Abstract:
Electronic Medical records document the treatment plan and patient care. This study intends to identify trained nurses' reception toward using EMR in the wards. A non-experimental cross-sectional survey covered the multi-discipline area. A stratified random sampling method in which the population in this research consisted of n= 138 trained nurses. Results found that the trained nurses tended to document the data at the nurse's station compared to the bedside entry. It's also shown that the demographics variable significantly correlated with attitude domains. Hence with the research results, it is envisaged to benefit the nurses and organization and hopefully can become the catalyst for the Ministry of Health in further improving and elevating the system throughout all hospitals in Malaysia. Abstract:
Abstract Background Fall prevention programs are multidisciplinary, but nursing care plays a central and important role. However, management of falls are not addressed in the nursing clinical training curriculum in Malaysia. Methodology This study was conducted in Hospital Kuala Lumpur in the month of April and May 2019. A structured questionnaire with 10 questions on attitudes of a nurse in the management of falls and 10 questions on knowledge of falls in the hospital were distributed randomly to nurses of the Medical and Oncology departments. Results There were 260 participants: Medical (73%) and Oncology (27%). Among all the subjects, 84% were female. 17% of the subjects were junior nurses (less than 1 year of service) and 15.2% were seniors (>10 years of service). Majority only had a basic diploma in nursing (97%). The average score of the nurses’ attitude towards falls was 9 points, where 85.4% had a score of 8 or more, representing a good attitude towards the understanding of their role as a nurse in the management of falls. In view to knowledge, the average score was 8 and 19% scored full marks. Those who had worked for longer for the organisation and nurses from the medical department had more respondents scoring full marks. The nurses with poorer attitude are reflected by their lower average knowledge score of 6, 2 marks lower than those with a good attitude. Conclusion The study showed that the Malaysian nurses have a very good attitude towards the management and prevention of falls. This study showed that attitude of the nurse in the prevention of falls is correlated to their knowledge about falls. Therefore, nursing management of falls should be routinely introduced into the nursing curriculum. References 1. Abstract:
Evaluation of the perception of safety attitudes among physicians and nurses in hospitals is important to ensure optimum patient care. The objectives are to assess the perception of medical personnel on safety attitudes at their workplace and to measure the correlation between domains and factors studied. A cross-sectional study involving 160 physicians and 304 nurses is conducted at a teaching hospital. A validated Safety Attitudes Questionnaire (SAQ) consisting of 6 domains is used to measure the perception of medical personnel on safety attitude at their workplace. The Mann-Whitney test was performed for the comparison of the mean scores between two categorical variables and Spearman’s correlation coefficient was used to evaluate the relationship between two numerical variables in terms of strength and direction. Job satisfaction (73.4 ± 17.6) and management perception (56.1 ± 12.9) domains recorded the highest and lowest mean scores respectively. Doctors showed the highest perceived positive attitudes towards stress identification (57.5%) whereas perception of management (9.4%) has the lowest score, and the nurses showed the highest perceived positive attitudes towards job satisfaction (74.3%), and a low score of perception of management (10.9%). Overall, climate safety and stress recognition domains showed significant correlations with age, level of education, years in specialty, and history of attending safety training. The study results indicated that the medical personnel had low positive safety attitudes towards the management perceptions domain. However, they reported a high level of job satisfaction domain. It is imperative for the management team to take the necessary steps to ensure the personnel develops a positive safety attitude.

Abstract:
Background: Effective and skilful communication is crucial and an important element in the quality of nursing care. This enables nurses to assess patients' needs and provide them with the appropriate physical care, emotional support, knowledge transfer and exchange of information. However, nurses find it difficult to communicate effectively and therapeutically with their patients. Aim: To identify the nurses’ and adult inpatients’ perceptions of barriers to effective nurse-patient communication. Methodology: A descriptive study was used to collect data from nurses (n=40) and adult inpatient (n=63) in multidisciplinary wards in a private hospital in Malaysia. Two sets of self-administered questionnaire for the two different groups of participants, the patients and nurses were used. Data were analysed using the SPSS version 20.0. Results: Nurses and adult in patient perceived heavy workload, dialect and negative attitudes towards nurses as main barriers that hinders effective communication among nurses and patients. Conclusion: Communication places an important part in patients’ satisfaction of care and the ability of the nurse to provided patient centred care. Nurses should take in to account those communication barriers that affect the patients and be confident enough to take the first step to initiate communication and be able to handle patients’ dynamic emotions.

