These include cases related to orthopaedics, urology, paediatrics, neurology, and other branches of medicine. Each chapter covers both surgical and imaging techniques, to give the radiographer a better idea of what is required in terms of imaging and technique, along with comprehensive positioning graphics and accompanying high-quality radiograph images. The techniques and methods demonstrated are fully explained, and will allow staff to confidently perform imaging for procedures not covered in the text. Also included are sections on the practical skills required for working in theatres (such as team work and safe practice), infection control, radiation protection, exposures, and image quality, as well as discussions about the function, systems, and usage of intraoperative imaging equipment. This includes both image intensifier (II) systems and the newer flat-panel detector systems. Image artefacts and the effects of under- and overexposure are also covered, with examples of radiograph images and details on how to remedy them. Each chapter is separated by specialty and body region for quick reference and ease of navigation, while key points and imaging considerations are highlighted in each procedure for emphasis.

Additional aspects of visual field examination are explored including those of monocular, binocular, and junctional field defects, congruity vs. incongruity, macular sparing vs. macular splitting, density, wedge-shaped homonymous field loss, and monocular temporal crescent. Various new techniques of automated perimetry are also considered including SITA, FASTPAC, and SWAP. Neurostimulation describes an array of interventions that involve targeted stimulation of peripheral nerve, spinal cord, or the brain. Although few high-quality studies of neurostimulation techniques have been done and the techniques are seldom used in the management of pain related to serious illness, a better understanding of the available treatments and the emergence of newer technologies may increase access and use in the future. Transcutaneous electrical nerve stimulation is considered to be safe and may be used as an adjunct to pharmacotherapy in the routine management of chronic pain. Concerns about electrode placement near tumour masses continue, however, despite reassuring data, and for now, this approach should be used cautiously in those with metastatic disease. The recent advent of non-invasive central nervous system neurostimulation therapies-transcranial direct current stimulation and transcranial magnetic stimulation-offers promising new treatments for pain.

Abstract:
Behavior and molecular genetics informs knowledge of the etiology, structure, and development of the Five Factor Model (FFM) of personality. Behavior genetics uses quantitative modeling to parse the relative influence of nature and nurture on phenotypes that vary within the population. Behavior genetics research on the FFM has demonstrated that each domain has a heritability (proportion of variation due to genetic influences) of 40–50%. Molecular genetic methods attempt to identify specific genetic mechanisms associated with personality variation. To date, findings from molecular genetics are tentative, with significant results failing to replicate and accounting for only a small percentage of the variance. However, newer techniques hold promise for finding the “missing heritability” of FFM and related personality domains. This chapter presents an overview of commonly used behavior and molecular genetic techniques, reviews the work that has been done on the FFM domains and facets, and offers a perspective for future directions.

The clinical role of 3D and 4D scanning, of some of the newer Doppler techniques, and of fetal ECG for example, will become clearer in time, particularly with respect to assessing fetal cardiac function. Indications for and value of fetal cardiac interventions are being actively evaluated. Drug therapy for fetal conditions in addition to arrhythmias is only in its infancy with no clear consensus at present—this will change....

This chapter is devoted to programming techniques which rely on the C++ template mechanism. This mechanism, which is the basis of a computing style known as generic programming, allows whole families of functions and classes to be easily written. It is described early in the chapter. A host of extremely useful template functions and classes is universally available in the C++ standard library, including container classes (vector, list, set, and map) and algorithms used to sort, shuffle, and otherwise manipulate or query their contents. The chapter closes with an introduction to the newer constructs of modern C++: smart pointers, lambda functions, the auto keyword, range-based for loops, and more. An application to group theory is explored in the exercises.

