Abstract:
Abstract The sex determination and differentiation process of economically important crustaceans have been regarded as the focus of aquaculture for a long time, because of the sex-related weight differences. Neocaridina denticulata sinensis makes a suitable animal model for studying crustaceans because it can reproduce many times under artificial control and has a short reproductive cycle. Male and female sex characteristics of the adult rice shrimp Neocaridina d. sinensis are morphologically obvious, but not in embryos and juvenile stages. At present, sex-specific DNA markers have not yet been developed. To produce a reliable molecular marker for sex in Neocaridina and to investigate molecular sex differentiation, we therefore focused on identifying sex-specific transcriptomic differences. In this study, we found three sex-specific expression genes, NDM, Sushi, and NDF, after screening a large number of transcriptome data. NDM and Sushi are male-specific expression genes, and NDF is a female-specific expression gene. Semi-quantitative RT-PCR analysis showed that NDM and NDF can act as molecular markers for the sex identification of Neocaridina in different developmental stages, especially sex identification for embryos and juveniles with the same morphological characteristics. However, Sushi can only act as a molecular marker for the sex identification of Neocaridina in adult stages. Furthermore, in situ hybridization showed that a strong positive signal of NDM was detected in the male testis. At the same time, we explored the relationship between these three genes and sex differentiation. The results of RNA interference treatment show that knockdown of nd-IAG (Neocaridina denticulata sinensis insulin-like androgenic gland hormone) can change the expression of NDM and NDF. On the basis of the expression of the male-specific gene NDM and the female-specific gene NDF, we developed a molecular test that for the first time allows the unambiguous sex determination of Neocaridina samples lacking external sex-specific features from juvenile stages onward.

ABSTRACT The global spread of carbapenemase-producing Enterobacteriaceae (CPE) that are often resistant to most, if not all, classes of antibiotics is a major public health concern. The NDM-1 carbapenemase is among the most worrisome carbapenemases given its rapid worldwide spread. We have developed and evaluated a lateral flow immunoassay (LFIA) (called the NDM LFIA) for the rapid and reliable detection of NDM-like carbapenemase-producing Enterobacteriaceae from culture colonies. We evaluated the NDM LFIA using 175 reference enterobacterial isolates with characterized β-lactamase gene content and 74 nonduplicate consecutive carbapenem-resistant clinical isolates referred for expertise to the French National Reference Center (NRC) for Antibiotic Resistance during a 1-week period (in June 2016). The reference collection included 55 non-carbapenemase producers and 120 carbapenemase producers, including 27 NDM producers. All 27 NDM-like carbapenemase producers of the reference collection were correctly detected in less than 15 min by the NDM LFIA, including 22 strains producing NDM-1, 2 producing NDM-4, 1 producing NDM-5, 1 producing NDM-7, and 1 producing NDM-9. All non-NDM-1 producers gave a negative result with the NDM LFIA. No cross-reaction was observed with carbapenemases (VIM, IMP, NDM, KPC, and OXA-48-like), extended-spectrum β-lactamases (ESBLs) (TEM, SHV, and CTX-M), AmpCs (CMY-2, DHA-2, and ACC-1), and oxacillinases (OXA-1, -2, -9, and -10). Similarly, among the 74 referred nonduplicate consecutive clinical isolates, all 7 NDM-like producers were identified. Overall, the sensitivity and specificity of the assay were 100% for NDM-like carbapenemase detection with strains cultured on agar. The NDM LFIA was efficient, rapid, and easy to implement in the routine workflow of a clinical microbiology laboratory for the confirmation of NDM-like carbapenemase-producing Enterobacteriaceae .