Abstract:
The burden of diabetes continues to increase in Malaysia, and the public primary health sector has an insufficient number of health care providers well-trained in diabetes care. The Ministry of Health Malaysia collaborated with Steno Diabetes Center to educate primary care doctors and nurses on the fundamentals of clinical diabetes care using a competency-based approach that blends e-learning, classroom-based learning, and clinic-based group work. This programme is called Steno REACH Certificate Course in Clinical Diabetes Care (SRCC). The aim of this study was to assess the effectiveness of the SRCC intervention in improving diabetes-related knowledge, attitudes, skills and clinical practices among non-specialised doctors and general nurses working in public health clinics in Malaysia. This paper presents the study protocol. A quasi-experimental, mixed-methods study based on Solomon’s Four Group Design was applied. Non-specialist doctors and general nurses from ten health clinics were randomly selected to receive the educational intervention. Comparison clinics were purposive selected matching on proxy indicators for quality of diabetes care. The intervention consisted of 50 hours of e-learning, 48 hours of classroom-based learning and approximately 25 hours of work-based learning that covered all main aspects of clinical diabetes care and delivered over a six-month period. Primary outcomes were changes in diabetes-related knowledge, attitudes, skills, and clinical practice. Patients’ perceptions regarding the quality of care provided were classified as a secondary outcome. Other outcome measures included patients' assessment of their chronic disease care and providers' perceptions, attitudes and perceived barriers in care delivery. Results from this study will inform future educational approaches within the Malaysian health system. The study is unique because it evaluated a pertinent public health topic using a very robust methodology.

Abstract:
Introduction: Early Warning Sign (EWS) is a tool made up of vital signs chart and scoring to detect any changes in patient so that immediate and appropriate care can be further determined and provided. The objectives of the study are to assess the level of knowledge, attitudes, and practice of nurses in assessing patient using EWS scoring and to identify association between socio-demographic with knowledge, attitudes, and practice, as well as to determine the associations between knowledge with attitudes, attitudes with practice and knowledge of nurses in assessing patient using EWS scoring. Methods: A quantitative cross- sectional study with convenience sampling study was conducted on 132 nurses from medical, surgical and orthopedics wards in Sultan Ahmad Shah Medical Centre (SASMEC) from November 2020 until January 2021. Printed questionnaires were distributed to the nurses in English language. The questionnaire consisted of two parts. Part A was about socio-demographic data (area and on years of nursing practice, & service, area of practice, educational level, as well as whether holding post-basic/ advance course certificate, attending life support training and ICU outreach participant). Part B was about knowledge, attitudes, and practices of EWS. Data analysis was interpreted in descriptive and inferential analysis using Statistical Package for the Social Sciences (SPSS) version 20.0. Results: A total of 108 respondents involved in the study represents 82% response rate. Majority of respondents scored low in knowledge questions with grade F (n=106, 98.3%). One respondent scored grade D, correctly answering 8 over 13 questions (62%). Only one respondent obtained grade B with 85% score. Based on the result, there are more participants (n=58, 53.7%) achieving good attitudes while respondents with poor attitudes were (n=50, 46.3%). Moreover, 59 respondents (54.6%) have higher practice level than mean score, indicating good EWS practice. The remaining 49 respondents (45.4%) scored below the mean score cut off point indicating poor practice. The mean values are different depending on working areas in terms of attitudes and practice. The association between attitudes and practice (r= 0.641, p-value<0.01) is confirmed having a positive and strong correlation. Conclusion: The study revealed that despite having poor knowledge on EWS, the nurses have demonstrated good attitudes and practice in detecting patients’ progressive deteriorating conditions; thus, confirming that there are associations in area of practice with attitudes and practice. The results also shows that there is a significant difference between attitudes and practice of nurses in assessing patient using EWS with (p- value< 0.05).

Background: Pain Management is a medical approach that draws on science and alternative healing disciplines to study the prevention, diagnosis, and treatment of pain. Operating theatre nurses play the leading role in pain management and require thorough knowledge and skill in managing pain. Significant: Nurses, especially those working in a palliative setting, are considered to demonstrate a high level of knowledge regarding pain management principles with adequate understanding on matters such as a vital sign of patients in response to pain and type of analgesic drugs available. Aim: This study aims to determine the knowledge and attitude regarding pain management among operating theatre nurses in Hospital Melaka. Objective To determine the knowledge and attitude towards pain management among operating theater nurses in Hospital Melaka. Methods: A descriptive, cross-sectional survey was employed to determine operating theatre nurses' knowledge and attitude towards pain management in Hospital Melaka. The total sampling method was used to draw the respondents. An adapted version of The Nurses' Knowledge and Attitudes Survey Regarding Pain (NKASRP) tool was used to test the knowledge and attitude of operating theatre nurses in Hospital Melaka. Normality tests were used to determine the normality of data distribution, and descriptive statistics were used to analyze the data to present quantitative descriptions of variables in this study. Limitations: Although this study was carefully prepared, there were some unavoidable limitations. There is a lack of time for this study because during this study was performed, and it is a pandemic COVID-19, most of the nurses do not have enough time to answer the questionnaire because of their workload. Findings: This study showed that 77.9% of operating theatre nurses in Hospital Melaka had a high level of knowledge, and 88.4% had a high attitude regarding pain management. Nurses specializing in the perioperative course have a slightly higher level of knowledge (78.2%) and attitude (87.3%) than respondents who specialized in the perioperative course. In general, all operating theatre nurses in Hospital Melaka had adequate knowledge and attitude toward pain management. Pain management is effectively managed by operating theatre nurses in the hospital. Implications: However, all nurses need to adhere to best practices in pain management by increasing their theoretical and practical knowledge to improve pain management procedures in the future. Abstract:
Purpose This paper aims to examine the effects of high-performance work practices (HPWPs) (participation, training and compensation) on nurses’ job performance (task and contextual) through the mediating role of job satisfaction. As nurses form the bulk of health-care professionals, their performance at work is crucial in determining patient satisfaction regarding care quality. HPWPs have been recognized as having the ability to affect employees’ work attitudes and behaviours positively. Specifically, these practices foster job performance. Design/methodology/approach Self-administered questionnaires were used to collect the survey data on 639 staff nurses working in large private hospitals in Malaysia. Data were subsequently analysed using the partial least squares method. Findings The findings indicate that job satisfaction serves to mediate the relationships between the three HPWPs (participation, training and compensation) and the two dimensions of job performance (task performance and contextual performance). Research limitations/implications First, as all variables were measured using self-reports, a common-method bias could exist (Podsakoff et al., 2003). Hence, future researchers may want to combine self-assessments and supervisory or peer assessment to improve the validity of the outcomes. Second, the cross-sectional nature of this study limits our ability to make causal inferences. Bias could happen because the study examined both exogenous and endogenous variables at the same time. Thus, a longitudinal approach taken in the future could cross-validate the current findings and provide additional support regarding the causality of the HPWPs-job performance relationship. Third, the data were collected from staff nurses working in large private hospitals in Peninsular Malaysia. Thus, one should be careful to generalize the findings to different health-care professional groups and organizations. Practical implications From the practical perspective, it is evident from the findings that as job satisfaction is able to enhance job performance and given the need for nurses to provide quality health-care services, private hospital authorities concerned with encouraging greater job performance among their nursing workforce need to provide adequate support to their employees. This could be achieved through the implementation of HPWPs. Perceptions of the extent of a hospital’s HPWPs in terms of participation, training and compensation, have significant and positive effects on nurses’ level of job satisfaction. Therefore, it would be worthwhile for private hospitals to encourage more opportunities for nurses to participate in decision-making regarding their work. In addition, frequent training activities will be able to enhance nurses’ knowledge, skills and abilities, resulting in greater satisfaction Originality/value To the best of the authors’ knowledge, this is the first attempt to investigate the effects of HPWPs on nurses’ job performance in the Malaysian private health-care context. As studies using Eastern samples are relatively limited, the findings from this study would serve to expand the extant literature from a cross-cultural perspective.

