Abstract:
Iron is vital for all living organisms, but due to its ability to readily accept or donate electrons, it is also potentially toxic. Finely tuned mechanisms have evolved to control iron homeostasis at the systemic and cellular level. The peptide hormone hepcidin controls systemic iron homeostasis by binding to and degrading the iron exporter, ferroportin, leading to decreased iron efflux from duodenal enterocytes, macrophages and hepatocytes into the blood stream. Cellular iron homeostasis is regulated by the IRE / IRP system, which controls the levels of proteins involved in iron uptake, utilization, export and storage in a coordinated manner. Excess intracellular iron is stored and detoxified in ferritin. In this work, we study how an excess of iron is managed at the systemic and cellular level. In chapter II, we hypothesize that dietary and parenteral iron loading have differential effects on body iron status and hepcidin expression in mice. We perform time – course experiments and compare the effects of dietary and parenteral iron loading on circulating and tissue iron parameters. We show that dietary iron overload exceeds the capacity of hepcidin to lower body iron levels, and parenteral iron loading elicits a delayed hepcidin response. We provide evidence that circulating holo – transferrin and hepatocytic iron are the sole iron signals for hepcidin activation. In chapter III, we examine how an excess of iron is managed at the cellular level. We hypothesize that ferritin might benefit from an alternative, IRES - dependent mechanism of translation. We inhibit global or ferritin - specific cap – dependent translation initiation and challenge the cells with iron. We show that ferritin by – passes both the global and the specific inhibition of translation. We then test for the presence of an IRES in the 5'UTR of ferritin mRNA and further validate this sequence.
Le fer est vital pour tous les organismes vivants, cependant étant donné son habilité à donner ou accepter des électrons facilement, il a aussi le potentiel d'être toxique. Des mécanismes très précis ont évolué pour contrôler l'homéostasie du fer aux niveaux systémique et cellulaire. L'hormone peptidique, hepcidine, contrôle l'homéostasie du fer au niveau systémique par la dégradation de la ferroportine, l'exportateur cellulaire du fer. En conséquence, l'efflux du fer des entérocytes, des macrophages et des hépatocytes vers la circulation diminue. Au niveau cellulaire, le système IRE / IRP contrôle, d'une manière coordonnée, les niveaux des protéines impliquées dans l'acquisition, l'utilisation, l'exportation et le stockage du fer. L'excès de fer est stocké dans la ferritine. Dans ce travail, nous examinons comment l'excès de fer est géré aux niveaux systémique et cellulaire. Dans le chapitre II, nous émettons l'hypothèse que les surcharges orale et parentérale en fer ont des effets différents sur l'homéostasie systémique du fer et sur l'expression de l'hepcidine chez les souris. Nous comparons les effets des surcharges orale et parentérale en fer aux niveaux circulatoire et tissulaire. Nous démontrons que la surcharge orale en fer excède la capacité hypoferrémique de l'hepcidine alors que la surcharge parentérale en fer induit une réponse retardée de l'hepcidine. Nous apportons aussi la preuve que la holo – transferrine circulatoire et le fer hépatocytaire sont les signaux uniques de l'activation ferrique de l'hepcidine. Dans le chapitre III, nous examinons comment l'excès de fer est géré au niveau cellulaire. Nous émettons l'hypothèse que la ferritine bénéficie d'un mécanisme alternatif de traduction, dépendant d'une séquence IRES. Nous inhibons l'initiation de la traduction dépendante de la coiffe 5' globalement, ou spécifiquement pour la ferritine, et traitons les cellules avec une source de fer. Nous démontrons que la ferritine surpasse le blocage global ou spécifique de la traduction dépendante de la coiffe 5'. Nous testons la présence d'une séquence IRES dans l'extrémité 5' de l'ARNm et par la suite nous la validons.

