Abstract:
Anticholinergic toxidrome can be a side effect of medications or herbal therapies or can result from intentional or inadvertent overdose of prescribed medications or abused substances. The diagnosis of anticholinergic toxicity involves symptomatology, including tachycardia, hyperthermia, central nervous system dysfunction including delirium, and urinary retention. The diagnosis can be challenging, as it is based on clinical symptoms with presentation mimicking other etiologies. Early diagnosis and intervention with treatment are key with the specific identification of the agent involved and other confounding substances. Discontinuation of the agent(s) involved as early as possible and providing supportive care are early interventions, with additional therapeutic options determined by the severity of the toxicity and the agent(s) involved.

Abstract:
The most significant risk of hypoglycemia is damage to the central nervous system. Iatrogenic insulin overdose by healthcare professionals as well as self-administered overdoses are known to occur. Of particular concern to anesthesia care team members is that hypoglycemia under general anesthesia may be difficult to diagnose clinically, because general anesthesia masks the neuroglycopenic symptoms of stupor and coma. In fact, anesthesiologists are the physicians most at risk for administrating an insulin medication error. A structured system of monitoring and administering insulin promotes patient safety. This chapter will review the appropriate assessment and management of patients suffering from insulin overdose, with particular focus on perioperative challenges.

Abstract:
In contrast to general intensive care units, the neurosciences ICU admits fewer cases of incidental overdoses with illicit or prescribed drugs than cases of adverse drug effects. Overdoses of illicit drugs can be seen in conjunction with traumatic brain injury and anoxic-ischemic injury. In fact, the illegal drug is often the main culprit in the acute brain injury. Alcohol intoxication remains frequent and the intoxication signs depressing brain function may merge with clinical signs of an acute structural brain injury. This chapter discusses toxins causing laboratory abnormalities, first treatment considerations, and antidotes, including commonly used drugs for detoxification. When available, antidotes for toxidromes are provided. The chapter also summarizes the neurology of drug overdose.
