Abstract:
GPR120 is implicated as a lipid receptor in the oro-sensory detection of dietary fatty acids. However, the effects of GPR120 activation on dietary fat intake or obesity are not clearly understood. We investigated to determine whether the binding of TUG891, a novel GPR120 agonist, to lingual GPR120 modulates fat preference in mice. We explored the effects of TUG891 on obesity-related hormones and conducted behavioral choice tests on mice to better understand the physiologic relevance of the action of TUG891. In cultured mouse and human taste bud cells (TBCs), TUG891 induced a rapid increase in Ca2+ by acting on GPR120. A long-chain dietary fatty acid, linoleic acid (LA), also recruited Ca2+ via GPR120 in human and mouse TBCs. Both TUG891 and LA induced ERK1/2 phosphorylation and enhanced in vitro release of glucagon-like peptide-1 from cultured human and mouse TBCs. In situ application of TUG891 onto the tongue of anesthetized mice triggered the secretion of pancreatobiliary juice, probably via the tongue-brain-gut axis. Furthermore, lingual application of TUG891 altered circulating concentrations of cholecystokinin and adipokines, associated with decreased circulating LDL, in conscious mice. In behavioral tests, mice exhibited a spontaneous preference for solutions containing either TUG891 or LA instead of a control. However, addition of TUG891 to a solution containing LA significantly curtailed fatty acid preference. Our study demonstrates that TUG891 binds to lingual GPR120 receptors, activates the tongue-brain-gut axis, and modulates fat preference. These findings may support the development of new fat taste analogs that can change the approach to obesity prevention and treatment.

AbstractRecent evidence has raised the possibility of the existence of a sixth taste modality – that is, taste for fat – which is mediated by lingual CD36 and plays a role in obesity. Consequently, the genetic polymorphism of CD36 has been shown to be associated with altered oro-sensory detection of dietary lipids. In the present study, we investigated the relationship between oro-sensory perception of linoleic acid (LA), two CD36 polymorphisms (rs1527483 and rs3212018), obesity parameters and craving habits for dietary lipids in young Czech adults. We also sequenced 5 and 6 exons of CD36 to trace out any new mutation that might be responsible for the difference in taste perception. We observed that craving for dietary lipids was correlated with anthropometric parameters (P<0·05) and LA detection threshold (P=0·033). The participants with the CC genotype of the rs1527483 polymorphism had lower BMI (P=0·011), waist circumference (P=0·005), waist:height ratio (P=0·010) and higher sensitivity for LA (P=0·037) than the participants with the CT and TT genotypes. Interestingly, we did not observe any association between the rs3212018 polymorphism and the studied parameters. Moreover, we did not observe any mutation in exons 5 and 6 of the CD36 gene in these subjects. Finally, we can state that rs1527483, but not rs3212018, is associated with high body weight in young Czech subjects.

Dopamine receptor-blocking antipsychotics, first introduced into clinical practice in 1952, were hailed as a panacea in the treatment of a number of psychiatric disorders. However, within 5 years, this notion was to be shattered by the recognition of both acute and chronic drug-induced movement disorders which can accompany their administration. Tardive syndromes, denoting the delayed onset of movement disorders following administration of dopamine receptor-blocking (and also other) drugs, have diverse manifestations ranging from the classic oro-bucco-lingual dyskinesia, through dystonic craniocervical and trunk posturing, to abnormal breathing patterns. Although tardive syndromes have been an important part of movement disorder clinical practice for over 60 years, their pathophysiologic basis remains poorly understood and the optimal treatment approach remains unclear. This review summarises the current knowledge relating to these syndromes and provides clinicians with pragmatic, clinically focused guidance to their management.

