Abstract:
This paper is presenting the literary materials about the necessity and importance of basic research in the field of medicine, particularly ophthalmology. These data revealed the analysis of well-known studies conducted by our famous scientists from different branches of medicine to explore different aspects of the functioning of the retina, to investigate the mechanisms of formation of the visual image, to define the role of individual cells in the function of color vision with different methods, to reveal the effects of ionizing radiation on the lens after Chernobyl disaster. This article contains the information about the results of learning the peculiarities of physiological and pathological processes in retina cells on the intracellular and membrane levels using the unique experimental techniques. The history aspects of some basic research in medicine and ophthalmology are revealed too. On these data the authors demonstrate the evidence of high information content and the importance of interdisciplinary research in the resolving of complex medical problems for the future of ophthalmology with involving the more informative methods of different subjects of medicine.

Abstract:
Background:Maintenance of regular physical activity is associated with better physical and mental health (1). In addition, sleep disturbances and mood disorders are common in chronic inflammatory diseases (2).Objectives:The aim of this study was to determine the relationship between physical activity, sleep quality, anxiety and depression in patients with FMF.Methods:A total of 56 patients (30 female, 26 male) with Familial Mediterranean Fever (FMF) were enrolled in the study. They were diagnosed with FMF based on the Livneh diagnostic criteria (3). International Physical Activity Questionnaire - Short Form (IPAQ) was used to evaluate health-related physical activity. Patient-reported sleep quality was evaluated with the Pittsburgh Sleep Quality Index (PSQI). Hospital Anxiety and Depression Scale (HADS) was used to evaluate anxiety and depression in FMF patients. Patients who were 20-55 years old was included in the study.Results:The mean age was 32.23±9.87 years. The mean disease duration of the patients was 12.24±7.61 years. The education level of 51.8% (n=29) of the participants was undergraduate and above. The rate of those who did not work was 32.1% (n=18) and 48.2% (n=27) of the participants were married. The mean of scores of IPAQ was 2333.99±2640.29, the mean of PSQI was 6.14±3.66, the mean of HADS-Anxiety was 7.89±5.03 and HADS-Depression was 7.01±4.44. The patients were classified according to physical activity categories as low (25.0%, n=14), moderate (41,1% n=23) and high (33.9%, n=19). Physical activity categories were not found associated with PSQI (p=0.437), HADS-Anxiety (p=0.363) and HADS-Depression (p=0.861). The relationships of scores of IPAQ, PSQI and HADS were demonstrated Table 1.Conclusion:This study confirmed that patients with FMF suffer from sleep disturbances, anxiety and depression. Abstract:
Background:Psoriasis and psoriatic arthritis (PsA) are heterogenous diseases with various disease manifestations and phenotypes. Psoriasis has a bimodal age of onset being early (before the age of 40, type 1) and late. The impact of this classification on the PsA features is not well understood.Objectives:To compare the PsA characteristics of patients with early- and late-onset psoriasis in a large, multicenter databaseMethods:PSART-ID (Psoriatic Arthritis-International Database) is a prospective, multicenter web-based registry (www.trials-network.org) of patients with PsA. A detailed data collection was performed including demographics (sex, age, duration of education, smoking status, BMI), skin features (psoriasis onset date, type, initially involved site of skin, nail involvement (ever) and family history) and PsA characteristics (type of articular involvement and presence of axial, dactylitis (ever), enthesitis (ever), family history) and indices for disease activity and function (DAPSA, Leeds enthesitis index, BASDAI, BASFI, patient and physician global assessment, pain, HAQ-DI). We grouped according to the age at psoriasis onset (early onset, psoriasis before the age of 40 (EOPsO); late-onset, psoriasis after the age of 40 (LOPsO)), patient and disease characteristics of the groups were compared (1). Due to the differences among groups, following adjustments weer made: BMI for age, nail involvement for PsO disease duration, axial PsA for PsA disease duration.Results:A total of 1634 (62.8% females; EOPsO, 1108 (67.8%); LOPsO, 526 (32.2%)) patients with PsA was recruited. Rate of over-weight patients was higher in LOPsO group (66.8% vs. 86.8%, p<0.001; adjusted for age - aOR 1.55 (1.11-2.20; % 95 CI)). The EOPsO group had the scalp involvement as the initial site of skin disease more often than the LOPsO group (56.7% vs. 43.0%, p<0.001), whereas extremity involvement was more frequent as the initial finding in the LOPsO group (EOPsO vs. LOPsO 63.8% vs. 74.2%, p<0.001). Nail involvement (ever) was more prominent in EOPsO group, however, the significance was disappeared when adjusted for psoriasis duration. Interaction between gender and both axial disease and psoriatic disease family history were found (axial disease in man; EOPsO vs LOPsO; 38.0% vs. 25.4%; p=0.005; adjusted for PsA duration - aOR 0.56 (0.38-0.84; %95 CI) // psoriatic disease family history in females; EOPsO vs LOPsO; 39.5% vs. 30.1%; p=0.003; OR 0.65 (0.50-0.86; %95 CI)). Duration between PsO and PsA was significantly longer in EOPsO group (148 vs. 24 months, p<0.001). In EOPsO group, more patients had PsO preceeding PsA than LOPsO group (81.8% vs. 60.6%, p<0.001), however, synchronous disease -defined as the diagnosis of PsO and PsA within the same year- was more common in LOPsO group (16.6% vs. 30.3%, p<0.001) (Table 1). Abstract:
Lymphangioleiomyiomatosis (LAM) is a rare progressive cystic lung disease that affects almost exclusively women. LAM can occur sporadically, or can be associated with tuberous sclerosis complex (TSC); a rare disorder with multiorgan involvement effecting the brain, kidneys, heart, liver, skin and eyes and is associated with intellectual disability, epilepsy and autism spectrum disorders. Dr.Terraneo PhD project was developed with the aim to expand clinical knowledge about diagnosis and follow up as well as to analyze pathogenic aspect of the development of the disease. As a first step of the PhD project, the association between LAM and other features of TSC (e.g. demography, extrapulmonary manifestations, genetic mutations..) was investigated as well as the role of pulmonary function tests (PFTs) for LAM diagnosis. Our results demonstrate that age, but not PFTs, is independently associated with LAM development in patients with TSC. PFTs, even if indicated to assess impairment in lung function, result feasible in a limited number of patients due to cognitive impairment, and are not significantly useful for LAM diagnosis in women with TSC. Successively, the case of a patients with coexistence of three rare diseases (autoimmune hepatitis/primary biliary cirrhosis overlap syndrome, lymphangioleiomyomatosis/tuberous sclerosis complex (LAM-TSC), and sarcoidosis) was described. We speculated that the dysregulation of the pathway involving mTOR and MAPK and their interaction might play a role in the pathogenesis of diseases other than TSC, including sarcoidosis. In the last part of PhD project, the serum levels of VEGF-D, VEGF-C, MMP-2 and MMP-7 were assessed in a cohort of patients affected with S-LAM and TSC with and without LAM. Our results showed that VEGF-D, MMP-2 and MMP7 were higher in patients with LAM than in patients without. VEGF-D was confirmed as the biomarkers with the highest accuracy for LAM diagnosis. MMP-2 and MMP-7 could be a promising biomarker of LAM.