FundaÃÃo de Amparo à Pesquisa do Estado do CearÃ
O objetivo deste estudo foi avaliar comparativamente a evoluÃÃo do tratamento da anemia em pacientes com insuficiÃncia renal crÃnica (IRC), em hemodiÃlise, em uso de eritropoetina recombinante humana (EPO), tratados com ferro oral e ferro endovenoso em Fortaleza, CearÃ, no perÃodo de janeiro de 2004 a janeiro de 2005. Trata-se de um estudo transversal observacional descrito em duas clÃnicas de hemodiÃlise em Fortaleza, CearÃ. Os dados foram coletados a partir dos prontuÃrios e mapas de exame realizados mensalmente pelos pacientes. De 165 casos selecionados, apenas 89 (54%) compuseram o banco de dados da pesquisa. Deste total 65 (71%) pacientes estavam usando exclusivamente sulfato ferroso oral, 09 (10%) faziam uso exclusivo do sacarato de hidrÃxido de ferro III administrado por via endovenosa, e 18 (19%) faziam uso de ambos. Os parÃmetros analisados durante o estudo foram: idade, sexo, tempo de diÃlise, doenÃa de base, dosagem da hemoglobina, determinaÃÃo do hematÃcrito, dosagem sÃricas do ferro sÃrico, ferritina sÃrica, Ãndice de saturaÃÃo da transferrina, capacidade latente e total de ligaÃÃo do ferro. Analisando os parÃmetros hematolÃgicos conclui-se que a anemia encontra-se bem controlada e estabilizada. PorÃm, a maioria dos pacientes apresentou ferritina sÃrica <400ng/ml e Ãndice de saturaÃÃo da transferrina (IST) <20%. Constatou-se, que houve melhora da anemia nos pacientes com IRC em hemodiÃlse, tratados com EPO e ferro oral. Todavia, para otimizar o tratamento da anemia na IRC deve-se levar em consideraÃÃo as complicaÃÃes da doenÃa de base, o grau da anemia, os estoques de ferro e os efeitos adversos relacionados a ferroterapia.
The object of this study was to compare the effectiveness of polynuclear iron (III) hydroxide sucrose complex, administered intervenously, with iron sulfate in the treatment of anemia in 89 patients with chronic renal insufficiency in hemodialysis, with r-HuEPO, in Fortaleza from January 2004 to January 2005. It was a transversal prospective study conducted in two hemodialysis clinics in Fortaleza â Cearà .the data were collected from the patients medical charts and expressed as arithmetic mean + - SD. In accordance with treatment, the patients were grouped in terms of treatment :oral and intervenous iron, and intervenous iron. The following parameters were analyzed during the study: age, gender, length of dialysis , baseline, hemogloibin, hematocrit , serum iron , serum ferritin, saturation level of transferrina, latent capacity and bonding of iron. It was establish that there occurred no superior effectiveness with polynuclear iron (III) hydroxide sucrose complex, administered intervaneously, in the treatment of anemia in patients with IRC in hemodialysis, in the use of r-HuEPO. The analyzed parameters during the study verified indirectly the supply of iron. Consequently, it was not possible to establish a confident and precise determination of the deficiency or oversupply of iron in these patients.

This is a study of the picture stones of Gotland and the oral tradition connected to them. This study consists of two main parts; in the main part the discussion focus on the oral tradition and the continuity of the same, where the memory plays a significant role. It also contains a discussion of the physical environment and its influences of the oral tradition. The second part concentrates around the picture stones, the variation of the scenes and the numerous of them.

Abstract:
During 20th century main concern was to have sanitary water flowing through the tap. In 21st century constant supply of safe drinking water is common at any home in USA. Hence consumers pay attention to aesthetic quality of tap water. Odorous algal metabolites in source water and metals introduced to drinking water due to corrosion of pipes in the distribution system cause taste, odor and color problems, and result in complaints and perception of tap water as unhealthy. Millions of dollars are spent each year by water industry to address and prevent these issues. This research focused on some of the taste-and-odor issues associated with drinking water. First aim was to understand when geosmin, 2-MIB, and nonadienal become detectable, employing two-resistance mass transfer theory to determine the concentration of odorants in bathroom air. Results showed that water temperature and odorant concentration in water play an important role. Next focus was to develop an international odor standard to be used for training of sensory analysis panelists. There are many sensory methods to monitor drinking water to detect the off-flavors however an odor standard has been missing. Hexanal was studied with trained flavor profile analysis panels and was proposed as an ideal odor reference standard to be used for training and sensory assessment of water samples. Main focus was to understand metallic flavor of drinking water caused by iron and copper. It was shown that metallic sensation has taste and retronasal components creating the flavor and humans are very sensitive to it. Occurrence of lipid oxidation in the oral cavity was shown when metals were ingested, that produces carbonyls which are responsible for the metallic flavor. Antioxidants and chelators were investigated to study prevention of lipid oxidation and, chelators were determined to be more effective. Oral epithelial cell cultures were developed as a model for oral cavity to further investigate lipid oxidation and effectiveness of the antioxidants and chelators. This dissertation is a result of inter-disciplinary work and possibly a good example for how problems may be solved by incorporating different methods and point of views from several disciplines.

