Abstract:
The purpose of this article is to contribute to existing literature about how activity theory might be used in music education research. It draws from the author's doctoral action research into the assessment of group composing for New Zealand's secondary school qualification, the National Certificate of Educational Achievement (NCEA). It outlines and explains how activity theory was used to examine three interacting activities: social jamming, group composing and achievement in the NCEA. Analysis revealed a relationship between students’ identities, their achievement in NCEA group composing, and socio-economic disparity.

Abstract:
Students who are underachieving in secondary school are likely to hold maladaptive motivation orientations that, unless changed, will have a negative impact on their future achievement. In this study 57 students from two schools with large Pasifika populations were offered supplementary teaching and learning opportunities via two different study skills programmes to improve their motivation and achievement. Participants (including 28 Pasifika students) were randomly assigned to either a traditional study skills (TS) or a motivation-enhanced study skills (MS) programme. NCEA results showed that students in the MS intervention attained more credits and showed significantly greater reduction in the negative motivation orientation uncertain control, compared to students in the TS skills programme. Students also reported that their relationships with their teachers and how their teachers communicated with them about learning was important to their motivation and achievement. Based on these findings, recommendations are made for teacher use of internally assessed NCEA standards as the context for interventions designed to enhance student motivation and achievement, in light of proposed changes to NCEA.

Abstract:
Characterizing the types of elder abuse and identifying the characteristics of perpetrators are critically important. This study examined the types of elder abuse reported to the National Center on Elder Abuse (NCEA) resource line. Calls were coded with regard to whether abuse was reported, types of abuse alleged, whether multiple abuse subtypes occurred, and who perpetrated the alleged abuse. Of the 1,939 calls, 818 (42.2%) alleged abuse, with financial abuse being the most commonly reported (449 calls, 54.9%). A subset of calls identified multiple abuse types (188, 23.0%) and multiple abusers (149, 18.2%). Physical abuse was most likely to co-occur with another abuse type (61/93 calls, 65.6%). Family members were the most commonly identified perpetrators (309 calls, 46.8%). This study reports the characteristics of elder abuse from a unique source of frontline data, the NCEA resource line. Findings point to the importance of supportive resources for elder abuse victims and loved ones.

Abstract:
This study looks at the impact that the National Certification of Educational Achievement (NCEA) has had on teacher collegiality in New Zealand. It is an exploratory study using an in case and cross case method, located in four secondary schools with a range of demographics. I was interested in gathering the information from teachers in three key roles: Assistant teacher, Head of Department and Principal's Nominee, finding out what their views were on the change that the NCEA has bought to their professional lives and the impact made on their collegiality. The literature reviewed shows there is an international appreciation of the value of collegiality in schools but there is a fragile nature of collegiality that challenges its strength. The complexity of school culture and the symbiotic relationship between it and collegiality contributes to challenge of the management and development in secondary schools. The findings showed the teachers in this study considered there to have been a deepening in collegiality as a result of increased sharing of material, professional communication through moderation and professional development, and a heightened respect for professional practice and understanding of personalities. There are threats from reduced socialisation, workload, loss of autonomy and the fragility of collegiality. These elements have created a shift in school culture. How teacher collegiality can best be supported using this assessment policy has been explored with features involving school organisation and increasing deep collegial activities such as collegial observation, marking, moderating and review being identified as beneficial.

