Chizick, Jarett. "Ankylosing Spondylitis & Chronic Pain Syndrome: Bridging the Gap Between Perpetuated Medicine & Holistic Therapies." ScholarWorks @ UVM, 2015. http://scholarworks.uvm.edu/graddis/329.

Full text
Abstract:
Ankylosing Spondylitis (AS) and Chronic Pain Syndrome (CPS) can be treated in many different ways. I found a problem in the balance of healing modalities surrounding diagnosis and care of illness and disease. This struggle is not singular to AS and CPS, but universal to physical and mental concerns. Some effective treatments and therapies are not recognized as such or are just beginning to become so. The scope of my work reflects on the course of my life. It was heavily influenced by the way my medical care was managed from an early age and how it evolved over the years. Through my educational program, I examined the necessity to bridge the gap between treatment paradigms and to expand on a broader, more inclusive, healing rubric. This rubric includes a broader emphasis on skill-based and complementary and alternative medicines. The viability to incorporate holistic health therapies earlier in life is explored through my use of the Scholarly Personal Narrative (SPN) qualitative research method. I chose this methodology because scientific fact could be argued either way for one therapeutic approach over another. By incorporating lived experience through SPN the union and cohesion necessary in all healing modalities, and their positive aspects, can be seen. The truth becomes self-evident. The results of this examination showed awareness earlier in life toward alternative and holistic treatments being paramount. Parents and educators lack information concerning modern therapeutic approaches. It also showed each situation will vary, but choice in treatment for ailments and illness of all kinds is not only viable, but highly recommended and researched. Access issues such as health insurance remain obstacles with some treatments and therapies, while others are a matter of cost prohibition, such as nutrition therapies. The implications of my work indicate a need for earlier incorporation of holistic healing programs and skill based therapies alongside perpetuated medical models in early childhood development and education. In conclusion, awareness towards medical concerns and how we as a society treat them can be improved upon by systemically incorporating less harmful therapies earlier in life. Fostering relations between medical providers, care providers and educators for students' wellbeing should be the foreground of any educational policy. Educators and parents alike should be made aware of and take advantage of effective skill-based treatments before a physical or mental condition surfaces or medication only approaches are authoritatively recommended. Integrating programs that build strong mental resilience and focus on youth development and education can reduce the necessity for more invasive treatments or medications should an ailment or illness develop.

Abstract:
The utility of narrative as a persuasive mechanism has been increasingly investigated in recent years especially within the context of health behaviors. Although many studies have noted the effectiveness of narrative-based persuasive appeals, conceptual inconsistencies have made it difficult to determine what specific aspects of narrative messages lead to the most effective persuasive outcomes. In the present study, 145 female college students were randomly assigned to read one of four narrative health messages about a female freshman college students experiences with the human papillomavirus (HPV). Two elements of the narrative message structure were manipulated: the message frame (gain framed vs. loss framed), and the grammatical person of the text (first-person vs. third-person). The messages were presented via the medium of an online blog. After reading a narrative participants responded to a brief questionnaire designed to measure perceptions of threat regarding HPV contraction, perceptions of efficacy regarding HPV prevention, and intentions to get the Gardasil vaccine. Participants exposed to loss framed messages reported higher levels of perceived threat (susceptibility and severity) than participants exposed to gain framed messages although participants in the gain framed message conditions reported higher levels of perceived self-efficacy. Significant correlations were also found between levels of reported character identification and the two threat variables. No effects were found for grammatical person.

Bryant, Cindy G. "Evaluating the use of Narrative Therapy and Art Therapy among Women Who Have Experienced the Trauma of Domestic Violence." Digital Commons at Loyola Marymount University and Loyola Law School, 2020. https://digitalcommons.lmu.edu/etd/906.