La anemia en gestantes es un problema de salud pública importante a nivel mundial. Según la prevalencia registrada, en Perú persiste como un problema moderado. La pobre adherencia a la suplementación oral de hierro es una de las principales barreras para la efectividad de esta intervención. El objetivo del presente estudio fue determinar la variación de la adherencia a la suplementación oral de hierro en el tiempo y los factores asociados a esta. Para ello, se reanalizó la base de datos de la vigilancia centinela: “Estudio de adherencia a la suplementación con Hierro durante la gestación en las direcciones de salud de Apurímac y Ayacucho” de la Dirección General de Epidemiología (DGE), el cual tuvo un diseño de cohorte prospectiva longitudinal. Se realizó un análisis descriptivo de las 416 gestantes registradas y un análisis bivariado y multivariado en base a 376 gestantes. Se identificaron las variables sociodemográficas, gestacionales, relacionadas a la suplementación en el embarazo anterior y actual. Para el análisis de los factores asociados a la adherencia se usó el modelo GEE. En la segunda y tercera visita de seguimiento la adherencia incrementó en 4% y 8%, mientras que en la quinta y sexta, se redujo en 8% y 20,6%, respectivamente. Los factores que redujeron la adherencia fueron: falla en la dispensación (25, 3%), náuseas (9,5%), interacción de náuseas y vómitos (13,5%), y no le cae bien el suplemento (11%). En conclusión, la adherencia disminuye con el tiempo y los factores relacionados al suplemento y la falla en la distribución se relacionan a la reducción de esta
Anemia in pregnant women is a major public health problem worldwide. According to the registered prevalence, in Peru it persists as a moderate problem. Poor adherence to oral iron supplementation is one of the main barriers to the effectiveness of this intervention. The aim of this study was to determine the variation in adherence to oral iron supplements over time and the factors associated with it. To do this, the sentinel surveillance database of the "Study of Adherence to Iron Supplementation During Pregnancy in the Health Directions of Apurimac and Ayacucho", of general directorate of epidemiology (DGE), a longitudinal prospective cohort, was reanalyzed. A descriptive analysis of the 416 registered pregnant women and a bivariate and multivariate analysis based on 376 pregnant women was carried out. Sociodemographic, gestational, supplementation in the previous and current pregnancy variables were identified. For the analysis of the factors associated with adherence, the GEE model was used. At the second and third follow-up visit, adherence increased by 4% and 8%, while at the fifth and sixth, it decreased by 8% and 20,6%, respectively. The factors that reduced adherence were: dispensing failure (25,3%), nausea (9,5%), interaction of nausea and vomiting (13,5%), and did not like the supplement (11%). In conclusion, adherence decreases with time and the factors related to the supplement and the failure in the distribution are related to its reduction.

