To experimentally evaluate the impact of health-focused and environment-focused messages from the Meatless Monday campaign, we conducted an online randomized experiment among US adults aged 18 years or older (n = 1244). Abstract:
Enterocytozoon bieneusi is a common intracellular parasite that infects a wide range of hosts, including humans and companion animals, raising concerns of zoonotic transmission. However, there is limited epidemiological information on the prevalence and genotypes of E. bieneusi in sheltered dogs and cats in Sichuan province, southwestern China. A total of 880 fecal samples were collected from shelters in different cities of Sichuan province, including 724 samples from dogs, and 156 samples from cats. Enterocytozoon bieneusi was determined by sequence analysis of the ribosomal internal transcribed spacer (ITS). Overall, the prevalence of E. bieneusi was 18% (158/880), and the parasite was detected in 18.8% (136/724) and 14.1% (22/156) of the dogs and cats examined, respectively. Sequence analysis revealed the presence of five genotypes in dogs, including three known genotypes CD9 (n = 92), PtEb IX (n = 41), and Type IV (n = 1), and two novel genotypes SCD-1 (n = 1) and SCD-2 (n = 1). Similarly, four genotypes were identified in cats, including CD9 (n = 11), Type IV (n = 6), D (n = 4), and PtEb IX (n = 1). Genotypes D and Type IV have previously been identified in humans and are reported in sheltered dogs and cats in the present study, indicating that these animals could be as potential sources of human microsporidiosis infections.

Introduction Ponatinib inhibits the BCR::ABL1 kinase, including a variant with threonine-to-isoleucine at position 315 (T315I) (Cortes et al. N Eng J Med. 2013; 369:1783-96); however, it has been reported that ponatinib is associated with a higher risk of major arterial events than other TKIs among patients with CML, those with a resistance/intolerance (r/i) to prior TKIs, and with patients with relapsed/refractory (r/r) Ph + ALL. ( Leuk Lymphoma. 2016; 57(6):1300-10.) Owing to the low prevalence of this disease (Ning et al. Exp Hematol Oncol 2020; 9 (29)), SEER Cancer Statistics Review 1975-2018. National Cancer Institute 2021, Ching-Hong et al. N Engl J Med 2004; 350:1535-48), the amount of data concerning patients treated with ponatinib is limited, indicating that the safety and efficacy of ponatinib require further evaluation. Therefore, we conducted a post-marketing surveillance (PMS) study to assess the safety and efficacy of ponatinib, focusing on arterial occlusive events (AOEs), to optimize its use in clinical practice in Japan. Methods This PMS study investigated the safety and efficacy of ponatinib (ICLUSIG ®, Otsuka Pharmaceutical Co., Ltd., Tokyo, Japan) in patients with r/i CML who have been treated with TKIs and patients with r/r Ph + ALL. We collected clinical data from all patients in Japan who started ponatinib administration between November 21, 2016, and June 30, 2018. The observation period was two years (or until the end of ponatinib administration). AEs and ADRs were classified using the Medical Dictionary of Regulatory Affairs (MedDRA) version 25.0. Surveillance was conducted in accordance with the Japanese Ministerial Ordinance on Good Post-Marketing Study Practice for Drugs. Results A total of 807 patients were enrolled from 334 facilities. Safety analysis was conducted for all 724 patients; 421 (58.1%) participants were males with a sample median age of 62.0 years. The median (range) follow-up duration was 258.0 (1-1,436) days for the overall population, 729.5 (7-1,436) days for patients with CML-CP, and 168.0 (1-1,254) days for patients with Ph + ALL. As for safety data, adverse reactions (ADRs) were observed in 58.29% (422/724) of participants. The most common ADRs included platelet count decreases (8.15% - 59/724), hypertension (7.73% - 56/724), rash (5.56% - 41/724), decreased neutrophil count (4.83% - 35/724), and abnormal hepatic function (4.28% - 31/724). The most common serious ADR was a 2.49% (18/724) decrease in the platelet count. The incidence of AOEs was 6.49% (47/724) among the overall population, 6.22% (12/193) among patients with CML-CP, and 5.90% (23/390) among patients with Ph + ALL. The incidence of AOEs per 100 person-year was 6.8 events in total, 4.5 events in CML-CP, and 7.1 events in Ph + ALL. The incidence of cerebrovascular events was higher than that of cardiovascular events and peripheral vascular events in patients with CML-CP; there were no deaths related to AOEs in patients with CML-CP during the follow-up duration of PMS. Univariate logistic regression analysis revealed significant associations between AOEs and age (OR 1.039, 95% CI 1.017-1.062), hypertension (OR 2.137, 95% CI 1.169-3.907), and diabetes (OR 1.950, 95% CI 1.026-3.707). Regarding efficacy data, among the 89 patients with CML-CP who had not achieved MMR (MMR; BCR-ABL1transcripts [IS] ≤ 0.1%) before treatment with ponatinib, the cumulative MMR rates at weeks 12, 24, 52, and 104 were 22.8%, 36.9%, 58.8%, and 67.2%, respectively. Among the 19 patients with the BCR::ABL1 T315I mutation, the cumulative MMR rates at 12, 24, 52, and 104 weeks were 42.1%, 47.4%, 75.3%, and 75.3%, respectively. In patients with Ph + ALL, the CCyR rates at weeks 0, 12, 24, 52, and 104 were 35.4% (51/144), 77.1% (108/140), 73.6% (53/72), 83.6% (46/55), and 80.0% (36/45), respectively. Conclusions This two-year post-marketing surveillance of patients with r/i CML and r/r Ph+ ALL has demonstrated the general favorable safety and efficacy of ponatinib to patients in Japan. The main adverse reactions were those events reported in the clinical trials up until the time of approval. Furthermore, regarding AOE, no expression levels exceeding the results of the clinical trial were confirmed. However, patients with advanced age and who have predisposing factors for AOEs should be closely monitored for possible adverse vascular events.

Abstract:
Bamboo rats (Rhizomys sinensis) are widely farmed in Guangdong, China, but the distribution and public health potential of Cryptosporidium spp. in them are unclear. In this study, 724 fecal specimens were collected from bamboo rats in Guangdong Province and analyzed for Cryptosporidium spp. using PCR and sequence analyses of the small subunit rRNA gene. The overall detection rate of Cryptosporidium spp. was 12.2% (88/724). By age, the detection rate in animals under 2 months (23.2% or 13/56) was significantly higher than in animals over 2 months (11.2% or 75/668; χ2 = 6.95, df = 1, p = 0.0084). By reproduction status, the detection rate of Cryptosporidium spp. in nursing animals (23.1% or 27/117) was significantly higher than in other reproduction statuses (6.8% or 4/59; χ2 = 7.18, df = 1, p = 0.0074). Five Cryptosporidium species and genotypes were detected, including Cryptosporidium bamboo rat genotype I (n = 49), C. parvum (n = 31), Cryptosporidium bamboo rat genotype III (n = 5), C. occultus (n = 2), and C. muris (n = 1). The average numbers of oocysts per gram of feces for these Cryptosporidium spp. were 14,074, 494,636, 9239, 394, and 323, respectively. The genetic uniqueness of bamboo rat genotypes I and III was confirmed by sequence analyses of the 70 kDa heat shock protein and actin genes. Subtyping C. parvum by sequence analysis of the 60 kDa glycoprotein gene identified the presence of IIoA15G1 (n = 20) and IIpA6 (n = 2) subtypes. The results of this study indicated that Cryptosporidium spp. are common in bamboo rats in Guangdong, and some of the Cryptosporidium spp. in these animals are known human pathogens.

Disabilities and handicaps affect health-related quality-of-life (HRQOL) and dysphoria symptoms (helpless, hopeless, worthless, dissatisfied with life, depressed, suicidal). Archived 2009 Canadian Community Health Survey (CCHS) data (N=124,188) replicated 49 positive correlations (p<.001) of seven kinds of disability measured by the Health Utilities Index (HUI) with seven measures of dysphoria. Cognition and pain disabilities appeared most dysphoric. Individuals (N=724) in the 2009 CCHS data with HUI total scores less than 0.00 are defined by HUI protocols as “worse than dead.” This HUI categorization has doubtful validity based on 17 measures of disability, suicidality, self-perceived health, and social engagement.

