Abstract:
Through a field experiment at a seafood-processing plant, I examine how working alongside friends affects employee productivity and how this effect is heterogeneous with respect to an employee’s personality. This paper presents two main findings. First, worker productivity declines when a friend is close enough to socialize with. Second, workers who are higher on the conscientiousness scale show smaller productivity declines when working alongside a friend. Estimates suggest that a median worker is willing to pay 4.5 percent of her wage to work next to friends. (JEL C93, J24, J28, J31, L66, O15, P23)

Abstract:
The role of fibronectin (FN) in tumorigenesis and malignant progression has been highly controversial. Cancerous FN plays a tumor-suppressive role, whereas it is pro-metastatic and associated with poor prognosis. Interestingly, FN matrix deposited in the tumor microenvironments (TMEs) promotes tumor progression but is paradoxically related to a better prognosis. Here, we justify how FN impacts tumor transformation and subsequently metastatic progression. Next, we try to reconcile and rationalize the seemingly conflicting roles of FN in cancer and TMEs. Finally, we propose future perspectives for potential FN-based therapeutic strategies.

Abstract:
Background. This study examined how risk perceptions and social norms around e-cigarettes are associated with susceptibility (i.e., openness to using the product in the next 12 months) of e-cigarettes and smoking among adolescents. Methods. We analyzed data from a 2016 representative survey of 8,718 middle school students in Mexico. The study sample was restricted to students who had tried neither e-cigarettes nor cigarettes, ( N = 4471). Students reported on the risks of e-cigarettes compared with those of cigarettes, and product-specific norms were measured by assessing current use by family members, at least one close friend, and, for e-cigarettes, by perceived societal acceptability of use (i.e., acceptability among people in general). Adjusted prevalence ratios (APR) were estimated using generalized estimating equation models that regressed e-cigarette societal acceptability on study variables. Adjusted generalized estimating equation models also regressed susceptibility for each product on study variables. Results. Susceptibility to both e-cigarettes and smoking was higher among students who reported that their family and friends used only cigarettes or both products when compared with students whose family and friends did not use either of these products. Friend use of e-cigarettes was associated with e-cigarette susceptibility (APR = 1.33) but not smoking susceptibility. Students who perceived that e-cigarettes were less risky than smoking were more susceptible to e-cigarette use (APR = 1.45). The association between e-cigarette susceptibility and friend or family use was not mediated by societal acceptability. Conclusions. E-cigarette use among family and peers appears associated with susceptibility to use e-cigarettes in a way that is similar to the patterns found for cigarettes. However, the influences appear somewhat specific to the type of product that network members use.

Tay, Sock P. "New strategies towards the next generation of skin-friendly artificial turf surfaces." Thesis, Loughborough University, 2016. https://dspace.lboro.ac.uk/2134/22979.