Abstract:
Grave X was found in 1952 during construction work in Tuna in Badelunda parish, in the province of Västmanland. Objects from this 3rd Century grave were dispersed and the stone grave covering and cist-like wooden burial chamber were cut almost in half as a result of the construction work that unearthed it. The purpose of this dissertation is to create a better understanding of Tuna in Badelunda and to place Grave X and the person buried there in context. Due to my interest in Grave X and the person in this grave, the scope of the study is limited to Tuna during the Late Roman Iron Age. What kind of place may Tuna in Badelunda have been during that time? Which kinds of knowledge may the person in Grave X have possessed and what roles may this person have had in local society? How may this person have acted in Tuna in Badelunda in particular? Why was this person buried in the specific type of structure that was Grave X? To answer these questions, ancient monuments and phenomena in the Tuna area, objects from the grave and construction details of the grave are discussed. Specifically, I examine the name Tuna, stone enclosures, hillforts of Bejby borg-character and travel routes, beads, golden rings in the shape of snakes, vessels and serving utensils, and the stone grave covering and cist-like chamber. Since Grave X was partly ruined when discovered, comparisons are made to about 20 similar graves from other parts of Scandinavia in order to get an idea of what may have been lost from Grave X. A performative-constructive gender perspective is of importance in this dissertation, as well as the concept of creolization. The kinds of knowledge and the societal roles the person in Grave X may have had can be summarized in five categories or contexts of action: production within the (social-political) economy of the farm, ritual performances, physical communication, textile production, and oral performances with the telling of stories and relating of memories. Possible personal strategies in relation to the activities the person in question was involved in are seen as important. One way this dissertation takes up this subject is through the discussion of the role the person may have had in greetings and farewells in the yard of the farm (Sw. tun, gårdsplan). Greetings and farewells were probably of importance, and Tuna is discussed as a crossroads. This means that although a local perspective is advocated in this dissertation, Tuna may not be seen as an isolated community, but rather as a small place that to a great extent partook in the larger world. This can also be seen in Grave X; when the person in this grave was buried, the living made choices that both expressed local traditions and made reference to far-away places. In contrast to the surrounding graves, the person in Grave X was not cremated. One of many possible reasons may have been a desire to emphasize the person’s personality and gender as well as roles in society.

Le travail comprend quatre parties. C’est dans la troisième partie que les thèmes extraits des contes sont classés sous deux catégories principale (valeurs d’action, valeurs morales) prévues comme base de l'analyse et les contes sont expliqués a l’aide de symboles. Les résultats obtenus montrent différentes valeurs éducatives que les contes peuvent transmettre.

L’art du naqqâli en Iran est basé sur une longue tradition de récits épiques portant sur des héros de la légende nationale iranienne. La principale source de ces épopées, Le Livre des rois de Ferdowsi, est un œuvre fondamentale de la culture nationale. Les conteurs professionnels «naqqâl» récitent, en les adaptant, les passages du Livre des rois pendant des heures à l’assistance dans certaines maisons de thé, plus connues sous l'appellation de cafés traditionnels. La représentation du naqqâli qui dure approximativement une heure et demi comporte trois parties d’environ une demi-heure chacune. Une session habituelle commence par une introduction poétique suivie d’une entrée en matière pour introduire l’histoire et s’achève par l’histoire proprement dite. Le début et la fin de chaque partie sont marqués par une combinaison de salavât (prière récitée en chœur) et de récitations de poèmes classiques qui ne sont pas nécessairement liés à l’histoire elle-même, mais servent en général à retenir l’attention du public pendant la représentation. Les qualités de la récitation du naqqâl reposent sur un ensemble de méthodes complémentaires permettant de donner plus de relief au spectacle : les méthodes oratoires, gestuelles et l’approche musicale. Les naqqâl utilisent les rouleaux de parchemin appelés tumâr pour préparer la représentation et s’aider pendant la récitation tout en prenant des libertés par rapport au texte dans le but de retenir l’attention de leur auditoire. Les caractéristiques formelles spécifiques de ce genre de représentation qui se rapporte à ses contraintes minimales sont : la durée fixe d’une représentation et son découpage sérialisé, l’influence plus ou moins directe de la structure du tumâr et en particulier sa division en trois niveaux d’unités structurelles : le chapitre, la partie et l’épisode
The art of naqqâli in Iran is based on a long tradition of epics involving heroes of the Iranian National legend. The main source of these epics, Ferdowsi’s Book of the Kings, is a cornerstone of the national culture. Over long hours, professional naqqâl recite passages from the Book of the Kings to the audience in tea-houses, known as traditional cafes. The naqqâli performance lasts about one and half hours and up to three parts for about half an hour each. A session usually begins with an introduction followed by a poetic prelude to outline the story and ends with the story itself. The beginning and end of each part are salavât (prayer recited in unison) and recitations of classic poems which are not necessarily related to the history itself, but in general serve the purpose of gaining the public attention during the performance. The quality of the naqqâl’s recitation based on a set of methods to give a larger dimension to the show such as: oratory techniques, gesture and musical approach. Naqqâl use parchment scroll called tumâr prepared for the performance which help with the recitation while taking liberties with respect to the text in order to attract the attention of their audience. The formal features of this type of performance which refer to its minimal constraints are : the fixed duration of performance and its serial form, the more or less direct influence of the structure of the tumâr, particularly its division into 3 levels of structural units : chapter, instalment and episode