Abstract Objectives NDM carbapenemases have spread worldwide. However, little information exists about the impact of NDM-producing Enterobacteriaceae in Spain. By WGS, we sought to elucidate the population structure of NDM-like-producing Klebsiella pneumoniae and Escherichia coli in Spain and to determine the plasmids harbouring blaNDM-like genes. Methods High-resolution SNP typing, core-genome MLST and plasmid reconstruction (PlasmidID) were performed on 59 NDM-like-producing K. pneumoniae and 8 NDM-like-producing E. coli isolated over an 8 year period in Spain. Results Five major epidemic clones of NDM-producing K. pneumoniae caused five important nationwide outbreaks: ST437/NDM-7, ST437/NDM-1, ST147/NDM-1, ST11/NDM-1 and ST101/NDM-1; in contrast, the spread of NDM-producing E. coli was polyclonal. Three blaNDM types were identified: blaNDM-1, 61.2%; blaNDM-7, 32.8%; and blaNDM-5, 6%. Five K. pneumoniae isolates co-produced other carbapenemases (three blaOXA-48 and two blaVIM-1). The average number of acquired resistance genes was higher in K. pneumoniae than in E. coli. The plasmids encoding blaNDM-like genes belonged to IncFII, IncFIB, IncX3, IncR, IncN and IncC types, of which IncF, IncR and IncC were associated with MDR. The genetic surroundings of blaNDM-like genes showed a highly variable region upstream of ISAba125. Conclusions In recent years NDM-producing K. pneumoniae and E. coli have emerged in Spain; the spread of a few high-risk K. pneumoniae clones such as ST437/NDM-7, ST437/NDM-1, ST147/NDM-1, ST11/NDM-1 and ST101/NDM-1 have caused several interregional outbreaks. In contrast, the spread of NDM-producing E. coli has been polyclonal. Plasmid types IncFII, IncFIB, IncX3, IncR, IncN and IncC carried blaNDM, and the same IncX3 plasmid was detected in K. pneumoniae and E. coli.

Far-UV circular dichroism (CD) spectra of NDM-1 and NDM-1I35Tand NDM-1I35Senzymes suggest local structural rearrangements in the secondary structure with a marked reduction of α-helix content in the mutants.

: The New Delhi metallo-β-lactamase-1 (NDM-1) is a typical carbapenemase and plays a crucial role in antibiotic-resistance bacterial infection. Phylogenetic analysis, performed on known NDM-variants, classified NDM enzymes in seven clusters. Three of them include a major number of NDM-variants. In this study, we evaluated the role of the V88L substitution in NDM-24 by kinetical and structural analysis. Functional results showed that V88L did not significantly increase the resistance level in the NDM-24 transformant toward penicillins, cephalosporins, meropenem, and imipenem. Concerning ertapenem, E. coli DH5α/NDM-24 showed a MIC value 4-fold higher than that of E. coli DH5α/NDM-1. The determination of the kcat, Km, and kcat/Km values for NDM-24, compared with NDM-1 and NDM-5, demonstrated an increase of the substrate hydrolysis compared to all the β-lactams tested, except penicillins. The thermostability testing revealed that V88L generated a destabilized effect on NDM-24. The V88L substitution occurred in the β-strand and low β-sheet content in the secondary structure, as evidenced by the CD analysis data. In conclusion, the V88L substitution increases the enzyme activity and decreases the protein stability. This study characterizes the role of the V88L substitution in NDM-24 and provides insight about the NDM variants evolution.

This study reports an infectious case involving an NDM-1-producing Citrobacter freundii and further explored the potential threat of the bla NDM-1 gene by analysing the characteristics of the NDM-1-encoding plasmid sequence. A bla NDM-1-positive C. freundii with high resistance to carbapenems was separated from a clinical patient suffering from a urinary tract infection. S1 nuclease-based plasmid analysis followed by Southern blot hybridization, a conjugation experiment and electrotransformation confirmed that the bla NDM-1 gene was located on a plasmid. High-throughput sequencing of the bla NDM-1-postive plasmid (pCFNDM-CN) showed that it was a 54 kb IncX-type plasmid and contained a backbone region and a variable region with two β-lactamase genes (bla NDM-1 and bla SHV-12). The NDM-1 composite transposon in the variable region was surrounded by IS26 and IS5-truncated ISAba125, and shared a high sequence similarity to the bla NDM-1 surrounding structure in Acinetobacter spp. Our research suggested that the NDM-1 composite transposon might play an essential role in mobilization of the bla NDM-1 gene from Acinetobacter spp. to Enterobacteriaceae.

