Abstract:
ObjectiveOrgan transportation requires innovation. We recently showed that unmanned aircraft systems (UAS) could transport human organs. There are no data addressing UAS acceptance among healthcare providers. Conceptually, UAS implementation may improve delivery of care through improved efficiency. We sought to learn surgical perspectives on current and innovated organ transport systems.MethodsAn Institutional Review Board exempt, pretested, 5-point Likert scale web-based survey was undertaken. Transplant surgeons taking kidney transplant offers in the USA (n=174) were sampled.ResultsOf 174 surveys, 122 were delivered successfully, and 55 responses collected. Mean age was 48.1 (range 34–64), and 80% were male. Forty-two (76.4%) surgeons felt cold ischaemia time reduction to 8 hours would increase organ acceptance rates. More than 23% of respondents were fearful and 34.5% nervous regarding drones. Nearly all (92.7%) respondents believed drones could help people; 90.9% felt the mode of transportation was irrelevant to their decision to accept an organ but that speed and quality were most important. Only 16.4% of surgeons believed the current system is adequate for our transportation needs.ConclusionsSurgeons feel the present system of organ transportation needs reform, and an innovated system using UAS might improve care. An innovated organ transportation system involving UAS may lead to fear and anxiety among transplant surgeons, suggesting that research and education are required ahead of adoption.

Abstract:
Objective. Severe acute pancreatitis (SAP) patients usually develop persistent organ dysfunction which causes the majority of deaths. It is important for SAP patients to receive centralized diagnosis and treatment in an experienced tertiary center. China, as a vast country with uneven distribution of medical resources, should take advantage of air medical transportation to meet the challenge of patient transfer among different regions. The aim of this study was to evaluate the safety and effectiveness of air transport for SAP patients via extra long distance. Methods. This was a retrospective analysis of all air medical transportations for SAP patients admitted to Jinling Hospital from January 2010 to December 2016. The general characteristics, transportation process, and clinical outcomes of these patients were recorded, and the safety and effectiveness of air transport were evaluated. Results. All the 20 SAP patients were successfully transferred by chartered aircraft without any occurrence of severe transport-associated complications. The mean transport time was 5.86 hours and the average transport distance was 1530 kilometers. The majority of SAP patients got timely intervention and the ultimate mortality rate was 15%. Conclusions. Air medical transport appears to be safe and effective for SAP patients with vital organ dysfunctions during the extra long-distance transportation.

Abstract:
Introduction: All living things need a certain amount of oxygen which is obtained from free air, which is continuous throughout life, but cannot be stored in the body as a reserve. Oxygen is distributed throughout the body to the mitochondria of cells which are used in metabolic processes along with glucose to produce ATP (energy source for cell activity) and remove carbon dioxide (CO2). Literature Review: The exchange of oxygen as a metabolic material for the body and carbon dioxide as the end product of the body’s metabolism is done through a process called respiration. Blood pumped by the heart carries oxygen from the lungs to all body tissues and brings back the blood containing carbon dioxide from the tissues back to the lungs for gas exchange. Oxygen transport in the blood takes two forms, namely bound to hemoglobin/Hb (the largest) and dissolved. Meanwhile, the transportation of carbon dioxide in the blood takes 3 forms, namely: carbonic ion (the largest), dissolved, and binds to Hb. Conclusion: The consumption of oxygen in the body requires processes and is associated with several organ systems. If one of the systems is not functioning properly, it can cause oxygen deficiency, thus the cells do not consume enough oxygen, which can cause anaerobic metabolism and if it becomes severe it will cause the death of cells, organs, and the individual.