Iron catalyzes the formation of mutagenic reactive oxygen species (ROS), which are critically involved in the whole process of cancer formation including cancer cell initiation, proliferation, metabolism, angiogenesis and metastasis. Recent evidence suggests iron chelators may work as a potential anti-tumor agent through disturbing the iron metabolism. Actually, there is an interesting case report showing that the iron chalation therapy with deferasirox, intended to treat transfusional iron overload initially, induced complete remission on a patient of refractory and relapsed acute myeloid leukemia (AML). The anti-proliferation effects of various iron chelators were also demonstrated in many cancer cell lines, including neuroblastoma, cervical carcinoma, ovarian, and human leukemia. Nevertheless, the anti-leukemic activity and mechanism of iron chelators remained largely to be clarified. Our study is to examine the anti-leukemic activity and explore the potential anti-leukemic mechanism of the widely available oral iron chelator, the Deferasirox (DFX). Our results showed that low concentration of DFX (clinically achievable concentration) could effectively inhibit the growth of primary human AML cells as well as human AML cell lines (THP1 and HL60) and murine AML cell line (WEHI-3). DFX disturbs the cell cycle of THP1 cells by increasing the percentage of sub-G1 phase and decreasing the percentage of G2-M phase in a dose-dependent manner. We clearly showed that DFX induce caspase-3-related apoptosis and also inhibit ERK phosphorylation, which may account for the anti-leukemic effect of DFX. We also try to enhance the anti-leukemic effect through combination of DFX with the most commonly used chemotherapeutic agents for AML – the Doxorubicin (DOX) and Cytarabine (Ara-C). Interestingly, we found DFX may antagonize the anti-leukemic effect of DOX when DFX and DOX were added to the cell culture concomitantly. We found DFX markedly reduces the intracellular ROS generated by DOX, which may well explain the antagonistic effect between these two drugs. On the other hand, the concomitant use of DFX and Ara-C has a synergistic effect on AML cells. The ultimate goal of this study is to improve current AML treatment, especially elderly patients, which are frequently unable to tolerate intensive chemotherapy. We now plan to apply these viable information to design a clinical trial of using DFX and low-dose Ara-C for the treatment of elderly AML.

Oral squamous cell carcinoma is one of the top ten malignancies in many developing countries. According to Department of Health in Taiwan, oral cancer is ranked fourth leading cause of cancer death in male’s group. How to cure cancer while decrease the side effect and mortality is a big challenge to cancer therapy. In previous published study, we discovered that iron-core-gold-shell nanoparticles (Fe@Au NPs) have selective cytotoxicity in oral cancer cells (DOK, OECM1, SCC25, HCDB1, SCC9), but not in normal cells (hNOK, HUVEC, Vero, and gingiva fibroblast). In addition, Fe@Au NPs can cause mitochondria membrane potential lost and induce autophagy in OEC-M1. However, the mechanism of the selective cytotoxicity remains unsolved. In this study, we discovered that many iron-containing nanoparticles (FeAu alloy, Fe@Ag, Armor-Fe) also have the same selective cytotoxicity in OECM1. Furthermore, there is literature report that iron-platinum alloy (FePt) nanoparticles exhibit high cytotoxicity to ovarian cancer cell (A2780). These results suggest that iron element is a key factor in cytotoxicity effect. Base on this thinking, we identified through the Fe/O ratio analyzed by EDS that these iron-containing nanoparticles share one common characteristic, the non-oxidized iron, a possible key factor of the cytotoxicity. Otherwise, we also found that these iron-containing nanoparticles with high Fe/O ratio would induce severe ROS in cancer cells and caused irreversible mitochondria membrane potential lost. Furthermore, these iron-containing nanoparticles could also induce autophagy that caused autophagic cell death in cancer cells through the increased dose and time. In animal study, the iron-containing nanoparticles could inhibit tumor growth. In summary, we discover from this study that the non-oxidized iron of the iron-containing nanoparticles can function as a ROS inducer that can cause cancer cell death through autophagy. According to these findings, we hope to develop a new theranostic anti-cancer drug that can kill cancer cells more effectively and safely in the future.

