Abstract:
Depression and pain are commonly comorbid and often disabling conditions. This chapter reviews current evidence regarding the extent of comorbidity of these two conditions across different settings. The authors briefly summarize popular theories regarding the frequently disputed causal nature of the pain-depression relationship and address the implications of comorbid pain and depression with regard to treatment outcomes and symptom presentation. Additionally, key shared neurobiological substrates consistent with the symptom presentation and the covariation of pain and depression are discussed. The chapter concludes with a summary of a variety of treatment approaches currently employed in the treatment of pain and depression as well as empirical evidence concerning the efficacy of these various approaches.

Abstract:
Persistent pain is an unpleasant sensory and emotional experience that continues for a prolonged period of time and that may or may not be associated with a recognizable disease process. Older people may suffer from conditions such as knee osteoarthritis, low back pain, neck pain and headache, neuropathic pain, fibromyalgia, and cancer-related pain. Pain may impact physical function, psychological function, and other aspects of quality of life. A thorough history and physical examination may optimally assess a person with a persistent pain complaint in the context of a multifactorial pathway from accumulated impairments in multiple systems. Older people may perceive that integrative medicine treatments based on lifestyle and lower-risk therapies may help them get relief from pain and improve quality of life. Follow-up evaluation of positive and negative effects of therapeutic modalities and medications may guide the treatment plan.

To provide safe and effective pain relief, the palliative advanced practice registered nurse (APRN) must possess exceptional pain assessment skills including thorough history-taking and physical examination. Biological, psychological, social, and spiritual factors should be considered part of a complete assessment and serve as a guide for the development of a comprehensive plan of care. APRNs must have comprehensive knowledge of pain management options including appropriate pharmacologic and nonpharmacologic therapies. The chapter includes a discussion of various pain syndromes, physical therapy, interventional techniques, and cognitive-behavioral strategies in addition to medical management of pain. Universal precautions to detect aberrant behavior is described, and safe prescribing practices are outlined.
