Abstract:
Intragastric balloons (IGB) have been used in the treatment of obesity for over 30 years. The first notable IGB device (the Garren Edwards Gastric Bubble) was withdrawn from the market due to patients’ poor weight loss and a high rate of complications. Several subsequent devices have been designed to address these shortcomings, but high-quality data are needed to compare complication rates among devices. Mortality across all IGB variants is extremely low compared to other bariatric procedures. Complications are mainly associated with visceral injury related to device insertion and retrieval, and with the presence or migration of an in-situ device, such as gastric ulceration and perforation, intolerance, gastrointestinal obstruction, and, rarely, acute pancreatitis. The incidence of postoperative venous thromboembolism (VTE) has not been extensively investigated, and it is not clear whether VTE is device-related or is due to increased VTE risk in bariatric patients.

Nanoscale devices are distinguishable from the larger microscale devices in their specific dependence on physical phenomena and effects that are central to their operation. The size change manifests itself through changes in importance of the phenomena and effects that become dominant and the changes in scale of underlying energetics and response. Examples of these include classical effects such as single electron effects, quantum effects such as the states accessible as well as their properties; ensemble effects ranging from consequences of the laws of numbers to changes in properties arising from different magnitudes of the inter-actions, and others. These interactions, with the limits placed on size, make not just electronic, but also magnetic, optical and mechanical behavior interesting, important and useful. Connecting these properties to the behavior of devices is the focus of this textbook. Description of the book series: This collection of four textbooks in the Electroscience series span the undergraduate-to-graduate education in electrosciences for engineering and science students. It culminates in a comprehensive under-standing of nanoscale devices—electronic, magnetic, mechanical and optical in the 4th volume, and builds to it through volumes devoted to underlying semiconductor and solid-state physics with an emphasis on phenomena at surfaces and interfaces, energy interaction, and fluctuations; a volume devoted to the understanding of the variety of devices through classical microelectronic approach, and an engineering-focused understanding of principles of quantum, statistical and information mechanics. The goal is provide, with rigor and comprehensiveness, an exposure to the breadth of knowledge and interconnections therein in this subject area that derives equally from sciences and engineering. By completing this through four integrated texts, it circumvents what is taught ad hoc and incompletely in a larger number of courses, or not taught at all. A four course set makes it possible for the teaching curriculum to be more comprehensive in this and related advancing areas of technology. It ends at a very modern point, where researchers in the subject area would also find the discussion and details an important reference source.

The anaesthetist will expect and rely on each medical device to work first time and not to compromise the safety of the user, the patient, their relatives, or other healthcare workers in the vicinity. The equipment will also be expected to be effective, that is, that it will perform as expected when used in a defined population of patients (e.g. small children). Manufacturers and users of equipment use risk management procedures to reduce the risk to patients and others of using the equipment. Following use, the equipment will need to be reprocessed to make it safe for use for a subsequent patient, or disposed of safely.

Organic electronics is a platform for very low cost and high performance optoelectronic and electronic devices that cover large areas, are lightweight, and can be both flexible and conformable to irregularly shaped surfaces such as foldable smart phones. Organics are at the core of the global organic light emitting device (OLED) display industry, and also having use in efficient lighting sources, solar cells, and thin film transistors useful in medical and a range of other sensing, memory and logic applications. This book introduces the theoretical foundations and practical realization of devices in organic electronics. It is a product of both one and two semester courses that have been taught over a period of more than two decades. The target audiences are students at all levels of graduate studies, highly motivated senior undergraduates, and practicing engineers and scientists. The book is divided into two sections. Part I, Foundations, lays down the fundamental principles of the field of organic electronics. It is assumed that the reader has an elementary knowledge of quantum mechanics, and electricity and magnetism. Background knowledge of organic chemistry is not required. Part II, Applications, focuses on organic electronic devices. It begins with a discussion of organic thin film deposition and patterning, followed by chapters on organic light emitters, detectors, and thin film transistors. The last chapter describes several devices and phenomena that are not covered in the previous chapters, since they lie outside of the current mainstream of the field, but are nevertheless important.

