of modern means of transport in trade and agriculture. Through various sources such as newspapers, reports, protocols, photographs and correspondence – mostly from the Caetité Municipal Public Archives – I am looking to demonstrate the scope and contradictions present in the creation of a modern imaginary, especially focusing on the city of Caetité and its surroundings

One of the most problems faced by the financial manager is to maintain a balance between resources and applications of short and long term. Working Capital Management appears as a tool to improve the profitability of the company without representing a loss in their ability to pay. This study aims to evaluate if the financial performance achieved by companies belonging to the Stock Exchange of Sao Paulo, from 1999 to 2008, has a dependent relationship with the companies liquidity, measured by Fleuriet´s Model. Also know as Dynamic Analysis Model, the Fleuriet s Model consists of an analysis technique of balance sheets that evaluates the liquidity perspective of operations. and to that end, the balance sheet accounts are reclassified. In this study, the variable Fleuriet s Model was measured by three ways: first just from the perspective of liquidity, with the measurement of the Treasury s Balance and Liquidity s Index; and then from the perspective of the model as a whole, being taken an ordinal scale represent the possible financial structures. In relation to the financial performance adopted as a measure of corporate profitability, the Return on Equity (ROE), and a measure of stock return, the Earnings per Share (EPS). The corporate data used in calculo of variables were collected from financial statements, while the macroeconomic variables were obtained from institutions such as the IPEA and the Central Bank. The data were analyzed with a technical of Panel Data. The results of the initial estimate for the population, indicated that relationship of dependence between the ROE and the Fleuriet s Model and between the LPA and the Fleuriet s Model. When market sector is considered, the Fleuriet s Model is significant and has the opposite pattern expected. For the metalworking and steel industry, the variable Fleuriet s Model significant was Treasury s Balance. Since the textile sector presented the Liquidity s Index as a variable Fleuriet s Model significant. Finally, we have that, although it has been found relationship between the economic and financial performance and the Fleuriet s Model, the estimated models had low explanatory power, making it difficult to generalize the results.
Um dos grandes problemas enfrentados pelo gestor financeiro está na manutenção de um equilíbrio entre recursos e aplicações de curto e longo prazo. A administração do Capital de Giro aparece como uma ferramenta capaz de melhorar a rentabilidade da empresa, sem que isso represente uma perda na sua capacidade de pagamento. Este estudo tem por objetivo avaliar se o desempenho econômico-financeiro obtido pelas empresas pertencentes à Bolsa de Valores de São Paulo, no período de 1999 a 2008, tem relação de dependência com a liquidez das mesmas, mensurada através do Modelo Fleuriet. Também conhecido como Modelo de Análise Dinâmica, o Modelo Fleuriet consiste em uma técnica de análise de balanços que avalia a liquidez pela perspectiva das operações, sendo que, para tal, as contas do balanço patrimonial são reclassificadas. Neste estudo, a variável Modelo Fleuriet foi representada por três medidas: primeiramente, apenas sob a ótica da liquidez, com a mensuração do Saldo de Tesouraria e do Índice de Liquidez; e, posteriormente, sob a ótica do modelo como um todo, sendo adotada uma escala ordinal, para representar as estruturas financeiras possíveis. Já em relação ao desempenho econômico-financeiro, adotou-se uma medida de rentabilidade corporativa, o Retorno sobre o Capital Próprio (ROE), e uma medida de retorno acionário, o Lucro por Ação (LPA). Os dados corporativos utilizados nos cálculos das variáveis foram coletados junto às demonstrações financeiras, enquanto que as variáveis macroeconômicas foram obtidas junto às instituições como o IPEA e o Banco Central. Os dados foram analisados sob a técnica de Dados em Painel. Os resultados da estimação inicial, para a população, indicaram a existência de relação de dependência entre o ROE e o Modelo Fleuriet e entre o LPA e o Modelo Fleuriet. Quando o setor de atuação é considerado, o Modelo Fleuriet mostra-se significativo e apresenta o comportamento inverso esperado. Para o setor de metalurgia e siderurgia, a variável do Modelo Fleuriet significativa foi o a Estrutura de Balanço. Já o setor têxtil apresentou o Índice de Liquidez como a variável do Modelo Fleuriet significativa. Por fim, tem-se que, embora tenha sido encontrada relação entre o desempenho econômico-financeiro e o Modelo Fleuriet, os modelos estimados apresentaram baixo poder de explicação, dificultando a generalização dos resultados.