Abstract:
Background Nigeria has low antiretroviral therapy (ART) coverage among HIV-positive pregnant women. In a previous cluster-randomized trial in Nigeria, Baby Shower events resulted in higher HIV testing coverage and linkage of pregnant women to ART; here, we assess outcomes of Baby Shower events in a non-research setting. Methods Baby Shower events, including a prayer ceremony, group education, music, gifting of a “mama pack” with safe delivery supplies, and HIV testing with ART linkage support for HIV-positive pregnant women, were conducted in eighty sites in Benue State, Nigeria. Client questionnaires (including demographics, ANC attendance, and HIV testing history), HIV test results, and reported linkage to ART were analyzed. Descriptive data on HIV testing and ART linkage data for facility-based care at ANC clinics in Benue State were also analyzed for comparison. Results Between July 2016 and October 2017, 10,056 pregnant women and 6,187 male partners participated in Baby Shower events; 61.5% of women attended with a male partner. Nearly half of female participants (n = 4515, 44.9%) were not enrolled in ANC for the current pregnancy, and 22.3% (n = 2,241) of female and 24.8% (n = 1,532) of male participants reported they had never been tested for HIV. Over 99% (n = 16,240) of participants had their HIV status ascertained, with 7.2% of females (n = 724) and 4.0% of males (n = 249) testing HIV-positive, and 2.9% of females (n = 274) and 2.3% of males (n = 138) receiving new HIV-positive diagnoses. The majority of HIV-positive pregnant women (93.0%, 673/724) were linked to ART. By comparison, at health facilities in Benue State during a similar time period, 99.7% of pregnant women had HIV status ascertained, 8.4% had a HIV-positive status, 2.1% were newly diagnosed HIV-positive, and 100% were linked to ART. Conclusion Community-based programs such as the faith-based Baby Shower intervention complement facility-based approaches and can reach individuals who would not otherwise access facility-based care. Future Baby Showers implementation should incorporate enhanced support for ART linkage and retention to maximize the impact of this intervention on vertical HIV transmission.

Zusammenfassung. Ziel dieser Arbeit ist die Analyse der psychometrischen Qualität des Fremd- und des Selbstbeurteilungsbogens zur Erfassung von Angst- und Zwangsstörungen (FBB-ANZ/SBB-ANZ) in zwei klinischen Stichproben (n = 182 bzw. n = 130). Ergänzend wurde die Faktorenstruktur der Bögen in zwei Feldstichproben (n = 724 bzw. n = 281) untersucht. Für die Skalen beider Fragebögen ergaben sich größtenteils zufriedenstellende interne Konsistenzen (.59 ≤ α ≤ .93). Exploratorische Hauptachsenanalysen mit Promax-Rotation ergaben stichprobenübergreifend für beide Fragebögen Faktoren, welche die a priori definierten, ICD-10- und DSM-IV-basierten Subskalen weitgehend abbilden. Zwischen den Subskalen innerhalb der Fragebögen zeigten sich geringe bis moderate Interkorrelationen, die für eine gute Abgrenzbarkeit der Skalen sprechen. Es fanden sich größtenteils moderate Beurteilerübereinstimmungen zwischen den korrespondierenden Skalen des Fremd- und Selbstbeurteilungsbogens. Die Ergebnisse weisen darauf hin, dass die Fragebögen überwiegend reliable und valide Instrumente darstellen. Weiterer Untersuchungsbedarf besteht insbesondere hinsichtlich der konvergenten und divergenten Validität der Subskalen.

A survey of approximately 4,000 questionnaires completed by parents of autistic children provided ratings on a variety of treatments and interventions. Among the biomedical treatments, the use of high-dosage vitamin B6 and magnesium (n = 318) received the highest ratings, with 8.5 parents reporting behavioral improvement to every one reporting behavioral worsening. Deanol (n = 121) was next most highly rated, with 1.8 parents reporting improvement to each one reporting worsening. Fenfluramine (n = 104) was third, with a ratio of 1.5:1. Thioridazine hydrochloride (Mellaril), by far the most often used drug on the list (n = 724), was fourth with a helped-worsened ratio of 1.4:1. The research literature on the use of vitamin B6-magnesium is briefly reviewed, and mention is made of recent findings regarding high-dosage folic acid in autism and biotin in Rett syndrome. Rumen nitrogen metabolism values were estimated by the use of a single injection of15(NH4)2SO4into the rumen of sheep and consecutive15N enrichment measurements in the rumen ammonia pool, rumen non-NH3-N (NAN) pool, rumen purine pool and blood urea-N (BUN) pool for a period of 24 h. Synthesis and degradation of N compounds in the rumen and passage of N to and from the rumen were evaluated on a chemical rather than a microbial basis; microbial fractions were not separated. This model was examined in two experiments. In Expt 1 a ram (55 kg) was given a semi-synthetic diet (1067 g dry matter (DM), 22.8 g N) in which soya-bean meal provided over 90% of the N. In Expt 2, two rams (45 kg) were given in three consecutive periods a semi-synthetic basal diet containing: (1) roasted soya-bean meal (SBM, 725 g DM, 14.8 g N/d); or (2) fishmeal (FM, 728 g DM, 15.5 g N/d); or (3) raw soya-bean meal (RSBM, 724 g DM, 13.8 g N/d). In all these rations, the main protein source provided over 90% of the N. In Expt 1, 68.3% of N intake was degraded directly to NH3in the rumen, 21.2% escaped rumen degradation and 10.5% was incorporated into stable N compounds in the rumen. Net NH3transfer to the blood was 30.4%, NH3flow from the rumen was 6.6% and rumen NAN output was 63% of N intake. In Expt 2, rumen NAN output was larger (7.67, 14.36 and 8.89 g N/d for diets containing SBM, FM and RSBM respectively;P< 0.05) and net NH3loss to the blood was smaller (6.1, 0.39 and 4.17 g N/d for diets SBM, FM and RSBM respectively;P< 0.05) for diet FM as compared with the soya-bean diets. The percentage of rumen NAN that was synthesized from NH3was larger for diet RSBM (36.4, 40.3 and 49.1 for diets SBM, FM and RSBM respectively;P< 0.05) than for the other two rations. NH3pool sizes (g N) were 0.463, 0.385 and 0.301 for diets SBM, FM and RSBM respectively (P< 0.05), while their hourly turnover rates were 15.8, 26.1 and 5.12 for diets SBM, FM and RSBM respectively (P< 0.01), indicating no correlation between pool size and its turnover rate. The existence of a small and rapidly turning over NAN pool to which NH3is first bound (SNAN1) was indicated by the difference in the shape of the NAN and purines15N-enrichment curves in the first 3 h after label injection. The fact that the rate of NH3initial binding to SNAN1 pool was related neither to ammonia pool size nor to SNAN1 pool size suggests the existence of an unknown control mechanism at this site.

Background Bone formation and loss are the characteristic clinical manifestations of leprosy, but the mechanisms underlying the bone remodeling with Mycobacterium leprae (M. leprae) infection are unclear. Methodology/Principal findings Osteocytes may have a role through regulating the differentiation of osteogenic lineages. To investigate osteocyte-related mechanisms in leprosy, we treated osteocyte-like cell with N-glycosylated muramyl dipeptide (N.g MDP). RNA-seq analysis showed 724 differentially expressed messenger RNAs (mRNAs) and 724 differentially expressed circular RNA (circRNAs). Of these, we filtered through eight osteogenic-related differentially expressed genes, according to the characteristic of competing endogenous RNA, PubMed databases, and bioinformatic analysis, including TargetScan, Gene Ontology, and Kyoto Encyclopedia of Genes and Genomes. Based on these results, we built a circRNA–microRNA (miRNA)–mRNA triple network. Quantitative reverse-transcription polymerase chain reaction and western blots analyses confirmed decreased Clock expression in osteocyte-like cell, while increased in bone mesenchymal stem cells (BMSCs), implicating a crucial factor in osteogenic differentiation. Immunohistochemistry showed obviously increased expression of CLOCK protein in BMSCs and osteoblasts in N.g MDP–treated mice, but decreased expression in osteocytes. Conclusions/Significance This analytical method provided a basis for the relationship between N.g MDP and remodeling in osteocytes, and the circRNA–miRNA–mRNA triple network may offer a new target for leprosy therapeutics.