For all abdominal aortic aneurysm endografts, the major challenge is minimizing the risk of a type I endoleak. Percutaneous placement of an abdominal aortic endograft has become a widely-performed procedure. With several devices available on the market, there are many device-specific and experience-based considerations in planning and performing these procedures safely and with good outcomes. Although not always evidence-based, reviewing some case-specific scenarios can introduce techniques or lead to standards of practice that reduce suboptimal outcomes or prevent complications in future procedures. This chapter discusses deployment finesse of the Cook Zenith Flex and Zenith LP stent grafts, but many of the described concepts apply to other abdominal endografts and even thoracic endograft procedures.

Abdominal aortic aneurysms are a common pathology encountered by the interventionalist. Most endovascular repairs are performed with conventional bifurcated devices. However, there are situations in which the use of an aorto-uni-iliac (AUI) device is required because the use of a bifurcated graft is not feasible. Standard indications for use of an AUI include a narrow aortic segment precluding delivery and adequate expansion of a bifurcated graft, unilateral iliac occlusion, tortuosity, severe stenosis, and the presence of iliac aneurysmal disease. Occasionally, an AUI may be used for aortoiliac occlusive disease or as a bailout technique with conventional endografting. This chapter elaborates on the use of two bifurcated stent grafts for creation of an AUI endograft.

Abstract:
From a corpus of Greek epics known in antiquity as the “Epic Cycle,” six poems dealt with the same Trojan War mythology as the Homeric poems. Though they are now lost, these poems were much read and much discussed in ancient times, not only for their content but for their mysterious relationship with the more famous works attributed to Homer. This study shows that these lost poems belonged, compositionally, to essentially the same tradition as the Homeric poems. It demonstrates that various compositional devices well-known from the Homeric epics were also fundamental to the narrative construction of these later works. Yet while the “cyclic” poets constructed their works using the same traditional devices as Homer, they used these to different ends and with different results. The essential difference between cyclic and Homeric epics lies not in the fundamental building blocks from which they are constructed, but in the scale of these components relative to the overall construction of poems. This sheds important light on the early history of epic as a genre, since it is likely that these devices originally developed to provide large-scale structure to shorter poems and have been put to quite different use in the composition of the monumental Homeric epics. This study includes many new suggestions about the overall form of lost cyclic epics and about the meaning and context of the few surviving verse fragments.

Mechanical devices that include home appliances, automobiles, and airplanes are typically driven by electric motors or combustion engines through gearboxes and other linkages. Airplane wings, for example, have hinged control surfaces such as ailerons. Now imagine a wing that has no hinged control surfaces or linkages but that instead bends or warps to assume an appropriate shape, like the wing of a bird. Such a device could be enabled using an electro-active polymer technology based on electronic artificial muscles. Artificial muscles act directly on a structure, like our leg muscles that are attached by tendon to our bones and that through phased contraction enable us to walk. Sensory feedback from our muscles enables proprioceptive control. So, for artificial muscles to be used appropriately we need to pay attention not only to mechanisms for muscle actuation but also to how we can incorporate self-sensing feedback for the control of position.

From natural disasters to private funerals, digital media are playing a central role in the documentation and commemoration of shared significant events and individual loss experiences. Yet few studies have fully engaged with the increasing role mobile media play in making meanings related to traumatic events across different individual and collective contexts. Haunting Hands provides the first in-depth study into understanding the role of mobile media in memorialization and bereavement as a cultural and social practice. Throughout the chapters in this book, we explore how mobile devices are both expanding upon older forms of memory-making and creating new channels for affective cultures whereby the visual, textual, oral, and haptic manifest in new ways. Encompassing everything from phones to tablets, mobile media are not only playing a key role in how we represent and remember life, but also in how we negotiate the increasingly integral role of the digital within rituals in and around death. Haunting Hands posits how, during times of distress, mobile media can assist, accompany, and at times augment the disruptive terrain of loss. The book expands upon debates in the area of online memorialization in that the mobile device itself takes prominence, not only for its communicative or social function, but also for the ways in which it can contain as well as generate an intimate space within it. In this way, the device becomes an important companion for mobile-emotive grief as the bereaved engage with emotionally charged digital content in solitary, sometimes secretive, and sometimes shared ways.