Full text
Abstract:
The issue of skin friction related injuries has been one of the problems challenging the artificial sports turf industry. It has been identified by users as a major factor impeding acceptance of artificial turf at the professional level. However, information explaining the mechanisms for skin-turf abrasion is limited and little progress has been made, it appears, to derive an appropriate testing method for product approval or in evidence of improvement of the skin-friendliness of these products in sport surface surfaces. This research project focused on exploring the potential for improving the skin-friendliness of artificial turfs through a multi-faceted approach: identifying the contribution of the abrasive-components in modern artificial turf surfaces through mechanical testing; while critically evaluating currently available skin friction standards , evaluating strategies for polymer material modifications to reduce the skin-surface friction; and the designing of an appropriate bench-top set-up for the lab-based assessment of material skin-friendliness. The lack of understanding of skin-turf interaction was addressed by identifying the turf-component that has the greatest influence on the skin-turf friction with the mechanical device used in the current industry standard. The skin -turf frictional profiles of a series of third generation (3G) turf surfaces were examined, in combination with independent measurements of the silicone skin surface roughness pre- and post-friction testing. Results indicated that turf carpets without any infill material exhibited the highest frictional values while surfaces completely filled with either sand or rubber displayed similarly low frictional values, independent of infill type. Morphological measurements also showed the largest decrease in surface roughness for skin samples tested on carpet-only surfaces, indicating a smoothening effect via abrasion. This abrading effect is alleviated with the addition of infill to the surface, with fully-filled surfaces having the least damage to the skin s. This unprecedented study suggests that the carpet may have the largest influence on the overall frictional behaviour of an artificial turf surface narrowing down the turf component to be targeted when applying product improvements to address skin-friendly properties. The strategy of material surface modification was then employed, to study the effect of polyzwitterionic brushes on improving the skin-friendliness of the identified polypropylene substrate. To address the intended application for artificial turfs, a bench-top test was developed to investigate the frictional properties of the hydrated samples outside of commonly used aqueous environments, where an excess of lubricating water molecules is absent. Photo-grafted poly(sulfobetaine methacrylate) (pSBMA) brushes of various irradiation durations were prepared and the improvement in frictional properties was studied. Frictional measurements using silicone skin tips, under both dry and hydrated surface conditions, showed that the applied modification was capable of forming a stable lubrication layer in the absence of excess water, significantly reducing the coefficient of friction by up to 78.8 %. The pSBMA brushes also provided the additional advantage of antifouling exhibiting resistance towards pathogenic Staphylococcus aureus with almost zero surface colonization for well-grafted samples. The low skin -sample friction under ambient conditions and desirable fouling-resistance highlights the potential of pSBMA brushes as a modification strategy for achieving skin-friendly surfaces targeted at reducing the risk of skin abrasions. The tribological implications of counter-surface selection were investigated. Frictional assessments of the pSBMA-modified samples were carried out using standard steel tribo-tips, in addition to the skin tips used. Measurements with the skin tips showed that the hydrated pSBMA brushes were successful in reducing initial skin -sample friction though the effect diminishes with extended testing, attributed to the drying of the interfacial water. The standard steel tribo-tips were unable to reciprocate these results, returning consistently low frictional values regardless of extent of surface modification or hydration. These observations draw attention to the importance of counter-surface selection in frictional assessments, highlighting how appropriate test materials can identify characteristic surface properties while providing an interaction that simulates that of the intended application. The simple experimental set-up used may potentially be enhanced as an intermediate product qualification method in the manufacturing of skin-friendly artificial turf yarns.

Abstract:
Between 1994 and 2004 the American sitcom Friends was tremendously successful among American television viewers. Since its original broadcast, the series has continued to gain in popularity among new generations of viewers around the world, thanks in large part to its availability on the streaming service Netflix. If this televisual fiction has recently become the object of academic study, until now, very little attention has been paid to its maternal figures who nonetheless play an essential role in its diegetic sphere. Indeed, throughout its ten seasons, Friends rethinks and reworks this emblematic figure and proposes innovative writings of her which seem to take into account (an in turn, perhaps, contribute to) certain societal changes at work at the turn of the millennium in the United States. Thus, lesbian motherhood, gestational surrogacy, single motherhood and adoption are among the different forms of motherhood interrogated by this series, of which the comic imperative is a fundamental generic element. Anchored in Cultural Studies and Motherhood Studies, this dissertation proposes an in-depth analysis of the representations of mothers and motherhood in the sitcom genre in general, and in Friends in particular, through micro-readings of scenes as well as analyses of its pregnancy and maternity narrative arcs. The study takes into account the aesthetic, narratological, generic and ideological aspects of this iconic sitcom to reveal the complexities of the discourses surrounding mothers and the different forms of maternity

In the unique post-war and post-socialist arena several recent studies onformer Yugoslavian countries indicate that young people confronted by challengesof risk and uncertainty are turning away from the national and politicalsphere. Instead, they often seek trust and opportunities in their networks and relationshipson a personal level. This thesis explores the narratives and everydaypractices of young adults in Sarajevo regarding their friendship sociabilities. The aim is to provide an insight on how friendships are characterised, experiencedand related to the specific social and economic organisations that influencesthem. Based on fieldwork carried out in Sarajevo, Bosnia & Herzegovina myproject wants to present how young adults are reflecting and developing aspirationsin the light of their available choices and opportunities in life and how theyare using their friends in order to access various types of benefits and support. It is argued that in specific contexts friendships constitute the main social capitals and orientation points for young people. In order to explore and navigate through the different interests and agendas present in their community when it comes to personal education, career development and housing young peoplenecessarily choose to favour the reliance on their informal solidarities in form offriendships.