The ampA gene has a role in cell migration in Dictyostelium discoideum. Cells overexpressing AmpA show an increase in cell migration, forming large plaques on bacterial lawns. A second-site suppressor of this ampA-overexpressing phenotype identified a previously uncharacterized gene, ndm, which is described here. The Ndm protein is predicted to contain a coiled-coil BAR-like domain—a domain involved in endocytosis and membrane bending. ndm-knockout and Ndm-monomeric red fluorescent protein–expressing cell lines were used to establish a role for ndm in suppressing endocytosis. An increase in the rate of endocytosis and in the number of endosomes was detected in ndm− cells. During migration ndm− cells formed numerous endocytic cups instead of the broad lamellipodia structure characteristic of moving cells. A second lamellipodia-based function—cell spreading—was also defective in the ndm− cells. The increase in endocytosis and the defect in lamellipodia formation were associated with reduced chemotaxis in ndm− cells. Immunofluorescence results and glutathione S-transferase pull-down assays revealed an association of Ndm with coronin and F-actin. The results establish ndm as a gene important in regulating the balance between formation of endocytic cups and lamellipodia structures.

Abstract:
Huntington's disease (HD) is a progressive neurodegeneration which has symptoms such as movement dysfunction, cognitive abnormalities, and psychiatric disturbances. Neurodegeneration in HD is characterised by pathology that spreads throughout the cortico-striatal network. There is growing recognition that the transfer of mutant huntingtin (mHTT) protein is cellular and shaped by structural organisation of the brain, provides a general framework underlying progressive spread of degeneration in HD. However, relatively little is known regarding how such progression occurs over time. This knowledge is critical to inform future drug discovery efforts where neuroimaging methodology can be used to develop the potential therapeutic compounds in targeting vulnerable neural circuits in HD. This research aimed to develop and apply a novel graph theoretical network diffusion model to predict how and where in the brain neurodegenerative process is seeded in HD. We used longitudinal MRI scans (N=106 Premanifest HD (pre-HD) and N=89 Healthy Controls), collected 12 and 24 months from the Track-On HD study. We implemented Network diffusion model (NDM), which was previously applied to symptomatic individuals with HD, in the Premanifest HD population. To evaluate the spatial patterns in change in brain volume of HD brain regions compared to controls, we segmented the T1-weighted structural MRI scans into 82 brain regions using FreeSurfer based tools. A linear mixed-effects model is statistically used to evaluate the brain volume differences (across 82 brain regions) in different time visits. The outcome of this statistical approach revealed that degeneration over time in pre-HD compared to controls shows extensively higher (p<0.05). The putamen and caudate showed the highest atrophy sub-cortically. Cortically, the highest atrophy is observed in the visual cortex (lateral occipital) and temporal cortex (superior and middle temporal). Next, we implemented NDM to simulate the longitudinal pattern of degeneration across the 82 brain regions. The NDM assumes that the linear diffusion of pathological proteins facilitates pathology spread in the HD brain via the brain's structural connectome. Therefore, we used a canonical connectome from the healthy brain (82 x 82 IIT connectome) to simulate the brain's spread process. We found that initiating the diffusion process from pallidum, thalamus, and putamen predicts a pattern of degenerations (predicted atrophy) which is significantly correlated (p<0.05, corrected) with longitudinal degeneration measured using MRI scans. However, the predictive ability of NDM was moderate at best (maximum Pearson correlation between predicted and measured atrophy = 0.48, p <0.0001). These findings suggest that NDM can only moderately predict the pattern of degeneration in the HD brain. Overall, this thesis explores how NDM can better understand the progression of degeneration in HD. Our comparative study between healthy and patient’s groups statistically showed a progressive pattern of atrophy in the cerebrum. Furthermore, the results from NDM suggests that NDM could be helpful to model the progression of atrophy in subcortical and cortical regions in HD. Thus, NDM may build bio-physically informed machine learning algorithms for predicting future degeneration from baseline MRI scans.

