Abstract:
Quantitative and qualitative analysis of bacterial isolates in water and fish samples (Oreochromis niloticus and Clarias gariepinus) from Jebba Lake were carried out using primary isolation media and microbact identification kits. The concentration of heavy metals, Lead (Pb), Copper (Cu), Cadmium (Cd), Chromium (Cr) and Arsenic (As) were determined using Atomic Absorption Spectrophotometer (AAS). Samples were collected from five (5) different stations. Station Jl.1 and Jl.2 are located at the upper course of the lake and were characterized by high human activities (washing, bathing, mining and animal husbandry operations) especially in sample station Jl.2. Sample station Jl.3 has low human activities, however it has a tributary with high mining activities. Station Jl.4 has high domestic washings but low animal husbandry operation and low mining activities. Station Jl.5 has low domestic washings, low animal husbandry operation and low mining activities. Mean faecal coliform count of sample station Jl.1 (580.83 CFU/100ml) and station Jl.2 (700.83 CFU/100ml) were above maximum permissible limit for Federal Environmental Protection Agency (FEPA). Faecal coliform count of sample stations Jl.3, Jl.4 and Jl.5 were below maximum permissible limit. Maximum faecal coliform count during the wet season was 1600.00 CFU/100ml, while the maximum count during the dry season was 920.00 CFU/100ml. Bacterial species such as Aeromonas caviae, Aeromonas hydrophila, Pseudomonas fluorescens – 25, Escherichia coli, Moraxella species, Mannheimia (pasturella) haemolytica, Pseudomonas aeruginosa, Pseudomonas fluorescens – 35, Escherichia hermannii were found in water samples and species such as Vibrio alginolyticus, Moraxella species, Escherichia hermannii, Vibrio parahaemolyticus, Aeromonas hydrophila were isolated from fish intestines and gills. Concentration of Cu, Cr and As in water was observed to be within permissible limit but the concentration of Pb and Cd were above tolerable limit. In fish organs, the concentration of heavy metals were within permissible limit except that of Pb and As in Oreochromis niloticus fish intestines (ONFI) (0.38±0.00 and 0.03±0.00 respectively) and Clarias gariepinus fish intestines (CGFI) (0.40±0.00 and 0.03±0.01 respectively) which were above maximum permissible limit by standard organizations. Abstract:
ObjectiveWe aimed to determine the prevalence and risk factors for osteonecrosis of the femoral head (ONFH) in a multicentre cohort of patients with antineutrophil cytoplasmic antibody-associated vasculitis (AAV).MethodsOne hundred and eighty-six AAV patients who underwent radiographs and MRI screening of bilateral hip joints at more than 6 months after initial remission induction therapy (RIT) were retrospectively assessed for the presence of ONFH.ResultsAmong 186 AAV patients, 33 (18%) were diagnosed with ONFH. Among the patients with ONFH, 55% were asymptomatic and 64% had bilateral ONFH. Seventy-six per cent of ONFH joints were in precollapse stages (stage ≤2), whereas 24% of ONFH joints were in collapse stages (stage ≥3). Moreover, 56% of the precollapse stage joints were already at risk of future collapse (type ≥C-1). Even in asymptomatic ONFH patients, 39% of the precollapse stage joints were type ≥C-1. Prednisolone dose of ≥20 mg/day on day 90 of RIT was an independent risk factor for ONFH in AAV patients (OR 1.072, 95% CI 1.017 to 1.130, p=0.009). Rituximab use was a significant beneficial factor against ONFH (p=0.019), but the multivariate analysis rejected its significance (p=0.257).ConclusionEighteen per cent of AAV patients developed ONFH, and two-thirds of the ONFH joints were already in collapse stages or at risk of future collapse. Prednisolone dose of ≥20 mg/day on day 90 of RIT was an independent risk factor for ONFH. A rapid reduction of glucocorticoids in RIT and early detection of precollapse ONFH by MRI may decrease and intervene ONFH development in AAV patients.

Abstract:
Introduction: The purpose of this study was to investigate the incidence of necrosis and insufficiency fracture of the hip and knee using magnetic resonance imaging (MRI) in renal transplant patients. Methods: We examined 300 consecutive renal transplant patients using MRI of the hips and knees (T1-weighted coronal MRI) preoperatively and at 3 and 6 months postoperatively. Oblique axial-plane T1-weighted MRI and short-tau inversion-recovery T2-weighted MRI were performed parallel to the femoral neck when distinguishing the osteonecrosis of the femoral head (ONFH) from subchondral insufficiency fracture (SIF) of the femoral head was difficult. Results: ONFH was observed in 3 hips (3 cases, 1.0%). Osteonecrosis of the knee was observed in 2 knees (1 case, 0.3%). SIF of the femoral head was observed in 5 hips (3 cases, 1.0%). Bilateral simultaneous onset of SIF of the femoral head was observed in 2 cases (0.7%). Insufficiency fracture of the knee was observed in 2 knees (1 case, 0.3%). Conclusions: When it was difficult to distinguish ONFH from SIF of the femoral head, oblique axial-plane T1-weighted MRI and the short-tau inversion-recovery T2-weighted MRI parallel to the femoral neck were useful to establish the correct diagnosis.

Abstract:
Background and purpose: Little is known on the use of metal artifact reduction sequence (MARS) MRI to diagnose osteonecrosis of the femoral head (ONFH) after fixation of femoral neck fractures (FNF) with conventional metal implants present. We compared MARS MRI with radiography in diagnosing ONFH. Secondarily, we determined whether signs of ONFH on MARS MRI correlate with patient-reported outcomes (PROs) via Oxford Hip Score (OHS) and pain (VAS).Patients and methods: 30 adults under 60 years treated with internal fixation after FNF were prospectively included (2015–2018) at 2 hospitals. They were followed up with radiography and PROs at 4, 12, and 24 months and MARS MRI at 4 and 12 months. OHS < 34 or VAS pain > 20 was considered significant. Results: At 12 months, 14 patients had a pathological MRI. 3 of 14 had ONFH on radiographs at 12 months, increasing to 5 at 24 months, and 4 had unfavorable PROs. 2 of 5 patients with ONFH signs on both MRI and radiography had unfavorable PROs. 1 of 10 patients with normal MRI and radiography had unfavorable 2-year PROs. 4 patients had inconsistent MRI results, of which 1 developed ONFH. 1 patient dropped out. Conclusion: Information from a pathological MRI was not useful, as a majority remained free from symptoms and ONFH signs on radiographs. Furthermore, PROs did not correlate with imaging results. MARS MRI findings must be better understood before being taken into clinical practice. However, a normal MARS MRI seems to be a good prognostic finding.