Full text
Abstract:
This research study is aimed at evaluating the artwork created by a woman who has experienced the trauma of domestic violence. A qualitative research approach was used to capture the phenomenological outcomes in the art created during art therapy sessions using the theoretical lens of narrative therapy to analyze the art. For this study the subject was a young woman from Central America who had come into therapy per court mandate to address the issues of domestic violence at a community-based mental health center where the researcher was conducting her second- year practicum. The data (art) was gathered after each art therapy session. Five sessions in total were conducted where the subject was given various art making directives. The subject created six total pieces of artwork during the five sessions. The researcher then analyzed the artwork created by the subject using a phenomenological approach while looking for common themes found in the artwork using a narrative therapy lens with which to view the artwork. Many themes uncovered in the artwork were parallel to narrative therapy theory such as resilience and empowerment and held the possibility of assisting the subject who experienced domestic violence in finding new outcomes.

Abstract:

This dissertation, broadly, examines how power dynamics manifest during clinical interactions related to vague and gendered medical symptoms, such as pelvic pain. To examine this issue, I approach my research questions through multiple methods including a critical discourse analysis of the medical discourse on pelvic pain, a survey experiment administered to healthcare workers in the US, and a narrative analysis of pelvic pain patient experiences. While the focus of this research is on pelvic pain, the analyses presented here reach far beyond ideas of power dynamics and pelvic pain. Rather, the findings from this research speak to theoretical discussions medical sociologists have been having for decades. Specifically, findings provide new insight on: 1) the limits of evidence-based medicine as a biomedical paradigm, 2) how fundamental causes of health inequality intersect with each other as well as other factors, such as gender, important in predicting health outcomes and 3) how discussions of metamechanisms in fundamental cause theory can inform our understanding of the accumulation of cultural health capital. In providing such insight, this dissertation uses the case of pelvic pain to integrate multiple perspectives and theories in medical sociology to drive the field forward in a way that acknowledges the many ways power is simultaneously constituted in the clinical interaction. From the role of gender, race, and class in power relations, to the ways medical knowledge, discourse, and authority dictate the clinical interaction, this research covers a wide range of sociological theories and concepts. In doing so, this dissertation sheds new light on current understandings of power in the clinical interaction and its relationship to inequitable health outcomes in the US.

Abstract:
Non surgical endodontic treatment consists of three steps: instrumentation, irrigation, and obturation. The obturation is a process that aims to seal in a three dimensional way the complex canal system of the tooth. The aim of this work is to perform a narrative review of the literature to analyze and compare two filling techniques: Continuous Heat Wave and Carrier Based, comparing the effectiveness, advantages and disadvantages of each technique reported in the current scientific literature. As a result of this analysis, we understand that there is no significant difference between the success of the two filling techniques, but that they have limits of use to be taken into account, such as certain characteristics of dental anatomy or the risk of extrusion of filling material. Thus, the choice of one technique or the other by the health professional will depend on anatomical factors and particularities of each clinical case.

Abstract:
The increase in life span of the human being leads to the appearance of new challenges for the health sciences. As a result of getting old, chronic and degenerative diseases started to occupy a predominant role in the development and clinical research in the last decades. It is the case of degenerative processes that occur in the oral cavity after teeth loss. Mandibular residual ridge resorption is a widely studied process from the middle of last century and many research relate this process with multiple factors. Recently, the understanding of the biological processes that occur after tooth extraction has been amplified. This work is a narrative revision concerning the genetic factors involved in mandibular residual ridge resorption (RRR). The relationship between some SNPs of some genes and RRR was stablished. All investigations point out for the need of studies with bigger sample sizes that allow the clinician to choose the best behaviour and have positive impact in treatments outcome based on the identification of genetic variations.