Este estudo apresenta uma anÃlise empÃrica dos reflexos da implantaÃÃo do Decreto n 29.560/2008, no cenÃrio da arrecadaÃÃo do Estado. Tal decreto institui a cobranÃa de imposto atravÃs do regime de âSubstituiÃÃo TributÃria (ST)â pelo modelo Carga lÃquida em algumas empresas pertencentes ao âComÃrcio Atacadistaâ. O objetivo deste estudo à analisar o impacto do regime de SubstituiÃÃo TributÃria na arrecadaÃÃo do ICMS. Para isso, utilizaram-se dados da arrecadaÃÃo mensal dos contribuintes do Estado do CearÃ, totalizados por cÃdigo nacional de atividade econÃmica (CNAE), pertencente ao comÃrcio atacadista e ao decreto, no perÃodo de janeiro de 2005 a Dezembro de 2011, usando para isto o modelo economÃtrico de regressÃo linear mÃltipla com dados dos contribuintes relacionados na polÃtica do governo pelo decreto, arrecadaÃÃo mensal de ICMS do estado e para compor uma variÃvel dummy com objetivo de determinar o perÃodo em que os contribuintes foram atingidos pelo efeito do decreto, todos os valores foram deflacionados pelo IPCA, tomando por base o ano de 2005. ItÂs well known that new public policies were deployed in the state of Cearà in order to increase tax collection and so ensure a broad development in the state. By means of an empirical analysis, weÂre driving at the effects on the state revenue post-29.560/2008 law implementation, which establishes the tax charging under the label âtax replacingâ considering the so called net model. It is employed by some companies belonging to wholesaler settings. Since we take this view, we are committed to actually investigating the tax replacing effects upon ICMS, a local tax. A special econometric model is used for making this process clear: the multiple linear regression containing taxpayers data. We further remark that their month data were gathered concerning the period from January 2005 to December 2011. In the system here a variable dummy is taken to determine the period in which taxpayers were hit by decreasing effect, all values were deflated by the IPCA - AMPLO, based on the 2005 year. The results show a positive impact concerning tax substitution on the ICMS collection of Ceara state.

This thesis presents the findings of a project with the overarching aim of exploring the role of School Based Assessment (SBA) in mathematics as a component alongside external written examinations for qualifications. The research was carried out in New Zealand during the implementation of the National Certificate of Educational Achievement (NCEA). The NCEA was implemented in 2002. The NCEA is targeted at three levels: Level 1 for Year 11 (fifth form) to replace the School Certificate Examinations (SC); Level 2 for Year 12 (sixth form) to replace Sixth Form Certificate Examinations (SFC) and; Level 3 for Year 13 (seventh form) to replace University Bursaries (UB). The NCEA uses standards based methods of assessment as opposed to its predecessors (SC, SFC, and UB) which used norm referenced methods of assessment. The examination at each level is divided into internal and external assessments. The internal assessment called Internal Achievement Standards (assessed by teachers in schools) is an SBA component. It is this component of the NCEA Level 1 examination which formed the focus of the study. A qualitative approach was used in the research through interviews, school visits part of which involved participant observation in assessment situations, and participant observations at professional development (PD) workshops for teachers. Four different groups of participants were interviewed for this research: policy-makers at the New Zealand Ministry of Education (MoE) and the New Zealand Qualifications Authority (NZQA), The Mathematics Expert Panel, the facilitators of professional development (PD) workshops for teachers, and teachers of NCEA Level 1 mathematics. A total of 58 persons were interviewed. The perspective of each of these groups was necessary since each group had its specific role to play in the implementation of the NCEA. The policy-makers were asked to articulate the rationale for the SBA component of the NCEA. The Mathematics Expert Panel, which had decided on the areas of the secondary school mathematics best suited for SBA were asked to explain how this was done. The facilitators of the PD workshops recounted their experiences as they facilitated the teachers on how to assess against standards. Teachers who had the role of practical implementation were asked to articulate what they thought the rationale for an SBA was. They were also asked about the issues they confronted and the challenges they faced as they were introduced to a new regime of assessment. Both policy-makers and teachers are agreed that the rationale for an SBA component is to assess those objectives of the curriculum which are not appropriately, adequately and validly assessed in an external examination. A Framework was developed to gauge suitability of topics in the secondary school�s curriculum for SBA. The topics of the secondary curriculum found to be best suited for SBA are some parts of Geometry and Trigonometry, Measurement, and Statistics and Probability. The findings of this research provide adequate evidence that SBA has the potential for making a wider range of the school mathematics curriculum open to assessment. School Based Assessment also has the potential to get teachers at the secondary level to use pedagogical approaches that they rarely use when they teach for external examinations only. Facilitators of the PD workshops and the teachers of NCEA mathematics were of the opinion that the NCEA was too hurriedly implemented. Teachers confronted many teething problems and had many concerns during the first year of implementation. However, after three years, most of the teachers believe that the concerns they had during the first year are no longer issues for worry. There are implications for education systems wishing to implement innovative ideas that place heavy demands on teachers. The results here suggest that teachers need time before they are expected to change their normal manner of doing things in school. If adequate time is given at the initial stages then the process become a bit less hectic for them.