Nkoi, Barinyima. "Techno-economic studies of environmentally friendly Brayton cycles in the petrochemical industry." Thesis, Cranfield University, 2014. http://dspace.lib.cranfield.ac.uk/handle/1826/9260.

Full text
Abstract:
Brayton cycles are open gas turbine cycles extensively used in aviation and industrial applications because of their advantageous volume and weight characteristics. With the bulk of waste exhaust heat and engine emissions associated, there is need to be mindful of environmentally-friendliness of these engine cycles, not compromising good technical performance, and economic viability. This research considers assessment of power plants in helicopters, and aeroderivative industrial gas turbines combined-heat-and-power (ADIGT-CHP) in the petrochemical industry. Thus, it consists of two parts: part A focuses on performance analysis of helicopter gas turbines, while part B entails technoeconomic and environmental risk assessment of ADIGT-CHP in the petrochemical industry. The investigation encompasses comparative assessment of simple cycle (SC) and advanced gas turbine cycle options including the component behaviours and the environmental and economic analysis of the systems. The advanced cycles considered include: recuperated (RC), intercooled (IC), intercooled-recuperated (ICR), and low pressure compressor zero-staged (LPC-ZS), cycles. The helicopter engines are analysed and subsequently converted to small-scale ADIGT engines. Also, modelling combined-heat-and-power (CHP) performances of small-scale (SS), and large-scale (LS) ADIGT engines is implemented. More importantly, a large part of the research is devoted to developing a techno-economic model for assessing, predicting, and comparing viability of simple and advanced cycle ADIGT-CHP in the petrochemical industry in terms of net present value (NPV), internal rate of return (IRR), and simple payback period (SPBP). The techno-economic performances of the ADIGT-CHP cycles are measured against the conventional case of grid power plus on-site boiler. Besides, risk and sensitivity of NPV with respect to uncertain changes in grid electricity cost, gas fuel cost, emission cost, and electricity export tariff, are investigated. Two case studies underlie the development of the techno-economic model. One case study demonstrates the application of the model for large-scale (LS) ADIGT-CHP, and the other for small-scale (SS) ADIGT-CHP, all in the petrochemical industry. By so doing, techno-economic and environmental risk analysis framework (a multi-disciplinary preliminary design assessment tool comprising performance, emissions, economic, and risk modules) is adapted to ADIGT-CHP in the petrochemical industry, which is the aim of this research. The investigation and results led to the conclusions that advanced cycle helicopter and ADIGT engines exhibit higher thermal efficiencies than simple cycle, and that savings exist in operational costs of ADIGT-CHP above the conventional case. Thus, for both SS ADIGT-CHP, and LS ADIGT-CHP cases, all ADIGT-CHP cycles are profitable than the conventional case. For LS ADIGT- CHP category, the IC ADIGT-CHP is the most profitable, whereas for SS ADIGT-CHP category, the RC ADIGT-CHP is the most profitable. The contribution to knowledge of this research is the development of a technoeconomic model for assessing, predicting, and comparing viability of simple and advanced cycle ADIGT-CHP in the petrochemical industry in terms of NPV, SPBP, and IRR over the conventional case of grid power plus on-site boiler. A second contribution is the derivation of simple and advanced cycle small-scale ADIGT and ADIGT-CHP from helicopter engines. Cont/D.

Johansson, Eva. "Nätvänner : En studie om förekomsten av meningsskapande sociala relationer på nätet och hur de kan förändra relationsmönster." Thesis, Växjö University, School of Social Sciences, 2006. http://urn.kb.se/resolve?urn=urn:nbn:se:vxu:diva-467.

Full text
Abstract:

Net surfers make friends online. This study, by Eva Johansson, shows that the net users who use the net for relational purpose can form meaningful and sustainable social relations with other net users whom they have never met in real life (IRL), and may never meet face to face. 208 net users have answered a survey about social relations on the net. The purpose of this study is to find out if meaningful social relations can develop on the net; and function as a feasibility study for future research in this sphere. Using the net for relational purposes is becoming a growing trend. The answers in this mainly qualitative study gives a hint of what social relations, formed and developed on the net, may mean for the individual. It can mean everything from practical help and finding those with similar interests, to strong social support in difficult life situations. A majority of the respondents are women and many of them are middle aged. They have a considerable skill and experience as net users, and do use the net for their own purposes. About a third of the respondents spend more social time with net friends than IRL-friends today, compared to ten years ago. The time used for social interaction on the net is mainly taken from leisure time but also from time spent with family members and with friends face to face.