Abstract:
Abstract Introduction Osteonecrosis of the femoral head (ONFH) is characterized by insidious onset of hip joint pain, limited range of motion and disability in sickle cell disease (SCD) patients. Prevalence of ONFH, rates of hip replacement surgery and post-operative course have not been previously described in population-based cohort studies. We retrospectively reviewed a large SCD patient cohort from California's Office of Statewide Planning and Development (OSHPD) Patient Discharge Database (PDD), and report the risk factors and clinical outcomes for SCD patients with ONFH. Methods Patients were ascertained from the PDD from 1991 to 2013, through an iterative search algorithm using ICD-9 codes. In addition to descriptive statistics and univariate comparisons with the chi-square test, we used multivariable Cox proportional hazards models (with age as the time scale), to analyze factors associated with ONFH diagnosis, including sex, SCD severity and antecedent acute chest syndrome (ACS; data available from 2003-2013). Patients with more severe SCD were defined as those who averaged 3 of more hospitalizations per year. Antecedent ACS was considered as a time-dependent covariate to first ONFH diagnosis after 2003. We estimated the cumulative incidence of ONFH, accounting for the competing risk of death, by SCD severity and antecedent ACS diagnoses, with differences assessed by the Gray's test for equality. We also determined rates of all re-admissions to the PDD and emergency department within 30-90 days after hip replacement surgery, including re-admissions for venous thromboembolism (VTE) and painful vasoocclusive crises (VOC). All data were analyzed using SAS 9.4 software, and results presented as hazard ratios (HR) and 95% confidence intervals (CI). Results Of the 6,237 SCD patients identified, 1,356 (22%) were diagnosed with ONFH (Table 1). Three hundred and eight of the SCD patients with ONFH underwent hip arthroplasty. The median ages of ONFH diagnosis and hip replacement surgery were 27 and 36 years, respectively. In Cox regression analyses over the entire study period, both males (HR 1.13, CI 1.01 - 1.27) and SCD severity (HR 2.75, CI 2.44 - 3.11) were associated with ONFH. In Cox regression analyses from 2003-2013, SCD severity (HR 2.89, CI 2.48 - 3.36) and antecedent ACS (HR 1.56, CI 1.31 - 1.84) were independently associated with ONFH diagnosis. The cumulative incidence curves corroborate the association between SCD severity, antecedent ACS, and ONFH (Figures 1 and 2; p<0.0001)). By 40 years of age, 36% of patients with severe SCD (vs. 14% without severe disease) and 36% with antecedent ACS (vs. 12% without ACS) had developed ONFH. Among SCD patients with ONFH, 43%, 51% and 58% were re-admitted postoperatively within 30, 60 and 90 days, respectively. At 60 and 90 days after discharge for hip arthroplasty, 22% and 28% were readmitted for VOC, respectively. Conclusions The overall prevalence of ONFH in this SCD cohort was 22%, and approximately 23% of the ONFH patients underwent hip arthroplasty (5% of the entire SCD cohort). Thus, ONFH is a common complication of SCD, which often requires surgical intervention. ONFH was independently associated with SCD severity and ACS, as has been previously suggested (Kato, Gladwin et al. 2007). Readmission within 30-60 days after hip surgery was common in this SCD cohort, and often due to painful VOC. Of the 144 readmissions within 90 days of discharge from hip replacement surgery, only 2 (~1%) were due to VTE, which is lower than reported in non-sickle cell patients undergoing hip replacement surgery. Our large, populations-based cohort study provides insight into the frequency and risk factors of ONFH in SCD patients, and also reveals that post-operative readmissions are common. Abstract:
Objective: To evaluate the predicting value of 99mTc-hydroxydiphosphonate (HDP) pinhole bone scan in development of osteonecrosis of femoral head (ONFH) in patients with femoral neck fracture after cannulated screw fixation. Methods: Pinhole bone scan of patients with metallically fixed femoral neck fracture from 2001 to 2015 were retrospectively reviewed. Initial pinhole bone scan was obtained within 2–3 weeks after surgery. Findings of initial pinhole bone scan were divided in to 4 groups. Group CU included cold defect in affected femoral head, group HU with no cold defect. Group PP with increased uptake along the inserted screws and group PN with no increased uptake along the inserted screws. More than 6 months of follow-up with pinhole bone scan and clinico-radiological evidence for ONFH was reviewed. Results: 72 patients (mean age 54.01 years, male 22, female 50) were included. 19 patients were in group CU, 53 in group HU. 60 patients were in group PP, 12 in group PN. During the follow-up, 13 patients were diagnosed as ONFH. 9 (47.36%) patients in group CU developed ONFH and 4 (7.5%) in group HU. 4 (6.66%) patients in group PP developed ONFH and 9 (75%) in group PN. Conclusions: To predict ONFH of femoral head followed by neck fracture, many imaging techniques with variable results were known. In this study, cold defect in early postoperative pinhole bone scans could predict ONFH, and loss of increased uptake along screw inserted site could be a strong indicative sign of ONFH. Further evaluation with a larger population is necessary.

Abstract:
Abstract Free vascularized fibular grafting was reported a favorable method to prevent the collapse of the femoral head. This study analyzed the mid- to long-term outcomes of avascular fibular grafting (AVFG) for osteonecrosis of the femoral head (ONFH) with 6- to 8-year follow-up. From March 2009 to March 2012, 34 patients (48 hips) were diagnosed with ONFH operated with AVFG in Jiangsu Province Hospital of traditional Chinese Medicine. We retrospectively reviewed the clinic outcomes of these patients and evaluated the differences in outcomes by diverse stages [Association Research Circulation Osseous (ARCO)] and types [China–Japan Friendship Hospital (CJFH) classification] annually. The hip survival rate, Harris hip score (HHS), activity level and imaging stability were calculated. The average follow-up time was 6.7 years. At the last follow-up, 34 of 48 hips survived totally. According to the ARCO stage, the overall clinical success rate for hip preserving were 76.0% (19/25) in II stage, 85.7% (12/14) in IIIA stage and 33.3% (3/9) in IIIB stage. Referring to the CJFH classification system, the hip survival rate were 100% (2/2) in M type, 90.9% (10/11) in C type, 77.8% (14/18) in L1 type, 57.1% (8/14) in L2 type and 0% (0/3) in L3 type. The mean visual analog scale (VAS) score, HHS and Western Ontario McMaster Osteoarthritis index (WOMAC) were significantly improved at the final follow-up compared with pre-operative values (P < 0.001). The AVFG operation can increase the hip function and improve patients’ lives quality. The mid- to long-term efficacy can satisfy fundamental life requirements, especially for those early-stage and small-scale patients who suffer ONFH to avoid or put off the time of total hip arthroplasty surgery.