The prevalence of toxigenic Clostridium difficile in Korea has been reported to be approximately 60–80 %. Although the prevalence of the tcdA−tcdB+ C. difficile strain was less then 5 % prior to the year 2000, it has become an emerging nosocomial pathogen in Korea. Therefore, we have attempted to determine the multicentre nationwide prevalence of tcdA+tcdB+ and tcdA−tcdB+ C. difficile for epidemiological purposes. C. difficile strains (n=724, 30 from 2000, 80 from 2001, 74 from 2002, 76 from 2003, 179 from 2004, 285 from 2005) were obtained retrospectively from January 2000 to December 2005 from in-patients at 6 hospitals, all of whom were suspected of having C. difficile-associated disease (CDAD), colitis or pseudomembranous colitis. The numbers of participating hospitals varied yearly (1 in 2000, 2 in 2001–2003, 3 in 2004, 5 in 2005). The hospitals were located in Seoul (n=4), Kyunggi Province (n=1) and Busan (n=1), Korea. PCR assays for tcdA and tcdB genes were conducted using 724 unduplicated C. difficile isolates. The mean prevalence of tcdA+tcdB+ and tcdA−tcdB+ C. difficile strains over the 6 years was 51.8 % (38.4–59.3 %) and 25.8 %(10–56.0 %), respectively. The mean prevalence of tcdA−tcdB+ C. difficile strains was less than 7 % until 2002, but began to increase in 2003 (13.2 %) and achieved a peak in 2004 (50.3 %). In 2005, the mean prevalence of tcdA+tcdB+ and tcdA−tcdB+ C. difficile strains was 47.7 % (30.9–60.3 %) and 27.0 % (17.6–54.8 %), respectively. This nationwide epidemiological study showed that tcdA−tcdB+ C. difficile strains have already spread extensively throughout Korea, and our results provide basic data regarding the controversies currently surrounding the toxigenicity of tcdA−tcdB+ C. difficile. The use of enzyme immunoassays capable of detecting both TcdA and TcdB is strongly recommended for the diagnosis of CDAD in microbiology laboratories, in order to control the spread of the tcdA−tcdB+ strains of C. difficile.

Data from the Alberta Agriculture Food and Rural Development data base, which keeps inventories of elk producers and velvet production information (n = 12 724), were used to estimate genetic and phenotypic variances, heritability and repeatability estimates for velvet antler using REML methods. The heritability based on the animal models I and II was 0.27 ± 0.03 and the repeatability ranged from 0.31 ± 0.02 to 0.35 ± 0.03 depending on the animal model used. Antler yield is expected to respond to selection as the heritability is medium, but non-additive and environmental effects can also influence production. SUMMARYThe prevalence and genotype distribution of human papillomavirus (HPV) infection in women with normal cervical cytology varies widely according to the population studied. Two non-overlapping population-based cohort studies of women aged ⩾30 years for the periods 2008–2009 (n=5026) and 2004–2005 (n=10 014) were analysed. The prevalence rate of HPV was 11·0% (95% CI 10·5–11·6). HPV infection was significantly associated with age, menopausal status, and inversely associated with hormone replacement therapy. There was an increasing trend of α3/α15, α5/α6, and multiple HPV infections with increasing age. The five most common types were HPV52, 18, 53, 58 and 70, while HPV16, 31, 33 ranked 21st, 25th, and 16th, respectively, in the merged cohort with normal cytology (n=14 724). HPV16, 31, and 33 were significantly associated with abnormal cytology, which could have resulted in their rarity in the total merged cohort (n=15 040).

Abstract The platelet integrin αIIbβ3 is the prototypic example of regulated integrin function. Thus, αIIbβ3 is present in a resting conformation on unstimulated platelets, but switches to an active conformation following platelet stimulation. Recent experiments suggest that disrupting a heteromeric interaction between the αIIb and β3 transmembrane (TM) and cytoplasmic domains shifts αIIbβ3 from its resting to its active conformation. However, structural information about the heteromeric interaction is sparse. Thus far, the structure of the TM heterodimer has only been studied by molecular modeling. Interactions between soluble cytosolic tail peptides have been studied by NMR spectroscopy, but these studies may not reflect native contacts because they fail to account for constraining TM domain interactions. To obtain an NMR structure for the αIIbβ3 cytosolic tail heterodimer that reflects its native structure, we expressed 13C- and 15N-labeled peptides corresponding to αIIb residues 988–1008 and β3 residues 713–762 in E. coli. Residues 987 in αIIb and 712 in β3 were replaced with cysteines, based on modeling that predicts the resultant disulfide bond will fix the peptides in their native orientation. Crosslinked heterodimers were dissolved in dodecylphosphocholine micelles at pH 6.5 and analyzed at 37°C on a 750 MHz NMR spectrometer. Previously, we presented a preliminary analysis of this construct indicating that when constrained by the proximal disulfide bond, the αIIb and β3 cytoplasmic tails interact and the cytosolic tail of β3 consists of three helices. We have now solved the final structure which defines the β3 interface that interacts with the αIIb cytoplasmic tail. The αIIb-β3 heterodimer interface is dynamic, but can be localized to β3 residues 716 and 719 because they have different chemical shifts in the crosslinked heterodimer than they do in the component monomers. This positions β3 residue 723 at the αIIb-β3 interface, consistent with the putative Arg995-Asp723 salt bridge. Interestingly, the αIIb tail is natively unstructured so a static interface for αIIb could not be identified. Additionally, the completed structure defines the relative orientations of the three β3 helices. The β3 cytoplasmic tail contains a sharp kink at residue 724 that fixes the membrane embedded helix (residues 713–723) and the first cytoplasmic helix (residues 725–736) at a right angle. The kink was defined by multiple NMR parameters including NOE distance restraints between residues 721 and 727. The distal cytoplasmic helix (residues 746–757) is related to the rest of the molecule by a flexible loop (residues 737– 745). N15 NOESY-HSQC crosspeak intensities provide evidence that the flexible loop and distal helix undergo increased motion relative to the first two helices, and the final structure reflects this motion because there is no preferred orientation for the distal helix relative to the first two helices. Lastly, the distal helix and flexible loop are joined by β3’s canonical NPXY motif which forms an N-terminal cap for the distal helix. In conclusion, we have solved the NMR structure of a disulfide-crosslinked αIIb/β3 cytoplasmic tail heterodimer. Our analysis indicates that, when constrained by a disulfide bond, the αIIb and β3 cytoplasmic tails interact, providing one mechanism for maintaining αIIbβ3 in a resting state.

We tested the validity and reliability of the Emotion Awareness Questionnaire (EAQ), and examined the correlation between emotional awareness and social anxiety with three samples of Chinese adolescent students. In Sample 1 (n = 724) we used exploratory factor analysis to determine the scale's construct validity, finding that the 26-item, sixfactor measure had high accuracy, and a subsequent confirmatory factor analysis indicated a good fit to the data. Sample 2 (n = 464) results show that the EAQ had good psychometric properties, and its concurrent validity was confirmed with the Trait Emotional Intelligence Questionnaire–Short Form as a criterion. Sample 3 (n = 216) results show that emotional awareness was significantly and negatively correlated with social anxiety. Cronbach's alpha internal consistencies in the three samples were .71, .74, and .73, respectively. The revised scale was determined to be reliable for examining emotional awareness among Chinese adolescents as well as the correlation between emotional awareness and social anxiety in this population.

Esse estudo objetivou realizar a prospecção de patentes das principais aplicações biológicas do gênero Jatropha. Para a realização da pesquisa, utilizaram-se as bases Instituto Nacional de Propriedade Industrial do Brasil (INPI), European Patent Office (Espacenet), United States Patent and Trademark Office (USPTO), World Intellectual Property Organization (WIPO) e os descritores A base da WIPO obteve maior número de patentes (n = 724), predominando a classificação C12N (n = 153). O número de patentes depositadas sobre o gênero de plantas Jatropha pode ser considerado alto quando pesquisado de forma geral (n = 1434), porém, ao se especificar apenas os efeitos farmacológicos do gênero, são encontrados poucos depósitos de patentes (n = 38), o que demonstra a falta de incentivo no registro de patentes direcionadas para essa tecnologia na área farmacológica, apesar de ser um campo de interesse comercial, assim como, um direcionamento das pesquisas científicas no atendimento às reais necessidades da sociedade e do setor econômico.

