Abstract:
Damping materials are used extensively for reduction of vibration and noise. These damping materials have viscoelastic characteristics and are used by automotive and other industries. Testing of these materials is important in order to predict their performance and traditionally the damping properties are measured by the Oberst method. This paper discusses an alternate method called the Center Impedance method where force and response are measured directly and the damping properties are obtained. The Center Impedance method is easy to use requiring only standard vibration equipment for excitation, namely, shaker, and is easy to control the experiment for repeatability. Results of beams tested by both Oberst and Center Impedance methods are presented in order to validate this test method.

AbstractBackground:Caregivers are often unprepared and overwhelmed with the responsibilities of providing care to stroke survivors, which can lead to negative physical and psychological effects.Purpose:To evaluate the impact of the Family Informal Caregiver Stroke Self-Management (FICSS) program on burden and life changes resulting from providing care among family caregivers of stroke survivors.Methods:A prospective pre-test and post-test design using quantitative and qualitative data was used to evaluate the program with a convenience sample of 42 caregivers. The four-module facilitated program consisted of small group-guided discussion. Quantitative evaluations were completed at baseline, 2 weeks and 6 months (post-intervention), and qualitative data were collected at 2 weeks and 6 months. Life changes and burden were measured using the Bakas Caregiving Outcome Scale (BCOS) and the Oberst Caregiving Burden Scale (OCBS), respectively.Results:The BCOS scores increased consistently over time, showing significant differences at 6 months compared with 2 weeks (mean difference: 5.29, 95% confidence interval [CI]: 0.30-10.28,p=0.04) and baseline (mean difference: 7.58, 95% CI: 2.92-12.23,p=0.001). The OCBS time scores decreased consistently over time, showing a significant difference at 6 months compared with baseline (mean difference: −5.20, 95% CI: −0.96 to −9.44,p=0.02). The OCBS difficulty scores fluctuated over time, resulting in no overall difference from baseline to 6 months. Qualitative themes were consistent with the positive quantitative findings.Conclusion:Study results suggest that the FICSS program may result in reduced caregiver burden and improved life changes resulting from providing care.

Abstract:
— Background: Osteomyelitis is an infectious-inflammatory destructive bone disease caused by nonspecific and specific microflora, or microbial communities. Diabetes mellitus is the most common pathology associated with osteomyelitis and may be responsible for exacerbation of chronic osteomyelitis. Objective: to demonstrate a new patented technique for surgical treatment of osteomyelitis of distal phalanx (I finger) on the example of a clinical case. Methods: A female patient P. 34 years old, who has type 2 diabetes mellitus, decompensated. Daily glucosometry shows changes in capillary blood glucose from 7 to 25 mmol/L during the last month; the last blood test for HbA1c — 10.4%, it should be noted that the patient adheres to the diet prescribed by the endocrinologist and receives drug therapy according to treatment standards, however, attempts to compensate the disease have been failed. The patient suffers from obesity of 1 degree (BMI = 30.11). Results: Operation: after preparation of the surgical site with Betadin’s solution, local anesthesia according to Oberst-Lukashevich (injection of 3 ml 2% Lidocaine solution on each side into the lateral surfaces of the I finger of the right hand from the place of harness application, proximal to the intervention area) was provided. Extraction of the nail plate with dissection of soft tissues along the phalanx and along the projection of the nail bed and rotation of tissues downwards to expose the bone destruction focus. Then necrectomy of the damaged bone was performed followed by sanitization and returning of soft tissues to their place without suturing. Conclusion: The given example of surgical treatment of finger osteomyelitis has undeniable advantages due to a number of reasons: elimination of traumatization of finger’s working surface, achievement of cosmetic effect, preservation of aesthetic and function of hand, as well as all types of sensitivity of finger skin. It is worth noting that this method of surgical treatment of osteomyelitis has more favorable course of the postoperative period due to a shorter duration of pain syndrome and the general reaction of the body.

Der zum Zwecke der Personalauswahl entwickelte Analytische Test (AT) dient der Erfassung der Fähigkeit zum induktiven und deduktiven schlussfolgernden Denken bei Bewerbern im oberen Fähigkeitsbereich. Zur Erhöhung der Akzeptanz und zur Verbesserung der augenscheinlichen Validität des Verfahrens sind die Aufgaben des AT semantisch in den Kontext beruflicher Situationen eingekleidet. In sechs Validierungsstudien erwies sich der AT als hoch auswertungsobjektiv und hinreichend schwierig, um auch noch im oberen Fähigkeitsbereich zu differenzieren. Die interne Konsistenz und Retestreliabilität waren ebenso zufriedenstellend wie die konvergente und die diskriminante Validität in Bezug auf Schulnoten, verschiedene Intelligenztests und die Big Five. Berichtet werden auch empirische Belege für die Konstruktvalidität, Akzeptanz und hohe g-Sättigung des AT.

Abstract:
Using the MTS testing machine, the uniaxial compressive test of varisized da-qing limestones were undertaken, and the effect of dimensions about compressive strength, peak strain, elastic modulus and destructional forms of rock specimens were studied. It demonstrates that along with the increase of length-diameter ratio, peak strain and compressive strength turn smaller, elastic modulus gradually increases, the destruction of rock samples have a transformation from splitting failure to shear failure. Combined with the test results, Obert L model and Yang Shengqi model, the two size-effect models with extensive applications are analyzed and contrasted. And the conclusion is drawn that Obert L model has a relatively broad applicability, while Yang Shengqi model has a stronger Targeting and higher accuracy. Thus based on the Yang Shengqi model, the size-effect model of da-qing limestone is put forward, and the result indicates that this model corresponds well with the test results, having certain practical value.