Introduction: The New Delhi metallo-β-lactamase (NDM-1) is an enzyme synthesized by bacterium which hydrolyses β-lactam antibiotics and inhibits their antibiotic effects. Its active site is highly diverse and the hydrolysis mechanism is not yet fully understood. So, there is currently no clinically useful inhibitor for this enzyme. Due to this β-lactam resistant characteristics and the ease of horizontal gene transfer between bacteria, NDM-1 producing bacteria poses a great threat to public health. Objective: Structural analysis on Zn-Zn distance in the active site of NDM-1 was carried out in order to gain a better insight on its hydrolysis mechanism. Method: Structures of NDM-1 were obtained from PDB database and were viewed on PyMOL. Out of 22 NDM-1 structures searched, 12 of them fulfilled the inclusion criterion and were included in this study. Zn-Zn distances were compared with different factors including pH and temperature during crystallization, presence of water in the active site and nature of ligand bound to active site. Results: A trend was observed when Zn-Zn distances were compared with pH. There was also a correlation between Zn-Zn distance and nature of ligand. Distance between the 2 zinc ions in the active site increased in acidic pH value of 6.5 or below and it favours substrate hydrolysis while neutral or alkaline pH shortened the distance between zinc ions leading to hydrolysis inhibition. Discussion: One NDM-1 structure can hydrolyse the substrate in the absence of water molecule. It suggested that there might be another alternative mechanism which does not involve water molecule as nucleophile. Future study can be carried out to verify this hypothesis. On the other hand, it was also suggested that a lower pH, likely to be a pH value of 6.5 or below, could lead to a relatively higher Zn-Zn distance, likely to be 4.00 Å or above. A Zn-Zn distance of 4.00 Å or above would facilitate the hydrolysis mechanism. In other words, a pH value lower than 6.5 might also promote the hydrolysis action against β-lactams. These give us a deeper understanding into the hydrolysis mechanism of NDM-1 and might be useful in future development of clinical inhibitor to tackle the public health threat brought by NDM-1.

Människan ställs dagligen inför en mängd beslut. Att fatta beslut i dynamiska eller naturliga miljöer ställer höga krav på människan. Det är ofta mycket som står på spel, problemen är dåligt strukturerade, miljön är föränderlig och besluten måste fattas under tidspress. När flera personer ska beslut tillsammans kan beslutsfattandet organiseras på olika sätt t.ex. genom en demokratisk eller en hierarkisk organisationsstruktur. Denna rapport undersöker hurivida en hierarkisk organisationsstruktur är bättre lämpad än en demokratisk, för såväl experter som noviser, vid distribuerat beslutsfattande i en dynamisk militär domän. En experimentell studie genomfördes med studenter och officerare i en mikrovärld som skapats utifrån ett kommersiellt krigsstrategispel. I grupper om fyra personer skulle försöksdeltagarna utföra två militära uppdrag. Resultatet visar att studenterna eliminerade signifikant fler fiender än officerarna. Inga andra signifikanta skillnader erhölls. Resultatet kan ha påverkats av ett antal faktorer, studenterna hade t.ex. större vana av datorer, PC-spel och strategispel än officerarna.