Atualmente, há um reconhecimento generalizado sobre o papel e a eficácia das Instituições de Solidariedade Social, quer como promotoras de Desenvolvimento Económico quer como agentes de Crescimento Económico. No entanto, para estas instituições subsistirem precisam de apoios, quer de pessoas particulares quer de agentes públicos. Analisando o caso português, entre os anos de 2003 a 2007 (por disponibilidade de dados) observou-se nesta Dissertação que a maioria das Instituições de Solidariedade Social recorre a apoios oriundos das autarquias como fontes de receitas e de proveitos que aguentam a atividade destas mesmas Instituições de Solidariedade Social. Assim, este trabalho vai procurar identificar os principais determinantes responsáveis pela distribuição heterogénea que se observou para os 308 municípios do país. Os resultados empíricos mostraram que municípios atentos a investimentos de índole social ou cultural são os mais generosos na atribuição de transferências por residente no município. Em contrapartida, municípios muito populosos tendem a sentir necessidades diversificadas o que lhes deixa menor margem para a atribuição de transferências para as Instituições de Solidariedade Social. Observou-se, ainda que, nalguns casos, a presença de dinâmica empresarial acentuada facilita a canalização de fundos vindos das autarquias para as Instituições de Solidariedade Social.
Nowadays, there is a widespread recognition of the role and effectiveness of social solidarity institutions, either as promoters or as agents of Economic Development and of Economic Growth. However, these institutions need to be supported, either from individuals’ income or from public grants. Analyzing the Portuguese case, between the years 2003 to 2007 (because of data availability) it was observed in this thesis that most institutions of social solidarity receive their support from local municipalities as sources of revenue and income. This study will seek to identify the major determinants responsible for this heterogeneous distribution that was observed for the 308 municipalities of the country. The empirical results show that municipalities that pay attention to cultural investments are the most generous in the allocation of transfers/grants per resident to the Third Sector organizations headed in the region. In contrast, very populous counties tend to have diversified needs that leaves less space for the allocation of transfers to institutions of social solidarity. It was also observed that in some cases the presence of strong business dynamics facilitates the channeling of funds from municipalities to the Social Solidarity Institutions.

Sjögren’s can affect women’s sexual function. Physical symptoms that can occur with Sjögren’s include vaginal dryness and pelvic pain. The age at which people are diagnosed with Sjögren’s may also coincide with perimenopause or menopause, with accompanying hormonal changes. Other factors, such as mood, fatigue, and general challenges of living with a chronic illness, can in turn affect sexual function and sexual satisfaction for women with Sjögren’s. Dry glandular tissue leads Sjögren’s patients to experience symptoms associated with sexual dysfunction. These include pelvic dryness (with painful intercourse), oral dryness (that can affect kissing), and bronchial changes making it difficult to bring up mucus. Inflammation also correlates with a reduction in libido and fatigue. The management of sexual dysfunction in Sjögren’s includes local moisturizing measures, secretogogues, anti-inflammatory drugs, anxiety reduction measures, hormonal interventions, and relaxation techniques. A multidisciplinary, supportive approach is important.

A majority of the men interviewed were predominantly attracted to women, not men. They described complicated situations and feelings that reworked sex with men as necessary and compatible with straight identification and masculinity. Several reported having sexual kinks for penises but not sexual attractions to men. About three quarters reported unintentional changes to their sexual attractions or desires as they grew older. Additionally, over a third reported that their wives lost interest in sex, sometimes because it became painful or uncomfortable. Most preferred women for romantic partnerships, but were wary of casual hookups with women. The trait they felt made women better romantic partners—emotional compatibility—also made many uncomfortable with women as hookup partners, as they felt that women would become too attached. This was especially true for married men, most of whom did not want to threaten their marriage by having sex with another woman. Others had sex with men specifically to form nonromantic connections with men, such as to relieve loneliness or satisfy a need for touch. Several had sex with men to relieve the pressures of masculinity. Having another man penetrate them allowed some men to experience pleasure without having to be in control.