Relevance: improvement of the existing device, thanks to development of the electric scheme of control of temperature that will eliminate possible errors during transportation. The aim of the thesis is to model the electrical circuit of temperature control and modeling of the fluid circulation system in the cooling system, which will keep the heart in a warm perfusion state during transportation. Thesis tasks: 1. Analysis of the literature on the existing methods of transportation of the donor heart. 2. Analysis of modern devices. 3. Modeling of the case in the SolidWorks environment. 4. Abstract:
This study sought to expand current narrative persuasion models by examining the role of subtext processing. The extended elaboration likelihood model suggests that transportation leads to persuasion by reducing counterarguments to stories’ persuasive subtexts. The model implicitly argues that transportation should reduce total subtext processing, including counterarguments and intended elaboration. But this study reasoned that people with stronger eudaimonic motivation to have meaningful entertainment experiences, would put more effort into processing stories’ subtexts while engaging with the narrative. Because less eudaimonically motivated individuals may be at risk for missing the subtext, it was also expected that adding a supplemental conclusion scene that reiterates the intended message would facilitate persuasion.Following a pre-test survey, 201 undergraduate students were randomly assigned to view an episode of the crime drama Numb3rs: one of two versions of “Harvest,” designed to promote organ donation (with or without a conclusion scene), or a control episode. After viewing, participants completed a thought-listing task and second survey. Results show that “Harvest” did not result in persuasive outcomes related to organ donation. Transportation was a marginally significant positive predictor of total subtext processing. Contrary to predictions, eudaimonic motivation negatively predicted amount of total subtext processing.Eudaimonic motivation also negatively (but marginally) predicted doctor mistrust, but this effect was moderated by conclusion condition: eudaimonic motivation was negatively associated with doctor mistrust only in the no conclusion condition. Eudaimonic motivation was also negatively (but marginally) associated with intended elaboration. Further examination showed that, compared to people with low eudaimonic motivation, those with high eudaimonic motivation were less likely to engage in intended elaboration, but only in the no conclusion condition. This pattern of findings provides indirect evidence that intended elaboration was responsible for decreasing doctor mistrust among people with high eudaimonic motivation who saw the conclusion. But surprisingly, intended elaboration was not directly related to any persuasive outcomes.The findings tentatively suggest that transportation and subtext processing can coexist and that eudaimonic motivation can affect the extent to which viewers engage in subtext processing during narrative engagement. The results also indicate that supplemental conclusions may be useful tools for narrative persuasion.

Extracorporeal membrane oxygenation (ECMO) is generally used in patients with severe respiratory and/or cardiac failure or in patients waiting for solid organ transplantation. ECMO is performed by specialized centers, but patients in need of ECMO are frequently taken care of at other centers. Thus, many ECMO centers have developed transport programs with a mobile unit. Actually, more than 1400 transports on ECMO have been reported. The reported survival of patients transferred with ECMO is equivalent to that reported for patients not transferred with ECMO. This chapter summarizes the story of the ECMO transport, its development and improvement over time and provides suggestions to new centers willing to start this activity. It is imperative, that the staff performing the ECMO transport must be properly trained and experienced not only in ECMO cannulation and management, but also in intensive care transport and air transport medicine.

Polymeric nanoparticles have been increasingly studied and applied in a variety of areas, most commonly in biomedicine. The efficiency and toxicity are two aspects that need to be considered for nanoparticles, and both are closely related to the pharmacokinetics of nanoparticles. In this chapter, the pharmacokinetics of polymeric nanoparticles were introduced at the whole body level (including absorption, distribution, metabolism, and excretion), organism level (transportation within organs and pass through physiological barriers), cell levels (binding to cell surface, endocytosis, intracellular transition, and exocytosis), and molecule level (protein binding and ligand-receptor binding). Examples were also given to illustrate the modeling of the pharmacokinetics of polymeric nanoparticles at different levels. A comprehensive understanding of the pharmacokinetics of polymeric nanoparticles will facilitate the applications in various areas such as drug delivery and disease diagnosis.