Exposure to chemical substances that can produce endocrine disrupting effects represents one of the most critical public health threats nowadays. In line with the regulatory framework implemented within the European Union (EU) to reduce the levels of endocrine disruptors (EDs) for consumers, new and effective methods for ED testing are needed. The OBERON project will build an integrated testing strategy (ITS) to detect ED-related metabolic disorders by developing, improving and validating a battery of test systems. It will be based on the concept of an integrated approach for testing and assessment (IATA). OBERON will combine (1) experimental methods (in vitro, e.g., using 2D and 3D human-derived cells and tissues, and in vivo, i.e., using zebrafish at different stages), (2) high throughput omics technologies, (3) epidemiology and human biomonitoring studies and (4) advanced computational models (in silico and systems biology) on functional endpoints related to metabolism. Such interdisciplinary framework will help in deciphering EDs based on a mechanistic understanding of toxicity by providing and making available more effective alternative test methods relevant for human health that are in line with regulatory needs. Data generated in OBERON will also allow the development of novel adverse outcome pathways (AOPs). The assays will be pre-validated in order to select the test systems that will show acceptable performance in terms of relevance for the second step of the validation process, i.e., the inter-laboratory validation as ring tests. Therefore, the aim of the OBERON project is to support the organization for economic co-operation and development (OECD) conceptual framework for testing and assessment of single and/or mixture of EDs by developing specific assays not covered by the current tests, and to propose an IATA for ED-related metabolic disorder detection, which will be submitted to the Joint Research Center (JRC) and OECD community.

In 1927 Nikolai Zabolotskii first began making public appearances with the young Leningrad poets Daniil Kharms and Aleksandr Vvedenskii. Kharms and Vvedenskii had been allied in an informal group called the Chinari, a name Kharms intended to signify “enfants terribles.“ In the autumn of 1927, however, they decided to join with Zabolotskii in the OBERIU (Ob“edinenie real'nogo iskusstva). The nucleus of this group was composed of Zabolotskii, Kharms, Vvedenskii, Konstantin Vaginov, Igor’ Bakhterev, and Boris Levin, although at various times others participated in the OBERIU performances. It was not necessarily easy to become allied with the group: Kharms and Zabolotskii would test the creative potential of candidates by having them answer questions like “Where is your nose?” and “What is your favorite dish?“The OBERIU held readings in dormitories and military sections and at the Union of Poets and the Institute of the History of the Arts.

Context: Due to modernization, overeating becomes as trending topic due to its disturbance on our energy balance system and body weight. Behavioural issues such as overeating and sedentary lifestyle have impact on health through an increase in body mass index due to excessive deposition of body fat. Thus, it impacts on our body weight through positive energy balance in which there is an increase energy intake and decrease energy expenditure. On the other hand, it can be controlled through appropriate social support and optimum mental health. Aims: The objective of this study was to reveal predictor role of social support on mental health among obese and non-obese using regression analysis, to know mean difference in social support and mental health using Mann-Whitney test, and to identify the level of social support and mental health among obese and non-obese individuals. Methods and Material: The study design was non-experimental, quantitative study with descriptive in nature. The participants were 250 obese and 250 non-obese people and were collected from in Cuddalore district of Tamil Nadu, India. Both obese and non-obese participants were asked them to fill questionnaires with psychological tools namely the mental health inventory-38 (MHI-38) and multidimensional perceived social support scale (MDPSS). Results: The data distribution were in non-normal pattern and the statistical test used were non-parametric test. The results stated that the predictor variable social support has shown significant positive impact on mental health. The effect of social support over mental health was high among non-obese (R = .406, R2 = .168, adjusted R2=.164, co-efficient B = .667 and p = .000) than obese (R = .522, R2 = .272, adjusted R2 = .270, co-efficient B = .729 and p = .000). Social support and mental health have significant mean BMI difference among obese and non-obese in which obese people have higher social support and low mental health than non-obese people.

Der Hämoccult-Test war intermittierend positiv, eine Blutungsquelle konnte in früheren oberen und unteren Endoskopien nicht gefunden werden. Die Kapselendoskopie zeigte multiple vaskuläre Malformationen im Jejunum und Ileum. Insbesondere zeigt unser Fallbeispiel die grosse Bedeutung der Kapselendoskopie zur Lokalisation von okkulten Blutungen im Bereich des Dünndarms. Wir diskutieren die verschiedene diagnostische Möglichkeiten und Therapieoptionen.

ZusammenfassungAn zehn venengesunden Probanden (5 Männern und 5 Frauen; Durchschnitts-alter: 25,5 ± 3 Jahre) wurde die Bedeutung der Beweglichkeit im Sprung- und Kniegelenk für den Blutrücktransport aus den Beinvenen beim Gehen phlebodynamometrisch untersucht. Die Probanden gingen auf einem Laufbandergometer (Geschwindigkeit: 1,5 km/h; keine Steigung; Gehzeit: 1 Minute) zunächst ohne Bewegungseinschränkung (Versuch I), dann mit Bewegungseinschränkung im Kniegelenk (Versuch II) und schließlich mit Bewegungseinschränkung im oberen Sprunggelenk (Versuch III). Primäre Zielvariable war der maximale venöse Druckabfall (ΔPmax) unter Belastung, sekundäre Zielvariablen die venöse Druckausgleichszeit (T0) bzw. die halbmaximale Druckausgleichszeit (Tmax1/2) nach Belastung. Der maximale Druckabfall im Beinvenensystem wurde durch die Aufhebung der Beweglichkeit im Knie- (p = 0,0323) bzw. im oberen Sprung-gelenk (p = 0,0051) signifikant reduziert. Die Verminderung von ΔPmax durch die Bewegungseinschränkung betrug für das Kniegelenk 12,0% und für das obere Sprunggelenk 21,5% (p = 0,0469). Demgegenüber wurden die venösen Auffüllzeiten (T0 und Tmax1/2) durch die Bewegungseinschränkung nicht beeinflußt (p >0,05; jeweils Vorzeichen-Rang-Test nach Wilcoxon). Diese Ergebnisse zeigen, daß Bewegungseinschränkungen im Knie- und insbesondere im oberen Sprunggelenk selbst bei venengesunden Probanden zu einer ambulatorischen venösen Hypertonie führen. Da diese ein gleichzeitig bestehendes Venenleiden aggravieren oder für sich selbst den venösen Blutabtransport stören können, sollten zur Gesamtbeurteilung der funktionellen Auswirkungen von Beinvenenerkrankungen stets die Gelenkfunktionen mitberücksichtigt werden. Dies gilt für die klinische Praxis ebenso wie für gutachterliche Fragestellungen.