Nitric oxide synthase (EC 1.14.13.39) catalyses the conversion of arginine, NADPH and oxygen to nitric oxide and citrulline, using haem, (6R)-5,6,7,8-tetrahydro-L-biopterin (tetrahydrobiopterin), calmodulin, FAD and FMN as cofactors. The enzyme consists of a central calmodulin-binding sequence flanked on the N-terminal side by a haem-binding region that contains the arginine and tetrahydrobiopterin sites and on the C-terminal side by a region homologous with NADPH:cytochrome P-450 reductase. By using domain boundaries defined by limited proteolysis of full-length enzyme, recombinant haem-binding regions of rat brain neuronal nitric oxide synthase were expressed and purified. Two proteins were made in high yield: one, corresponding to residues 221-724, contained bound haem and tetrahydrobiopterin and was able to bind Nω-nitro-l-arginine (nitroarginine) or arginine; the other, containing residues 350-724, contained bound haem but was unable to bind tetrahydrobiopterin, nitroarginine or arginine. These results showed that rat brain neuronal nitric oxide synthase contains a critical determinant for arginine/tetrahydrobiopterin binding between residues 221 and 350. Limited proteolysis with chymotrypsin of the former protein resulted in a new species with an N-terminal residue 275 that retained the ability to bind nitroarginine, further defining the critical region for arginine binding as being between 275 and 350. Comparison of the sequences of nitric oxide synthase and the tetrahydrobiopterin-requiring amino acid hydroxylases revealed a similarity in the region between residues 470 and 600, suggesting that this might represent the core region of the pterin-binding site. The stoichiometries of binding of substrate and cofactors to the recombinant domains were not more than 0.5 mol/mol of monomer, suggesting that there might be a single high-affinity site per dimer.

9113 Background: EGFR mutations are present in more than 10% of patients (pts) with NSCLC in the US. While EGFR with tyrosine kinase inhibitors (TKIs) are effective, acquired resistance is expected. Known mechanisms include acquired EGFR mutations (e.g. 718V, c797x, 724s, 721s or T790M); copy number amplifications in MET, ERBB2, and PIK3CA; gene fusion events; and histological transformation. We herein present the prevalence of resistance mutations in the largest reported cohort of EGFR mutant NSCLC. Methods: Non-small cell lung cancer (NSCLC) tumor samples were submitted to Caris Life Sciences (Phoenix, AZ) for NextGen Sequencing (NextSeq, 592 Genes) and whole exome sequencing (NovaSeq, WES). PD-L1 expression was tested by IHC using 22c3 (Dako) and TPS scores were reported (cutoff >1). TMB was measured by totaling somatic mutations (TMB-high cut-off > 10 mutations per MB), genomic loss of heterozygosity (gLOH) was determined by WES. Patient treatment information was obtained from insurance claims data. Results: A total of 27,848 NSCLC tumors were evaluated and 3,223 (12%) had a EGFR sensitizing mutations. We found 60 tumors with common missense resistance mutations: 790 (n = 30, 0.9%), 797 (n = 38, 1.2 %), 718 (n = 11, 0.3%), 724 (n = 7, 0.2%) and 721 (n = 4, 0.1). Table 1 describes the frequencies, PD-L1 expression and the most common co-mutations. TMB-H (> = 10) was found in 12.5% of the tumors and dMMR/MSI-H in 1.8%. The most prevalent co-alterations were TP53 54%), gLOH (28%), CTNNB1 (19%), NFKB1A (13%), APC (10%), PIK3CA (11%), SMAD4 (9%) and other 15 co-mutations in less than 7% were seen. In the 30 T790M mutants, in addition to TP53 mutations, other prevalent co-mutations were PIK3CA (14%) and CTNNB1 (17%). In 797-mutant tumors, in addition to T790M, the most prevalent co-mutations were TP53 (53%), CTNNB1 (22%), APC (16%) and PIK3CA (11%). L718 mutations co-occurred with either L858R (8/11), exon 19 (3/11) or T790M mutations (3/11). G724 mutations were found in 7 patients (0.02%) and G721 mutations in 4 patients (0.01%). Conclusions: Acquired resistance in EGFR mutant NSCLC is very heterogeneous and their frequency is still low most likely due to lack of enough sequencing of EGFR resistant tumors. While T790M and C797S mutations are well described, this report also notes a significant number of L718V mutations, primarily in osimertinib-treated pts with an original L858R. These data support the NGS evaluation of patients with resistant EGFR mutant lung cancers. [Table: see text]

Receiver operating characteristic (ROC) curve analyses evaluated the sensitivity/specificity of the BASC-2-EF GSS at discriminating between children with/without ADHD or RD. Results: Area under the curve (AUC) scores indicated the sensitivity/specificity of the BASC-2-EF GSS at discriminating between children with/without ADHD (TRS: AUC = .831, p < .001; PRS: AUC = .919, p < .001), with/without RD (TRS: AUC = .724, p = .001; PRS: AUC = .615, p = .101), and with ADHD or RD through post hoc analysis (TRS: AUC = .674, p = .006; PRS: AUC = .819, p < .001). Conclusion: The findings support utilizing the BASC-2-EF GSS when differentiating ADHD from RD and typical development.

ABSTRACT Foscarnet is widely used for the treatment of acyclovir-resistant herpesvirus infections, and foscarnet-resistant herpesvirus infections are a serious concern in immunocompromised patients. Twenty-seven single-plaque isolates of herpes simplex virus type 1 (HSV-1) resistant to foscarnet were selected from foscarnet- and acyclovir-sensitive HSV-1 strain TAS by exposure to foscarnet, and the DNA polymerase genes were analyzed. The sensitivities of these mutants to foscarnet, cidofovir, S2242, acyclovir, ganciclovir, and penciclovir were determined. A single amino acid substitution, double amino acid substitutions, and a combination of a single amino acid substitution with a deletion or insertion of amino acid residues in the viral DNA polymerase were demonstrated in 21, 4, and 2 isolates, respectively. Of the 27 isolates, an amino acid substitution of serine for asparagine at amino acid position 724 in the DNA polymerase (724 S-N) was detected in 8 isolates. An amino acid substitution in conserved region II was demonstrated in these eight isolates as well as four other isolates. The mutation in the DNA polymerase responsible for resistance to foscarnet was located between the pre-IV region and conserved region V, especially within conserved region II. All the isolates were sensitive or hypersensitive to cidofovir and ganciclovir. Seven, 5, and 15 of the 27 isolates were also sensitive to S2242, acyclovir, and penciclovir, respectively. Thus, most of the foscarnet-resistant HSV-1 isolates were sensitive or hypersensitive to cidofovir and ganciclovir.