Introduction: Individuals who have experienced childhood maltreatment (CM) present a higher sensitive profile to stress. This profile have been associated with dysregulation of the immune system, characterized by reactivation of herpesvirus and increased cellular senescence markers. New traumatic or stressful experiences, such as breast cancer diagnosis, in those sensitized individuals can trigger an increase in stress levels possibly leading to important immunological changes. Here we investigate the presence of immunosenescence markers in women newly diagnosed with breast cancer with and without history of CM. Methods: Twenty-nine women newly diagnosed with breast cancer, without start the treatment, were recruited. Fifteen with history of CM (CM+) and fourteen without history of CM (CM-). Twenty-seven women without breast cancer and CM were selected as the control group. Peripheral blood was assessed by lymphocyte subsets by flow cytometry (B cells, CD4+, CD8+, NK cells, activated T cell, regulatory T cell, early and intermediated T cell, senescent and exhaustion T cells. CMV serology was determinate by ELISA method. Results: The groups CM+ and CM- present similar cancer stage and family history of breast cancer. In peripheral lymphocyte subpopulations we found significantly reduced Early-differentiated T cell (p <0.0001), while intermediate-differentiated T cell (p<0.0001), senescence T cell (p<0.0001) and exhaustion t cells (p<0.0001) were increased in CM+ and CM? patients. The mean fluorescent intensity (MFI) was analyzed and the CD27 expression on CD4 T cells was found lower in controls as compared to CM+ and CM? groups (p = 0.002).There was no difference of CMV IgG levels between CM + and CM? groups (p=0.24), but between controls and CM groups, this association becomes significant (p= 0.008) . Conclusion: Our findings suggest that the presence of CM is not directly related with immunosenescence in women newly diagnosed with breast cancer. However, when evaluated women with breast cancer and healthy controls, this senescent profile is evident. Future longitudinal studies are necessary to explore the role of senescent cells in the disease progression and treatment response.

Esse perfil est? relacionado com a desregula??o do sistema imunol?gico, sendo caracterizado pela presen?a de um aumento de marcadores de senesc?ncia celular e reativa??es de infec??es virais latentes. O acontecimento de novas experi?ncias traum?ticas, como o diagn?stico do c?ncer de mama, pode acarretar em um aumento dos n?veis relacionados ao estresse, provocando altera??es imunol?gicas importantes. Nesse estudo n?s investigamos a presen?a de marcadores de imunossenesc?ncia em mulheres rec?m diagnosticadas com c?ncer de mama com ou sem hist?rico de c?ncer de mama. M?todos: Vinte e nove pacientes rec?m diagnosticadas com c?ncer de mama foram recrutadas para esse estudo, sendo que quinze mulheres reportaram presen?a de maus tratos na inf?ncia (CM+), enquanto quatorze n?o vivenciaram situa??es envolvendo abuso ou neglig?ncia nesse mesmo per?odo (CM-). Para compor o grupo controle, foram selecionadas vinte e sete mulheres sem c?ncer de mama e que negaram a presen?a de maus tratos. Os linf?citos foram isolados das c?lulas mononucleares do sangue perif?rico (PBMCs), e imunofenotipados por citometria de fluxo para investigar a presen?a dos seguintes subgrupos: c?lulas B, CD4+, CD8+, c?lulas NK, c?lulas T ativadas, c?lulas T reguladoras, c?lulas T rec?m diferenciadas, c?lulas T intermedi?rias, c?lulas T senescentes e com perfil de exaust?o. A sorologia IgG para citomegalov?rus (CMV) foi realizada atrav?s do m?todo ELISA. Resultados: Os grupos CM+ e CM- n?o apresentaram diferen?as significativas em rela??o ao estadiamento da doen?a ou hist?rico familiar. Nas subpopula??es de linf?citos encontramos uma redu??o significativa de c?lulas T rec?m diferenciadas (p <0.0001), nos grupos CM+ e CM-, enquanto c?lulas T intermedi?rias (p <0.0001), senescentes (p <0.0001) e de exaust?o (p <0.0001) apresentaram-se aumentadas nos mesmos grupos. Ao analisar a m?dia da intensidade de fluoresc?ncia (MFI) encontramos uma express?o diminu?da de CD27 em c?lulas T CD4 nos controles quando comparado as mulheres com c?ncer de mama expostas e n?o expostas a maus tratos (p = 0.002). N?o observamos diferen?as entre os grupos CM+ e CM- em rela??o a sorologia IgG para citomegalov?rus entre os grupos CM + e CM - (p=0.24), entretanto foi presenciado entre os controles e indiv?duos com maus tratos (p= 0.008). Abstract:
The thesis provides a socio-historical perspective through which to evaluate the first five years of implementation (2002 to 2006) of social studies within the National Certificate of Educational Achievement [NCEA] in New Zealand secondary schools. The experiences of both lead educators, with responsibilities at a national level, and classroom teachers provide insights into the personal, contextual and institutional factors which have enabled and constrained the implementation process, especially at NCEA level one. The inclusion of social studies as a subject for the new qualifications and assessment system in New Zealand, the NCEA, heralded a significant opportunity for this integrated subject to gain academic status and acquire a unique identity within the senior secondary school curriculum. Paradoxically it set a relatively strongly framed assessment system beside a curriculum that has traditionally been weakly classified and framed (Bernstein, 1971). This paradox has created tensions for teachers who have responded in different ways, from full implementation to a more functional approach. Two groups of teachers were identified in the course of this study. The idealists are passionate advocates for senior social studies and are likely to implement it to all three NCEA levels in their school. The pragmatists, on the other hand, are more likely to offer only level one social studies, typically to their more academically able year 10 (Form 4) students for extension purposes, and also to induct them into the assessment requirements of the NCEA system before they study the traditional social science subjects at levels one, two and three. This pragmatic approach reflects past practices of the pre- NCEA, School Certificate era (1945-2001). It continues to reinforce the low status and unclear identity of senior social studies within the social sciences as well as within an already overcrowded senior school curriculum. At the end of this first five year period of implementation the viability of senior social studies is at a critical juncture, with its on-going success not yet assured.

The assistance to women who have breast cancer is studied in a Reference Center in Paraiba and also the way this assistance is performed in a School Hospital maintained by SUS (Single Health System) is questioned. Breast cancer demands institutional organization, provision of financial, material and human resources, requiring, from the health system, effective assistance with new technologies which make it possible for the population their access to specialized medical services although it not always is able to guarantee those services nor the rights which the legislation granted them, inhibiting a proper relationship between the health professional and the patient. The theme is discussed through a transdisciplinary knowledge view and has as its theoretical referential the contribution of classical and contemporary authors from the human and social sciences and, as an empirical research strategy, the structured interview. The objectives of the research were: identify how the assistance to women with breast cancer is carried on at a Reference Center on Oncology in Campina Grande, Paraiba, identifying their difficulties and their satisfaction with the received assistance; draw up a profile of the women with breast cancer who were assisted in this Reference Center; understand their gynecological and obstetric antecedents, life styles, age group and stage of the disease when the treatment started; check their knowledge about their rights and which benefits they had received. Most women ranged between 40 and 59 years old (63%), which corresponds to the risk range of developing breast cancer. As to their occupations, 38.3% were housewives and 30.1% retired, whose family income was among those who received between less than a minimum salary and one minimum salary (58.2%). This population was mainly constituted of married women (60.2%), whose most frequent schooling was an incomplete elementary school (27.6%) and complete elementary school (24.1%), which added up to 51.6%. It was observed that the majority of the women seemed to be satisfied with the assistance received, noting that a minimum care was enough to define this satisfaction, although it is perceived that the access to the health system does not ensure the ideal attention conditions they need; it was verified that the availability of the services and the assistance itself are seen (in the local culture) as a favor and not as a right. It is also observed that only 30% of the women mentioned that they knew about their rights and the most mentioned ones were the disease assistance (13%), the medicines (13%) and the treatment (12%), which represent the most important triad to face the disease and around which the oncologic assistance most focus on. It is concluded that the condition of the users? minimum existential of a public health unit and the condition of belonging to a lower social stratum were variables that influenced the respondents? satisfaction in relation to the assistance received but the importance of the Reference Center for the women with breast cancer?s assistance for the whole region cannot be denied as well as the need to broaden the way the policy of the oncologic assistance in Brazil in the local realm is seen