Zusammenfassung. Die empirische Erfassung psychischer Merkmale erfolgt in der Regel mit Instrumenten, die auf der Grundlage der klassischen Testtheorie entwickelt wurden. Seit den 60er Jahren bietet sich hierzu mit der Item Response Theory (IRT) eine Alternative an, die verschiedene Vorteile verspricht. Auf ihrer Grundlage können u.a. computeradaptive Tests (CATs) entwickelt werden, welche die Auswahl der vorgelegten Items dem Antwortverhalten der Patienten anpassen und damit eine höhere Messgenauigkeit bei reduzierter Itemzahl ermöglichen sollen. Wir haben verschiedene Schritte zur Entwicklung eines CAT zur Erfassung von Angst unternommen, um zu prüfen, ob sich die theoretischen Vorzüge der IRT auch in der praktischen Umsetzung bestätigen lassen. In dem vorliegenden Beitrag wird die Entwicklung der zu Grunde liegenden Itembank dargestellt. Hierfür wurde auf Daten von N = 2348 Patienten zurückgegriffen, die an der Medizinischen Klinik mit Schwerpunkt Psychosomatik der Charité zwischen 1995 und 2001 im Rahmen der Routinediagnostik ein umfangreiches Set etablierter konventioneller Fragebögen computergestützt beantwortet hatten. Diese beinhalteten 81 Items, die in einem Expertenrating für das Merkmal Angst als relevant angesehen wurden. Die Eigenschaften dieser Items wurden anhand ihrer residualen Korrelationen nach konfirmatorischer Faktorenanalyse (MplusTM), ihrer Antwortkategorienfunktion (TestgrafTM) und ihrer Diskriminationsfähigkeit (ParscaleTM) überprüft. Es verblieben 50 Items, die für die Anwendung eines polytomen Zwei-Parameter-Modells (Generalized-Partial-Credit-Model) als geeignet angesehen werden können. Orientiert man sich an einer Reliabilität von ρ ≥ .90 und legt für den computeradaptiven Testalgorithmus einen Standardfehler von ≤ .32 fest, so zeigen Simulationsstudien, dass die Merkmalsausprägung für Angst im Bereich von ± 2 Standardabweichungen um den Mittelwert der Stichprobe mit ca. 7 Items ermittelt werden kann. Zudem legen die Simulationsstudien nahe, dass der CAT-Algorithmus das Merkmal in den oberen und unteren Ausprägungen besser zu differenzieren vermag als die konventionell berechnete Summen-Skala des STAI (State).

Als einziges weiteres Instrument nennt die S1-Leitlinie zum geriatrischen Assessment der Stufe 2 den 20-Cents-Test (20-C-T). Neben Kraft und Feinmotorik ist jedoch das erfolgreiche Platzieren der Hand eine Voraussetzung für die Selbstversorgung. Ziel der Arbeit Zur standardisierten seitengetrennten Überprüfung des alltagsrelevanten Positionierens der Hand im Sitzen wurde der 8‑Punkte-Greifraum-Test (8P-GRT) entwickelt. Die Studie diente der Ermittlung von Gütekriterien des 8P-GRT bei geriatrischen Krankenhauspatienten. Material und Methoden Zwischen dem 31.07.2019 und dem 23.09.2019 wurden im Krankenhaus Rotes Kreuz Lübeck – Geriatriezentrum 82 stationäre Patienten mithilfe des 8P-GRT, des Shoulder Pain and Disability Index (SPADI), einem zu den Handpositionen des 8P-GRT korrespondierenden Fragebogen zu Aktivitäten der Selbstversorgung, der Handkraftmessung und dem 20-C‑T untersucht. Ergebnisse Die Interrater-Reliabilität betrug 0,99, die Retest-Reliabilität 0,95 für die rechte und 0,90 für die linke Seite. Auf die Person bezogen, trat ein Deckeneffekt (beidseits Score 8) bei 4,1 % (n = 3) auf; kein Bodeneffekt wurde beobachtet. Die interne Konsistenz (Cronbachs α) des gemäß Faktorenanalyse zweifaktoriellen Tests betrug 0,78 (rechts) bzw. 0,76 (links). Jeder der anderen Tests korrelierte enger mit dem 8P-GRT auf der rechten Seite, wobei die Korrelation mit dem oben genannten Fragebogen am höchsten war (−0,72), gefolgt vom SPADI (−0,60). Diskussion Eine standardisierte Erhebung der Handkraft, Feinmotorik und aktiven Positionierung der Hand (z. B. 8P-GRT) fügt die Hauptaspekte der Funktionsfähigkeit der oberen Extremitäten zu einem Gesamtbild zusammen.

Zusammenfassung Hintergrund Der Einsatz von Antibiotika gehört zum medizinischen Alltag. Ein dabei häufig eingesetzter Wirkstoff ist die Gruppe der Chinolone. Physiotherapeutisch relevant sind deren muskuloskeletale Nebenwirkungen, insbesondere auf das Sehnengewebe. Meist sind die Achillessehnen durch Tendinopathien oder Rupturen betroffen. Es besteht ein Mangel an Literatur bezüglich der Pathophysiologie als auch der Rehabilitation dieser klinischen Muster. Ziel Diese Fallstudie evaluierte die Anwendung eines exzentrischen Trainingsprotokolls bei chinoloninduzierter Tendinopathie der Achillessehne. Methode Die physiotherapeutische Rehabilitation einer 48-jährigen Frau mit unilateraler Tendinopathie der Achillessehne unter Einnahme des Antibiotikums Ciprofloxacin nach Lungentransplantation wird beschrieben. Dabei wurde ein kriterienbasierter Ansatz mit ausschließlich Hands-off-Therapie unter Einsatz eines exzentrischen Belastbarkeitsaufbaus der Achillessehne angewendet. Ergebnisse Nach 9 Einzeltherapien fanden sich Verbesserungen beim 1-Minute-Sitz-Stand-Test, der Range of Motion bei Dorsalextension des oberen Sprunggelenks rechts, beidseitige Kraft von Knieextensoren und -flexoren, Dorsalextensoren und Plantarflexoren des oberen Sprunggelenks, beim VISA-A-Fragebogen sowie bei Schmerz und Partizipation. Schlussfolgerungen Bei chinoloninduzierter Tendinopathie der Achillessehne ist ein exzentrisch betontes Trainingsprotokoll durchführbar und wirksam, wobei die zahlreichen Komorbiditäten der primär betroffenen Population zu beachten sind. Da Physiotherapeuten aufgrund der demografischen Entwicklung zunehmend mit hochkomplexen Fällen konfrontiert werden, ist es für sie wichtig, die unerwünschten Wirkungen von Medikamenten zu kennen.