Foulke-Abel, Jennifer. "Natural Product Biosynthesis: Friend or Foe? From Anti-tumor Agent to Disease Causation." Thesis, 2010. http://hdl.handle.net/1969.1/ETD-TAMU-2010-12-8705.

Full text
Abstract:
Biosynthetic natural products are invaluable resources that have been gleaned from the environment for generations, and they play an essential role in drug development. Natural product biosynthesis also possesses the latent ability to affect biological systems adversely. This work implements recent advances in genomic, proteomic and microbiological technologies to understand further biosynthetic molecules that may influence progression of human disease. Azinomycin A and B are antitumor metabolites isolated from the terrestrial bacterium Streptomyces sahachiroi. The azinomycins possess an unusual aziridine [1,2-a] pyrrolidine ring that reacts in concert with an epoxide moiety to produce DNA interstrand cross-links. Genomic sequencing of S. sahachiroi revealed a putative azinomycin resistance protein (AziR). Overexpression of AziR in heterologous hosts demonstrated the protein increases cell viability and decreases DNA damage response in the presence of azinomycin. Fluorescence titration indicated AziR functions as an azinomycin binding protein. An understanding of azinomycin resistance is important for future engineering and drug delivery strategies. Additionally, the S. sahachiroi draft genome obtained via 454 pyrosequencing and Illumina sequencing revealed several silent secondary metabolic pathways that may provide new natural products with biomedical application. β-lactoglobulin (BLG), the most abundant whey protein in bovine milk, has been observed to promote the self-condensation of retinal and similar α,β-unsaturated aldehydes. BLG is a possible non-genetic instigator of cycloretinal and A2E accumulation in the macula, a condition associated with age-related macular degeneration. BLG-mediated terpenal condensation has been optimized for in vitro study with the retinal mimic citral. In rabbits fed retinal and BLG or skim milk, cycloretinal formation was detected in the blood by 1H-NMR, and SDS-PAGE analysis indicated BLG was present in blood serum, suggesting the protein survives ingestion and retains catalytic activity. Mass spectrometry and site-directed mutagenesis provided mechanistic insight toward this unusual moonlighting behavior. The experiments described in this dissertation serve to further natural product biosynthesis discovery and elucidation as they relate to consequences for human health. Efforts to solve azinomycin biosynthesis via enzymatic reconstitution, characterize compounds produced by orphan gene clusters within S. sahachiroi, and obtain a clear mechanism for BLG-promoted cycloterpenal formation are immediate goals within the respective projects.

Ireland, Emily. "Married Women’s Litigation in the English Court of Chancery, 1698-1758." Thesis, 2020. http://hdl.handle.net/2440/129205.

Full text
Abstract:
This thesis examines married women’s litigation by a next friend, against their husbands and others, in the English court of Chancery during the period 1698 to 1758. Contributing to existing debates concerning married women’s use of legal agency during the early modern period, it explores the extent to which married women had independent access to justice, could manage their separate property, and could make law in eighteenth-century England. Many historians have shown that the theoretical strictures of coverture were not as suffocating as they appear during this period. Reflecting this, Chancery treated women not entirely as feme covert or feme sole, but bestowed upon them something in between – pseudo-feme sole status. The law impacted the extent to which married women could use legal agency, but each married woman’s ability to exercise pseudo -feme sole status was also, in part, dictated by those closest to her- her husband, family, friends and acquaintances. The main focus of this thesis is the relationship between married female litigants and those whom we may term quasi-legal actors: Individuals who were generally not legal professionals, but fulfilled a legal role as well as a social or personal role in the married female litigant’s life. Examining the law regulating quasi-legal actors, and the actions and attitudes of those appointed to these positions, aids our understanding of married women’s litigation as a socio-legal practice and the strong link between the courtroom and the community. The extent to which a married woman could exercise legal agency was dependent on the actions, both positive and negative, of her other legal agents. Trustees facilitated married women’s property holding, next friends enabled married women’s access to justice, husbands might become defendants in a marital dispute, and neighbours turned deponents if a case progressed to the stage of providing proof. In every instance, the relationship between a married woman and these various actors created different balances of power between them. These relationships existed within the broader conversational relationship present between litigants and judges within the legal system.