BACKGROUND Iron deficiency anaemia (IDA) is a common condition in patients presenting for surgery and is found in up to 75% of non-cardiac surgical patients. Pre-operative haemoglobin (Hb) is a strong predictor of transfusion requirement and it should, as part of a comprehensive blood conservation approach, be optimised whenever possible. Treatment options for iron deficiency anaemia include oral and intravenous iron or red blood cell transfusion. That both, anaemia and red blood cell (RBC) transfusion expose the patient to unnecessary risks is supported by an ever-increasing body of evidence. Consequently, allogeneic RBC transfusion should be avoided whenever possible and alternative treatment modalities which optimize the patient’s own red cell mass should be exhausted. However, anxiety over the short and long term effects of intravenous iron have limited its more widespread use. Newer dextran-free compounds, however, provide a safer treatment option. OBJECTIVE The objective of this systematic review was to critically appraise, synthesise and present the best available evidence related to the effectiveness and economic aspects of intravenous iron administration on the correction of iron deficiency anaemia in the peri-operative period. DATA SOURCES A comprehensive search was undertaken on major electronic databases from 2001 to 2012. The search was conducted in English, German, Italian and Dutch. REVIEW METHODS Randomized controlled trials, quasi-randomized controlled trials and quasi-experimental studies were included in the review. Critical appraisal and data extraction were undertaken using the Joanna Briggs Institute critical appraisal instrument and the standard data extraction form for evidence of effectiveness. Anaemia correction was as defined by study authors RESULTS The quantitative component of the review identified two randomized controlled trials for inclusion. One study of 76 patients with menorrhagia evaluated the effectiveness of pre-operative intravenous (IV) iron compared with oral iron in anaemic patients scheduled for surgery – the nature of which was not reported, aiming for a 1 g/L haemoglobin increase preoperatively. The intravenous iron group had greater increases in the Hb level than the oral iron group (3.0 vs. 0.8 g/dl, respectively; p < 0.0001). One other study evaluated the effectiveness of pre-operative intravenous (IV) iron compared with placebo in patients with bowel cancer scheduled for resectional surgery, looking at haemoglobin changes between recruitment and day of admission. A small subgroup of these patients was anaemic but no difference was demonstrated in haemoglobin changes between the groups. One additional RCT was found examining the effectiveness of IV iron in cardiac surgical patients, but full text examination and appraisal revealed that the majority of patients enrolled in these studies were not anaemic. The search for the economic component of the review revealed no randomized controlled trials examining the cost effectiveness of preoperative correction of iron deficiency anaemia with intravenous iron CONCLUSIONS Our review found insufficient data to make firm conclusions about the efficacy of pre-operative intravenous iron administration for the correction of anaemia based on clinical trial settings. Neither could we establish firm conclusions on the potential cost savings due to intravenous iron supplementation. IMPLICATIONS FOR PRACTICE There is inadequate RCT evidence at present to guide clinical practice. IMPLICATIONS FOR RESEARCH Our study found insufficient data to make firm conclusions about the efficacy, safety and cost effectiveness of pre-operative intravenous iron administration for the correction of anaemia pre-operatively. Adequately powered RCTs are required that evaluate and report the efficacy, safety and potential cost savings of intravenous iron administration as a treatment modality for iron deficiency anaemia.

Abstract:
Through the creation of a graphic novel based on a Romanian Holocaust survivor’s testimony, this study attempts to clarify the role of artistic creation in meaning-making during Holocaust and genocide education. In facilitating empathy and moral education, the creative process encourages a deeper exploration of these troubling topics than is possible within the confines of a traditional academic approach. In order to understand this process, I worked with the testimony of Zoly Zamir, who escaped Bucharest following the Iron Guard Rebellion of 1941. The creation of the graphic novel took me from Austin to Houston and Romania, where I sought to trace the echoes of history in architecture and environment. Translating Zamir’s story into word and image produced an empathetic bond to the narrative and the region, facilitating a deeper understanding of the hows and whys of the Holocaust. That engagement spurred a desire to continue to ask questions, to look beyond a regimented understanding and view the broader implications of the history.