Zusammenfassung: Der Test «Schnitte» ( Fay & Quaiser-Pohl, 1999 ) ist ein neues Verfahren zur Erfassung des räumlichen Vorstellungsvermögens. Die Ergebnisse einer Untersuchung zur Evaluierung dieses Raumvorstellungstests an 236 mathematisch besonders befähigten Schülerinnen und Schülern (10. und 11. Klasse) zweier Gymnasien in Sachsen-Anhalt mit mathematisch-naturwissenschaftlichem Profil werden vorgestellt. Der Test erwies sich dabei als besonders geeignet zur Erfassung der Raumvorstellungsfähigkeit im oberen Leistungsbereich. Korrelationen zu anderen Leistungstests (d2: 0.05; MTVT: 0.54) und zu Schulnoten (Physik: 0.81; Deutsch: 0.06) lassen sich als Belege für die Validität des Verfahrens interpretieren. Die Meßgenauigkeit, erfaßt über die zeitliche Stabilität (Retestreliabilität), wurde ebenfalls als zufriedenstellend (r = .72) nachgewiesen. Außerdem konnten geschlechtsspezifische Leistungsunterschiede beobachtet werden, die aber deutlich geringer ausgeprägt waren als in bisherigen Untersuchungen mit den «Schnitten». Alle Befunde werden vor dem Hintergrund der nach bestimmten Kriterien selektierten und im mathematisch-naturwissenschaftlichen Bereich gezielt geförderten Stichprobe diskutiert.

Da die Pathogenese die Therapie des TOS mitbestimmt, ist die Einteilung gemäss der betroffenen Struktur in das neurogene, venöse und arterielle TOS (nTOS, vTOS und aTOS) sinnvoll. Häufig ist jedoch von Mischformen auszugehen, die dann meistens auch unter dem Begriff «unspezifisches TOS» in die Gruppe des nTOS eingereiht werden. Da kein diagnostischer Goldstandard-Test zur Verfügung steht, nimmt die exakte Anamnese und klinische Untersuchung weiterhin einen grossen Stellenwert ein. Diagnostische Erfahrungen und therapeutische Fortschritte haben jedoch zu hoffnungsvollen Möglichkeiten im herausfordernden Management dieses Krankheitsbildes geführt.

Ein Ziel der anschließenden Rehabilitation ist die möglichst selbstständige Bewältigung der vielfältigen Aufgaben des täglichen Lebens. Neue Therapieverfahren nutzen verschiedene Hardwarekomponenten, um digitale Therapieinhalte umzusetzen. Diese können eine sinnvolle Ergänzung zu etablierten Standardverfahren darstellen. Ziel der Arbeit In dieser Studie soll der Effekt eines innovativen bewegungstherapeutischen Therapiekonzepts mit einem Roboterball auf motorische Parameter bei Schlaganfallpatienten überprüft werden. Material und Methoden An der Crossover-Studie nahmen 25 Patienten (60,0 ± 10,0 Jahre, 172,5 ± 13,8 cm, 79,5 ± 13,8 kg, 89,8 ± 72,6 Monate post-stroke) teil. Der Interventions- sowie der Kontrollzeitraum umfassten jeweils 12 Wochen. Das Training mit dem Roboterball erfolgte als Ergänzung zur Standardtherapie zweimal pro Woche für jeweils 45 min. Verschiedene spielerische Inhalte wurden mithilfe von einem Tablet und einem Smartphone durchgeführt. Ergebnisse Die isometrische Greifkraft konnte um 4,5 ± 3,6 kg (p = 0,000) erhöht werden. Die unilaterale Geschicklichkeit im Roundblock-Test verbesserte sich um 7,5 ± 6,3 gültige Versuche (p = 0,000). Die durch den QuickDASH-Fragebogen erhobene subjektive Gesundheitswahrnehmung der Patienten verbesserte sich um 12,4 ± 13,0 Punkte (p = 0,001). Diskussion Die zusätzliche Therapie mit dem Roboterball konnte die Funktionsfähigkeit der oberen Extremitäten bei chronischen Schlaganfallpatienten verbessern sowie deren subjektive Gesundheitswahrnehmung positiv beeinflussen. Hingegen stagnierte die Leistungsfähigkeit bei alleiniger Standardtherapie. Moderat betroffene Patienten profitierten am stärksten. Sehr stark ausgeprägte motorische oder kognitive Symptomatik führte teilweise zum Dropout. Die Ergebnisse sollten mit größeren Stichproben verifiziert werden.

Obest or obesity is the term used to show the accumulation of body fat that exceeds the normal limit. Excessive accumulation of body fat can often seem reluctant easily. People who are overweight are more receptive than normal weight people to external hunger cues, such as food taste and smell, or time to eat. Fat people tend to eat if they feel like eating, not eating when they are hungry. The type of research used was analytic observational research with cross sectional design. The population in this study were all students at Tomohon Christian High School 1 which was 1063 students. The sample was calculated in Slovak with a total of 43 students. Primary data is data about diet and the level of nutritional knowledge collected directly during the research. In this study most have a moderate level of knowledge with a total of 20 people (46.5%), a good level of knowledge of 20 people (32.6%), and less knowledge level of 9 people (20.9%).The results of the analysis using statistical test fisherexacttest with a value of p <0.05, which means there is a significant relationship between intake of levels of nutritional knowledge with obesity.