Estuda-se a assist?ncia a mulheres com c?ncer de mama, em um Centro de Refer?ncia da Para?ba, indagando-se como se d? a assist?ncia em oncologia, oferecida por um Hospital- Escola, que o mant?m atrav?s do Sistema ?nico de Sa?de (SUS). O c?ncer de mama exige organiza??o institucional, provimento de recursos materiais, humanos e financeiros, demandando ao sistema de sa?de assist?ncia eficiente e com novas tecnologias que possibilitem o acesso da popula??o aos servi?os m?dicos especializados, mas nem sempre consegue garantir esses servi?os, nem os direitos que a legisla??o conferiu ? popula??o, inibindo a adequada rela??o entre sistema de sa?de, profissional e paciente. Discute-se o tema em uma vis?o transdisciplinar do conhecimento, tendo como referencial te?rico aportes de autores cl?ssicos e contempor?neos das ci?ncias humanas e sociais em sa?de e, como estrat?gia de pesquisa emp?rica, a entrevista estruturada. Delimitaram-se como objetivos: identificar como ? realizada a assist?ncia a mulheres com c?ncer de mama em um Centro de Refer?ncia em Cancerologia, no munic?pio de Campina Grande, PB, identificando suas dificuldades e satisfa??o com a assist?ncia recebida; elaborar o perfil das mulheres com c?ncer de mama assistidas nesse Centro; conhecer seus antecedentes ginecol?gicos e obstetr?cios, estilos de vida, faixa et?ria e est?gio da doen?a quando iniciou o tratamento; verificar seus conhecimentos acerca dos seus direitos e quais os benef?cios recebidos. A maioria das mulheres encontrava-se na faixa et?ria entre 40 e 59 anos (63,1 %), correspondente ? faixa de risco para desenvolvimento do c?ncer de mama. Quanto ? ocupa??o, 38,3 % eram do lar e 30,1 % aposentadas, cuja renda familiar se concentrava entre aquelas que ganhavam menos de um sal?rio e um sal?rio m?nimo (58,2 %). Essa popula??o era constitu?da em sua maioria por mulheres casadas (60,2 %), com n?vel de escolaridade mais frequente no ensino fundamental incompleto (27,6 %) e fundamental completo (24,1 %), perfazendo um total de 51,6 %. Constatou-se que a maioria das mulheres mostrou-se satisfeita com a assist?ncia recebida, observando-se que um m?nimo de cuidado lhes foi suficiente para definir essa satisfa??o, embora se perceba que o acesso ao sistema de sa?de n?o garante as condi??es ideais para a aten??o de que necessitam; constatase que a disponibilidade de servi?os e de atendimento ? vista (na cultura local) como favor e n?o como direito. Observa-se ainda que apenas 30 % das mulheres mencionaram conhecer os seus direitos, sendo os mais citados o aux?lio doen?a (13 %), o medicamento (13 %) e o tratamento (12 %), que se apresentam como a tr?ade mais importante para o enfrentamento da doen?a, e em torno dos quais mais se foca a assist?ncia ?s mulheres no ?mbito da assist?ncia oncol?gica. Conclui-se que a condi??o de m?nimo existencial das usu?rias de uma unidade p?blica de sa?de e a condi??o de pertencimento a um baixo estrato social foram vari?veis que influenciaram a satisfa??o das entrevistadas com rela??o ? assist?ncia recebida, mas n?o se pode negar a import?ncia do Centro de Refer?ncia para a assist?ncia ?s mulheres com c?ncer de mama para toda a regi?o, bem como a necessidade de se ampliar o olhar em torno da pol?tica de assist?ncia oncol?gica no ?mbito local

This study is about the users' experiences of the services offered by League Against C?ncer that are in treatment of breast cancer, focusing on how the health/illness process develops. It is distinguished, in this context, the National Politics of Health, approaching the sprouting, implantation and consolidation process of the Unified Health System and its shocks on the installment of health services to the low-income population as a legit right from Citizen Constitution. It has as an objective to analyze how the social-economic extract of these women intervenes with such process, the aspects who involve the cancer while a pathology, approaching the signals, symptoms, forms of prevention, diagnosis, among other aspects that configure themselves as important points to the understanding of these experiences, since the diagnosis, treatment and control phases. The research was carried out with 25 women, between the months of January and February of year of 2005. The used technique was the scriptstructured interview, whose universe was defined through intentional sample. The following distinguishes as a result of the research: The majority of the women has its origins on the interior of the state, possesses a familiar income from one to three minimum wages and didn't make any kind of prevention before diagnosis, currently make treatment, beyond other aspects. It becomes necessary posterior studies on this social problematic, with respect to the personal, professional, familiar and social daily behavior of these women. It is worth mentioning that the role of the social assistant in the developed work with these women, understanding all the impediments associated with such experiences, as well as giving information about the rights of the patients with cancer, conquered through the years