Abstract:
L’augmentation des bactéries multirésistantes (BMR) aux antibiotiques est un problème majeur de santé publique. Le premier objectif de la thèse a été de rechercher la présence, peu documentée, de BMR dans la communauté en Tunisie. Pour la première fois, nous isolons une Klebsiella pneumoniae de séquence type ST147 productrice de la carbapénèmase NDM-1 dans ce milieu, à Sfax. Nos données indiquent aussi une proportion inhabituellement élevée (47%) d’Escherichia coli produisant deux ß-lactamases à spectre élargi. Quatre d’entre eux, avec CTX-M-15 et CTX-M-27, se divisent en 2 souches clonales de type A-ST617 (2 isolats) et B2-ST131 subclade C2 (2 isolats). Toutes contiennent un plasmide avec la même combinaison allélique, F31:A4:B1 ; suggérant une possible dissémination de ce réplicon. Lors d’une autre étude (milieu communautaire, Djerba), une souche clonale Eh22 d’Enterobacter hormaechei multirésistante, contenant un plasmide conjugatif IncHI2 de 300 kpb, a été isolée chez 2 patients sans lien épidémiologique apparent. Le plasmide a été séquencé et montre la présence de différents gènes de résistance incluant 4 gènes codant pour des ß-lactamases (blaTEM-1, blaDHA-1, blaCTX-M-3 et blaSHV-12). Dans une seconde partie, nous avons étudié chez Eh22 la résistance à la colistine (CS), antibiotique de dernier recours. Après avoir sélectionné un mutant in vitro, nous avons montré pour la première fois, chez Enterobacter spp, que cette résistance pouvait être due à une mutation dans le gène codant pour MgrB, un régulateur négatif du système à 2 composants PhoQP qui permet la synthèse de groupements cationiques sur le lipopolysaccharide, cible de la CS. Dans une dernière partie, des molécules de type bactériocines actives sur les BMR ont été recherchées à partir d’une collection de Bacillus thurengiensis. L’une d’elle, BUPM103, inhibe la croissance de BMR. Une analyse in silico a permis d’identifier le gène d’une potentielle bacthuricine F103 (11 kDa) qui a été produite dans E. coli. Le surnageant de sécrétion filtré a montré une activité d’inhibition de la croissance de K. pneumoniae multirésistante, contrairement au contrôle (sans sécrétion). Cette bacthuricine recombinante pourrait constituer une alternative thérapeutique pour le traitement des BMR
The increase of multidrug-resistant bacteria (BMR) to antibiotics is a major public health problem. The first objective of the thesis was to search for the presence of BMR, poorly documented in the community in Tunisia. For the first time, we isolate a Klebsiella pneumoniae belonging to the sequence type ST147 producing carbapenemase NDM-1 in this setting, at Sfax. Our data also indicate an unusually high proportion (47%) of Escherichia coli producing committally two extended-spectrum ß-lactamases. Four of them, with CTX-M-15 and CTX-M-27, are divided into 2 clonal strains of type A-ST617 (2 isolates) and B2-ST131 subclade C2 (2 isolates). All contain a plasmid with the same allelic combination, F31: A4: B1; suggesting a possible dissemination of this replicon. In another study (community-based, Djerba), a multiresistant clonal strain Eh22 of Enterobacter hormaechei, containing a 300 kbp conjugative plasmid of IncHI2, was isolated from 2 patients without apparent epidemiological relationship. The plasmid was sequenced and shows the presence of different resistance genes including 4 genes encoding β-lactamases (blaTEM-1, blaDHA-1, blaCTX-M-3 and blaSHV-12). In a second part, we studied in Eh22, the resistance to colistin (CS), antibiotic of last resort. After selection of an in vitro mutant, we showed for the first time, in Enterobacter spp, that this resistance can be due to a mutation in the gene encoding MgrB, a negative regulator of the 2-component PhoQP system that allows the synthesis of cationic residues on lipopolysaccharide, target of CS. In the last part, bacteriocins-like molecules active on BMR were searched in a collection of Bacillus thurengiensis. One of them, BUPM103, inhibits the growth of BMR. The gene for a potential bacthuricin F103 (11 kDa) was identified by an in silico analysis and it was produced in E. coli. The filtered supernatant secretion showed a growth inhibitory activity against a multiresistant K. pneumoniae, in contrast to control (without secretion). This recombinant bacthuricine could constitute a therapeutic alternative for the BMR treatment