Zusammenfassung Hintergrund Die Kontrolle einer myoelektrischen Prothese erfordert ein umfangreiches rehabilitatives Training, welches auf repetitiven Übungen basiert, angeleitet unter physiotherapeutischer Aufsicht. Doch zuhause fehlt vielen Patienten die Motivation, die Übungen aus der Physiotherapie weiterzuführen. Mobile Spiele auf dem Smartphone können zu einer Langzeit-Motivation beitragen, das Heimtraining mit der notwendigen Intensität fortzuführen. Patienten und Methodik Wir entwickelten ein Trainingssystem, welches aus einer spielbasierten mobilen Rehabilitationsanwendung besteht, die mit dem Muskelsignal des Patienten gesteuert wird, außerdem einem Tablett zum Spielen der App, einem Elektrodenarmband und einem Handbuch. Bisher haben zwei Patienten an dieser Studie teilgenommen. Sie wurden gebeten die App für 4 Wochen zu Hause, 5 Mal pro Woche, für 10 bis 15 Minuten zu benutzen. Gemäß eines Prä- und Post-Test-Designs wurden die neuromuskulären Parameter der Patienten vor und nach dem mobilen Training untersucht. Evaluiert wurden u. a. die maximale Kontraktionskraft, Muskelseparation, proportionale Ansteuerung und Muskelausdauer, sowie die Nutzerstatistiken während der App-Benutzung. Resultate Nach dem Training mit der App konnte eine signifikante Verbesserung (p < .01) aller untersuchten klinischen Parameter zur myoelektrischen Steuerung einer Prothese erzielt werden. Die Nutzerstatistiken ließen eine hohe Motivation zur Benutzung des Spiels und dem zusätzlichen Ausführen eines diagnostischen EMG-Tests bei einem Patienten erkennen, der andere teilnehmende Patient jedoch hatte zwar das Spiel gespielt, jedoch den EMG-Test vernachlässigt und diesen nur zur Hälfte absolviert. Conclusio Die Trainings-App „MyoBeatz“ bietet nicht nur Anleitung und Feedback zur korrekten Ausführung von myoelektrischen Kommandos, sondern erhält auch die Motivation des Patienten durch verschiedene Spielmodi und Feedbackelemente. Durch eine Übersicht des Trainingsfortschritts in Form von Nutzer-Statistiken und Highscores kann der Rehabilitationsprozess überwacht und verglichen werden. Es konnte gezeigt werden, dass Patienten mit Amputation der oberen Extremität nach der Nutzung der spielbasierten App ihre neuromuskuläre Kontrolle, Kraft und Koordination signifikant verbessern konnten, so dass sie das Potential einer myoelektrischen Prothese voll ausschöpfen können.

A high-pressure pneumatic catapult works under extreme boundaries such as high-pressure and rapid change of pressure and temperature, with the features of nonlinearity and gas-solid convection. In the thermodynamics processes, the pressure is much larger than the critical pressure, and the compressibility factor can deviate from the Zeno line significantly. Therefore, the pneumatic performance and thermo-physical properties need to be described with the real gas hypothesis instead of the ideal gas one. It is found that the analytical results based on the ideal gas model overestimate the performance of the catapult, in comparison to the test data. To obtain a theoretical model with dynamic leakage compensation, leakage tests are carried out, and the relationship among the leakage rate, pressure and stroke is fitted. The compressibility factor library of the equation of state for compressed air is established and evaluated by referring it to the Nelson-Obert generalized compressibility charts. Based on the Peng–Robinson equation, a theoretical model of the high-pressure pneumatic catapult is developed, in which the effects of dynamic leakage and the forced convective heat transfer between the gas and the metal wall are taken into account. The results from the theoretical model are consistent with the data from ejection tests. This research presents an approach to study the performance of a high-pressure pneumatic catapult with high precision.

Abstract Studies on cresceatic fractures and crescentic gouges were carried out in the central part of the Alpine piedmont glaciation and the Quaternary Alpine mountain ice sheet. The test sites were situated in the pro-glacial area of the Hornkees south Berliner Hütte (Oberer Zemmgrund, Zillertal, Tyrol, Austria). This is also the area of the former confluence with another glacier which flowed at about 90° to Hornkees during the late-glacial period. It was therefore possible to make a comparison between different older glacier systems, different flow directions, and different ice thicknesses. About 12 000 minor features were measured by close-range photogrammetry; the result was 240 pairs of stereophotographs which were interpreted on first-order equipment. The single forms were registered point by point by coordinates. It was therefore possible to obtain three-dimensionally registered forms. One important result was that the late-glacial and the post-glacial glaciers of the Zemmgrund produced minor features with a large variation in size. A further result was the definition of possible glacier-flow directions by interpretation of the different directions of crescentic fractures and crescentic gouges. By this means it was possible to obtain several working reconstructions of the late- and post-glacial stream lines of the merged glaciers.

About 12 000 minor features were measured by close-range photogrammetry; the result was 240 pairs of stereophotographs which were interpreted on first-order equipment. The single forms were registered point by point by coordinates. It was therefore possible to obtain three-dimensionally registered forms. One important result was that the late-glacial and the post-glacial glaciers of the Zemmgrund produced minor features with a large variation in size. A further result was the definition of possible glacier-flow directions by interpretation of the different directions of crescentic fractures and crescentic gouges. By this means it was possible to obtain several working reconstructions of the late- and post-glacial stream lines of the merged glaciers.

