This study aims to evaluate the effect of 0.5% (5mg/ml) and 1% (10mg/ml) alendronate sodium (ALN) associated or not with lyophilized bovine bone (Bio-Oss®) on the repair of calvaria bone defects in rabbits. Two bony defects, 10mm diameter circular, were created in 40 male New Zealand white rabbits. The animals were randomized into five (n = 8) groups: GI = Bio-Oss®; GII = Bio-Oss® + ALN 0.5%; GIII = Bio-Oss® + ALN 1%; GIV = ALN 0. 5%; GV = ALN 1%. The left defect was filled with the corresponding substance of each experimental group and the right defect was filled with autogenous blood clot (control). All cavities were lined and covered with a biological membrane. The animals were euthanized 30 and 60 days after craniotomy. The percentage of new bone formation was assessed by microradiographic analysis of calvaria. The images obtained were analyzed using the public domain program Image J®, to assess quantitatively the percentage of bone repair. The values obtained were analyzed using the software SPSS® Statistics, using descriptive statistics (mean, standard deviation and confidence intervals) and inferential ("t" - Student and ANOVA/Tukey). The intragroup results showed that in GI, GII and GIII the percentage of bone repair was significantly higher in the experimental side compared to their respective controls; and was not statistically difference in GIV and GV groups, between experimental and controls sides. The intergroup results showed the following percentage of repair at 30 days: GI = 61.91% (± 10.61), GII = 80.39 (± 8.68), GIII = 81.60% (± 5.42), GIV = 48.45% (± 6.54), GV = 41.45% (± 8.98) and at 60 days: GI = 83.23% (± 14.43), GII = 98.72% (± 2.54), GIII = 92.66% (± 9.22), GIV = 50.18% ( ± 3.03), GV = 37.46% (± 6.26). It is concluded that the addition of alendronate in concentrations of 0.5 and 1%, to lyophilized bovine bone, accelerate the process of bone repair, being more significant in lower concentration. The use of ALN alone delayed the process of bone repair, but without statistical significance.
A presente pesquisa teve como objetivo avaliar o efeito do alendronato de sódio (ALN), nas concentrações de 0,5 (5mg/ml) e 1% (10mg/ml), associadas ou não ao osso bovino liofolizado (Bio-Oss®), na reparação de defeitos ósseos produzidos em calvária de coelhos. Foram utilizados 40 coelhos machos brancos da raça Nova Zelândia. Na calvária de cada animal, foram confeccionados dois defeitos medindo 10 mm de diâmetro. Os animais foram distribuídos aleatoriamente em cinco grupos (n=8 cada): GI = Bio-Oss®; GII = Bio-Oss®+ALN 0,5%; GIII = Bio-Oss®+ALN 1%; GIV = ALN 0,5%; GV = ALN 1%. A cavidade esquerda foi preenchida pela substância correspondente a cada grupo experimental e a cavidade direita preenchida por coágulo sanguíneo autógeno (controle de cada grupo). Todas as cavidades foram forradas e cobertas por membrana biológica. Os animais foram eutanasiados aos 30 e 60 dias após a craniotomia. O percentual de neoformação óssea foi avaliado por meio de análise radiomicrográfica da calvária. As imagens obtidas por radiomicrografia foram analisadas através do programa de domínio público Image J®, para avaliar quantitativamente o percentual de reparo ósseo. Os valores obtidos pelo Image J® foram submetidos à análise com auxílio do software SPSS® Statistics, por meio de estatística descritiva (média, desvio-padrão e intervalos de confiança) e inferencial (teste t Student e ANOVA/ Tukey). Na comparação intragrupos, os resultados demonstraram que nos grupos GI, GII e GIII o percentual de reparo foi significantemente mais elevado no lado experimental em relação aos respectivos controles; nos grupos GIV e GV não foram observadas diferenças estatisticamente significativas no percentual de reparo entre os lados experimental e controle. Na comparação intergrupos, os resultados mostraram os seguintes percentuais de reparo aos 30 dias: GI = 61,91% (± 10,61), GII = 80,39 (± 8,68), GIII = 81,60% (± 5,42), GIV = 48,45% (±± 6,54), GV = 41,45% (± 8,98). Aos 60 dias: GI = 83,23% (± 14,43), GII = 98,72% (± 2,54), GIII = 92,66% (± 9,22), GIV = 50,18% (± 3,03), GV = 37,46% (± 6,26). Conclui-se que a adição do ALN, nas concentrações de 0,5 e 1%, ao osso bovino liofilizado aplicado topicamente nos defeitos ósseos acelerou o processo de reparo ósseo, sendo mais significativo na menor concentração. O uso do ALN isoladamente retardou o processo de reparo ósseo, mas sem significância estatística.

