Many have difficulty tolerating enteral nutrition in the early weeks of life until gastrointestinal motility has matured. Some develop necrotizing enterocolitis (NEC) which carries a high risk of morbidity and mortality, and may be regarded as a failure of adaptation to postnatal life. Optimum nutrition should allow adequate growth in the short term, free of metabolic and other complications, with long-term fulfilment of both genetic growth and developmental potential....

Abstract:
Infants born at 24 weeks’ gestation now have a 40% chance of survival, rising to 80% at 26 weeks. Many have difficulty tolerating enteral feeds because of gastrointestinal immaturity; during this time parenteral nutrition is commonly given. Undernutrition in the early weeks of life may have lasting effects on developmental outcomes and increase the risk of certain chronic diseases in adult life (e.g. hypertension, cardiovascular disease, diabetes). Breast milk appears to confer some protection against necrotizing enterocolitis and be good for brain development. There has been a resurgence of investment in milk banks so that donor milk from nursing mothers in the community can be processed and given to preterm infants whose mothers cannot provide sufficient milk of their own. When breast milk is unavailable, preterm formula should be used, and following discharge from hospital (when many infants are showing a growth deficit), a nutrient-enriched formula can be given.

The Acute Care of at-Risk Newborns (ACoRN) program trains health care providers to stabilize that most challenging and enigmatic of medical patients: the unwell newborn. Early assessment, intervention, and management of at-risk or unstable infants can be critical for their survival and long-term health. Clinical care standards and educational programs to address these requirements are needed. The ACoRN program provides a unique, prioritized, and systematic approach to newborn stabilization for health care professionals with any degree of experience. ACoRN-trained providers learn to gather information, prioritize, intervene appropriately, and deliver high quality care to at-risk and unwell newborns in any setting. Because research and practice have advanced dramatically in recent years, the need for a new ACoRN text, the program’s centrepiece, became essential—hence the development of this new edition, which reflects current guidelines and evidence-based best practices. ACoRN teaches the concepts and skills required to stabilize unwell newborns through system-based algorithms (Sequences), each with its own chapter: respiratory, cardiovascular, neurology, surgical conditions, fluid and glucose, jaundice, thermoregulation, and infection. The ACoRN mnemonic defines stabilization steps and chapter structure: alerting signs, core steps, organization of care, response, next steps, and specific diagnosis and management. Each chapter includes educational objectives, key concepts, learning points, and at least one case scenario with questions and answers to reinforce content and learnings. This book is written for any health professional who may be required to participate in the stabilization of sick or preterm babies within their scope of practice.

This chapter discusses the epidemiology of adult congenital heart disease (ACHD). Congenital heart disease (CHD) is the most common major congenital defect and affects approximately 1 in 100 of all newborns. Sixty years ago, only 20% of all children born with CHD survived to adulthood. This has now increased to over 90%. As a result of this improved survival, there are now more adults than children with CHD in developed countries. The vast majority of these patients will require lifelong care.

Abstract:
Preterm birth is associated with increased risk of perinatal brain injury. Although there has been little headway made in reducing preterm birth rates, survival of infants born prematurely has improved greatly. Because of this, the neurodevelopmental consequences related to prematurity have become significant issues, especially in those infants born at less than 32 weeks gestation. Hypoxic-ischemic encephalopathy commonly leads to neonatal brain injury both before and after delivery. While perinatal birth asphyxia accounts for a proportion of neonatal brain injury in neonates younger than 37 weeks, preterm birth is the more significant risk factor. This chapter explores the neurodevelopmental consequences associated with preterm birth, the pathophysiology of perinatal brain injury, and the imaging modalities used to assess the newborn brain. Finally, various neuroprotective interventions in clinical use and in development will be described.