This chapter explores a question often asked about survivors of clerical sexual abuse: do they remain Catholic? Such a question, this chapter argues, fails to account for the complex reality. Survivors were abused as youngsters so they were usually unable to determine this for themselves. The insistence of adults that children and teenagers who were abused continue going to church was another way of denying the reality of the abuse. (“They drove me to my abuser,” one survivor said of his parents.) Many survivors remained faithful Catholics into adulthood. But most survivors describe a moment when being at Mass became physically and emotionally painful. For many the decision to stay or leave was not simple or final. Some survivors developed strategies for protecting themselves from further fear and harm as they continued attending Mass; others found ways of being both inside and outside the church; still others made different choices over time. The struggle of many survivors with the church in which they were religiously formed, encountered the sacred, and were abused—abuse that always had religious context and significance—offers a revealing perspective on Catholics and Catholicism in the late twentieth and early twenty-first centuries.

In this chapter, Johnson’s interlocutors share stories about sexual awakenings, sexual encounters, sexual desire, and sexual violence. Since their narratives move between and beyond pleasure and pain, the chapter is divided into three sections—the first focuses on sexual awakenings and the narrators’ various emotional attachments to sex with other women; the second centers women’s sexual behaviors and practices; and the third focuses squarely on these women’s painful experiences of sexual violence, often in adolescence and at the hands of male relatives. Importantly, these narrators stress that their sexual desires have not developed in response to these violent experiences, illustrating that their articulation of same-sex attraction is conscious and affirmative.

I question its interpretation by scholars as a self-evidently sexually mimetic, punitive murder method: in fact, the earliest accounts of this murder present it primarily as painful, torturous, and undetectable through outward inspection. Importantly, too, these earliest accounts emerge before the formation of a consensus on whether Edward’s transgressions were sexual, let alone whether they specifically constituted sex with men. This analysis prompts a reassessment of the place of this narrative in the history of queer sexuality, and of the murder scene in Marlowe’s Edward II, while also further illuminating the literary priorities of medieval and early modern chroniclers.

Abstract:
A female patient is presented, who developed thrombotic and hemorrhagic skin necrosis of the feet and toes during an acute episode of severe vitamin K deficiency due to cholestasis in the absence of coumarin treatment. Painful blue toes and feet progressed to erythematous swelling and bluish discoloration with blister formations and immanent gangrene. The histo-pathology of skin excisions from the erythematous skin lesions showed extravasation of erythrocytes and extensive fibrin thrombus formations in capillaries and venules as has been described in patients with coumarin skin necrosis. At the time of painful acrocyanosis the results of coagulation investigations (platelets 109 × 109/1, APTT 56/35 sec., PT 61/15 sec., ThrombotestR (TT) less than 3%, NormotestR (NT) less than 10%, fibrinogen 1,6 g/l, absence of fibrin monomers and degradation products, factor V 1.00 u/ml, antithrombin lil 1.03 u/ml and alfa2 antiplasmin 0.97 u/ml were consistent with severe vitamin K deficiency. Measurements of vitamin K dependent factors revealed very low levels for procoagulant factor VII (16%) and protein C (22%) antigen concentration and normal levels for procoagulant factor II (97%) and procoagulant factor X (87%) antigen concentration. After substitution of 5 mg vitamin K1 both the TT and NT normalized.These data confirm the hypothesis that an imbalance within procoagulant and anticoagulant vitamin K dependent factors (severe protein C and factor VII deficiency as compared to the procoagulant factors IX, X and II due to rapid development of vitamin K deficiency) contributes to the pathogenesis of thrombo-hemorrhagic skin necrosis and that the so-called coumarin necrosis is not due to drug toxicity.