Abstract:
Abstract. A group of small pelagic fish captured between 1981 and 2012 within El Niño area 1–2 by the Ecuadorian fleet was correlated with the oceanographic Multivariate ENSO Index (MEI), and the Oceanographic El Niño Index (ONI) referred to El Niño region 3–4. For the period 1981–2012, total landings correlated poorly with the indexes, but during 2000–2012 (cold PDO) they proved to have a 14–29 % association with both indexes; the negative slope of the curves suggested higher landing during cold events (La Niña) and also indicated a tendency to decrease at extreme values ( > 0.5 and < −1.0). Round herring (Etrumeus teres) fourth-quarter (Q4) landings were related to the MEI in a nonlinear analysis by up to 80 %. During moderate or strong La Niña events landings noticeably increased. Bullet tuna (Auxis spp.) catches showed a negative gradient from cold to warm episodes with an R2 of 0.149. For Chilean jack mackerel (Trachurus murphyi) irregular landings between 2003 and 2007 were observed and were poorly correlated (R2 < 0.1) with ONI or MEI. Anchovy (Engraulis ringens) captured in Ecuadorian waters since 2000 had an R2 of 0.302 and 0.156 for MEI and ONI, respectively, but showed a higher correlation with the cold Pacific Decadal Oscillation (PDO). South American pilchard (Sardinops sagax) was higher than −0.5 for the ONI and MEI, and landings dramatically decreased; however, Q4 landings correlated with ONI and MEI, with R2 of 0.109 and 0.225, respectively (n = 3). Linear correlation of Q4 indexes against the following year's Q1 landings had a linkage of up to 22 %; this species could therefore be considered a predictor of El Niño. Chub mackerel (Scomber japonicus) landings did not have a significant linear correlation with the indexes for 1981–2012 and therefore could not be considered a valid predictor. Chuhueco (Cetengraulis mysticetus) is a local species with high landings during El Niño years and, conversely, remarkably low landings during La Niña years. Additionally, chuhueco availability and landings were negatively affected by cold PDOs. Pacific thread herring (Opisthonema spp.) showed a 24 and 36 % relationship between landings (Q1) and the MEI and ONI (Q4). Therefore, results suggest that the South American pilchard and Pacific thread herring could be considered good species to use as predictors of El Niño in region 1–2 (Ecuador), especially when average Q4 MEI ∕ ONI is used against the next trimester Q1 landing. All species were prone to lower landings and/or fishing availability during strong–extreme events (ONI/MEI, > 1.0 and < −1.0), and were also shown to be affected by the PDO. In the long term, landings decreased under warm PDO and vice versa, and therefore PDO fluctuations could be used to help manage these fisheries and to help the industry in long-term planning.

Abstract:
BackgroundAdult-onset Still’s disease (AOSD) is a rare systemic autoinflammatory disease, characterized by symptoms such as spiking fever, arthralgia, sore throat, and skin rashes. The detailed pathogenesis of AOSD is not yet understood, hence the management often involves the use of corticosteroids. Among glucocorticoid-associated complications, osteonecrosis of the femoral head (ONFH) is rare and not immediately life threatening, however, delayed diagnosis can lead to a significant reduction in patients’ quality of life and often requires surgical treatment. Although it is of great importance for rheumatologists to understand ONFH, little is known about ONFH in patients with AOSD because most studies on ONFH in adult have focused on systemic lupus erythematosus (SLE) despite AOSD being heavily glucocorticoid-exposable similar to SLE.ObjectivesTo investigate the prevalence of ONFH complications among AOSD patients and explore its risk factors.MethodsWe reviewed all consecutive patients with AOSD who have visited the Keio University Hospital from 2012 to 2022 retrospectively. We investigated the development of ONFH after treatment and assessed relevant clinical features and laboratory data. To compare the frequency of ONFH in patients with AOSD with that in those with other rheumatic diseases, we also investigated prevalence of OFNH in patients with other rheumatic musculoskeletal diseases including SLE, idiopathic inflammatory myopathy (IIM), and vasculitis who had a history of treatment with high-dose (0.8 – 1mg/kg) glucocorticoid. All statistical analyses were performed using JMP 15 (SAS Institute Inc., Cary, NC, USA).ResultsWe included 108 AOSD patients in the analysis and collected information from their charts. The mean age of onset was 44 years old, and 77% of the patients were female. Out of 108 patients, 9 (8.3%) developed OFNH throughout the mean observation period of 12.8 years. All 9 patients with OFNH had a history of high-dose corticosteroid therapy, and 8 of 9 (88.9%) received glucocorticoid pulse therapy. Univariate analysis revealed that male and complication of serositis or remarkable leukocytosis (>15000/µL) were associated with development of ONFH in AOSD patients, in addition to the history of treatment with high-dose glucocorticoid and pulse therapy (Table 1.)Next, we compared the frequency of ONFH development among 60 AOSD patients with high-dose corticosteroid treatment history with that of patients with other rheumatic diseases including SLE (n=312), IIM (n=84), small-vessel vasculitis (n=123), and large-vessel vasculitis (n=59). We identified that AOSD patients were second most likely to develop ONFH, following SLE; ONFH prevalence was 15.0% in AOSD, 20.5% in SLE 6.0% in idiopathic inflammatory myopathy, 1.6% in small-vessel vasculitis, and 1.6% in large-vessel vasculitis (Figure 1).ConclusionThe complication of ONFH is frequent in patients with AOSD, the second highest after SLE in rheumatic musculoskeletal diseases. Glucocorticoid pulse therapy, high-dose glucocorticoid therapy, male sex, and remarkable leukocytosis and serositis are risk factors for developing ONFH.References[1]Choi HR, Steinberg ME, Y Cheng E. Abstract:
BackgroundSymptomatic osteonecrosis of the femoral head (ONFH) and avascular osteonecrosis (AVN) are two types of osteonecrosis which is a serious complication of systemic lupus erythematosus (SLE). Even their pathophysiology of them is similar, though the onset timing, symptoms and signs, and other clinical characteristics are different.ObjectivesTo compare the clinical characteristics of SLE combined with symptomatic ONFH and AVN.MethodsWe retrospectively analyzed the clinical data of 86 cases of SLE with symptomatic ONFH and AVN treated in the Department of Rheumatology and Immunology of The First Affiliated Hospital of Xi’an Jiaotong University from April 2013 to December 2021, and compared the similarities and differences between them.ResultsA total of 2730 patients with SLE were reviewed, including 60 patients combined with ONFH (male 9, female 51) and 26 patients combined with AVN (male 2, female 24). Morbility of ONFH was significantly higher than AVN. Compared with AVN group, the course of SLE was longer [(71.33±53.32) vs (48.69±35.11) months],the time of glucocorticoids (GCs) use was longer [(67.47±51.71) vs (37.77±42.73) months], and the cumulative dosage of GCs was larger [(9.17±5.46) vs (6.02±6.66) g] in ONFH group. SLEDAI score was lower in ONFH group than in AVN group [(2.57±3.39) vs (4.46±4.81)]. The ONFH group had more Raynaud’s phenomenon and lower serum Vitamin-D3 level compared with AVN group. Anti-cardiolipin antibodies (ACA) positive (15.38%) and edema as the initial symptom (46.15%) were more common in AVN group. Bilateral lesions were more common in ONFH group (68.33% vs 42.31%). All of the above differences were significant (P<0.05).ConclusionOur results indicate that the long time and large cumulative dosage exposure of GCs, stable SLE state, Raynaud’s phenomenon and low Vit-D3 value are more suggestive of ONFH than AVN. ACA positive and edema as an initial symptom are just the reverse.References[1]OINUMA K, HARADA Y, NAWATA Y, et al. Abstract:
Glucocorticoid-induced osteonecrosis of the femoral head (ONFH) is a refractory disease. The treatment options for ONFH, especially nonsurgical ones, merit further investigation. To evaluate the combinatorial therapeutic effects of platelet-rich plasma clot releasate (PRCR) and umbilical cord mesenchymal stem cells (UC-MSCs) on glucocorticoid-induced ONFH, a dexamethasone (DEX)-treated cell model and a high-dose methylprednisolone (MPS)-treated rat model were established. Cell counting kit-8 (CCK-8) assay was performed in vitro to determine the optimum dosage of PRCR for UC-MSC viability. The effects of PRCR, UC-MSCs, and PRCR + UC-MSCs on cell viability, apoptosis, migration, and differentiation capacities of DEX-treated bone marrow mesenchymal stem cells (BMSCs) and human umbilical vein endothelial cell (HUVECs) were explored via Transwell assays. Western blotting was conducted to evaluate the expression levels of RUNX2, VEGF, caspase-3, and Bcl-2 in the coculture systems. Ultrasound-guided intra-articular PRCR, UC-MSCs, and PRCR + UC-MSC injections were performed on the ONFH model rats. Microcomputed tomography, histological and immunohistochemical analyses, tartrate-resistant acid phosphatase (TRAP) staining, and terminal deoxynucleotidyl transferase dUTP nick end labeling (TUNEL) staining were used to assess the therapeutic effects of PRCR and UC-MSCs on bone loss and necrosis induced by high-dose MPS. Results of this study revealed that the in vitro application of PRCR, UC-MSCs, and PRCR + UC-MSCs reversed the impaired proliferation and migration capacities and resisted apoptosis of BMSCs and HUVECs induced by DEX. Moreover, the PRCR and UC-MSC application significantly improved the alkaline phosphatase (ALP) and alizarin red (ALR) staining of BMSCs and tube formation capacity of HUVECs and promoted the protein expression of RUNX2 in BMSCs and VEGF in HUVECs. Similarly, in the ONFH rat model, the intra-articular injection of UC-MSCs and PRCR improved the subchondral bone mass parameters; promoted the expression of ALP, RUNX2, and VEGF; suppressed osteoclast overactivity; and resisted cell apoptosis. The combination of PRCR and UC-MSCs shows promising therapeutic effects in treating glucocorticoid-induced ONFH. The current study provides important information on intra-articular therapy, paving the way for the clinical management of ONFH in the future.