Abstract:
The dissemination of members of the Enterobacteriaceae family with extended spectrum beta-lactamases (ESBLs) and metallo-beta-lactamases (MBLs) has become a global concern. ESBLs and MBLs have been reported in humans, domestic animals, wildlife and the environment, and their isolation frequencies are increasing rapidly worldwide.  Most studies have been performed in developed countries and quite few in developing countries, where the antibiotic consumption is often poorly controlled. To explore the environmental contamination of antibiotic resistance in Bangladesh, and of ESBLs and MBLs in particular, fecal samples from poultry and wild birds were studied in this thesis. Samples were collected from both sick birds (poultry having Escherichia coli infections) and healthy birds (free-range poultry, seagulls and crows) residing in different environmental niches. Samples from patients and fresh/sea water were included, to follow the chain of antibiotic resistance in bacteria from humans to the environment. Information regarding the antibiotic usage in poultry production was also collected. The susceptibility of avian E. coli isolates cultured with and without selective pressure was tested against antibiotics commonly used in human and veterinary medicine in Bangladesh. Special attention was paid to ESBL-producing isolates, which were further characterized genetically. The results of the studies showed that E. coli isolates from commercial poultry, free-range poultry, gulls and crows were resistant to several classes of antibiotics, and that the level and spectrum of antibiotic resistance varied between different bird populations. There was no NDM-producer found among the birds, but ESBL-producing Enterobacteriaceae could be found in up to 59% of the crows, the birds with the highest carriage rate of multiresistant  Enterobacteriaceae of all bird species studied. The most common ESBL-type was CTX-M-15, which also is the most common in the human population in Bangladesh. Birds also shared clinically important sequence types with humans, including E. coli clone O25b-ST131. In conclusion, ESBL-producing bacteria with multiresistance are easily spread to wild birds. Their opportunistic feeding behavior at poorly managed hospital waste dumps and nearby water bodies makes them into both reservoirs and active spreaders. The high level of antibiotic resistant and ESBL-producing bacteria in the bird population of Bangladesh is worrying, and there is no easy solution in sight. Nationwide programs are necessary to both improve the management of hospital waste and sewage and the control of the antibiotic usage to prevent further environmental contamination.

Visualising uncertainty information has been a research area for the past decade or so, and this thesis contains the results of an experiment that examines whether prior research on icon degradation for showing uncertainty can be used in a simulated aircraft cockpit environment. Using icon degradation has been suggested as being effective to combat overconfidence bias, as well as to accurately convey information about uncertainty. Two icon sets using icon degradation were taken from prior research, and one new icon set using shape change and colour change was created for comparison. Subjects flew a flight simulator while reading icons to evaluate the uncertainty displayed, and also evaluating their own confidence in their reading. The results show that shape change leads to much higher accuracy in reading icons, and slightly higher levels of confidence. Furthermore, icon degradation results in a higher variance in reading icons and an increase in errors when no time-pressure or distraction is present. This suggests that the suitability of icon degradation for showing uncertainty is questionable in all situations, and that other design approaches such as shape change should be considered. Furthermore, problems were uncovered in the prior research that the old icons were taken from, and these problems call into question the general approach used in that research. Une mini-banque d'adn genomique de la bacterie r. Capsulatus a ete construite dans le vecteur lamdagem-11 pour explorer les regions de l'adn adjacentes aux genes nuo8 et nuo9. Le dechiffrage de la sequence d'un fragment d'adn de 4. 915 pb, localise a portee immediate du gene nuo9, a permis de mettre en evidence 4 genes (nuo10, nuo11, nuo12, nuo13) codant des proteines hydrophobes respectivement analogues aux sous-unites nd6, nd4l, nd5 et nd4 du complexe l mitochondrial. Des souches mutantes de r. Capsulatus ont ete obtenues par insertion d'une cassette de resistance a la kanamycine dans les genes nuo8 et nuo12. Ces mutations retentissent sur le developpement de la bacterie: le developpement aerobie est fortement ralenti, le developpement photoheterotrophique necessite du co#2, les bacteries ne se developpent pas en photoautotrophie. La caracterisation polarographique et biochimique des souches mutantes montre l'absence totale d'activite nadh:ubiquinone oxydoreductase. Les spectres rpe, realises sur les membranes des souches mutantes reduites par le dithionite montrent l'absence des signatures caracteristiques des centres fe-s du complexe l de la bacterie. Ces resultats demontrent que le locus nuo de r. Capsulatus, code pour la nadh:ubiquinone oxydoreductase de la bacterie. La destruction de l'un ou l'autre des genes nuo8 et nuo12 entraine l'absence d'assemblage du complexe l dans la membrane bacterienne. Le retentissement des mutations sur le developpement bacterien est attribue a des perturbations de la balance redox