Background: Pyruvate kinase (PK) deficiency is a rare, congenital hemolytic anemia caused by homozygous or compound heterozygous mutations in the PKLR gene, with a prevalence of 3.2-8.5 individuals per million in Western populations. Over 350 pathogenic variants in the PKLR gene have been reported, with the majority being missense substitutions. Although their frequency differs across regions and populations, the geographic distribution of PKLR variants has not yet been fully investigated. Aim: To evaluate the PKLR genotype distribution by region among patients (pts) with PK deficiency in the merged real-world database of the Peak Registry (NCT03481738) and PK Deficiency Natural History Study (NHS; NCT02053480). Methods: The NHS was a global, longitudinal, observational study that enrolled pts with PK deficiency from 2014-2017 (study completed May 2020). The Peak Registry is an ongoing global registry which was initiated in 2018 to build on the NHS with expanded pt recruitment and geographic reach, broader data collection, and longer pt follow-up. Comparable data from these two studies were integrated to form a merged database providing enlarged sample size and prolonged follow-up from pts who participated in both studies. This analysis included pts with available genotype data from the NHS and Peak Registry, including those who were siblings; a large Amish cohort of pts carrying the homozygous p.R479H variant was excluded to avoid reporting bias. Genotype classifications were aligned between the two studies. Data were extracted from the merged database of NHS and Peak (data cutoff date: 13May2022) and summarized descriptively by region. Regions were defined as: Northern (N) Europe (Denmark, Ireland, Netherlands, UK); Central (C) Europe (Czech Republic, Germany, Switzerland); Southern (S) Europe (France, Italy, Spain, Portugal); Asia (S Korea, Thailand, Turkey); N America (Canada, US). Results: 362 pts were included in the analysis (N Europe, N=59; S Europe, N=62; C Europe, N=51; Asia, N=23; N America, N=167), 97 (27%) of whom were siblings. Most pts (72%) had compound heterozygous PKLR variants (Table). Homozygous mutations were most common among pts from Asia (15/23; 65%) and were least common among pts from S Europe (8/62; 13%). Almost half of pts from N Europe (49%) had a compound heterozygous missense/missense (M/M) genotype, while a similar proportion from S Europe (44%) had a compound heterozygous missense/non-missense (M/NM) genotype. A total of 724 PKLR variants were identified, including 197 unique variants, 26 of which were newly identified. Most were missense mutations (76%), followed by nonsense (10%), splice site (7%), and frameshift (5%) mutations. The majority of mutated alleles in the cohort were single nucleotide variants (90%), followed by small deletions (4%), large deletions (4%), and insertions (2%). The amino acid variant with the highest observed frequency was p.Arg510Gln, both overall (158/724; 22%), and in N Europe (31/118; 26%), C Europe (27/102; 26%), and N America (92/334; 28%), but it was not observed in Asia (0/46; 0%). The second most common variant overall, p.Arg486Trp (67/724; 9%), was the most frequent in S Europe (31/124; 25%). When stratified by genotype, p.Arg510Gln was the most common missense mutation in homozygous pts in each region (40-67%) except in Asia. In Asia, p.Thr157Ala, p.Lys541Asn, and p.Arg194Pro were the most common mutations overall (4/46; 9%), and also the most common missense mutations in homozygous pts (4/28; 14%). One hundred and seventy-four unique variants were observed in compound heterozygotes. In S Europe (27/102; 26%) and Asia (2/10; 20%), p.Arg486Trp was the most frequently observed missense mutation in compound heterozygote pts. Overall and in all regions except for Asia (where it was absent), p.Glu241Ter was the most common non-missense mutation in compound heterozygote pts (32/254; 13%); this was p.Gly540Ter (3/6; 50%) in Asia. Conclusions: This analysis, which used the largest real-world data source for pts with PK deficiency, confirms the genetic heterogeneity of the disease, showing that the mutation frequency in PKLR varies substantially based on geographic distribution and that the identification of novel genetic variants is relatively common in PK deficiency.

Las dificultades de regulación emocional se asocian con la ansiedad y problemas en relaciones sociales, entre otros, lo que sustenta evaluarlas y tener escalas adaptadas a cada contexto. El objetivo del presente estudio fue determinar la validez y fiabilidad de la Escala de Dificultades de Regulación Emocional (DERS) en muestras comunitarias colombianas y proporcionar baremos de calificación para la evaluación en Colombia. Se incluyeron 1435 participantes en dos estudios. En el primero (n = 724, Medad = 21.00; DTedad = 2.98) se llevaron a cabo un análisis factorial exploratorio y de fiabilidad, mientras que en el segundo (n = 711, Medad = 21.56; DTedad = 3.68) se realizó un análisis factorial confirmatorio. Los resultados mostraron una estructura de cinco factores, similar a la versión original, que explicó el 64.1% de la varianza total. El análisis confirmatorio reveló adecuados indicadores de ajuste y de fiabilidad superiores a .80. Los resultados sugieren que el DERS es válido y confiable para evaluar la desregulación emocional en muestras colombianas.

The efficiency of integrated microbial biotechnologies for obtaining several practically valuable metabolites in one technological process is determined both by the maximum concentration of these substances and their properties. This is especially true for secondary metabolites, the composition and properties of which vary depending on the cultivation conditions of the producer. Aim. To research the effect of tryptophan (a precursor of auxin biosynthesis) in the culture media on the synthesis of certain exometabolites by Rhodococcus erythropolis IMV Ac-5017, Acinetobacter calcoaceticus IMV B-7241, and Nocardia vaccinii IMV B-7405 as well as their properties. Methods. R. erythropolis IMV Ac-5017, A. calcoaceticus IMV B-724, and N. vaccinii IMV B-7405 were cultivated in a medium containing refined and waste sunflower oil, biodiesel waste, or ethanol as a carbon source. The concentration of tryptophan in the medium was 300 mg/L. Surfactants were extracted from the supernatant of the cultural liquid with a modified Folch mixture. Phytohormones were isolated from the supernatant by sequential extraction with organic solvents after surfactant extraction. Thin-layer chromatography was used for preliminary purification and concentration of phytohormones. Qualitative and quantitative determination of auxins was performed using high-performance liquid chromatography. The antimicrobial activity of surfactants was analysed by the minimum inhibitory concentration. The activity of enzymes of surface-active glycoand aminolipids biosynthesis (phosphoenolpyruvate synthetase, phosphoenolcarboxykinase, and NADP+-dependent glutamate dehydrogenase) was determined spectrophotometrically during the oxidation of NADH or NADP. Results. It was found that the presence of tryptophan in the culture medium of the strains under study did not affect the number of synthesized surfactants, which was 1.80−1.90, 1.55−1.75, and 1.50−1.65 g/L, respectively. At the same time, cultivation of R. erythropolis IMV Ac-5017, A. calcoaceticus IMV B-724, and N. vaccinii IMV B-7405 in the media with tryptophan increased the number of phytohormones: it was higher than the amount of phytohormones synthesized during cultivation without a precursor. The introduction of tryptophan into the culture medium of the strains was accompanied by the formation of surfactants. These compounds showed 2−4 times higher antimicrobial activity against the phytopathogenic bacteria (Agrobacterium tumefaciens UCM B-1000, Pseudomonas syringae UCM B-1027T, Xanthomonas vesicatoria UCM B-1106, Pectobacterium carotovorum UCM B-1075T, Clavibacter michiganensis IMV B-102 and Pseudomonas syringae pv. tomato IMV B-9167) than compounds synthesized on a medium without a precursor. The antimicrobial activity of surfactants synthesized by A. calcoaceticus IMV B-7241 in the presence of tryptophan either did not change compared to that for surfactants obtained without tryptophan, or increased slightly. Data on the activity of surfactant biosynthesis enzymes correlated with the indicators of their antimicrobial activity. In the presence of tryptophan in the culture medium of N. vaccinii IMV B-7405 and R. erythropolis IMV Ac-5017, NADP+-dependent glutamate dehydrogenase activity in the cells of these strains (a key enzyme for biosynthesis of aminolipids responsible for antimicrobial activity) increased almost by 1.4 times compared to that on a tryptophan-free medium. Conclusions. As a result of this work, it was found that the presence of tryptophan in the culture medium of researched strains did not affect the number of surfactants. The antimicrobial activity of surfactants against phytopathogenic bacteria either increased or remained unchanged compared to that established for surfactants synthesized without a precursor of auxin biosynthesis. The obtained data testify to the high efficiency of the potential use of surfactants complex preparations and phytohormones in crop production to stimulate the growth of plants and biocontrol of phytopathogenic bacteria.