Abstract:
Background: Adolescents’ sexual health is vital to overall health and well-being to achieve sustainable development goals. Yet, research on the strategies used by Malaysian school health teams regarding their experiences of providing sexual health information to adolescents is sparse. Objective: This study was conducted to explore the experiences of school health teams in Malaysia who provide unmarried adolescents with sexual health information either during school health visits or at health clinics, with a particular interest in the strategies they use to educate these young people. Methods: This qualitative study used semi-structured interview data from twenty participants from four multidisciplinary school health teams. The participants included staff nurses, medical officers, and family medicine specialists. The transcripts were analysed for common themes. Results: Four main themes were identified: discourse on risk, being selective, using scare tactics and maintaining own honour. The findings indicate that the interactions between school health teams and unmarried adolescents were not always ‘adolescent friendly’. The school health teams tended to use discourses of ‘risk’ or scare tactics to encourage abstinence in the adolescents they advised. Staff were also selective about the information they gave, prioritising notions of ‘maintaining honour’ over ‘safer sex’ messages. Conclusion: This study revealed how school health teams perceived sexual health education to be about moral issues and social attitudes. Influenced by both culture and religion, the sexual health services provided were underpinned by a ‘moral’ approach and promoted abstinence. However, most of the nurses in this study held the belief that sexual health knowledge acts as an encouragement for sexual activity. Thus, as part of school health teams, nurses need to embrace evidence that improved sexual health education may delay sexual initiation and prevent unintended pregnancy and HIV/STDs.