Leber´s hereditary optic neuropathy (LHON), a disease affecting vision, is caused by several point mutations in mitochondrial DNA. Mutations leading to a defect NADH ubiquinone oxidoreductase protein will affect the respiratory chain and cause a disturbed ATP production. It is still unknown why this defect leads to the degeneration of retinal ganglion cells and cells in the opticus nerve as well as demyelination of axons in these areas. Analysis of mitochondrial DNA is an important tool in the diagnosis of the disease. At the present time analysis is based on cleavage by restriction enzymes, which only detects two of the most frequent mutations: m.3460G>A and m.11778G>A. This is far too few considering that more than 30 mutations are known to be associated with LHON. Therefore a new analysis method is requested. Here we describe a method based on the sequencing of the mitochondrial genes MT-ND1, MT-ND4 and MT-ND6, which will detect more than 15 different point mutations associated with the disease. To validate the analysis, DNA from 31 patients with LHON symptoms were sequenced; of these 10 were found to be positive for a LHON mutation. This result indicates that the sequencing analysis will be more effective in diagnosis of LHON than restriction enzymes.


Lebers hereditära optikus neuropati (LHON) är en sjukdom som beror på genetiska förändringar i arvsmassan som leder till att cellens energiomsättning rubbas. Detta gör att nervceller i ögat och synnerven bryts ned vilket leder till en synnedsättning. En patient som drabbas av LHON har inga symptom fram till dess att synen börjar försämras. Sjukdomsförloppet går snabbt och på bara några veckor är patienten ofta helt blind. Diagnostik av LHON idag utgörs av flera undersökningar av öga och synfält. Diagnosen bekräftas av en analys av arvsmassan som finns i mitokondrien, cellens energifabrik. Här beskriver vi en ny förbättrad analysmetod baserad på DNA sekvensering, dvs. bestämning av baserna i mitokondriella arvsmassan. För att utvärdera analysen har vi undersökt 31 patienter med misstänkt LHON - av dessa visade sig 10 bära på en sjuklig förändring. Resultatet visar att sekvensering med fördel kan ersätta den tidigare analysmetoden då fler sjukliga förändringar kan påvisas och utförandet av analysen är mer användarvänligt.

A method is proposed for fitting the so-called contact stiffnesses (CSs) of interface elements for a nonlinear dynamic model (NDM) of a bladed disk with integral contact couplings. The method is based on comparison between frequencies of the resonant response of NDM and known natural frequencies in limiting linear cases. For this purpose, an effective approach for calculation of the resonant response NDM is presented allowing CSs to be picked individually. The method is demonstrated for the case of steam turbine bladed disk equipped with 48 inch blades.

Methods of near dry machining (NDM) are becoming a suitable alternative to reduce costs and eliminate, significantly, high amounts of fluid which would be rejected at the end of its life. This work presents a comparison between conventional fluid application (by gravity and high volume ~ 150 liters/hr) and the method of NDM (applying cutting fluid by spray ~ 60 ml/hr). Considering that investigation was carried out by turning a low machinability material like AISI 316 stainless steels, severe cutting temperatures made possible the evaluation of the efficiency of fluid spray in lubrication. Results suggest that only in a few conditions of severe heating, conventional method presented slightly better tool life and, for most of the investigated conditions, method of NDM showed good lubrication in tool-workpiece interface and enough cooling, determining an excellent performance of tool life and integrity of cutting edge.

Infecções relacionadas à assistência à saúde (IRAS) são um dos principais problemas de saúde pública em todo o mundo, especialmente as infecções causadas por bactérias multirresistentes (MDR). Com o surgimento do vírus SARS-CoV-2, houve um grande aumento de internações em unidades de terapia intensiva por pacientes infectados pelo novo coronavírus, com consequente uso intensivo de antimicrobianos nos mesmos, representando maior risco para a seleção e aumento da disseminação de bactérias MDR. Os carbapenêmicos são drogas beta-lactâmicas de amplo espectro muito utilizadas no tratamento de infecções graves causadas por bacilos Gram-negativos. Apesar do amplo espectro de ação destes antibióticos, tem sido frequente o isolamento de cepas produtoras de enzimas denominadas carbapenemases do tipo NDM, que confere resistência a todos os antibióticos beta-lactâmicos, inclusive os carbapenêmicos. O objetivo deste estudo foi investigar, por meio de levantamento bibliográfico de artigos científicos publicados até setembro de 2021, o impacto da COVID-19 na disseminação das carbapenemases do tipo NDM. Com o levantamento bibliográfico realizado, foi descrito a presença de cepas carreadoras de NDM em pacientes acometidos pela COVID-19 em vários países como a França e a Espanha; também foi relatado um aumento de detecções de bactérias MDR durante a pandemia. Além disso, dois estudos mostraram diminuição da disseminação de bactérias MDR devido à maior preocupação com o cumprimento de medidas de Prevenção e Controle de Infecção (PCI). A literatura científica sugere que a pandemia de COVID-19 pode ter tido um impacto negativo no aumento da resistência aos antimicrobianos e na disseminação de carbapenemases.

