The chapter on the epidemiology of pain and addiction opens with a discussion of epidemiological principles and then proceeds to review two categories of data: that which are available on the use of opioids to treat chronic pain, and that reflecting opioid use disorder (OUD). The data are provided with graphical representations that may aid in discussions and in teaching. An accompanying glossary of epidemiological terminology facilitates understanding of the data. Conclusions are drawn from the prevalence of use and of mortality that contribute to an understanding of the origins and direction of the opioid use crisis. An accompanying text box provides resources for further exploration of the topics discussed in the chapter.

This chapter defines terms such as “opiate” and “opioid” and explains their physiological functions as agonist, antagonist, and partial agonist. It notes the long history of medicinal opioid use for acute pain, as well as the curse of addiction with chronic use, highlighting the three opioid use epidemics in the United States: the morphine addiction of the Civil War era, the heroin addiction of the Vietnam War era, and the current epidemic of prescription opioid addiction. The chapter describes the commonly encountered natural, synthetic, and semi-synthetic opioids, noting their therapeutic use, potential for abuse, and their use in management of opioid use disorders. Of particular interest are the sections on detoxification and relapse prevention using methadone, buprenorphine, and naltrexone. The sections also explain the rationale for their use against the epidemiological background and recent exploration of genetic influences. The chapter also emphasizes the need for an integrated strategy in overcoming opioid addiction.

Abstract:
Managing pain in the patient with substance use disorder can be challenging. This chapter describes those challenges and provides strategies to address them. Specifically it discusses the prevalence and specific considerations for commonly abused substances, the need for aggressive communication among perioperative clinicians, and a strategy to decrease acute postoperative pain and associated complications using opioid-sparing, multimodal analgesia. It includes a discussion of the concept of equianalgesic opioid doses and management of opioid-related side effects including respiratory depression, with regard to buprenorphine, naltrexone, and methadone. Specific consideration is given to the surgical patient treated with buprenorphine, and a defined clinical plan is outlined.

Patients with chronic pain tend to be complex and can present with multiple comorbidities, including anxiety, depression, functional disabilities, and substance misuse or abuse. The burgeoning rate of prescription opioid misuse and abuse and opioid-related fatalities has generated a great deal of scholarly activity on understanding the etiology of opioid misuse/abuse and developing risk assessment and mitigation strategies to curb this public health crisis. Balancing effective pain management and reducing the risk of opioid misuse/abuse and diversion can be a daunting endeavor, as is controlling pain in patients with pain and concomitant substance use disorders. This chapter provides an overview of the prevalence of opioid misuse/abuse in patients with chronic pain. It covers pharmacologic and nonpharmacologic therapies for patients with pain and co-occurring opioid use disorder and also discusses the challenges and opportunities to improve pain care and reduce misuse and abuse of opioids.