As a vital part of patient care delivery, patient safety culture contributes to the quality of care provided by nurses. Safe patient care is positively linked to the attitudes of nurses. This study aimed to assess the perception of nurses working in a newly established teaching hospital. A cross-sectional study involving 194 nurses from three different units was conducted by using a 24-item Hospital Survey of Patient Safety Culture. Data on gender, working unit, age, years of working, and attendance in workshops on patient safety were also collected. The majority of the nurses had a positive total score of patient safety culture. The lowest score was 76 (63%), and the highest score was 120 (96%). The awareness on patient safety culture significantly differed between gender, years of working, and working units. Post-hoc comparisons using Tukey’s HSD test yielded a significant difference between nurses from critical care units and those from medical and surgical units. The mean score and total positive score on awareness on patient safety culture of the former were higher than those of the latter. Overall, the majority of the staff nurses in International Islamic University Malaysia Medical Center had a positive total score on awareness on patient safety culture. Awareness on patient safety, which is considered crucial worldwide, should be enhanced to influence the development of a positive patient safety culture within hospitals. This implementation would directly develop high-quality care to patients and positively impact health organizations.Abstrak Kesadaran Perawat terhadap Budaya Keselamatan Pasien di Rumah Sakit Universitas yang Baru Dibangun. Sebagai bagian penting dari pemberian perawatan pasien, budaya keselamatan pasien berkontribusi pada kualitas perawatan yang diberikan oleh perawat. Perawatan pasien yang aman secara positif terkait dengan sikap perawat. Penelitian ini bertujuan untuk menilai persepsi perawat yang bekerja di rumah sakit pendidikan yang baru dibangun. Sebuah studi cross-sectional yang melibatkan 194 perawat dari tiga unit yang berbeda dilakukan dengan menggunakan Survei Rumah Sakit Budaya Keselamatan Pasien. Data tentang jenis kelamin, unit kerja, usia, tahun kerja, dan kehadiran dalam lokakarya tentang keselamatan pasien juga dikumpulkan. Mayoritas perawat memiliki skor total positif dari budaya keselamatan pasien. Skor terendah adalah 76 (63%), dan skor tertinggi adalah 120 (96%). Kesadaran tentang budaya keselamatan pasien berbeda secara signifikan antara jenis kelamin, tahun kerja, dan unit kerja. Perbandingan post-hoc menggunakan uji HSD Tukey menghasilkan perbedaan yang signifikan antara perawat dari unit perawatan kritis dan mereka dari unit medis dan bedah. Skor rata-rata dan skor total positif pada kesadaran tentang budaya keselamatan pasien dari yang pertama lebih tinggi daripada yang terakhir. Secara keseluruhan, mayoritas staf perawat di International Islamic University Malaysia Medical Center memiliki skor total positif pada kesadaran tentang budaya keselamatan pasien. Kesadaran akan keselamatan pasien, yang dianggap penting di seluruh dunia, harus ditingkatkan untuk memengaruhi perkembangan budaya keselamatan pasien yang positif di rumah sakit. Abstract:
A new coronavirus was discovered in China that causes serious respiratory diseases. Nurses play an important role in handling patients with COVID-19. This study aims to assess the level of knowledge, attitude, and practice towards handling COVID-19 among nurses. A descriptive cross-sectional study was done, with questionnaires on nurses' knowledge, attitude, and practice regarding handling COVID-19. The majority of nurses (99.2%) have good knowledge, while 39.0% had a positive attitude, and 65.9% of nurses had good practice in handling patients with COVID-19. This study employs the combined technology acceptance model (TAM) and theory of planned behavior (TPB) (i.e., C-TAM-TPB) that extended with self-efficacy and knowledge of information technology to examine HIS acceptance by Malaysian nurses. A cross-sectional survey was conducted in three public hospitals in Malaysia to gather data from practicing nurses with experience of using HIS. Structural equation modeling using the partial least squares (PLS) method was utilized to evaluate measurements and structural models. The results stress that a nurses intention was significantly influenced by three factors, including his or her attitude, the subjective norm, and perceived behavioral control. The results also indicate that the model can explain over half of the variance in the intention to use HIS.

The study aimed to assess the knowledge and attitude on the implementation of play therapy for children delivered by staff nurses in the Training Institute of the Ministry of Health Malaysia at Kubang Kerian, Kota Bharu, Kelantan. Collected data retrieved from Google survey form from the 95 respondents. There is a relationship between knowledge and attitude (sig<0.05) in the implementation of play therapy for children delivered by staff nurses. Therefore it is concluded that the staff nurses should implement play therapy with the correct knowledge and proper attitude among children confined in the hospital settings.

Patient education is the process of influencing patient behaviour with using a combination method such as teaching, counselling and behaviour modification techniques to increasing the knowledge and maintain or improve health. Patient education is one way of communication between patient and nurses, which provides the necessary knowledge for enhancing patient care and potentially affecting their health outcomes. By doing the exploration on the current patient education practice, the study perhaps to overcome the issue of hospital readmission and patient able to manage their activities of daily living after receiving effective patient education. The study aims to investigate the perception and practice of patient education among nurses in one of the public hospital in Malaysia. The objective of this study was to identify the perception and factors influencing the practice of patient education among public hospital nurses. Stratified Random Sampling was used for the sampling technique. A total of 246 staff nurses were involved in this study. A self-administered questionnaire was used in this study for obtained data. Statistical Package for the Social Sciences (SPSS) for Windows, version 21 was used to analyse the data. The study revealed that most of the nurses perceived patient education as a high priority in nursing care and it was nursing responsibility to provide patient education. Findings indicated that lack of time, inadequate staffing and lack of knowledge were significant factors that influenced the practice of patient education. Years of working experience among the nurses was found to be significantly associated with priority for patient teaching. In conclusion, public hospital nurses were having a positive attitude towards patient education but could not practice completely. In order to enhance patient education, a more strategy is required to address inadequate staffing, lack of time, and lack of knowledge. Objectives This study is designed to determine the knowledge, attitude, and practice of food poisoning and its factors among postgraduate students in Universiti Putra Malaysia. Methods A cross-sectional study was conducted among 212 respondents who were selected through simple random sampling. Data was generated using a validated and reliable self-administered questionnaire. Results The majority of the respondents are male, aged less than 35 years old, non-Malaysian, single, first-degree holders, not working, received a monthly income of less than 3264 Malaysian Ringgit, aware of food poisoning outbreak and the sources of their information of the food poisoning outbreak were television, the internet, newspaper, Online journals, friends, Facebook, community, nurse, drinking raw milk for the second time, information from their parents, relatives, restaurant, and radio. Majority had previous history of food poisoning illness but didn’t correctly confirm the causes of their food poisoning illness. Majority had poor knowledge, acceptable attitude, and good practice of food poisoning. A significant association was observed for citizen, marital status, awareness of food poisoning outbreak, and previous history of food poisoning illness with knowledge. Gender and awareness of food poisoning outbreak were significantly associated with attitude. Attitude and practice were significantly associated. Logistic regression revealed that being married, awareness of food poisoning outbreak, and previous history of food poisoning illness are predictors for good knowledge. Female respondents and awareness of food poisoning outbreak are predictors for acceptable attitude. Conclusion Documentation of the identified poor knowledge and factors affecting knowledge, attitude, and practice provides essential information on the baseline indicators towards the risk of food poisoning among the respondents. A relevant interventional program is highly recommended to prevent the potential risks of food poisoning outbreak among them.