Nonlocality was discovered by John Bell in 1964, in the context of the debates about quantum theory, but is a phenomenon that can be studied in its own right. Its observation proves that measurements are not revealing pre-determined values, falsifying the idea of “local hidden variables” suggested by Einstein and others. One is then forced to make some radical choice: either nature is intrinsically statistical and individual events are unspeakable, or our familiar space-time cannot be the setting for the whole of physics. As phenomena, nonlocality and its consequences will have to be predicted by any future theory, and may possibly play the role of foundational principles in these developments. But nonlocality has found a role in applied physics too: it can be used for “device-independent” certification of the correct functioning of random number generators and other devices. After a self-contained introduction to the topic, this monograph on nonlocality presents the main tools and results following a logical, rather than a chronological, order.

This article appears in theOxford Handbook of New Audiovisual Aestheticsedited by John Richardson, Claudia Gorbman, and Carol Vernallis. This chapter explores concepts of interactivity as they relate to sound production in video games. A guiding assumption of the chapter is that interactivity is a definitive paper of new digital aesthetics in general and gaming in particular. And yet, the question of interactivity has not been addressed with sufficient stringency in scholarly research. At the heart of the chapter are these questions: What makes interactive sound different from noninteractive sound? Where doesinteracting withsound fit into our understanding of our experience of sound and music in media? How do we begin to approach interactive sound from a theoretical perspective? The implications of interactivity are examined, specifically the notion of sound as a feedback device and as a control mechanism. . In these ways the chapter works toward a more comprehensive understanding of sounds in new media contexts that addresses their particularity in interactive contexts rather than resting on previous assumptions about the primacy of sounds as narrative devices.

If a quantum state completely specified the properties of a system to which it was assigned then application of quantum theory to an interaction intended to correlate properties of a measured system to those of a measuring device would leave that device recording no determinate outcome, contrary to what we observe. This is the quantum measurement problem. But the problem does not arise if the function of a quantum state assignment is not descriptive but prescriptive, so that all quantum state assignments are relational. Models of decoherence can certify the empirical significance of rival claims about which measurement outcome a device records, but their application does not explain but presupposes that exactly one such claim is true. The reality criterion which Einstein and colleagues applied to show the incompleteness of quantum description of reality is inapplicable to their chosen system while a slightly modified criterion is false.

Catheter-associated infections, which often present as sepsis, include primary bloodstream infections that occur in the presence of intravascular catheters. They are not related to an infection at another site and are defined as a primary bloodstream infection with documented colonization of the device and microbiologically proven, device-related bloodstream infection. Multiple hospitals have started to implement standardized quality control interventions to minimize catheter-related bloodstream infections. Ultrasound-guided line placement results in a decrease in mechanical complication and the number of attempts, which in turn reduces the risk of infection. Preparing the skin with 0.5% chlorhexidine or alcohol containing chlorhexidine solutions has been shown to reduce line-related infections. Antimicrobial lock solutions should be used in patients with recurrent catheter-associated infections and high-risk groups, such as those on total parenteral nutrition, dialysis, or oncologic patients. The preservation of skin integrity surrounding the device decreases the risk of infection.

There are two ways of envisaging ‘social investment’ today: with or against social protection. The first is to think of social investment as accompanying existing social protection; here, we need to devote serious consideration to what social protection actually is, as the mainstream English expression ‘welfare state’ does not suffice. The second way is to devise social investment as a vehicle to destroy social protection and to help make Mario Draghi’s 2012 remark come true—according to him the European social model ‘is already gone’. In the economics profession, the advocates of the second solution are more numerous. This chapter tries to assess the chances of the second conjecture by closely examining the notion of ‘social impact assessment’ (SII) that has picked up currency in European social policy debates since the onslaught of the global financial crisis.

These devices fall into two main categories: those involving the retention and those involving the transfer of title. Such financing devices are often called ‘quasi-security’ interests, to acknowledge that their economic function, in ‘securing’ the performance of obligations, is the same as that of true ‘security’. The chapter also considers the characteristics that they normally display, dividing up the discussion by the nature of the collateral involved: goods, investment products, and receivables.

Em março de 2020, a Organização Mundial da Saúde (OMS) comunicou ao mundo a pandemia pela proliferação do patógeno SARS-CoV-2 (novo corona vírus) que, ao infectar o ser humano causa a doença COVID-19 (SAÚDE, 2020). Devido ao fácil contágio, várias medidas de combate à pandemia foram divulgadas e implantadas, dentre elas o distanciamento social, a recomendação de fechamento dos estabelecimentos, o isolamento em casa, entre outras. Neste sentido, o curso de Fisioterapia do Centro de Ciências da Saúde e do Esporte (CEFID) da Universidade do Estado de Santa Catarina (UDESC), considerando a sua responsabilidade social e ética com a comunidade e principalmente com os usuários em atendimento, está buscando opções para fornecer aos pacientes, durante este período de excepcionalidade, uma forma alternativa de cuidado.