Abstract:
Purpose Sickle cell disease (SCD) is the most common cause of femoral head osteonecrosis (ONFH) during childhood with an overall prevalence of 10%. In children, spontaneous revascularization can occur, as in Legg-Calve-Perthes disease. Consequently, the aim of treatment is to restore proper hip containment to prevent joint arthritis. This is the first study reporting long-term results at skeletal maturity of non-operative and surgical treatments for ONFH in SCD children. Methods All children with ONFH due to SCD were retrospectively reviewed. At initial evaluation, extension of osteonecrosis was radiographically defined using Catterall, lateral pillar Herring and Ficat classifications. Subluxation of the femoral head with Reimers migration index > 30% required surgical treatment including femoral varus osteotomy and/or pelvic osteotomies. Conservative treatment including non-weight bearing and physiotherapy was performed in the remaining cases. Outcomes were assessed at skeletal maturity using the Harris Hip Score (HHS) and the Stulberg classification. Total hip arthroplasty and Stulberg 5 were defined as failures. Results A total of 25 hips in 17 patients were included (mean follow-up 7.5 years SD 3.4). Mean age at diagnosis was 11.4 years SD 2.9. In all, 15 hips (60%) were classified Catterall 3 and 4 and Herring B and C. A total of 13 patients (52%) underwent surgical treatment. At skeletal maturity, mean HHS was good (81 SD 17), 12 hips (48%) were classified Stulberg 1 and 2, seven hips (28%) were classified Stulberg 3 and 4. Conclusion Both treatments led to good functional results with 75% of congruent hips at skeletal maturity. Level of Evidence IV

Abstract:
Aims Circular RNAs (circRNAs) are a novel type of non-coding RNA that plays major roles in the development of diverse diseases including osteonecrosis of the femoral head (ONFH). Here, we explored the impact of hsa_circ_0066523 derived from forkhead box P1 (FOXP1) (also called circFOXP1) on bone mesenchymal stem cells (BMSCs), which is important for ONFH development. Methods RNA or protein expression in BMSCs was analyzed by quantitative real-time polymerase chain reaction (qRT-PCR) or western blot, respectively. Cell Counting Kit 8 (CCK8) and 5-ethynyl-2’-deoxyuridine (EdU) were used to analyze cell proliferation. Alkaline phosphatase (ALP) activity, ALP staining, and Alizarin Red S staining were employed to evaluate the osteoblastic differentiation. Chromatin immunoprecipitation (ChIP), luciferase reporter, RNA pull down, and RNA immunoprecipitation (RIP) assays were combined for exploring molecular associations. Results Circ_0066523 was upregulated in osteogenic induction process of BMSCs. Silencing circ_0066523 restrained the proliferation and osteogenic differentiation of BMSCs. Mechanistically, circ_0066523 activated phosphatidylinositol-4,5-bisphosphate 3-kinase / AKT serine/threonine kinase 1 (PI3K/AKT) pathway via recruiting lysine demethylase 5B (KDM5B) to epigenetically repress the transcription of phosphatase and tensin homolog (PTEN). Functionally, AKT signalling pathway agonist or PTEN knockdown counteracted the effects of silenced circ_0066523 on BMSC proliferation and differentiation. Conclusion Circ_0066523 promotes the proliferation and differentiation of BMSCs by epigenetically repressing PTEN and therefore activating AKT pathway. This finding might open new avenues for the identification of therapeutic targets for osteoblast differentiation related diseases such as ONFH. Cite this article: Bone Joint Res 2021;10(8):526–535.

Abstract:
Our retrospective study assessed the effects of treatment of early stage ONFH with extracorporeal shock wave therapy. 335 patients (528 hips) were treated with shockwave therapy in our institution. Each patient underwent two sessions. The hips were divided into two groups according to whether the lateral pillar of the femoral head (LPFH) was preserved: LPFH and non-LPFH groups. Patients were followed up at 3, 6, and 12 months after the treatment. Most of the patients (83.9% hips) demonstrated pain reduction and improved mobility of the treated joint (visual analogue scale score,P=0.00006; Harris hip score,P=0.00091). During the follow-up period, 16 hips failed following femoral head collapse and required hip arthroplasty (2 hips in LPFH group and 14 hips in non-LPFH group). The lesion size decreased after ESWT. However, the differences were statistically not significant (LPFH group,P=0.091; non-LPFH group,P=0.087). A significant reduction in bone marrow edema was observed after treatment (LPFH group,P=0.007; non-LPFH group,P=0.016). High-energy extracorporeal shock wave therapy resulted in considerable improvement in early stage ONFH, which can effectively relieve pain and improve the function of the hip.