Introduction: Ventilator-associated pneumonia (VAP) is the most common nosocomial infection in intensive care units (ICUs). One factor causing VAP is aspiration of oral colonisation, which may result from poor oral care practice. Oral care using tooth brushing can prevent formulation of dental plaque that can be a reservoir for microbes causing VAP. Methodology: A cross-sectional survey was conducted among 124 nurses, using a self-administered questionnaire, to determine methods used, frequency, and attitude of nurses toward oral care provided to mechanically ventilated patients in Malaysian ICUs. Results: Methods for oral care and their frequency of use varied between nurses even in the same unit. Cotton with forceps was used by 73.4% of the nurses. Some nurses used forceps and gauze (65%) or spatulas and gauze (36%). Toothbrushes were used by 50.8% of the nurses. Nurses in this hospital reported to have positive attitude toward providing oral care. Conclusions: The survey showed the need to have standardised oral care protocols in ICUs to improve quality of oral care provided to ventilated patients.

Patient safety is attributed to the responsibility of each component that exists in the healthcare system in an organization. Accidents may happen based on as a result of lack of attention, mistaken perception, or improper cognition or attention such as distraction [1]. Nurses are considered to carry high job stress, and low sleep quality problems due to uneven working shifts compared to other occupations [2]. Nurses are often associated with overwhelming workloads and busy schedules, and this situation could compromise their work performance and patient safety [3]. This study aims to determine the prevalence of job stress and the patient safety culture among the nurses in the East Coast Teaching Hospital, Malaysia. There were 188 nurses who were randomly selected and agreed to participate in this study. The assessment was conducted using validated questionnaires, comprised of Expanded Nursing Stress Scales and Safety Attitude Questionnaire. The data were analyzed using descriptive analysis and Pearson’s correlation test. The study showed that 61.5% of the respondents suffered mild job stress, while 1.4% experienced severe job stress (Table 1). More than half of the respondents (74.1%) gave negative responses towards patient safety culture at their workplace showing their low awareness on patient safety culture (Table 2). Stress recognition factor (80.4%) was reported as the main factor for the negative response on patient safety culture among respondents. There was a significant inverse linear relationship between job stress and patient safety culture (r=-0.338, p<0.001). The result revealed that the majority of the nurses enrolled in this study experienced unsatisfactory patient safety culture levels. Most of the respondents expressed appreciation for their job, however, several other domains received less positive responses. The overall patient safety culture can be improved by providing measures to correspond with the stress factors experience by the nurses.

Objective: This paper reports the medical record analysis findings to identify nurses, and junior doctors risk assessment and response to the incidence of deteriorating patient in general ward. Methods: A medical records analysis was conducted at a medical ward in one Malaysian hospital. Patients that were admitted for more than 24 hours and scored at least 3 points on the Modified Early Warning Sign (MEWS) assessment with their last 24 hours parameters calculated and nurses and junior doctor’s responses were analysed retrospectively. Results: Out of 200 records obtained, only 10 patients’ medical records met the study inclusion criteria. Three main themes were evident in the study, namely ‘track and trigger’, ‘hierarchical intervening’ and ‘attitude’. Conclusion: This study demonstrates that some nurses and junior doctors experience difficulties in effectively responding to patient deterioration which often occurs at a critical juncture. This study highlights factors which need to be addressed to increase patient safety and improve medical outcomes during hospitalisation.

Abstract:
Purpose The purpose of this paper is to validate the Press Ganey Questionnaire (PGQ) (Bahasa Melayu version) using Hospital Universiti Sains Malaysia (HUSM) patients. Design/methodology/approach This cross-sectional study comprised 252 patients visiting HUSM. Patients were selected using the convenience sampling method. The PGQ (Bahasa Melayu version) had three main factors: during your visit; your care provider and overall assessment. Data were analyzed using the structural equation modeling. Findings The exploratory factor analysis resulted in item reduction from 21 to 17, which contained four factors with eigenvalues greater than 1. Meanwhile, confirmatory factor analysis results showed that data fitted the model: χ2/df at 1.764, comparative fit index at 0.952, Tucker–Lewis index at 0.941 and root mean square error of approximation at 0.073. The average variance extracted value for the four factors was greater than 0.50, which indicated that PGQ convergent validity was met. Overall, PGQ produced good reliability with composite reliability score equals to 0.966. Four factors were reclassified as “during your registration,” “hospital staff attitude,” “doctor’s attitude” and “overall assessment.” Research limitations/implications Patient satisfaction is an important and frequently used indicator for measuring healthcare quality; hence, a validated and reliable instrument is important for measuring patient satisfaction that leads to healthcare service quality assessment. Practical implications Validated PGQ provides some useful information for doctors, medical assistants, nurses and staff in the emergency department to help them become more prominent and efficient in their role as healthcare providers. Social implications Validated PGQ will help healthcare providers to deliver the best and exceptional care toward emergency patient, and thus improve their quality of work life. The findings in this study can be used as a guide or as baseline data for further research in this area. Originality/value The PQG (Bahasa Melayu version) was confirmed as a reliable and valid instrument for measuring patient satisfaction. This research is the first PGQ validation study in Southeast Asia, specifically focusing on Malaysian respondents.