Trata-se de um estudo sobre as viv?ncias das usu?rias dos servi?os da Liga Contra o C?ncer que est?o em tratamento de c?ncer de mama enfocando como se d? o processo sa?de/ doen?a. Destaca-se, neste contexto, a Pol?tica Nacional de Sa?de, abordando o surgimento, implanta??o e processo de consolida??o do Sistema ?nico de Sa?de e seus embates na presta??o dos servi?os de sa?de ? popula??o de baixa renda enquanto direito, a partir da Constitui??o Cidad?. Tem como objetivo analisar de que maneira o extrato s?cio-econ?mico dessas mulheres interfere em tal processo, os aspectos que envolvem o c?ncer, enquanto patologia, abordando os sinais, sintomas, formas de preven??o, diagn?stico, entre outros aspectos que se configuram como pontos importantes para o entendimento dessas experi?ncias, desde as fases de diagn?stico, tratamento e controle. A pesquisa foi realizada com 25 mulheres, entre os meses de janeiro e fevereiro de 2005. A t?cnica utilizada foi a entrevista estruturada a partir de um roteiro, cuja defini??o do universo se deu atrav?s da amostra intencional. The present study aimed to perform an epidemiological survey of oral cancer cases occurred in patients treated in S?o Vicente de Paulo Hospital (HSVP) Oncology Department (Passo Fundo RS Brazil) from January 1999 to December 2009. A research was performed by retrospective analysis of patients with a diagnosis of oral cancer evaluating gender, age and race of these patients, the location of mouth injuries and their clinical staging. The risk factors smoking and alcoholism, education level and occupation were also evaluated. During the study period, 307 patients were admitted with oral cancer in the HSVP. It was observed that 87,9% were male, 92,5% were caucasian, average age of 58,36 years old and 6th decade of life was more affected with 36,2% of patients. The more frequent anatomical location was the tongue (46%), and the histological type was the squamous cell carcinoma (93,5%). A majority of lesions was classified as stage IVa (32,6%) and surgery was the most performed treatment (51,1%). Most patients was low education level (63,5%) and farmer (33,6%). Smoking and alcohol consumption were found in 73,3% and 67,4% patients, respectively. Thus, it is concluded that the knowledge of these data is essential to get an oral cancer patients profile in our State, allowing identification of the risk factors and injuries? prevention, reducing the number of new cases and the early diagnosis and treatment of this disease.
O presente estudo teve por objetivo realizar um levantamento epidemiol?gico dos casos de c?ncer bucal em pacientes atendidos no Servi?o de Oncologia do Hospital S?o Vicente de Paulo HSVP (Passo Fundo - RS - Brasil) no per?odo de janeiro de 1999 a dezembro de 2009. Realizou-se uma pesquisa a partir da an?lise retrospectiva dos prontu?rios de pacientes com diagn?stico de c?ncer bucal, na qual foram avaliados os fatores: g?nero, idade e ra?a dos pacientes, al?m da localiza??o anat?mica das les?es na boca e seu estadiamento cl?nico. Os fatores de risco tabagismo e etilismo tamb?m foram avaliados, al?m da escolaridade e ocupa??o dos pacientes. Durante o per?odo estudado, 307 pacientes foram admitidos com c?ncer bucal no HSVP. Observou-se que 87,9% dos casos eram do sexo masculino, 92,5% eram da ra?a branca, com idade m?dia de 58,36 anos e que a sexta d?cada de vida foi a mais afetada com 36,2% dos pacientes acometidos. A localiza??o anat?mica preferencial foi a l?ngua (46%), e o tipo histol?gico mais frequente foi o carcinoma de c?lulas escamosas, representando 93,5% dos casos. A maioria das les?es encontrava-se em est?gio IVa (32,6%) e o tratamento mais frequente foi a excis?o cir?rgica (51,1%). A maioria dos pacientes apresentava baixa escolaridade (63,5%) e eram trabalhadores rurais (33,6%). Conclui-se, portanto, que o conhecimento desses dados ? essencial para obter um perfil dos pacientes portadores de c?ncer bucal na regi?o, possibilitando a identifica??o dos fatores de risco e a preven??o das les?es, viabilizando a redu??o do n?mero de novos casos e o diagn?stico e tratamento precoce da doen?a.