In the past 50 years, classification systems for renal neoplasms have become increasingly complex as distinctive morphologic patterns in renal neoplasms have been recognized and correlated with clinical findings. In addition to classic histopatology, more sophisticated diagnostic tools, including electron microscopy, immunohistochemistry, cytogenetics, and molecular diagnostic techniques have greatly influenced distinctions between various types of renal neoplasms. The current World Health Organization classification of renal neoplasms encompasses nearly 50 distinctive renal neoplasms categorized as malignant or benign tumours. These categories have been expanded during recent years to incorporate newer histotypes, thus suggesting that the next revision of this classification will incorporate some recently recognized entities. In this chapter, we examine clinicopathologic and genetic features of the renal tumours most often seen in clinical practice.

Echocardiography plays a pivotal role in the management of patients with dilative cardiomyopathy and conduction disease, particularly in the setting of cardiac resynchronization therapy (CRT). Current CRT guidelines recommend the echocardiographic assessment of left ventricular size and function. Furthermore, echocardiography has the potential of analysing regional myocardial mechanics with high temporal resolution and without radiation burden or danger for the patient. Assessment of left ventricular dyssynchrony has therefore become the next challenge. Besides the visual approaches, newer methods of functional imaging such as tissue Doppler and speckle tracking allow the exact quantification of regional myocardial function. This chapter reviews the current status of left ventricular dyssynchrony assessment by echocardiography and introduces emerging techniques which can better link conduction abnormalities and mechanical events and, thus, potentially improve clinical decision-making in this field.

This article discusses the role of ‘spatial interdependence’ between units of analysis by using a symmetric weighting matrix for the units of observation whose elements reflect the relative connectivity between unit i and unit j. It starts by addressing spatial interdependence in political science. There are two workhorse regression models in empirical spatial analysis: spatial lag and spatial error models. The article then addresses OLS estimation and specification testing under the null hypothesis of no spatial dependence. It turns to the topic of assessing spatial lag models, and a discussion of spatial error models. Moreover, it reports the calculation of spatial multipliers. Furthermore, it presents several newer applications of spatial techniques in empirical political science research: SAR models with multiple lags, SAR models for binary dependent variables, and spatio-temporal autoregressive (STAR) models for panel data.

Axonal injury is currently considered to be the structural substrate behind most concussion-related neurological dysfunction. Because the principal generators of EEG fields are graded excitatory and inhibitory synaptic potentials of pyramidal neurons, the EEG is well suited for characterizing large-scale functional disruptions associated with concussion induced metabolic and neurochemical changes, and for connecting those disruptions to deficits in behavior and cognition. This essay provides an overview of the use of EEG and newly developed analytical procedures for the measurement of functional impairment related to sport concussion. Elevations in delta and theta activity can be expected in a percentage of athletes and change in asymmetry and coherence may also be present. Newer techniques are likely to be of critical importance for understanding the anatomical and physiological basis of cognitive deficits and may provide additional insight into susceptibility to future injury. Computational modeling may advance our understanding of concussion.

This chapter provides a comprehensive review of current airway management set against its historical context and likely future developments in the field. Developments in equipment design are discussed against the background of a short review of the anatomy and physiology relevant to clinical airway management. An exploration of airway devices examines progress in design from the first facemasks and early hands-free delivery systems, through to current second-generation supraglottic airways and the future of providing improved protection against aspiration. Continuing advances in tracheal tube and cuff design are set alongside developments in techniques and equipment for laryngoscopy and possibilities for supplementing capnography in confirmation of correct tube placement within the trachea. The use of newer drugs to facilitate control of airway reflexes is also discussed. The importance of using optimal evidence-based techniques in airway management is highlighted in the reduction of complications. This covers preoperative evaluation of the airway, planning a strategy, induction of anaesthesia, and establishing a clear airway through to safe termination of anaesthesia, emergence, tracheal extubation, and recovery. Techniques for dealing with complications if they arise are described. Drawing on lessons from the Fourth National Audit Project of the Royal College of Anaesthetists and the Difficult Airway Society ‘Major complications of airway management in the United Kingdom’ (NAP4) and the general literature, emphasis is placed on high-risk areas of airway management and areas where the existing knowledge base is not covered in depth in other texts.