Esta pesquisa teve como objetivo analisar o efeito da variação do número de camadas de reforço no comportamento a flexão de painéis sanduiches com faces em compósitos cimentícios reforçados com tecido de juta e núcleo de poliestireno extrudado (XPS). A preferência da utilização de fibras naturais e não as sintéticas deve-se ao fato do custo e produção serem reduzidos. O núcleo em XPS proporciona a redução do peso do painel, aumentar o seu momento de inercia, além de melhorar o isolamento térmico. Para melhorar a aderência e prevenir a delaminação entre o núcleo e a face foram realizadas ranhuras em paralelos no núcleo com espessura e profundidade de dois milímetros espaçadas igualmente em 20 milímetros. Para caracterização do material utilizado, foram realizados ensaios de tração direta nas fibras, fios e nos tecidos nas direções da trama e urdume, após a análise dos resultados os tecidos na direção do urdume tiveram um desempenho ligeiramente superior à trama. Foram produzidos 5 painéis com o mesmo tipo de núcleo variando de 1 a 5 camadas de reforço na face. Os resultados dos ensaios de flexão em quatro pontos mostram que o desempenho do painel sanduíche é diretamente afetado pelo número de camadas de reforço, tendo suas tensões máximas, rigidez e tenacidade evoluindo gradativamente até o painel com quatro camadas de reforço. Já o painel com cinco camadas de reforço teve seu desempenho inferior.,

Low back pain or low back pain is a set of painful manifestations, affecting 90% of the population, including the elderly and young people. It has numerous causes, such as joint degeneration, biomechanical and functional changes, incorrect posture, among others. Thus, there is limitation in ROM, pain, burning sensation and functional incapacity. Electrotherapy is the use of electrical currents for different therapeutic purposes. It is used to control pain, reduce edema and muscle spasm. To identify the effects of electroanalgesia on pain in patients with low back pain chronic. Anon-systematic review of the literature was performed, where randomized clinical trials published between 2006 and 2020 were selected, according to the highest score in the scorePEDro. The search involved the databases PEDro e PubMed, through the following terms: electroanalgesia, chronic, back pain, backache, TENS, electrotherap, placebo, massage. Six studies with a PEDro score between 7 and 10 were selected. In all studies, electroanalgesia was compared with massage or placebo. In five studies, when TENS was compared with massage, there was an immediate reduction in pain, improvement in disability and a reduction in medication consumption. In one study, after ten sessions there was no difference between TENS and interferential current. Another study showed that laser was more effective than TENS in reducing pain. There is no difference between TENS and interferential current. Electroanalgesia appears, in the short term, to reduce pain, medication consumption, as well as improve disability. Laser is more effective than TENS in reducing pain.

Em um painel fotovoltaico, em virtude de obstáculos ou do sombreamento não uniforme, os módulos podem estar expostos a diferentes níveis de irradiância, ocasionando o surgimento de mais de um pico de potência. Para extrair a máxima potência de um sistema fotovoltaico, um rastreador do ponto de máxima potência deve ser utilizado. Este dispositivo é constituído de um conversor CC-CC, capaz de alterar as características da carga conectada ao painel. O objetivo deste trabalho é analisar algumas das diferentes técnicas de rastreamento aplicadas em condições de sombreamento parcial. Este tipo de análise é importante, pois caso um máximo local seja rastreado em detrimento ao máximo global, ocorrerá uma perda de potência significativa. Os resultados computacionais comprovam que técnicas convencionais não são adequadas para essas condições e que as técnicas baseadas em inteligência artificial obtém melhores desempenhos.

Telhados verdes têm emergido como uma tecnologia construtiva capaz de proporcionar uma série de benefícios ambientais, sendo usualmente ligados à sustentabilidade do ambiente construído. Embora tenha aumentado o número de pesquisas científicas acerca dessas coberturas, pesquisas voltadas ao estudo específico da camada de drenagem ainda são pontuais. Nesse contexto, neste trabalho, cinco protótipos de telhado verde com diferentes tipos de camada de drenagem foram monitorados durante as estações de inverno e verão (2018/2019) em Curitiba – PR, com um total de 19 eventos de chuva analisados, a fim de avaliar a influência da camada de drenagem quanto sua capacidade de retenção de escoamento pluvial. Os materiais drenantes utilizados na construção dos telhados verdes foram: (TV1) painel modular, (TV2) sem camada de drenagem, (TV3) argila expandida, (TV4) brita graduada e (TV5) tapete drenante. Através dos resultados observa-se que cada tipo de material drenante apresentou um desempenho singular, podendo ser utilizados para otimizar o desempenho dos telhados verdes para determinada região e clima, assim, fazem-se necessárias mais pesquisas que busquem compreender a adaptação e o desempenho de diferentes sistemas em diferentes condições climáticas, além da busca por materiais mais eco eficientes e com menor impacto ambiental.