Abstract:
Summary Ameloblastoma, a semi-benign odontogenic tumor, is divided into four subtypes based on the clinical presentation, radiological manifestations and histological characteristics. Radiologically, it manifests itself in the form of uni- or multicystic intrabony expansive lesions, with destruction of the medullary zone of the existing bone, resorption and possible perforation of the buccal and lingual cortex. The patient, a 62-year-old woman, came to the Clinic for Maxillofacial Surgery in Niš with an anamnestically present painless tumefaction in the anterior segment of the mandible present for the past three years. In the course of an intraoral examination in the region of symphysis and parasymphysis of the lower jaw on the left side, an oval-shaped tumor was spotted, 5x3 cm in size; palpation showed that it was hard, painless and motionless. Multislice computed tomography (MSCT) of the oro - and hypopharynx and neck showed an extensive tumor formation of the soft tissue density in the anterior segments of the lower jaw, with the signs of destruction of bone tissue of the buccal and lingual cortex of the lower jaw. The frequency of recurrence depends on the histological type of ameloblastoma and the degree of radicalism of the performed surgical intervention. Ameloblastoma in most cases is definitely diagnosed in the advanced stage of the disease due to the absence of symptoms and very slow tumor progression.

Pregnancy induces general changes that increase the susceptibility to diseases of the stomatognathic system, generating complications that can endanger the life of the mother and fetus. Purpose: Clinical-statistical evaluation of diseases in the oro-maxillo-facial region found in pregnant women, in order to increase the efficiency of diagnosis and surgical treatment. Data and methods: Have been studied 86 pregnant patients aged 17-40 years (mean 28.8 ± 2). Group 1, prospective study, ambulatory within the dental clinic “Omni Dent”, duration 1-year, clinical examination of 27 patients. Group 2, retrospective study, in the hospital, OMF surgery department of IMSP IMU, duration 6 years, 59 patients. Results: Group 1: consulted patients in trimesters: 1st — 5 (19.23%), 2nd — 11 (42.31% ), 3rd — 10 (38.46%). Diagnosis: pregnancy gingivitis — 8 (29.63%), caries — 7 (25.93%), pericoronitis — 4 (14.81%), root rot — 3 (11.11%). Group 2: treatment in the trimesters: 1st — 17 (28.81%), 2nd — 22 (37.29%) and 3rd — 20 (33.9%). The cause of infection, lower molars — 34 (82.92%), upper molars — 4 (9.76%), lower premolars — 3 (7.32%). Diagnosis: acute lymphadenitis — 7 (11.86%), pericoronitis — 17 (28.81%), and odontogenic abscess — 22 (37.29%) in the retromolar spaces — 12 (29%), subperiosteal — 9 (22%), the mandibulo-lingual groove- 7 (17%), submandibular5 (12%). Conclusion: Surgical treatment of dental diseases and their complications can be avoided by addressing patients in time.

Background: Hamartomas are developmental tumor-like malformations, comprising a mixture of cytologically mature tissues that are indigenous to their anatomical location. Within the oral cavity, these lesions are usually multifocal, relatively small, and asymptomatic, and have been frequently linked to present as a manifestation of the oro-facial-digital-syndrome. Case Presentation: A 2-day-old female newborn was referred with two masses protruding from the oral cavity, feeding difficulties, and dribbling of milk during feeding. The patient had no dysmorphic features and no obvious associated anomalies. On careful examination, there was one mass protruding from the undersurface of a bifid tongue with another larger mass that seemed to originate from the hard palate. Complete surgical excision of both masses and repair of the bifid tongue were done to relieve the baby’s feeding difficulties. Histopathological examination of both specimens confirmed the diagnosis of multiple hamartomas. Conclusion: The occurrence of multiple and large oral hamartomas in newborns might result in feeding difficulties that necessitate urgent surgical intervention. The association of lingual and palatal hamartomas, with bifid tongue and cleft palate, is a unique observation that is not reported in the English literature.