BackgroundCurrent recommendations suggest that the diagnostic workup of Undifferentiated Arthritis (UA) should include radiographs of hands and feet (x-HF) as presence of Rheumatoid Arthritis (RA) associated erosions might be of diagnostic and prognostic value. The diagnostic role of x-HF in RA is reflected in ACR/EULAR 2010 criteria, with erosions typical for RA allowing classification of RA even without fulfillment of the scoring system [1]. Additionally, early erosivity is one of the indicators of a poor RA prognosis [2]. RA seems to evolve into a milder disease at presentation, caused by a secular trend towards milder disease, or lead time effects (presenting earlier in disease course) [3]. Both trends would further decrease the prevalence of RA-associated erosions and thus utility of x-HF might have decreased both for diagnosis and identifying poor prognosis patients.ObjectivesTo investigate the prevalence and diagnostic and prognostic value of RA-associated erosions on x-HF in a recent cohort of UA patients.MethodsA retrospective cohort study in consenting UA patients visiting the rheumatology department at the Sint Maartenskliniek between 2012-2018 was done. UA suspected for RA was defined as 1/ current arthritis, 2/ Rheumatoid Factor (RF)/anti-Cyclic Citrullinated Peptide (anti-CCP) were tested, 3/ x-HF was made, 4/ RA was mentioned in the differential diagnosis. Radiological reports of x-HF were judged by experienced musculoskeletal radiologists, and all x-HF with an erosion mentioned in the report were reassessed by two experienced rheumatologists for typical RA-associated erosions. The prevalence of erosions was estimated, as well as whether RA classification (yes/no) or prognostic group (good/poor) were changed by erosivity. Subgroup-analyses were done for seronegative patients (non-elevated RF/anti-CCP) and patients without elevated Acute Phase Reactants (APR; i.e. ESR and CRP) as they might be expected to gain the most diagnostic and prognostic value from conducting x-HF.Results724 patients were included in the analysis, of which 293 (40.5%) patients were eventually clinically diagnosed with RA during the time they were treated. There was a high agreement between 2010 ACR/EULAR classification criteria for RA and the clinical diagnosis reported by the treating rheumatologist (sensitivity of 79% and specificity of 89.6%). RA-associated erosions were found in 30 out of 724 included patients (4.1%, 2.9-5.9). Of the 437 seronegative patients, five (1.1%, 0.5-2.7) radiographs showed erosive disease. Eight out of 385 patients with non-elevated APR (2.1%, 1-4.1) had erosive disease. The x-HF resulted in a change to a positive RA classification in four out of 724 patients (0.5%, 0.2-1.2). X-HF erosivity changed the classification to prognostically unfavorable in five patients (0.6, 0.2-1.4),Table 1. Purpose: To investigate public knowledge, awareness and practice regarding COVID-19 in Saudi Arabia.Methods: A cross-sectional descriptive study was conducted, with a 16-item self-administered questionnaire. Data was collected using social media as the platform a day after the lockdown commenced in Saudi Arabia and data collection lasted from March 25 to April 25, 2020. Chi-square test was performed to determine the association between the variables.Results: The results showed that 96 % (n = 1505) of adults were aware of the clinical symptoms of COVID-19 and 57.9 % (n = 908) correctly identified that a stuffy nose and sneezing were less common in persons infected with the virus. A majority (95.4 %, n = 1495) agreed that there was no established therapeutic treatment for COVID-19. Further, 91.7 % (n = 1437) agreed that the virus could spread via respiratory droplets and only 46.2 % (n = 724) acknowledged that wearing face mask helped prevent transmission. The results also showed that 83.2 % (n = 1299) of the participants thought that visiting crowded places increased the likelihood of transmission.Conclusion: The findings of this study reveal that adults in Saudi Arabia are cognizant of COVID-19 and maintained good attitudes and practices regarding it. Furthermore, health education programs aimed at improving awareness of the disease and appropriate control measures will be beneficial in curbing the virus pandemic. Children exposed to violent war-like and repeated political violence often experience a continued threat to life and their sense of safety, as well as a disruption of daily functioning. The purpose of the study was to examine the psychological impact of exposure to Israeli occupation on Palestinian school children in the West Bank and Gaza, Palestine. We assessed the association between exposure to occupation and the severity of posttraumatic symptoms and the inter-relationship between posttraumatic symptoms, functional impairment, somatic complaints, and coping strategies in school children. Palestinian students ( n = 2100) from grades 9—11 were screened from both the West Bank ( n = 1235) and Gaza ( n = 724) and responded to self-report questionnaires. Results showed that extensive exposure to violence was associated with higher levels of posttraumatic distress and more somatic complaints in both the West Bank and Gaza regions. More Gaza than West bank students reported symptoms meeting the criteria for PTSD, and more girls than boys in both groups reported somatic complaints. Thus, school-based screening can be an effective method for case identification of students showing PTSD symptoms as a result of exposure to political violence.

IntroductionThe long-term safety and efficacy of lisdexamfetamine dimesylate (LDX) in children and adolescents with attention deficit/hyperactivity disorder (ADHD) was evaluated in a European 2-year, open-label study (SPD489-404).ObjectiveTo evaluate the time-course of treatment-emergent adverse events (TEAEs) in SPD489-404.MethodsParticipants aged 6–17 years received open-label LDX (30, 50 or 70 mg/day) for 104 weeks (4 weeks dose-optimization; 100 weeks dose-maintenance).ResultsAll enrolled participants (n = 314) were included in the safety population and 191 (60.8%) completed the study. TEAEs occurred in 282 (89.8%) participants; most were mild or moderate. TEAEs considered by the investigators as related to LDX were reported by 232 (73.9%) participants with the following reported for ≥ 10% of participants: decreased appetite (49.4%), weight decreased (18.2%), insomnia (13.1%). TEAEs leading to discontinuation and serious TEAEs occurred in 39 (12.4%) and 28 (8.9%) participants, respectively. The median (range) time to first onset and duration, respectively, of TEAEs identified by the sponsor as being of special interest were: insomnia (insomnia, initial insomnia, middle insomnia, terminal insomnia), 17.0 (1–729) and 42.8 (1–739) days; weight decreased, 29.0 (1–677) and 225.0 (26–724) days; decreased appetite, 13.5 (1–653) and 169.0 (1–749) days; headache, 22.0 (1–718) and 2.0 (1–729) days. Abstract In 2007, three mature hemi-boreal stands were selected from Järvselja forest district, South-East Estonia to establish one-hectare-large test plots for the international inter-comparison experiment of radiation models (RAMI). All trees with a stem diameter at breast height greater than 4 cm were mapped and measured in the field. In summer 2019, the forests were inventoried again. Here we present a summary of changes that occurred in the forest structure – mainly growth and mortality. In the birch stand basal area G has increased from 23.3 m2 ha-1 to 28.2 m2 ha-1 in the upper layer and the number of trees N has decreased from 654 to 565 ha-1. In the upper layer of spruce stand G has increased from 30.9 m2 ha-1 to 35.4 m2 ha-1 and N has decreased from 774 to 724 ha-1 and N substantially decreased in the lower layers from 912 to 577 ha-1. In the pine stand G has increased from 28.3 m2 ha-1 to 29.1 m2 ha-1 and N decreased from 1116 to 971 ha-1. The three test stands can be used now for validating remote sensing data-based estimates of forest inventory variables at single tree level.

OBJETIVO: Describir la experiencia de nuestro servicio en el empleo de la pancreatoduodenectomía u operación de Whipple para el tratamiento de Patologías malignas periampulares, su tiempo operatorio y la mortalidad operatoria. MATERIAL Y MÉTODOS: Se revisó la historia clínica de los pacientes sometidos a pancreatoduodenectomía durante 1994-97 en el Servicio de Cirugía V del Hospital Nacional Guillermo Almenara. RESULTADOS: Se encontraron 9 pacientes; el 56% (n=5) eran varones y el 44% (n=4) mujeres; el promedio de edad fue 69 años. El tiempo operatorio promedio fije 270 min (rango 360-724 min). Los diagnósticos histopatológicos fueron: 66,7% (n=6) adenocarcinoma de cabeza de páncreas; 11,1% (n=1) adenocarcinoma de la ampolla de Vater; 11,1% adenocarcinoma de tipo intersticial de duodeno; 11,1% adenocarcinorna primario de colédoco terminal. La técnica operatoria empleada fue resección de Whipple con anastomosis pancreático yeyunal termino-lateral en el 66,7% (n=6), anastomosis pancreático gástrica término-lateral en el 22,2% (n=2) y anastomosis pancreático yeyunal término-terminal con telescopaje en el 11,1%. La técnica de Whipple clásica sin conservación del píloro fije empleada en el 22,2% y la Whipple modificada con conservación de píloro en el 77,8% (n=7). En ningún caso se reoperó. El tiempo de hospitalización postoperatorio promedio fije 25 días. Mortalidad operatoria 22% a los 30 días de postoperatorio. CONCLUSIONES: Se utilizo preferentemente la operación de Whipple modificada con preservación de píloro, y en el manejo del muñón pancreático, las anastomosis al yeyuno.