Abstract:
This study examined the impact of reformed fishing and reef management practices in a community-based marine ornamental fishery in Tejakula sub-district, Bali, Indonesia, on the development of effective community-based natural resource management. With the absence of cyanide fishing for one decade, the coral reef condition has improved from 26% of area coverage to 53%. T-test result shows that there is no significant difference in individual fishing revenue and a considerable decrease in total fishery revenue. The fishers have benefited in resource conservation and a 73% decrease in fishing costs. The overall number of ornamental fishers has declined, due to social pressure, a smaller overall fishing space because of No Take Zones (NTZ), and fewer orders for ornamental fish.Environmental education for local community members is critical to maintain adherence to the reformed method of fishing. Clear definition of the NTZS policy should be acknowledged to avoid recurring conflict among stakeholders.

Abstract:
This qualitative study uses narrative inquiry (Connelly & Clandinin, 1988, 1990, 2001) and self-study to investigate ways to further understand and facilitate the integration of holistic philosophies of education with media literacy pedagogies. As founder and director of the Youth Media Literacy Project and a self-titled Imagitator (one who agitates imagination), I have spent over 10 years teaching media literacy in various high schools, universities, and community centres across North America. This study will focus on my own personal practical knowledge (Connelly & Clandinin, 1982) as a culture jammer, educator and cancer survivor to illustrate my original vision of a ‘holistic media literacy pedagogy’. This research reflects on the emergence and impact of holistic media literacy in my personal and professional life and also draws from relevant interdisciplinary literature to challenge and synthesize current insights and theories of media literacy, holistic education and culture jamming.

Abstract:
We examine the problem of assigning plots of land to prospective buyers who prefer living next to their friends. In this setting, each agent's utility depends on the plot she receives and the identities of the agents who receive the adjacent plots. We are interested in mechanisms without money that guarantee truthful reporting of both land values and friendships, as well as Pareto optimality and computational efficiency. We explore several modifications of the Random Serial Dictatorship (RSD) mechanism, and identify one that performs well according to these criteria, We also study the expected social welfare of the assignments produced by our mechanisms when agents' values for the land plots are binary; it turns out that we can achieve good approximations to the optimal social welfare, but only if the agents value the friendships highly.

Mizrahi, Itzhak, and Bryan A. White. Exploring the role of the rumen microbiota in determining the feed efficiency of dairy cows. United States Department of Agriculture, October 2011. http://dx.doi.org/10.32747/2011.7594403.bard.

Full text
Abstract:
Expanding world hunger calls for increasing available food resources. Ruminants have the remarkable ability to convert human-indigestible plant biomass into human-digestible food products, due to a complex microbiome residing in the rumen compartment of their upper digestive tract. One way to tackle the problem of diminishing food resources is to increase the animals' energetic efficiency, i.e., the efficiency with which they convert energy from feed, thereby increasing food availability while lowering the environmental burden, as these animals would produce more and eat less. We hypothesize that the cow's feed efficiency is dependent on the taxonomic composition, coding capacity and activity of its reticulorumenmicrobiota. To test this hypothesis, three aims are defined: (1) Evaluation of the feed efficiency of 146 dairy cows and defining two groups representing the highest and lowest 25% using the Israeli group's unique facility; (2) Comparing these two groups for microbiota diversity, identity and coding capacity using next-generation sequencing and metagenomic approaches; (3) Comparing the reticulorumenmicrobiota metabolic activity parameters. We measured feed efficiency in 146 milking cows and analyzed the taxonomic composition, gene content, microbial activity and metabolomic composition of rumen microbiomes from the 78 most extreme animals. Lower richness of microbiome gene content and taxa was tightly linked to higher feed efficiency. Microbiome genes and species accurately predicted the animals' feed-efficiency phenotype. Specific enrichment of microbes and metabolic pathways in each of these microbiome groups resulted in increasing valuable metabolites and decreasing unusable ones such as methane in efficient animals. This ecological and mechanistic understanding of the rumen microbiome could lead to an increase in available food resources and environmentally friendly livestock agriculture.