Praxia represents the totality of the gestures and movements necessary to perform complex voluntary actions in order to accomplish a goal. Praxia has two aspects: on the one hand, intentional activity and on the other hand a mnestic activity based on memorizing the succession over time and space of the various movements to be performed in order to accomplish the intentional action. Programming the fulfillment of a voluntary action in order to achieve a well-defined purpose proceeds gradually, at different successive levels. The first level is the conceptual one. The person aims to achieve a certain purpose, sets the purpose of the voluntary motor action. The second level is that of the kinetic formulation of action, gesture. The individual disintegrates from the mnesic stock of all kinetic formulas he has learned during his ontogenetic development those formulas that are most appropriate to achieve the proposed action. The third level includes central and peripheral motor innervation, pyramidal, extrapyramidal, cerebellar and medullary motoring ways, which are the peripheral performers of the voluntary action. At this level the harmonious interactions between the different muscles involved in the movements are established, as well as the harmonious innervation of the synergic muscles, fixators, agonists and antagonists. The study comprises 5 patients admitted in the clinic of psychiatry in Constan�a, who presented oro-facial-lingual apraxia, apraxic dysphasia and aphasia, appeared in various evolutionary phases of intracranial neoformative processes. Apraxia was due to the increase of the kinetic engraving threshold, to the laughter they are sequenced by the logic of gesture efficiency; the often repeated, deeply fixed, kinetic engraves are automated in time.The co-ordination needed to perform a gestional motor task in order to accomplish a determined action has significant psychological implications.

O condiloma acuminado é uma doença sexualmente transmissível causada pelo Papilomavírus humano (HPV). Freqüente na região anogenital e rara na cavidade oral. No entanto, o aumento da atividade oro-sexual vem cada vez mais favorecendo a instalação da infecção do HPV na mucosa oral. O presente relato descreve um condiloma na cavidade oral em um paciente do sexo masculino, de 35 anos, com história de prática de sexo oral, cujo diagnóstico foi confirmado pela biópsia. O tratamento instituído foi efetuado com o uso de ácido tricloacético a 80%, ocorrendo remoção completa da lesão. Foi feito uma revisão da literatura acerca da incidência, quadro clínico, diagnóstico, tratamento e profilaxia.

Introduction : L’objet de cet article est d’évaluer les effets d’une gouttière d’éducation fonctionnelle souple dans le traitement d’interception des dysmorphoses de classe II division 1. Materiels et Methodes : Une étude EGIDYS observationnelle prospective, multicentrique et non comparative a été menée chez 81 enfants de classe II division 1, présentant un ou plusieurs troubles dysfonctionnels. Les patients ont porté une gouttière souple de la gamme EF Line® à raison de deux heures minimum par jour et toute la nuit pendant douze mois, associée à des exercices d’éducation en fonction des cas. Ils ont été suivis après un mois, quatre mois et douze mois de traitement. La ventilation, la position linguale, la tension des lèvres et de la musculature péri-orale, la forme des arcades et les measures céphalométriques dento-squelettiques ont été évaluées au début et après un an d’éducation fonctionnelle. Resultats : À l’inclusion, l’examen fonctionnel montre que la majorité des enfants présentent une position de langue basse au repos, une ventilation buccale ou mixte, un tonus musculaire labial et péri-oral anormal et un maxillaire souvent étroit. L’examen céphalométrique initial enregistre les parameters suivants : surplomb, recouvrement incisif, inclinaison des incisives inférieures par rapport à la ligne A-Pog, angles SNA, SNB et ANB. Après douze mois d’éducation fonctionnelle avec une gouttière souple EF Line®, on observe une amelioration des fonctions de la sphère oro-faciale, objectivée par une correction de la position de la langue au repos, ainsi que de l’indice lingual en faveur d’une position de la langue contre le palais, une réduction de la tension de la lèvre inférieure au repos et en fonction, ainsi que des muscles buccinateurs, une augmentation de la capacité à respirer par le nez et un changement de forme du maxillaire en faveur d’une forme arrondie. On observe également une réduction importante du surplomb et du recouvrement incisif, ainsi qu’une baisse significative de l’angle ANB due à sa composante SNB qui est augmentée. Cette première étape du plan de traitement orthodontique aboutit à 73 % d’amélioration des dysmorphoses de classe II division 1. La correction de la malocclusion et des troubles fonctionnels est corrélée de manière significative à une bonne observance du port de la gouttière EF Line®. Conclusions : Cette étude a permis de montrer que les éducateurs fonctionnels souples EF Line®, de par leur conception, corrigent efficacement les troubles fonctionnels liés à une mauvaise position linguale et que cela entraîne des modifications dento-squelettiques positives par rapport à l’absence de traitement. D’un point de vue clinique, les gouttières souples EF Line® ont un intérêt dans le prétraitement orthopédique et orthodontique des dysmorphoses de classe II division 1.

