Full textBackground: Sexual health is a human right for everyone, including elderly people. Even though taboos and prejudices may contradict this. Sexuality and intimacy may show itself as more than penetrative sex, such as hand holding or compliments. It is included in nursing staffs’ duties to help their patient achieve the best possible health, sexual health, and to document this, especially for registered nurses. Aim: The aim of this literature review was to explore nursing staffs’ experiences in conversations about sexuality and intimacy with elderly people living in nursing homes. Method: A literature study with qualitative approach was written. The databases PubMed and CINAHL were used. Eleven articles were selected and quality-reviewed. Result: Diffusion of responsibility prevailed when some of the nursing staff felt that it was part of their profession to address sexuality and intimacy, while others of nursing staff thought that it was the patient's responsibility to raise conversations about the subject. Nursing staff experienced the conversations as uncomfortable due to norms, the current taboo about the subject and the lack of knowledge. Nursing staffs’ approach varied depending on factors such as upbringing and religion. They also acted differently depending on their level of comfort. Barriers, communication difficulties and norms prevented open communication about sexuality between nursing staff and elderly people. Conclusion: It was found that nursing staff felt uncomfortable and poorly prepared for conversations about sexuality and intimacy with elderly people. Lacking support from the work team and inadequate or non-existent routines on how to proceed increased the silence about the subject. Further education about the subject is needed to implement routines and to enable nursing staff to develop within their professions.

Full textBackground: Hip fracture prevention in the elderly is of major importance with an ageing world-wide population. Elderly living in nursing homes have a particularly high rate of hip fractures. Hip protectors are a recent development in prevention of hip fracture caused by falling. The aim of this literature review was to examine if use of hip protectors are an effective nursing intervention in the prevention of hip fractures among the elderly living in nursing homes. Method: Literature review and manifest content analysis was used. Result: In eleven studies were hip protectors considered to be effective in the prevention of hip fractures. In four of the studies they were considered ineffective. The target group was those at greatest risk of hip fracture including high-risk of falling, the frail and women with low BMI. Discussion: All of the studies had difficulties with compliance of hip protector use which probably decreased their effectiveness. Hip protectors did not prevent all hip fractures even when worn correctly. Differences in the effectiveness of hip protector types were not able to be assessed as only one type was used in one than one study. Conclusion: Nurses need to carefully assess which elderly need hip protectors and to continue assessing as needs change. Nurses must educate caregivers and the elderly on the correct use of hip protectors and their limitations.

Full textBakgrund Världshälsoorganisationen rapporterar att det inte finns någon exakt statistik över hur många äldre personer över hela världen som lider av undernäring, men tillgängliga data tyder på att cirka 15–60 procent av de äldre personer som vårdas på sjukhus, särskilt boende och inom hemsjukvården lider av undernäring. Det är viktigt att den legitimerade sjuksköterskan är medveten om olika omvårdnadsåtgärder och hur de kan genomföras för att förbättra den äldre personens näringsstatus. Syfte Syftet med denna studie var att undersöka tillämpade omvårdnadsåtgärder för att förebygga och behandla undernäring hos äldre personer på sjukhus och särskilt boende. Metod Studiedesignen var en litteraturöversikt med 15 vetenskapliga artiklar inkluderade. Datainsamlingen gjordes i databaserna PubMed och CINAHL. Resultatartiklarna var en blandning av både kvalitativa och kvantitativa artiklar. En integrerad analys utfördes för att sammanställa de 15 artiklarna. Resultat Omvårdnadsåtgärderna hade en övergripande positiv effekt på de äldre personernas näringsstatus. Näringsdrycker var den vanligaste åtgärden. Både miljömässiga och sociala faktorer påverkade matupplevelsen. Den äldre personen var sällan involverad i sin nutritionsvård och utvärdering av omvårdnadsåtgärderna var sällsynt. Samarbetet mellan vårdpersonal, anhöriga och den äldre personen var otillräckligt. Slutsats Undernäring hos äldre personer är ett vanligt förekommande problem som ofta bortprioriteras inom vården. Den vanligaste åtgärden var insättande av näringsdryck men det finns andra åtgärder som ger goda resultat och är mer individanpassade som ofta förbises av sjuksköterskan. Den äldre personens medverkan i sin vård gällande nutrition kan ökas och förbättras, såväl som medverkan av anhöriga. Vikten av att behandla och förebygga undernäring är ett ämne som skulle kunna betonas mer inom geriatrisk vård.