Eine chronische gastrointestinale Blutung kann sich durch sichtbare, wiederholte Blutabgänge ab ano (in Form von Meläna oder Frischblut) oder durch wiederholte Hämatemesis manifestieren. Als okkulte gastrointestinale Blutungen bezeichnet man einen wiederholt positiven Hämokkult-Test und/oder eine Eisenmangelanämie ohne sichtbaren Blutabgang. Die Hauptursache für eine Eisenmangelanämie wiederum ist ein Blutverlust vorwiegend aus dem Magen-Darm-Trakt (bei Frauen kann es auch im Rahmen einer starken Menstruationsblutung zu einer Eisenmangelanämie kommen). Etwa 5% aller Patienten mit chronischer gastrointestinaler Blutung weisen trotz einer initialen Abklärung mittels Gastroskopie und Kolonoskopie keine erfassbare Blutungsquelle auf. Die Mehrzahl der Blutungsquellen liegt in dieser Situation im Dünndarm (z.B. Angiodysplasien, Meckel-Divertikel, NSAR-Enteropathie, Neoplasien). Falls bei wiederholter Blutungsanämie eine Blutungsquelle im oberen Verdauungstrakt und im Kolon/terminalen Ileum ausgeschlossen werden kann, sollte eine Blutungsquelle im Dünndarm gesucht werden. Dies kann mittels Push-Enteroskopie, Kapsel-Endoskopie, Dünndarmpassage, nuklearmedizinischen Untersuchungen, Angiographie oder intraoperativer Enteroskopie erfolgen. Welche Untersuchungsmethode jeweils angewendet wird, hängt von der klinischen Situation des Patienten ab. Die intraoperative Enteroskopie ist für Patienten mit wiederholten, schweren Blutungen indiziert, oder wenn mittels anderer Verfahren eine Läsion identifiziert werden konnte (z.B. bei der Kapsel-Endoskopie), welche mit konventionellen endoskopischen Verfahren nicht behandelt werden kann. Dieser Artikel gibt eine Übersicht bezüglich Differentialdiagnose und Abklärungsstrategie bei Patienten mit einer chronischen gastrointestinalen Blutung.

Zusammenfassung Gegenstand und Ziel: Anhand einer kontrollierten klinischen Studie sollte bei akut an Bronchopneumonie erkrankten Kälbern die therapeutische Wirkung einer Inhalation mit hypertoner (9%iger) NaCl-Lösung geprüft werden. Material und Methoden: 100 Kälber (Alter 7–64 Tage) aus einem Milchviehbetrieb wurden nach Auftreten einer Bronchopneumonie auf die Test- und Kontrollgruppe randomisiert. Die Testgruppe erhielt zusätzlich zur standardisierten antibiotischen Therapie während der ersten fünf Tage täglich einmal eine Inhalation mit 9%iger NaCl-Lösung (Einzeldosis 35 mg; Aerosol- Provokationssystem, APS; Fa. Jaeger GmbH, Würzburg). Der Krankheitsverlauf wurde anhand der klinischen Befunde und der Ergebnisse von Lungenfunktionsmessungen (Impuls- Oszilloresistometrie-System, Fa. Jaeger GmbH, Würzburg) über sieben Tage dokumentiert. Ergebnisse: Nach der Inhalation zeigten sich die Lungenfunktionsparameter wie folgt signifikant verändert: Die respiratorische Reactance war im gesamten Frequenzbereich (5–15 Hz) erhöht, die respiratorische Resistance bei 5 Hz deutlich erniedrigt. Diese Befunde, wie auch die zunehmende Frequenzunabhängigkeit der Resistance im Bereich von 5 bis 15 Hz, deuten auf eine Verminderung obstruktiver und/oder restriktiver Ventilationsstörungen hin. Allerdings spricht die Zunahme der respiratorischen Resistance im Frequenzbereich von 10 bis 15 Hz für eine Erhöhung der Strömungswiderstände in den zentralen und/oder oberen Atemwegen. Nach Inhalation verringerten sich Atemzugvolumen, Atmungsfrequenz sowie Atemminutenvolumen signifikant. Hinsichtlich der Entwicklung der klinischen Befunde wie auch der Lungenfunktionsparameter ergaben sich für die Inhalationsgruppe zwar etwas günstigere Werte, doch waren die Unterschiede zwischen beiden Behandlungsgruppen nicht signifikant. Schlussfolgerung und klinische Relevanz: Eine Inhalation von aerosolierter 9%iger NaCl-Lösung führt bei Kälbern mit akuter Bronchopneumonie zwar kurzzeitig zu einer Verbesserung der Lungenfunktion, in Anbetracht des damit verbundenen erheblichen Aufwandes erscheint dieses Therapieverfahren derzeit aber noch nicht für die Praxis geeignet.

Abstract:
Measurements such as weight and body mass index (BMI) have been considered practical and sensitive for predicting risks and health outcomes, however, they are not related to muscle mass. Therapeutic weight loss inevitably leads to the loss of variable amounts of skeletal muscle mass. We evaluated changes in body composition and estimated glucose disposal rate (eGDR) after 48 weeks of a program of lifestyle changes in overweight/obese patients with type 1 diabetes. The patients were invited to participate in a lifestyle change program (diet, daily steps, and resistance exercises without supervision at home). Body composition was evaluated by DXA, body weight, BMI, waist circumference (WC), and insulin sensitivity measured by eGDR at the beginning and at the end of the intervention. Continuous variables were expressed as mean ± standard deviation. For comparisons, Student's t-test was used. To determine correlations, the Pearson correlation coefficient was used. P<0.05 were considered significant. We included 13 T1D patients, aged 43.6 ±13.7 years, with a disease duration of 21.7 ± 11.6 years. The lifestyle intervention significantly reduced body weight 86.6±10.9 kg vs. 81.9±11.2 kg (p=0.018), BMI 29.2±3.04 kg/m2 vs. 27.5±2.36 kg/m2 (p=0.017), CC 95.4±8.8 cm vs. 88.8±8.4 cm (p=0.0001), fat mass index 10.1±2.85 kg/m2 vs. 8.9±2.24 kg/m2 (p=0.031), with increased the eGDR 7.61±1.5 mg/kg/min vs. 8.20±1.6mg/kg/min (p=0.026). But also, there was a reduction in the appendicular musculoskeletal index 8.3±0.8 kg/m2 vs. 8.06±0.8 kg/m2 (p=0.016). In conclusion, our lifestyle change program for people with T1D and overweight/obesity achieved a positive impact on body weight, BMI, waist circumference, fat mass, and eGDR, but not a positive impact on skeletal muscle, maybe for low adherence to resistance exercise. The knowledge of body composition is necessary to plan personalized nutritional and physical exercise interventions for the preservation of muscle tissue and avoid sarcopenia in this population.