ABSTRACT The interconnection between the lingual nerve (LN) and the hypoglossal nerve, the LN and the inferior alveolar nerve (IAN), and the LN and the mylohyoid nerve (MHN) has already been documented in the literature. Despite the fact that variations in the course of the MHN in regard to the mandible are regularly observed, they have yet to be well documented in the anatomical or surgical literature. This anatomical variety necessitates that surgeons and anesthesiologists who routinely perform oro-surgical interventions and nerve blocks in the face for various neuralgias enhance their knowledge and awareness in order to avoid unintended nerve injury. Abstract:
A key functional effect of intranasal administration of the neuropeptide oxytocin is on top-down control of social attention. However, an oro-mucosal administration route may be better tolerated for chronic therapeutic use and evidence suggests that some functional effects of oxytocin can be mediated via peripheral routes. The current study investigated if oxytocin administered orally using medicated lollipops can both increase blood oxytocin concentrations and influence social attention and state anxiety. In a randomized, double-blind, clinical trial adult male participants received oral oxytocin (24IU) or placebo 30-min before completing a well-established anti-saccade paradigm which can assess treatment effects on both top-down and bottom-up attention. Oxytocin administration modulated top-down social attentional processing by increasing anti-saccade error rates on both social and non-social stimuli although it only increased response latencies for social cues. Anti-saccade errors were also positively associated with the proportionate increase in plasma oxytocin concentrations. A comparison analysis showed that oral oxytocin administration increased blood concentrations to a similar degree as given by lingual spray, although less than when given intranasally. Importantly, attentional and anxiolytic effects of oxytocin in the anti-saccade task were similar across intranasal, lingual, and oral administration routes. These findings demonstrate that oral administration of oxytocin, similar to via intranasal and lingual routes, can modulate top-down social attention and state anxiety and support its potential for therapeutic use. Abstract:
Metformin is the most frequently prescribed first line therapy for Type 2 Diabetes Mellitus (T2DM) and it is one of the fewer antihyperglycaemics associated with improvements in the morbidity and mortality of cardiovascular disease associated with T2DM. Although there are major beneficial effects but it is shown to have disadvantages with long-term use of metformin. Recent studies have shown that metformin induces malabsorption of Vitamin B12, which may increase the risk of developing Vitamin B 12 deficiency. Vitamin B12 is one of the integral nutritional components that affect the oral health with individuals with decreased levels exhibit various oral manifestations such as glossitis, glossodynia, recurrent ulcers, angular cheilitis, dysgeusia, lingual paraesthesia, burning sensations and pruritis. Most of the vitamin B12 deficiencies are associated with malabsorption syndrome, gastrectomy cases, and elderly people. The prevalence of oral manifestations with regard to metformin induced Vitamin B12 has to be considered as new paradigm in routine diagnosis and investigations. This review likewise centers around the mechanism involved in metformin induced vitamin B12 deficiency and possible implications in the diagnosis and management of oro-mucosal lesions associated with such deficiency.