Abstract:
Neuromuscular diseases as a consequence of brain damage are complex phenomena involving disuse, immobility, brain tissue remodeling and cortical function remapping. They may have various causes and strike any part of the population. The vicious circle leading to a worsening of the patients’ conditions proceeds through muscle shortening by contractures, disruption of the normal reflex behavior and sensory problems, development of spasticity [1]. Physical rehabilitation alone or in association with surgery or pharmacological treatments can be useful in limiting those degenerations. Besides manual rehabilitation, splints and braces are prescribed to control the limb posture and obtain stretching of the muscles. The role of those orthoses is to maintain the paretic limb in a set ‘physiological’ position and let it relax into that posture, in an attempt to reduce muscle rigidity and contractures. However applying a fixed constraint to the limb and waiting for relaxation to take place, may cause discomfort, pain, skin rash, and sundry different complications [2]. Also, any residual voluntary movement is prevented by a fixed-angle splinting. In addition, all these negative characteristics limit tolerability and daily application times. This work presents a different way to promote limb repositioning, based on the application of NiTi-alloy-based dynamic splints, which favor mobility and any residual use of the affected limb. Furthermore it suggests that application of mild contact forces prolonged in time has the advantage of feeling less painful and uncomfortable for the patients, improving overall treatment tolerability.

ABSTRACT Cervical syndrome is one of the most common painful conditions of modern times. It can be defined as a set of disorders caused by degenerative changes to the joints of the cervical spine. The clinical picture of a person affected by the cervical syndrome includes pain, increased tone of the neck muscles, as well as the muscles of the shoulder girdle, reduced range of motion of the cervical spine, etc. This study aimed to determine the effects of exercise programs on cervical syndrome based on a systematic review of previous research. PubMed, Google Scholar, SCIndex, and Medline databases were all searched to compile data on the effects of exercise programs on cervical syndrome. The following keywords were used in the search: cervical syndrome, physical exercise, and neck pain. The inclusion criteria were that the papers were written in English, were available in full text, and were published between 2015 and 2020. Based on the set criteria, 13 studies were included in the final analysis. Based on their analysis, it was determined that various forms of physical exercise programs have numerous positive effects among people affected by the cervical syndrome (reduction of pain, increased range of motion of the cervical spine, increased muscle strength and endurance of neck muscles, relaxation of cervical flexor muscles, reduction of pain in response to pressure, reduction of nocturnal neck pain, etc.). Physical exercise programs lead to strengthening and increased flexibility. By strengthening the musculature, the support that the muscles give to the surrounding joints increases, while the increase in flexibility affects the function of the joints and enables a greater range of motion, which is reduced by cervical syndrome. Due to all the previously mentioned positive effects of physical exercise programs among people affected by the cervical syndrome, physical exercise programs should be included as their first-line treatment.

Review question / Objective: The aim of this study is to perform a meta-analysis to evaluate the effectiveness of electroacupuncture in the treatment of painful diabetic peripheral neuropathy (PDNP).And to provide data support for electroacupuncture as an effective means to treat pain of nervous system diseases. Condition being studied: Diabetes mellitus (DM) affects more than six hundred million population worldwide till 2045. The most common form is chronic, distal, and symmetric sensorimotor polyneuropathy, while other uncommon forms include asymmetric or focal neuropathy, such as diabetic muscle atrophy, trunk radiculopathy, and compression palsy. About 11.4% and 40.5% of patients have severe and moderate pain respectively. Currently, symptomatic treatment of PDPN is based on the application of medications that target the symptoms of PDPN. However, the clinical efficacy of PDPN patients varies greatly from individual to individual.Traditional Chinese medicine electroacupuncture have shown its unique advantages in the treatment of PDPN. Although its mechanism is complex and unclear, it can still be used in the clinical treatment of PDPN for a long time. We therefore present a systematic review of the benefits of electroacupuncture in improving PDPN by including the as many as possible randomized controlled trials.