Flood disaster is the commonest natural disaster with huge impact on healthcare services in Malaysia. The FloodDMQ-BM© questionnaire was developed as a tool to assess the knowledge, attitude, and practice of healthcare providers regarding patient management during a flood disaster. We aim to further validate the FloodDMQ-BM© questionnaire by using Confirmatory Factor Analysis (CFA) and Item Response Theory (IRT).This cross-sectional study involved doctors, nurses and paramedics working in the Emergency Department of Hospital Universiti Sains Malaysia, Hospital Raja Perempuan Zainab II and Hospital Kuala Krai. Respondents were required to complete the FloodDMQ-BM© questionnaire. The responses were analysed by using CFA and IRT to establish its validity and reliability. A total of 215 respondents participated in this study. CFA analysis with Maximum Likelihood Robust as the estimation method, on the attitude and practice components resulted in good factor loadings (>0.5) in nearly all items and excellent model fit indices values (CFI = 0.96-0.98, TLI = 0.95-0.96, SRMR = 0.04-0.05, RMSEA = 0.07). Meanwhile, IRT analysis on the knowledge section showed a good two-way marginal fit based on S-X2, and a good model fit with RMSEA of 0.08. Based on the 2PL model by using the IRT assessment of the knowledge section, one item in the knowledge section (K3) was removed (chi-squared residual >4) resulting in improved model fit. The included items had well-standardized loadings (>0.3) and marginal reliability of 0. 651.Our results confirmed that the FloodDMQ-BM© questionnaire displayed valid and reliable psychometric properties.

Fisher exact test analysis was used to determine the association between socio-demographic characteristics with knowledge and attitude level. In addition, the overall knowledge and attitude were analysed using the sum score of each outcome based on Bloom’s cut-off point. Results: Of the 150 nurses, 66.7% had high knowledge level about BRCA genetic testing, and 58% were positive towards genetic testing. The participants’ mean age was 28.9 years (SD = 6.70). Years of working experience (p = 0.014) significantly influenced knowledge level on BRCA genetic testing, whereas speciality working experience (p <0.001) significantly influenced BRCA genetic testing attitudes. Conclusions: The results show that most nurses have adequate knowledge of BRCA genetic testing. However, their attitude could be termed negative. Therefore, targeted education programs on BRCA genetic testing and risk are needed to improve the knowledge and attitude of nurses and, ultimately, can educate the women and increase health-seeking behaviour among eligible women.

Abstract Background Interprofessional learning (IPL) is a key challenge in Malaysia in incorporating the current profession-specific medical education into the interprofessional learning curriculum. Safe practices would be enhanced with improved collaboration among the health professionals when they learn with, from, and about each other. The main objective of this study was to determine the significant differences in self-assessment, attitude, and perception of interprofessional learning among doctors and nurses in a teaching hospital in Klang Valley, Malaysia. The second objective was to determine if there is any significant difference in the selected demography factors, mean and total scores between doctors and nurses in self-assessment, attitude, and perceptions of IPL aspects. Methods A total of doctors (39) and nurses (37) were recruited for an interventional study on the interprofessional learning approach on hospital acquired infection control. The participants responded to the University of West England Interprofessional (UWEIP) questionnaire at baseline consisting of four dimensions in IPL aspects; Self-assessment on communication and teamwork skills (CTW), interprofessional learning (IPL), interprofessional interaction (IPI), and interprofessional relationship (IPR). The Cronbach alpha value for the total questionnaire was established at 0.79. Results The majority of doctors scored positive in CTW, IPL, IPR, and neutral in IPI. Nurses’ also recorded the highest positive scores in CTW, IPL, and IPR, and neutral in IPI. Negative scores were found in CTW and IPI. A significant difference was revealed between doctors and nurses in IPL attitude; p = 0.024 and there was no significant difference in other dimensions (p > .05). Results also found a significant difference between participants’ and non-participants of IPL training sessions; p = 0.009. Conclusions This study revealed the infusion of interprofessional learning training among the health professionals displayed better self-assessments, attitudes, and perceptions towards collaborative practices.