Abstract:
The concept of a system has a long and distinguished history in the social sciences. In fact, Mattelart and Mattelart (1998) claim that “the idea of society as an organism, that is, a whole composed of organs performing predetermined functions, inspired the earliest conceptions of a ‘science of communication’”. We begin this chapter with a brief historical overview of the major systems perspectives that have been utilized in social theory and research: structural-functionalism, cybernetics, and general systems theory. We then apply recent developments in complex systems theories to organizational networks. In doing so, we look at communication and knowledge networks from the perspective of agent-based modeling and self-organizing systems. Mattelart and Mattelart (1998) trace the early growth of systems thinking in the social sciences. Adam Smith’s (1776) classic work, The Wealth of Nations, postulated that a laissez-faire system, the division of labor, and channels of communication and transportation were crucial aspects of economic prosperity. The key to economic and therefore social success was the unrestricted circulation of messages, materials, and money through secure networks. According to Mattelart and Mattelart (1998), Francois Quesnay, a French physician and economist, published an economic chart (tableau economique) in 1758. “The chart offers a macroscopic vision of an economy of ‘flows’ in the form of a geometrical zigzag figure in which the lines expressing exchange between human beings and the land, as well as among the three classes making up society, cut across each other and become intertwined”. The Mattelarts note that Claude Henri de Saint Simon’s eighteenth-century work also applied systems theory to the concept of networks. Saint-Simons’s theory conceived of society as “an organic system, a bundle or fabric of networks.... He attributed strategic importance to the development of a system of communication routes”. Out of this background Herbert Spencer (1820–1903) developed the first integrated theory of society built on a direct analogy with biological systems.

Abstract:
Despite great progress in tissue engineering, there still exist limitations in engineering and manufacturing thick tissues. The engineered construct is still trapped in a geometrically simple and thin structure due to an inefficient transportation system. In traditional scaffolding, the media exchange rate mainly depends on media diffusion. Even a porous scaffold cannot provide enough media for the high metabolic rate of thick tissue. Embedding microfluidic channels has great potential to increase media exchange capability. The existing microfluidic channel fabrication methods are limits as well. Microfluidic channels with interconnectivity, 3D dimension, or complex geometry have not been achieved. To address these issues, a novel printable microfluidic channel fabrication method is introduced in this paper. This research investigates the manufacturability of novel cellular micro-fluidic channels. The proposed micro-fluidic channels can directly construct a scaffold that will provide both support of mechanical integrity and fluid transport in 3D. Abstract:
This paper presents the three-dimensional (3D) modeling and simulation of heat transfer within a portable blood cooling system with respect to space and time. The cooling system under study includes a cooler box with two cooling cylinders and eight blood bags. The cooler has the shape of a rectangular box with walls made of thermal-insulation material. The cooling cylinders protrude into the inner space through the bottom of the cooler box. Eight blood bags are arranged around these two cooling cylinders. The cooling cylinders use desiccant absorbing agents in combination with evaporative processes under vacuum pressure to achieve the cooling effect. Transient thermal response of the system is simulated by employing a computational approach. The computational domain includes the air inside the cooler box and the blood inside the blood bags. Heat from the surroundings penetrating the system through the insulation walls of the cooler box is included in the model. It is assumed that initially the blood is at the uniform ambient temperature. Once the cooling cylinders are activated, they draw heat from the air and the blood inside the cooler box. A lumped-parameter model is used to model the heat drawn by the cooling cylinder. The resulting solution of temperature distribution within the blood bags and the air in the cooler box is presented in the form of graphical visualization. Representative temperatures within the blood bags are presented as functions of time during a period of 12 hours. The computational model and results will be useful for the development and optimization of the design of portable cooling devices for the transportation of blood and similar biological tissues such as an organ for transplant.