Abstract:
RESULTADOS: A CM promove mudanças físicas e genéticas. Abstract:
Soviet propaganda promised liberation of women from household shackles, glorifying them as ‘heroines’, who embody love for family, work and communist ideals. Behind ideology, the ‘second shift’ burdened mothers with tedious housework, childrearing, and professional workload. Nowadays their daughters, who were born in the turmoil of collapse of USSR, experience motherhood differently, with the aid of information and technologies, that seemingly ease childcare and everyday life in democratic Latvia. Although mothering is a subjective experience and each next generation questions decisions of the previous one, contemporary motherhood favours different childrearing methods, rooted in evidence-based sources, Western medicine practitioners, and democratized family models in contrast to Dr. Spock’s advice, home remedies or physical punishment. ‘Intensive mothering’ ideology adds to the pressures of modern motherhood, deeming the mother entirely responsible for social, psychological and cognitive well-being of her children. By employing the theoretical framework of Arlie Hochschild, this article explores the unspoken truths, doubts, and grievances of 21st century mothers towards their ‘mothers-heroines’ of USSR. The ‘deep story’ has been constructed, intertwining narratives, gained from eight phenomenological semi-structured interviews with new mothers. The ‘deep story’ has been supplemented by a case study of a viral post (Facebook, March 2021) by a contemporary mother, reflecting on advantages of modern motherhood in comparison to mothering in 1985, sparking a heated debate. The ‘deep story’ of contemporary mothers unfolds the layers of unarticulated feelings – from resentment to gratefulness, from anger to love. Inner conflict between respecting parents, and following an individual path is also present.

Abstract:
This paper reflects on the tribulations faced by the public school-going learners in Karachi during Covid-19. It also sheds light on the innovative pilot strategies that we used to support their education during the closure of the school. Through the reflective narrative case study approach, fellow teachers first gathered a few stories from learners and parents, living in their neighborhoods. They reported cases of child labor, slavery, and early marriages. We then discussed, planned, and piloted strategies to teach and support learners of grades 5 to 8 during Covid-19. We prepared educational packs that included educational games, assignments, and an easy-to-use guidebook. We also offered phone-based academic counseling to learners. Strategies like, taking parents and community leaders into confidence, and offering free vocational education, medical checkups, and medicines to parents and learners worked successfully. Around 17 girl children and 32 male children, who were engaged in child labor during Covid-19 returned to their routine education. Throughout the process, the role of community leaders was pertinent. Our contextual education model for marginalized children and parents can be used by other educational institutions during an uncertain time.

Abstract:
National Library of Medicine e do National Institutes Health (PubMed) e das bases de dados Web of Science.