Introduction: Islamic prayers involve certain physical movements and positions. These positions may be difficult to achieve in Muslim patients who suffer from any illness that limit their physical movements. Therefore, Islam introduced the concept of Rukhsah or exemptions. The objective of this study was to assess the knowledge, attitudes and practices of healthcare workers related to performing prayers during illness. Materials and Methods: A cross sectional study was conducted among 121 healthcare workers in Hospital Langkawi, between 22nd to 24th April 2014. Simple random sampling method was used to select the participants among healthcare workers in Hospital Langkawi to attend a workshop on Rukhsah Ibadah. The participants were given a set of questionnaire to answer and all the data were collected just before the talk began. Results: The majority of the participants 108 (89.3%) were female and more than half of them 74 (61.2%) were staff nurses. The mean age for the participants was 32.4 (SD: 9.42) years. About 73 (60.3%) of the participants have good knowledge, 62 (51.2%) have good attitude and 73 (60.3%) have good practice towards religious obligations and Rukhsah. Bivariate analysis shows no correlation between age and knowledge, attitude and practice score. There was also no correlation seen between knowledge, attitude and practice score. Conclusion: Almost half of the participants have good knowledge, attitude and practice on performing prayers during illness. By conducting proper training and coaching, it is hoped that their level of understanding towards Rukhsah can be improved.

Introduction: The COVID-19 pandemic has had an unprecedented impact in Asia and has placed significant burden on already stretched healthcare systems. We examined the impact of COVID-19 on safety attitudes among healthcare workers (HCWs) as well as their associated demographic and occupational factors, and measures of burnout, depression and anxiety. Methods: A cross-sectional survey study utilising snowball sampling was performed involving doctors, nurses and allied health professions from 23 hospitals in Singapore, Malaysia, India and Indonesia between 29 May 2020 and 13 July 2020. This survey collated demographic data and workplace conditions and included three validated questionnaires: Safety Attitudes Questionnaire (SAQ); Oldenburg Burnout Inventory; and Hospital and Anxiety Depression Scale. We performed multivariate mixed model regression to assess for independent associations with the SAQ Total Percentage Agree Rates (PAR). Results: We obtained 3,163 responses. A SAQ Total PAR of 35.7%, 15.0%, 51.0% and 3.3% was calculated among respondents from Singapore, Malaysia, India and Indonesia, respectively. Burnout scores were highest among respondents from Indonesia and lowest in respondents from India at 70.9%–85.4% versus 56.3%–63.6%, respectively. Multivariate analyses revealed that meeting burnout and depression thresholds, and shifts lasting ≥ 12 hours were significantly associated with lower SAQ Total PAR. Conclusion: Addressing factors contributing to high burnout and depression, and placing strict limits on work hours per shift may contribute significantly towards improving safety culture among HCWs and should remain priorities as this pandemic continues.

As a vital part of patient care delivery, patient safety culture contributes to the quality of care provided by nurses. Safe patient care is positively linked to the attitudes of nurses. This study aimed to assess the perception of nurses working in a newly established teaching hospital. A cross-sectional study involving 194 nurses from three different units was conducted by using a 24-item Hospital Survey of Patient Safety Culture. Data on gender, working unit, age, years of working, and attendance in workshops on patient safety were also collected. The majority of the nurses had a positive total score of patient safety culture. The lowest score was 76 (63%), and the highest score was 120 (96%). The awareness on patient safety culture significantly differed between gender, years of working, and working units. Post-hoc comparisons using Tukey’s HSD test yielded a significant difference between nurses from critical care units and those from medical and surgical units. The mean score and total positive score on awareness on patient safety culture of the former were higher than those of the latter. Overall, the majority of the staff nurses in International Islamic University Malaysia Medical Center had a positive total score on awareness on patient safety culture. Awareness on patient safety, which is considered crucial worldwide, should be enhanced to influence the development of a positive patient safety culture within hospitals. This implementation would directly develop high-quality care to patients and positively impact health organizations. Abstrak Kesadaran Perawat terhadap Budaya Keselamatan Pasien di Rumah Sakit Universitas yang Baru Dibangun. Sebagai bagian penting dari pemberian perawatan pasien, budaya keselamatan pasien berkontribusi pada kualitas perawatan yang diberikan oleh perawat. Perawatan pasien yang aman secara positif terkait dengan sikap perawat. Penelitian ini bertujuan untuk menilai persepsi perawat yang bekerja di rumah sakit pendidikan yang baru dibangun. Sebuah studi cross-sectional yang melibatkan 194 perawat dari tiga unit yang berbeda dilakukan dengan menggunakan Survei Rumah Sakit Budaya Keselamatan Pasien. Data tentang jenis kelamin, unit kerja, usia, tahun kerja, dan kehadiran dalam lokakarya tentang keselamatan pasien juga dikumpulkan. Mayoritas perawat memiliki skor total positif dari budaya keselamatan pasien. Skor terendah adalah 76 (63%), dan skor tertinggi adalah 120 (96%). Kesadaran tentang budaya keselamatan pasien berbeda secara signifikan antara jenis kelamin, tahun kerja, dan unit kerja. Perbandingan post-hoc menggunakan uji HSD Tukey menghasilkan perbedaan yang signifikan antara perawat dari unit perawatan kritis dan mereka dari unit medis dan bedah. Skor rata-rata dan skor total positif pada kesadaran tentang budaya keselamatan pasien dari yang pertama lebih tinggi daripada yang terakhir. Secara keseluruhan, mayoritas staf perawat di International Islamic University Malaysia Medical Center memiliki skor total positif pada kesadaran tentang budaya keselamatan pasien. Introduction: The notion of competency in pre-hospital emergency medical service (EMS) personnel is mainly focused on the professional proficiency that he/she has to provide intervention outside of hospital setting. Consequently, the effectiveness of pre-hospital EMS performance very much depends upon the capability of the personnel at the scene and as well during transport to the definitive care center. The aim of this study is to appraise and explore the competency of pre-hospital care staff and provide strategies for improvement. Methods: A mixed method approach combining the qualitative and quantitative study design. Of 134 staffs only 111(82.84%) returned the questionnaire. Nine semi-structured interviews and two focus group discussions were performed. The main informants were nurses/assistant medical officers, nurse/assistant medical officer administrators, and emergency physicians. Results: From the quantitative finding on the competency of staff handling pre-hospital EMS, all had the essential knowledge and skills. However, most of them reported not having good knowledge and skills for invasive procedures (31%-61%), include giving medications (61%-66%). The qualitative information provided insight about the issues and strategies for the personnel in regards to competency. All the relevant qualitative data were merged into 5 categories relating issues and 5 categories to strategies that could affect the competency of the personnel. Conclusion: Pre-hospital EMS systems need to consider that the competency of pre-hospital EMS personnel has to come along with the responsible attitude of the staff itself, the support of medical direction, clear protocol for guiding them whenever needed, and continuous professional-development courses require them to maintain their professional proficiency.

