We reformulate M, I, and C in our preferred format: parallelized elimination rules whose major premises have no proof-work above them. An effective binary operation [ , ] of reduction is recursively defined on proofs. It secures Cut-Elimination for each of M, I, C, and Classical Core Logic. The common form is: If proof P uses premises X to prove A and proof P′ uses A with other premises Y to prove B, then [P,P′] uses premises in X,Y to prove [either ⊥ or] B. Cut-Elimination for the orthodox systems omits the last bit of bracketed material; while Cut-Elimination for the two core systems includes it. So the core systems can enjoy epistemic gains that the orthodox systems cannot. We end by showing how to extract from any intuitionistic proof a core proof of a result at least as strong; and how to do the same with any classical proof.

Abstract:
The language of justice belongs to a discourse of free, autonomous individuals who can be properly responsible for their actions, and appropriately blamed and resented. The Buddhist critique of these latter attitudes goes beyond prudential considerations of the bad effects of anger. Getting to the roots of anger means getting to the metaphysical picture of distinct individuals that is necessary for resentment of injustice to arise. This essay argues that dependent arising moves the criterion of correctness in individuation from correspondence with reality to efficacy in eliminating suffering. This shift carries with it a shift in attributions of agency and patient, of perpetrator and victim. Such attributions are correct when so ascribing agency facilitates the elimination of suffering. “Real” responsibility, and the freedom this requires as well as the determinism that threatens it, disappear as issues, replaced with a Buddhist ethics of care (karuṇ ā) grounded in dependent arising.

Abstract:
Elective surgery on poorly prepared patients suffering with chronic pain and comorbid substance dependence is increasingly shown to confer suboptimal outcomes—both clinical and economic. Achieving biopsychosocial “fitness for surgery” for these patients often requires a process similar to preoperative optimization of cardiac and other chronic diseases, with modification/elimination of risk factors (in many cases shared with those diseases). These risk factors are not so much genetic or uncontrollable, but rather behavioral, and comprise toxic thoughts and toxic habits. The preoperative optimization program for chronic pain patients presented in this book focuses on high-yield modifiable targets that are supported by the literature and the authors’ clinical experience. These comprise tobacco cessation, preoperative opioid reduction or elimination, slow-wave sleep enhancement, nutritional and exercise “prehabilitation,” and reduction of anxiety and pain catastrophization.