Abstract:
Introduction: During primary breast cancer (BC) treatment, both systemic and local therapies are used to eliminate tumoral cells and reduce the risk of recurrence or disease progression. However, despite the efficacy and success of these treatments, most patients have their quality of life affected by some treatment-related side effects. Among them, fatigue and reductions in cardiorespiratory fitness are commonly observed in response to treatment toxicities during and even following primary treatment. To date, exercise has been considered an effective intervention to counteract these side effects. In the past few years, guidelines from the American Cancer Society, American College of Sports Medicine, and Exercise and Sport Science Australia were published highlighting the importance of being physically active before or after a cancer diagnosis. Recently, the Brazilian Clinical Oncology Society also started a new guideline in exercise and oncology. However, even with numerous studies demonstrating that exercise is effective, the relationship between its prescription variables and effects on these outcomes is unclear. Consequently, it is of great interest to understand the effects of different exercise modalities (e.g., resistance training, aerobic exercise or combined resistance, and aerobic exercise) and their effects on fatigue and cardiorespiratory fitness. Objective: The aim of this study was to describe the effects and moderators of exercise on fatigue and cardiorespiratory fitness in women with BC. Methods: This is a narrative literature review concerning the exercise effects and moderators of exercise response on fatigue and cardiorespiratory fitness in women with BC. The search was undertaken in PubMed using the following terms: “cancer” AND “exercise” AND (“fatigue” OR “cardiorespiratory fitness”) in November 2021. Given the specificity of the topic and outcomes of interest, we selected seven systematic reviews with meta-analysis to describe the exercise effects and moderators of exercise response on fatigue and cardiorespiratory fitness in BC patients. Results: In summary, the design of supervised exercise programs could benefit women with BC. In addition, exercise could result in greater effects in patients presenting higher levels of fatigue when compared to those who do not present. Some examples of supervised exercise programs are in studies from the Supervised Trial of Aerobic Versus Resistance Training (START), Combined Aerobic and Resistance Exercise (CARE), and Optimal Training Women with BC trials (OptiTrain). These studies prescribed resistance training, aerobic exercise, and combined resistance and aerobic exercise, 2–3 exercise sessions per week, 1–3 sets of 8–12 repetitions at 60–70% of one-repetition maximum (1RM) per resistance exercise, and 20–30 min of continuous or high-intensity interval aerobic exercise at 13–15 of the rated perceived exertion (RPE) scale. Regarding the exercise program prescription, supervised, high-intensity, or nonlinear schedule aerobic exercises are also associated with greater effects on cardiorespiratory fitness. The fact that supervised exercise results in greater benefits in cardiorespiratory fitness compared to unsupervised programs (supervised exercise, ES=0.34, 95%CI 0.28–0.40; unsupervised exercise, ES=0.19, 95%CI 0.07–0.32) is an important information. Conclusion: Sufficient evidence indicates that exercise promotes significant effects on fatigue and cardiorespiratory fitness in women with BC. In addition, specific subgroups of patients based on age and baseline levels appear to respond more favorably than others. Regarding contraindications, the exercise prescription should occur accordingly to and with the clearance of the oncologist and the medical team, respecting patients’ individualities, the feasible period for exercise, symptoms, and treatment course.

Abstract:

Abstract:

Abstract:
The research was a retrospective study of twenty-five Good Manufacturing Practice (GMP) inspection reports (from March 2017 through to December 2018) of a national medicine regulatory agency, drug Inspectorate, in West Africa, designed to assess the inspectors’ expertise in the act of inspection report writing. The investigation examined a paper-based tool of thirteen pre-registration Inspection reports and twelve GMP reassessment reports written prior and following an intervention program by external GMP trainers to enhance inspectors’ skill in pharmaceutical cGMP inspection. The study made use of quantitative analysis to investigate each team’s expertise in the act of writing GMP inspection report. Likewise, each report’s compliance with the requirements of three regulatory standards on GMP inspection report writing was ascertained. Impact of intervention program on lead inspectors’ competence was assessed. Lastly, gap in each team writing effectiveness, and lead inspectors’ abilities to deliver an effective report were determined. The results showed one of the inspection team (4.0%) wrote an excellent report. Two (8.0%) of the twenty-five inspection teams penned good inspection reports. Eleven (44.0%) teams drafted needs improvement reports and the remaining eleven teams (44.0%) prepared unacceptable reports. The excellent report and the two good reports had report format that meet expectation. One (50.0%) of the good reports showed the authors possess excellent knowledge of cGMP technical areas. The remain good report (50.0%) revealed the writers’ knowledge.as good. The excellent report showed the authors displayed partial mastery in the use of objective evidence while the two good reports disclosed theirs as having partial and evolving abilities. One of the teams (50.0%) that wrote good reports displayed good use of third person narrative past tense in report writing whereas the other team used the same tense and voice excellently. Generally, a sort of marginal level of performance was prominent among the inspection teams. A gap, if not tackled, will slow down regulatory process through increase report review, litigations that query report factual accuracy (AIHO, 2017) and delay in issuance of marketing authorization. In conclusion, trainings on quality attributes, such as technical content (Quality Management System (QMS) and Site), the use of objective evidence, assignment of risk levels to GMP violations and citing of applicable laws, regulation and guidelines that substantiate GMP observations, were recommended, to enhance knowledge sharing and regulators’ performance in the act of writing inspection report.