The reasons for surgery were as follows: burst fracture of the first lumbar vertebra (n = 5), degenerative disc at L1–L2 (n = 4), spondylodiscitis at L1–L2 (n = 8) and at T11–L1 (n = 1), pathological fracture of the first lumbar vertebra (n = 2), and pseudarthrosis at T11–L1 after failed reconstruction (n = 1). The anterior surgical procedure was performed via an extracoelomic mini approach. Intersomatic fusion was performed in 13 patients, corpectomy and bone grafting in 4, corpectomy and vertebral replacement with titanium cage packed with bone cement in 3, and removal of a cage in a failed fusion and bridging the defect with a strut bone graft in 1. Posterior instrumentation of the affected segment was performed in the same sitting. RESULTS The mean ± standard deviation of operative time of the anterior procedure was 101.2 ± 36.5 minutes. The mean blood loss during the anterior procedure was 724 ± 483.5 ml. The procedure was safe. No pseudarthrosis was encountered, and the reconstructed bone was stable in the three patients in whom bone cement was used. CONCLUSION The extracoelomic mini approach is less invasive; it results in less incisional morbidity, and it avoids opening the pleural and peritoneal cavities.

In youths, two cut-offs (133 and 155 mg/dL) have been proposed to identify high glucose levels at the 1 h (G60) mark during an oral glucose tolerance test (OGTT). We evaluated which cut-off was more closely associated with isolated impaired glucose tolerance (IGT) and cardiometabolic risk (CMR) in 1199 youth with overweight/obesity (OW/OB) and normal fasting glucose and/or HbA1c. The disposition index (DI) was available in 724 youths. The sample was divided by two cut-offs of G60: <133 mg/dL (n = 853) and ≥133 mg/dL (n = 346), or G60 < 155 mg/dL (n = 1050) and ≥155 mg/dL (n = 149). Independent of the cut-off, youths with high levels of G60 showed higher levels of G120, insulin resistance (IR), triglycerides to HDL ratio (TG/HDL), alanine aminotransferase (ALT), and lower insulin sensitivity (IS) and DI than youths with lower levels of G60. The percentage of youths showing IGT, IR, low IS, high TG/HDL ratio, high ALT, and low DI was 50% higher in the G60 ≥ 133 mg/dL group than in the G60 ≥ 155 mg/dL one. In youths with OW/OB and IGT, a cut-off of G60 ≥ 133 mg/dL is more useful than G60 ≥ 155 mg/dL to identify those at high risk of IGT and altered CMR profile.

The covid-19 pandemic has affected the lives of every Filipino adolescent, adults, and LGBQ students. Knowledge of these affecting factors is limited. The study's primary purpose was to determine the level of stress among adolescents and adult students. Nine-hundred thirty (930) students participated in the study (First-year College and Junior High School students, n=724; Graduate Students, n=206). Result showed that among adult students which were grouped into gender: 30.67% male (n=23), 65.33% female (n=49), and 4% LGBQ (n=3) experienced a high level of Covid-19 stress-related symptoms, while 27.03% male (n=10), 67.57% female (n=25), and 5.41% LGBQ (n=2) scored lower level of stress. Likewise, among adolescent students, 32.32% male (n=83), 66.79% female (n=177), and 1.89% LGBQ (n=5) experienced high level of stress while 43.65% male (n=50), 52.64% (n=60), and 3.51% LGBQ (n=4) reported to have low stress level. Moreover, male adolescents and female adults were found to have higher Covid-19 stress-related symptoms. Consequently, k-means scores showed that 36.60% of adolescent students scored high (mean scores of 2.78-4.00) while 15.7% scored low (mean scores of .00-1.69). Meanwhile, among adult students, 36.4% scored a higher level of stress (mean scores of 2.81-4), and 45.6% reported having a lower level of stress (mean scores of .03-1.81). Also, adolescent students are prone to experience higher stress levels than adult students living in the Philippine region. Adult students may have established better coping mechanisms compared to adolescents. The researchers discussed the importance of designing and providing an evidence-based psychological intervention to address the participants' needs. A qualitative study may also be conducted to unveil the phenomenon of stress-related experiences among adults, adolescents, and LGBQ students to better explain the current study results

724 Background: Adjuvant FOLFOX is the standard of care for pts with stage III colon cancer and a platform to test new agents. Improvements in supportive care and development of newer treatments for metastatic disease impact the overall outcome of pts treated with adjuvant therapy. We examined the trend of disease-free survival (DFS), survival after recurrence (SAR), and overall survival (OS) of pts treated with FOLFOX over a ten-year period. Methods: Outcomes of colon cancer pts enrolled in 6 adjuvant trials receiving FOLFOX alone were analyzed. Pt enrollment period was dichotomized as old (1998 – 2003) vs. new era (2004 – 2009) based on the FDA approval date of bevacizumab. 3/5yr event-free rates were estimated by adjusted Kaplan-Meier methods. Outcomes were compared between the two eras by Cox model. To control for potential confounding effects, all analyses were adjusted for age, gender, performance score, T/N stage, tumor sidedness and histologic grade. Results: In total, 7,230 pts (stage II [n = 1122]; stage III [n = 6,108]) were identified; 32% and 68% were treated in the old and new era, respectively. Stage III pts enrolled in the new era were significantly younger, more likely to have T4/N2 disease, higher tumor grade, and left-sided tumors. After adjusting for pt characteristics, no difference in DFS was seen over time. However prolonged SAR and OS were observed in pts treated in the new era (Table). 5yr OS increased from 76% to 80% over time. Median SAR improved from 14.8 to 26.4 months. Conclusions: Stage III colon cancer pts who received adjuvant FOLFOX experienced significant improvements in SAR and OS—but not DFS—over time. In view of the improvements in OS and nearly doubled SAR, the optimal duration of OS follow-up to evaluate the benefits of adjuvant therapy (currently 5 years) should be reassessed, and ACCENT investigators are planning to conduct these additional analyses using their large pooled-trial database. 211 Background: Prevention of CINV in the delayed phase (24-120 h post-chemotherapy) and over multiple cycles of chemotherapy remains a challenge. NEPA, a fixed combination of the NK1 receptor antagonist (RA) netupitant (300 mg) and the 5-HT3 RA palonosetron (PALO; 0.5 mg), has demonstrated efficacy in multiple studies, in both acute and delayed phases, during the first cycle of moderately or highly emetogenic chemotherapy (MEC and HEC, respectively) regimens. Two clinical trials evaluated NEPA over multiple cycles of chemotherapy. We report data for the delayed phase for each cycle. Methods: Both studies were Phase 3, double-blind, active-controlled studies. In study 1 (MEC), patients were randomized 1:1 to receive a single oral dose of NEPA (n = 724) or PALO 0.5 mg (n = 725) on Day 1; following cycle 1, patients could participate in a multi-cycle extension phase. In study 2 (MEC or HEC), patients were randomized 3:1 to receive a single oral dose of NEPA on Day 1 (n = 309) or oral aprepitant (APR) 125 mg plus oral PALO 0.5 mg on Day 1, then APR 80 mg/d on days 2 and 3 of each cycle (n = 103). In both studies, all patients also received dexamethasone. Efficacy endpoints included complete response (CR; no emesis, no rescue medication) and no significant nausea. Results: In both studies, CR rates were consistently numerically higher with NEPA (Study 1 range: 77%-89%; Study 2 range: 83%-93%) than with PALO (Study 1; range: 69%-83%) or APR/PALO (Study 2; range: 78%-88%) in each cycle up to cycle 6 (Table). In both studies, rates of no significant nausea in the NEPA group were similar to or higher than in the control group. NEPA was well tolerated in both studies; treatment-related adverse events included constipation and headache. Conclusions: These studies demonstrate sustained efficacy of NEPA (administered as a single dose on Day 1) across multiple cycles of MEC or HEC for prevention of CINV in the delayed phase. Background Morphine reduces myocardial ischemia-reperfusion injury in vivo and in vitro. The authors tried to determine the role of opioid delta1 receptors, oxygen radicals, and adenosine triphosphate-sensitive potassium (KATP) channels in mediating this effect. Methods Chick cardiomyocytes were studied in a flow-through chamber while pH, flow rate, oxygen, and carbon dioxide tension were controlled. Cell viability was quantified by nuclear stain propidium iodide, and oxygen radicals were quantified using molecular probe 2',7'-dichlorofluorescin diacetate. Results Morphine (1 microM) or the selective delta-opioid receptor agonist BW373U86 (10 pM) given for 10 min before 1 h of ischemia and 3 h of reoxygenation reduced cell death (31 +/- 5%, n = 6, and 28 +/- 5%, n = 6 [P &lt; 0.05], respectively, 53 +/- 6%, n = 6, in controls) and generated oxygen radicals before ischemia (724 +/- 53, n = 8, and 742 +/- 75, n = 8 [P &lt; 0.05], respectively, vs. 384 +/- 42, n = 6, in controls, arbitrary units). The protection of morphine was abolished by naloxone, or the selective delta1-opioid receptor antagonist 7-benzylidenenaltrexone. Reduction in cell death and increase in oxygen radicals with BW373U86 were blocked by the selective mitochondrial KATP channel antagonist 5-hydroxydecanoate or diethyldithiocarbamic acid (1,000 microM), which inhibited conversion of O2- to H2O2. The increase in oxygen radicals was abolished by the mitochondrial electron transport inhibitor myxothiazoL Reduction in cell death was associated with attenuated oxidant stress at reperfusion. Conclusion Stimulation of delta1-opioid receptors generates oxygen radicals via mitochondrial KATP channels. This signaling pathway attenuates oxidant stress and cell death in cardiomyocytes.