O c?ncer de mama pode ocasionar sintomas como dor, fadiga, diminui??o da capacidade funcional, ansiedade, estresse que podem comprometer a qualidade de vida (QV) das pacientes. Durante o processo de doen?a a espiritualidade pode ser uma ferramenta de enfretamento que possibilite uma melhor QV. Nesse contexto, o objetivo geral do estudo foi avaliar a influ?ncia da dor e da espiritualidade na QV de mulheres com c?ncer de mama. Trata-se de um estudo observacional, transversal e quantitativo, realizado no per?odo de julho de 2014 a abril de 2015, em um hospital de refer?ncia no tratamento do c?ncer em Natal/RN. Avaliou-se uma amostra de 400 mulheres com c?ncer de mama resultando em tr?s grupos: 118 sem met?stase (MTX), 160 com MTX locorregional e 122 com MTX ? dist?ncia. Foram aplicados os instrumentos um question?rio complementar abordando vari?veis sociodemogr?ficas e cl?nicas, European Organization for Research and Treatment for Cancer Quality of Life Questionnaire-Core 30 (EORTC QLQ-C30) e o m?dulo espec?fico Breast Cancer BR23 para avaliar QV, Spiritual Perspective Scale para avaliar espiritualidade e Short-Form McGill Pain Questionnaire para as pacientes com queixa ?lgica. Os dados foram analisados pela estat?stica descritiva e inferencial. O Teste de Fisher e Qui-quadrado utilizados para verificar a exist?ncia de rela??o de depend?ncia entre espiritualidade e vari?veis sociodemogr?ficas e cl?nicas; ANOVA verificou a associa??o entre a QV e as vari?veis cl?nicas, sendo o Teste de Tukey aplicado para ver a magnitude dessa associa??o. Como resultados, tivemos a alta preval?ncia de dor (71.7%) das pacientes e identificamos uma associa??o da presen?a de dor com QV. Na avalia??o da Sa?de Global (ESG) pelo EORTC QLQC30 o grupo sem MTX, m?dia 55.3 (SD=24.8) para as que tinham dor, 69.7 (SD=19,2) para sem dor (p=0.001); MTX locorregional, com dor tiveram score 59.1 (SD=21.3), e as sem dor 72.4 (SD=18.6) (p<0.001). No grupo MTX ? dist?ncia, as pacientes com dor m?dia 48.6 (SD=23.1) quanto 67.6 (SD=20.4) sem dor, (p=0.002). Quanto a espiritualidade, independentemente do estado da doen?a, as mulheres evang?licas demonstraram maior espiritualidade: no grupo sem MTX (p=0,013), com MTX loco-regional (p=0,011) e no grupo com MTX ? dist?ncia (p=0,009). Quando o n?vel de espiritualidade foi analisado associado as escalas EORTC, 39,8% das pacientes sem MTX tiveram maior n?vel de espiritualidade, tanto na Escala de Sintomas (p=0,04) quanto na ESG (p=0,02). Em conclus?o, temos que o c?ncer de mama foi associado com a diminui??o da capacidade funcional, comprometendo a QV das mulheres com c?ncer de mama; as pacientes com maiores n?veis de espiritualidade apresentaram maiores valores de QV, independentemente da presen?a ou n?o de MTX; e que a dor foi bastante prevalente na amostra, diminuindo a QV das mulheres com c?ncer de mama.