Abstract:
BackgroundOsteonecrosis of the femoral head (ONF) occurs frequently (3-40%) in patients who are treated with corticosteroids for SLE. MRI can correctly visualize pathological abnormalities of the femoral head, and it is much more sensitive than plain radiography. To analyze the risk factors associated with steroid-induced ONF, it seems important to detect early changes in the femoral head by MRI and early clinical events caused by steroid therapy. For treatment of SLE, several strategies have been selected according to the clinical conditions of patients, and there are slight differences between multicenter hospitals. In this study, the treatment performed at two hospitals, and the treatment strategy, selection of steroid, initial dose of steroid and drugs used together with steroid have been quite similar. Therefore, it should be possible to clarify the background factors associated with ONF.ObjectivesThe purpose of this study were to clarify the factors related to silent ONF in patients with SLE and, in case of ONF, to clarify the clinical characteristics between unilateral and bilateral.MethodsOne hundred thirty-two patients (18 males and 114 females) with SLE were selected on the basis of having been newly diagnosed and requiring high-dose prednisolone, including pulse therapy with methylprednisolone, as the initial treatment. All the patients initially underwent plain radiography and MRI at the start of corticosteroids to detect any early changes in the femoral head. Subsequently these examinations were performed three months thereafter. The laboratory parameters were evaluated at the beginning of steroid treatment and at one month thereafter. All statistical analyses were performed with SPSS v. 13 (SPSS Inc., Chicago, IL, USA). Statistical tests were considered significant at aP(two sided) < 0.05, and marginal significant atP= 0.05–0.10. Tests were 2-tailed, and differences at p < 0.05 were considered significant.ResultsBy three months after the start of corticosteroid treatment, asymptomatic ONF was diagnosed by MRI in 33 patients (25.0%), being bilateral in 21 patients and unilateral in 12. Serological activity (C3, C4, CH50 and anti-ds DNA antibody), renal function (eGFR, serum creatinine and urinary protein), anti-phospholipid antibodies, and SLEDAI were not correlated with asymptomatic ONF. BMI, BSA, and the initial dose of prednisolone per unit body weight, BMI and BSA were not correlated with silent ONF. Additionally, the occurrence of ION was not related to SLEDAI. However, patients with angiitis tended to increase the incidence of ONF (p=0.069). Patients with a higher triglyceride level both of before (p=0.002) and 4 weeks after the start of steroid treatment (p=0.004) and total cholesterol level 4 weeks after the start of steroid treatment (p=0.036) showed a significantly higher in patients with asymptomatic ONF. In patients with ONF, the level of creatinine clearance was tended to lower (p=0.062). In unilateral and bilateral studies, both anti-ds DNA antibody and C3 were not correlated. However, despite no difference in initial steroid dosage, CH50 was significantly lower and C4 was tended to lower in patients with unilateral ONF.ConclusionAsymptomatic ONF is common in patients with SLE. Both of high triglyceride and total cholesterol levels are important risk factors for ONF. In unilateral and bilateral studies, excess steroid dosage relative to serologic activity of SLE might contribute to the development of bilateral ONF. In the pathogenesis ONF, it was suggested that SLE activity and steroids are closely related.Reference[1]Kuroda T, et al. Abstract:
Background: This study investigated the effect of using adenovirus-mediated expression of bone morphogenetic protein 2 (Ad-BMP-2) and basic fibroblast growth factor (bFGF) in bone marrow mesenchymal stem cells (BMSCs) in combination with a demineralized bone matrix (DBM) to repair osteonecrosis of the femoral head (ONFH) in Beagle dogs. Methods: A total of 30 Beagle dogs were selected for the isolation of BMSCs, which were cultured and transfected with the recombinant adenovirus vector Ad-BMP2-bFGF-GFP (carrying BMP-2 and bFGF) or a control adenovirus plasmid (encoding green fluorescent protein (Ad-GFP)). The expression of the transfected BMP-2 and bFGF proteins was detected by Western blotting. After transfection, the BMSCs were induced to undergo osteoblastic differentiation. The DBM was prepared to construct a DBM/BMSC complex. Beagle models of canine femoral head defects and necrosis were established and divided into control, DBM, DBM/BMSC, vector Ad-BMP2-bFGF-GFP and Ad-GFP groups. The composite graft was then implanted, and new bone morphology was visualized via X-ray at 3, 6 and 12 weeks after the operation. Hematoxylin and eosin (HE) staining and Masson’s trichrome staining were used to identify new bone formation. Immunohistochemistry was performed to calculate the density of new blood vessels. The compressive and bending strength of the BMSCs was evaluated at 12 weeks after the operation. Results: BMSCs were successfully isolated. The protein expression of BMP-2 and bFGF was significantly higher in the Ad-BMP-2/bFGF group than the normal and Ad-GFP groups. Compared with the control group, at 12 weeks after the operation, the DBM, DBM/BMSC, vector Ad-BMP2-bFGF-GFP and Ad-GFP groups showed a larger area of new bone, higher X-ray scores, greater neovascularization density, and increased compressive and bending strength. The most significant modifications occurred in thevector Ad-BMP2-bFGF-GFP group. Conclusion: The results indicate that the use of Ad-BMP-2/bFGF-modified BMSCs in conjunction with DBM could successfully repair ONFH in a dog model by promoting bone formation and angiogenesis.

Abstract:
Purpose: To evaluate the clinical and radiographic mid-term results of short-stem total hip arthroplasty (THA) in patients with osteonecrosis of the femoral head (ONFH). Methods: We reviewed 83 cases that underwent Metha stem between November 2010 and November 2012. The appearance of bone trabeculae development and radiolucent line were reviewed and HHS was recorded at 6 months postoperatively then yearly to evaluate the clinical results. Results: The mean age of patients was 43.8 years with the mean follow-up 69.3 (60–84) months. Harris Hip Score improved significantly from 44.7 preoperatively to 99.6 at the last follow-up ( p < 0.0001). There were 4 cases (4.8%) of intraoperative femoral fractures. There was 1 case (1.2%) of distal stem perforation that had stable bone ingrowth and there was 1 case (1.2%) of 5-mm subsidence, which was then stable at 3 months postoperatively. Bone trabecular development was detected at zone 1 (65.1%), 2 (69.9%), 3 (14.4%), 4 (1.2%), 6 (97.6%) and 7 (81.9%). There was 1 case (1.2%) where radiolucent lines were observed in zones 1 and 7. There was 1 femoral stem revision from periprosthetic fracture 4 years after index surgery and 1 cup revision from aseptic loosening. Kaplan-Meier survivorship with the end point of stem revision for any reason was 98.8% and for aseptic loosening it was 100% at 7 years. Conclusions: The mid-term clinical and radiographic results of the Metha stem in patients with ONFH were promising. Its design enables preservation of the bone stock and the bone trabeculae appear to confirm the assumption of proximal force transmission.

Abstract:
(1) Background: Osteonecrosis of the femoral head (ONFH) is characterized by impaired vascularization with ischemia resulting in bone cell death, leading to the deterioration of the hip joint. Mesenchymal stem/stromal cells (MSCs) are an attractive potential therapeutic approach in this setting. The aim of this study is to evaluate the clinical improvement in terms of pain and quality of life, as well as the safety of the procedure during the follow-up of patients. (2) Methods: A Phase I–II Open-Label Non-Randomized Prospective clinical trial was conducted. Eight patients with idiopathic ONFH and stage < IIC in the ARCO classification were included. Four weeks before therapy, 40 mL of autologous bone marrow was obtained, and MSCs were expanded under Good-Manufacturing-Practice (GMP) standards. Study medication consisted of a suspension of autologous BM-derived MSCs (suspended in a solution of 5–10 mL of saline and 5% human albumin) in a single dose of 0.5–1 × 106 cells/kg of the patient, administered intraosseously with a trocar and under radioscopic control. Per-protocol monitoring of patients included a postoperative period of 12 months, with a clinical and radiological assessment that included the visual analog scale (VAS), the Harris scale, the SF-36, and the radiological evolution of both hips. In addition, all patients were further followed up for eight years to assess the need for long-term total hip replacement (THR) surgery. (3) Results: Median age of patients included was 48.38 ± 7.38 years, and all patients were men. Autologous MSCs were expanded in all cases. There were no adverse effects related to cell administration. Regarding efficacy, both VAS and ODI scores improved after surgery. Radiologically, 12.5% of patients improved at the end of follow-up, whereas 50% improved clinically. No adverse effects related to the procedure were recorded, and none of the patients needed THR surgery within the first year after MSC therapy. (4) Conclusions: The use of autologous MSCs for patients with ONFH disease is feasible, safe in the long term, and potentially effective.