Analysis of the influence of passive exoskeletons for overhead activities on tool holding times, local physiological strain and the range of motion of the upper limbs Objectives: This paper presents three laboratory studies on the second stage of the Institute of Ergonomics (Institut für Arbeitswissenschaft, IAD) concept for the application of passive exoskeletons in practice. An analysis is conducted on objective and subjective outcomes in relation to changes in loading/strain, holding times and range of motion when using three selected exoskeletons for overhead activities. Methods: Surface electromyography (EMG) is used to determine physiological muscular strain. Relevant muscles in the arm-shoulder area and in the back are selected for this purpose on the basis of reference literature and preliminary studies. Subjective outcomes concerning the discomfort of the test subjects during overhead activities with and without exoskeleton are recorded by means of the Corlett-Bishop scale. The neutral-zero-method was used to determine the range of motion. Results: The IAD concept for the analysis and evaluation of physical stress and strain when using passive exoskeletons in overhead work in the laboratory and in practice is described. On the basis of the laboratory studies carried out, it was found that muscular strain is significantly reduced when using the analysed exoskeletons in the arm-shoulder system, but increases at the same time in the back. Similarly, holding times were shown to be longer depending on the holding position. The range of movement in the area of overhead work is not restricted by the exoskeleton. In the case of free movements, it could be shown that the arms were held above shoulder height to an increasing degree when using an exoskeleton. Conclusions: The three studies carried out provide information on changes in stress and strain, holding times and range of motion when using three passive exoskeletons for overhead activities. Implications are derived for the use of passive exoskeletons in practice. The aim should be to produce further findings from practical field studies with working persons as well as with adapted ergonomic analysis methods. Abstract:
The aim - to identify the frequency of Helicobacter pylori infection, atrophic and metaplastic changes in the gastric mucosa in patients with Barrett's esophagus. Materials and methods. A retrospective study of 7392 cases of complex esophagogastroscopy with a biopsy of the epithelium of the mucous membrane of the esophagus and stomach, followed by a morphological study, was conducted on the basis of the Medical Centre “Oberig” clinic for the period 2010-2018. Statistical data was performed using the MedStat package. To assess the prevalence of changes in the esophagus and stomach, a 95% confidence interval was calculated using the Fisher angular transformation method. The probability of differences between group averages and relative values ​​was determined using the method of parametric statistics by calculating the t-Student criterion. The difference between the average values ​​was considered significant at p <0.05. To compare the frequencies in different groups, the Pearson chi-square test was used. Results. Endoscopically cylindrical columnar metaplasia of the esophagus epithelium was detected in 2994 patients (40.5% CI 39.4% -41.6% at a significance level of p = 0.05). In a histological examination, esophageal epithelial metaplasia was confirmed in 2910 patients (39.4% CI 38.3%-40.5%). 876 cases of gastric-type esophagus epithelial metaplasia were diagnosed (30.1%, CI 28.4-31.5%); 2034 cases of intestinal type metaplasia (69.9%, CI 68.2-71.6%). Histological examination of biopsy samples of the gastric mucosa in 5640 (76.3%, CI 75.3-77.3%) patients revealed atrophy of the gastric mucosa. In 2532 cases (34.3%, CI 33.2-35.3%), complete and incomplete intestinal metaplasia of the gastric epithelium was diagnosed. In 4524 (61.2%, CI 60.1-62.3%) patients, Helicobacter pylori was detected. In the presence of H. pylori, gastric esophageal epithelial metaplasia was detected in 510 of 3558 (14.3%, CI 13.2-15.5%, p = 0.05), intestinal metaplasia of the esophagus epithelium - in 966 of 4014 (24.1% CI 22.8 -25.4%, p = 0.05). Conclusion. Esophageal epithelial metaplasia is a common pathology, accounting for 39.4% among patients who underwent esophagogastroscopy. The presence of a strong connection between intestinal metaplasia of the epithelium of the esophagus and atrophy of the gastric mucosa with intestinal metaplasia of the epithelium of the stomach was found. Esophageal epithelial metaplasia is significantly less common with Helicobacter pylori gastritis, therefore the presence of H. pylori has a protective effect on the occurrence of esophageal epithelial metaplasia.