The absence of teeth leads to bone resorption, rehabilitate edentulous individuals through implants can be difficult when this is a severe resorption. The maxillary sinus lift is an alternative to allow the placement of implants in patients with extreme jaw bone resorption, particularly in the posterior maxillary. Objectives: Given the variety of grafts available for maxillary sinus lift, this systematic literature review aims to compare the use of Bio-Oss? with the use of autogenous bone with regard to implants survival rate. Methods: Through MeSH terms, entry terms and Boolean operators, a search for clinical and comparative studies of use of Bio-Oss? and use of autogenous bone for maxillary sinus lifting in PubMed, Embase and Cochrane Library. Results: The search initially found 709 articles that after excluding duplicates resulted in 672 jobs that had the title and summary read. 60 studies were read in full and 08 selected for analysis. The best implant survival rate found was 100% using isolated Bio-Oss?, using autogenous bone alone or a combination of autogenous bone with Bio-Oss?. The lower survival rate found was 82.4% for implants placed in the maxillary sinus filled with autogenous bone. There was no statistically significant difference between groups of the included studies. Conclusion: Still not possible to consider the Bio-Oss? as the gold standard for maxillary sinus floor augmentation, but we can suggest that, as the autologous bone, the biomaterial has excellent implant survival rates.

Objetivos: Diante da variedade de enxertos dispon?veis para levantamento de seio maxilar, esta revis?o sistem?tica de literatura visa comparar o uso de Bio-Oss? com uso de osso aut?geno no que diz respeito a taxa de sobreviv?ncia de implantes. Materiais e m?todos: Atrav?s de MeSH terms, entry terms e operadores booleanos, buscou-se por artigos cl?nico-comparativos de uso de Bio-Oss? e osso aut?geno para levantamento de seio maxilar nas bases PubMed, Embase e Biblioteca Cochrane. Resultados: A busca detectou inicialmente 709 artigos que ap?s exclus?o de duplicatas resultaram em 672 trabalhos que tiveram t?tulo e resumo lidos. 60 estudos foram lidos integralmente e 08 selecionados para an?lise. A maior taxa de sobreviv?ncia de implantes encontrada foi 100% para uso isolado de Bio-Oss?, uso de osso aut?geno isoladamente ou uso de combina??o de osso atu?geno com Bio-Oss?. A menor taxa de sobreviv?ncia encontrada foi 82,4% para implantes instalados em seio maxilar preenchido com osso aut?geno. N?o houve diferen?a estatisticamente significativa entre os grupos dos trabalhos inclu?dos. Conclus?o: Ainda n?o ? poss?vel considerar o Bio-Oss? como padr?o ouro para levantamento de seio maxilar, por?m podemos inferir que assim como o osso aut?geno, o biomaterial apresenta excelentes taxas de sobreviv?ncia de implantes.