Ionizing radiation has been increasingly applied in medicine and firmly established as an essential tool for diagnosis. There is high possibility for medical radiation workers to receive doses that are considerably higher than recorded by their dosimeters due to lack of knowledge about ionizing radiation, lack of training in radiation protection, and attitude of the workers themselves toward radiation protection. The purpose of this study is to estimate the radiation risk due to occupational exposure to ionizing radiation among medical diagnostic workers at hospitals in Malaysia. Also the objective is to determine the knowledge of occupational radiation exposure and radiation safety among the workers. The assessment was made based on the collective doses collected from film badge of the workers. The results of risk assessment show the mean annual collective effective dose based on type of X-ray procedure in this study was 5.445mSv, which is much lower compared to the whole body exposure dose limit, set by the ICRP Publication 60. A survey on knowledge of occupational radiation exposure and radiation safety was conducted using questionnaire and it was found that vast majority of respondents were aware of radiation safety with 91.3% answered the specific questions regarding radiation protection at workplace correctly. Unfortunately only 30.4% of the respondents fully understand the hazard they are exposed to. The study reveals that there is a critical need to educate not only medical radiation workers but also medical doctors and nurses to decrease unnecessary occupational exposure to radiation hazard.

Acute Kidney Injury (AKI) is common but is often under-recognized. Delayed diagnosis may lead to patients’ increased morbidity and mortality. AKI is fully established with various guideline but remained lacking in early detection through knowledge, understanding, assessment, and poor identification. High incidence and mortality rate of AKI in Malaysia could be assumed to be due to inadequate early nursing assessment and intervention. Hence an education programme for nurses with valid and reliable tools need to be developed. The aim of this study to develop a valid and reliable research tools for nursing risk assessment education programme in the detection of AKI from surgical patients. The tools were developed within four phases. Phase 1, the development of instruments through research and literature review. Phase 2, the validity process. The tools were submitted for relevancy and validation by expert panels. Phase 3, a pilot study conducted to test the instruments and phase 4 involved analyzing the tools. Two types of research tools developed in the study. Type 1: The Knowledge, Attitude and Practice (KAP) of AKI questionnaire and Type 2: The Nursing Risk Assessment AKI form. Overall Content Validity Index value for type 1 instrument was 0.96 and type 2 instrument was 0.95 indicating adequate standard of acceptability. Overall Cronbach’s Alpha value was at 0.81. KAP of AKI questionnaire and the Nursing Risk Assessment AKI form are valid tools to assess nurses KAP level and the early detection of patients at risk of AKI respectively.

Abstract Background This study aimed at determining the effectiveness of an innovative approach using interprofessional simulation scenarios (IPSS) in improving knowledge, attitude, and practice (KAP) of hospital-acquired infection control (HAIC) among health professionals. Methods The interventional study was conducted in a teaching hospital in Malaysia. Purposive sampling was used to recruit participants from surgical, intensive care, and other units. Thirty-six health professionals in the experimental and forty in the control group completed the study. All subjects participated in an interactive lecture and demonstrated four IPSS on HAIC i.e. (i) taking blood specimen (ii) bedsore dressing (iii) collecting sputum for acid-fast bacilli and (iv) intermittent bladder catheterization. Each team consisted of a doctor and a nurse. A self-administered questionnaire on KAP on HAIC was completed by respondents during the pre-, immediately and, post-intervention. An independent t-test was conducted to measure the significance between the experimental and control group. Results The mean scores for KAP among the experimental group increased following the intervention. Significant differences in scores were seen between the two groups post-intervention (p < 0.05). Overall, using the four procedures as surrogates, the interprofessional learning approach in HAIC intervention showed improvement among the participants in the experimental group following structured instructions. The IPSS approach in HAIC clearly shows its relevance in improving learning outcomes. Conclusions Well-designed interprofessional simulated scenarios can be effective in skills training in improving KAP in HAIC among health professionals.