Abstract:
The authors have studied daily decision-making processes in groups under uncertainty, with an exploratory field study in the medical domain. The work follows the tradition of naturalistic decision-making (NDM) research. It aims to understand how groups in this high reliability context conceptualize and internalize uncertainties, and how they handle them in order to achieve effective decision-making in their everyday activities. Analysis of the survey data shows that uncertainty is thought of in terms of issues and sources (as identified by previous research), but also (possibly a domain-specific observation) as a lack of personal knowledge or skill. Uncertainty is accompanied by emotions of fear and shame. It arises during the diagnostic process, the treatment process and the outcome of medical decision making. The most frequently cited sources of uncertainty are partly lacking information and inadequate understanding owing to instability of information. Descriptions of typical group decisions reveal that the individual himself is a source of uncertainty when a lack of knowledge, skills and expertise is perceived. The group can serve as a source of uncertainty if divergent opinions in the decision making group exist. Three different situations of group decisions are identified: Interdisciplinary regular meetings (e.g. tumor conferences), formal ward meetings and ad hoc consultations. In all healthcare units concerned by the study, only little use of structured decision making procedures and processes is reported. Strategies used to handle uncertainty include attempts to reduce uncertainty by collecting additional information, delaying action until more information is available or by soliciting advice from other physicians. The factors which ultimately determine group decisions are hierarchy (the opinion of more senior medical staff carries more weight than that of junior staff), patients’ interest and professional competence. Important attributes of poor group decisions are the absence of consensus and the use of hierarchy as the predominant decision criterion. On the other hand, decisions judged to be effective are marked by a sufficient information base, a positive discussion culture and consensus. The authors identify four possible obstacles to effective decision making: a steep hierarchy gradient, a poor discussion culture, a strong need for consensus, and insufficient structure and guidance of group decision making processes. A number of intervention techniques which have been shown in other industries to be effective in improving some of these obstacles are presented.

Abstract:
New high precision lithogeochemistry and Nd and Hf isotopic data were collected on felsic rocks of the Rambler Rhyolite formation from the Ming volcanogenic massive sulphide (VMS) deposit, Baie Verte Peninsula, Newfoundland. The Rambler Rhyolite formation consists of intermediate to felsic volcanic and volcaniclastic rocks with U-shaped primitive mantle normalized trace element patterns with negative Nb anomalies, light rare earth element-enrichment (high La/Sm), and distinctively positive Zr and Hf anomalies relative to surrounding middle rare earth elements (high Zr-Hf/Sm). The Rambler Rhyolite samples have epsilon-Ndt = -2.5 to -1.1 and epsilon-Hft = +3.6 to +6.6; depleted mantle model ages are TDM(Nd) = 1.3-1.5 Ga and TDM(Hf) = 0.9-1.1Ga. The decoupling of the Nd and Hf isotopic data is reflected in epsilon-Hft isotopic data that lies above the mantle array in epsilon-Ndt -epsilon-Hft space with positive ?epsilon-Hft values (+2.3 to +6.2). These Hf-Nd isotopic attributes, and high Zr-Hf/Sm and U-shaped trace element patterns, are consistent with these rocks having formed as slab melts, consistent with previous studies. The association of these slab melt rocks with Au-bearing VMS mineralization, and their FI-FII trace element signatures that are similar to rhyolites in Au-rich VMS deposits in other belts (e.g., Abitibi), suggests that assuming that FI-FII felsic rocks are less prospective is invalid and highlights the importance of having an integrated, full understanding of the tectono-magmatic history of a given belt before assigning whether or not it is prospective for VMS mineralization.