Abstract:
ABSTRACTFusarium langsethiaeis a fungal pathogen of cereal crops that is an increasing problem in northern Europe, but much of its epidemiology is poorly understood. The species produces the mycotoxins T-2 and HT-2, which are highly toxic. It was hypothesized that grain aphids,Sitobion avenae, may transmitF. langsethiaeinoculum between wheat plants, and a series of transmission experiments and volatile chemical analyses was performed to test this. Manual translocation of aphids from inoculated to uninfected hosts resulted in pathogen DNA accumulation in hosts. However, the free movement of wingless aphids from infected to healthy plants did not. The addition of winged aphids reared onF. langsethiae-inoculated wheat seedlings to wheat plants also did not achieve successful pathogen transfer. While our data suggested that aphid transmission of the pathogen was not very efficient, we observed an increase in disease when aphids were present. After seedling inoculation, an increase in pathogen DNA accumulation in seedling leaves was observed upon treatment with aphids. Furthermore, the presence of aphids on wheat plants withF. langsethiae-inoculated ears not only led to a rise in the amount ofF. langsethiaeDNA in infected grain but also to an increase in the concentrations of T-2 and HT-2 toxins, with more than 3-fold higher toxin levels than diseased plants without aphids. This work highlights that aphids increase the susceptibility of wheat host plants toF. langsethiaeand that aphid infestation is a risk factor for accumulating increased levels of T-2 and HT-2 in wheat products.IMPORTANCEFusarium langsethiaeis shown here to cause increased contamination levels of grain with toxins produced by fungus when aphids share the host plant. This effect has also recently been demonstrated withFusarium graminearum, yet the two fungal species show stark differences in their effect on aphid populations. In both cases, aphids improve the ability of the pathogens to cause and initiateFusariumhead blight (FHB) disease in wheat, butF. langsethiaemay be able to act as a dispersal agent.F. langsethiaecontributes harmful toxins to wheat grain that need to be controlled, but as yet, its epidemiology is unresolved. This work reveals insights into the role aphids play in promoting the successful colonization of this species in wheat and the benefit of controlling aphid populations on crops that are at high risk of FHB.

Abstract:

Abstract:
Wpływy rosyjskie w antroponimii mieszkańców dziewiętnastowiecznego Piotrkowa Trybunalskiego analizowano w trzech płaszczyznach, tj. 1) obraz społeczny ludności rosyjskiej w XIX wieku, wynikający z zapisów w księgach meldunkowych i metrykalnych Piotrkowa; 2) charakterystyka morfologiczna nazwisk należących do przedstawicieli społeczności rosyjskiej; 3) adaptacja fonetyczna, graficzna i morfologiczna antroponimów rosyjskich do polskiego systemu językowego.

Abstract:
Subject. Some features of the clinical picture, hygienic and microbiological state of the oral cavity, acidity and free-radical oxidation of the oral fluid in patients with recurrent aphthates on the background of acid-dependent diseases of the gastrointestinal tract are considered. The aim is to assess the relationship between clinical and laboratory parameters in patients with recurrent oral aphthae associated with acid-dependent diseases of the gastrointestinal tract. Methodology. A comprehensive clinical and laboratory examination of 58 young people with revealed recurrent aphthae of the oral cavity against the background of acid-dependent diseases of the gastrointestinal tract was carried out. The level of oral hygiene was assessed using the ONI-s index, the state of free-radical oxidation of the oral fluid - the "Chemilumimer-003" device, individual representatives of the normal flora were identified by inoculating the contents from the surface of aphthous elements on special differential diagnostic media of the HiCrome series. The acidity of the oral fluid was determined using a universal indicator litmus paper pH-1-14, short-term intragastric pHmetry was performed directly during fibrogastroduodenoscopy. Results. In 46.4 % of those examined, recurrent aphthae were found on the oral mucosa. In persons with chronic gastritis and duodenitis, the frequency of recurrences once a year was 37.9 %, 2-3 times a year ― 62.0, with the presence of chronic pancreatitis against the background of hyperacid gastric secretion ― 44.8 and 55.2, respectively. In patients with recurrent aphthae, a low level of oral hygiene, a violation of its microbiome, and a change in the acidity of the oral fluid were revealed. Conclusions. The presence of recurrent aphthae complicates individual oral hygiene, because accompanied by severe pain and paresthetic symptoms. This is manifested by an increase in the ONI-S index and a disturbance in the state of the normobiome in the form of an increase in the growth of resident microflora against the background of changes in the acidity of the oral fluid. Chemiluminescence indices correlate with the hygienic state of the oral cavity, qualitative changes in the normobiota and pH of the oral fluid in the studied clinical groups.

Abstract:

Abstract:
ABSTRACT Background Athletic patients with osteonecrosis of the femoral head have few desirable therapeutic options that preserve athletic ability. A total hip arthroplasty for this young and active patient population would be catastrophic for patients’ chances of returning to vigorous athletic endeavors. Therefore, other therapeutic alternatives must be considered prior to enlisting this patient population for a total hip arthroplasty. Materials and methods We retrospectively evaluated 15 patients (19 hips) who presented at an average age of 28.5 years (12-46) and stages 2 (6/19), 3 (2/19), 4 (9/19) and 5 (2/19) of osteonecrosis of the femoral head (ONFG). All patients were participating in strenuous athletic activity and were treated with free vascularized fibular autografting (FVFG) to the femoral head. Results Postoperative evaluation of pain symptoms and functional activity showed improvements in all patients. The average follow-up time was 8 years. Harris hip scores significantly increased from an average preoperative score of 72.5 to an average postoperative score of 94. Seventy-five percent of patients were able to return to their sport after recovery and all patients reported being satisfied with the procedure and would repeat their decision to have surgery. Three patients’ hips were converted to arthroplasty at 3, 11 and 17 years post-FVFG. Conclusion Our results demonstrate FVFG as a successful treatment in athletes with osteonecrosis of the femoral head. This procedure has the potential to reduce pain, increase activity and allow most to return to their sport, an achievement often not possible with other treatment options. Abstract:

Abstract:

Abstract:
Tiu dubsenceco povas cefe kasi kaj do pliigi maljustecon. La teorio de lingva malegaleco praecas, do "malegalaj lingvokapabloj," "lingva potenco," "lingva seksismo," "lingva diskriminado" kaj similaj konceptoj malklaras. Diversaj malegalec-mezuroj male decidas ĉu, ekzemple, Hindujo ĉu Sovetunio lingve pli malegalas. Jursciencaj argumentoj pri lingvaj rajtoj, ekz. de Kloss kaj Van Dyke, malprave supozas ke endas kaj do justas trakti lingvojn malegale. Pli inda analizo agnoskas triopon de "lingvaj egalecoj": (1) same trakti lingvojn, (2) egale trakti lingvojn, (3) egale trakti lingvanojn.