Full textNurses may share a commonality of issues which can affect their willingness and ability to respond as post-disaster emergency care providers. Guided by expectancy, locus of control, and chaos theory, a systematic literature review was conducted to identify the barriers which affect nurses’ willingness and ability to report to their unit after a disaster occurs. Briggs methodology guided this systematic review, and Fineout-Overholt’s and Melnyk levels of evidence were used to evaluate the reliability of information and effectiveness of their interventions. Fifteen articles meeting the inclusion criteria (addressed nurses’ willingness to report to their unit or to contact the incident command center for mobilization, published in 2005 or after, and written in English) were reviewed. Twelve were systemic reviews of descriptive and qualitative studies (Level 5), one was a cohort study (Level 4), one was a report of expert committees (Level 7), and one reported findings from a pilot study. Five articles reported personal barriers related to the nurses’ home caregiver responsibilities and four articles reported personal barriers related to nurses’ concern for personal and family safety. Three articles reported institutional barriers related to unsure availability of necessary safety equipment and two articles reported lack of disaster preparedness. Developing a disaster plan that includes emergency phone numbers, a prepared backpack of basic survival gear, and a plan for emergency child and elder care arrangements, as well as providing disaster training for nurses was recommended. Understanding health provider needs and willingness to respond to emergency situations contributes to positive social change by contributing to disaster risk reduction and ensuring safer and more resilient communities.

Full textAmputation affects men and women of all ages, classes, races and religions. The patients’ reactions are, however, entirely unique. This is a literature study comprising fourteen scientific articles which make up the results. The psychosocial effects of amputation can be daunting and considering the mountain of literature on the subject, it is therefore necessary to start by explaining some of the research on stress, crisis, and coping. Brief descriptions of Lazarus’s and Cullberg’s work have been selected to represent a sample of existing theories which have grappled with these psychosocial concepts. The purpose of this study is to describe the various coping strategies utilized by patients who have undergone amputation. The results consist of seventeen coping strategies which are divided up into five theme groups (positive coping, avoidance coping, social support, maladaptive coping, and religion). Evidence is provided which describes how the various coping strategies can actually manifest themselves in reality as witnessed by amputee patients. In conclusion, amputees use various coping strategies at different stages of recovery to deal with the trauma of their loss; these strategies can be observed and identified in both clinical and out-patient settings.

Full textPurpose: To describe how the role of the nurse practitioner has evolved as described in the literature from 2000–2012 using Bronfenbrenner's (1979) Socio-Ecological Model (SEM) of behavior change as adapted by McLeroy, Bibeau, Steckler, and Glanz (1988).
Data Collection: This study is based on the review of five large databases for literature related to nurse practitioners role evolution between the years 2000-2012. Databases used for the search were Academic Search Complete, PubMed (Medical Publications), Cumulative Index to Nursing and Allied Health Literature (CINAHL), Medline (US National Library of Medicine), SCOPUS, and Cochrane Database of Systematic Review. The search terms were based on the five levels of the Socio-Ecological Model: Intrapersonal, Interpersonal, Organizational, Community, and Public Policy. Included in the search were the terms, role and utilization of nurse practitioners.
Results: Using the Socio-Ecological Model demonstrated how the role and utilization of nurse practitioners has changed from 2000-2012. Nurse practitioners, once serving rural, underserved, and uninsured populations now have opportunities to provide health care in many settings.
Implications: Nurse practitioners are important primary care providers. An increase in patient acuity and the complexity of health care have created a need for additional health care providers in primary care. Nurse practitioners are well suited to play a significant role in the present and future delivery of health care. Nurse practitioners as primary care providers need to be at the table when policy is made that relates to health care delivery.