Abstract:
This chapter demonstrates how financing devices that involve either the retention or the transfer of title, although performing an equivalent function to security interests, are generally not considered ‘security’ under English law. Hence, the general characteristics that security interests display when they are enforced, in particular the obligation of the secured creditor to account for any surplus and the obligation of the debtor to make good any deficit, do not rise. The financing devices, being straightforward commercial contracts, are enforced according to their terms, without the application of any of those ‘security’ principles. However, in so far as these devices perform a security function, their express terms often reflect these ‘security’ characteristics, with the contract often providing for a financial adjustment so as to preclude the ‘secured creditor’ receiving a ‘windfall’ or suffering a ‘shortfall’.

Chapter 2 presents a kind of computation currently unfamiliar to most, the analog computer. Fifty years ago, they were considered viable competitors to the newer digital computer. Analog computers compute by the use of physical analogs, using, for example, voltages, currents, or shaft positions to represent numbers. They compute using the device properties, not logic. Examples include the balance, a simple device known for millennia; the “Antikythera mechanism,” a complex astronomical calculator from the first century BC; the slide rule; the US Navy’s Mark I fire control computer used for much of the 20th century to aim naval gunfire; and electronic analog computers built in large numbers after World War II. Analog computers can have advantages in ruggedness, simplicity, and reliability but lack the flexibility of digital computers.

A Editora Pantanal em seu 3º Volume do E-book “Tópicos nas ciências da saúde”, com seis capítulos traz novos temas no atuar da medicina. A obra, vem a materializar o anseio da Editora Pantanal na divulgação de resultados, que contribuem de modo direto no desenvolvimento e saúde humana. No primeiro capitulo o trabalho nos apresenta uma patologia que, embora muitos pensem que não é comum, tem grande impacto em nossa população mundial. A frequência desta patologia na década de 80 - 90 foi de 2 - 4 x 10.000 habitantes, porém com estudos atuais e levando em consideração não apenas o transtorno autista, mas todos os transtornos generalizados do desenvolvimento ou TEA (sigla em inglês), nesse novo cenário as estimativas aumentam de 21 para 35 x 10.000 habitantes. Com uma intervenção comportamental intensiva precoce, terapia cognitivo-confuctual e treinamento em habilidades sociais, obteve-se que em alguns casos leves os sintomas desaparecem, razão pela qual o diagnóstico precoce e o apoio incondicional da família são necessários; tudo isso refletido em nosso primeiro tópico. Em seguida, nosso pequeno volume faz uma incursão no campo das vitaminas que, como muitos estudiosos sabem, há um total de 13 vitaminas classificadas em dois grupos, solúveis em água (8 do complexo B e vitamina C) e quatro solúveis em gordura; A; D; E e K, que desempenham um papel fundamental no nosso organismo porque participam nos processos e reações que nele ocorrem e é importante não só tomá-los na forma de comprimidos, mas também incorporá-los através de uma alimentação equilibrada, saudável e saudável, para mim sobretudo a fonte da juventude porque atrasa o envelhecimento devido à sua ação antioxidante, aqui mostramos vários deles nas suas diferentes funções. Por fim, encerramos nosso livro com a apresentação de um caso onde mostramos que não é importante apenas tratar o somático, mas fazer um diagnóstico psicossocial do indivíduo se quisermos obter bons resultados em nossa prática profissional. Abstract:
Digital computers are built from hardware of great simplicity. First, they are built from devices with two states: on or off, one or zero, high voltage or low voltage, or logical TRUE or FALSE. Second, the devices are connected with extremely fine connections, currently on the order of size of a large virus. Their utility, value, and perceived extreme complexity lie in the software controlling them. Different devices have been used to build computers: relays, vacuum tubes, transistors, and integrated circuits. Theoretically, all can run the same software, only slower or faster. More exotic technologies have not proved commercially viable. Digital computer hardware has increased in power by roughly a factor of 2 every 2 years for five decades, an observation called Moore’s Law. Engineering problems with very small devices, such as quantum effects, heat, and difficulty of fabrication, are increasing and may soon end Moore’s Law.