Introdução: A comunicação bucoantral tem como principais causas etiológicas os processos patológicos, traumatismos e cirurgias. Sendo comum sua ocorrência durante exodontias de elementos dentários superiores posteriores devido ao íntimo contato dos ápices radiculares com o assoalho do seio maxilar. Objetivo: Realizar um relato de caso de um paciente com comunicação bucoantral tratado com o uso da técnica da bola adiposa de Bichat. Caso Clínico: Paciente do sexo feminino, com 51 anos de idade, apresentando comunicação bucoantral há 6 anos após exodontia do elemento dental 16. Realizado fechamento da comunicação bucoantral sob anestesia local com tracionamento e sutura da bola de Bichat sobre a falha óssea. Após 01 ano de proservação, observamos a ausência de comunicação bucoantral, bem como da sintomatologia, resultando no sucesso do tratamento. Conclusão: Conclui-se que a utilização do retalho pediculado do corpo adiposo bucal é uma alternativa útil para o fechamento e tratamento de comunicações buco sinusais, sendo que o deslizamento da bola de Bichat é um método seguro para fechamento de tais comunicações e que esta é uma técnica de simples execução, com poucas complicações e limitações.Descritores: Seio Maxilar; Sinusite Maxilar; Fistula Bucoantral.ReferênciasZartida AIG, Lugo RR. Abstract:
El éxito del tratamiento endodóntico va a depender de la limpieza y la conformación del sistema de conductos radiculares. En la práctica clínica el uso de irrigantes provee una desinfección química de los conductos radiculares y remoción del barrillo atrapado en el interior del conducto gracias a la acción eficiente de las soluciones quelantes. El propósito de este estudio fue calcular la eficacia de la eliminación del barrillo dentinario en el tercio apical, con la utilización de EDTA al 17% y Ácido cítrico al 10% activando con diferentes dispositivos y técnicas como son la irrigación ultrasónica pasiva (PUI) y las puntas EDDY (ED). Se utilizaron 45 dientes premolares divididas aleatoriamente en cinco grupos: 4 grupos experimentales (n=10) y 1 grupo control (n=5). En cada raíz se preparó dos surcos longitudinales en vestibular y lingual, las cuales fueron recubiertas con oro para su observación bajo el microscopio electrónico de barrido para de este modo cuantificar los tubulos dentinarios abierto. Para el análisis estadístico se usó la prueba de Shapiro-Wilk, Kruskal-Wallis y una comparación multiple con U Mann-Whitney las cuales demostraron que la activación con EDDY ha tenido los puntajes más bajos del experimento con una diferencia de 8 puntos con respecto de PUI con EDTA al 17% y una diferencia con PUI de 13 puntos con Acido cítrico al 10% en el tercio apical. No obstante, ninguno de los grupos experimentales fue capaz de remover todo el barrillo dentinario en el tercio apical, pero si presentaron diferencias significativas con respecto al grupo control, por lo que se recomienda el uso de soluciones quelantes y sistema de activación para poder obtener mejor índice de limpieza y asegurar un éxito tratamiento de conducto.

Abstract:
Introduction: Oro-lingual angioedema has been reported to occur in 1 to 5% of AIS patients receiving rt-PA therapy, in small samples with prospective screening. We sought to describe the prevalence and predictors of angioedema within a large administrative database. Methods: This analysis utilized the Premier database, a representative sampling of US hospitals that includes approximately 15% of hospital discharges, with access to pharmacy records. Study period was fiscal yrs 2006-2010. Patients with primary admitting diagnosis of AIS were included (ICD 9 codes 433.x1, 434.x1, and 436), AIS patients receiving thrombolytics were identified using ICD-9 code 99.10 or pharmacy use (Alteplase, 50 or 100mg vials). Patients with angioedema were identified by code 995.1. Associations between patient demographics and angioedema were assessed using logistic regression. Results: A total of 257,992 ischemic stroke patients were within the database. These patients were 47 % male, 16% black, and mean age of 71 (SD 14). There were 8792 (3.4%) IS patients treated with rt-PA, these patients were more likely to be younger (mean age 69 (SD 15) vs 71 (14)), white (66% vs 63%) compared to those not treated, p-values <0.01. There were 129 (0.05%) IS patients with angioedema (0.33% in rt-PA treated, vs. .04% in not-treated, p value <0.01) . Multivariable model of predictors of angioedema among those patients treated with rt-PA are presented below. Discussion: Within a large administrative database, we found a much smaller percentage of angioedema reported among rt-PA treated patients than has been previously reported, likely in part due to under-documentation of milder angioedema events. We also found that rt-PA treated women had much higher risk of angioedema than men, which has not been reported previously. Unfortunately, admission medications are not available within this database, so concomitant angiotensin-converting enzyme medication use (a known risk factor for angioedema) is unknown.