Full textObjetivo: Esta revisão tem por objetivo saber como os diferentes enxertos influenciam na recuperação pós ligamentoplastia do ligamento cruzado anterior (LCA), na manutenção do nível de atividade desportiva e score funcional ao longo do tempo. Metodologia: Foi efetuada uma pesquisa computorizada nas bases de dados B-on, EBSCO, Medline, PEdro e Pubmed para identificar estudos clínicos randomizados que, após reconstrução ligamentar do LCA, comparassem os efeitos ao fim de pelo menos dois anos dos enxertos do tendão rotuliano e semitendinoso/gracilis, com publicações entre 2004 e 2014. Resultados: Nesta revisão foram incluídos 5 estudos clínicos randomizados envolvendo 344 indivíduos com a classificação metodológica de 9 em 10 na escala de PEDro, que compararam os diferentes enxertos com programas de reabilitação e instrumentos de avaliação. Conclusão: Constatou-se que a reconstrução anatómica artroscópica do LCA com enxertos de osso-tendão-osso (OTO) em comparação com semitendinoso e gracílis (ST-G) são ambas técnicas seguras e efetivas. Ambos enxertos mostrarem resultados bastantes similares em termos de avaliação subjetiva quantificada com diferentes escalas.
Objective: This review aims to find out how the different grafts influence the recovery after ligamento plasty of anterior cruciate ligament, in maintaining the level of sporting activity and functional score over the time. Methodology: computerized search was performed in the databases B -on, EBSCO, Medline, Pubmed and PEdro to identify randomized clinical trials that made the comparison the effects at least two years after ACL reconstructing of the patellar tendon and semitendinosus / gracilis grafts, , with publications between 2004 and 2014 . Results: this review include 5 randomized clinical trials involving 344 individuals with methodological classification of 9 in 10 in the PEDro scale. that compared the different grafts with rehabilitation programs and instruments evaluation. Conclusion: It was found that arthroscopic anatomic ACL reconstruction with grafts bone-tendon-bone (OTO) compared with semitendinosus / gracilis (ST-G) are both safe and effective techniques. Both grafts show quite similar results in terms of subjective assessment quantified with different scales.

Full textTo the first essential step to forensic identification of human remains (anthropological study of race, sex, age, etc) it is necessary a previous cleaning of the bones, to remove decomposing soft tissues. Medico-legal inconclusive or non identified cases, by using these traditional methods, could be subjected to DNA analysis. However, in spite of advances in human identification techniques, specially by PCR amplified DNA, some limitations that affect the ability to obtain DNA in human remains still persist. Therefore, the aim of this study was to provide additional support from morphological analysis, to help forensic analysts personnel to utilise more efficiently the DNA, extracted from compact bones of human remains in decomposition or already skeletonized corpse, it means without soft tissues, with special emphasis in the legal-medicine practice. Femoral compact bones were obtained from: 7 human remains found on the ground, in different degree of decomposition which were cleaned by boiling to remove soft tissues; also studied were collections of bones from 8 corpses having undergone natural decomposition: 6 human remains exhumed after 3 years from a common public cemetery in São Paulo City; 1 case from amazon region and 1 case with no information, both cases remained from long time (more than 3 years) in contact with soil. All eight cases, were not boiled as no soft tissue were adhered. As a control, five cadavers 12 to 16 hours post mortem were also used. The compact bones histological sections were stained by haematoxilin and eosin and the loci CSF1PO, TPOX, TH01, F13A01,FESFPS, vWA, D16S539, D7S820, D13S317 and amelogenin were amplified by PCR.The procedure for boiling the human remains utilised in the Legal Medicine Institute of São Paulo would have increased the eosinophily of bone matrix and, in some cases, promoted the desaggregation of the osteons. In addition these procedures would have removed the cells, but in some cases would have removed possible inhibitors of the PCR, favouring in this way the analysis of DNA obtained from these samples. The limiting factor to obtain successful analysis in bones submitted to boiling seem to be the low quantity of nuclei present in these samples. For the other hand, in bones not cleaned by boiling, the presence of preserved red cells and oscteocyte nuclei inside the lacunae indicates better preservation of cells in relation to those bones cleaned by boiling. The limiting factor to obtain successful DNA analysis in bones exhumed or in contact of soil, is the suggestive presence of inhibitors of PCR. Porous and brittle bones from human remains, without soft tissues that are not processed by boiling, present alterations through loss of mineralised matrix, although it is still possible to found preserved cells in these samples. The results presented in this work clarify concerns about viability of DNA for identification analysis. They also help to establish better strategies for optimisation of DNA extraction and analysis in compact bones of human remains.