Pesis, Edna, Elizabeth J. Mitcham, Susan E. Ebeler, and Amnon Lers. Application of Pre-storage Short Anaerobiosis to Alleviate Superficial Scald and Bitter Pit in Granny Smith Apples. United States Department of Agriculture, January 2013. http://dx.doi.org/10.32747/2013.7593394.bard.

Full text
Abstract:
There is increased demand for high quality fruit produced and marketed with reduced chemical inputs to minimize toxic effects on human health and the environment. Granny Smith (GS) apple quality is reduced by two major physiological disorders, superficial scald and bitter pit (BP). These disorders cause great loss to apple growers worldwide. Superficial scald is commonly controlled by chemical treatments, mainly the antioxidant diphenylamine (DPA) and/or the ethylene action inhibitor, 1-methylcyclopropene (1–MCP). Both chemicals are ineffective in controlling bitter pit incidence. We proposed to investigate the beneficial use of non-chemical, abiotic stress with low O2 (LO2) applied for 10d at 20°C on GS apple fruit. During the project we expanded the treatment to more apple cultivars, Golden Delicious (GD) and Starking Delicious (SD) and another pome fruit, the pear. Apple and pear have similar physiological disorders that develop during cold storage and we examined if the LO2 treatment would also be effective on pear. Application of 0.5% LO2 atmosphere for 10d at 20°C or 500ppb 1-MCP at 20°C prior to cold storage at 0°C, was effective in reducing superficial scald in GS apple. Moreover, LO2 pretreatment was also effective in reducing bitter pit (BP) development in California GS and Israeli GD and SD apples The BP symptoms in GS from California were much more prominent, so the effect of LO2 was more dramatic than the effect on the Israeli cvs. GD and SD, nevertheless the LO2 treatment showed the same trend in all cultivars in reducing BP. The LO2 and 1-MCP -treated fruit exhibited lower levels of ethylene, - farnesene and its oxidation product, 6-methyl-5-hepten-2-one (MHO), as determined by SPME/GC-MS analysis. In addition, LO2 pretreatment applied to California Bartlett or Israeli Spadona pears was effective in reducing superficial scald, senescent scald and internal breakdown after 4 m of cold storage at 0°C. For GS apple, low-temperature storage resulted in oxidative stress and chilling injury, caused by increased production of superoxide anions which in turn led to the generation of other dangerous reactive oxygen species (ROS). Using confocal laser-scanning microscopy and H2O2 measurements of apple peel, we observed ROS accumulation in control fruit, while negligible amounts were found in LO2 and 1-MCP treated fruit. Gene-expression levels of ROS-scavenging enzymes were induced by the various pretreatments: catalase was induced by LO2 treatment, whereas Mn superoxide dismutase was induced by 1-MCP treatment. We assume that LO2 and 1-MCP pretreated fruit remained healthier due to reduced production of ethylene and reactive oxygen substances, such as MHO, during cold storage. The LO2-treated apple exhibited greener peel and firmer fruit after 6 m of cold storage, and the fruit had high crispiness leading to high taste preference. In both pear cultivars, the LO2 treatment led to a reduction in internal breakdown and browning around the seed cavity. We tested the LO2 pre-storage treatment on a semi-commercial scale that would be applicable to a small organic grower by sealing the fruit within the plastic field bins. The treatment was most effective with a continuous flow of nitrogen through the bins; however, a single 6 hour flush of nitrogen was also fairly effective. In addition, we determined that it was very important to have the oxygen levels below 0.5% for approximately 10 days to achieve good scald control, not counting the time required to reduce the oxygen concentration. Our LO2 technology has been proven in this project to be effective in reducing several physiological disorders developed in pome fruit during cold storage. We hope that our non-chemical treatment which is friendly to the environment will be used in the near future for the organic apple and pear industry. The next step should be an analysis of the cost-benefits and commercial feasibility.