Abstract:
In 2012 and 2013, was studied the occurrence, biology and harmfulness ofByctiscus betulae(L.) (Rhynchitidae) on forest woody plants in the Brno region. Leaf rolls were found on 13 species (and 10 genera) of woody plants. Most frequently, they occurred onFagus sylvatica,Tiliaspp. (namely onT. cordata),Salix capreaandPopulus tremula. The beetles hibernate in ground pupal chambers and appear on woody plants in the last decade of April. After hibernation, females live on average seven (males five) weeks, and damage on average 40 (males 25) cm2of leaves. The females create leaf rolls on annual shoots they have gnawed, exceptionally (in 2%) on leaves with damaged petioles. In the rolls onF. sylvatica, they roll on average 5.6 leaves whose total average area is 49.5 cm2and lay on average 5.1 eggs. In the rolls onTiliaspp., they roll on average 3.3 leaves whose total area is 63.2 cm2and lay on average 4.9 eggs. In the rolls onS. caprea, they roll on average 3.3 leaves whose total area is 38.3 cm2and lay on average 3.5 eggs. The maximum number of laid eggs (on average 8.0) was observed in the leaf rolls onVitis vinifera. The average number of eggs in the leaf rolls was decreasing from May to July. The number of eggs was increasing with the increasing total leaf area rolled. The females lay on average 50 eggs. OnP. tremula,S. capreaandPyrus pyrasterthey create about 15 leaf rolls, onF. sylvaticaandTiliaspp. about 10 leaf rolls and onV. viniferaca. 8 leaf rolls. Embryogenesis lasts from 8 to 9 days. Larvae develop over 3 instars and damage ca. 300 mm2of leaves. Coming of age within 4−7 weeks, they leave the leaf rolls from mid-June to the beginning of September.

Abstract:
As more and more religious content appears in anime, the actual meaning of its use in anime may be different. This phenomenon is known as religious cosmetics introduced by Jolyon Baraka Thomas. Religious cosmetics introduces the term religious vocabulary and religious imagery. This study focuses on analyzing religious vocabulary, and aims to prove whether the Shinto religious vocabularies that appears in the anime Enen no Shouboutai season 1 (2019) are religious cosmetics. The method used in this study is qualitative descriptive. The use of words in the anime Enen no Shouboutaiseason 1 will be compared with the meaning of the vocabulary from the Shinto religious dictionary compiled by Brian Bocking. The results showed that there are eighteen data of religious vocabularies that appear in anime Enen no Shouboutai season 1, which can be divided into five categories, namely (1) ‘amaterasu’, (2) ‘kami’, (3) ‘matsuri’, (4) ‘oni’, and (5) ‘sake’. However, among those five categories appeared throughout the series, four were found as Shinto religious vocabularies which used as religious cosmetics while one category (3) ‘matsuri’ was not used as religious cosmetics. The reason is because the meaning and the use of the word is getting wider, not only used in Shinto related topics, but also in daily life generally.

Abstract:
Varying abundance and dispersal of the two-spotted spider mite (Tetranychus urticaeKoch, 1836, Acari: Prostigmata: Tetranychidae) onMi-tomato plants differing in allelic combinationIt is well known that the presence of theMi-1.2gene determines the resistance of tomato plants (Solanum lycopersicumL.) to 3 root-knot nematode species and some insect herbivores of the order Hemiptera. In this study, the density and dispersal of two-spotted spider mites (Tetranychus urticae) on tomato plants of 4 cultivars were evaluated under greenhouse conditions, to find out whether tomatoes differing in allelic combination of theMi-1.2gene exhibit similar or different resistance to the mite pest. The results show, for the first time, that the same initial number of mites develops into populations that vary in abundance and distribution depending on tomato allelic composition of theMi-1.2gene. The results indicate that the mite-pest develops more slowly on tomato plants of cultivar ‘Motelle’, carrying 2 dominant alleles for theMi-1.2gene, than on heterozygous ‘Altess F1’ tomatoes and both cultivars carrying only recessive alleles of this gene: ‘Moneymaker’ and ‘Plaisence F1’. This suggests that theMi-1.2gene may be involved in tomato resistance against spider mites when this dominant gene is expressed homozygously, but this needs to be verified by further research.

Abstract:
Każda z uczelni nieco inaczej definiuje cele kształcenia swoich absolwentów: jednym zależy, aby posiadali oni wiedzę i umiejętności potrzebne do pomiaru przestrzeni i zjawisk, innym – aby zdobyte dane przestrzenne przetwarzali metodami informatycznymi, a jeszcze innym – aby stosowali geoinformację i geoinformatykę do poznawania mechanizmów funkcjonowania środowiska i społeczeństwa.

Abstract:
TPS780 Background: Patients with pancreatic cancer have the highest rate of weight loss among all advanced cancers. Of which, the majority develop cachexia, characterized by progressive and involuntary loss of weight and skeletal muscle mass. In preclinical studies, interleukin-1-alpha (IL-1α) antagonism has been found to neutralize tumor angiogenesis and onco-inflammation. Early phase single-agent studies in cancer cachexia have demonstrated increased lean body mass and decreased fatigue, pain, and appetite loss. The present study aims to establish the safety of an IL-1α antagonist (Bermekimab) in combination with nanoliposomal irinotecan (Nal-Iri) and 5-fluorouracil (5FU)/folinic acid (FA) in patients with advanced pancreatic adenocarcinoma and cachexia who have failed gemcitabine-based chemotherapy. Methods: This is a single arm, single center, phase I study. The primary objective is to assess the maximum tolerated dose (MTD) of Bermekimab in combination with Nal-Iri and 5FU/FA. MTD is defined as the dose such that the probability of dose-limiting toxicities at MTD is θ = 0.33. The first cohort of up to 3 patients will receive 7.5 mg/kg Bermekimab, 50 mg/m2 nanoliposomal irinotecan, 2000 mg/m2 5FU, and 400 mg/m2 FA and the subsequent doses will be determined by the escalation with overdose control (EWOC) algorithm. Treatment is administered on days 1 and 15 of each 28-day cycle. Key inclusion criteria include: advanced or locally advanced pancreatic adenocarcinoma that has progressed through or intolerant of gemcitabine-based chemotherapy, cachexia defined as > 5% unexplained weight loss 6 months prior to screening or as documented by physician, ECOG PS 0-2 or KPS ≥ 60%, and normal organ and marrow function. Secondary objectives are to assess weight, lean body mass, inflammatory cytokines, overall survival, progression free survival, patient quality of life, and functional status. Since January 2019, 23 patients have been screened and 21 enrolled. Clinical trial information: NCT03207724.

Abstract:
The local expansion method is a novel and promising community detection algorithm. Just based on part of network information, it can detect overlapping communities effectively, but some problems exist such as seed node aggregation, poor quality and inaccurate community coverage. Therefore, we propose a local expansion overlapping community detection algorithm based on dispersed seeds. There are four essential parts of this algorithm: 1) We firstly generate non-overlapping partitions of the network, and locate seed nodes with the largest influence in their own partition by using a new index of node influence, which combines the information centrality of nodes and the number of k-order neighbors. 2) Secondly, on the condition of the neighborhood overlap measure maximization, seed nodes merge unseeded nodes to generate a preliminary seed community; 3) Then based on the community conductance gain, the allocated nodes are screened and the free nodes are assigned to the seed community; 4) In the end, a node-community similarity based on common connection edge is proposed to re-allocate new free nodes and obtain the final community structure. This method can make the community distribution more proper and the coverage more reasonable. The experimental results on some artificial data and real network data show that the algorithm performs well on overlapping community indicators such as EQ and ONMI, while the community detection results are more stable.