BackgroundJuvenile Idiopathic Arthritis (JIA) is the most common chronic rheumatic disease in children. Clinical evaluation of arthritis is a subjective measurement. Musculoskeletal ultrasound (MSUS) is increasingly being utilized in children. In order to provide objective assessments of arthritis, reliable and validated MSUS scoring systems are needed. Recently, a joint-specific scoring system for the assessment of arthritis of the pediatric ankle, wrist and finger joints were proposed[1, 2].ObjectivesThis study aims to further validate the MSUS scoring system for the assessment of finger, wrist, and ankle arthritisMethodsJIA patients with finger, wrist, and ankle arthritis as per expert clinician evaluation are eligible for this study. A comprehensive wrist, ankle, and finger (2-5th metacarpal and 2-5th proximal interphalangeal joints) MSUS examination by an American College of Rheumatology MSUS certified pediatric rheumatologist is performed an all enroll subjects. MSUS are scored using the semiquantitative (grade 0 to 3) scoring systems previously proposed. A subset of participants receives an MRI with contrast of the targeted joint immediately after MSUS performance. MRI findings are read for presence of synovitis, tendonitis and enthesitis by a pediatric radiologist with expertise on musculoskeletal imaging. For the analysis of the current report, MRIs were classified as having presence or absence of findings of JIA disease based on the pediatric radiologist expert opinion. MSUS were classified as normal (grade 0 and 1) or abnormal (grade 2 and 3). Cohen’s Kappa Coefficient was used to assess the level of agreement between MSUS (normal and abnormal) and MRI with contrast (absence or presence of disease) for each targeted joint. Tetrachoric Correlation was used to assess the strength of the association. Fisher’s Exact test was used to determine statistical significance at p < 0.05 level.ResultsA total of 52 children aged 2 – 18 years old have been enrolled in this study. To date 20 MSUS of the finger(s), 33 MSUS of the wrist, and 43 MSUS of the ankle joints have been collected. Three MRI of the 2-5 fingers (12 fingers total), 6 MRI of the wrist, and 5 MRI of ankle have been obtained through the study. Preliminary results of this effort reveal that for the ankle joint there is moderate agreement (Kappa coefficient 0.53 (confidence interval (CI)=0.14-0.92)) between MRI and US and a strong positive tetrachoric correlation (0.79) (p = 0.03). While the Kappa coefficient for comparing MRI and US finding on wrist joint indicated moderate agreement (Kappa 0.62 [0.15-1.00]) with a strong tetrachoric correlation (0.83) the Fisher’s exact test indicated a trend rather than statistically significant results (p = 0.07). The Kappa coefficient for MCPs and PIPs joints indicated strong agreement between MRI and US (Kappa 0.83 [CI=0.52-1.00] and 0.80 [CI=0.43-1.00] respectively) and were supported by an almost perfect Tetrachoric Correlation of 0.99 (p < 0.01). We anticipate to complete enrollment of subjects by the summer of 2022. Analysis of collected data is underway.ConclusionThe strong to moderate association of MSUS with contrast-enhanced MRI, suggest that MSUS can provide an objective and accurate assessment of ankle, wrist and finger arthritis at the bedside. Abstract Background: Mantle cell lymphoma (MCL) formally remains an incurable disease. After immunochemotherapy induction, rituximab (R) maintenance can prolong remission duration, but most patients are highly exposed to relapse. The chemotherapy-free combination of lenalidomide and rituximab combo (R2) has demonstrated its activity in MCL, but has never been used as maintenance after immunochemotherapy and never been compared to RM. In the MCL R2 Elderly clinical trial (EUDRACT: 2012-002542-20), we studied different induction regimens and randomized R and R2 maintenance in responders to first-line induction. Here the results of the maintenance phase are reported. Methods: Seven countries participated in this open-label, double randomized trial of the European MCL Network. Previously untreated patients(pts) &gt;60 yrs not eligible for high dose therapy with stage II-IV MCL were included. Initially, patients were randomized between 8 cycles of 3-weekly R-CHOP (rituximab, cyclophosphamide, doxorubicin, vincristine, prednisone) or 6 cycles of alternating 3-weekly R-CHOP and 4-weekly R-HAD (rituximab, cytarabine, dexamethasone). Subsequently, patients in complete or partial remission (CR, CRunconfirmed or PR) underwent a second randomization between maintenance with rituximab every 2 months or R2 (lenalidomide 15 mg or 10 mg during 3 weeks every 4 weeks plus rituximab). Second randomization was stratified for induction regimen, country group, MCL international prognostic index (MIPI) and response (CR/CRu vs PR). Both maintenance regimens were continued for 24 months. Primary outcome was PFS, determined as time from second randomization until progression or death from any cause, censored at the last tumor assessment date. The primary evaluation was done strictly according to ITT. This superiority trial was designed to detect a hazard ratio of 0.64 with a power of 80% (158 events in 443 randomized patients) in a two-sided log-rank test with significance level 5% and no interim analyses. In the run-in period of 6 months, patients responding to R-CHOP induction given outside of the trial could be randomized directly for maintenance treatment. Results: Randomization was closed in Nov 2019. Out of 624 pts included in the study, 620 were randomized for induction and 514 responded to induction (87 % ORR, CR/CRu 41%) and 495 were randomized for maintenance. Median age was 71 yrs, 70% male, 89% stage IV, 47% intermediate and 46% high risk MIPI. After a median follow-up of 2.1 years from maintenance randomization and with 182 observed PFS events, pts in the R2 maintenance arm had a significantly prolonged PFS in comparison to R alone (p=0.0003). The 2-year PFS was 76.6% (95%CI=[70.1 ; 81.9]) in the R2M arm vs. 60.8% in the R arm (95%CI=[53.7 ; 67.2]). So far, Overall survival (OS) was not different between both arms, with 2-year OS of 87.3% in the R2M arm (95%CI=[81.6 ; 91.4]) and 85.8% (95%CI=[79.8 ; 90.0]) in the R arm. Adverse events (AEs) were more pronounced with R2M. Recurrent (&gt; 5% in one arm) AEs of grade &gt; 3 were: neutropenia (50.0% vs 18.8%), respiratory tract infection (5.5% vs. 0.8%), and skin cancer (5.5% vs 2.0%). In 46% percent of the patients in the R2M arm, the dose of lenalidomide had to be reduced at least once. Conclusions: R2M in patients responding to induction immunochemotherapy is superior to RM for PFS but not OS. Safety data showed more toxicity in the R2M arm. Abstract:
This paper presents a practical framework and its applications of motion tracking algorithms applied to structural dynamics. Tracking points (“features”) across multiple images are a fundamental operation in many computer vision applications. The aim of this work is to show the capability of computer vision (CV) for estimating the dynamic characteristics of two mechanical systems using a noncontact, markerless, and simultaneous single input multiple output analysis. Kanade–Lucas–Tomasi trackers are used as virtual sensors on mechanical systems’ video from a high speed camera. First we introduce the paradigm of virtual sensors in the field of modal analysis using video processing. To validate our method, a simple experiment is proposed: an Oberst beam test with harmonic excitation (mode 1). Then with the example of a helicopter blade, frequency response functions’ (FRFs) reconstruction is carried out by introducing several signal processing enhancements (filtering and smoothing). The CV experimental results (frequencies and mode shapes) are compared with the classical modal approach and the finite element model (FEM) showing high correlation. The main interest of this method is that displacements are simply measured using only video at fps respecting the Nyquist frequency.