Abstract:
As a student undertaking a Longitudinal Integrated Clerkship (LIC)1 based in a GP practice in a rural community in the North of Scotland, I have been lucky to be given responsibility and my own clinic lists. Every day I conduct consultations that change my practice: the challenge of clinically applying the theory I have studied, controlling a consultation and efficiently exploring a patient's problems, empathising with and empowering them to play a part in their own care2 – and most difficult I feel – dealing with the vast amount of uncertainty that medicine, and particularly primary care, presents to both clinician and patient. I initially consulted with a lady in her 60s who attended with her husband, complaining of severe lower back pain who was very difficult to assess due to her pain level. Her husband was understandably concerned about the degree of pain she was in. After assessment and discussion with one of the GPs, we agreed some pain relief and a physio assessment in the next few days would be a practical plan. The patient had one red flag, some leg weakness and numbness, which was her ‘normal’ on account of her multiple sclerosis. At the physio assessment a few days later, the physio felt things were worse and some urgent bloods were ordered, unfortunately finding raised cancer and inflammatory markers. A CT scan of the lung found widespread cancer, a later CT of the head after some developing some acute confusion found brain metastases, and a week and a half after presenting to me, the patient sadly died in hospital. While that was all impactful enough on me, it was the follow-up appointment with the husband who attended on the last triage slot of the evening two weeks later that I found completely altered my understanding of grief and the mourning of a loved one. The husband had asked to speak to a Andrew MacFarlane Year 3 ScotGEM Medical Student 2 doctor just to talk about what had happened to his wife. The GP decided that it would be better if he came into the practice - strictly he probably should have been consulted with over the phone due to coronavirus restrictions - but he was asked what he would prefer and he opted to come in. I sat in on the consultation, I had been helping with any examinations the triage doctor needed and I recognised that this was the husband of the lady I had seen a few weeks earlier. He came in and sat down, head lowered, hands fiddling with the zip on his jacket, trying to find what to say. The GP sat, turned so that they were opposite each other with no desk between them - I was seated off to the side, an onlooker, but acknowledged by the patient with a kind nod when he entered the room. The GP asked gently, “How are you doing?” and roughly 30 seconds passed (a long time in a conversation) before the patient spoke. “I just really miss her…” he whispered with great effort, “I don’t understand how this all happened.” Over the next 45 minutes, he spoke about his wife, how much pain she had been in, the rapid deterioration he witnessed, the cancer being found, and cruelly how she had passed away after he had gone home to get some rest after being by her bedside all day in the hospital. He talked about how they had met, how much he missed her, how empty the house felt without her, and asking himself and us how he was meant to move forward with his life. He had a lot of questions for us, and for himself. Had we missed anything – had he missed anything? The GP really just listened for almost the whole consultation, speaking to him gently, reassuring him that this wasn’t his or anyone’s fault. She stated that this was an awful time for him and that what he was feeling was entirely normal and something we will all universally go through. She emphasised that while it wasn’t helpful at the moment, that things would get better over time.3 He was really glad I was there – having shared a consultation with his wife and I – he thanked me emphatically even though I felt like I hadn’t really helped at all. After some tears, frequent moments of silence and a lot of questions, he left having gotten a lot off his chest. “You just have to listen to people, be there for them as they go through things, and answer their questions as best you can” urged my GP as we discussed the case when the patient left. Almost all family caregivers contact their GP with regards to grief and this consultation really made me realise how important an aspect of my practice it will be in the future.4 It has also made me reflect on the emphasis on undergraduate teaching around ‘breaking bad news’ to patients, but nothing taught about when patients are in the process of grieving further down the line.5 The skill Andrew MacFarlane Year 3 ScotGEM Medical Student 3 required to manage a grieving patient is not one limited to general practice. Patients may grieve the loss of function from acute trauma through to chronic illness in all specialties of medicine - in addition to ‘traditional’ grief from loss of family or friends.6 There wasn’t anything ‘medical’ in the consultation, but I came away from it with a real sense of purpose as to why this career is such a privilege. We look after patients so they can spend as much quality time as they are given with their loved ones, and their loved ones are the ones we care for after they are gone. We as doctors are the constant, and we have to meet patients with compassion at their most difficult times – because it is as much a part of the job as the knowledge and the science – and it is the part of us that patients will remember long after they leave our clinic room.