Full textAlcoholism is considered a chronic disease, which brings harm to an individuals health. Bones also suffer serious damage resulting from the chronic consumption of alcohol, since it induces defects in the mineralization and reduction of the thickness of the cortical and the cancellous bone. Xenografts have been a good option for regenerative treatments in response to the high search for reconstructive treatments for bone losses and lesions. Given the demand of patients who make chronic use of alcohol and present bone loss, needing xenografts, the aim of this study was to compare, by means of a histomorphological and histomorphometric analysis, the action of the biomaterials Bio-Oss® and OrthoGen® in the repair process of bone defects in the tibia of rats submitted or not to experimental alcoholism. Eighty male Wistar rats (Rattus norvegicus) aged 60 days were randomly separated into four experimental groups with 20 animals each: Bio-Oss® Water Group (GAB), Bio-Oss® Ethanol Group (GEB), OrthoGen® Water Group (GAO) and OrthoGen® Ethanol Group (GEO). The animals in the GEB and GEO were submitted to gradual adaptation to alcohol and later kept at 25% for 90 days. After this period, all animals were submitted to experimental surgery. A surgical cavity with approximately 3 mm of diameter was made in the proximal tibial epiphysis and in animals from the experimental groups GAB and GEB it was filled with the Bio-Oss® biomaterial. Animals from the experimental groups GAO and GEO had their cavity filled with the OrthoGen® biomaterial. After 10, 20, 40 and 60 days post-surgery, five animals in each group, per period, were euthanized. The histomorphological results showed that the animals from the GAB and GEB groups presented blood cells and connective tissue in the 10 and 20 days periods, and in the periods of 40 and 60 days there was formation of new bone, with the GEB group presenting greater delay in formation when compared to the GAB. In the GAO and GEO groups, bone formation started in the periods of 40 and 60 days, with a delay in formation when compared to the Bio-Oss® biomaterial. In the histomorphometric analysis, comparing the groups that received nonalcoholic liquid diet with different biomaterials (GAB and GAO), there was a significant difference in bone formation in all periods, with the Bio-Oss® Water Group presenting greater bone formation. In the groups that received an alcoholic diet, with different biomaterials (GEB and GEO), there was a difference in bone formation in the periods of 20, 40 and 60 days, in which the Bio-Oss® biomaterial presented greater means. Comparing the groups that received the same biomaterial, but with a different liquid diet (GABXGEB and GAOXGEO), there was a significant difference in bone formation in all periods (except for GAOXGEO; 10 days), in which the groups that received a liquid diet with only water presented better results. In conclusion, the Bio-Oss® biomaterial presented better performance in bone neoformation when compared to the Ortoghen®, and the alcoholic diet affected negatively the repair process.

Full textOs glicocorticóides são farmacos amplamente utilizados em medicina veterinária, seu emprego pode desencadear uma série de efeitos adversos. O ensaio clínico realizado teve como objetivo a avaliação das possíveis alterações na densidade mineral óssea após a terapia com prednisona, utilizando a tomografia computadorizada helicoidal. No estudo, foram utilizados oito cães hígidos sem raça definida, adulto-jovens, sendo 4 machos e 4 fêmeas. A prednisona foi administrada por via oral aos animais na dose diária de 2mg kg-1 de peso durante 30 dias. A densidade mineral óssea foi determinada a partir da obtenção de valores de radiodensidade da região de osso cortical e osso trabecular do corpo vertebral da segunda vértebra lombar, imediatamente após o período da administração do medicamento. O protocolo experimental permitiu a caracterização significativa (P<0,05) diminuição da radiodensidade do osso trabecular do corpo vertebral da segunda vértebra lombar, entretanto não se constatou desmineralização significativa na região cortical. Nenhum dos cães do grupo experimental apresentou fratura patológica ao término da administração do medicamento. Comprovou-se que as alterações no metabolismo ósseo de cães submetidos à terapia com prednisona na dose de 2mg kg-1 ocorrem precocemente apenas em osso trabecular, tornando-se recomendado monitoramento dos pacientes para prevenção de fraturas patológicas.