Abstract:
This paper investigates the combined effects of specific impact direction and impact location on the serious-to-maximum (AIS3–6) thoracic injuries of drivers in frontal impacts based on the 1995–2009 data from the United States Department of Transportation (US DOT) National Automotive Sampling System/Crashworthiness Data System (NASS/CDS). The selected sample is limited to three impact locations near the driver side (distributed, offset and corner) and two impact directions (pure frontal and oblique) treated as the frontal direction, resulting in a total of six crash configurations. The risks of thoracic injury for drivers in all frontal crash configurations are evaluated. The relative risks with 95% confidence intervals are calculated. Binary logistic regressions are fitted to the datasets for further examination of the effects of impact direction and impact location on the serious-to-maximum thoracic injuries. Occupant characteristics and crash severity are also included as explanatory variables. Overall, impact location and impact direction have considerable influences on thoracic injury pattern and severity for drivers. For distributed and corner deformation, oblique loading is approximately 3 times more likely to lead to thoracic injures than pure frontal loading. Conversely, the relative risk is 3.44 for offset deformation, which indicates that, for this impact location, frontal impact is more associated thoracic injuries compared to oblique impact. The effects of impact location and impact direction on serious-to-maximum injuries for three types of anatomical structures (organ, skeletal and vessel) are assessed as well. In addition to crash related variables (impact location and impact direction), results of the binary logistic regressions also indicate that crash severity (OR, 7.67–81.35) and occupant characteristics, including age (OR, 4.80–20.83), gender (OR, 1.16) and BMI (OR, 1.81), significantly affect the risks of thoracic injuries in frontal motor vehicle collisions.

The article discusses the mechanized process of post-harvest processing of root crops and onions. Unresolved problems associated with the imperfection of the design of the separating organs of the lines of post-harvest processing are noted. Data on the content of up to 44% impurities in the heap of onion sets are given, of which 42% are soil lumps, the largest percentage of them are lumps with a diameter of 15 to 22 mm. A variational distribution curve is presented in a pile of soil lumps commensurate with onion-set in thickness. A schematic diagram of a device for the separation of a pile of root crops and bulbs with a description of the process is proposed, which allows combining the transportation and cleaning of a pile of onions from impurities. The use of the device will reduce injury and improve the quality of separation of root crops and bulbs.

Engineers and scientists are able to understand and analyze the behavior of complex engineering systems in a wide range of critical technologies through hierarchical modeling followed by simulation of the model operation. This process results in a high fidelity integrated model as each level in the hierarchy is modeled in sufficient detail. The overall objective of this effort is to develop a sophisticated hierarchical model of the human body, followed by simulation of the model operation. In this initial research phase, the feasibility of the concept is explored and a framework for the model is described. A six-level model consisting of the whole body as a system, system of systems, organs, tissues, cells, and molecules is proposed and described. This paper explains that the human body is amenable to such hierarchical modeling and describes the benefits that can be achieved. The systems in the body deal with numerous processes: electrical, chemical, biochemical, energy conversion, transportation, pumping, sensing, communications, and so on. Control volume models for the organs in the body capture the mass and energy balance and chemical reactions. Tissue can be represented similar to structural components made of various biomaterials. Cells can be represented as a manufacturing and maintenance workforce assisted by molecular reactions. Following the representation of a healthy body, simulation runs by inserting faults and/or deficiencies in the operational parameters into the model could reveal the causes for specific diseases and illnesses. Such modeling and simulation will benefit medical, pharmaceutical, nutritional specialists, and engineers in designing, developing, and delivering products and services to enable humans to lead healthy lives.

Regional Air Mobility (RAM) focuses on building upon existing airport infrastructure to transport people and goods using innovative aircraft that offer a huge improvement in efficiency, affordability, and community-friendly integration over existing regional transportation options. These aircraft, which typically carry less than 20 passengers or an equivalent weight in cargo, are flexible in terms of where they can take off and land, even using existing runways and infrastructure to maximize compatibility with today’s airports. This project examines the feasibility of RAM supporting high-speed transportation for high-priority passenger and cargo movement within Fresno County and connection to coastal urban centers. Some examples of high-priority passengers and cargo could include, but would not be limited to, medical patients needing specialized and/or emergency treatment, organ transport, and critical medical supply deliveries. Electrification of aviation is happening, and Fresno County has the potential to combine our existing closely spaced underutilized airport infrastructure, early demonstration, and experience with electric aircraft, renewable energy opportunities, central location within the state, and the need to open the door for new industry opportunities for youth to take advantage of this “Third Revolution” in aviation.