Abstract:
Politička reorganizacija država i javnih uprava, 2. stalno obnavljanje demokratskog zahtjeva i 3. nastupanje menadžerske paradigme.

Abstract:
1. The responses of units in the ventral cochlear nucleus in anesthetized guinea pigs have been measured to best-frequency tones, noise bands geometrically centered around the unit best frequency, and noise bands asymmetrically positioned around the best frequency. 2. Each unit isolated was characterized using peristimulus time histograms (PSTHs) to best-frequency tones at 20 and 50 dB suprathreshold, frequency-intensity response areas and rate-versus-level functions in response to best-frequency tones and wideband noise. The data reported here are derived from full analyses of 5 chopper units and 17 onset units. The onsets were divided into onset-I (OnI), onset-L (OnL), and onset-C (OnC) by the criteria described by Winter and Palmer: the PSTHs of OnI units show only an onset response, OnL units respond with a single spike at onset followed by a low level of sustained activity, and OnC units have PSTHs with one to four onset peaks and low levels of sustained discharge. 3. In response to geometrically centered noise bands of constant spectral density, the discharge of chopper units and one OnI unit increased over a relatively narrow range of bandwidths, corresponding to the equivalent rectangular bandwidth calculated from their response area, and then became constant. In contrast, OnL and OnC units showed increases in discharge rate with noise bandwidth over very wide ranges of bandwidth. The growth of the discharge rate with noise bandwidth was approximately linear on double logarithmic axes and therefore could be described by a power function with an exponent of 0.37. This relation held even for noise levels near threshold. 4. When noise bands with constant spectral density (at the input to the earphone) were presented with one edge fixed at the unit's best frequency, the discharge rate of most chopper units and the one OnI unit increased over a narrow range of bandwidths and then became constant. This pattern was observed irrespective of whether the second edge of the noise was progressively increased above, or decreased below, the best frequency. For two of the chopper units, in which lateral inhibitory sidebands could be demonstrated, increasing the noise bandwidth led first to increases and then to decreases in the discharge rate as the noise energy impinged upon the sideband. The chopper units act like energy detectors with a filter corresponding to their single tone response area, but, for some units, with the addition of inhibitory sidebands. 5. For the OnL and OnC units, increasing the noise bandwidth above or below best frequency caused progressive increases in the discharge rate over wide ranges of bandwidth. These increases occurred even for low noise spectral densities. The growth in discharge rate for these onset units was well fitted at all spectral density levels by power functions: one above best frequency and one below. At levels of the noise 40 dB above the unit threshold, the point at which the discharge rate reached 90% of its maximum was, on average, about 2 octaves below best frequency and 1 octave above. For some onset units, changes in the discharge rate were seen as the noise bandwidth was varied over about 14 kHz, which is about one-third of the total frequency hearing range of the guinea pig. 6. The data for onset units is consistent with the hypothesis that onset units in the ventral cochlear nucleus achieve their precision in the temporal domain by integration of the inputs from auditory nerve fibers with a wide range of best frequencies. The range of frequency over which onset units integrate frequency matches that of the inhibitory input to dorsal cochlear nucleus neurons, suggesting a possible role as an inhibitory interneuron.

Abstract:
Background:Osteonecrosis of the femoral head (ONFH) is a common disease caused by many trauma factors and un-trauma factors. Among those un-trauma factors, steroid-induced osteonecrosis of the femoral head (SNFH) accounted for a large proportion and mainly concentrated in young people. SNFH has been reported as an irreversible disease and associated with the damage of blood vessels and the loss balance of bone homeostasis. Circulating endothelial progenitor cells (CEPCs), one part of circulating endothelial cells (CECs), are immature precursor cells with proliferative potential. The damage of vascular endothelial cells in SNFH has been confirmed by many studies, but the changes of CECs and CEPCs in the peripheral blood of patients with SNFH have not been studied yet. Objectives:The objective of the study is to explore the number of CECs and CEPCs in SNFH patients and normal people and then investigate whether EC-secreted exosomes (EC-exos) could prevent the progression of SNFH in rat model and its mechanism of action. Methods:We collect peripheral blood of 3 SNFH patients and 3 heathy people and detected the levels of CECs and CEPCs by Flow cytometer. TEM, NTA and western blot was used to characterize the isolated EC-exos. Annexin V-FITC/PI double staining with flow cytometric analysis and western blot were used to evaluate MC3T3-E1 cells apoptosis. CCK-8, scratching experiment and transwell were used to evaluate MC3T3-E1 cells viability and migration ability. Micro-CT and morphological staining were used to evaluate the progress of SNFH in rat model. Results:Firstly, we found that the number of CECs and CEPCs in the peripheral blood was decreased in SNFH patients than normal people. Then our results indicated that EC-exos could improve the migration, viability and prevent apoptosis of osteoblasts under dexamethasone by activating the PI3K/AKT/Bcl-2 signal pathway in vitro. Finally, our Micro-CT and morphological staining results in SNFH rat model revealed that EC-exos prevented the progression of SNFH.Conclusion:EC-exos could enhance the cell viability and migration ability of osteoblasts under dexamethasone and play an anti-apoptosis role against steroids-induced osteoblast apoptosis by activating the PI3K/AKT/Bcl-2 signal pathway. EC-exos prevented the progression of SNFH in rat model.References:[1]Zalavras CG, Lieberman JR. Osteonecrosis of the femoral head: evaluation and treatment. Abstract:

Abstract:
1. The responses of onset units in the cochlear nucleus of the anesthetized guinea pig have been measured to single tones, two-tone complexes, and broadband noise (BBN; 20-kHz bandwidth). The onset units were subdivided into three groups, onset-I (OnI), onset-L (OnL), and onset-C (OnC), on the basis of a decision tree using their peristimulus time histogram (PSTH) shape and discharge rate in response to suprathreshold best-frequency (BF) tone bursts. 2. PSTHs were constructed from responses either to single tones at a unit's BF or to BBN as a function of level. When sufficient sustained activity could be elicited from the unit, arbitrarily defined as > 100 spikes/s, a coefficient of variation (CV) was calculated; the majority were characterized by a CV that was similar to transient chopper units (0.35 < CV < 0.5). First spike latency decreased monotonically with increasing sound level. For the majority of onset units, the first spike timing was very precise. 3. BF rate-level functions recorded from OnL and OnC units did not show any signs of discharge rate saturation at the highest sound levels we have used (100-115 dB SPL). No systematic relationship was observed between the threshold at BF and the shape of the rate-level function. BBN rate-level functions were typically characterized by higher discharge rates than in response to BF tones. However, for OnI units and a minority of other onset units, there was little difference in the shape of their rate-level functions in response to BF tones or BBN. 4. The threshold of most onset units to BBN was similar to the threshold to a BF tone that had similar overall root-mean-square (RMS) energy. The BBN threshold was, on average, 5.5 dB greater than the BF threshold. This result contrasts with that found in auditory-nerve fibers recorded in the same species, with the use of an identical sound system, where the threshold to BBN was, on average, 19.4 dB higher. The mean threshold difference between BBN and BF tones for a population of chopper units recorded in the same series of experiments was 17.7 dB. The relative thresholds to BBN and BF tones indicated that the bandwidths near the onset units' BF threshold were broader than could be estimated with the use of single tones. Ten units were characterized by bimodal response areas.(ABSTRACT TRUNCATED AT 400 WORDS)