724 Background: Small bowel adenocarcinoma (SBA) is a rare malignancy with poor prognosis. Data describing the clinical and pathologic characteristics of this disease are sparse, and few studies examine the genetic footprint of SBAs within the continuum of the gastrointestinal tract. Methods: All patients with small bowel adenocarcinoma for whom primary tumor tissue was available from 1993 to 2021 at six institutions were included. A hidden genome classifier (HGC) was developed based on genomic features from 286 gastroesophageal cancers (foregut) and 286 colorectal cancers (hindgut) using targeted tumor sequencing. The SBA samples were run through the HGC to obtain the predicted probability of either foregut or hindgut lineage. For patients submitted to curative intent resection, overall survival (OS) was calculated from 90 days post resection until date of last follow up. Cox regression was used to examine factors associated with OS and to construct the multivariable model. Results: Of a total of 243 patients with SBA, 60% were male. The most common anatomic locations were duodenum (53%), jejunum (28%), and ileum (12%). HGC prediction of foregut lineage was higher for duodenal (55%) and intestinal-type periampullary (64%) tumors, while prediction of hindgut lineage was higher for ileal (68%) and jejunal (54%) tumors. Patients were then divided into three HGC prediction groups based on the lowest-, highest-, and inter-quartile ranges: foregut (n=61), characterized by gene amplification of CDK12, CD3, EGFR, and KRAS and cytoband segmentation of chromosomes 8-p21.1 and 15-q26.3; hindgut (n=61), characterized by APC and KRAS mutations; and mixed-type (n=121) with elements of both. Among those undergoing curative-intent surgery (n=160), median OS was 73 months. On multivariable analysis, HGC prediction group (hindgut vs mixed-type, HR 3.22, 95%CI 1.54-6.73), age (HR 1.03, 95%CI 1.01-1.05) and positive lymph node status (HR 1.96, 95%CI 1.02-3.76) were associated with decreased survival (Table). Neither anatomic site nor the presence of other driver mutations was associated with survival. Conclusions: Small bowel adenocarcinomas display genomic heterogeneity. A novel hidden genome classifier that stratifies SBAs based on homology to foregut or hindgut genomic alterations may be superior to anatomic location for characterizing disease biology. A total of 766 mosquitoes containing four species: Culex quinquefasciatus (n=724; 94.5%), Anopheles gambiaes.l. (n=37; 4.8%), Culexannulioris (n=4; 0.5%) and Anopheles funestus (n=1; 0.1%) in descending order of importance, were collected. Cx.quinquefasciatus was the most predominant mosquito species in school dormitories across the three LGAs. Cx. annulioris and An. gambiaes.l were most abundant in school dormitories located in Oredo and Ikpoba-okha LGAs respectively. An.funestus occurred only in school dormitories located in Oredo LGA. Analysis of the relationship between meteorological data and species abundance varied significantly (P<0.05) only among An. gambiaes.l with rainfall. Scholars have recently started to integrate personality traits into models of political participation. In this paper, we present the results of a survey experiment (N = 724) designed to test whether negative political messages differentially impact people with different personality traits. We found evidence that individuals with high scores on agreeableness were less likely, and individuals with high scores on extraversion were more likely, to report intending to participate in politics than their counterparts after being exposed to negative political messages. Agreeableness and extraversion also interacted with negative messages to influence specific intentions to make a political donation, attend a meeting, rally, or event, and volunteer for a political campaign. We also found suggestive evidence that agreeableness interacted with negativity to influence turnout intentions. The results of this study have important implications for the study of political engagement, the ways in which people interact with political information, and the practice of democratic politics.

Nail-biting is one of the most common behavioral problems in children. This study aimed to examine factors affecting nail-biting among adolescents and the effects of psychological state and social support on nail-biting. This cross-sectional study was conducted between January and May of 2014 in seven schools in Istanbul ( N = 724). Data were collected using the Personal Information Form, Brief Symptom Inventory, and Child and Adolescent Social Support Scale. This study found that 48.2% of adolescents in the 11- to 17-years-old age group had a habit of nail-biting. Anxiety, depression, negative self image, somatization, hostility, and three global index scores of adolescents who bit their nails were significantly higher than in those who did not. Social support scores of those who bit their nails were significantly lower than in those who did not in the ‘mother’ and ‘classmates’ subscales and total scores. Psychological state and social support were factors affecting nail-biting among adolescents.

ORP150 (oxygen-regulated protein 150) is a chaperonin expressed in tissues undergoing hypoxic or endoplasmic reticulum stress. In the present study, we investigated plasma levels of ORP150 in patients with AMI (acute myocardial infarction) and its relationship with prognosis, together with a known risk marker N-BNP (N-terminal pro-B-type natriuretic peptide). Plasma from 396 consecutive patients with AMI was obtained for measurement of ORP150 and N-BNP. Mortality and cardiovascular morbidity (acute coronary syndromes/heart failure) was determined during follow-up. A specific ORP150 assay detected the 150 kDa protein in plasma extracts, including 3 and 7 kDa fragments. During follow-up (median, 455 days), 43 (10.9%) patients died. Both N-BNP and ORP150 levels were higher in those who died compared with the survivors [N-BNP, 724 (14.5–28840) compared with 6167 (154.9–33884) pmol/l (P<0.0005); ORP150, 257 (5.9–870.9) compared with 331 (93.3–831.8) pmol/l (P<0.001); values are medians (range)]. In a Cox regression model for mortality prediction, both N-BNP (odds ratio, 5.06; P<0.001) and ORP150 (odds ratio, 2.39; P<0.01) added prognostic information beyond creatinine and the use of thrombolytics. A Kaplan–Meier survival analysis revealed that ORP150 added prognostic information to N-BNP, especially in those with supra-median N-BNP levels. A simplified dual-marker approach with both markers below and either above or both above their respective medians effectively stratified mortality risk (log rank statistic for trend, 32.7; P<0.00005). ORP150 levels were not predictive of other cardiovascular morbidity (acute coronary syndromes or heart failure). In conclusion, ORP150 and peptide fragments derived from it are secreted following AMI and provide independent prognostic information on mortality. High levels associated with endoplasmic reticulum/hypoxic stress predict a poor outcome.

Magnetic-activated cell sorting (MACS) can be used to separate apoptotic sperm with high proportions of fragmented DNA from the rest, thus improving the overall quality of the seminal sample. Therefore, the aim of this retrospective study was to investigate the efficiency of the MACS technique to increase reproductive outcomes in patients with high levels of sperm DNA fragmentation (SDF) undergoing intracytoplasmic sperm-injection (ICSI) cycles. In this study, we analyzed a total of 724 assisted-reproduction-technique (ART) cycles that were divided into two groups: the study group (n = 366) in which the MACS selection technique was performed after density-gradient centrifugation (DGC), and the control group (n = 358) in which only DGC was used for sperm selection. Reproductive outcomes were analyzed in both groups according to three different ART procedures: preimplantation genetic testing for aneuploidy (PGT-A), and autologous and oocyte-donation cycles. The MACS group showed significantly lower miscarriage rates in autologous ICSI cycles, higher pregnancy rates in oocyte-donation cycles, and a significant increase in live-birth rates in both autologous and oocyte-donation cycles. Overall, these results suggested that the MACS technique can be effectively used to eliminate sperm with high SDF levels, and therefore may help to improve reproductive outcomes in couples undergoing ART.