Breast cancer can lead to symptoms such as pain, fatigue, decreased functional capacity, anxiety, and stress which can compromise patients' quality of life (QoL). Through the illness process spirituality can be a coping resource that could be associated with better QoL. In this context, the overall study objective is to evaluate the influence of pain and spirituality on QoL in women living with breast cancer. This is an observational, cross-sectional and quantitative study, carried out from July 2014 to April 2015, in a hospital cancer center reference at Natal / RN. A sample of 400 women with breast cancer was evaluated in three groups: 118 without metastasis (MTX), 160 with locoregional MTX and 122 with MTX at a distance. A complementary questionnaire addressing sociodemographic and clinical variables and Instruments were applied: the European Organization for Research and Treatment for Cancer Quality of Life Questionnaire-Core 30 (EORTC QLQ-C30) and a disease-specific module for Breast Cancer BR23 to evaluate QoL, Spiritual Perspective Scale to assess spirituality and Short-Form McGill Pain Questionnaire was used to to describe pain. Data were analyzed by descriptive and inferential statistics. The Fisher and Chi-square tests were used to verify the existence of dependence relationship between spirituality and sociodemographic and clinical variables; ANOVA assessed the association between QoL and clinical variables, and the Tukey test was applied to indicate the magnitude of this association. The results showed a high prevalence of pain (71.7%) in patients and identified an association between the presence of pain and QoL. In the Global Health Scale (GHS) by the EORTC QLQC30 the group without MTX, mean 55.3 (SD=24.8) for those who had pain, 69.7 (SD=19.2) for no pain (p = 0.001); MTX locoregional, with pain had score 59.1 (SD=21.3), and those without pain 72.4 (SD=18.6) (p<0.001). In the MTX distance group, patients with mean pain 48.6 (SD=23.1) and 67.6 (SD=20.4) without pain (p=0.002). Considering spirituality, regardless of disease status, evangelical women showed greater spirituality: in the group without MTX (p=0.013), with locoregional MTX (p=0.011) and in the MTX group at a distance (p = 0.009). When the level of spirituality was analyzed associated with the EORTC scales, 39.8% of patients without MTX had a higher level of spirituality, both in the Symptom Scale (p=0.04) and GHS (p=0.02). In conclusion, we found that breast cancer was associated with decreased functional capacity, compromising the QoL of women with breast cancer; patients with higher levels of spirituality had higher values of QoL, regardless of the presence or absence of MTX, and that pain was very prevalent in the sample, decreasing the QoL of women with breast cancer.

Abstract:
Ischemic Stroke (IS) is a multifactorial disease characterized by symptoms resulting from arterial thrombotic processes. These cases are mainly caused by atherosclerosis, a chronic inflammatory disease that results from vascular injury and lipid deposition on the wall of the arteries. This study aimed to investigate the association between the polymorphisms Xba, I, I/D and Eco RI in the apolipoprotein B and PLA1A2 of the glycoprotein GPIIb / IIIa genes and ischemic stroke in a group of young patients by means PCR/RFLP technique. We evaluated 113 patients, 48 of them from Minas Gerais State and 65 from Rio de Janeiro State. The control group was composed by 291 healthy subjects from Minas Gerais State. There was no difference between patients from Minas Gerais and Rio de Janeiro when compared the frequency of each polymorphisms. However, it was observed a greater frequency of the E- mutant allele (Eco RI ) in the patients compared to the control group. The alleles X+, D, and A2 associated to predisposition to ischemic stroke showed frequencies significantly increased in patients only when the comparison was established to, at least, two polymorphism simultaneouly. In addition, presence of the allele A1 (GPIIb/IIIa), coupled to the absence of the E-mutant allele (Eco R1), showed to be strongly associated to a lower predisposition to the IS development. Thus, it can be concluded that the presence of E- mutant allele configures as a possible risk factor for development of IS.
O Acidente Vascular Cerebral Isquemico (AVCi) e uma doenca multifatorial, caracterizada por manifestacoes clinicas resultantes de processos tromboticos arteriais. Estes processos tem como principal causa a aterosclerose, um processo inflamatorio cronico resultante de lesao vascular e deposicao lipidica na parede das arterias. Este estudo teve como objetivo investigar a associacao entre polimorfismos Xba I, I/D e Eco RI no gene da apoB, PLA1A2 no gene da glicoproteina GPIIB/IIIA e AVCi em um grupo de pacientes jovens, atraves da tecnica de PCR-RFLP. Foram avaliados 113 pacientes, sendo 48 deles provenientes do estado de Minas Gerais e 65 do estado do Rio de Janeiro. O grupo controle foi composto por 291 individuos saudaveis provenientes do estado de Minas Gerais. Nao houve diferenca significativa entre os pacientes de Minas Gerais e do Rio de Janeiro quando comparada a frequencia de cada um dos polimorfismos. Foi observada uma maior frequencia do alelo mutante E- (Eco RI) no grupo de pacientes quando comparado ao grupo controle. Os alelos X+, D e A2 associados a predisposicao a doenca apresentaram frequencias significativamente maiores no grupo de pacientes apenas quando a comparacao foi feita com, pelo menos, dois polimorfismos simultaneamente. Alem disso, a presenca do alelo A1 (GPIIb/IIIa), combinada a ausencia do alelo mutante E- (Eco RI), mostrou-se fortemente associada a uma menor predisposicao ao desenvolvimento de AVCi. Assim, pode-se concluir para os polimorfismos estudados que a presenca do alelo mutante Econfigura-se como possivel fator de risco para desenvolvimento de AVCi.
